Meer in het bijzonder heeft de onderhavige uitvinding betrekking op het gebruik van Bacillus coagulans of Bacillus subtilis of een combinatie van beide stammen voor de preventie of behandeling van functionele gastro-intestinale stoornissen, zoals bijvoorbeeld functionele dyspepsie. De onderhavige uitvinding beschrijft ook het gebruik van Bacillus coagulans of Bacillus subtilis of een combinatie van beide stammen als alternatieve therapie of aanvullende therapie voor maagzuurremmende behandelingen.

GEBIED VAN DE UITVINDING De uitvinding heeft in het algemeen betrekking op het gebruik van probiotica voor het behandelen van functionele maag-darmstoornissen. Meer in het bijzonder heeft de onderhavige uitvinding betrekking op het gebruik van Bacillus coagulans of Bacillus subtilis of een combinatie van beide stammen voor de preventie of behandeling van functionele gastro-intestinale stoornissen, zoals bijvoorbeeld functionele dyspepsie. De onderhavige uitvinding beschrijft ook het gebruik van Bacillus coagulans of Bacillus subtilis of een combinatie van beide stammen als alternatieve therapie of aanvullende therapie voor maagzuurremmende behandelingen.
ACHTERGROND VAN DE UITVINDING Functionele gastro-intestinale (GI) aandoeningen, ook bekend als aandoeningen van de darm- herseninteractie, omvatten een aantal afzonderlijke idiopathische stoornissen die verschillende delen van het maagdarmkanaal beïnvloeden en omvatten viscerale overgevoeligheids- en motiliteitsstoornissen. Voorbeelden van functionele maag-darmaandoeningen zijn functionele dyspepsie, functionele pijn op de borst, functionele zuurbranden, oprispingsstoornissen, misselijkheid en braken, functioneel opgeblazen gevoel of opgezette buik, reflux- overgevoeligheid, prikkelbare darmsyndroom, functionele constipatie, functionele diarree. Functionele dyspepsie is een functionele maag-darmaandoening die verwijst naar chronische of recidiverende symptomen van het bovenste deel van het maag-darmkanaal (GI) afkomstig uit het gastro-duodenale gebied met een significante invloed op de levens van patiënten (Talley NJ et al. 2015. Functional Dyspepsia. N Eng! J Med 373, 1853-63). Organische oorzaken zijn maagzweer, Helicobacter pylori infectie, galwegaandoeningen en esofagogastrische kankers. Volgens de criteria van Rome omvat functionele dyspepsie (FD) de diagnostische categorieën van epigastrisch pijnsyndroom (EPS) met epigastrische pijn of brandend gevoel, en postprandiaal stress syndroom (PSS) met maaltijd-gerelateerde volheid of vroege verzadiging, die onverklaard zijn na routinematig onderzoek, inclusief bovenste Gl-endoscopie (Stanghellini Vet al. 2016. Een recente Cochrane-analyse concludeerde dat protonpompremmers (PPI) effectiever zijn bij FD in vergelijking met placebo en histamine-2-receptorantagonisten (Pinto-Sanchez MI et a/. 2017. 22. BE2019/5201 PPl's worden echter ook geassocieerd met mogelijke langetermijnbijwerkingen, zoals een verhoogd risico op Gl-infecties en bacteriële overgroei in de dunne darm (Lo W et al. 2013. Proton Pump Inhibitor Use and the Risk of Small Intestinal Bacterial Overgrowth: A Meta- analysis. Clin Gastroenterol Hepatol 11, 48390). Recente studies hebben ook veranderingen in zowel het maagmicrobioom (Tsuda A et al. 2015. Influence of Proton-Pump Inhibitors on the Luminal Microbiota in the Gastrointestinal Tract. Clin Trans Gastroenterol 6, e89; Paroni Sterbini F et al. 2016. Effects of Proton Pump Inhibitors on the Gastric Mucosa-Associated Microbiota in Dyspeptic Patients. App! Environ Microbiol 82, 6633-44) als fecaal microbioom (Jackson MA et al. 2016. Proton pump inhibitors alter the composition of the gut microbiota. Gut 65, 749-56; Imhann F et a!. 2016. Proton pump inhibitors affect the gut microbiome. Gut 65, 740-8) tijdens PPI-behandeling gemeld met een toename van de orale flora, hoogstwaarschijnlijk als gevolg van de verlaging van de maagzuurbarrière en het ontbreken van bacteriedoding in de met zuurremmers behandelde maag. PPl's kunnen het maagmicrobioom beïnvloeden door de relatieve aanwezigheid van Streptococcus te verhogen, ongeacht de aanwezigheid of afwezigheid van H. pylori, wat kan bijdragen aan de persistentie van FD-symptomen met PPI of kan leiden tot langetermijnbijwerkingen (Paroni Sterbini F et al. 2016. Verschillende publicaties hebben verhoogde duodenale mucosale permeabiliteit en laaggradige ontsteking aangetoond bij FD-patiënten, wat correleert met maaltijd-gerelateerde symptomen (Kindt S et al. 2009. De oorzaken van het barrière-defect en immuun-activatie zijn onbekend, maar mogelijkheden zijn psychologische stress, luminale voedselcomponenten, (gal)-zuur en microbiota (Vanheel H et al. 2014. Onlangs bleek een probiotische behandeling gunstig te zijn voor postprandiale dyspeptische symptomen (lgarashi M et al. 2017. Alteration in the gastric microbiota and its restoration by probiotics in patients with functional dyspepsia. BMJ open Gastroenterol 4, e000144) mogelijk door veranderingen in de flora, hoewel deze hypothese niet werd getest in deze studie. Bacillus bacteriën zijn gram-positieve stammen die in staat zijn om endosporen te vormen die zeer resistent zijn tegen maagzuur (Elshaghabee FMF et al. 2017. Bacillus As Potential Probiotics: Status, Concerns, and Future Perspectives. Front Microbiol 8, 1490). Bovendien zijn sporenvormende probiotica of 'sporebiotica' stabiel bij kamertemperatuur en leveren ze meer levensvatbare bacteriën aan de dunne darm in vergelijking met traditonele probiotische supplementen (Cutting SM. 2011. Een combinatie van orale spore-gebaseerde probiotica (B. indicus, B. subtilis, B. coagulans, B. licheniformis and B. clausii) toonde aan dat de combinatie postprandiale endotoxemie verlaagt, wat kan resulteren uit een verhoogde darmpermeabiliteit (McFarlin BK et al. 2017. Oral spore- based probiotic supplementation was associated with reduced incidence of post-prandial dietary endotoxin, triglycerides, and disease risk biomarkers. De onderhavige uitvinding beschrijft het gebruik van Bacillus coagulans of Bacillus subtilis of een combinatie van beide stammen voor de profylaxe en/of behandeling van functionele GI- stoornissen, zoals bijvoorbeeld functionele dyspepsie. Bovendien beschrijft de onderhavige uitvinding ook het gebruik van Bacillus coagulans of Bacillus subtilis of een combinatie van beide stammen als alternatieve of aanvullende therapie voor maagzuurremmende behandelingen. In een ander aspect beschrijft de onderhavige uitvinding het gebruik van Bacillus coagulans of Bacillus subtilis sporen of een combinatie van beide stammen voor de preventie of behandeling van andere ziekten gekozen uit het metabool syndroom, zoals hoge cholesterolniveaus en hoge bloedsuikerspiegels; stoornissen van het immuunsysteem, zoals chronische ontstekingen, allergieën en auto-immuunziekten; psychische stoornissen zoals depressie en angst; en urogenitale aandoeningen, zoals urogenitale infecties.
SAMENVATTING VAN DE UITVINDING De onderhavige uitvinding beschrijft het gebruik van Bacillus coagulans of Bacillus subtilis of een combinatie van beide stammen voor de profylaxe/preventie en/of behandeling van functionele gastro-intestinale aandoeningen bij een patiënt. In het bijzonder is de uitvinding gericht op een combinatie die B. subtilis en B. coagulans-sporen omvat voor gebruik bij de preventie en/of behandeling van een functionele gastro-intestinale (GI) stoornis bij een patiënt. Genoemde functionele Gl-stoornis wordt gekozen uit functionele dyspepsie, functionele pijn op de borst, functionele zuurbranden, overmatig boeren, misselijkheid en braken, functioneel

In een verdere uitvoeringsvorm wordt de functionele Gl-stoornis gekozen uit functionele dyspepsie, functionele pijn op de borst, functioneel zuurbranden, overmatig boeren, misselijkheid en braken, functioneel opgeblazen gevoel of uitzetting van de buik. Dus in een nog meer geprefereerde uitvoeringsvorm, verschaft de onderhavige uitvinding een combinatie van B. subtilis en B. coagulans sporen voor het gebruik in de preventie en/of behandeling van functionele dyspepsie bij een patiënt.
Bovendien beschrijft de onderhavige uitvinding een combinatie die B. subtilis en B. coagulans sporen omvat voor gebruik als een alternatieve of aanvullende therapie voor maagzuurremmende behandelingen bij een patiënt.
In een andere uitvoeringsvorm beschrijft de uitvinding het gebruik van Bacillus coagulans of Bacillus subtilis sporen of een combinatie van beide stammen voor de preventie en/of behandeling van andere ziekten gekozen uit het metabool syndroom, zoals hoge cholesterol- en bloedsuikerniveaus; stoornissen van het immuunsysteem, zoals chronische ontstekingen, allergieën en auto-immuunziekten; psychische stoornissen zoals depressie en angst; en urogenitale aandoeningen, zoals urogenitale infecties bij een patiënt.
In een ander aspect van de uitvinding omvat de combinatie voor gebruik volgens de verschillende uitvoeringsvormen van de onderhavige uitvinding B. subtilis en B. coagulans sporen waarbij de genoemde B. subtilis en B. coagulans sporen in een 1:1, een 1:2 of een 2:1- verhouding zijn.
In nog een ander aspect van de uitvinding omvat de combinatie voor gebruik volgens de verschillende uitvoeringsvormen van de onderhavige uitvinding B. subtilis en B. coagulans, waarbij de B. subtilis de vorm B. subtilis var natto is.
In een verder aspect omvat de gebruikscombinatie volgens de onderhavige uitvinding B. subtilis en B. coagulans, waarbij de B. subtilis sporen zijn afgeleid van stam MY02 zoals gedeponeerd bij de BCCM met inschrijvingsnummer LMG P-31319. Genoemde stam is gedeponeerd bij de Belgian Coordinated Collection of Micro-Organisms (BCCM) (Universiteit Gent, KL Ledeganckstraat 35, 9000 Gent, België) op 14 maart 2019 met inschrijvingsnummer LMG P- 31319 en hierin verder ook aangegeven als MY02 stam. In een ander aspect omvat de gebruikscombinatie volgens de onderhavige uitvinding B. subtilis en B. coagulans, waarbij de B.
coagulans sporen zijn afgeleid van stam MYO1 zoals gedeponeerd bij de BCCM met inschrijvingsnummer LMG P-31318. Genoemde stam is gedeponeerd bij de Belgian Coordinated Collection of Micro-Organisms (BCCM) (Universiteit Gent, KL Ledeganckstraat 35, 9000 Gent, België) op 14 maart 2019 met inschrijvingsnummer LMG P-31318 en hierin verder ook aangegeven als MY01 stam.
5, BE2019/5201 In nog een ander aspect omvat de gebruikscombinatie volgens de onderhavige uitvinding B. subtilis en B. coagulans, waarbij de B. subtilis sporen zijn afgeleid van stam MY02 zoals gedeponeerd bij de BCCM met inschrijvingsnummer LMG P-31319 en waarbij B. coagulans sporen zijn afgeleid van stam MY01 zoals gedeponeerd bij de BCCM met inschrijvingsnummer LMGP-31318. In een ander aspect verschaft de onderhavige uitvinding een farmaceutische samenstelling voor gebruik bij de preventie en/of behandeling van een functionele gastro-intestinale aandoening bij een patiënt.
Genoemde farmaceutische samenstelling omvat een combinatie die B. subtilis en B. coagulans sporen omvat volgens de verschillende uitvoeringsvormen zoals hierboven beschreven.
De farmaceutische samenstelling omvat verder een of meer farmaceutisch aanvaardbare dragers, verdunningsmiddelen of excipiënten.
In een verdere uitvoeringsvorm is de farmaceutische samenstelling volgens deze uitvinding voor gebruik bij de preventie en / of behandeling van een functionele gastro-intestinale aandoening gekozen uit functionele dyspepsie, functionele pijn op de borst, functionele zuurbranden, overmatig boeren, misselijkheid en braken, functionele opgeblazen gevoel in de buik of uitzetting, reflux- overgevoeligheid, prikkelbare darm-syndroom, functionele constipatie, functionele diarree; bij voorkeur gekozen uit functionele dyspepsie, functionele pijn op de borst, functionele zuurbranden, overmatig boeren, misselijkheid en braken, functioneel opgeblazen gevoel of uitzetting van de buik.
In een nog meer geprefereerde uitvoeringsvorm is de farmaceutische samenstelling volgens de uitvinding bedoeld voor gebruik bij de preventie en/of behandeling van functionele dyspepsie bij een patiënt.
In een andere uitvoeringsvorm beschrijft de uitvinding het gebruik van een farmaceutische samenstelling omvattende een combinatie omvattende Bacillus coagulans en Bacillus subtilis volgens de verschillende uitvoeringsvormen voor de preventie en/of behandeling van andere ziekten gekozen uit het metabool syndroom, zoals hoge cholesterol- en bloedsuikergehalte; stoornissen van het immuunsysteem, zoals chronische ontstekingen, allergieën en auto- immuunziekten; psychische stoornissen zoals depressie en angst; en urogenitale aandoeningen, zoals urogenitale infecties bij een patiënt.
In een andere uitvoeringsvorm beschrijft de onderhavige uitvinding een farmaceutische samenstelling die B. subtilis en B. coagulans-sporen en een of meer farmaceutisch aanvaardbare dragers voor gebruik als een alternatieve therapie of aanvullende therapie voor maagzuurremmende behandelingen bij een patiënt omvat.
De farmaceutische samenstelling voor gebruik volgens de verschillende uitvoeringsvormen van de uitvinding kan verder een of meer ingrediënten omvatten die zijn gekozen uit de lijst die plantenextracten omvat, zoals gember, artisjok, eiwitten, vitaminen, mineralen of
-6- BE2019/5201 spijsverteringsenzymen, zoals amylase, cellulase, lipase, beta-galactosidase, papaïne, bromelaïne en proteasen.
In een andere uitvoeringsvorm heeft de farmaceutische samenstelling voor gebruik volgens de verschillende uitvoeringsvormen van de uitvinding de vorm van een tablet, een capsule, een staafje of elke andere eetbare vorm, waaronder maar niet beperkt tot functionele voedingsmiddelen, sap, dranken of snoepjes. In een andere uitvoeringsvorm is de farmaceutische samenstelling van de onderhavige uitvinding een product van de zuivelindustrie, drankenindustrie, voedingsindustrie of farmaceutische industrie, of het is een natuurlijk product.
In een verder aspect van de uitvinding is de combinatie of farmaceutische samenstelling volgens de verschillende uitvoeringsvormen van de uitvinding bedoeld voor gebruik bij de preventie en/ of behandeling van functionele gastro-intestinale stoornissen, zoals functionele dyspepsie, bij een patiënt. Genoemd onderwerp kan een menselijk subject zijn, zoals een volwassene of een baby. In een andere uitvoeringsvorm kan genoemde patiënt een dier zijn, een huisdier of een productiedier, maar bij voorkeur is de patient een mens. De onderhavige uitvinding verschaft ook een B. subtilis stam. Genoemde B. subtilis stam is gedeponeerd bij de Belgische gecoördineerde verzameling micro-organismen (BCCM) (Universiteit Gent, K.L. Ledeganckstraat 35, 9000 Gent, België) op 14/03/2019, met inschrijvingsnummer LMG P-31319. De onderhavige uitvinding verschaft verder ook een B. coagulans stam. Genoemde B. coagulans stam is gedeponeerd bij de Belgische gecoördineerde verzameling micro-organismen (BCCM) (Universiteit Gent, K.L. Ledeganckstraat 35, 9000 Gent, België) op 14/03/2019 met inschrijvingsnummer LMG P-31318.
KORTE BESCHRIJVING VAN DE FIGUUR Met specifieke verwijzing nu naar de figuur, wordt benadrukt dat de getoonde bijzonderheden bij wijze van voorbeeld zijn en voor doeleinden van illustratieve bespreking van de verschillende uitvoeringsvormen van de onderhavige uitvinding alleen. Ze worden gepresenteerd als de oorzaak van het verschaffen van wat, naar men denkt, de meest bruikbare en gemakkelijke beschrijving is van de principes en conceptuele aspecten van de uitvinding. In dit opzicht wordt geen poging gedaan om constructieve details van de uitvinding in meer detail te tonen dan noodzakelijk is voor een fundamenteel begrip van de uitvinding. De beschrijving die met de tekeningen is gemaakt maakt aan de vakman duidelijk hoe de verschillende vormen van de uitvinding in de praktijk kunnen worden belichaamd. GEDETAILLEERDE BESCHRIJVING VAN DE UITVINDING De onderhavige uitvinding is gebaseerd op de ontdekking dat een combinatie die B. subtilis en B. coagulans omvat bijzonder bruikbaar is bij de behandeling en/of preventie van functionele gastro-intestinale stoornissen, zoals functionele dyspepsie, bij een mens. In het bijzonder hebben de uitvinders gevonden dat een combinatie die B. subtilis en B. coagulans sporen omvat nuttig zijn bij de behandeling en/of preventie van functionele gastro-intestinale stoornissen, zoals functionele dyspepsie, bij een mens. Bovendien hebben de uitvinders gevonden dat de combinatie die B. coagulans en B. subtilis sporen omvat symptomen van epigastrische pijn en branderig gevoel, pijn op de borst en brandend maagzuur vermindert. Deze klachten worden vaak behandeld met maagzuur- suppressietherapieën zoals protonpompremmers. Daarom verschaft de uitvinding in een verdere uitvoeringsvorm een combinatie of farmaceutische samenstelling die B. coagulans en B. subtilis sporen volgens de verschillende uitvoeringsvormen van de uitvinding omvat die ook voor gebruik als een alternatieve therapie of aanvullende therapie is voor maagzuurremmende behandelingen zoals protonpompremmers in een patiënt. De onderhavige uitvinding verschaft aldus een combinatie of farmaceutische samenstelling die B. coagulans en B. subtilis omvat voor gebruik bij de behandeling van functionele gastro- intestinale stoornissen. De bacteriën zijn meestal in de vorm van bacteriële sporen of als vegetatieve bacteriële cellen, of een mengsel van beide. In een bijzonder geprefereerd geval in de onderhavige uitvinding zijn de bacteriën in de vorm van bacteriesporen die kunnen ontkiemen in het maagdarmkanaal. In een andere uitvoeringsvorm kunnen ze zodanig zijn behandeld dat zij niet kunnen ontkiemen. De sporen kunnen bijvoorbeeld behandeld zijn met warmte of zijn onderworpen aan een autoclaaf om kieming te voorkomen. In een andere uitvoeringsvorm zijn de bacteriën in de vorm van vegetatieve cellen. De sporen of vegetatieve cellen kunnen in één uitvoeringsvorm in geïsoleerde vorm worden verschaft. In nog een andere uitvoeringsvorm kan het microbiële materiaal een extract van bacteriële cellen zijn. In nog een andere uitvoeringsvorm kan het microbiële materiaal metabolieten van bacteriële cellen zijn die vrijkomen tijdens celgroei in het kweekmedium. In een andere, verdere uitvoeringsvorm kan het microbiële materiaal elke combinatie van bacteriesporen, vegetatieve bacteriële cellen, extract van bacteriële cellen of metabolieten van bacteriële cellen zijn. In een voorkeursuitvoeringsvorm worden de bacteriën in de verschillende uitvoeringsvormen van de uitvinding verschaft als levensvatbare sporen die kunnen ontkiemen in het maagdarmkanaal.
De farmaceutische formulering volgens de verschillende uitvoeringsvormen van de uitvinding vereist dat de bacteriën B. subtilis en B. coagulans of de overeenkomstige sporen daarvan geïsoleerd worden uit eventuele kweekmedia voor groei. De vakman is zich goed bewust van de technieken die beschikbaar zijn voor het isoleren van de levensvatbare bacteriën of sporen
-8- BE2019/5201 uit een groeicultuurmedium zoals het gebruik van centrifugatie, filtratie, micromanipulatie en dergelijke.
Voor opslag op middellange en lange termijn worden de geïsoleerde bacteriën bij voorkeur in een droge toestand gehouden, zoals bijvoorbeeld door het gebruik van vriesdrogen of sproeidrogen.
In één uitvoeringsvorm van de uitvinding zijn de B. subtilis en B. coagulans die worden gebruikt bij de vervaardiging van de farmaceutische samenstellingen gesproeidroogde B. subtilis en B. coagulans, verkregen door sproeidrogen van de bacteriën met behulp van een beschermend saccharide middel, in het bijzonder een trehalose als bescherming.
Een als voorbeeld dienend proces dat geschikt is voor het sproeidrogen van de bacteriën van de onderhavige uitvinding is bijvoorbeeld te vinden bij Sunny-Roberts en Knorr (Int Diary J., 19
(2009) 209-214). Eenmaal geïsoleerd, kunnen de B. subtilis en B. coagulans van de onderhavige uitvinding worden bereid met elke bekende of anderszins effectieve methode voor het farmaceutisch formuleren of vervaardigen van het geselecteerde product.
Werkwijzen voor het bereiden van de farmaceutische samenstellingen volgens de onderhavige uitvinding kunnen worden gevonden in "Remington's Pharmaceutical Sciences", Mid.

De farmaceutische samenstelling volgens de verschillende uitvoeringsvormen van de uitvinding kan bijvoorbeeld samen met gebruikelijke excipiënten, verdunningsmiddelen of dragers worden geformuleerd tot orale tabletten, capsules, sprays of orale vloeistoffen (bijvoorbeeld suspensies, oplossingen, emulsies), poeders of elke andere geschikte doseringsvorm.
Niet-beperkende voorbeelden van geschikte en farmaceutisch aanvaardbare excipiënten, verdunningsmiddelen en drager kunnen worden gevonden in "Handbook of Pharmaceutical Excipients", tweede editie, American Pharmaceutical Association, 1994, en kunnen omvatten: vulstoffen en versnijdingsmiddelen zoals zetmeel, suikers, mannitol en siliciumderivaten; bindmiddelen zoals carboxymethylcellulose en andere cellulosederivaten, alginaten, gelatine en polyvinylpyrrolidon; vocht-inbrengende middelen zoals glycerol; desintegratiemiddelen zoals calciumcarbonaat en natriumbicarbonaat; middelen voor het vertragen van oplossen zoals paraffine; resorptieversnellers zoals quaternaire ammoniumverbindingen; oppervlakte-actieve middelen zoals acetylalcohol, glycerolmonostearaat, adsorptieve dragers zoals kaoliniet en bentoniet; dragers zoals propyleenglycol en ethylalcohol en smeermiddelen zoals talk, calcium- en magnesiumstearaat en vaste polyethylglycolen.
De combinatie en farmaceutische samenstelling volgens de uitvinding zijn in de eerste plaats geschikt voor gebruik bij volwassenen en baby's.
Hoewel ze ook geschikt zijn voor gebruik bij dieren, in het bijzonder huisdieren en productiedieren.
Voorbeelden hiervan zijn honden, katten, konijnen, paarden, koeien, varkens, geiten, schapen en pluimvee.
De term "patiënt" zoals hierin gebruikt omvat daarom zowel mensen als dieren.
-9- BE2019/5201 De samenstelling voor gebruik volgens de onderhavige uitvinding kan in de vorm zijn van een tablet, een capsule, een sachet, een staafje of elke andere eetbare vorm waaronder, maar niet beperkt tot, functioneel voedsel, sap, dranken of snoepjes. De term "eetbare vorm", zoals hierin gebruikt, is bedoeld om alle consumeerbare producten, in het bijzonder voedselproducten, te omvatten, en het kan vast, in gelvorm of vloeibaar zijn. De term omvat zowel kant-en-klare producten als producten die worden geproduceerd met behulp van de probiotische samenstelling als starter alleen, of in combinatie met conventionele starters of andere probiotica. De voedselproducten kunnen bijvoorbeeld producten zijn van de zuivelindustrie of drankenindustrie. Als alternatief kan het een natuurlijk product zijn.
In de onderhavige uitvinding betekent een "zuivelproduct" elke vloeibare of half-vaste melk of wei-product met een variërend vetgehalte. Het zuivelproduct kan b.v. koemelk, geitenmelk, schapenmelk, magere melk, volle melk, melk gerecombineerd uit melkpoeder en wei zonder enige bewerking zijn, of een verwerkt product, zoals yoghurt, gestremde melk, wrongel, zure melk, zure volle melk, karnemelk, een ander zure melkproduct, zoals villi, vulling van snackrepen, enz. Een andere belangrijke groep omvat melkdranken, zoals wei-dranken, gefermenteerde melk, gecondenseerde melk, zuigelingmelk, ijs; melkbevattend voedsel zoals snoep.
De producten volgens de verschillende uitvoeringsvormen van de uitvinding kunnen ook worden geconcentreerd en als ingrediënten worden gebruikt. Verder kunnen de producten ook worden gedroogd en worden gebruikt in de vorm van poeder of lyofilisaat. De producten zijn ook toepasbaar als capsules, pillen of tabletten. De producten kunnen ook worden gebruikt bij de bereiding van functionele voedingsproducten, gezondheids- en welzijnsproducten voor consumptie, of andere overeenkomstige producten. Het kan ook in een diervoeder zitten. Mogelijke vormen zijn capsules, pillen of tabletten, bijvoorbeeld vervaardigd in conventionele werkwijzen die worden gebruikt bij de bereiding van een dergelijk product, bijvoorbeeld in de farmaceutische industrie. Dit, de vorm van elk voedingsproduct, voedselmateriaal en/of de farmaceutische producten en het diervoeder is niet in het bijzonder beperkt.
Het volgende voorbeeld illustreert de onderhavige uitvinding. Het voorbeeld mag niet worden gebruikt als een beperking van de beweringen op welke manier dan ook.
Voorbeeld 1 Objectieven van de studie Ons objectief is om het effect van sporenvormende probiotica op dyspeptische symptomen en parameters van bloed, speeksel en ontlasting bij FD-patiënten met en zonder zuuronderdrukking door protonpompremmers (PPI) te vergelijken met placebo.
_10- BE2019/5201 Primaire eindpunten Het effect van sporebiotica versus placebo op klinische symptomen, beoordeeld met de LPDS- vragenlijst.
Secundaire eindpunten Het effect van sporebiotica versus placebo op parameters van bloed, speeksel en ontlasting.
Verkennende eindpunten zijn het effect van sporebiotica versus placebo op galzuur-ademtests om bacteriële overgroei te beoordelen (cohort FD1). Studieontwerp Prospectieve gerandomiseerde, placebogecontroleerde studie in 2 FD-cohorten met open-label verlengingsfase: « cohort 1FD (met PPI's): onderzoeksprocedures bij opname (1) en na 8 weken met PPl's + sporebiotica of placebo (2) gevolgd door 8 weken met PPl's + sporebiotica (open label); «cohort 2FD (zonder PPl's): onderzoeksprocedures bij opname (1) en na 8 weken sporebiotica of placebo (2) gevolgd door 8 weken sporebiotica (open label). Studiemedicatie a) Medicinaal product en doseringsregime voor onderzoek FD-patiënten (met of zonder PPl's) zullen worden gerandomiseerd naar sporenvormende probiotica of placebo.
Sporenvormende probiotica in dit onderzoek zullen een combinatie zijn van Bacillus coagulans MYO1 en Bacillus subtilis natto MYO2 en toegediend als 1 capsule tweemaal daags (2,5 x 10° CFU per capsule) gedurende 8 weken.
Placebo zal soortgelijke capsules zijn en ook tweemaal per dag worden toegediend.
Tijdens de open-label verlengingsfase worden sporebiotica nog eens 8 weken gegeven. b) Medicinale aansprakelijkheid Resterende capsules worden elke 8 weken geteld. c) Naleving door de patiënt We zullen elke maand contact opnemen met patiënten per e-mail om de naleving te verifiëren. d) Gelijktijdige medicatie (niet het medicinaal studieproduct) Patiënten worden verzocht zich te onthouden van gelijktijdig toegediende medicatie.
Selectie en terugtrekking van patiënten a) Inclusiecriteria Patiënten komen in aanmerking voor de studie als aan alle van de volgende criteria is voldaan: - Patiënten >18 jaar met FD-diagnose (Rome IV-criteria}: - epigastrische pijn of brandend gevoel 1 of meerdere dagen per week (EPS) - postprandiale volheid of vroege verzadiging die ernstig genoeg is om invloed te hebben
_11- BE2019/5201 op reguliere activiteiten gedurende 3 of meer dagen per week (PSS) - FD-cohort 1: dagelijkse PPI-behandeling gedurende de laatste 4 weken (elk type en elke dosis) - FD-cohort 2: geen PPl-behandeling gedurende de laatste 8 weken - Man of vrouw (niet zwanger, geen borstvoeding en anticonceptie gebruikend of postmenopauzaal) - Normale stoelgang (ontlasting eenmaal per 3 dagen tot 3 keer per dag) - Schriftelijke geïnformeerde toestemming in bijzijn van getuige - Toegang tot thuisvriezer (-18 tot -20 °C) - Geschikt om de studievereisten te begrijpen en eraan te voldoen b) Exclusiecriteria Patiënten worden uitgesloten van het onderzoek als aan een van de volgende criteria wordt voldaan: - Elke actieve somatische of psychiatrische aandoening die dyspeptische symptomen kan verklaren (stabiele dosis enkelvoudig antidepressivum toegestaan voor psychiatrische indicatie, geen beperking voor andere indicaties) - Overheersende symptomen van gastro-oesofageale refluxziekte (GORZ) of prikkelbare darm- syndroom (PDS) - Gebruik van immunosuppressiva of antibiotica <3 maanden - Geschiedenis van grote buikoperaties (behalve blindedarmoperatie, cholecystectomie of splenectomie) - Persoonlijke voorgeschiedenis van diabetes mellitus type 1, coeliakie of inflammatoire darmaandoening - Diabetes mellitus type 2 (inclusief therapie) - Actieve maligniteit (inclusief therapie) - Bekende HIV-, HBV- of HCV-infectie (inclusief therapie) - Magnetiseerbare objecten (bijvoorbeeld cochleaire implantaten, neurostimulator, pacemaker, metaalfragmenten of implantaten) of claustrofobie (MRI-veiligheidscriteria met aanvullende toestemmingsvorm alleen in cohort 2FD, andere procedures kunnen nog steeds plaatsvinden) - Aanzienlijk alcoholgebruik (> 10 eenheden/week) c) Selectie van de deelnemers FD-patiënten zullen worden geworven op de polikliniek voor algemene en functionele aandoeningen van de maag-darm of bij de endoscopie-eenheid.
Advertenties worden verspreid via het tijdschrift 'Metro' en opgehangen als posters in het ziekenhuis en de universiteit, die ook worden geüpload onder 'oproepstudies' op de intranetwebsite van UZ Leuven.
Patiënten die op deze advertentie reageren, worden uitgenodigd voor een studiebezoek.
Patiënten kunnen ook worden gerekruteerd uit verwijzende ziekenhuizen als aan alle inclusie- en exclusiecriteria is
_12- BE2019/5201 voldaan. d) Randomisatieprocedure De randomisatielijst tussen sporebiotica en placebo zal worden gedaan door de sponsor met behulp van een online randomisatietoo! (http://www.randomization.com/) met blokgrootten van
5. De randomisatielijst wordt door de sponsor bewaard voor zowel cohort 1FD en 2FD. De onderzoeksbehandeling zal worden genummerd volgens de randomisatielijst en wordt verstrekt door de sponsors aan de onderzoekers die blind blijven voor de randomisatiecode. e) Terugtrekking van patiënten De patiënt heeft het recht om zich op elk moment terug te trekken uit het onderzoek, zonder het toekomstig beheer en behandeling te beïnvloeden. De patiënt kan uit het onderzoek worden teruggetrokken als: - De patiënt een ongewenst voorval ervaart dat door de onderzoeker als schadelijk beschouwd wordt voor het welzijn van de patiënt - De patiënt de geschiktheidscriteria van het onderzoeksprotocol schendt - De patiënt de behandeling begint met verboden gelijktijdige medicatie - De patiënt niet voldoet aan de vereiste onderzoeksprocedures, bijvoorbeeld het invullen van de intake of follow-up (maandelijkse) vragenlijsten Afhankelijk van de uitgevoerde procedures en de ingevulde vragenlijsten, kunnen gegevens van een patiënt die zich terugtrekt uit het onderzoek nog steeds worden gebruikt. fl Verwachte tijdsduur van de studie De duur van het onderzoek is 16 weken. De duur van de volledige proef van het eerste bezoek van de eerste patiënt tot het laatste bezoek van de laatste patiënt zal naar verwachting 24 maanden zijn.
