"Respirateur" "Respirateur"
La présente invention a pour objet un respirateur particulièrement destiné à fournir une assistance ventilatoire en respiration spontanée consciente et notamment une aide inspiratoire qui consiste, pendant la phase inspiratoire, à imposer une pression fixe du mélange gazeux inspiré, le débit dudit mélange étant fonction de l'appel d'air du patient.
On connaît divers types de respirateur qui
se subdivisent de la manière suivante :
- les respirateurs pour la ventilation artificielle ou mécanique qui comprennent les respirateurs pour la ventilation à pression contrôlée (IPPB) dans lesquels on impose , d'une part , une pression inspiratoire maximum déclenchant lorsqu'elle est atteinte la phase expiratoire (voir les courbes montrées à la figure 1) et, d'autre part, un débit dans les voies aériennes qui diminue quand la pression croît et les respirateurs pour la ventilation à volume contrôlé
(IPPV) dans lesquels on impose un volume déterminé du mélange gazeux insufflé, le débit inspiratoire ainsi qu'un temps de pose inspiratoire (voir les courbes montrées à la figure 2) ;
Ces deux types de respirateur présentent l'inconvénient de ne pas permettre d'assurer la fonction aide inspiratoire susdite en ventilation spontanée du fait qu'ils imposent les variations du débit des voies aériennes. En ventilation à pression contrôlée,le débit inspiratoire est déterminé par les modifications
de pression , tandis qu'en ventilation à volume contrôlé, le débit est directement imposé par le respirateur alors que la pression varie en fonction de la compliance thoracopulmonaire du patient. L'imposition d'une pression positive respiratoire permet de lutter contre la résistance des voies aériennes du patient, lorsque celle-ci est perturbée, il est évident que la chute du débit ressortant des courbes montrées à la figure 1, lors de la phase inspiratoire et dès la montée en pression du mélange gazeux, ne permet pas une aide inspiratoire en respiration spontanée consciente. Il est également évident que les respirateurs pour la venti-. lation à volume contrôlé ne permettent pas d'assurer l'aide inspiratoire susdite du fait que tous les paramètres sont imposés.
Ces respirateurs pour la.ve ntilation artificielle présentent en outre l'inconvénient de faire courir aux patients des risques de barotraumatismes dus soit aux pics de pression (en volume contrôlé), soit aux pressions trop élevées (en pression contrôlée).
- les servo-respirateurs.
On connaît l'intérêt, pour le traitement ou la prévention des insuffisances respiratoires aiguës, d'utiliser, en ventilation consciente spontanée, la pression positive continue des voies aériennes (CPAP Continuous Positive Airway Pressure). L'association de l'aide inspiratoire et de la pression positive ex-piratoire (SPEEP Spontaneous Positive End Expiratory Pressure) permettrait de lutter contre l'acidose respiratoire en prenant en charge le surcroît de travail ventilatoire responsable de la décompensation.
Les servo-respirateurs susdits, qui permettent d'assurer ces deux fonctions de pression positive continue et d'aide inspiratoire, présentent toutefois l'inconvénient d'être des appareils très sophistiqués, très coûteux et difficilement transportables qui fatalement sont en nombre trop restreint pour répondre aux besoins réels. Ces servo-respirateurs ont également l'inconvénient de nécessiter une alimentation électrique en plus de l'alimentation en mélange gazeux sous pression.
L'invention a pour but de procurer un respirateur à commandes mécaniques , extrêmement simple et de là peu coûteux , capable de remplir soit la fonction aide inspiratoire, soit la fonction pression positive continue (CPAP) ou encore l'aide inspiratoire associée
à la pression positive expiratoire (SPEEP). Ce respirateur, destiné à être fabriqué en grande série pour répondre aux besoins réels notamment soit dans les unités de soins intensifs pour le traitement des décompensations respiratoires, soit en kinésithérapie

table et excessivement fiable du fait de sa simplicité et , d'autre part, de réaliser une économie sérieuse du mélange gazeux par rapport aux systèmes utilisés actuellement pour réaliser une pression positive continue en ventilation spontanée et qui nécessitent des débits considérables de mélanges gazeux qui sont de l'ordre de 30 à 50 litres par minute. Enfin le respi-rateur suivant l'invention présente également l'avantage de ne nécessiter qu'une seule source d'énergie,
à savoir un mélange gazeux sous pression et d'être d'un entretien nul étant donné qu'il peut, vu son coût, être soit entièrement, soit partiellement, jeté après une durée limitée d'utilisation chez un patient.
A cet effet, suivant l'invention, ce respirateur comprend un boîtier indéformable dans lequel sont délimitées trois chambres juxtaposées séparées par des cloisons, une chambre, dite chambre inspiratoire, une chambre, dite chambre expiratoire, et une chambre intermédiaire située entre les chambres inspiratoires et expiratoires, la cloison séparant la chambre inspiratoire de la chambre intermédiaire étant déformable .tandis que la cloison séparant cette dernière de la chambre expiratoire étant indéformable, un conduit reliant la chambre inspiratoire à une source de mélange gazeux sous pression, des moyens agencés dans la chambre inspiratoire pour débiter le mélange gazeux sous pression à inspirer, des moyens de commande desdits moyens débitant le mélange agencés pour être actionnés par la cloison déformable de la chambre intermédiaire,
une ouverture réalisée dans la cloison indéformable de la chambre intermédiaire mettant celle-ci en communication avec la chambre expiratoire, des moyens pour obturer temporairement cette ouverture, des moyens de commande desdits moyens d'obturation de l'ouverture agencés pour être actionnés par la cloison déformable de la chambre intermédiaire pour que l'ouverture soit fermée quand les moyens précités débitent le mélange gazeux dans la chambre inspiratoire, une tubulure re-liant une ouverture pratiquée dans la chambre inspiratoire à un dispositif, tel qu'embout buccal, masque facial, etc., une tubulure reliant une seconde ouverture pratiquée dans la cloison indéformable de la chambre intermédiaire audit dispositif,
un clapet antiretour associé à cette dernière tubulure et agencé pour autoriser le passage du mélange expiré de cette tubulure vers la chambre intermédiaire et se fermer automatiquement lors de l'inspiration et une ouverture met-

Suivant une forme de réalisation de l'invention, l'ouverture susdite mettant la chambre expiratoire à l'atmosphère est obturée par une valve destinée
à s'ouvrir automatiquement quand une pression prédéter-minée est atteinte dans ladite chambre expiratoire, la pression d'ouverture de la valve étant réglable.
Suivant un mode de réalisation avantageux de l'invention, le respirateur comprend un ballon d'amortissement communiquant avec la chambre expiratoire.

rement avantageuse de l'invention, la chambre intermédiaire comporte une ouverture raccordée par une canalisation au conduit mettant en communication la chambre inspiratoire avec la source de mélange gazeux sous pression, cette canalisation étant reliée audit conduit en amont des moyens susdits agencés pour débiter le mélange gazeux à inspirer, un débitmètre étant intercalé dans cette canalisation.

) vention ressortiront de la description des dessins annexés au présent mémoire et qui représentent, à titre d'exemple non limitatif, une forme de réalisation particulière du respirateur suivant l'invention.
Le respirateur suivant l'invention et représenté à la figure 4 comprend un boîtier indéformable 1 dans lequel sont délimitées trois chambres juxtaposées 2, 3 et 4 séparées par des cloisons 5 et 6, une chambre 2, dite chambre inspiratoire, une chambre 3, dite chambre expiratoire, et une chambre intermédiaire 4 située entre les chambres inspiratoire 2 et expiratoire 3. La cloison 5 qui sépare la chambre inspiratoire 2 de la chambre intermédiaire 4 est déformable tandis que la cloison 6, qui sépare cette chambre 4 de la chambre expiratoire 3, est indéformable. Un conduit 7 relie la chambre inspiratoire 2 à une source de mélange gazeux sous pression non représentée et des moyens
8 sont agencés dans cette chambre 2 pour débiter le mélange gazeux sous pression à inspirer sous l'action de moyens de commande 9 agencés pour être actionnés par la cloison déformable 5 de la chambre intermédiaire. Une ouverture 10 est réalisée dans la cloison indéformable 6 de la chambre intermédiaire 4, tandis que des moyens 11 sont prévus sur le respirateur pour obturer temporairement cette ouverture 10, ainsi que des moyens de commande 12 desdits moyens 11 agencés pour être actionnés par la cloison déformable 5 de la chambre intermédiaire 4, afin que l'ouverture 10 soit fermée quand les moyens 8 précités débitent le mélange gazeux dans la chambre inspiratoire 2. Une tubulure
13 relie une ouverture 14 pratiquée dans la chambre inspiratoire 2 à un dispositif non représenté, tel qu' embout buccal, masque facial, etc., tandis qu'une tubulure 15 relie une seconde ouverture 16 pratiquée dans la cloison indéformable 6 de la chambre intermédiaire 4 audit dispositif, un clapet anti-retour 17 étant associé à cette tubulure 15 et agencé pour autoriser le passage du mélange expiré de cette tubulure 15 vers
la chambre intermédiaire 4 et se fermer automatiquement lors de l'inspiration, la chambre expiratoire 3 étant mise à l'atmosphère par une ouverture 18.
En vue d'un déclenchement efficace de l'admission du mélange gazeux dans la chambre inspiratoire
2, la chambre intermédiaire 4 du respirateur suivant l'invention comprend avantageusement une ouverture 19 reliée par une canalisation 20 au conduit 7 raccordant les moyens 8 susdits, situés dans la chambre inspiratoire 2, à la source de mélange gazeux sous pression.
Le raccord de la canalisation 20 au conduit 7 est situé en amont des moyens 8 et un débitmètre 21 est intercalé dans cette canalisation 20, le réglage de ce dernier permettant de régler le temps de mise en pression pour atteindre le plateau de pression 22 (voir Figure 3,courbe de pression) .
Pour éviter les accidents pouvant résulter d'une pression du mélange gazeux trop importante dans
la chambre inspiratoire et créée par exemple par une obstruction dans la tubulure 15 ou dans l'ouverture 18 de la chambre expiratoire 3, une ouverture 23 est pratiquée dans la chambre inspiratoire 2 pour mettre celleci en communication avec l'atmosphère, cette ouverture
23 étant munie d'une valve 24, normalement fermée, agencée pour s'ouvrir automatiquement lorsque la pression du mélange gazeux a atteint, dans la chambre inspiratoire, une valeur prédéterminée.
Pour permettre, dans le respirateur suivant l'invention, l'association de l'aide inspiratoire et
de la pression positive expiratoire (SPEEP), l'ouverture 18 pratiquée dans la chambre expiratoire 3, pour mettre cette dernière en communication avec l'atmosphère, est munie d'une valve 25 qui ne s'ouvre que lorsqu'une pression prédéterminée est atteinte dans la chambre expiratoire, la pression d'ouverture de cette valve 25 étant réglable afin d'autoriser le réglage de la pression de base de la pression positive expiratoire, un ballon d'amortissement 26 communiquant avec la chambre expiratoire 3 étant avantageusement prévu.
Dans le but d'éviter tout effort au patient pour le déclenchement de l'admission du mélange gazeux dans la chambre inspiratoire 2, la chambre intermédiaire 4 présente une troisième ouverture 27 qui la met en communication avec la chambre expiratoire 3. Cette ouverture 27 est munie d'un clapet anti-retour 28 qui autorise le passage dans le sens chambre expiratoire
3 vers la chambre intermédiaire 4 et qui interdit le passage dans le sens inverse.
L'équipement du respirateur suivant l'invention est avantageusement complété par un humidificateur
29 intercalé dans la tubulure 13, par un filtre antibactérien 30 également intercalé dans la tubulure 13 entre l'ouverture 14 de la chambre inspiratoire 2 et l'humidificateur et destiné à conserver stérile ladite chambre inspiratoire 2 et par un manomètre 31 associé
à cette dernière pour permettre la vérification des valeurs soit de la pression d'aide inspiratoire, soit de la pression d'aide inspiratoire et de la pression positive expiratoire quand le respirateur est utilisé avec ces deux pressions en association.
Dans la forme de réalisation du respirateur illustrée à la figure 4, la cloison déformable 5 séparant la chambre intermédiaire 4 de la chambre inspiratoire 1 est constituée par une membrane 32 rigide dans sa zone centrale 33 et souple à sa périphérie 34, les moyens de commande 9 des moyens 8 débitant le mélange gazeux dans la chambre inspiratoire 2, ainsi que les moyens de commande 12 des moyens d'obturation 11 de l'ouverture 10 mettant en communication les chambres intermédiaire et expiratoire, coopérant avec la zone centrale rigide 33 de la membrane. Ces moyens 8 sont constitués par un détendeur , une vanne à pointeau 34, à ressort de rappel du pointeau et un injecteur 35, l'orifice 36 de ce dernier étant tourné vers l'ouverture 14 de la chambre inspiratoire 2 à laquelle est raccordée la tubulure 13.
La section de cet orifice
36 est inférieure à celle de l'ouverture 14 et l'orifice est situé à faible distance de cette dernière de manière à obtenir dans la tubulure 13 une pression supérieure à celle régnant dans la chambre inspiratoire 2, cette différence de pression variant en fonction
du débit de mélange gazeux. Les moyens de commande 9 du pointeau 34 de la vanne sont constitués par un premier levier 37 pouvant pivoter librement, son extrémité. 38,autour d'un axe fixe 39 s'étendant perpendiculairement à l'axe du pointeau 34 et dont l'autre extrémité 40 prend appui sur.un second levier 41 entre les extrémités 42 et 43 de celui-ci. L'extrémité 42
de ce second levier 41 peut pivoter librement autour d'un axe fixe 44 parallèle à l'axe 38 du premier le- <EMI ID=4.1>
prend appui sur la zone centrale indéformable 33 de la membrane 32. Les axes 39 et 44 des leviers sont situés de part et d'autre du pointeau 34 et le premier levier porte, entre ses deux extrémités, une vis 45 de position réglable coopérant avec le pointeau de la vanne
et destinée à permettre le réglage du débit de la vanne en fonction du déplacement de la zone centrale 33
de la membrane 32. Les deux leviers 37 et 41 sont profilés pour, d'une part, qu'ils soient toujours en contact quelles que soient les positions du pointeau 34 et de la membrane 32 et, d'autre part, pour que la vis
45 actionne le pointeau 34 de la valve pour débiter le mélange gazeux dès que le volume de la chambre intermédiaire 4 s'accroît à partir de son volume minimum et pour que la vis 45 permette au pointeau de fermer la valve, sous l'action du ressort de pointeau, quand le volume de la chambre intermédiaire 4 tend vers son minimum.
Les moyens d'obturation 11 de l'ouverture 10, réalisée dans la cloison indéformable 6 de la chambre intermédiaire 4 pour mettre celle-ci en communication avec la chambre expiratoire 3, sont constitués par un clapet 46 fixé à l'extrémité 47 d'un levier 48 pouvant pivoter librement, à son autre extrémité 49, autour d'un axe 50 parallèle à ladite cloison indéformable 5, par un ressort 51 prenant appui sur le levier 47, entre l'extrémité 47 de ce dernier et l'axe 50 de manière à exercer sur ce levier une force tendant à appliquer le clapet 46 sur les bords de l'ouverture 10 et par un système de réglage 52 de la tension du ressort 51 qui permet de modifier l'importance du plateau de pression 22 (voir courbe de pression de la figure 3).
Les moyens d'obturation 11 sont disposés dans la chambre expiratoire 3 tandis que le bouton de commande 53 du système de réglage 52 est situé à l'extérieur du respirateur. Les moyens de commande 12 précités de ces moyens d'obturation 11 sont constitués par un ergot 54 fixé au clapet 46 à l'opposé du levier 48 et qui s'étend à travers l'ouverture 10 et la chambre intermédiaire 4 pour que son extrémité libre 55 coopère avec la zone indéformable 33 de la membrane 32, la longueur dudit ergot
54 étant telle que, d'une part, l'ouverture 10 est obturée au plus tard quand la chambre intermédiaire 4 atteint son volume maximum et, d'autre part, l'ouverture 10 est dégagée au plus tard quand la chambre intermédiare atteint son volume minimum.
Le boîtier du respirateur est avantageusement constitué de deux éléments pourvus de moyens d'assemblage étanches, un des éléments comprenant la chambre expiratoire 3 et la chambre intermédiaire 4 qui peut être jeté après un usage limité par un patient, tandis que l'autre élément comprend la chambre inspiratoire 2 qui peut être réutilisée, cette chambre restant stérile grâce; au filtre antibactérien 30 intercalé dans la tubulure 13.
Si l'on se réfère aux courbes illustrées à la figure 3, on constate que le débit est indépendant de la pression, néanmoins, cette dernière agit sur la mécanique du thorax, contre les forces de rappel, en augmentant le volume inspiré pour une force musculaire identique. En d'autres termes, la pression modifie le débit de façon indirecte. On constate aussi que le plateau de pression 22 apparaît très rapidement, de sorte que la pression est utilisable rapidement pour lutter contre la résistance des voies aériennes. De
ce fait, la pression maximum peut être plus basse, ce qui élimine les risques de lésionsbarotraumatiques.
La courbe de pression montrée à la figure 3 est semblable à celle d'un respirateur pour la ventilation à volume contrôlé dans laquelle le pic de pression serait remplacé par un plateau de pression. Mais, pendant le plateau de pression 22, le débit du respirateur suivant l'invention fonctionnant en aide inspiratoire n'est pas nul, ce qui différentie ce respirateur, d'une part,
des respirateurs pour la ventilation à volume contrôlé qui ont un débit nul dès que la pression atteint son maximum et, d'autre part, de la fonction "apneustique" existant sur certains respirateurs pour la ventilation à pression ,contrôlée.
Bien que la valeur de temps de mise en pression soit imposée, cette pression apparaissant de façon progressive sous le contrôle d'une constante de temps déterminée par le débitmètre réglable 21, la durée du plateau inspiratoire 22 est libre, la phase expiratoire n'étant déclenchée que lorsque le débit des voies aériennes est nul.
Le débit inspiratoire étant libre, les volumes pulmonaires sont dès lors modifiés par le régime des pressions en fonction des compliances de l'appareil thoraco-pulmonaire. La pression positive expiratoire (SPEEP), lorsqu'elle est associée à l'aide inspiratoire, modifie la capacité résiduelle fonctionnelle en fonction de la compliance totale et le gain
de volume courant (AV ) apporté par l'aide inspira-toire est déterminé théoriquement et ceci, pour un travail inspiratoire inchangé, également par la compliance totale. Aux courbes de pression et de débit , montrées à la figure 3, ont été ajoutées, en pointillé, les courbes de pression positive continue des voies aériennes (CPAP) qui permettent de montrer la modification qu'apporte la pression d'aide inspiratoire sur la courbe de débit tout comme d'en déterminer le gain de volume courant.
Certains essais cliniques du respirateur suivant l'invention ont déjà été effectués et ont-donné de très bons résultats dans les cas suivants :
1. Hypoxémie réfractaire : - ARDS
- embolie pulmonaire assistance au masque facial avec "SPEEP" : de 5 à
10 cmH20 aide inspiratoire : de 0 à 15 cmH 0 en fonction de la PaC02 et du confort respiratoire.
2. Acidose respiratoire : - décompensation d'une bron-
chopathie obstructive
- hypoventilation alvéolaire postopératoire (chirurgie abdominale,vasculaire ou thoracique) assistance avec masque facial ou intubation si bronchorrhée profuse "SPEEP" = 0
aide inspiratoire : de 5 à

3. Prévention postopératoire : - après chirurgie abdominale

Le respirateur suivant l'invention fonctionne de la manière suivante :
Le respirateur, qui ne fonctionne qu'en déclenchement volontaire soit en assistance inspiratoire,soit
en pression positive des voies aériennes ou encore en assistance inspiratoire combinée à une pression positive expiratoire, est commandé par le patient lorsqu'il inspire.
Cette inspiration crée une dépression dans les tubulures 13 et 15 et dans la chambre inspiratoire 2 tandis
que le clapet antiretour 17 se ferme . Du fait de la différence de pression régnant entre la chambre inspi-ratoire 2 et la chambre intermédiaire 4, la membrane
32 se déplace vers la gauche et actionne, par le biais des leviers 37 et 41, la vanne à pointeau pour admettre le mélange gazeux sous pression dans la chambré inspiratoire 2 et la tubulure 13, ledit déplacement
de la membrane assurant la fermeture de l'ouverture 10 par le clapet 46 poussé par le ressort 51, le clapet antiretour 28 étant fermé. L'agencement de l'injecteur
35 permet de créer une dépression dans la chambre inspiratoire 2 par rapport à la tubulure 13, cette dépression augmentant quand le débit de mélange augmente de sorte que le patient ne doit fournir aucun effort pour assurer le maintien de la membrane 32 dans sa position correspondant à l'admission du mélange gazeux, plus l'appel de mélange du patient est important, plus le débit dudit mélange est important.
Une augmentation contrôlée de la pression dans la chambre intermédiaire 4 s'effectue à travers l'ouverture 19, via la canalisation 20 et le débitmètre 21, jusqu'à une valeur déterminée par la force du ressort 51 réglable par le bouton 53, la valve antiretour 28, qui est fermée empêchant la transmission directe de l'augmentation de pression dans le circuit respiratoire qui s'équilibre instantanément par le jeu de la membrane 32 et des moyens 8 de distribution du mélange gazeux. Lorsque
la pression dans la chambre intermédiaire 4 a atteint la valeur déterminée susdite, le clapet 46 libère l'ouverture 10 et le mélange s'échappe par cette dernière dans la chambre expiratoire 3 pour maintenir le volume du ballon d'amortissement 26 et assurer ainsi la stabilité de la pression positive expiratoire et surtout diminuer la pression de déclenchement du respirateur lors de la phase inspiratoire, Dès que le patient expire, le clapet 17 s'ouvre, un équilibre de pression s'établit entre les chambres 2 et 4 et la membrane 32 se déplace vers la droite et l'admission de mélange gazeux dans le respirateur est coupée, l'air expiré s' échappant par l'ouverture 10 qui est ouverte et par la valve 25.
En cas d'appel d'air important du patient , le débit de mélange à travers l'ouverture 19 de la chambre intermédiaire 4 ne permettrait pas de compenser la pression négative régnant dans cette chambre , de sorte que la membrane 32 se stabiliserait et que le patient devrait fournir un effort important pour la déplacer suffisamment vers la gauche pour autoriser le débit du mélange dans la chambre inspiratoire (pic négatif de pression en déclenchement). Pour éviter cette situation , le respirateur suivant l'invention est muni du clapet 28 qui s'ouvre en cas d'appel d'air violent pour mettre la chambre 4 en communication avec la chambre expiratoire 3 et le ballon d'amortissement 26, ce qui permet un déplacement instantané, sans effort du patient, de la membrane 32 jusqu'à sa position assurant le débit du mélange gazeux.
Il doit être entendu que l'invention n'est nullement limitée à la forme de réalisation décrite
et que bien des modifications peuvent être apportées
à cette dernière sans sortir du cadre du présent brevet.
C'est ainsi que l'on pourrait prévoir, sur
le respirateur suivant l'invention, différents acces-soires permettant de lui assurer un déclenchement automatique pour une fonction particulière permettant
un volume par minute et/ou un volume courant garantis.
On pourrait également prévoir de munir ledit respirateur d'une annexe électronique simple permettant d'assurer le monitoring de l'assistance ventilatoire.
On pourrait encore prévoir un respirateur
dont le boîtier est constitué de trois éléments distincts pouvant être réunis entre eux de manière étanche, un des éléments constituant la chambre inspiratoire tandis que les deux autres constituent respectivement la chambre intermédiaire et la chambre expiratoire, ces éléments étant agencés pour recevoir des accessoires et notamment des accessoires assurant le déclenchement automatique susdit se disposant entre la chambre intermédiaire et la chambre expiratoire.

The present invention relates to a respirator particularly intended to provide ventilatory assistance in conscious spontaneous breathing and in particular an inspiratory aid which consists, during the inspiratory phase, of imposing a fixed pressure of the inspired gas mixture, the flow rate of said mixture being a function of the air call from the patient.
We know various types of respirator which
are subdivided as follows:
- respirators for artificial or mechanical ventilation which include respirators for controlled pressure ventilation (IPPB) in which one imposes, on the one hand, a maximum inspiratory pressure triggering when it is reached the expiratory phase (see the curves shown in Figure 1) and, on the other hand, a flow in the airways which decreases when the pressure increases and the ventilators for ventilation with controlled volume
(IPPV) in which a determined volume of the blown gas mixture is imposed, the inspiratory flow rate as well as an inspiratory exposure time (see the curves shown in Figure 2);
These two types of respirator have the disadvantage of not making it possible to ensure the aforementioned inspiratory aid function in spontaneous ventilation because they impose variations in the airway flow. In pressure-controlled ventilation, the inspiratory flow is determined by the modifications
pressure, while in volume-controlled ventilation, the flow rate is directly imposed by the ventilator while the pressure varies according to the patient's thoracopulmonary compliance. The imposition of a positive respiratory pressure makes it possible to fight against the resistance of the airways of the patient, when this is disturbed, it is obvious that the drop in flow emerging from the curves shown in FIG. 1, during the inspiratory phase and as soon as the pressure of the gas mixture rises, does not allow inspiratory aid in conscious spontaneous breathing. It is also evident that ventilators for ventilation. lation with controlled volume do not allow to assure the aforementioned inspiratory aid because all the parameters are imposed.
These respirators for artificial la.ve ntilation also have the drawback of making patients run the risk of barotrauma due either to pressure peaks (in controlled volume) or to excessively high pressures (in controlled pressure).

We know the interest, for the treatment or prevention of acute respiratory insufficiency, of using, in spontaneous conscious ventilation, the continuous positive pressure of the airways (CPAP Continuous Positive Airway Pressure). The combination of inspiratory aid and positive ex-piratory pressure (SPEEP Spontaneous Positive End Expiratory Pressure) would fight respiratory acidosis by taking care of the additional ventilatory work responsible for decompensation.
The aforementioned servo-respirators, which make it possible to ensure these two functions of continuous positive pressure and inspiratory aid, have the disadvantage of being very sophisticated, very expensive and difficult to transport devices which are inevitably too few in number to meet real needs. These servo-respirators also have the drawback of requiring an electrical supply in addition to the supply of pressurized gas mixture.
The object of the invention is to provide a ventilator with mechanical controls, extremely simple and hence inexpensive, capable of fulfilling either the inspiratory aid function, the continuous positive pressure function (CPAP) or the associated inspiratory aid.

table and excessively reliable because of its simplicity and, on the other hand, to achieve a serious economy of the gas mixture compared to the systems currently used to achieve a continuous positive pressure in spontaneous ventilation and which require considerable flow rates of gas mixtures which are on the order of 30 to 50 liters per minute. Finally, the respi-tor according to the invention also has the advantage of requiring only one energy source,
that is to say a gaseous mixture under pressure and to be of zero maintenance since it can, considering its cost, be either entirely or partially, discarded after a limited period of use in a patient.

) vention will emerge from the description of the drawings annexed to this specification and which represent, by way of nonlimiting example, a particular embodiment of the respirator according to the invention.
The respirator according to the invention and shown in Figure 4 comprises a non-deformable housing 1 in which are delimited three juxtaposed chambers 2, 3 and 4 separated by partitions 5 and 6, a chamber 2, called the inspiratory chamber, a chamber 3, called expiratory chamber, and an intermediate chamber 4 located between the inspiratory 2 and expiratory chambers 3. The partition 5 which separates the inspiratory chamber 2 from the intermediate chamber 4 is deformable while the partition 6, which separates this chamber 4 from the expiratory chamber 3 , is undeformable. A conduit 7 connects the inspiratory chamber 2 to a source of gas mixture under pressure not shown and means
8 are arranged in this chamber 2 to deliver the gaseous mixture under pressure to be inspired under the action of control means 9 arranged to be actuated by the deformable partition 5 of the intermediate chamber. An opening 10 is made in the non-deformable partition 6 of the intermediate chamber 4, while means 11 are provided on the respirator to temporarily close this opening 10, as well as control means 12 of said means 11 arranged to be actuated by the partition deformable 5 of the intermediate chamber 4, so that the opening 10 is closed when the aforementioned means 8 deliver the gas mixture into the inspiratory chamber 2. A tube
13 connects an opening 14 made in the inspiratory chamber 2 to a device not shown, such as mouthpiece, facial mask, etc., while a tube 15 connects a second opening 16 made in the non-deformable partition 6 of the intermediate chamber 4 to said device, a non-return valve 17 being associated with this tube 15 and arranged to allow the passage of the exhaled mixture from this tube 15 towards
the intermediate chamber 4 and close automatically during inspiration, the expiratory chamber 3 being vented through an opening 18.
For an effective triggering of the admission of the gas mixture into the inspiratory chamber
2, the intermediate chamber 4 of the respirator according to the invention advantageously comprises an opening 19 connected by a pipe 20 to the duct 7 connecting the above-mentioned means 8, located in the inspiratory chamber 2, to the source of gaseous mixture under pressure.
The connection of the pipe 20 to the pipe 7 is located upstream of the means 8 and a flow meter 21 is inserted in this pipe 20, the adjustment of the latter making it possible to adjust the pressurization time to reach the pressure plate 22 (see Figure 3, pressure curve).
To avoid accidents that could result from excessive gas mixture pressure in
the inspiratory chamber and created for example by an obstruction in the tubing 15 or in the opening 18 of the expiratory chamber 3, an opening 23 is made in the inspiratory chamber 2 to put this in communication with the atmosphere, this opening
23 being provided with a valve 24, normally closed, arranged to open automatically when the pressure of the gas mixture has reached, in the inspiratory chamber, a predetermined value.
To allow, in the respirator according to the invention, the association of the inspiratory aid and
positive expiratory pressure (SPEEP), the opening 18 made in the expiratory chamber 3, to put the latter in communication with the atmosphere, is provided with a valve 25 which opens only when a predetermined pressure is reached in the expiratory chamber, the opening pressure of this valve 25 being adjustable in order to authorize the adjustment of the base pressure of the positive expiratory pressure, a damping balloon 26 communicating with the expiratory chamber 3 being advantageously provided.
In order to avoid any effort to the patient for triggering the admission of the gaseous mixture into the inspiratory chamber 2, the intermediate chamber 4 has a third opening 27 which places it in communication with the expiratory chamber 3. This opening 27 is fitted with a non-return valve 28 which allows passage in the expiratory chamber direction
3 towards the intermediate chamber 4 and which prohibits passage in the opposite direction.
The equipment of the respirator according to the invention is advantageously supplemented by a humidifier
29 interposed in the tubing 13, by an antibacterial filter 30 also interposed in the tubing 13 between the opening 14 of the inspiratory chamber 2 and the humidifier and intended to keep said inspiratory chamber 2 sterile and by a pressure gauge 31 associated
to the latter to allow verification of the values of either the inspiratory aid pressure, or the inspiratory aid pressure and the positive expiratory pressure when the ventilator is used with these two pressures in combination.
In the embodiment of the respirator illustrated in FIG. 4, the deformable partition 5 separating the intermediate chamber 4 from the inspiratory chamber 1 is constituted by a membrane 32 rigid in its central zone 33 and flexible at its periphery 34, the control means 9 of the means 8 delivering the gas mixture into the inspiratory chamber 2, as well as the control means 12 of the closing means 11 of the opening 10 bringing the intermediate and expiratory chambers into communication, cooperating with the rigid central zone 33 of the membrane. These means 8 are constituted by a pressure reducer, a needle valve 34, with a needle return spring and an injector 35, the orifice 36 of the latter being turned towards the opening 14 of the inspiratory chamber 2 to which the tubing 13.
The section of this hole
36 is less than that of the opening 14 and the orifice is located a short distance from the latter so as to obtain in the tubing 13 a pressure greater than that prevailing in the inspiratory chamber 2, this difference in pressure varying according to
gas mixture flow rate. The control means 9 of the needle valve 34 of the valve consist of a first lever 37 which can pivot freely, its end. 38, around a fixed axis 39 extending perpendicular to the axis of the needle 34 and the other end 40 of which bears on a second lever 41 between the ends 42 and 43 thereof. The end 42
of this second lever 41 can pivot freely around a fixed axis 44 parallel to the axis 38 of the first le- <EMI ID = 4.1>
is supported on the non-deformable central zone 33 of the membrane 32. The axes 39 and 44 of the levers are located on either side of the needle 34 and the first lever carries, between its two ends, a screw 45 of adjustable position cooperating with the valve needle
and intended to allow adjustment of the valve flow rate as a function of the displacement of the central zone 33
of the membrane 32. The two levers 37 and 41 are profiled so that, on the one hand, they are always in contact whatever the positions of the needle 34 and the membrane 32 and, on the other hand, so that the screw
45 actuates the needle valve 34 of the valve to deliver the gas mixture as soon as the volume of the intermediate chamber 4 increases from its minimum volume and so that the screw 45 allows the needle to close the valve, under the action of the needle spring, when the volume of the intermediate chamber 4 tends to its minimum.
The closure means 11 of the opening 10, produced in the non-deformable partition 6 of the intermediate chamber 4 to put the latter in communication with the expiratory chamber 3, consist of a valve 46 fixed to the end 47 of a lever 48 which can pivot freely, at its other end 49, about an axis 50 parallel to said non-deformable partition 5, by a spring 51 bearing on the lever 47, between the end 47 of the latter and the axis 50 so as to exert on this lever a force tending to apply the valve 46 to the edges of the opening 10 and by a system 52 for adjusting the tension of the spring 51 which makes it possible to modify the size of the pressure plate 22 (see pressure curve in Figure 3).
The closure means 11 are arranged in the expiratory chamber 3 while the control button 53 of the adjustment system 52 is located outside the respirator. The aforementioned control means 12 of these shutter means 11 consist of a lug 54 fixed to the valve 46 opposite the lever 48 and which extends through the opening 10 and the intermediate chamber 4 so that its end free 55 cooperates with the non-deformable zone 33 of the membrane 32, the length of said lug
54 being such that, on the one hand, the opening 10 is closed at the latest when the intermediate chamber 4 reaches its maximum volume and, on the other hand, the opening 10 is released at the latest when the intermediate chamber reaches its minimum volume.
The respirator housing advantageously consists of two elements provided with leakproof assembly means, one of the elements comprising the expiratory chamber 3 and the intermediate chamber 4 which can be discarded after limited use by a patient, while the other element comprises the inspiratory chamber 2 which can be reused, this chamber remaining sterile thanks; to the antibacterial filter 30 inserted in the tubing 13.
If we refer to the curves illustrated in Figure 3, we see that the flow is independent of the pressure, however, the latter acts on the mechanics of the chest, against the restoring forces, increasing the volume inspired for a force identical muscle. In other words, pressure changes the flow rate indirectly. It is also found that the pressure plate 22 appears very quickly, so that the pressure can be used quickly to combat the resistance of the airways. Of
this fact, the maximum pressure can be lower, which eliminates the risk of barotraumatic lesions.
The pressure curve shown in Figure 3 is similar to that of a ventilator for volume-controlled ventilation in which the pressure peak would be replaced by a pressure plate. But, during the pressure plate 22, the flow rate of the respirator according to the invention operating as an inspiratory aid is not zero, which differentiates this respirator, on the one hand,
respirators for ventilation with controlled volume which have a zero flow as soon as the pressure reaches its maximum and, on the other hand, the "apneustic" function existing on certain respirators for ventilation with pressure, controlled.
Although the pressurization time value is imposed, this pressure appearing progressively under the control of a time constant determined by the adjustable flow meter 21, the duration of the inspiratory plate 22 is free, the expiratory phase not being triggered only when the airway flow is zero.
The inspiratory flow being free, the pulmonary volumes are consequently modified by the pressure regime according to the compliances of the thoraco-pulmonary apparatus. Positive expiratory pressure (SPEEP), when combined with inspiratory aid, changes functional residual capacity based on total compliance and gain
tidal volume (AV) provided by the inspiratory aid is determined theoretically and this, for unchanged inspiratory work, also by total compliance. To the pressure and flow curves, shown in Figure 3, have been added, in dotted lines, the continuous positive airway pressure (CPAP) curves which show the modification brought about by the inspiratory aid pressure on the flow curve as well as determining the gain in tidal volume.
Certain clinical trials of the respirator according to the invention have already been carried out and have given very good results in the following cases:
1. Refractory hypoxemia: - ARDS
- pulmonary embolism assistance with the facial mask with "SPEEP": from 5 to
10 cmH20 inspiratory aid: 0 to 15 cmH 0 depending on PaC02 and respiratory comfort.
2. Respiratory acidosis: - decompensation of a bron-
obstructive chopathy
- postoperative alveolar hypoventilation (abdominal, vascular or thoracic surgery) assistance with facial mask or intubation if profuse bronchorrhea "SPEEP" = 0
inspiratory aid: from 5 to

3. Postoperative prevention: - after abdominal surgery

The respirator according to the invention operates as follows:
The respirator, which only works when triggered voluntarily, either by inspiratory assistance or
positive airway pressure or inspiratory assistance combined with positive expiratory pressure, is controlled by the patient when inhaling.
This inspiration creates a depression in the tubes 13 and 15 and in the inspiratory chamber 2 while
that the non-return valve 17 closes. Due to the pressure difference between the inspi-ratory chamber 2 and the intermediate chamber 4, the membrane
32 moves to the left and actuates, by means of levers 37 and 41, the needle valve to admit the gaseous mixture under pressure into the inspiratory chamber 2 and the tubing 13, said displacement
of the membrane ensuring the closing of the opening 10 by the valve 46 pushed by the spring 51, the non-return valve 28 being closed. The layout of the injector
35 makes it possible to create a depression in the inspiratory chamber 2 relative to the tubing 13, this depression increasing when the flow of mixture increases so that the patient does not have to exert any effort to maintain the membrane 32 in its position corresponding to the greater the patient's mixing call, the greater the flow rate of said mixture.
A controlled increase in the pressure in the intermediate chamber 4 takes place through the opening 19, via the pipe 20 and the flow meter 21, up to a value determined by the force of the spring 51 adjustable by the button 53, the valve non-return valve 28, which is closed preventing direct transmission of the increase in pressure in the respiratory circuit which is instantly balanced by the play of the membrane 32 and the means 8 for distributing the gas mixture. When
the pressure in the intermediate chamber 4 has reached the above-determined value, the valve 46 releases the opening 10 and the mixture escapes through the latter into the expiratory chamber 3 to maintain the volume of the damping balloon 26 and thus ensure the stability of the positive expiratory pressure and especially decrease the triggering pressure of the respirator during the inspiratory phase, As soon as the patient expires, the valve 17 opens, a pressure balance is established between chambers 2 and 4 and the membrane 32 moves to the right and the admission of gas mixture into the respirator is cut off, the exhaled air escaping through the opening 10 which is open and through the valve 25.
In the event of a significant air intake by the patient, the flow of mixture through the opening 19 of the intermediate chamber 4 would not make it possible to compensate for the negative pressure prevailing in this chamber, so that the membrane 32 would stabilize and that the patient should make a significant effort to move it sufficiently to the left to allow the flow of the mixture in the inspiratory chamber (negative pressure peak on trigger). To avoid this situation, the respirator according to the invention is provided with the valve 28 which opens in the event of a violent air call to put the chamber 4 in communication with the expiratory chamber 3 and the damping balloon 26, this which allows instantaneous movement, without effort of the patient, of the membrane 32 to its position ensuring the flow of the gas mixture.
It should be understood that the invention is in no way limited to the embodiment described
and that many changes can be made
to the latter without departing from the scope of this patent.
This is how we could predict, on
the respirator according to the invention, various accessories allowing it to be triggered automatically for a particular function allowing
guaranteed volume per minute and / or tidal volume.
Provision could also be made to provide said respirator with a simple electronic annex enabling monitoring of ventilatory assistance.
We could still provide a respirator
the housing of which consists of three distinct elements which can be joined together in leaktight manner, one of the elements constituting the inspiratory chamber while the other two constitute respectively the intermediate chamber and the expiratory chamber, these elements being arranged to receive accessories and in particular accessories ensuring the abovementioned automatic triggering arranged between the intermediate chamber and the expiratory chamber.
