Abstract
免疫チェックポイント制御剤との併用によりがん治療の有効性を向上させる新たな治療法が開示されている。本発明の抗がん剤は、N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドである化合物、薬剤的に許容されるその塩、又は前記化合物若しくは前記塩の溶媒和物を有効成分とするものであり、少なくとも1種の免疫チェックポイント制御剤と組み合わせて用いられる。免疫チェックポイント制御剤は、例えば、アンタゴニスト性抗PD-1抗体等の免疫チェックポイント阻害剤である。
Description
本発明は、N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドと免疫チェックポイント制御剤との併用によるがん療法に関する。
抑制性の免疫チェックポイント受容体又はそのリガンドに結合して免疫抑制シグナルの伝達を阻害する免疫チェックポイント阻害剤(Immune Checkpoint Inhibitor; ICI)は、抗PD-1抗体、抗PD-L1抗体、抗CTLA-4抗体が臨床使用されるに至り、がん治療において標準治療の一つとして位置づけられるまでに至っている。しかし、ICIの奏効率は5~30%程度で、すべてのがんに有効でもない。この問題を克服すべく、ICI同士の併用療法やICIと他の薬剤との併用療法が検討され、臨床試験も多数行なわれている。
これまでに検討が進められてきたICIを用いた抗がん併用療法の例として、ICI同士の併用療法(例えば、非特許文献1~3等)、anti-VEGF/TKIとPD-1/PD-L1阻害剤との併用療法(非特許文献4)、コビメチニブ等のMEK阻害薬とPD-1/PD-L1阻害剤との併用療法(非特許文献5)、Galunisertib、Vactosertib等のTGF-β阻害剤とPD-1/PD-L1阻害剤との併用療法(非特許文献6~8)、抗CD4抗体とICIとの併用療法(特許文献1、2)等がある。また、TGF-βとPD-L1をブロックする二機能性融合タンパク質Bintrafusp alfaのように、免疫チェックポイントと他の分子を同時に標的とする融合タンパク質分子も開発されている(非特許文献9)。
これらの併用療法により有効性向上が見られているが、さらに有効ながん療法が待望されている。
免疫チェックポイント分子には、抑制性の受容体及びリガンドの他、免疫促進シグナルを伝達する共刺激性の受容体及びそのリガンドも存在する。共刺激性免疫チェックポイント分子としては、CD28、ICOS、CD137/4-1BB、OX40などが知られている。共刺激性免疫チェックポイント受容体又はそのリガンドに結合して免疫促進シグナルの伝達を促進するアゴニスト性分子は、免疫チェックポイント刺激剤ないしは活性化剤などと呼ぶことができる。
免疫チェックポイント活性化剤もがん等の治療剤として研究開発が行なわれてきたが、ICIと比べると実用化の例は少なく、難航しているようである。例えば、B細胞性慢性リンパ性白血病をもターゲットとして開発されていた抗CD28抗体は、治験中に多臓器不全という安全性面から深刻な事件が発生したために試験実施承認が取り消された(非特許文献10)。この事件から、免疫チェックポイント活性化剤には過剰活性化のリスクが高いことが危惧されている。抗CD137/4-1BB抗体では、臨床試験において、抗腫瘍効果があるものの重篤な肝毒性がみられた例(Urelumab)や、安全性は高いものの効能が低く、ICI(抗PD-L1抗体)等との併用で臨床試験が続けられている例(Utomilumab)が知られており、肝毒性を誘発せずに高い抗腫瘍効果を発揮できる抗ヒトCD137/4-1BB抗体の開発が続けられている(非特許文献11~13)。

本発明は、免疫チェックポイントをターゲットとする免疫チェックポイント制御剤、とりわけ実用化が進んでいる免疫チェックポイント阻害剤との併用によりがん治療の有効性を向上させる新たな治療法を提供することを目的とする。
本願発明者らは、肺線維症等のコラーゲン過剰生産が関与する疾患の予防・治療に有用な化合物として開発されたアミド誘導体(WO 2014/003124)に着目して鋭意研究した結果、特定のアミド誘導体が免疫チェックポイント制御剤との併用により優れた抗がん作用を発揮することを見出し、本願発明を完成した。
すなわち、本発明は、N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミド(以下、アミド誘導体Aということがある)と免疫チェックポイント制御剤の併用によるがん療法であり、以下の態様を包含する。
[1] N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドである化合物、薬剤的に許容されるその塩、又は前記化合物若しくは前記塩の溶媒和物を有効成分とし、少なくとも1種の免疫チェックポイント制御剤と組み合わせて用いられる、抗がん剤。
[2] 前記抗がん剤の投与量が、前記有効成分の量で1日当り0.1 mg/kg体重~10 mg/kg体重である、[1]記載の抗がん剤。
[3] 前記抗がん剤が経口剤である、[1]又は[2]記載の抗がん剤。
[4] 前記免疫チェックポイント制御剤が、アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、アンタゴニスト性抗LAG-3抗体、アンタゴニスト性抗TIM-3抗体、及びアンタゴニスト性抗CTLA-4抗体から選択される少なくとも1種の免疫チェックポイント阻害剤である、[1]~[3]のいずれか1項に記載の抗がん剤。
[5] 前記免疫チェックポイント制御剤が、アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、及び抗PD-L2抗体から選択される少なくとも1種である、[4]記載の抗がん剤。
[6] N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドである化合物、薬剤的に許容されるその塩、又は前記化合物若しくは前記塩の溶媒和物の有効量を、少なくとも1種の免疫チェックポイント制御剤と組み合わせてがん患者に投与することを含む、がんの治療方法。
[1] N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドである化合物、薬剤的に許容されるその塩、又は前記化合物若しくは前記塩の溶媒和物を有効成分とし、少なくとも1種の免疫チェックポイント制御剤と組み合わせて用いられる、抗がん剤。
[2] 前記抗がん剤の投与量が、前記有効成分の量で1日当り0.1 mg/kg体重~10 mg/kg体重である、[1]記載の抗がん剤。
[3] 前記抗がん剤が経口剤である、[1]又は[2]記載の抗がん剤。
[4] 前記免疫チェックポイント制御剤が、アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、アンタゴニスト性抗LAG-3抗体、アンタゴニスト性抗TIM-3抗体、及びアンタゴニスト性抗CTLA-4抗体から選択される少なくとも1種の免疫チェックポイント阻害剤である、[1]~[3]のいずれか1項に記載の抗がん剤。
[5] 前記免疫チェックポイント制御剤が、アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、及び抗PD-L2抗体から選択される少なくとも1種である、[4]記載の抗がん剤。
[6] N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドである化合物、薬剤的に許容されるその塩、又は前記化合物若しくは前記塩の溶媒和物の有効量を、少なくとも1種の免疫チェックポイント制御剤と組み合わせてがん患者に投与することを含む、がんの治療方法。
本発明の抗がん剤は、アミド誘導体Aのフリー体、薬剤的に許容されるその塩、又はアミド誘導体A若しくはその塩の溶媒和物(以下、本明細書において、これらをまとめて「アミド誘導体A」ということがある)を有効成分とするものであり、免疫チェックポイント制御剤との併用により優れた抗がん作用を発揮する。アミド誘導体Aは抗線維化作用を有することが公知の化合物であるが、がんに対しては、抗線維化作用を示す濃度よりはるかに低い濃度で抗腫瘍効果を、とりわけ免疫チェックポイント制御剤と併用したときに示す。肺線維症モデル動物への経口投与で肺線維症に対する効果が確認できなかった投与量でも、免疫チェックポイント制御剤との併用により腫瘍増殖を抑制できることを確認済みである(データ示さず)。線維症への効果はTGF-β阻害活性によるものであり、肺線維症には効果がない低用量で抗腫瘍効果が得られるという事実は、アミド誘導体Aの抗腫瘍効果がTGF-β阻害活性によるものではないことを示唆している。類似した作用を有する公知の低分子化合物Galunisertibの抗がん剤としての有効量と比較しても、アミド誘導体Aの有効量ははるかに低く(下記実施例参照)、アミド誘導体AはGalunisertibよりも薬効について優っていると考えられる。また、例えば経口の錠剤として製剤化した場合、Galunisertibは毎回大量の錠剤を服用する必要があるが、本願の抗がん剤は例えば1回に1個の錠剤を服用すれば済むので、がん患者の服薬の負担を軽減できる。
本発明の抗がん剤は、下記構造のアミド誘導体A(フリー体)、アミド誘導体Aの薬剤的に許容される塩、又はアミド誘導体A若しくは薬剤的に許容されるその塩の溶媒和物を有効成分として含有し、免疫チェックポイント制御剤と組み合わせて用いられる。
「組み合わせて用いる」、「併用する」という語は、アミド誘導体Aと免疫チェックポイント制御剤をそれぞれ別個の剤として用いる態様、同一製剤中にアミド誘導体Aと免疫チェックポイント制御剤を含有する配合剤として用いる態様の両者を包含する。本発明の抗がん剤は、典型的には前者の態様をとる剤であり、免疫チェックポイント制御剤は、本発明の抗がん剤とは別個の剤として調製されたものを用いるのが一般的である。免疫チェックポイント制御剤を複数種類併用する場合も同様であり、同一製剤中に複数の免疫チェックポイント制御剤を含有した剤を用いても良いが、一般には複数の免疫チェックポイント制御剤をそれぞれ別個の剤として組み合わせることが好ましい。各物質を別個の剤として組み合わせた場合には、各物質の投与部位、投与時期、投与回数、投与量などを個別に最適化することができる利点がある。
「組み合わせて投与する」、「併用投与する」という語は、複数の有効成分(アミド誘導体A及び免疫チェックポイント制御剤)を患者に対し同時に、順次に、又は別々に投与することを意味する。順次に投与するとは、1の有効成分の投与が完了した後すぐに続けて次の有効成分の投与を行なうことをいう。別々に投与とは、間隔を空けて複数の有効成分を投与することをいい、例えば同日中に数時間程度以上の間隔を空けて、あるいは1クールの治療期間中の別の日に投与することをいう。同時に投与する場合、別個の剤として製剤された有効成分を同時に投与してもよいし、同一製剤中に複数の成分を含有した剤を投与してもよい。
1クールとは、がん療法の分野における一般的な意味の通り、投薬期間と休薬期間を合わせた小単位の期間をいう。単剤療法、多剤併用療法のいずれの場合でも、抗がん剤を1週間ないしは数週間程度投与する投薬期間と、1週間程度の休薬期間を1クールとし、患者の状態やがんの縮小効果などに応じて医師により決定された回数のクール(通常数クール)を実施するのが一般的である。もっとも、抗がん剤の副作用の程度によっては、休薬期間を設けずに一定期間継続して投与することも可能である。本発明の抗がん剤は、休薬期間を設けてもよいし、設けなくてもよい。
アミド誘導体A自体はWO 2014/003124に開示される公知の化合物であり、肺線維症等のコラーゲン過剰生産が関与する疾患の予防・治療に有用な化合物として開発された、コラーゲン産生阻害作用を有するアミド誘導体のうちのひとつである。アミド誘導体Aが抗がん作用を有し、抗がん剤として有用であることは、WO 2014/003124には一切記載されていない。アミド誘導体Aは、WO 2014/003124に記載される方法、具体的には、実施例62に記載される方法により、N-(4-シアノビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミド(化合物I)からN-(4-(ヒドロキシアミジノ)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミド(化合物II)を経て合成することができる。
本発明の抗がん剤は、アミド誘導体Aの薬剤的に許容される塩を含有していてもよい。薬剤的に許容される塩としては、例えば、塩酸塩、硫酸塩、リン酸塩、臭化水素酸塩等の無機酸塩、シュウ酸塩、マロン酸塩、クエン酸塩、フマル酸塩、乳酸塩、リンゴ酸塩、コハク酸塩、酒石酸塩、酢酸塩、トリフルオロ酢酸塩、マレイン酸塩、グルコン酸塩、安息香酸塩、アスコルビン酸塩、メタンスルホン酸塩、p-トルエンスルホン酸塩、ケイ皮酸塩等の有機酸塩、ナトリウム塩、カリウム塩、カルシウム塩、マグネシウム塩、アンモニウム塩等の無機塩基塩、メチルアミン塩、ジエチルアミン塩、トリメチルアミン塩、トリエチルアミン塩、ピリジニウム塩、トリエタノールアミン塩、エチレンジアミン塩、グアニジン塩等の有機塩基塩が挙げられる。
本発明の抗がん剤は、アミド誘導体A若しくは薬剤的に許容されるその塩の溶媒和物を含有していてもよい。溶媒和物の具体例としては、水和物及びエタノール和物などを挙げることができるが、これらに限定されず、医薬として許容される溶媒との溶媒和物であればいかなるものであってもよい。
本発明において、アミド誘導体Aのフリー体、その塩、又はこれらの溶媒和物と併用する「免疫チェックポイント制御剤」とは、免疫チェックポイント分子の機能を制御することで免疫細胞の活性化を促進する物質であり、抑制性の免疫チェックポイント分子に対して阻害的に働く物質(免疫チェックポイント阻害剤)と、共刺激性の免疫チェックポイント分子に対して促進的に働く物質(免疫チェックポイント活性化剤、又は免疫チェックポイント刺激剤)とが包含される。「免疫チェックポイント分子」という語には、免疫チェックポイントとして機能する受容体とリガンドの両者が包含される。
免疫チェックポイントとは、免疫系が自己の体を攻撃しないための免疫逃避機構である。T細胞上には免疫チェックポイント受容体が存在し、がん細胞や抗原提示細胞上に発現しているリガンドと相互作用する。T細胞はMHC分子上に提示された抗原を認識して活性化し、免疫反応を起こすが、並行して生じる免疫チェックポイント受容体-リガンドの相互作用によりT細胞の活性化が調節を受ける。免疫チェックポイント受容体には共刺激性のものと抑制性のものがあり、両者のバランスによってT細胞の活性化及び免疫反応が調節を受けている。
「免疫チェックポイント阻害剤」は、抑制性の免疫チェックポイント分子に対するアンタゴニストであり、抑制性の免疫チェックポイント受容体に結合し、該受容体とそのリガンドとの結合を阻害する、アンタゴニスト活性を有する抗体;抑制性の免疫チェックポイントリガンドに結合し、該リガンドのその受容体への結合を阻害する抗体;抑制性の免疫チェックポイントリガンドに基づいて設計された、受容体を活性化しない可溶性のポリペプチド;又は該ポリペプチドを発現可能なベクターが包含される。対象となる抑制性の免疫チェックポイント分子として、受容体としてはPD-1、CTLA-4、LAG-3、TIM-3、BTLA、KIR等を挙げることができ、リガンドとしてはPD-L1(PD-1のリガンド)、PD-L2(PD-1のリガンド)、GAL9(TIM-3のリガンド)、HVEM(BTLAのリガンド)等を挙げることができる。免疫チェックポイント阻害剤の具体例を挙げると、抑制性の免疫チェックポイント受容体に結合し、該受容体とそのリガンドとの結合を阻害する、アンタゴニスト活性を有する抗体としては、アンタゴニスト性抗PD-1抗体、アンタゴニスト性抗CTLA-4抗体、アンタゴニスト性抗LAG-3抗体、アンタゴニスト性抗TIM-3抗体、アンタゴニスト性抗BTLA抗体、アンタゴニスト性抗KIR抗体等を挙げることができ、抑制性の免疫チェックポイントリガンドに結合し、該リガンドのその受容体への結合を阻害する抗体としては、抗PD-L1抗体、抗PD-L2抗体、抗GAL9抗体、抗HVEM抗体等を挙げることができるが、これらに限定されない。
がん細胞は、抑制性の免疫チェックポイント受容体に対するリガンドを発現し、該受容体を利用して細胞傷害性T細胞による破壊から逃避している。従って、抑制性の免疫チェックポイント分子をターゲットとする免疫チェックポイント阻害剤を投与することで、がん細胞による免疫チェックポイント機構の利用を妨害し、CD8+T細胞によるがん細胞の殺傷を促進することができる。近年抗がん剤として実用化が進みつつある免疫チェックポイント阻害剤は、抑制性の免疫チェックポイント受容体又はそのリガンドを標的とした抗体である。メラノーマ、肺がん、白血病、胃がん、リンパ腫、腎臓がん等を対象に、抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、抗CTLA-4抗体、抗LAG-3抗体、抗TIM-3抗体等の開発が進んでいる。
「免疫チェックポイント活性化剤」は、共刺激性の免疫チェックポイント分子に対するアゴニストであり、共刺激性の免疫チェックポイント受容体と結合する、アゴニスト活性を有する抗体;共刺激性の免疫チェックポイントリガンドに基づいて設計された、受容体を活性化する作用を有する可溶性のポリペプチド;又は該ポリペプチドを発現可能なベクターが包含される。対象となる共刺激性の免疫チェックポイント分子として、受容体としてはCD137(別名4-1BB)、OX40、GITR等を挙げることができ、リガンドとしてはCD137L(CD137のリガンド)、OX40L(OX40のリガンド)、TNFSF18(GITRのリガンド)等を挙げることができる。共刺激性の免疫チェックポイント分子をターゲットとする免疫チェックポイント活性化剤を投与することで、免疫反応を促進し、それによりCD8+T細胞によるがん細胞の殺傷を促進することも可能である。免疫チェックポイント活性化剤の具体例を挙げると、共刺激性の免疫チェックポイント受容体と結合する、アゴニスト活性を有する抗体としては、アゴニスト性CD137抗体、アゴニスト性抗OX40抗体、アゴニスト性抗GITR抗体等を挙げることができるが、これらに限定されない。免疫チェックポイント活性化剤も、後述するとおり、医薬としての開発例が知られている。
1つの態様において、本発明の抗がん剤と併用する免疫チェックポイント制御剤は免疫チェックポイント阻害剤である。好ましい免疫チェックポイント阻害剤の具体例として、アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、アンタゴニスト性抗LAG-3抗体、アンタゴニスト性抗TIM-3抗体、及びアンタゴニスト性抗CTLA-4抗体から選択される少なくとも1種;アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、アンタゴニスト性抗LAG-3抗体、及びアンタゴニスト性抗TIM-3抗体から選択される少なくとも1種;あるいはアンタゴニスト性抗PD-1抗体、抗PD-L1抗体、及び抗PD-L2抗体から選択される少なくとも1種を挙げることができるが、これらに限定されない。
免疫チェックポイント制御剤の具体例を開発中ないし実用化されている医薬品等の公知例と共に例示すると、免疫チェックポイント阻害剤では、抗CTLA-4抗体(例えば、Ipilimumab(YERVOY(登録商標)、Tremelimumab、AGEN-1884)、抗PD-1抗体(例えば、ニボルマブ、Cemiplimab(REGN-2810)、ペンブロリズマブ(MK-3475)、Spartalizumab(PDR-001)、Tislelizumab(BGB-A317)、AMP-514(MEDI0680)、Dostarlimab(ANB011、TSR-042)、Toripalimab(JS001)、Camrelizumab(SHR-1210)、Genolimzumab(CBT-501)、Sintilimab(IBI308)、STI-A1110、ENUM 388D4、ENUM 244C8、GLS010、MGA012、AGEN2034、CS1003、HLX10、BAT-1306、AK105、AK103、BI 754091、LZM009、CMAB819、Sym021、GB226、SSI-361、JY034、HX008、ABBV181、BCD-100、PF-06801591、CX-188およびJNJ-63723283など)、抗PD-L1抗体(例えば、アテゾリズマブ(RG7446、MPDL3280A)、アベルマブ(PF-06834635、MSB0010718C)、デュルバルマブ(MEDI4736)、BMS-936559、STI-1010、STI-1011、STI-1014、KN035、LY3300054、HLX20、SHR-1316、CS1001(WBP3155)、MSB2311、BGB-A333、KL-A167、CK-301、AK106、AK104、ZKAB001、FAZ053、CBT-502(TQB2450)、JS003およびCX-072など)、抗PD-L2抗体(例えば、rHIgM12B7)、PD-L1融合タンパク質、PD-L2融合タンパク質(例えば、AMP-224)、抗TIM-3抗体(例えば、MBG453)、抗LAG-3抗体(例えば、BMS-986016、LAG525)、抗KIR抗体(例えば、Lirilumab)、PD-1拮抗剤(例えば、AUNP-12、BMS-M1~BMS-M10の各化合物、BMS-1、BMS-2、BMS-3、BMS-8、BMS-37、BMS-200、BMS-202、BMS-230、BMS-242、BMS-1001、BMS-1166、Incyte-1~Incyte-6の各化合物、CAMC-1~CAMC-4、RG_1およびDPPA-1など)、PD-L1/VISTA拮抗剤(例えば、CA-170など)、PD-L1/TIM3拮抗剤(例えば、CA-327など)等が挙げられる。免疫チェックポイント活性化剤では、抗CD137抗体(例えば、Urelumab、Utomilumab、LVGN6051)、抗OX40抗体(例えば、PF-04518600)等が挙げられる。また、上記既知の抗体の重鎖および軽鎖相補性決定領域(CDRs)または可変領域(VR)を含む抗体も免疫チェックポイント制御剤の一態様である。例えば、抗PD-1抗体の更なる一態様としては、ニボルマブの重鎖および軽鎖相補性決定領域(CDRs)または可変領域(VR)を含む抗体が挙げられる。
本発明の抗がん剤を免疫チェックポイント制御剤と組み合わせて投与する対象は、がん患者、すなわち、がんの治療を必要とする患者であり、現にがんを有する患者、及び外科手術によりがん病巣を切除した後の患者が包含される。患者は典型的には哺乳動物、特にはヒトであるが、これに限定されない。
本発明において、「がんの治療」という語には、患者のがんを治療する目的で行われる種々の医療処置が包含される。具体的には、原発がん、再発がん及び転移がんの治療の他、がんの再発及び転移の抑制も包含される。例えば、外科手術によりがん病巣を切除した後の患者に対し、再発防止の目的で本発明の抗がん剤を投与する態様も、「がんの治療」に包含される。「抗がん剤」という語には、がん(原発がん、再発がん、転移がん)の治療剤、がんの再発抑制剤、及びがんの転移抑制剤が包含される。「がん患者」という語には、上記したとおり、がんを現に有している患者の他、外科手術によりがん病巣を切除した後の患者も包含される。
本発明の抗がん剤が対象とするがんの種類は特に限定されず、固形がん及び血液がんを含む種々のがんに対して適用できる。固形がんの具体例としては、悪性黒色腫(例えば、皮膚、口腔粘膜上皮または眼窩内などにおける悪性黒色腫)、肺がん(例えば、扁平非小細胞肺がん(肺扁平上皮がん)及び非扁平非小細胞肺がん(肺腺がん、肺大細胞がん)等の非小細胞肺がん;小細胞肺がん)、頭頸部がん、腎臓がん(例えば、淡明細胞型腎細胞がん、多房嚢胞性腎細胞がん、乳頭状腎細胞がん、嫌色素性腎細胞がん、集合管がん)、乳がん、卵巣がん(例えば、漿液性腺がん、粘液性腺がん、明細胞腺がん)、鼻咽頭がん、子宮がん(例えば、子宮頸がん、子宮内膜がん、子宮体がん)、肛門がん(例えば、肛門管がん)、大腸がん/直腸がん/結腸がん、肝臓がん(例えば、肝細胞がん、胆管細胞がん)、食道がん(例えば、食道腺がん、食道扁平上皮がん)、胃がん、食道胃接合部がん、小腸がん、膵がん、尿路上皮がん(例えば、膀胱がん、上部尿路がん、尿管がん、腎盂がん、尿道がん)、前立腺がん、卵管がん、原発性腹膜がん、胸膜中皮腫、胆嚢がん、胆管がん、胆道がん、皮膚がん(例えば、メルケル細胞がん)、精巣がん(胚細胞腫瘍)、膣がん、外陰部がん、陰茎がん、小腸がん、内分泌系がん、甲状腺がん、副甲状腺がん、副腎がん、脊椎腫瘍、脳腫瘍、神経膠芽腫、神経膠肉腫、骨・軟部肉腫(例えば、ユーイング肉腫、小児横紋筋肉腫、子宮体部平滑筋肉腫)、及びカポジ肉腫が挙げられる。血液がんの具体例としては、悪性リンパ腫、白血病、多発性骨髄腫を挙げることができる。本発明の抗がん剤は、典型的には固形がんに対して好ましく用いられる。
本発明の抗がん剤の投与量は、がんの治療に有効な量であればよい。有効量は、腫瘍の大きさや症状、患者の年齢や体重等に応じて適宜選択され得る。本発明の抗がん剤の投与量は、がん患者に対し1日当りの有効成分量で0.1 mg/kg体重~50 mg/kg体重程度でよく、例えば0.1 mg/kg体重~40 mg/kg体重、0.1 mg/kg体重~30 mg/kg体重、0.1 mg/kg体重~20 mg/kg体重、又は0.1 mg/kg体重~10 mg/kg体重とすることができ、例えば0.1 mg/kg体重~7 mg/kg体重又は0.1 mg/kg体重~5 mg/kg体重まで投与量を減じることも可能である。これらの数値範囲の下限値は、0.2 mg/kg体重、0.3 mg/kg体重、0.4 mg/kg体重、又は0.5 mg/kg体重であってもよい。1日の投与は1回でもよいし、数回に分けて投与してもよい。本発明の抗がん剤の治療期間中の投与は、毎日でもよいし、1日~数日おきでもよい。アミド誘導体Aと同様に抗線維化作用と抗がん作用が知られており、抗がん剤としての臨床試験が進められている低分子化合物Galunisertibは、75 mg/kg体重を1日2回投与、1日の投与量が150 mg/kg体重で設定されているが、本発明の抗がん剤はこれよりもはるかに低用量でGalunisertibと同等の抗がん作用を発揮できるので、患者の服薬の負担を軽減できる。
本発明の抗がん剤の投与経路は特に限定されず、経口投与でも非経口投与でもよい。非経口投与は、筋肉内投与、皮下投与、静脈内投与、動脈内投与、経皮投与、経鼻投与等のいずれでもよい。全身投与でも局所投与でもよく、局所投与の場合、例えば、腫瘍組織内又はその近傍や、腫瘍近傍の所属リンパ節に投与することができる。本発明の抗がん剤は、経口投与で好ましく用いることができる。すなわち、本発明の抗がん剤の剤形は経口剤であることが好ましい。
アミド誘導体Aのフリー体、その塩、及びこれらの溶媒和物は、各投与経路に適した、薬剤的に許容される担体、希釈剤、賦形剤、結合剤、滑沢剤、崩壊剤、甘味剤、懸濁化剤、乳化剤、着色剤、矯味剤、安定剤等の添加剤と適宜混合して製剤することができる。製剤形態としては、錠剤、軟カプセル剤、硬カプセル剤、顆粒剤、散剤、シロップ剤などの経口剤や、注射剤、座剤、液剤、吸入剤、貼付剤などの非経口剤などを挙げることができる。製剤方法及び使用可能な添加剤は、医薬製剤の分野において周知であり、いずれの方法及び添加剤をも用いることができる。
本発明の抗がん剤に併用する免疫チェックポイント制御剤の投与量も、腫瘍の大きさや症状、患者の年齢や体重等に応じて適宜選択されるが、抗がん剤として単独で又は他の薬剤と併用して投与する場合に通常用いられている投与量、投与経路及び投与スケジュールを、本発明の抗がん剤と併用する場合にも採用できる。免疫チェックポイント制御剤は、がん患者に対し治療期間中に毎日又は数日おきに複数回投与するのが一般的である。免疫チェックポイント制御剤の投与経路は、経口投与でも非経口投与でもよいが、一般には筋肉内投与、皮下投与、静脈内投与、動脈内投与等の非経口投与が好ましい。全身投与でも局所投与でもよいが、全身投与が好ましい。
本発明の抗がん剤は、通常、用法・用量などが記載された添付文書が同梱ないし添付されて提供される。言い換えると、本発明の抗がん剤製品の製造において、アミド誘導体Aのフリー体、アミド誘導体Aの薬剤的に許容される塩、又はアミド誘導体A若しくは薬剤的に許容されるその塩の溶媒和物(有効成分)と、添付文書とが使用されうる。例えば、本発明の抗がん剤製品は、その包装の中に添付文書を含みうる。添付文書に記載される当該抗がん剤の投与量は、上記有効成分の量で1日当り0.1 mg/kg体重~10 mg/kg体重の範囲内から選択される投与量であってよい。また、添付文書には、当該抗がん剤の使用方法として免疫チェックポイント制御剤との併用療法(当該抗がん剤が少なくとも1種の免疫チェックポイント制御剤と併用されること)が記載されていてよい。
以下、本発明を実施例に基づきより具体的に説明する。もっとも、本発明は下記実施例に限定されるものではない。
1.大腸癌細胞移植マウスにおけるCFI2101+免疫チェックポイント制御剤併用の抗腫瘍効果
<材料>
担癌マウス:
7週齢の雄性BALB/c系マウスを1群8匹で使用し、Colon26マウス大腸癌細胞を2 x 105 cells /mouseで右側腹部皮下に移植した。
抗体:
抗マウスPD-L1抗体(clone 10F.9G2)はBioXcell社より購入した。
被検化合物:
N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミド(アミド誘導体A)は、WO 2014/003124 A1の実施例62に記載された方法で合成されたものを用いた。マウスへの投与には、アミド誘導体Aの塩酸塩(以下、CFI2101)を用いた。

<方法及び結果>
Colon26担癌マウスにおいて、CFI2101の用量を0.02, 0.2, 2.0, 20mg/kgに設定し、抗PD-L1抗体(100μg/mouse)との併用での効果と用量反応性を調べた。Colon26細胞 2x105個をBALB/cマウスの右側腹部皮下に移植し、移植後4, 8, 12日に抗PD-1抗体を腹腔内投与(i.p.)した。CFI2101は、0.5%メチルセルロースに懸濁して移植4日目より14日間連続経口投与した(p.o.)。移植4日目より個々のマウスの腫瘍体積を経時的に計測し、以下の計算式で腫瘍体積を算出した。

移植4日目より個々のマウスの腫瘍体積を経時的に計測した結果を図1に示す。また、腫瘍移植19日後の腫瘍体積を各投与群間で比較した結果を図2に示す(Control群に対する有意差 #: p< 0.05(Dunnett)、CFI2101+抗PD-L1抗体併用群の抗PD-1抗体単独群に対する有意差 ** p< 0.01(Dunnett))。CFI2101単独では有意な固形癌抑制作用はみられなかった。しかし、抗PD-L1抗体の併用群では、有意な抗腫瘍活性が得られ、CFI2101 0.02, 0.2, 2.0 mg/kgで用量反応性が確認できた。特に、CFI2101 2.0 mg/kg+抗PD-L1併用群では、8例中2例で固形腫瘍が排除された。CFI2101 20 mg/kg+抗PD-L1併用群においても固形腫瘍が排除されたマウスが見られた。これらの結果から、CFI2101は抗PD-L1抗体と併用で0.02~2mg/kgで用量反応性が確認され、至適用量は2 mg/kgとなった。
次いで、腫瘍免疫の成立を確認することを目的に、Colon26固形腫瘍が排除された個体3例および部分的に抑制された個体1例の左側腹部に新たにColon26 3x105個を移植し、左右の腫瘍増殖を測定した。2回目の腫瘍移植を行なった初回移植後33日目における右側腹部の腫瘍体積は表1の通りであった。
左右の腫瘍増殖の測定結果を図3に示す。右側腹部に移植した腫瘍がrejectあるいは殆ど抑制した3匹では、左に移植した腫瘍は固形腫瘍に増殖することはなく、完全に抑制された。一方、右側腫瘍が大きいマウスでは左側移植腫瘍の増殖も抑制されることはなく、右側腫瘍と同様に増殖した(図3のCFI2101 2mg/αPD-L1個体No.6)。以上の結果より、CFI2101と抗PD-L1抗体との併用投与で抗腫瘍免疫が成立していて、投与終了後も持続することが判明した。
2.メラノーマ細胞移植マウスにおけるCFI2101+免疫チェックポイント制御剤併用の抗腫瘍効果
<材料>
担癌マウス:
7週齢の雌性C57BL/6系マウスを1群8匹で使用し、B16マウスメラノーマ細胞を5 x 105 cells /mouseで右側腹部皮下に移植した。
抗体:
抗マウスPD-L1抗体(clone 10F.9G2)はBioXcell社より購入した。

WO 2014/003124 A1の実施例62に記載された方法で合成されたアミド誘導体Aの塩酸塩(CFI2101)を用いた。

個々のマウスの腫瘍体積を経時的に計測した結果を図4に示す。腫瘍移植14日後の腫瘍体積を各投与群間で比較した結果を図5に示す(Control群に対する有意差 *: p< 0.05(Dunnett))。B16担癌マウスではCFI2101単独では抑制作用は見られなかった。CFI2101と抗PD-L1抗体の併用では、腫瘍の排除には至らなかったものの、腫瘍増殖抑制効果は得られた。抗PD-L1抗体との併用では、0.4, 2 mg/kg で用量反応性が観察され、2 mg/kgが至適用量であった。
3.肺癌細胞移植マウスにおけるCFI2101+免疫チェックポイント制御剤併用の抗腫瘍効果

LLC担癌マウスにおいて、CFI2101の用量を0.02, 0.2, 2.0 mg/kgに設定し抗PD-L1抗体(100μg/mouse)との併用での効果と用量反応性を調べた。LLC細胞5x105個をC57BL/6マウスの右側腹部皮下に移植し、移植後4, 8日に抗PD-1抗体を腹腔内投与した。CFI2101は0.5%メチルセルロースに懸濁して移植4日目より8日間連続経口投与した。移植8日目より個々のマウスの腫瘍体積を経時的に計測し、以下の計算式で腫瘍体積を算出した。
腫瘍体積(mm3) = (長径; mm) x (短径; mm)2 x 0.5236
個々のマウスの腫瘍体積を経時的に計測した結果を図6に示す。腫瘍移植14日後の腫瘍体積を各投与群間で比較した結果を図7に示す(Control群に対する有意差:* p< 0.05, ** p< 0.01(Dunnett)。CFI2101+抗PD-L1抗体併用群の抗PD-L1抗体単独群に対する有意差 #: p< 0.05, ## p< 0.01(Dunnett))。CFI2101は単独でも0.2 mg/kgで有意なLLC固形癌抑制作用がみられたが、2 mg/kgでは有意な効果ではなかった。抗PD-L1抗体との併用では、0.02, 0.2 および2 mg/kgの3用量で有意な抗腫瘍活性が確認できた。抗PD-L1抗体単独群に対しても0.02および2mg/kgで有意な併用効果が得られた。特に2mg/kgの投与量では抗PD-L1抗体との併用で8例中1例では固形腫瘍が完全退縮され、3例でほとんど退縮する結果が得られた。
4.CFI2101とGalunisertibの抗腫瘍効果の比較
Galunisertibは、抗線維化作用を有する薬剤であるが、抗腫瘍剤としても開発されており、薬効薬理試験、臨床治験結果がすでに報告されている。本実施例では、Colon26担癌マウスモデルを用いてCFI2101とGalunisertibの抗腫瘍作用の比較試験を行なった。
Galunisertibは、抗線維化作用を有する薬剤であるが、抗腫瘍剤としても開発されており、薬効薬理試験、臨床治験結果がすでに報告されている。本実施例では、Colon26担癌マウスモデルを用いてCFI2101とGalunisertibの抗腫瘍作用の比較試験を行なった。
Colon26担癌マウスにおいて、CFI2101の用量を2 mg/kgで1日1回経口投与、Galunisertib(MidChemExpress)は、2および75 mgの2用量で1日2回の経口投与に設定した(4, 150 mg/kg/day)。Colon26細胞3x105個をBALB/cマウスの右側腹部皮下に移植し、移植後4, 8, 12日に抗PD-L1抗体を100μg/mouseで腹腔内投与した。CFI2101は移植4日目より14日間連続経口投与した。Galunisertibは移植4日目より15日間経口投与した。移植5日目より個々のマウスの腫瘍体積を経時的に計測した。また、腫瘍移植19日後の腫瘍体積を示し、各投与群間で比較した。
腫瘍移植19日後の腫瘍体積を各投与群間で比較した結果を図8に示す(Control群に対する有意差:* p< 0.05, ** p< 0.01(Dunnett))。Galunisertibは2 mg/kg, 75 mg/kg、 (1日用量:4 mg/kg, 150 mg/kg)の2用量で投与したが、2 mg/kgでは、単独でも抗PD-L1抗体併用でも、同用量のCFI2101より効果は弱い結果であった。75 mg/kg(150 mg/kg/day)は、Galunsertibで報告されている有効量であるが、この投与量でCFI2101 2 mg/kgと抗腫瘍効果が同様になった。
Galunisertibは、抗腫瘍剤としての開発のために、単剤に加えて、抗PD-1抗体との併用による治験も進められている(非特許文献7)。有効量を考慮した場合、CFI2101はGalunisertibよりも薬効について優っていると考えられる。
Claims (6)
- N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドである化合物、薬剤的に許容されるその塩、又は前記化合物若しくは前記塩の溶媒和物を有効成分とし、少なくとも1種の免疫チェックポイント制御剤と組み合わせて用いられる、抗がん剤。
- 前記抗がん剤の投与量が、前記有効成分の量で1日当り0.1 mg/kg体重~10 mg/kg体重である、請求項1記載の抗がん剤。
- 前記抗がん剤が経口剤である、請求項1記載の抗がん剤。
- 前記免疫チェックポイント制御剤が、アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、アンタゴニスト性抗LAG-3抗体、アンタゴニスト性抗TIM-3抗体、及びアンタゴニスト性抗CTLA-4抗体から選択される少なくとも1種の免疫チェックポイント阻害剤である、請求項1~3のいずれか1項に記載の抗がん剤。
- 前記免疫チェックポイント制御剤が、アンタゴニスト性抗PD-1抗体、抗PD-L1抗体、及び抗PD-L2抗体から選択される少なくとも1種である、請求項4記載の抗がん剤。
- N-(4-(5-オキソ-4,5-ジヒドロ-1,2,4-オキサジアゾール-3-イル)ビフェニル-3-イル)-3-(ピペリジン-1-イル)ベンズアミドである化合物、薬剤的に許容されるその塩、又は前記化合物若しくは前記塩の溶媒和物の有効量を、少なくとも1種の免疫チェックポイント制御剤と組み合わせてがん患者に投与することを含む、がんの治療方法。
