Abstract

血管内の物体を効率的に切断しつつ駆動シャフトの内部に取り込むことのできる医療デバイスを提供する。先端部25を有し軸Xを中心に回転可能な駆動シャフト20と、先端部25に設けられ、駆動シャフト20の回転により回転して物体を切断する刃面部40と、を含み、駆動シャフト20は、少なくとも刃面部40の基端側に駆動シャフト20の他の部分より屈曲しやすい柔軟部50を有し、駆動シャフト20の回転により柔軟部50が軸X方向に対して屈曲する医療デバイスである。

Description

医療デバイス
 本発明は、生体管腔の物体を除去するための医療デバイスに関する。
 血管内のプラークや血栓などによる狭窄部の治療方法は、バルーンにより血管を拡張する方法や、網目状またはコイル状のステントを血管の支えとして血管内に留置する方法などが挙げられる。しかしながら、これらの方法は、石灰化により硬くなっている狭窄部や、血管の分岐部で生じている狭窄部を治療することは、困難である。このような場合においても治療が可能な方法として、プラークや血栓などの狭窄物を切削して除去する方法がある。
 例えば特許文献1には、物体を切削する切削端が、回転する駆動シャフトの先端部に設けられたデバイスが記載されている。このデバイスは、駆動シャフトを回転させて、切削端により物体を切断し、駆動シャフトの内部に取り込むことができる。また、切削端の側面には、側孔が設けられており、側孔からも駆動シャフトの内部に物体を取り込むことができる。このデバイスは、手元に、吸引力を作用させる排出ポートを備えたハウジングを備えている。駆動シャフトの内部に取り込まれた物体は、基端側へ移動される。この後、物体は、駆動シャフトの排出孔からハウジング内に移動し、排出ポートから排出される。

 大径の血管、特に静脈においては、血液の流速が小さいため、大きな血栓が生じやすい。大きな物体を駆動シャフトの内部に取り込むには、物体を効率的に細かく切断する必要がある。
 本発明は、上述した課題を解決するためになされたものであり、血管内の物体を効率的に切断しつつ駆動シャフトの内部に取り込むことのできる医療デバイスを提供することを目的とする。
 上記目的を達成する本発明に係る(1)医療デバイスの一態様は、先端部を有し軸を中心に回転可能なシャフトと、前記先端部に設けられ、前記シャフトの回転により回転して物体を切断する刃面部と、を含み、前記シャフトは、少なくとも前記刃面部の基端側に前記シャフトの他の部分より屈曲しやすい柔軟部を有し、前記シャフトの回転により前記柔軟部が軸方向に対して屈曲する。
 上記目的を達成する本発明に係る(10)医療デバイスの他の態様は、先端部を有し軸を中心に回転可能なシャフトと、前記先端部に設けられ、前記シャフトの回転により回転して物体を切断する刃面部と、前記刃面部の基端側に設けられる少なくとも1つの柔軟部と、を含み、前記シャフトの回転により前記先端部が円錐形状をなすように回転する。
 上記のように構成した(1)、(10)の医療デバイスは、シャフトが回転することで柔軟部において屈曲し、先端部が首を振るように公転運動するため、刃面部を血管内の広い範囲に向けることができて、血管内の物体を効率的に切断することができる。
 (2)上記(1)の医療デバイスにおいて、前記柔軟部は、前記シャフトの前記刃面部の基端側に、軸方向に互いに離隔して2つ設けられていてもよい。これにより、医療デバイスは、シャフトが2箇所で屈曲し、シャフトの回転に伴いシャフトの先端部がより複雑に公転運動するため、刃面部をより広い範囲に向けることができる。
 (3)上記(1)または(2)の医療デバイスにおいて、前記シャフトの前記刃面部より先端側に先端柔軟部が設けられていてもよい。これにより、医療デバイスは、シャフトの回転に伴い、台形状をなすように先端部を変形させることができ、血管の外周側または内周側の血栓を刃面部で効果的に切断できる。
 (4)上記(1)~(3)のいずれかの医療デバイスにおいて、前記柔軟部は、前記シャフトの外側から、または、前記シャフトの内側から、カバーチューブで覆われていてもよい。これにより、医療デバイスは、柔軟部がスリット状である場合に、屈曲した際にスリットの端面が血管壁に接触しないようにし、また、スリットが開いた際の吸引力低下を抑制できる。
 (5)上記(1)~(4)のいずれかの医療デバイスにおいて、前記刃面部は、周面を有する中空状の本体部を備え、前記本体部は、前記周面に設けられて中空内部と外部とを連通させる開口部と、前記開口部の回転方向に向かって鋭利な刃面と、を含むようにしてもよい。これにより、医療デバイスは、刃面部を形成する本体部の内外を連通させる開口部に鋭利な刃面を有しているので、シャフトの回転によって開口部に位置する物体を刃面で切断して細かくし、本体部の内部に取り込むことができるので、物体を効率的に切断しつつ除去することができる。
 (6)上記(5)の医療デバイスにおいて、前記開口部は、当該開口部の先端縁と前記刃面の最先端との間を離隔させる切欠部を有し、前記刃面の最先端は鋭利な端部を有してもよい。これにより、医療デバイスは、刃面の最先端に形成される鋭利な端部を物体に刺すことができ、物体をより確実に切断することができる。
 (7)上記(1)~(6)のいずれかの医療デバイスにおいて、前記先端部は、前記軸から重心が偏心していてもよい。これにより、医療デバイスは、シャフトを回転させた際に、先端部が首を振りやすくすることができ、柔軟部をより屈曲させやすくすることができる。
 (8)上記(7)の医療デバイスにおいて、前記先端部の重心は、前記刃面部の前記刃面の位置する側と前記軸を挟んで反対側に位置してもよい。これにより、医療デバイスは、シャフトを回転させた際に、刃面部がシャフトの公転運動の内周側を向くようにすることができ、刃面部が血管壁に接触しないようにすることができる。
 (9)上記(2)の医療デバイスにおいて、前記シャフトの最先端は、前記刃面部を備えた切削端を有してもよい。これにより、医療デバイスは、シャフトの先端部が公転運動しつつ切削端で効率よく物体の切断を行うことができる。
 (11)上記(10)の医療デバイスにおいて、前記刃面部は、内部に空間を有する本体部を備え、前記本体部は、前記空間内に前記物体を取り込み可能に開口し、前記シャフトの軸方向に平行または非平行な一対の開口縁を備えた開口部と、を含み、前記一対の開口縁の少なくともいずれか一方は、鋭利な刃面であってもよい。これにより、医療デバイスは、刃面部を形成する本体部の内外を連通させる開口部に鋭利な刃面を有しているので、シャフトの回転によって開口部に位置する物体を刃面で切断して細かくし、本体部の内部に取り込むことができるので、物体を効率的に切断しつつ除去することができる。
 (12)上記(11)の医療デバイスにおいて、前記刃面は、前記軸を中心とした円周方向において、前記軸の基端側に向かって前記シャフトの回転方向と逆方向に傾斜するように設けられていてもよい。これにより、医療デバイスは、開口部の先端側の部分で捉えた物体を刃面で切断しやすくすることができる。
 (13)上記(10)の医療デバイスにおいて、前記シャフトの前記軸と同方向に沿って設けられ、ガイドワイヤが挿通可能なルーメンを有してもよい。これにより、医療デバイスは、ルーメンにガイドワイヤを挿通してシャフトを血管内で誘導することができる。
 (14)上記(13)の医療デバイスにおいて、前記ルーメンは、前記シャフトの前記軸の方向に沿って設けられる内管に有してもよい。これにより、医療デバイスは、内管に挿通して先端側に突出させたガイドワイヤにより、血管内でシャフトを誘導することができる。
 (15)上記(13)の医療デバイスにおいて、前記シャフトの外周側に外管を有し、前記ルーメンは前記外管に有してもよい。これにより、医療デバイスは、外管に挿通して先端側に突出させたガイドワイヤにより、血管内でシャフトを誘導することができる。

 以下、図面を参照して、本発明の実施の形態を説明する。なお、図面における各部材の大きさや比率は、説明の都合上誇張され実際の大きさや比率とは異なる場合がある。
 本実施形態に係る医療デバイス10は、急性下肢虚血や深部静脈血栓症において、血管内に挿入され、血栓やプラークなどの物体を切断して除去する処置に用いられる。本明細書では、デバイスの血管に挿入する側を「先端側」、操作する手元側を「基端側」と称することとする。なお、除去する物体は、必ずしも血栓やプラークに限定されず、生体管腔内に存在し得る物体は、全て該当し得る。
 医療デバイス10は、図1、2に示すように、駆動力および吸引力を発生させる駆動デバイス200に連結されて駆動される。医療デバイス10は、長尺であって回転駆動される駆動シャフト20と、駆動シャフト20を収容しシース31に覆われた外管30と、物体を切断する刃面部40を有する先端部25とを備えている。医療デバイス10は、さらに、駆動シャフト20の内部に配置される内管35と、駆動シャフト20の基端部に固定される基端シャフト90とを備えている。医療デバイス10は、さらに、駆動シャフト20の基端部を回転可能に収容するハウジング60と、術者が外管30を操作する操作ハンドル80と、駆動デバイス200から回転力を受ける回転入力部110とを備えている。
 駆動シャフト20は、刃面部40を有する先端部25に回転力を伝達する。駆動シャフト20は、切削した物体を基端側へ移動させるための吸引ルーメン21(内腔)が形成されている。駆動シャフト20は、駆動シャフト本体22と、基端部に固定される駆動シャフト基端部23と、補助部材24を備えている。駆動シャフト20は、外管30を貫通している。駆動シャフト20の基端部は、ハウジング60の内部に位置している。駆動シャフト20の基端部は、基端シャフト90に連結される。駆動シャフト20は、先端部25に、吸引ルーメン21が開口する開口部42を有している。開口部42は、切断により形成される吸引対象物であるデブリが入り込む入口である。駆動シャフト20は、基端部に、基端開口部26を有している。基端開口部26は、開口部42から駆動シャフト20の内部に入ったデブリが排出される出口である。基端開口部26は、駆動シャフト20の周方向の一部を欠落させて形成される。基端開口部26は、駆動シャフト20の基端部に、駆動シャフト20の内壁表面と外壁表面を貫通して形成される。したがって、基端開口部26は、側方(駆動シャフト20の中心軸Xと直交する方向)へ向かって開口している。基端開口部26の先端側の縁部26aは、中心軸Xと略垂直に形成されている。基端開口部26の基端側の縁部26bは、基端側へ向かって凸状となる円弧で形成される。基端開口部26は、回転方向に向かって鋭利な刃面を設けることにより、駆動シャフト20の回転によって基端開口部26に位置する物体を刃面で切断して細かくし、ハウジング60の外部に排出することができるので、物体を効率的に切断しつつ除去することができる。
 駆動シャフト本体22は、柔軟で、かつ基端側から作用する回転の動力を先端側に伝達可能な特性を有する。
 駆動シャフト基端部23は、略円管形状であり、基端開口部26が形成されている。駆動シャフト基端部23の先端部は、駆動シャフト20の外周面に被さって固定されている。駆動シャフト基端部23は、ハウジング60の内部に配置された第1の軸受67および第2の軸受68により回転可能に支持される。このため、駆動シャフト20は、高速で滑らかに回転可能である。なお、駆動シャフト20の形態は、回転の動力を伝達でき、内腔および基端開口部26を有すれば、特に限定されない。
 駆動シャフト基端部23の外周面には、駆動シャフト基端部23よりも中心軸Xに沿う長さが短い補助部材24が被さって固定されている。補助部材24は、略円管形状である。補助部材24には、基端開口部26と略同形状の開口部が、基端開口部26と重なる位置に形成されている。補助部材24の先端端面は、駆動シャフト基端部23を支持する第1の軸受67に突き当たっている。補助部材24の基端端面は、駆動シャフト基端部23を支持する第2の軸受68に突き当たっている。このため、補助部材24は、駆動シャフト基端部23に対する第1の軸受67および第2の軸受68の位置を、適切に保持する。また、補助部材24は、基端開口部26が形成される駆動シャフト基端部23を補強する。なお、補助部材24は、設けられなくてもよい。
 基端シャフト90の先端部は、駆動シャフト基端部23の内腔に基端側から入り込み、駆動シャフト基端部23に固定されている。基端シャフト90は、基端部に、駆動デバイス200から回転力を受ける回転入力部110が固定されている。基端シャフト90の基端部は、ハウジング60から基端側へ突出している。
 駆動シャフト本体22の構成材料は、特に限定されないが、例えば、ステンレス、Ta、Ti、Pt、Au、W、ポリエチレン、ポリプロピレンなどのポリオレフィン、ポリアミド、ポリエチレンテレフタレートなどのポリエステル、ETFE等のフッ素系ポリマー、PEEK(ポリエーテルエーテルケトン)、ポリイミド、などが好適に使用できる。また、複数の材料によって構成されてもよく、線材などの補強部材が埋設されてもよい。
 駆動シャフト基端部23、補助部材24および基端シャフト90の構成材料は、特に限定されないが、例えば、ステンレス、Ta、Ti、Pt、Au、W、形状記憶合金などが好適に使用できる。駆動シャフト基端部23、補助部材24および基端シャフト90の構成材料は、PEEK(ポリエーテルエーテルケトン)などのエンジニアリングプラスチック等の樹脂でもよい。
 回転入力部110は、図2に示すように、駆動シャフト20の基端端に固定される略円柱状の部材である。回転入力部110は、駆動デバイス200の回転駆動軸221と連結して回転の動力を受け取る部材である。回転入力部110の基端部は、回転駆動軸221が嵌合する嵌合凹部111を備えている。
 内管35は、図2に示すように、可撓性を備える管体であり、駆動シャフト20の内部に配置される。内管35は、駆動シャフト20の内部を流れるデブリにより、駆動シャフト20の内周面が損傷することを抑制する。内管35は、駆動シャフト20の内周面に固定されてもよいが、固定されなくてもよい。なお、内管35は、設けられなくてもよい。
 ハウジング60は、図2に示すように、先端側に配置される第1の部位61と、略中央に配置される第2の部位62と、基端側に配置される第3の部位63と、第4の部位64とを備えている。ハウジング60は、さらに、後述する駆動デバイス200の吸引チューブ231を接続可能な排出ポート70を有している。ハウジング60は、これらの部材が組み立てられることで、内部空間65を形成している。内部空間65は、排出ポート70の通路71と連通し、排出ポート70に接続される駆動デバイス200の吸引チューブ231と連通する。ハウジング60の内部には、第1の軸受67と第2の軸受68が配置されており、それぞれ駆動シャフト20をハウジング60に対して回転可能に支持する。
 第4の部位64は、第3の部位63と基端シャフト90の間に配置されるシール部69を保持している。シール部69は、内部空間65を密封しつつ、基端シャフト90の回転を許容する。
 駆動シャフト20の基端開口部26は、内部空間65に位置している。このため、吸引チューブ231から排出ポート70に作用する陰圧は、基端開口部26から駆動シャフト20の内部に作用する。
 排出ポート70は、図2に示すように、略円筒形状であり、一方の端部がハウジング60の外部側に位置し、他方の端部がハウジング60の内部空間65と連通している。排出ポート70は、一方の端部から他方の端部へ貫通する通路71が形成されている。
 駆動デバイス200は、図1に示すように、回転力を発生させる駆動部220と、吸引力を発生させる吸引部230とを備えている。
 駆動部220は、回転駆動軸221と、回転駆動軸221を回転させる第1のモータ222とを備えている。第1のモータ222の回転速度は、特に限定されないが、例えば5,000~200,000rpmである。
 吸引部230は、吸引チューブ231と、ポンプ232と、第2のモータ233と、廃液パック235とを備えている。吸引チューブ231は、医療デバイス10の排出ポート70に接続可能である。ポンプ232は、第2のモータ233により駆動されて、吸引チューブ231に陰圧を作用させる。また、ポンプ232は、吸引チューブ231を介して吸引した流体を、廃液パック235へ排出する。
 なお、駆動デバイス200の構成は、上述の例に限定されない。例えば、回転力を発生させる機構と、吸引力を発生させる機構は、別々のデバイスであってもよい。
 駆動シャフト20の先端部25に設けられる刃面部40は、図1、3に示すように、血栓、プラークや石灰化病変等の物体を切断して小さくする部材である。 ”切断”とは、接触する物体に力を作用させて、物体を小さくすることを意味する。切削後の物体の形状や形態は、限定されない。
 図3に示すように、駆動シャフト20の先端部25は、刃面部40と、刃面部40の基端側に配置される柔軟部50と、を備えている。先端部25の最先端部28は、先端に向かって外径が小さくなる弾丸状の形状を有している。
 刃面部40は、内腔45を有する中空で円筒状の本体部41を備えている。内腔45は、駆動シャフト20の吸引ルーメン21と連通している。本体部41は、駆動シャフト20の軸Xを中心とした周方向に延在する外周面41aを有し、この外周面41aに開口部42が設けられて、本体部41の中空内部と外部とを連通させる。開口部42は、空間である内腔45に物体を取り込み可能となるように開口している。開口部42は、駆動シャフト20の軸X方向にほぼ平行な第1開口縁42aと、第1開口縁42aと向かい合い軸X方向とは非平行な第2開口縁42bとで一対の開口縁を形成している。この一対の開口縁のうちの一方である第2の開口縁42bは、鋭利な刃面43である。第1の開口縁42aは、刃状となっていない。第1開口縁42aと第2開口縁42bは、開口部42の先端側において先端開口縁42cを介して連なっている。第1開口縁42aと第2開口縁42bは、開口部42の基端側において基端開口縁42dを介して連なっている。
 刃面43は、駆動シャフト20の軸X方向と非平行な方向に沿って延びている。刃面43は、軸Xを中心とした円周方向において、軸Xの基端側に向かって、第1開口縁42aとの距離が広がるように延びている。図3において、駆動シャフト20は、先端側から見て左周りの方向に回転する。刃面43は、軸Xを中心とした円周方向において、軸Xの基端側に向かって、先端側から見て駆動シャフト20の回転方向に対し他方周りとなる右回りの方向に沿う螺旋状に設けられる。開口部42は、当該開口部42の先端開口縁42cと刃面43の最先端との間を離隔させる切欠部44を有しており、刃面43の最先端は鋭利な端部43cを有する。切欠部44は、先端開口縁42cである切欠部先端側縁44aから切欠部基端側縁44bに掛けて滑らかに連なるU字状の溝である。しかしながら、鋭利な端部43cを構成するかぎり上記形状に限定されない。
 図4(a)に示すように、刃面43は、駆動シャフト20の回転により開口部42が回転する方向に向くように配置されている。刃面43は、開口部42の縁面を構成する第1面43aと、本体部41の周面41aを構成する第2面43bとが鋭角をなすように交わることで、物体を切断することが可能となる鋭利な形状を形成している。このため、刃面部40は、駆動シャフト20が回転すると開口部42に位置している物体を切断することができる。切断された物体は、本体部41の内腔45に取り込まれる。なお、図4(b) に示すように、開口部42の縁面を構成する第1面43aと、本体部41の内周面41bを構成する第2面43cとが鋭角をなすように交わって刃面43としても良い。これにより、物体の切断を行いつつ血管内壁への影響を抑制することが可能となる。
 図3に示すように、駆動シャフト20の開口部42より基端側には、駆動シャフト20の他の部分より屈曲しやすい柔軟部50が設けられる。柔軟部50は、周方向に沿って、複数の凹凸形状を有するように蛇行しつつ螺旋状に延びるスリットを駆動シャフト20に設けることで形成されている。ただし、柔軟部50は、駆動シャフト20の他の部分より屈曲しやすければよいため、図3に示すスリット形状以外の構成により柔軟にするものであってもよい。
 図5に示すように、駆動シャフト20の先端部25は、最先端部28が軸Xに対して非対称な形状を有している。このため、最先端部28の重心位置は、軸Xから偏心しており、最先端の位置28aは、軸Xから離れている。
 操作ハンドル80は、術者が把持する持ち手部81と、持ち手部81の先端部に回転可能に設けられる操作部82とを有している。図6(a)に示すように、操作ハンドル80より先端側に延びる外管30は、シース31に覆われている。シース31の先端部31aは、外管30や駆動シャフト20と共に直線状の状態となっている。図6(b)に示すように、操作ハンドル80の操作部82を回転操作することで、シース31の先端部31aが湾曲する。これにより、駆動シャフト20及び外管30は先端部31aの湾曲された方向に屈曲し、駆動シャフト20の先端部25を別の方向に向けることができる。別の態様としては、外管30が、先端部が予め一方向に向かって屈曲するように形状付けされており、シース31に覆われた状態では、直線状の状態となっていてもよい。この場合、操作ハンドル80の操作部82を回転操作することで、シース31が基端側に移動し、外管30の先端部が露出する。これにより、外管30は予め形状付けされた方向に屈曲し、駆動シャフト20の先端部25を別の方向に向けることができる。
 操作ハンドル80には、チューブ85が接続され、チューブ85の先には三方活栓86が接続されている。チューブ85は操作ハンドル80の内部と連通している。また、操作ハンドル80内において、駆動シャフト20の吸引ルーメン21が開放している。このため、三方活栓86を介して吸引ルーメン21に流体を注入する、または、吸引ルーメン21から流体を吸引することができる。
 医療デバイス10を血管内の物体を切断して吸引するために使用する方法につき説明する。術者は、予め、医療デバイス10の先端部を病変部の近傍に到達させておく。次に、術者は、駆動デバイス200の回転駆動軸221を、回転入力部110に接続する。続いて、術者は、吸引チューブ231を排出ポート70に接続する。この後、術者は、駆動デバイス200を作動させる。これにより、回転駆動軸221の回転と、吸引チューブ231の吸引が開始される。回転駆動軸221は、回転入力部110を回転させる。これにより、回転入力部110に固定された基端シャフト90が回転し、基端シャフト90とともに、駆動シャフト20が回転する。前述のように、駆動シャフト20の回転により、開口部42において回転方向を向く刃面43が物体を切断し、切断された血管は本体部41の内腔45に取り込まれる。
 図7に示すように、血管300内で駆動シャフト20が回転すると、駆動シャフト20の先端部25は、重心位置が軸Xから偏心した最先端部28が外側となるように、柔軟部50において屈曲し、首を振るように回転する。なお、図7を含む駆動シャフト20が回転した状態の図において、屈曲部50は網掛けで示している。図8に示すように、駆動シャフト20の先端部25は、駆動シャフト20の自転により軸Xを中心として回転しつつ、首振りにより公転運動し、先端側から見て円形状の軌跡を描くように回転する。また、図7に示すように、正面側から見ると、駆動シャフト20の先端部25は、円錐形状をなすように回転する。すなわち、屈曲部50を円錐の頂点としたときに、最先端28aが円錐底面の縁に沿った動きを呈する。これにより、刃面部40の開口部42は、血管300内において広い範囲を振れ回ることができ、効率的に物体を切断することができる。駆動シャフト20の回転数が小さく、駆動シャフト20が血管壁あるいはその他の物体からの抵抗を大きく受けない場合、駆動シャフト20の先端部25が自転する周期と、駆動シャフト20の先端部25が首を振るように公転する周期は同じとなる。刃面43を有する開口部42は、最先端部28の重心位置とは軸Xを挟んで反対側を向くように配置されているので、駆動シャフト20の回転時には、常に駆動シャフト20の回転中心側を向いた状態となる。このため、刃面43が血管壁に向かないようにすることができる。ただし、開口部42を最先端部28の重心位置と同じ側を向くように配置してもよく、この場合は、開口部42を常に血管壁側に向いた状態とすることができる。駆動シャフト20の回転数や血管壁への接触状態あるいは捩れ状態などによっては、駆動シャフト20の先端部の自転周期が公転周期より早くなることもあり、この場合は、開口部42が血管300内において様々な方向を向きながら、物体を切断する。
 吸引チューブ231は、排出ポート70およびハウジング60の内部空間65を介して駆動シャフト20の吸引ルーメン21に陰圧を作用させる。これにより、刃面部40の刃面43により切断された物体は、デブリとなって、本体部41の内腔45に取り込まれ、駆動シャフト20の吸引ルーメン21を基端側へ移動する。
 吸引されたデブリは、駆動シャフト本体22の基端端を通過し、側孔である基端開口部26に到達する。基端開口部26に到達したデブリは、基端開口部26からハウジング60の内部空間65へ排出される。内部空間65に排出されたデブリは、排出ポート70を経て吸引チューブ231へ排出される。
 排出ポート70から外部へ排出されたデブリは、吸引チューブ231を通ってポンプ232に到達する。ポンプ232に到達したデブリは、廃液パック235に排出される。病変部の切削およびデブリの吸引が完了した後、術者は、駆動デバイス200の動作を停止させる。これにより、駆動シャフト20の回転が停止し、かつポンプ232の吸引が停止する。これにより、刃面部40による物体の切断およびデブリの排出が停止される。この後、医療デバイス10を血管から抜去し、処置が完了する。
 柔軟部の変形例について説明する。図9(a)に示すように、駆動シャフト20に形成される柔軟部52は、周方向に沿って延びる短いスリットが、軸方向に互い違いに並んだ形状であってもよい。図9(b)に示すように、駆動シャフト20に形成される柔軟部53は、スリットが連続した螺旋形状を有する形状であってもよい。この場合、駆動シャフト20の厚みを調整したり後述するカバーチューブを設置したりすることにより、駆動力を伝達することが可能となる。さらに、図9(c)に示すように、駆動シャフト20に形成される柔軟部54は、螺旋状に延びる短いスリットが軸方向に互い違いに並んだ形状であってもよい。図9(d)に示す変形例の柔軟部56は、複数の環状体56aが隣接する環状体56aと軸方向に離脱しないように係合する一対の係合部56bを各々備えている。互いに隣り合う環状体56aの間には、円周方向から見て180°に位置する一対の係合部56bに対して90°の方向に一対の隙間56cを備えているので、一対の隙間56cを有する方向に曲がることが可能となる。図9(f)には別の変形例の柔軟部58の正面図を、図9(g)には図9(f)の平面図を、図9(e)には図9(f)のA-A断面図を、それぞれ示している。図9(e)~(f)に示す変形例の柔軟部58は、複数の環状体58aと、複数の環状体58aが互いに軸方向に離脱しないように、互いに環状体58aの円周方向180°の角度をなす位置で環状体58a同士を接続する一対の橋掛部58bと、から構成されている。互いに隣り合う環状体58aは、一対の間隙58cで隔てられているので、一対の間隙58cの方向に曲がりやすくなる。なお、柔軟部は、駆動シャフト20の他の部分より屈曲しやすければ、これら以外の形状を有していてもよい。
 図10に示すように、駆動シャフト20は、刃面部40より基端側に、柔軟部50と第2の柔軟部54とを軸方向に互いに離隔して有していてもよい。図11に示すように、柔軟部50と第2の柔軟部54とを有する駆動シャフト20を血管300内で回転させると、駆動シャフト20の先端部25は、柔軟部50と第2の柔軟部54がそれぞれ滑らかに屈曲し、より自由度の高い状態で公転運動する。これによって、血管300の内壁に対するダメージを抑えつつ、開口部42は、血管300内をくまなく動き回り、血栓をより効率的に切断することができる。
 図12(a)に示すように、駆動シャフト20の外面には、柔軟部50と第2の柔軟部54とを覆うようにカバーチューブ120を設けてもよい。カバーチューブ120は、柔軟性を有する材料で形成される。柔軟部50や第2の柔軟部54は、屈曲すると屈曲外周側のスリットが開き、これが血管壁に接触する可能性がある。カバーチューブ120で柔軟部50や第2の柔軟部54を覆うことで、屈曲した際にスリットの端面が血管壁に接触しないようにすることができる。また、カバーチューブ120により、柔軟部50や第2の柔軟部54が屈曲してスリットが開くことで、駆動シャフト20の吸引ルーメン21に生じる吸引力の低下を抑制できる。また、カバーチューブ120により、柔軟部50や第2の柔軟部54の過剰な屈曲を抑制することもできる。
 図12(b)に示すように、カバーチューブ122は、駆動シャフト20の内面に設けられて、柔軟部50と第2の柔軟部54とを覆うようにしてもよい。また、駆動シャフト20の外面のカバーチューブ120または駆動シャフト20の内面のカバーチューブ122は、1つの柔軟部50を有する駆動シャフト20に設けられてもよい。
 図13に示すように、駆動シャフト20の最先端部28は、軸Xに対して偏心する最先端に軸X方向に向かって突出する突起部140を有していてもよい。図14に示すように、駆動シャフト20が突起部140を有していることで、刃面部40を含む先端部25を、血管300を閉塞するような血栓310に挿入しやすくすることができる。また、先端部25が血栓310に挿入された状態で駆動シャフト20が回転し、柔軟部50が屈曲することで、駆動シャフト20の柔軟部50より基端側の部分が公転運動し、駆動シャフト20の血栓310に挿入された部分に力を加えることができる。これにより、刃面部40で血栓310を効率的に切断して除去することができる。
 図15に示すように、駆動シャフト20の最先端部29は、軸Xに対して偏心していない回転対称な形状を有していてもよい。この場合に、最先端部29は、軸X上に先端側に向かって突出する突起部141を有することができる。
 図16に示すように、駆動シャフト20は、刃面部40の基端側に配置された柔軟部55と、刃面部40の先端側に配置された先端柔軟部56とを有してもよい。この場合、駆動シャフト20が回転することで、柔軟部55と先端柔軟部56とが屈曲し、先端部25は台形状をなすように変形して回転する。柔軟部55と先端柔軟部56が屈曲した状態で、刃面部40が外周側を向くように配置されている場合、刃面部40は、血管壁に近い位置にある血栓を効果的に切断できる。柔軟部55と先端柔軟部56が屈曲した状態で、刃面部40が内周側を向くように配置されている場合、刃面部40は、血管の中央部にある血栓を効果的に切断できる。
 柔軟部55は、軸方向において刃面部40に近い部分55aと遠い部分55bとが、互いに逆方向に向かって屈曲する。このため、図17に示すように、柔軟部55は、スリットを挟んで隣接する部分を繋ぐ架橋部55cを、刃面部40に近い部分55aの遠い部分55bのそれぞれについて、屈曲内周側にのみ有する形状に形成されている。これにより、柔軟部55が架橋部55cを有する側を内周側とするように屈曲し、駆動シャフト20の回転時において図16に示すような一定の形状を形成することができる。なお、先端柔軟部56についても、一定の方向に屈曲するように同様の構成を有することができる。また、柔軟部55および先端柔軟部56が一定の方向に屈曲するようにするため、柔軟部55および先端柔軟部56の部分に予め形状付けをしておき、駆動シャフト20の血管への挿入時には、吸引ルーメン21に芯金を入れて柔軟部55および先端柔軟部56を真っ直ぐにしておいて、駆動シャフト20を回転させる際には芯金を抜去して、柔軟部55および先端柔軟部56が一定の形状をなすようにしてもよい。
 医療デバイスの別の変形例について説明する。図18~図24において、駆動シャフトの軸と同方向に沿って設けられ、ガイドワイヤが挿通可能なルーメンを有する医療デバイスの態様を示す。図18~20に示すように、先端部に刃面部153および柔軟部154を備える駆動シャフト150は、吸引ルーメン151内に軸Xに沿って内管152を有している。内管152は、ガイドワイヤを挿通するガイドワイヤルーメン152aを有している。図21に示すように、最先端部156は、駆動シャフト150の軸Xから偏心して最先端156aが位置し、ガイドワイヤルーメン152aの先端は、駆動シャフト20の先端側に向かって開口する開口部157を有する。このため、ガイドワイヤルーメン152aにガイドワイヤを挿通して駆動シャフト150を誘導することができる。
 さらに別の変形例について説明する。図22、23に示すように、駆動シャフト160の外周側に設けられる外管167には、軸方向一部に吸引ルーメン161と並行するようにガイドワイヤルーメン168が設けられる。ガイドワイヤルーメン168には、ガイドワイヤ169が挿通され、駆動シャフト160の挿入を誘導する。このように、ガイドワイヤルーメン168は、外管167の全長に渡って設けてもよいが、図22のようにラピッドエクスチェンジ(RX)型となるように設けてもよい。
 血管300内で外管167より先端側に位置する駆動シャフト160が回転すると、柔軟部164において屈曲し、首を振るように公転運動する。この公転運動を阻害しないように、駆動シャフト160を回転させる際には、図24に示すように、ガイドワイヤ169の先端部を予め基端側に移動させておく。
 さらに別の変形例について説明する。図25に示すように、駆動シャフト170は、最先端に刃面部173aを備えた切削端173を有している。また、駆動シャフト170は、切削端173より基端側に、軸方向に離隔して柔軟部174と先端柔軟部175とを備え、柔軟部174と先端柔軟部175との間は中間部176である。
 図26に示すように、駆動シャフト170の中間部176には、肉厚内部の周方向一部分に、駆動シャフト170の他の部分より比重の大きい錘部177が設けられている。中間部176が錘部177を有していることにより、中間部187の重心は駆動シャフト170の中心軸から偏心し、図27に示すように、血管300内で駆動シャフト170を回転させると、柔軟部174と先端柔軟部175とが屈曲し、駆動シャフト170は台形状をなすように変形して回転する。これに伴い、駆動シャフト170の最先端に設けられた切削端173は、血管300の中心軸に沿って回転することができ、刃面部173aによって血管300の中央部にある血栓を効果的に切断できる。図26において、錘部177は、中間部176の肉厚内に埋め込まれた態様で図示されているが、変形例として、錘部177が中間部176の外周に配置されていてもよい。この場合、錘部177は、必ずしも駆動シャフト170の他の部分より比重が大きい必要はなく、中間部187の重心を駆動シャフト170の中心軸から偏心させることができれば、錘部177の比重は特に限定されない。
 以上のように、本実施形態に係る(1)医療デバイス10は、先端部25を有し軸Xを中心に回転可能な駆動シャフト20と、先端部25に設けられ、駆動シャフト20の回転により回転して物体を切断する刃面部40と、を含み、駆動シャフト20は、少なくとも刃面部40の基端側に駆動シャフト20の他の部分より屈曲しやすい柔軟部50を有し、駆動シャフト20の回転により柔軟部50が軸X方向に対して屈曲する。このように構成した医療デバイス10は、駆動シャフト20が回転することで柔軟部50において屈曲し、先端部25が首を振るように公転運動するため、刃面部40を血管内の広い範囲に向けることができて、血管内の物体を効率的に切断することができる。
 (2)上記(1)の医療デバイス10において、柔軟部50は、駆動シャフト20の刃面部40の基端側に、軸X方向に互いに離隔して2つ設けられていてもよい。これにより、医療デバイス10は、駆動シャフト20が2箇所で屈曲し、駆動シャフト20の回転に伴い駆動シャフト20の先端部25がより複雑に公転運動し、刃面部40をより広い範囲に向けることができる。
 (3)上記(1)または(2)の医療デバイス10において、駆動シャフト20の刃面部40より先端側に先端柔軟部56が設けられていてもよい。これにより、医療デバイス10は、駆動シャフト20の回転に伴い、台形状をなすように先端部25を変形させることができ、血管の外周側または内周側の血栓を刃面部40で効果的に切断できる。
 (4)上記(1)~(3)のいずれかの医療デバイス10において、柔軟部50は、駆動シャフト20の外側から、または、駆動シャフト20の内側から、カバーチューブ120で覆われていてもよい。これにより、医療デバイス10は、柔軟部50がスリット状である場合に、屈曲した際にスリットの端面が血管壁に接触しないようにし、また、スリットが開いた際の吸引力低下を抑制できる。
 (5)上記(1)~(4)のいずれかの医療デバイス10において、刃面部40は、周面41aを有する中空状の本体部41を備え、本体部41は、周面41aに設けられて中空内部と外部とを連通させる開口部42と、開口部42の回転方向に向かって鋭利な刃面43と、を含むようにしてもよい。これにより、医療デバイス10は、刃面部40を形成する本体部41の内外を連通させる開口部42に鋭利な刃面43を有しているので、駆動シャフト20の回転によって開口部42に位置する物体を刃面43で切断して細かくし、本体部41の内部に取り込むことができるので、物体を効率的に切断しつつ除去することができる。
 (6)上記(5)の医療デバイスにおいて、開口部42は、当該開口部42の先端縁と刃面43の最先端との間を離隔させる切欠部44を有し、刃面43の最先端は鋭利な端部43cを有してもよい。これにより、医療デバイス10は、刃面43の最先端に形成される鋭利な端部43cを物体に刺すことができ、物体をより確実に切断することができる。
 (7)上記(1)~(6)のいずれかの医療デバイス10において、先端部25は、軸Xから重心が偏心していてもよい。これにより、医療デバイス10は、駆動シャフト20を回転させた際に、先端部25が首を振りやすくすることができ、柔軟部50をより屈曲させやすくすることができる。
 (8)上記(7)の医療デバイス10において、先端部25の重心は、刃面部40の刃面の位置する側と軸Xを挟んで反対側に位置してもよい。これにより、医療デバイス10は、駆動シャフト20を回転させた際に、刃面部40が駆動シャフト20の公転運動の内周側を向くようにすることができ、刃面部40が血管壁に接触しないようにすることができる。
 (9)上記(2)の医療デバイス10において、シャフト170の最先端は、刃面部173aを備えた切削端173を有してもよい。これにより、医療デバイス10は、シャフト170の先端部が公転運動しつつ切削端173で効率よく物体の切断を行うことができる。
 本実施形態に係る(10)医療デバイス10は、先端部25を有し軸Xを中心に回転可能な駆動シャフト20と、先端部25に設けられ、駆動シャフト20の回転により回転して物体を切断する刃面部40と、刃面部40の基端側に設けられる少なくとも1つの柔軟部50と、を含み、駆動シャフト20の回転により先端部25が円錐形状をなすように回転する。
 (11)上記(10)の医療デバイスにおいて、刃面部40は、内部に空間を有する本体部41を備え、本体部41は、空間内に物体を取り込み可能に開口し、駆動シャフト20の軸X方向に平行または非平行な一対の開口縁42a、42bを備えた開口部42と、を含み、一対の開口縁42a、42bのいずれか一方は、鋭利な刃面43であってもよい。これにより、医療デバイス10は、刃面部40を形成する本体部41の内外を連通させる開口部42に鋭利な刃面43を有しているので、駆動シャフト20の回転によって開口部42に位置する物体を刃面43で切断して細かくし、本体部41の内部に取り込むことができるので、物体を効率的に切断しつつ除去することができる。
 (12)上記(11)の医療デバイスにおいて、刃面43は、軸Xを中心とした円周方向において、軸Xの基端側に向かって駆動シャフト20の回転方向と逆方向に傾斜するように設けられていてもよい。これにより、医療デバイス10は、開口部42の先端側の部分で捉えた物体を刃面43で切断しやすくすることができる。
 (13)上記(10)の医療デバイス10において、シャフト150の軸Xと同方向に沿って設けられ、ガイドワイヤ169が挿通可能なルーメン152aを有してもよい。これにより、医療デバイス10は、ルーメン152aにガイドワイヤ169を挿通してシャフト150を血管内で誘導することができる。
 (14)上記(13)の医療デバイス10において、ルーメン152aは、シャフト150の軸Xの方向に沿って設けられる内管152に有してもよい。これにより、医療デバイス10は、内管152に挿通して先端側に突出させたガイドワイヤ169により、血管内でシャフト150を誘導することができる。
 (15)上記(13)の医療デバイス10において、シャフト160の外周側に外管167を有し、ルーメン168は外管167に有してもよい。これにより、医療デバイス10は、外管167に挿通して先端側に突出させたガイドワイヤ169により、血管内でシャフト160を誘導することができる。
 なお、本発明は、上述した実施形態のみに限定されるものではなく、本発明の技術的思想内において当業者により種々変更が可能である。例えば、医療デバイス10が挿入される生体管腔は、血管に限定されず、例えば、脈管、尿管、胆管、卵管、肝管等であってもよい。また、医療デバイス10と駆動デバイス200は、一体的に構成されてもよい。
 また、吸引力は、排出ポート70に接続される吸引部230ではなく、駆動シャフトによって発生してもよい。駆動シャフトは、アルキメディアンスクリューポンプの構造を備えていれば、回転することで、吸引力を発生できる。この場合、排出ポート70は、吸引力を発生させる吸引部230に接続されなくても、デブリを排出できる。

Claims (15)

  1.  先端部を有し軸を中心に回転可能なシャフトと、
     前記先端部に設けられ、前記シャフトの回転により回転して物体を切断する刃面部と、を含み、
     前記シャフトは、少なくとも前記刃面部の基端側に前記シャフトの他の部分より屈曲しやすい柔軟部を有し、
     前記シャフトの回転により前記柔軟部が軸方向に対して屈曲する、医療用デバイス。
  2.  前記柔軟部は、前記シャフトの前記刃面部の基端側に、軸方向に互いに離隔して2つ設けられている、請求項1に記載の医療用デバイス。
  3.  前記シャフトの前記刃面部より先端側に先端柔軟部が設けられている、請求項1に記載の医療デバイス。
  4.  前記柔軟部は、前記シャフトの外側から、または、前記シャフトの内側から、カバーチューブで覆われている、請求項1に記載の医療デバイス。
  5.  前記刃面部は、周面を有する中空状の本体部を備え、
     前記本体部は、前記周面に設けられて中空内部と外部とを連通させる開口部と、前記開口部の回転方向に向かって鋭利な刃面と、を含む、請求項1に記載の医療用デバイス。
  6.  前記開口部は、当該開口部の先端縁と前記刃面の最先端との間を離隔させる切欠部を有し、
     前記刃面の最先端は鋭利な端部を有する、請求項5に記載の医療デバイス。
  7.  前記先端部は、前記軸から重心が偏心している、請求項1に記載の医療デバイス。
  8.  前記先端部の重心は、前記刃面部の前記刃面の位置する側と前記軸を挟んで反対側に位置する、請求項7に記載の医療デバイス。
  9.  前記シャフトの最先端は、前記刃面部を備えた切削端を有する、請求項2に記載の医療デバイス。
  10.  先端部を有し軸を中心に回転可能なシャフトと、
     前記先端部に設けられ、前記シャフトの回転により回転して物体を切断する刃面部と、
     前記刃面部の基端側に設けられる少なくとも1つの柔軟部と、を含み、
     前記シャフトの回転により前記先端部が円錐形状をなすように回転する、医療用デバイス。
  11.  前記刃面部は、内部に空間を有する本体部を備え、
     前記本体部は、前記空間内に前記物体を取り込み可能に開口し、前記シャフトの軸方向に平行または非平行な一対の開口縁を備えた開口部と、を含み、
     前記一対の開口縁の少なくともいずれか一方は、鋭利な刃面である、請求項9に記載の医療用デバイス。
  12.  前記刃面は、前記軸を中心とした円周方向において、前記軸の基端側に向かって前記シャフトの回転方向と逆方向に傾斜するように設けられている、請求項10に記載の医療用デバイス。
  13.  前記シャフトの前記軸と同方向に沿って設けられ、ガイドワイヤが挿通可能なルーメンを有する、請求項10に記載の医療用デバイス。
  14.  前記ルーメンは、前記シャフトの前記軸の方向に沿って設けられる内管に有する、請求項13に記載の医療用デバイス。
  15.  前記シャフトの外周側に外管を有し、
     前記ルーメンは前記外管に有する、請求項13に記載の医療用デバイス。
