"Procédé de marquage de protéines à l'aide du technétium".
La présente invention a pour objet un procédé de marquage de protéines à l'aide du 99 m technetium comprenant la réduction de pertechnétate et la mise en contact du pertechnétate réduit avec au moins une des protéines choisies du groupe comprenant le fibrinogène et les immunoglobulines,pour provoquer le marquage de cette protéine.
Différentes techniques de marquage du fibrinogène ont été publiées dans la littérature. Ces données se
<EMI ID=1.1> Parmi ces procédés de marquage,existent la méthode électrolytique, la méthode enzymatique, la méthode à
la chloramine - T et la méthode au monochlorure d'iode. Ces techniques présentent un certain nombre d'inconvénients qui sont dus à l'utilisation de radioisotopes à demivie trop longue ( dépassant la demi-vie du firbrinogène ), une énergie non compatible avec les moyens de visualisation existant actuellement sur le marché et un degré d'irradiation trop important.

un degré d'irradiation minimal.
Le but de la présente invention est de présenter un procédé qui permet d'obtenir une solution injectable d'une protéine du groupe susdit marquée à l'aide du 99 m technétium qui permet de profiter pleinement des propriétés très intéressantes du technétium en tant que traceur isotopique et de remédier ainsi aux inconvénients du fibrinogène marqué à l'iode.
A cet effet, suivant l'invention,le mélange contenant la protéine précitée marquée à l'aide de technetium obtenu par la mise en contact du technétium réduit et de la protéine en cause, est soumise à une purification permettant l'élimination de produits de dégradation éventuels de cette protéine ainsi que de technétium non fixé.
Avantageusement, la purification consiste à séparer de cette protéine les substances ayant un poids moléculaire inférieur à 100.000.
Suivant une forme de réalisation particulièrement avantageuse on réalise la réduction du pertechnétate à un
pH compris entre 11 et 12.
Suivant une forme de réalisation préférée, du pertechnétate, contenu dans du sérum physiologique, est réduit par l'addition d'une solution d'acide acétique et de chlorure staneux suivi de l'addition d'une base pour amener le pH à
la valeur voulue.
L'invention concerne également une solution injectable contenant au moins une protéine choisie du groupe formé par le fibrinogène et les immunoglobulines, marquée à l'aide du 99 m technétium,obtenuesuivant le procédé décrit ci-dessus.
D'autres détails et particularités de '_'invention ressortiront de la description donnée ci-après, à titre d'exemple non limitatif,d'une forme de réalisation avantageuse du procédé suivant l'invention.

Ce complexe radioisotopique est particulièrement utile dans le radiodiagnostic d'affection inflammatoires du tissu conjonctif telles que la polyarthrite rhumatoïde entre autres, les tromboses vasculaires et les tumeurs malignes.
On utilise avantageusement du fibrinogène lyophilisé d'origine humaine injectable aux humains et dont la

nogène est stérile et est libre d'antigène australien et de
pyrogène. Il est conservé sous une forme lyophilisée en présence de sels sodiques.

Avant d'être mis en contact avec la protéine à marquer, en l'occurence le fibrinogène, le pertechnétate est réduit. Il est, en effet,généralement établi que le 99 m Tc

au stade +4 ou +5 avant de se fixer sur une molécule quelconque.
Le choix du pH est très important à cet égard et, suivant l'invention, on travaille à un pH de l'ordre de 11 à 12.
On utilise comme agent réducteur de préférence
le chlorure staneux. Le technétium réduit se fixerait alors, lorsqu'il est mis en contact avec le fibrinogène, sur les groupements tyrosyl de la molécule de celui-ci.
Le fibrinogène lyophilisé est dissout dans
de l'eau stérile et apyrogène et est ensuite
soumis à une dialyse pour séparer les sels sodiques susdits.
Dans un stade suivant. le pertechnétate réduit est mis en contact intense, par exemple par agitation, avec le fibrinogène en solution, de manière à provoquer le marquage
de celui-ci par le technétium.

obtenu, contenant la protéine marquée à l'aide de technétium, à une purification pour éliminer des produits de dégradation éventuels de cette protéine ainsi que du technétium non fixé
sur cette dernière.
Cette purification consiste de préférence à
séparer de cette protéine marquée les substances ayant un poids moléculaire inférieur à 100.000 et éventuellement inférieur
à 300.000 en faisant passer ledit mélange sur une membrane appropriée retenant les substances ayant un poids moléculaire supérieur à ceux donnés ci-dessus.
Par exemple, pour retenir les substances à poids moléculaire supérieur à 100.000 on utilise avantageusement une cellule Amicon modèle 12 avec une membrane du type XM 100 A.
Avantageusement, par une première purification,
on élimine le technétium libre et le technétium lié aux molécules dont le poids moléculaire est inférieur à 100.000 et, par
une deuxième purification subséquente, on élimine toutes les molécules dont le poids moléculaire est inférieur à 300.000.
Les différences techniques de contrôle, comme la chromatographie sur colonne et l'immunoélectrophorèse ont permis d'établir que la méthode de purification garantit une élimination de la majorité des produits de dégradation et du technitium libre. Dans la solution finale, il a été constaté, suivant l'invention, que le 99 m technétium est fixé à au moins 97% sur une molécule de fibrinogène, alors que les deux purifications successives permettent,pour certains cas particuliers, d'obtenir une solution de fibrinogène marquée à l'aide de technétium dans laquelle la quantité de technétium fixée sur une molécule de fi-

Les deux graphiques susdits permettent d'apprécier l'efficacité du procédé de purification suivant l'invention et de constater que le produit marqué est bien le fibrinogène
et ceci d'une manière très sélective.
L'exemple concret donné ci-après permet d'illustrer davantage le procédé suivant l'invention.
Exemple :
- réduction du pertechnétate.
10 mC de pertechnétate contenus dans 2 ml de sérum physiologique sont réduits grâce à l'addition, d'une part, d'un ml d'une solution aqueuse contenant 20 lamdas d'acide acétique 96% et
10 mg de SnC12.2H20 et, d'autre part, de 0,3 ml de NaOH 0,1 N pour amener le pH à 11,6
- Conditionnement du fibrinogène.
On dissout 1 g de fibrinogène lyophilisé dans 100 ml d'eau stérile et apyrogène. On dialyse le fibrinogène en solution pendant 18 heures à 4[deg.]C dans des sacs à dialyse du type Visking 1-8/32" contre une solution de NaCl 0,6%.
On divise le fibrinogène en solution en parties aliquotes
de 2 ml qu'on conserve en flacons à -20[deg.]C.
- Marquage proprement dit.
2 ml de fibrinogène sont agités pendant 15 minutes et à température ambiante avec la solution contenant le technétium réduit.
- Purification du fibrinogène.
Le technétium non fixé et les produits éventuels de dégradation sont éliminés par passage sur cellule Amicon modèle 12 avec une membrane du type XM 100 A.
- Contrôle final de la solution de fibrinogène marqué . Avant l'injection, le produit final obtenu est testé pour vérifier la stérilité et l'absence de pyrogène.
De la même façon, le procédé suivant l'invention est applicable pour marquer les immunoglobulines (IgG, IgA,

marquage du fibrinogène et des immunoglobulines a été conçue dans le but du diagnostic spécifique d'un nombre d'affections comme la polyarthrite rhumatoïde, les tromboses vasculaires, les tumeurs malignes.
Cette technique permet la mise en évidence des lésions au niveau des organes par la visualisation à l'aide de
la gamma-caméra à scintillation.
Il est bien entendu que l'invention n'est pas limitée aux.. formes de réalisation décrites et que bien des variantes pourraient être envisagées sans sortir du cadre du présent brevet.

Different techniques for labeling fibrinogen have been published in the literature. These data are

chloramine - T and the iodine monochloride method. These techniques have a certain number of drawbacks which are due to the use of radioisotopes with an excessively long half-life (exceeding the half-life of the firbrinogen), an energy incompatible with the visualization means currently existing on the market and a degree of too much irradiation.

The aim of the present invention is to present a process which makes it possible to obtain an injectable solution of a protein of the aforementioned group labeled with the aid of 99 m technetium which makes it possible to take full advantage of the very advantageous properties of technetium as a tracer. isotopic and thereby overcome the drawbacks of iodine-labeled fibrinogen.
To this end, according to the invention, the mixture containing the aforementioned protein labeled with technetium obtained by bringing the reduced technetium into contact with the protein in question, is subjected to purification allowing the elimination of products of possible degradation of this protein as well as unbound technetium.
Advantageously, the purification consists in separating from this protein substances having a molecular weight of less than 100,000.
According to a particularly advantageous embodiment, the reduction of the pertechnetate to a
pH between 11 and 12.
According to a preferred embodiment, pertechnetate, contained in physiological saline, is reduced by the addition of a solution of acetic acid and stanous chloride followed by the addition of a base to bring the pH to
the desired value.
The invention also relates to an injectable solution containing at least one protein selected from the group formed by fibrinogen and immunoglobulins, labeled with the aid of 99 m technetium, obtained by the method described above.
Other details and features of the invention will emerge from the description given below, by way of nonlimiting example, of an advantageous embodiment of the process according to the invention.

The choice of pH is very important in this regard and, according to the invention, the operation is carried out at a pH of the order of 11 to 12.

This purification preferably consists of
separate from this labeled protein substances with a molecular weight less than 100,000 and possibly less
to 300,000 by passing said mixture over a suitable membrane retaining substances having a molecular weight greater than those given above.
For example, to retain substances with a molecular weight greater than 100,000, an Amicon model 12 cell with a membrane of the XM 100 A type is advantageously used.
Advantageously, by a first purification,
free technetium and technetium bound to molecules whose molecular weight is less than 100,000 are eliminated and, by
a second subsequent purification, all molecules with a molecular weight of less than 300,000 are removed.
The technical differences in control, such as column chromatography and immunoelectrophoresis have made it possible to establish that the purification method guarantees elimination of the majority of degradation products and of free technitium. In the final solution, it was found, according to the invention, that the 99 m technetium is fixed to at least 97% on a fibrinogen molecule, whereas the two successive purifications make it possible, for certain particular cases, to obtain a solution of fibrinogen labeled with technetium in which the amount of technetium attached to a molecule of

The two aforementioned graphs make it possible to assess the efficiency of the purification process according to the invention and to observe that the labeled product is indeed fibrinogen.

Example:
- reduction of pertechnetate.
10 mC of pertechnetate contained in 2 ml of physiological serum are reduced thanks to the addition, on the one hand, of one ml of an aqueous solution containing 20 lamdas of 96% acetic acid and
10 mg of SnC12.2H20 and, on the other hand, 0.3 ml of 0.1 N NaOH to bring the pH to 11.6
- Conditioning of fibrinogen.
1 g of lyophilized fibrinogen is dissolved in 100 ml of sterile, pyrogen-free water. The fibrinogen in solution is dialyzed for 18 hours at 4 [deg.] C in dialysis bags of the Visking 1-8 / 32 "type against a 0.6% NaCl solution.
Divide fibrinogen in solution into aliquots
of 2 ml stored in flasks at -20 [deg.] C.
- Marking itself.
2 ml of fibrinogen are stirred for 15 minutes and at room temperature with the solution containing the reduced technetium.
- Purification of fibrinogen.
The unbound technetium and any degradation products are eliminated by passage through an Amicon model 12 cell with an XM 100 A type membrane.
- Final check of the labeled fibrinogen solution. Before injection, the final product obtained is tested to verify sterility and the absence of pyrogen.
Likewise, the method according to the invention is applicable for labeling immunoglobulins (IgG, IgA,
