PROCEDE ET PREPARATION D'ACIDES SULFAMYL-BENZOIQUES.
La présente invention concerne la préparation d'acides sulfamyl-benzoïques qui sont efficaces comme adjuvanta à utiliser en connexion avec l'administration de la pénicilline pour assurer une augmentation de la concentration ou du taux de la pénicilline dans le plasma sanguin, avec une dose d'administration donnée de pénicilline, permettant ainsi d'obtenir des taux de pénicilline très élevés dans le sang ou d'utiliser des quantités plus réduites de pénicilline pour réaliser un taux de pénicilline donné dans le sang, ou encore de prévoir une administration moins fréquente de la pénicilline, tout en maintenant dans le sang un taux de pénicilline adéquat aux fins bactéricides ou bactériostatiques.
L'invention concerne également la préparation de différentes formes de dosage, dans lesquelles un ou plusieurs des nouveaux composés sont incorporés pour l'administration par des voies diverses.
La pénicilline représente actuellement un agent thérapeutique bien déterminé, utilisé dans le traitement des infections bactériennes, en particulier les infections cocciques. Pour l'usage interne, elle est généralement administrée par voie intraveineuse, intramusculaire ou buccale. Là où il est nécessaire de réaliser des taux de pénicilline élevés dans le sang, on applique l'administration intraveinuse ou l'administration par transfusion intraveineuse continue.
La cause majeure des difficultés qu'on éprouve à maintenir m taux de pénicilline adéquat ou élevé dans le sans provient du fait que pratiquement toute la pénicilline contenue dans le sang est éliminée au cours d'un seul passage par les reins.
Il apparalt que la pénicilline est excrétée du sang presque quantitativement par les cellules épithéliales des tubuli, au moins dans les limites de concentration dans le plasma qui ont été explorées, avec le résultat que sa vitesse d'excrétion du flux sanguin est approximativement cinq fois celle de matières qui sont excrétées par la filtration glomérulaire

seule, l'excrétion tubulaire intervenant pour environ 80 (81) % et les glomérules pour environ 20 (19) %.
On a proposé divers moyens pour combattre les difficultés dues a l'élimination rapide de la pénicilline, tel que l'administration de celleci en suspension dans une matière huileuse, le mélange étant administré par injection intramusculaire. Bien que cette méthode permettre de prolonger l'intervalle entre injections consécutives, elle présente le désavantage que la pénicilline est encore excrétée presque quantitativement lors du passage du flux sanguin par le système rénal; par conséquent, elle ne permet pas d'employer des quantités de pénicilline moins élevées pour réaliser un taux de pénicilline donné dans le sang.
Le deuxième moyen qui a été proposé en vue de réduire la vitesse d'excrétion de la pénicilline, consistait à employer, conjointement avec cette dernière, une substance qui, tout comme la pénicilline, est excrétée sélectivement par les tubuli. Si l'on prévoit un rapport suffisamment grand entre la quantité de l'agent d'addition et celle de la pénicilline, cet expédient permet une réduction sensible de la vitesse d'excrétion de la pénicilline par les tubuli et ralentit ainsi notablement l'élimination de celle-ci. On a proposé ou utilisé à cette fin de nombreux agents, y compris le diodrast et l'acide hippurique ou leurs dérivés ou substances d'origine.
Toutefois, de tels agents ne semblent pas apporter une solution de valeur à ce problème, sauf dans des cas extrêmes, étant donné que, la réduction de la vitesse d'excrétion de la pénicilline étant le reflet du degré de surcharge des tubuli avec des substances qu'elles sont appelées à éliminer du sang, il est nécessaire de maintenir une très forte concentration de l'agent d'addition dans le flux sanguin pour assurer un rapport f avorable entre les quantités éliminées de l'agent et de la pénicilline; de plus, et vu que ces agents sont eux-mêmes rapidement éliminés du flux sanguin, il est nécessaire de les administrer en grandes quantités pour maintenir les concentrations élevées nécessaires dans le plasma.
Cela pose cependant un problème additionnel, parce que pour maintenir la concentration élevée de ces agents, on doit recou- rir à leur administration intraveineuse, puisqu'ils ne sont pas bien absorbés par le tractus gastro-intestinal.
La présente invention est basée sur la découverte que l'élimi- nation de la pénicilline depuis le flux sanguin par les tubuli des reins peut être efficacement arrêtée par des adjuvants selon la présente invention, ayant la formule générale

et leurs sels, dans laquelle R et R1 sont respectivement un radical alcoyle, les radicaux alcoyles représentés par R et R1 ayant un total combiné d'au moins cinq et pas plus de huit atomes de carbone, tandis que le groupe cartexyle est attaché à la position ortho, méta ou para du noyau de benzène par rapport au radical sulfonyle.
Il a été constaté que des dérivés d'acide sulfamyl-benzoïque, dans lesquels le groupe carboxyle se trouve en position para, sont des adjuvants particulièrement efficaces, et dans ce groupe, ceux dans lesquels les radicaux alcoyles représentés par R et R1 ont up total combiné de cinq ou six atomes de carbone, tels que l'acide para-(di-n-propylsulfamyl)-benzoïque et l'acide para-(di-isopropylsulfamyl)-benzoïque, ont prouvé être extrêmement efficaces pour inhiber l'excrétion rénale de la pénicilline.

Les acides sulfmyl-benzoïques selon l'invention sont relativement non-toxiques, ils sont solubles dans le plasma sanguin et, lorsqu'ils sont entraînas par le flux sanguin jusqu'au niveau des tubuli, ils agissent de façon à empêcher Inaction normale de ces derniers, consistant à éliminer la pénicilline du flux sanguin.
Les adjuvants eux-mêmes ne sont pas excrétés dans une grande mesure par les tubuli, et,-les preuves disponibles indiquent que, lorsque ces adjuvants entrent en contact avec les cellules épithéliales des tubuli, ils agissent de manière à arrêter leur action en empêchant le fonctionnement normal des cellules épithéliales, mais n'inhibent pas l'excré- tion de la pénicilline par une compétition avec celle-ci dans le cadre du fonctionnement normal de% tubuli.
Ainsi, les adjuvants agissent pour supprimer ou réduire très fortement l'excrétion de la pénicilline par les tubuli, et cela dans des concentrations dans le plasma d'environ 10 mg par 100 cc., ce qui constitue à peu près la ?'valeur de seuil?' pour les agents tels que l'acide p-amino-hippurique ou le diodrast, lesquels inhibent l'excrétion de la pénicilline par les tubuli par une sorte de compétition pour se partager le pouvoir d'excrétion disponible des tubuli.
Les adjuvants hautement efficaces selon l'invention réduisent presque à zéro l'excrétion de la pénicilline par les tubuli, et cela à des concentrations dans le plasma d'environ 10 mg par 100 cc, de sorte que l'élimination proprement dite de la pénicilline du flux sanguin se ramène essentiellement à celle qui résulte de la filtration glomérulaire. c'est-à-dire environ un cinquième de la vitesse normale (abstraction faite de la fixation dans le plasma). Les adjuvants sont également éliminés par les glomérules.
Les adjuvants peuvent être administrés par voie buccale, ou s'ils sont dissous en solution aqueuse, ils peuvent être administrés par voie intraveineuse ou par voie intramusculaire. Cette dernière méthode n'a pas encore donné de bons résultats avec une injection unique de la substance, car, d'une façon générale, une administration de plus de 2 ce. par injection intramusculaire n'est pas recommandable; or, les quantités d'adjuvant qu'il est désirable d'employer, c'est-à-dire de 4 à 16 g. par jour, rendent l'injection par cette voie impraticable. Toutefois, les acides sulfamyl-benzolques ou leurs sels se prêtent bien à une clyse continue intramusculaire ou sous-cutanée.
En général, une administration buccale des adjuvants au régime de 4 à 16 g. par jour suffit pour réduire la vitesse d'excrétion de la pénicilline dans une telle mesure que le taux dans le sang, pour une dose donnée de pénicilline administrée en solution aqueuse par voie buccale ou intramusculaire, est portée jusqu'au quadruple de celui obtenu sans l'emploi de l'adjuvante ce qui permet soit d'employer des quantités de pénicilline sensiblement moindres pour assurer un taux donné dans le sang, par exemple de réduire les doses de pénicilline jusqu'au quart environ de celles généralement employées, soit de réaliser des taux de pénicilline dans le sang plusieurs fois supérieurs à ceux réalisables par l'administration de la pénicilline par les voies ordinairement utilisées actuellement.
Les adjuvants selon l'invention ou leurs sels appropriés peuvent être administrés en mélange avec la pénicilline ou séparément.
Dans tous les cas, que 19 adjuvant soit administré en mélange avec la pénicilline ou séparément, la quantité d'adjuvant employée doit être calculée pour assurer une concentration d'adjuvant dans le flux sanguin suffisante pour arrêter pratiquement le mécanisme d'excrétion des tubuli.
L'effet maximum sera obtenu avec des concentrations d'adjuvant dans le plasma sanguin d'environ 5 à 15 mg. par 100 cc, réalisables moyennant des taux de dosage d'environ 4 à 16 g. par jour, par voie buccale, et des taux légèrement inférieurs par voie intraveineuse.
Les adjuvants peuvent être préparés dans toute forme de dosage appropriée, soit seuls, soit additionnés de pénicilline, tel qu'en forme d'une pastille comprimée, une capsule garnie sèche ou une capsule molle élastique. Il va évidemment de soi que d'autres ingrédients, tels que des liants,

diluants, excipients, substances antacides, ou autres composés inertes ou à action thérapeutique .peuvent être incorporés dans tout genre de dosage choisi, ensemble avec l'adjuvant ou l'adjuvant additionné de pénicilline, à condition que l'ingrédient ajouté ne détruise pas l'activité de l'adjuvant ou de la pénicilline.
De même, l'adjuvant et, si on le désire, la pénicilline, peuvent être dispersés dans une base oléagineuse, soit seuls ou ensemble avec d'autres substances appropriées, et être mis ensuite dans des capsules molles élastiques, ou bien on peut préparer une solution aqueuse et la mettre en ampoules. D'autres formes de dosage appropriées viendront aisément à 1'idée du spécialiste, les considérations exposées dans la présente description n'ayant pas pour but de limiter le mode d'emballage ou d'administration à ce qui est spécialement indiqué dans la présente description.
Les nouveaux acides sulfaimyl-benzolques selon l'invention, dans lesquels le radical carboxyle se trouve en position para par rapport au radical sulfonyle sur le noyau de benzène, sont en général fabriqués en préparant d'abord un halogénure de paracarboxybenzènesulfonyle par oxydation d'halogénure de para-toluène sulfonyle, ou en préparant l'halogénure de para-

cyanobenzènesul'onyle en traitant de l'acide para-sul.famy1-benzoÍque avec du pentachlorure de phosphore.
L'halogénure de para-carboxybenzènesulfonyle ou l'halogénure de para-cyanobenzènesulfonyle ainsi préparé est mis en réacticn avec la di-alcoylamine appropriée, en utilisant avantageusement un excès, par exemple deux ou trois fois la quantité équivalente, de l'amine, et, lorsqu'il est fait usage de l'halogénure de cyanobenzènesulfonyle comme réactif, en hydrolysant le produit ainsi formé pour obtenir le dérivé carboxyle correspondant. La réaction est, de préférence, effectuée en présence d'un solvant, tel que l'acétone ou la pyridine ou analogues, ou dans de l'hydroxy- de de sodium aqueux, de préférence en refroidissant.
Tout sous-produit formé pendant la réaction est enlevé en traitant le milieu de réaction avec une substance faiblement alcaline, telle qu'une solution aqueuse de bicarbonate de soude, en éliminant le sous-produit par filtration, et en récupérant en-

suite l'acide sulfamyl-benzolque désiré du filtrat par acidification.' Dans certains cas, il est possible de préparer les dérivés di-alcoyles par alcoylation d'un acide mono-alcoylsulfamyl-benzolque, de préférence en présence d'un solvant, tel que dans des alcalis aqueux.
Les dérivés d'acide ortho-suif anyl-benzoïque peuvent être préparés par réaction de saccharine avec un halogénure alcoyle, et en traitant le produit ainsi formé avec un alcoolate de métal alcalin, pour obtenir le produit final désiré.

par ex méthanol, en ajoutant le produit ainsi formé à 19alcoylamine, de préférence dissoute dans un solvant, et avantageusement en refroidissant, de façon à obtenir 19alcoylméta-(alcaylsulfamyl)-benzoate. L'ester est alors hydrolysé pour obtenir le dérivé d'acide benzoïque correspondant, selon les méthodes connues.
La préparation des composés selon l'invention est illustrée par les exemples suivants, sans toutefois y être limitée.

Exemple I. - Acide p-(Di-n-butylsul:famyl)-benzoIQue.
A 38,7 grammes (0,3 mole) de di-n-butylamine dissous dans 300 millilitres d'acétone sèche, contenus dans une bouteille ouverte munie d'un agitateur, on a ajouté par portions, en refroidissant et en agitant,

22,1 grammes (0,1 mole) de chlorure de p-caiboybenzénesulonyle. La réac- tion a été poursuivie pendant une demi-heure en remuant, après quoi la suspension a été concentrée au-dessus d'un bain de vapeur jusqu'à environ un tiers du volume original. Le résidu a été versé dans 250 millilitres d'eau, froide, la solution a été rendue acide avec de l'acide chlorhydrique dilué, et le produit brut a été enlevé par filtration. Ce produit a été dissous dans une solution diluée de bicarbonate de soude. La solution a été traitée avec du

charbon de bois décolorant et filtrée.
Après précipitation du produit de 1a solution au moyen d'un excès d'acide chlorhydrique dilué, le produit a été recristallisé à partir d'alcool à 50%. Le rendement était de 20 grammes (64%), point de fusion 163-1640.
Exemple II. - Acide p-(Di-n-propylsulfamyl)-benzoïque.
24 g. (0,11 mole) de chlorure de p-carboxybenzènesulfonyle cnt été ajoutés par petites portions à une suspension de 20 g. (0,146 mole) de di-n-propylamine dans 100 millilitres d'hydroxyde de sodium à 10%, en agitant vigoureusement, à une température de 15-25 . L'agitation a été poursuivie' pendant 15 minutes après 1?addition finale. La solutj.on claire a été trai= tée avec du charbon décolorant et filtrée. Le produit a été précipité par l'addition d'un excès diacide chlorhydrique. Le produit brut a été purifié par reprécipitation à partir d'une solution de bicarbonate et neoristallisation à partir d'alcool dilué. Le rendement était de 20 g. (64%), point de fusicn 194-196 .
En remplaçant la di-n-butylamine de l'Exemple 1 ou la di-n-propylamine de l'Exemple II par une quantité équivalente de n'importe quelle aitre di-alcoylamine, et en travaillant en substance comme décrit dans chacun de ces exemples, on obtient les acides para-(di-alcoylsulfamyl)-benzoïques correspondants. Des exemples d'autres composés préparés selon ces méthodes comprennent ceux repris dans le tableau A.

Une autre méthode de préparation des composés selon l'invention, spécialement les amines secondaires à nombreuses ramifications, est illustrée par l'exemple suivant.
Exemple III - Acide p-(Di-sec-butylsulfamyl)-benzoïque:
30,3 g. (0,15 mole) de chlorure de p-cyano benzènesulfonyle cot été ajoutés à 19,35 g. (0,15 mole) de di-sec-butylamine dans 50 millilitres de pyridine sèche, en refroidissant et agitant. On a laissé le mélange de réaction à la température du laboratoire jusqu-'au lendemain, après quoi on l'a versé dans 50 cc. diacide chlorhydrique concentre et 500 g. de glace pilée. Il s'est précipité une huile foncée qui s'est rapidement solidifiée.
Le solide a été enlevé par filtration et séché à 1?air, donnant un rendement de 22.5 g. (75%) de p-(di-sec-butylsulfamyl)-benzonitrile. fondant à 88-91.
Après des recristallisations répétées à partir d'éther d'isopropyle, le nitrile fondait à 97-99 . On a chauffé 20 g. (0,68 mole) de nitrile brut dans 150 millilitres d9hydroxyde de sodium à 10%, avec reflux, 4jusqu'à ce qu'il n'y eut plus de dégagement d'ammoniaque, (environ quatre heures). La solution a été traitée avec du charbon décolorant et filtrée, et le produit a été précipité du filtrat au moyen d'un excès diacide chlorhydrique. L'acide p-(Disec-butylsulfamyl)-benzoique ainsi obtenu a été dissous dans une solution diluée de bicarbonate de soude et reprécipité avec de l'acide chlorhydrique dilué. Le rendement était de 14 g. de produit ; de fusion 175-179 .

Trois recristallisations à partir d'alcool dilué ont donné 7,5 g. (18%) de produit; point de fusion 185 .
En remplaçant la di-butylamine secondaire de l'Exemple III par importe quelle autre di-alcoylamine, et en travaillant en substance comme décrit dans cet exemple, on obtient les acides para-(di-alcoylsulfamyl)- benzoïques correspondants, ayant un groupe alcoyle du type défini pour R et R1 dans la formule générale donnée à la page 3 de la présente description. Par ailleurs, l'invention sera illustrée, sans y être limitée, par les diverses formes de dosage ci-après de différentes compositions, pour l'administration par différentes voies.
Exemple (a) - Pastilles comprimées (ou on mélange uniformé- ment 10.000 g. de lactose et 100.000 g. de l'adjuvant, acide para-(di-n-propylsulfamyl-benzoïque. et l'on humidifie le mélange avec une quantité d'eau suffisante pour permettre une granulation facile. On mélange intimement 2000 g. d'amidon de mais séché, 500 g. de poudre de gomme de karaya, 2500 g. de talc et 1000 g. de stéarate de calcium et l'on mélange le tout uniforme'ment avec les 110.000 g. du mélange d'adjuvant et de lactose granulés. Le mélange final est ensuite pastillé (en employant des emporte-pièces à courbure standard et à moule de 1/2 pouce, soit environ 13 mm), ce qui donne 200. 000 pastilles de 0,58 g. chacune et qui contiennent chacune 0,5 g. d'adjuvant.
Si l'on remplace dans cet exemple la quantité d'adjuvant par la même quantité d'un autre adjuvant voulu quelconque répondant à la formule générale donnée ci-dessus, on obtient des pastilles de même poids unitaire et ayant la même teneur en un quelconque de ces autres adjuvants.
Des quantités mesurées de la composition de l'Exemple (a), contenant n'importe quel adjuvant choisi, ou simplement de l'adjuvant seul, peuvent être mises dans des capsules télescopiques de gélatine dure, contenant chacune 0,5 g. d'adjuvant.
Ou bien, l'adjuvant choisi peut être dispersé de manière homogène dans une quantité égale d'huile de maïs, et la composition peut, de la fagon connue, être mise dans des capsules élastiques molles, en feuille de gélatine, scellées hermétiquement, qui contiennent chacune un gramme de la

composition adjuvant-huile.
Exemple (b) - Ampoules : 10 Kgo de l'adjuvant acide para-(di-n-propylsulfa- myl)-bezoïque, sont mis en suspension dans 50 litres (avec une forte appro- ximation) d'eau distillée et l'on ajoute 1481 go d'hydroxyde de sodium pour favoriser la dissolution de l'adjuvant. On ajoute 390 g. de monophosphate potassique, ainsi que de l'eau distillée, de façon que le volume de la solu- tion soit égal à 50 litres (pH d'environ 7,4). La solution est ensuite in- troduite dans des ampoules pouvant contenir chacune 5 cc. de liquide, qui sont ensuite scellées à la flamme et stérilisées à l'autoclave pendant 20 minutes sous une pression de 15 Ibs par pouce carré.
Exemple (c) -Pastilles comprimées contenant de la pénicilline : on mélange 10.000 g. de lactose avec 100000 g. de l'adjuvant acide para-(di-n-propylsulfamyl)-benzoïque, et l'on granule comme dans l'exemple (a). On mélange 3375 g. de pénicilline sodium (1630 unités par mg), 2625 g. d'amidon de mais séché, 500 g. de poudre de gomme de karaya, 2500 g. de talc et 1000 g. de stéarate de calcium, dans une atmosphère contrôlée, ajustée à une humidité relative de 10% à 21 C, le tout étant ensuite mélangé, dans les mêmes condi- tions, avec la masse granulée d'adjuvant et de lactose, et ensuite pastillé avec le même moule que dans l'exemple (a), ce qui donne 200.000 pastilles pesant 0,6 g. et contenant chacune 0,5 g. d'adjuvant et 25.000 unités de pénicilline (plus un excédent de 10%).
N'importe quel autre adjuvant voulu peut remplacer l'adjuvant de l'exemple (b), dans les mêmes quantités que celui-ci, pour obtenir des pastilles de même poids, ayant la même teneur en n'importe quel autre de ces adjuvants.
Exemple (d) - Capsules sèches garnies avec pénicilline : Dans une atmosphère contrôlée comme dans l'exemple (c), on mélange intimement et uniformément 25 Kg. de l'adjuvant, acide para-(di-n-propylsulfamyl)-benzoïque, 850 g. de pénicilline sodium cristalline (comme celle utilisée ci-dessus) et 150 g. d'amidon de mais séché, le tout étant introduit dans des capsules, ce qui donne 50.000 capsules renfermant chacune 0,52 g. de mélange contenant 0,5 g d'adjuvant et 25.000 unités de pénicilline, (plus un excédent de 10%).
Exemple (e) - Capsules molles élastiques, avec pénicilline :Dans une atmosphère contrôlée comme dans l'exemple (c), on prépare une dispersion homogène de 1,69 Kg. de la même pénicilline sodium cristalline dans 48,31 Kg. d'huile de maïs et 50 kg. de l'adjuvant acide para-(di-n-propylsulfamyl)-benzoïque, dispersés de la même manière dans cette huile, la composition résultante étant introduite, de la manière connue, dans des capsules élastiques molles et hermétiquement fermées en feuille de gélatine, ce qui donne 100. 000 capsules renfermant chacune 1 g. net de composition et contenant 0,5 g. d'adjuvant et 25.000 unités de pénicilline (plus un excédent de 10 %).
Exemple (f) - Ampoules scellées à la flamme, remplies de pénicilline On introduit en remuant 12,5 Kg. de l'adjuvant acide para-(di-n-propylsulfanyl)benzoïque, dans 30 litres (avec une forte approximation) d'eau distillée stérile et exempte de substances pyrogènes, et l'on ajoute 1831 g. d'hydroxyde de sodium pour favoriser la dissolution de l'adjuvant. On ajoute ensuite 390 g. de monophosphate potassique; puis on introduit dans la solution, en remuant, et dans les mêmes conditions atmosphériques que dans l'exemple (c), 169 g. de la même pénicilline sodium cristalline, et l'on ajoute de l'eau en quantité suffisante pour amener le volume total de la solution à 30 litres.

Après rétablissement du volume du liquide à 20 cc., la solution résultante est tamponnée à un PH de 7,4 et contient 100.000 unités de pénicilline (plus l'excédent de 10 %) et 25 % d'adjuvant.
Exemple (g) - Ampoules de suspension huileuse, avec pénicilline:
3 Kg. de cire blanche USP (United States Pharmacopoeia) sont mélangés avec 35,127 KG. d'huile d'arachide épurée et chauffée suffisamment pour fondre et pour permettre une dispersion homogène de la cire blanche.

En outre, dans les compositions contenant de la pénicilline, il est recommandable, conformément à la pratique usuelle, d'y incorporer un excédent de pénicilline, notamment un excédent de dix pour cent sur la quantité indiquée sur l'étiquette, conformément à la pratique actuelle. After repeated recrystallizations from isopropyl ether, the nitrile melted 97-99. 20 g were heated. Example (d) - Dry capsules filled with penicillin: In a controlled atmosphere as in example (c), 25 kg of the adjuvant, para- (di-n-propylsulfamyl) -benzoic acid, is intimately and uniformly mixed, 850 g. of crystalline sodium penicillin (like that used above) and 150 g. of dried corn starch, the whole being introduced into capsules, which gives 50,000 capsules each containing 0.52 g. of mixture containing 0.5 g of adjuvant and 25,000 units of penicillin, (plus an excess of 10%).
Example (e) - Soft elastic capsules, with penicillin: In a controlled atmosphere as in example (c), a homogeneous dispersion of 1.69 kg. Of the same crystalline sodium penicillin in 48.31 kg. Of crystalline sodium is prepared. corn oil and 50 kg. para- (di-n-propylsulfamyl) -benzoic acid adjuvant, dispersed in the same way in this oil, the resulting composition being introduced, in the known manner, into soft elastic capsules and hermetically sealed in gelatin sheet, which gives 100,000 capsules each containing 1 g. net of composition and containing 0.5 g. adjuvant and 25,000 units of penicillin (plus a 10% excess).
