Abstract
일 양상은 유럽-동아시아인 데이터 기반 알츠하이머병 치매의 발병 위험군 또는 조기 증상 발현 위험군, 기억상실형 경도인지장애의 발병 위험군 및/또는 아밀로이드β 침착에 대한 PET 양성 위험군을 예측하기 위한 정보를 제공하는 방법에 관한 것이다. 일 양상에 따르면, 상기 방법은 최소 12개의 단일염기다형성만으로도 알츠하이머병 치매의 발병 위험군 또는 조기 증상 발현 위험군, 기억상실형 경도인지장애의 발병 위험군 및/또는 아밀로이드β 침착에 대한 PET(positron emission tomography) 양성 위험군의 예측이 정확하게 가능하며, 최대 80개까지 추가로 단일염기다형성을 사용하는 경우, 상기 위험군들의 예측력이 보다 우수하다. 또한, 상기 방법은 지표로서 연령, 성별, 교육연수 및 APOE 유전형을 더 확인함으로써, 상기 위험군들의 예측력이 보다 우수해지는 바, 상기 위험군들을 높은 정확도로 조기에 예측할 수 있다.

유럽-동아시아인 데이터 기반 알츠하이머병 치매의 발병 위험군 또는 조기 증상 발현 위험군, 기억상실형 경도인지장애의 발병 위험군 및/또는 아밀로이드β 침착에 대한 PET 양성 위험군을 예측하기 위한 정보를 제공하는 방법에 관한 것이다.
알츠하이머병은 전 세계적으로 약 5천만명에게 영향을 미치는 치매의 주요 원인이며, 인구 고령화로 인해 2050년까지 그 수가 3배로 증가할 것으로 예상된다. 특히, 인구 고령화가 빠르게 진행되는 동아시아에서 문제가 된다. 치매 환자의 거의 4분의 1이 동아시아에 거주하는 것으로 추정되며, 그 수는 20년마다 두 배로 증가할 것으로 예상된다.
알츠하이머병의 병리학적 과정은 임상적 치매가 발병하기 훨씬 전에 시작된다. 따라서 알츠하이머병 발병 위험이 높은 개인을 식별하는 것은 잠재적인 예방 및 치료 전략에 매우 중요하다. 알츠하이머병의 유전력은 60-80%로 추정되기 때문에 유전 정보는 알츠하이머병에 걸릴 위험이 높은 개인을 식별하는 데 사용될 수 있다. 이전 연구에서는 GWAS(genome-wide association studies)에서 확인된 단일염기다형성(single nucleotide polymorphism, SNP)의 유전적 효과를 집계하는 다유전자 위험 점수(polygenic risk score, PRS)가 알츠하이머병에 대한 유전적 위험이 높은 개인을 구별하는데 도움이 될 수 있다는 것을 입증하였다.
그러나 이전의 유전 연구는 주로 유럽 인종 집단에서 수행되었다. 따라서 비유럽 인구에 대한 PRS의 일반화 가능성은 아직 알려지지 않았다. 최근 연구에서는 다양한 표현형에 대해 비유럽계 조상 표본에서 유럽계 조상 유래 PRS의 예측력을 조사하였다. 유럽 인구에서 파생된 알츠하이머병에 대한 PRS는 히스패닉계가 아닌 흑인 및 카리브해 히스패닉계 인구에서 평가되었다. 그러나 아시아 인구에서 알츠하이머병에 대한 PRS의 성능은 아직 평가되지 않았다.
이에, 본 발명자들은 유럽 인구에 대한 이전 대규모 GWAS의 요약 통계와 동아시아 인구에 대한 이전 대규모 GWAS의 요약 통계를 META 분석한 결과를 사용하여 한국 인구에서 알츠하이머병에 대한 PRS의 인종 간 전이 가능성을 검증하는 것을 목표로 연구하였다. 그 다음 연구 결과를 한국 인구의 독립적인 코호트에 재현하였으며, 다유전자 위험 점수를 적용하여 알츠하이머병 치매(ADD), 기억상실형 경도인지장애(aMCI) 또는 아밀로이드 β(Aβ) 침착을 예측할 수 있는지를 평가하였다.

이하 본 발명을 실시예를 통하여 보다 상세하게 설명한다. 그러나, 이들 실시예는 본 발명을 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 한정되는 것은 아니다.
실시예
1. 검증과 기억상실형 경도인지장애(aMCI) 데이터세트
2013년 1월부터 2019년 7월까지 대한민국 14개 병원에서 전체 1,255명의 한국인 피험자를 모집하였다. 구체적으로, 삼성서울병원에서 954명, 알츠하이머병 조기 진단 및 예측을 위한 한국 뇌 노화 연구의 다기관 연구(Korean Brain Aging Study for Early Diagnosis and Prediction of AD)에서 202명, 치매 코호트 기반의 다기관 임상 연구 플랫폼 연구(multicenter clinical research platform study based on the Dementia Cohort)에서 99명을 모집하였다(표 1). 상세한 신경심리학적 검사 결과에 근거하여 알츠하이머병 치매(ADD: Alzheimer's disease dementia) 또는 기억상실형 경도인지장애(aMCI: amnestic mild cognitive impairment)로 진단된 피험자 또는 정상인지(CU: cognitively unimpaired)로 진단된 피험자를 포함하였으며, 가장 최근의 평가 시점에서 피험자들의 진단을 사용하였다. 알츠하이머병 치매(ADD)는 National Institute on Aging-Alzheimer's Association에 따라 유력 알츠하이머병 치매(ADD)에 대한 핵심 임상 기준으로 정의하였다. 기억상실형 경도인지장애(aMCI)는 Peterson의 기준에서 수정된 다음 기준에 따라 정의하였다: (i) 일상 생활 수행의 정상적인 활동, (ii) 객관적인 기억 장애, 즉, 언어기억 또는 시각기억 테스트에서 연령 및 교육 일치 규범의 16번째 백분위수 미만의 수행능력 그리고 (iii) 치매 없음.
피험자는 (i) PSEN1(Presenilin-1), PSEN2(Presenilin-2) 및 APP(Amyloid-beta precursor protein)와 같은 알려진 유전자에서 알츠하이머병(AD)에 대한 원인 유전자 돌연변이가 있는 경우에 제외하였고; (ii) 심각한 뇌허혈, 뇌경색 또는 뇌종양과 같은 뇌 자기공명영상에서 발견된 구조적 이상; 또는 (iii) 인지저하를 유발할 수 있는 기타 의학적 또는 정신과적 질병이 있는 경우에 제외하였다. 모든 피험자는 서면 동의서를 제공하였으며, 연구는 각 센터의 기관 검토 위원회의 승인을 받았다.

2. 재현 데이터세트
재현 데이터세트를 위해 한국의 20개 위탁 병원에서 379명의 한국인 데이터를 확보하였다. 구체적으로, 125명의 피험자는 2016년부터 2018년까지 모집된 만성 뇌혈관 질환 협력단(Chronic Cerebrovascular Disease consortium)의 인체자원은행(biobank)으로부터 확보하였으며, 나머지 254명의 데이터는 PRECISION medicine platform for mild cognitive impairment based on Multi-omics, Imaging 및 Evidence-based R&BD(PREMIER) 코호트에서 확보하였다. 또한, 검증 데이터 세트의 동일한 기준에 따라 알츠하이머병 치매(ADD) 또는 정상인지(CU)를 포함하였다.
3. 유전자형 분석 및 통계적 추정
DNA 검체는 Illumina Asian Screening Array BeadChip(ASA 칩, CA, CA)을 사용하여 유전자형을 분석하였다. 검체의 일부(n=125)는 Affymetrix 맞춤형 Korea Biobank Array chip(KBA 칩, Affymetrix, CA, USA)을 사용하여 유전자형을 분석하였으며, 두 가지 유형의 단일염기다형성(SNP: single-nucleotide polymorphisms) 데이터에 대해 품질 관리(QC: Quality control)를 수행하였다. SNP는 다음 기준에 따라 제거하였다: (i) <98%의 call rate, (ii) <1%의 소수 대립 유전자 빈도(MAF), 또는 (iii) <10-6의 P 값으로 Hardy-Weinberg 평형에서 상당히 벗어나는 유전자형 빈도. 품질 관리(QC) 후, 유전자형 데이터는 직접적으로 유전자형이 지정되지 않은 변이에 대한 유전자형을 추정하고, 서로 다른 유전자형 어레이(ASA 칩 및 KBA 칩)의 데이터 세트를 결합하기 위해 통계적 추정(imputation) 방법을 적용하였다. 유전자형의 통계적 추정(imputation)은 University of Michigan Imputation Server의 Haplotype Reference Consortium(HRC-r1.1 2016)에서 사용 가능한 모든 참조 Haplotype과 함께 Minimac4 소프트웨어를 사용하여 수행하였다. 결과적으로, 우리는 (i) <1%의 MAF 또는 (ii) 낮은 추정 품질(추정된 SNP의 경우 R2<0.8)로 통계적 추정(imputation) 후 품질 관리(QC)를 수행하였다. 두 유전자형 데이터 세트의 적절한 조합을 확인하기 위해, EIGENSTRAT를 사용하여 주성분 분석(PCA: principal component analysis)을 수행하였다. 또한, 1,000 Genomes Project 샘플에 대해 PCA를 수행하고, 인종 구별을 확인하기 위해 두 개의 유전자형 데이터 세트를 PCA 플롯에 투영하였다. 유전자형 데이터를 기반으로, 피험자는 다음 기준에 따라 제외하였다: (i) <95%의 call rate, (ii) 성 불일치, (iii) 이형 접합성 초과(평균에서 ±5 표준 편차[SDs: standard deviations]), 또는 (iv) KING 소프트웨어를 사용하여 추정된 2급 친족 이내의 혈연 관계를 가진 관련 쌍 중 하나.
한국 연구 집단과 1,000 Genomes Project 모집단의 주성분을 비교한 결과, 1,000 Genomes Project 데이터 세트가 있는 주성분 분석(PCA)에서 다른 동아시아 인구의 데이터와 인종적으로 중복이 존재하였다. 그러나 유전형 배열에 의한 계층화는 없었으며, 유전자형 배열에 따른 연구 피험자 간의 다유전자 위험 점수(PRS: polygenic risk score) 분포는 유전형 배열에 따라 큰 차이는 없었다(도 1a 내지 1c 및 도 2).
4. 아밀로이드 양전자 방출 단층 촬영 (PET)
검증 및 기억상실형 경도인지장애(aMCI) 데이터 세트의 일부 피험자(n=1,214)는 Discovery STE PET/컴퓨터 단층 촬영 스캐너(GE Medical Systems, Milwaukee, WI, USA)를 사용하여 수행된 아밀로이드 PET(Amyloid positron emission tomography)를 하였다. PET 이미지는 18F-플로르베타벤(18F-florbetaben) 또는 18F-플루테메타몰(18F-flutemetamol)의 정맥 주사 후 90분부터 시작하여 20분 동안 획득하였다. 아밀로이드β(Aβ: Amyloid β) 양성 또는 음성은 잘 훈련된 핵의학과 전문의에 의해 플로르베타벤 PET(florbetaben PET) 또는 플루테메타몰 PET(flutemetamol PET)에 대한 시각적 평가를 사용하여 결정하였다. 추적자 흡수(tracer uptake)에 대한 양성은 플로르베타벤 PET(florbetaben PET)의 경우 4개의 피질 영역(측두엽, 전두엽, 두정엽 및 후방 대상 피질)에서 평가하였고, 플루테메타몰 PET(flutemetamol PET)의 경우 5개 영역(측두 측두엽, 전두엽, 두정엽, 후대상 피질 및 선조체)에서 평가하였다. 아밀로이드 PET 양성은 양성 흡수의 증거를 가진 최소 하나의 피질 영역을 갖는 것으로 정의하였다.
5. GWAS 요약 통계
한국 인구에서 다유전자 위험 점수(PRS)의 전이 가능성을 조사하기 위해, 유럽 국제 알츠하이머 유전체 프로젝트(IGAP) META GWAS (21,982명의 AD 환자 및 41,944명의 대조군에서 11,480,632 SNP)와 동아시아에 기반을 둔 국립 노인병 센터 및 노인학(NCGG) 일본 GWAS(3,962명의 알츠하이머병(AD) 환자 및 4,074명의 대조군으로부터 4,852,957 SNP)에서 생성된 요약 통계를 활용하였다. 또한, METAL을 사용한 유럽 및 일본 GWAS 결과의 역분산 가중 고정 효과 메타 분석에서 획득한 유럽-동아시아인 메타 GWAS 결과(12,519,321 SNP)를 사용하여 meta-PRS를 도출하였다.
또한, 유럽-동아시아인 메타 GWAS에 대한 마이애미 플롯(Miami plot)을 진행하였다.
그 결과, 유럽-동아시아인 메타 GWAS를 통해 기존 유럽 인구에 대한 이전 GWAS의 요약 통계에서는 식별되지 않은 17번 염색체의 rs2526378 근처 복수의 단일염기다형성을 확인하였다(도 3).
또한, 유럽-동아시아인 메타 GWAS에 대한 분위수-분위수 플롯(Quantile-quantile plot)을 진행하였다.
그 결과, 관측된 P-value 값을 통해 Genomic inflation(Lambda value) 값을 계산하였고, 그 값은 1.058을 나타냈다. 이로써 수행된 유럽-동아시아인 메타 GWAS 분석 결과에서 유전적 팽창(Genomic inflation)이 보이지 않음을 확인하였다(도 4).
또한, 상기 유럽-동아시아인 메타 GWAS의 17번 염색체의 rs2526378에 대한 지역 플롯(Regional plot)을 진행하였다.
그 결과, 새로이 식별된 17번 염색체의 rs2526378과 근처 복수의 단일염기다형성이 높은 연관성을 보이는 것으로 확인하였다(도 5).
6. PRS 생성
이전 연구 데이터를 기반으로, APOE 영역으로부터 독립적인 PRS를 도출하기 위해 APOE를 둘러싼 3,877개의 SNP(염색체 19, 44,400-46,500kb, GRCh37/hg19)를 제외하였으며, PRS 계산을 위한 최상의 매개변수(P 값 임계값 및 연결 불균형(LD: linkage disequilibrium) r2 값)를 결정하기 위해 이전 유럽-동아시아인 메타 GWAS 결과를 기반으로 PRSice-2를 사용하였다. 요약 통계의 P 값과 효과 크기는 검증 데이터 세트(한국인 554명의 알츠하이머병 치매(ADD) 및 479명의 정상인지(CU) 대조군)에서 최상의 PRS 모델을 생성하는데 사용하였다. 최상의 모델을 도출하기 위해 유럽-동아시아인 메타 GWAS의 P 값 임계값 범위(5 x 10-8 - 1.0)를 바꾸어가며 SNP을 포함하여 검사하였다. 또한, 로지스틱 회귀에서 계산된 가장 큰 Nagelkerke의 R2 값을 보이는 임계값을 조사하기 위해, 1,000kb 내에서 연결 불균형(LD) r2(0.1-0.9) 범위를 검사하였다. 그 후, 검체 379명(159명의 알츠하이머병 치매(ADD) 케이스 및 220명의 정상인지(CU) 대조군)의 독립적인 데이터 세트와 기억상실형 경도인지장애(aMCI) 222명의 환자의 적용 데이터 세트에서 PRS의 연관성을 재현하기 위해 동일한 SNP와 가중치를 사용하였으며, 연구 데이터 세트 및 분석 단계를 나타내는 개요를 나타내었다(도 6).
그 결과, 다양한 임계값(P 값 및 LD 값)에 걸쳐, 유럽-동아시아인 메타 GWAS를 기반으로 한 PRS의 경우, 다양한 임계값 중에서 P 값 및 LD 값이 각각 4.12Х10-5 및 0.1인 경우에 가장 높은 Nagelkerke의 R2 값(0.023)을 확인하였다(도 7). 또한, 이 임계값으로부터 80개의 SNP를 선택하였고, 베타 계수를 PRS 생성에 사용하였다(표 2).

7. 알츠하이머병 치매(ADD) 진단을 위한 PRS 검증 및 재현
각 피험자의 PRS를 계산한 후, 유럽-동아시아인 메타 GWAS에 기초한 알츠하이머병(AD) 위험에 대한 요약 통계에서 파생된 PRS가 다변수 로지스틱 회귀 모델을 사용하여 연령, 성별, 교육연수, APOE ε4 보유상태 및 유전 조상의 처음 4개 주성분 (PCs: principal components)을 보정한 후 유효성 검사 및 복제 데이터 세트에서 알츠하이머병 치매(ADD) 진단과 연관되었는지 여부를 결정하기 위해 로지스틱 회귀 분석을 수행하였다. 또한, 알츠하이머병 치매(ADD) 진단과 PRS의 연관성이 APOE ε4 보인자 상태에 따른 차이를 확인하기 위해 피험자를 APOE ε4 보인자와 비보인자로 계층화한 후 동일한 분석을 수행하였으며, 이전의 유럽-동아시아인 메타 GWAS 결과를 기반으로 PRS를 개발하고 PRS 예측 성능을 비교하였다. OR(Odds ratio) 및 P 값은 다변수 로지스틱 회귀 분석(표준화된 PRS의 표준 편차 증가당 OR)을 사용하여 계산하였다.
그 결과, 높은 PRS는 연령, 성별, 교육 및 APOE ε4 상태의 영향을 보정한 후 알츠하이머병 치매(ADD)의 위험 증가와 관련이 있다는 것을 확인하였으며, PRS는 APOE ε4 보인자(교차비[OR]=2.82, 95% CI=1.75-4.59, P<0.001) 및 비보인자(OR=1.63, 95% CI=1.09-2.44, P=0.019) 모두에서 알츠하이머병 치매(ADD) 위험과 관련이 있다는 것을 확인하였다. 또한, 더 높은 PRS가 뇌에서 기억상실형 경도인지장애(aMCI) 및 아밀로이드β(Aβ) 침착의 증가된 위험과 유의하게 연관되어 있는 것을 확인하였다(표 3).

8. 다양한 표현형에서 PRS의 적용
연령, 성별, 교육연수, APOE ε4 보인자 상태 및 유전 조상의 처음 4 개 주성분(PCs)과 독립적으로 PRS가 기억상실형 경도인지장애(aMCI)를 예측하는지 여부를 평가하기 위해 기억상실형 경도인지장애(aMCI)가 있는 피험자에 대해 다변수 로지스틱 회귀 분석을 수행하였다. 아밀로이드β(Aβ) PET도 시행한 일부 피험자(n=1,214)로 PRS가 아밀로이드β(Aβ) 양성을 예측하는지 여부를 평가하기 위해 로지스틱 회귀 분석도 수행하였으며, 아밀로이드β(Aβ) PET를 수행한 연령, 성별, 교육연수 및 APOE ε4 보인자 상태의 영향을 조정하였다.
또한, PRS의 임상적 유용성을 테스트하기 위해 각 피험자의 알츠하이머병 치매(ADD) 진단을 예측하는 다변수 로지스틱 모델을 개발하였으며, 로지스틱 모델의 성능을 평가하기 위해 AUC(area under curve)를 측정하였다. 평균 AUC를 모델의 95% 신뢰 구간(CI: confidence interval)으로 보고 하였다.
그 결과, 예측 모델에서, 단지 임상적 요인(연령, 성별, 교육연수)만을 포함하는 경우는 0.589(95% CI=0.569-0.609)의 AUC를 나타내었으며, 상기 임상적 요인에 APOE ε4 상태를 포함하는 경우, 예측 성능이 증가하는 것을 확인하였다(AUC=0.697; 95% CI=0.679-0.716). 또한, 상기 임상적 요인과 APOE ε4 상태를 포함하고, PRS가 더 포함된 경우, 상기 임상적 요인과 APOE ε4 상태를 포함하는 경우에 비해 예측 성능이 현저히 향상되는 것을 확인하였다(AUC=0.710; 95% CI=0.692-0.728).
또한, PRS의 사분위수를 기반으로 피험자를 계층화하고, APOE ε4 유전자형 외에 위험 계층화에도 PRS를 사용할 수 있는지 여부를 평가하였으며, 높은 PRS를 가진 피험자가 낮은 PRS를 가진 피험자보다 알츠하이머병(AD)의 조기 발달을 보이는지 평가하였다. 또한, 마지막 임상 방문 시 연령 또는 알츠하이머병(AD) 발병 연령을 시간 변수로 하고 알츠하이머병(AD)을 상태 변수로하여 콕스 회귀 분석(Cox regression analysis)을 수행하였다.
그 결과, PRS와 APOE ε4 상태를 결합하는 경우, APOE ε4 보인자와 비보인자 모두에서 PRS 사분위수에 따라 알츠하이머병 치매(ADD), 아밀로이드β(Aβ) 침착 및 알츠하이머병 치매의 조기 증상 발병 위험이 단계적으로 증가하는 것을 확인하였다(표 4). 특히, 낮은 PRS 그룹의 APOE ε4 비보인자와 비교하여 매우 높은 PRS 그룹의 APOE ε4 보인자는 7.50배(95% CI=4.43-13.13), 14.91배(95% CI=8.59-26.84) 및 3.01배(95% CI=2.04-4.45)로 알츠하이머병 치매(ADD), 아밀로이드β(Aβ) 침착 위험 및 증상 발병 초기 연령이 각각 더 높은 것을 확인하였다. 이는 PRS가 알츠하이머병 병리(Aβ 침착, 타우(tau) 및 신경변성)와 관련 있음을 보여주었던 이전 연구 결과와 일치하며, 아밀로이드β(Aβ) 침착이 있는 환자를 식별하는 것은 예후를 예측하고, 항-Aβ 요법의 임상 시험을 위한 환자를 선택하는데 중요하다는 것을 확인하였다. 현재 아밀로이드β(Aβ) 침착 측정을 위한 진단 도구는 침습적(뇌척수액 검사)이거나 고가(PET)이다. 연구 결과는 덜 침습적인 방법(혈액 또는 타액 검체 평가)에서 얻은 유전자 데이터(PRS 및 APOE ε4 상태)가 아밀로이드β(Aβ) 양성에 대한 사전 선별에 사용될 수 있음을 강조한다.
또한, PRS가 높은 환자는 젊은 나이에 알츠하이머병 치매(ADD) 증상이 발생할 가능성이 더 높다는 것을 확인하였다. 증상 발병 시 평균 연령은 PRS가 낮은 그룹에 비해 PRS가 매우 높은 그룹에서 약 3.3세 더 젊다는 것을 확인하였다. APOE ε4가 알츠하이머병 치매(ADD)의 초기 증상 발병과 관련이 있다는 것은 잘 알려져 있으며, 우리의 결과로 PRS가 APOE ε4의 효과를 넘어 증상 발병 연령을 더욱 앞당긴다는 것을 확인하였다.

9. 통계학적 분석
PRS 분위수에 따른 피험자의 인구통계학적 특성과 임상적 특성에 대한 범주형 변수와 연속형 변수를 각각 개수(%)와 평균(SDs)으로 나타내었다. 범주형 변수와 연속형 변수에 대한 분산 분석에 카이제곱 검정(chi-square test)을 사용함으로써 P 값을 획득하였다(표 5). 양측 P 값을 보고하였으며, P 값 <0.05를 통계적으로 유의한 것으로 정의하였다. 또한, 모든 통계 분석 및 결과에 대하여 PLINK 1.90, R version 3.6.1(R Project for Statistical Computing) 및 MATLAB으로 시각화하였다.

4가지 인자(연령, 성별, 교육연수 및 APOE ε4)를 각각 포함하였을 때, 알츠하이머병 치매(ADD) 위험군 예측 모델에 사용되는 계산식을 획득하였다. 각 인자 별 정의; PRS는 상기 제시한 모델을 통해 개개인에 계산된 개개인 별 유전적 위험 점수이다; 연령은 연수를 기준으로 부여한다; 성별은 남성을 1, 여성을 2로 정의한다; 교육연수를 연수를 기준으로 부여한다; 상기 개체의 지표 중 APOE 유전형의 경우, ε2/ε2, ε2/ε3 및 ε3/ε3는 0점을 부여하고, ε2/ε4, ε3/ε4 및 ε4/ε4는 1점을 부여하여, 각 지표 별 점수를 얻는 단계를 더 포함할 수 있다. PRS 점수와 추가 4가지 인자를 종속항으로 하는 회귀 모델에 사용될 수 있는 하기 수학식 2에 해당하는 계산식을 획득하였다.

또한, 4가지 인자가 함께 고려된 PRS(80 SNPs)의 알츠하이머병 치매(ADD) 위험군의 예측 성능을 분석하였으며, AUC (Area Under Curve): 0.7101; Nagelkerke R2: 0.1775; P-value<0.0001의 값을 획득하였다.
(3) 55개의 SNP의 예측력
80개의 SNP 중, 추가 선별 과정으로써 한국인 데이터와 메타 분석 데이터(표 7)에서 알츠하이머병 치매(ADD) 환자 그룹과 대조 그룹 사이에 association β coefficient의 방향성이 같은 55개의 SNP을 선별하여 PRS를 구성하였다. 각 구성된 SNP 별 차이는 인자 유무의 차이이며, 인자 별로 추정된 계수는 동일하다(표 8). 또한, 선별된 55개의 SNP 별 계수를 고려하여, 위험군 예측 모델에 사용될 수 있는 하기 수학식 3에 해당하는 계산식을 획득하였다.

상기 PRS(55 SNPs)의 알츠하이머병 치매(ADD) 위험군의 예측 단독 성능을 분석한 결과, 추가로 선별된 55 SNPs가 80 SNPs 보다 알츠하이머병 치매(ADD) 위험군의 예측력이 우수하다는 것을 확인하였으며, AUC (Area Under Curve): 0.6140; Nagelkerke R2: 0.0565; P-value<0.0001의 값을 획득하였다.
또한, 4가지 인자가 함께 고려된 PRS(55 SNPs)의 알츠하이머병 치매(ADD) 위험군의 예측 성능을 분석한 결과, AUC (Area Under Curve): 0.7244; Nagelkerke R2: 0.2011; P-value<0.0001의 값을 획득하였다.

(4) 68개의 PRS 모델들의 성능
80개의 SNP들을 순차적으로 제외하며 구성한(P-value의 유의수준에 따라 덜 유의한 순서대로 줄여가며) PRS 모델들의 성능 및 68개의 PRS들을 획득하였다.
실험은 PRS (80 SNPs)에서 유의성 수준(P-value)이 상대적으로 높았던 SNP들을 순차적으로 제거하며 여러 PRS들을 구성하였고, 각 SNP는 단독 혹은 잘 알려진 4가지 인자(연령, 성별, 교육연수 및 APOE ε4)가 고려된 SNP로 평가하였으며, 성능 수준은 AUC와 유의수준 P-value로 비교하였다.
실험 결과, PRS(80 SNPs)에서 1개의 SNP가 제외된 PRS(79 SNPs) 모델부터 12개의 SNP들을 활용한 PRS까지 총 68가지의 PRS들이 알츠하이머병 치매(ADD) 위험군을 예측하는 인자로써 통계적인 유의성이 확보되며 작용하는 것을 확인하였다(표 9).
