Description

一种产妇宫口开启度监控装置及其控制方法 技术领域
本发明涉及产妇健康监控设备的领域,尤其是涉及一种产妇宫口开启度监控装置及及其控制方法。
背景技术
阴道检查被广泛用于产前检查,待产、产程过程中的产妇,医生及助产士都必须给产妇做阴道检查,以了解宫口开大情况,胎膜是否破裂,有无异常分泌物,胎先露等情况,据报道,我国每年的孕妇约1300-1400万左右,国家统计局公布,2021年我国出生人口1062万人,比2020年减少138万人,目前我国全面二孩及三孩政策放开提升生育率,中国的总剖宫产率约为46.5%,剖宫产率超过世界警戒线3倍多。
在实际临床工作中,对产程中子宫颈口的扩张情况、宫颈软硬程度、胎先露的下降情况以及羊膜囊是否完整或破裂等情况的判断往往依靠医生的临床经验和主观判断,子宫颈口在阴道深部,无法直接用肉眼观察,只能依靠产科医生和助产士频繁的用手指进行阴道检查,不同的医生可能存在对检查结果的判断的差异,其过程存在一定主观性。对这些关键环节的判断的差异性,造成产科医生在分娩过程中对产程处理的差异,如过早决定停止继续阴道试产而中转剖宫产提高不必要的剖宫产率的上升,如果产程处理过晚造成胎儿宫内窘迫,窒息,难产,产后大出血等母婴风险。产程过程中情况变化往往紧急而突然, 面对各种未知风险,产科医生往往会为了提前避免风险发生决定紧急剖宫产终止妊娠。这也是我国各地剖宫产率居高不下的原因之一。
除此之外,在待产到分娩的数小时到数十小时不等的时间内,由于医务人员为了观察宫颈口扩张情况而频繁进行阴道检查造成了产妇分娩中的身体痛苦和心理恐惧,也影响到产妇坚持顺产分娩的决心,间接造成分娩中的不配合,增加分娩难度,使剖宫产率上升。
发明内容
为了减少不同医护对产妇进行指检的情况,提高检查的准确度和产妇的舒适度,本申请提供一种产妇宫口开启度监控装置及其控制方法。
本申请提供的一种产妇宫口开启度监控装置采用如下的技术方案:
一种产妇宫口开启度监控装置,包括固定筒和膨胀组件,所述固定筒内螺纹连接有抵触件,所述抵触件的一端转动连接有抵触杆,所述抵触杆远离抵触件的位置铰接连接有调节杆,所述调节杆沿长度方向开设有条形孔,所述固定筒靠近调节杆的一端固定连接有滑块,所述滑块穿过调节杆上的条形孔,所述滑块远离固定筒的一端设置有固定板,所述膨胀组件设置在固定筒的外侧壁上,所述膨胀组件用于扩大产妇阴道内的空间;
所述调节杆远离抵触杆的一端连接有摄像头;
清洁组件,所述清洁组件包括鼓气泵和抽气泵,所述鼓气泵连接有鼓气管,所述抽气泵连接有抽气管,所述鼓气管和抽气管均与抵触 杆连接,所述摄像头位于鼓气管出口和抽气管进口之间的位置。
通过采用上述技术方案,将固定筒置入产妇的阴道中,膨胀组件膨胀,使得阴道内的空间扩大,摄像头对宫口的开启度进行拍摄,当需要对不同角度进行拍摄时,转动抵触件,抵触件推动抵触杆,抵触杆通过调节杆调节摄像头的角度,从而对宫口的开启度进行检测,同时鼓气管对对摄像头的镜头进行吹气处理,抽气管对摄像头的镜头进行抽气处理,从而减少了粘液在摄像头镜头表面的堆积,提高了拍摄的清晰度,从而提高了检查的精准度。
可选的,所述抵触件呈圆桶状,所述抵触杆与抵触件的底壁转动连接,所述鼓气管和抽气管穿过抵触件的底壁,所述鼓气管和抽气管分别与抵触杆和调节杆连接。
通过采用上述技术方案,避免在抵触件转动的过程中鼓气管和抽气管发生缠绕。
可选的,所述膨胀组件包括气囊,所述气囊套设在固定筒的外侧壁上,所述气囊通过主气管连接有微气泵。

通过采用上述技术方案,当摄像头检测的宫口位置发生偏移时,驱动电机通过主动齿轮和从动齿轮带动抵触件转动,进而带动抵触件沿固定筒的长度方向移动从而对摄像头的角度进行调节,通过对驱动电机对摄像头的角度进行调节,调节精度更高。
可选的,产妇宫口开启度监控装置还包括宫压探测器,所述宫压探测器与驱动电机电连接。
通过采用上述技术方案,当宫中压强达到40-60mmHg时,将固定筒置入产妇的阴道中,从而避免因固定筒置入过早或过晚对孕妇造成的不适,同时提高检测效率。
可选的,所述抵触杆和调节杆沿长度方向均开设有两凹槽,所述鼓气管和抽气管分别设置于两凹槽内。

本发明通过设置了抵触件和摄像头,将固定筒置入产妇的阴道中,膨胀组件膨胀,使得阴道内的空间扩大,摄像头对宫口的开启度进行 拍摄,当需要对不同角度进行拍摄时,转动抵触件,抵触件推动抵触杆,抵触杆通过调节杆调节摄像头的角度,从而对宫口的开启度进行检测,同时鼓气管对对摄像头的镜头进行吹气处理,抽气管对摄像头的镜头进行抽气、抽液处理,从而减少了粘液在摄像头镜头表面的堆积,提高了拍摄的清晰度,从而提高了检查的精准度;
本发明通过设置了主动齿轮、从动齿轮和驱动电机,当摄像头检测的宫口位置发生偏移时,驱动电机通过主动齿轮和从动齿轮带动抵触件转动,进而带动抵触件沿固定筒的长度方向移动从而对摄像头的角度进行调节,通过对驱动电机对摄像头的角度进行调节,调节精度更高;
本发明通过设置了宫压探测器,当宫中压强达到40-60mmHg时,将固定筒置入产妇的阴道中,从而避免因固定筒置入过早或过晚对孕妇造成的不适。

参照图1至图4,本发明提供的一种产妇宫口开启度监控装置的实施例,产妇宫口开启度监控装置,包括固定筒1和膨胀组件2,固定筒1内螺纹连接有抵触件4,抵触件4的一端转动连接有抵触杆5,抵触杆5远离抵触件4的位置铰接连接有调节杆6,调节杆6沿长度方向开设有条形孔7,固定筒1靠近调节杆6的一端固定连接有滑块8,滑块8穿过调节杆6上的条形孔7,滑块8远离固定筒1的一端设置有固定板9,膨胀组件2设置在固定筒1的外侧壁上,膨胀组件2用于扩大产妇阴道内的空间;
调节杆6远离抵触杆5的一端连接有摄像头10;调节杆6设置有两个,每个调节杆6均设置有摄像头10,两调节杆6对称分布。两个摄像头10减少了观察的视野盲区。
清洁组件3,清洁组件3包括鼓气泵31和抽气泵32,鼓气泵31连接有鼓气管33,抽气泵32连接有抽气管34,鼓气管33和抽气管34均与抵触杆5连接,摄像头10位于鼓气管33出口和抽气管34进口之间的位置。
抵触件4呈圆桶状,抵触杆5与抵触件4的底壁转动连接,鼓气管33和抽气管34穿过抵触件4的底壁,鼓气管33和抽气管34分别与抵触杆5和调节杆6连接。避免在抵触件4转动的过程中鼓气管33和抽气管34发生缠绕。
膨胀组件2包括气囊21,气囊21套设在固定筒1的外侧壁上,气囊21通过主气管连接有微气泵22。每个气囊21均通过微气泵22控制气囊21的膨胀与收缩,从而控制孕妇阴道内空间的大小,同时气囊21由乳胶材料制成,柔性好,对孕妇伤害更小。气囊21有五个,每个气囊21通过支气管与主气管连通,每个支气管均连接有控制阀,控制阀嵌设在固定筒1上,控制阀通过控制支气管的开合进而控制每个气囊21的鼓起与收缩,从而对孕妇起到按摩作用。同时微气泵22鼓出气体的温度与人体的温度相同,从而避免因温度过低导致孕妇身体不适的现象。
产妇宫口开启度监控装置还包括驱动电机11,抵触件4背离底壁的一端同轴固定连接有从动齿轮12,驱动电机11的输出轴同轴固定连接有主动齿轮13,从动齿轮12与主动齿轮13啮合,驱动电机11与固定筒1的内壁滑动连接,驱动电机11可沿固定筒1的长度方向滑动,抵触件4背离底壁的一端同轴滑动连接有转环14,驱动电 机11的输出轴与转环14转动连接,驱动电机11与摄像头10电连接,其中,所述的电连接是指通过连接实现其控制或者两者间具有控制逻辑。
当摄像头10检测的宫口位置发生偏移时,驱动电机11通过主动齿轮13和从动齿轮12带动抵触件4转动,进而带动抵触件4沿固定筒1的长度方向移动从而对摄像头10的角度进行调节。
产妇宫口开启度监控装置还包括宫压探测器,宫压探测器与驱动电机11电连接。其中,所述的电连接是指通过连接实现其控制或者两者间具有控制逻辑。当宫中压强达到40-60mmHg时,将固定筒1置入产妇的阴道中,从而避免因固定筒1置入过早或过晚对孕妇造成的不适,同时提高检测效率。
抵触杆5和调节杆6沿长度方向均开设有两凹槽,鼓气管33和抽气管34分别设置于两凹槽内。减少了抵触件4在转动过程中鼓气管33和抽气管34干涉现象的发生。
工作原理:
将固定筒1置入产妇的阴道中,膨胀组件2膨胀,使得阴道内的空间扩大,摄像头10对宫口的开启度进行拍摄,对宫口的开启度进行检测,同时鼓气管33对对摄像头10的镜头进行吹气处理,抽气管34对摄像头10的镜头进行抽气处理,从而减少了粘液在摄像头10镜头表面的堆积,提高了拍摄的清晰度,从而提高了检查的精准度;
当摄像头10检测的宫口位置发生偏移时,驱动电机11通过主动齿轮13和从动齿轮12带动抵触件4转动,进而带动抵触件4沿固定筒1的长度方向移动从而对摄像头10的角度进行调节。
第二实施例
本实施例的一种产妇宫口开启度监控装置,包括固定筒1和膨胀组件2,所述膨胀组件2设置在固定筒1的外侧壁上;
所述膨胀组件2包括气囊21,所述气囊21套设在固定筒1的外侧壁上,所述气囊21通过主气管连接有微气泵22,所述气囊21有多个,每个所述气囊21通过支气管与主气管连通,每个支气管均连接有控制阀,
在所述的固定筒1前端部设置有摄像组件,所述的摄像组件包括高清或超高清摄像头和照明灯,如高亮度的LED。
其中,固定筒为支撑部,所述的多个气囊整体构成环状结构,其被分割成多个气囊腔,如其轴向与其平行地设置的条形、环形气囊腔等,将其分割成多个独立控制的气囊腔,分别供气或排气膨胀或收缩可实现按摩效果,不同的顺序或持续时间起到的按摩效果不同,可根据实际按模型设定。气囊采用硅橡胶材质制成,产品轻薄无毒无味柔滑,具有化学惰性,不致病、不引起过敏,防止测量使用中对产妇阴道造成伤害和感染且能承受较大压力。优选地,所述的气囊为整体10cm左右长并将固定筒完全遮护其中或者仅固定筒前端部伸出1-3cm以获取更大的视角,固定筒构成了其硬支撑,同时所述的气囊的每个气囊腔内设置有压力传感器以控制微型气泵和电磁阀组的开 启。如采用薄片式的FSR402,FSR402传感器由高分子聚合物膜构成,具有超轻超薄的特点,适合对乳胶气囊内压力的检测。
气囊的进气、放气以及气囊自动封闭等全部由单片机接受传感器的信息采集通过矩阵电磁铁开启闭合实现控制。采用单片机作为主控,如C8051F340单片机作为主控,单片机加传感器实现宫口开启度实时监控装置和间歇期轻柔按摩的整体控制,稳定可靠,可实现全部功能的扩展应用。使得硬件接口设计变得非常简单。不需要再添加额外的电子元器件,就能方便稳定的实现USB的数据传输与控制,使得达到了很好的设计效果。由C8051F340单片机设置采用USB接口模式,非常方便的设计控制双层乳胶气囊。同时由于C8051F340支持(ISP)系统调试,简化了软硬件之间链接的调试与开发流程。极大的方便了整机设计。
本发明通过设计的宫口开启度监控装置,由金属或硬质塑料制成的固定筒1全部或后端部包裹在高柔软度医用级双层乳胶气囊中心,通过单片机控制启动充气组件撑开阴道观察通道,同步启动高清晰度摄像头,高清摄像头准确的对准宫口位置,在高亮度的LED的照射下,宫口开启度监控装置不仅从根本上避免了医务人员因使用手指进行盲检而导致测量数值的人为误差,从源头上解决了因检查宫口扩张程度进行反复指诊对产妇造成的下体不适和一定程度的恐惧心理影响。而且通过对双层气囊序贯状充气放气,能够在监控之外的间歇时段起到对阴道轻柔按摩的效果。有效缓解产妇的紧张状态。整套装置结构简单,监控接口使用方便,采用软性乳胶气囊安装取下非常灵活。产 妇没有不适感觉。适合替代当前传统的食指和中指伸入阴道内的阴道检查。
同时,为提高摄像组件的防水性能,高清摄像组件采用乳胶气囊内侧双层封闭粘结,保护摄像头和LED的正常工作。在监控的时段还可利用微型气泵工作发出的气流来防止粘液血水对摄像头的污染。其可以由单独的吹扫管路来实现,也可以由所述的气囊与所述的摄像组件对应处构造有微型出气部以对其进行吹扫,通过形成的侧吹风,避免了由子宫内渗出粘液,血液及羊水对摄影镜头的影响,通过一个或几个气囊对应的开口或者连接吹扫嘴可以简化整个结构和控制,优选地,还包括抽吸装置,其包括吸入口位于摄像组件处的排液管和抽吸泵,排液管即可设置与固定筒内以实现起走线,将血水等体液及时排出,也能进一步保证拍摄成像效果。
同时,在所述的固定筒1前部还设置有光照度传感器,如可见光光敏电阻器GL5539,将光敏电阻上分得的模拟电压信号转换为数字信号,传送给控制部,如单片机,由单片机判断此时的光照强度微调控制信号达到LED亮度实现最佳观察效果。通过采集光照度进而控制LED的光照效果,实现最佳的成像效果。
一般来说,固定筒的直径再5-8mm,气囊在收缩态1-2mm,在膨胀态一般20-25mm,在需要观察宫口开启度时,为保证摄像头较大的视野,所述的气囊的前端为直径变大的喇叭状,或者,所述的固定筒前端部设置有环形喇叭状气囊41。对乳胶气囊管的头部设计成喇叭 状开口,如其膨胀时直径可达30-35mm,增大宫口处气囊的张开量,气囊在充气打开后能够提供最大宫口开启度视野。
因为随着宫口的扩张,胎先露的下降,该装置伸入阴道的深度位置有所尺寸差异,为便于动态的进行修正,还包括水平和垂直方向的机械微调,即,所述的摄像组件固定在抵触杆前端部,所述的抵触杆尾部与所述的固定筒1间设置有二维调整机构以进行横向或纵向的调整,在具体实现上,在所述的抵触杆尾部设置XY平移调整架,在调整时还可将数据通过人为或者信息采集机构将位置调整状态记录,通过软件将不同的水平和垂直位置用刻度的形式显示在屏幕上,能够实现宫口位置开启度及胎先露下降情况的清楚监控。同时监控仪能够根据监测到的抬头及宫口开启度的状态实现Z轴位置的微调,即将XY平移调整架固定在可以相对固定筒1轴向移动的载台上,即可实现三个方向的调整,以更好适应的产程要求并保证摄像图像质量,其中XYZ的调整可以通过手动调节,也可以是通过微型电机来实现,具体结构不展开赘述。当胎头及宫口开启度达到单片机设置的阈值,监控仪发出报警提示,去除监控装置,进入最后接生流程。
本发明利用了单片机其丰富的外设功能控制乳胶气囊各气腔的循环充气过程,并辅以压力监测保证系统的正常运行及安全性要求,能基本满足监控过程中间歇期对阴道轻柔按摩的要求。将监控和轻柔按摩组合应用,系统更具有灵活性和扩展性,对于相关参数提供可调的设置功能,以及增加多模式显示的方式。为增加患者接受监控时阶段按摩的舒适度,应用微型碳钎维加热膜片进行加热使得进入序贯循 环的气体达到接近人体温度,能够在监控宫口开启度的同时有效提高产妇的生产前的舒适性。碳钎维加热膜片具有低成本、低功耗、体积小、抗干扰的特点,确保达到准确的气体加热效果,其可设置在气体进口处的加热腔内,气囊排出的气体还可返回加热腔内,以有效控制气体温度。
实施例三
所述的产妇宫口开启度监控装置的控制方法,其特征在于,包括以下步骤,
1)获取宫压强度以及宫缩信息;
2)在宫缩持续时间控制气囊同时或序贯膨胀或收缩,同时摄像组件启动工作;
3)在宫缩间歇期控制气囊全部收缩,摄像组件停止工作或者间歇式启动工作。
其中,宫压强度及宫缩等由宫压探测器获得,其采用帆布带装置固定在产妇的腹部。装置通过定制3cm宽的帆布带由定制的尼龙扣进行固定。固定带选用厚实的细帆布作为选材,长度大约为120cm,宽度为3-5cm,固定带的两侧分别装上尼龙扣,选用透气,全棉的布料在固定的里面覆盖一层,在舒适度方面有所提高,同时起到保护固定宫压探测器清洁的作用。可采用DF9-40膜片式压力传感器实现宫压数据的采集,为将采集的宫缩压力信息采集的稳定可靠,采用LM324运算放大器对采集的信息进行放大整形后输入到单片机的I/O口进 行处理,处理后的信号作为视频信号的一路由显示器进行波形输出显示。同时和摄像组件一起完成孕妇宫口开启度的观察、控制和报警。
一般情况下宫口的开启扩张是在规律的宫缩及胎先露的下降下进行的,该装置可连接一个分析宫口扩张及宫压强度,如第一产程中宫压强度一般在40-60mmHg,随着宫口扩张,压力逐渐升高到80-100mmHg,第二产程宫口开全后可达150mmHg,伴随着宫压升高,间歇期会缩短,宫压探测器可探测到宫缩持续时间及间歇时间,在宫缩持续时间里按摩阴道壁使产妇放松,间歇期里停止按摩让产妇休息,当宫口开全后装置自动停止工作。
产妇在生产的这段时间里要经历反复的阵痛,本发明在子宫口开启度观察的间歇期利用微型气泵和电磁阀序贯动作实现乳胶气囊的收缩,达到对阴道的轻柔按摩效果。微型泵体积小巧,工作介质为气态,用于阴道观察通道的开启和监控间歇期间阴道的轻柔按摩。
同时本发明还包括通过摄像信息采集、分析宫口扩张情况,及胎先露下降情况的步骤,摄像头生成矩阵扫描图像对宫口扩张程度进行准确的计算和测量,由显示器将宫口开启度的图像和数据清楚显示在屏幕上。
还包括当宫压强度超过预定阈值或者宫缩间歇期小于预定值时报警的步骤。轻柔的按摩放松软化子宫颈管相结合的功能,可以大大提高产科医生对产程的科学判断,同时降低产妇分娩痛苦,提高产妇舒适度和配合度,提高检查质量,降低剖宫产率,提高分娩安全。宫口开启度实时监控设备,是传统检查宫口开启度方式的一次彻底革 新,不仅有利于减少产妇在生产前反复检查带来的痛苦,而且实时的监控观察能够减少手动检查主观性的行为误差,精确的测量监控效果提高了临产准确性的评估,有利于带教讲学,提高对临产前检查客观认知的精确性。

Claims (10)

  1. 一种产妇宫口开启度监控装置,其特征在于:包括固定筒(1)和膨胀组件(2),所述固定筒(1)内螺纹连接有抵触件(4),所述抵触件(4)的一端转动连接有抵触杆(5),所述抵触杆(5)远离抵触件(4)的位置铰接连接有调节杆(6),所述调节杆(6)沿长度方向开设有条形孔(7),所述固定筒(1)靠近调节杆(6)的一端固定连接有滑块(8),所述滑块(8)穿过调节杆(6)上的条形孔(7),所述滑块(8)远离固定筒(1)的一端设置有固定板(9),所述膨胀组件(2)设置在固定筒(1)的外侧壁上;
    所述调节杆(6)远离抵触杆(5)的一端连接有摄像头(10);
    清洁组件(3),所述清洁组件(3)包括鼓气泵(31)和抽气泵(32),所述鼓气泵(31)连接有鼓气管(33),所述抽气泵(32)连接有抽气管(34),所述鼓气管(33)和抽气管(34)均与抵触杆(5)连接,所述摄像头(10)位于鼓气管(33)出口和抽气管(34)进口之间的位置。
  2. 根据权利要求1所述的一种产妇宫口开启度监控装置,其特征在于:所述膨胀组件(2)包括气囊(21),所述气囊(21)套设在固定筒(1)的外侧壁上,所述气囊(21)通过主气管连接有微气泵(22)。
  3. 根据权利要求2所述的一种产妇宫口开启度监控装置,其特征在于:所述气囊(21)有多个,每个所述气囊(21)通过支气管与主气管连通,每个支气管均连接有控制阀,控制阀嵌设在所述固定筒(1)上。
  4. 一种产妇宫口开启度监控装置,其特征在于:包括固定筒(1)和膨胀组件(2),所述膨胀组件(2)设置在固定筒(1)的外侧壁上;
    所述膨胀组件(2)包括气囊(21),所述气囊(21)套设在固定筒(1)的外侧壁上,所述气囊(21)通过主气管连接有微气泵(22),所述气囊(21)有多个,每个所述气囊(21)通过支气管与主气管连通,每个支气管均连接有控制阀,
    在所述的固定筒(1)前端部设置有摄像组件,所述的摄像组件包括摄像头和照明灯。
  5. 根据权利要求4所述的一种产妇宫口开启度监控装置,其特征在于,所述的气囊的前端为直径变大的喇叭状,或者,所述的固定筒前端部设置有环形喇叭状气囊。
  6. 根据权利要求4或5所述的一种产妇宫口开启度监控装置,其特征在于,所述的气囊与所述的摄像组件对应处构造有微型出气部以对其进行吹扫。
  7. 根据权利要求4或5所述的一种产妇宫口开启度监控装置,其特征在于,还包括抽吸装置,其包括吸入口位于摄像组件处的排液管和抽吸泵。
  8. 根据权利要求4或5所述的一种产妇宫口开启度监控装置,其特征在于,还包括用以对气体加热的加热部件。
  9. 根据权利要求4或5所述的一种产妇宫口开启度监控装置,其特征在于,所述的气囊内设置有压力传感器。
  10. 如权利要求4-9任一项所述的产妇宫口开启度监控装置的控制方法,其特征在于,包括以下步骤,
    1)获取宫压强度以及宫缩信息;
    2)在宫缩持续时间控制气囊同时或序贯膨胀或收缩;
    3)在宫缩间歇期控制气囊全部收缩。
