本発明は、マイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性を抑制させる物質を含む、HTLV-1関連脊髄症(HAM)の治療又は予防剤に関する。
Description
本発明は、HTLV-1関連脊髄症(HAM)の治療又は予防剤、及びHAMの治療方法に関する。また、本発明は、HAMの治療有効性判定方法、活動性評価方法又は診断方法、及びこれらの方法に用いられるキットに関する。
ヒトT細胞白血病ウイルス1型(Human T-cell Lukemia Virus Type 1;HTLV-1)は、血液中の白血球の1つであるT細胞等(主にCD4陽性T細胞)に感染するウイルスである。
HTLV-1に感染したT細胞は、脊髄に慢性の炎症を引き起こす。その結果、脊髄神経細胞の障害及び変性が引き起こされ、痙性脊髄麻痺等を惹起する。HTLV-1に感染した細胞を原因とする痙性脊髄麻痺は、HTLV-1関連脊髄症(以下、「HAM」ともいう。)と呼ばれる。HAMの病態メカニズムは、HTLV-1感染細胞が慢性的な炎症ループを引き起こすことであると考えられている。
HTLV-1に感染したT細胞は、脊髄に慢性の炎症を引き起こす。その結果、脊髄神経細胞の障害及び変性が引き起こされ、痙性脊髄麻痺等を惹起する。HTLV-1に感染した細胞を原因とする痙性脊髄麻痺は、HTLV-1関連脊髄症(以下、「HAM」ともいう。)と呼ばれる。HAMの病態メカニズムは、HTLV-1感染細胞が慢性的な炎症ループを引き起こすことであると考えられている。
HAMの症状としては、神経の組織障害による、両足の麻痺、痛み、及び排尿排便障害等が挙げられる。これらの症状が進行すると、車いす生活、寝たきり生活になる。HAMは日本国の指定難病に指定される疾患の一つである。現在、有効なHAMの治療方法は確立されておらず、専ら対症療法がおこなわれている。
HAMの治療方法として、例えば抗CCR4抗体を用いる方法、及びRGMa阻害物質を用いる方法等が提案されている(非特許文献1、特許文献1及び2)。

非特許文献1、特許文献1及び2に開示されるようなHAMの治療方法が知られているものの、上記のようなHTLV-1感染細胞による慢性的な炎症ループのトリガーとなる原因因子は同定されていない。
そこで、本発明者らは、原因因子を同定することができれば、当該因子を治療ターゲットとすることで、HAMを治療することができる新規の治療又は予防方法等を提供することができると考えた。
本発明が解決しようとする課題は、HAMを治療することができる新規の治療又は予防方法等を提供することである。

本発明者らは、上記課題を解決するために鋭意検討を重ねた結果、HTLV-1感染細胞による慢性的な炎症ループの原因因子として、マイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(以下、「MAP3K8」ともいう。)を見出した。そして、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させることがHAMの治療に有効であることを見出し、本発明を完成するに至った。

以下、本発明を実施するための形態(以下、「本実施形態」という。)について詳細に説明するが、本発明はこれに限定されるものではなく、その要旨を逸脱しない範囲で様々な変形が可能である。

本実施形態の一態様は、HAMの治療又は予防剤である。
本実施形態の一態様のHAMの治療又は予防剤は、マイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性を抑制させる物質を含む。
MAPKシグナル伝達経路は、細胞増殖に関わるシグナル伝達経路であり、マイトジェン活性化プロテインキナーゼ(mitogen-activated protein kinase;MAPK)、MAPKキナーゼ、及びMAPKKキナーゼ(MAPKKKとも称され、以下、「MAP3K」という。)の3種のキナーゼ群を含むプロテインキナーゼカスケードである。プロテインキナーゼは、ペプチド及び/又はタンパク質中のセリン及び/又はトレオニン及び/又はチロシンをリン酸化する酵素である。
本実施形態の一態様は、HAMの治療又は予防剤である。
本実施形態の一態様のHAMの治療又は予防剤は、マイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性を抑制させる物質を含む。
MAPKシグナル伝達経路は、細胞増殖に関わるシグナル伝達経路であり、マイトジェン活性化プロテインキナーゼ(mitogen-activated protein kinase;MAPK)、MAPKキナーゼ、及びMAPKKキナーゼ(MAPKKKとも称され、以下、「MAP3K」という。)の3種のキナーゼ群を含むプロテインキナーゼカスケードである。プロテインキナーゼは、ペプチド及び/又はタンパク質中のセリン及び/又はトレオニン及び/又はチロシンをリン酸化する酵素である。
MAPKは、哺乳類では4種のファミリー、具体的には細胞外シグナル制御タンパク質キナーゼ(extracellular signal-regulated protein kinase)1及び2(以下、「ERK1/2」という。);ERK5;c-JunN末端キナーゼ(JNK);及びp38 MAPKを代表とするファミリーに分類され、それぞれのファミリーが独立したカスケードを形成していることが知られている。
中でもERK1/2が関与するカスケードは主に増殖因子等により活性化され、下流のERK1/2は活性化されると核内へ移行して転写因子等と相互作用し細胞増殖シグナルを活性化する。ERK1/2は、MAPKキナーゼであるMAPK/ERKキナーゼ1及び2(以下、「MEK1/2」という。)により活性化(リン酸化)され、MEK1/2は、Rafを代表とする複数のMAP3Kにより活性化(リン酸化)され得る。
MAP3K8は、Tpl-2、COT、又はMEKK8等としても知られ、MEK1/2をリン酸化するMAP3Kの1種である。

本明細書において、リン酸化MEK1/2をp-MEK1/2と、リン酸化ERK1/2をp-ERK1/2とそれぞれいう場合がある。

本明細書において、リン酸化MEK1/2をp-MEK1/2と、リン酸化ERK1/2をp-ERK1/2とそれぞれいう場合がある。
本発明者らは、鋭意研究した結果、後述する実施例で示すように、HTLV-1感染細胞による慢性的な炎症ループの原因因子の1つがMAP3K8であることを見出した。すなわち、本発明者らはHAM患者の体内ではMAP3K8が特異的に過剰発現していることを見出した。また、当該知見に基づいて、HAM患者の体内ではMAP3K8が関与するMAPKシグナル伝達経路が過剰に活性化される結果、炎症性サイトカインが過剰に産生されていると推定し、当該MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制することがHAMの治療又は予防に有効であることを見出した。
したがって、上記MAP3K8が関与するMAPKシグナル伝達経路は、炎症性サイトカインの産生に寄与する伝達経路であってよい。上記炎症性サイトカインは、インターフェロン(IFN)、インターロイキン(IL)、腫瘍壊死因子(TNF)、及び/又はケモカインであってよい。上記炎症性サイトカインは、CXCL10、IFN-γ、TNF-α、IL-6、及び/又はIL-10であってよい。
HAMの治療又は予防剤に含まれる、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させる物質(以下、単に「抑制物質」ともいう。)は、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させる物質である。
本明細書において、「シグナル伝達経路の活性を抑制させる」とは、当該シグナル伝達経路におけるシグナル伝達の少なくとも一部分を阻害することを意味し、例えば少なくとも1つのプロテインキナーゼの活性を低下又は消失させること、少なくとも1つのタンパク質のリン酸化を阻害すること、及びリン酸化以外のシグナル伝達を阻害すること等を包含する。本明細書中、「活性の抑制」とは、抑制物質なしの状態において有する機能又は活性を低下させ、又は消失させることをも包含する。
本明細書において、「シグナル伝達経路の活性を抑制させる」とは、当該シグナル伝達経路におけるシグナル伝達の少なくとも一部分を阻害することを意味し、例えば少なくとも1つのプロテインキナーゼの活性を低下又は消失させること、少なくとも1つのタンパク質のリン酸化を阻害すること、及びリン酸化以外のシグナル伝達を阻害すること等を包含する。本明細書中、「活性の抑制」とは、抑制物質なしの状態において有する機能又は活性を低下させ、又は消失させることをも包含する。
抑制物質は、一態様において、MAP3K8によるMEK1/2のリン酸化、リン酸化MEK1/2によるERK1/2のリン酸化、及びリン酸化ERK1/2による転写因子等の活性化を含むプロテインキナーゼカスケードにおいて、少なくとも1つのプロテインキナーゼを阻害するか、少なくとも1つのプロテインキナーゼの少なくとも1つの基質と相互作用し、そのプロテインキナーゼカスケードのシグナル伝達経路の活性を抑制させ得る。抑制物質は、一態様において、MAP3K8が関与し、炎症性サイトカインの産生に寄与するMAPKシグナル伝達経路において、少なくとも1つのプロテインキナーゼを阻害するか、少なくとも1つのプロテインキナーゼの少なくとも1つの基質と相互作用し、そのプロテインキナーゼカスケードのシグナル伝達経路の活性を抑制させ得る。
抑制物質は、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制(低下又は消失)させる物質であれば特に限定されず、例えば、抗体等のタンパク質、ペプチド、ペプチド模倣物、核酸、炭水化物、脂質、又は低分子化合物等であってよい。
本明細書中、「低分子化合物」とは、タンパク質、ペプチド、ペプチド模倣物、核酸、炭水化物、及び脂質以外の化合物を意味し、例えば分子量が1000以下、900以下、800以下、又は700以下の化合物を意味する。
HAMの治療又は予防剤は、抑制物質を1種単独で含んでいてもよく、2種以上を組み合わせて含んでいてもよい。
本明細書中、「低分子化合物」とは、タンパク質、ペプチド、ペプチド模倣物、核酸、炭水化物、及び脂質以外の化合物を意味し、例えば分子量が1000以下、900以下、800以下、又は700以下の化合物を意味する。

MAP3K8遺伝子発現抑制剤は、MAP3K8遺伝子の発現に関与し、MAP3K8の発現を抑制する物質であれば特に限定されない。MAP3K8遺伝子発現抑制剤は、MAP3K8遺伝子のsiRNA(short interfering RNA)、shRNA(short hairpin RNA)、及びアンチセンスオリゴヌクレオチドであってよく、MAP3K8遺伝子の発現に関与する遺伝子のsiRNA、shRNA、及びアンチセンスオリゴヌクレオチドであってもよい。
MAP3K8遺伝子発現抑制剤が発現を抑制する遺伝子としては、例えばヒトMAP3K8遺伝子等が挙げられ、より具体的にはNCBIデータベースに保存されるヒトMAP3K8遺伝子が挙げられるが、これに限定されるものではない。種々の生物由来のMAP3K8遺伝子の塩基配列の情報は、GenBank等のデータベースから取得することができる。ヒトMAP3K8のアミノ酸配列及びヒトMAP3K8遺伝子の塩基配列の一例を配列番号1及び2にそれぞれ示す。
MAP3K8遺伝子発現抑制剤は、MAP3K8遺伝子の転写、翻訳、又はその両方を阻害することで遺伝子発現を抑制してよい。

MAP3K8遺伝子発現抑制剤は、MAP3K8遺伝子の転写、翻訳、又はその両方を阻害することで遺伝子発現を抑制してよい。
siRNAとは、標的であるMAP3K8遺伝子の発現を抑制することができる二本鎖RNAである。siRNAにおける塩基配列の長さ(塩基長)は特に限定されないが、好ましくは約30塩基未満、より好ましくは約19~27塩基、さらに好ましくは約21~25塩基である。
shRNAとは、一本鎖RNA中に部分的に回文状の塩基配列を含むことにより、分子内に二本鎖構造及び3’末端の突出部を有する、短いヘアピン構造からからなる約20塩基対以上の分子である。shRNAは、細胞内に導入された後、細胞内で約20塩基の長さに分解され、siRNAと同様に標的であるMAP3K8遺伝子の発現を抑制し得る。
siRNA及びshRNAは、人工的に化学合成してもよい。また、siRNA及びshRNAは、例えば、T7RNAポリメラーゼ及びT7プロモーターを用いて、鋳型DNAからアンチセンス及びセンスのRNAをインビトロで合成してもよい。
shRNAとは、一本鎖RNA中に部分的に回文状の塩基配列を含むことにより、分子内に二本鎖構造及び3’末端の突出部を有する、短いヘアピン構造からからなる約20塩基対以上の分子である。shRNAは、細胞内に導入された後、細胞内で約20塩基の長さに分解され、siRNAと同様に標的であるMAP3K8遺伝子の発現を抑制し得る。
siRNA及びshRNAは、人工的に化学合成してもよい。また、siRNA及びshRNAは、例えば、T7RNAポリメラーゼ及びT7プロモーターを用いて、鋳型DNAからアンチセンス及びセンスのRNAをインビトロで合成してもよい。
アンチセンスオリゴヌクレオチドは、MAP3K8遺伝子のDNA配列中の連続する約30塩基未満の塩基配列に対して相補的な、又はハイブリダイズするヌクレオチドであればよく、DNA又はRNAのいずれであってもよい。また、機能に支障がない限り修飾されたものであってもよい。アンチセンスオリゴヌクレオチドは常法によって合成することができ、例えば、市販のDNA合成装置によって容易に合成することができる。
抑制物質は、MEK1/2阻害剤であってもよい。
MEK1/2阻害剤は、MEK1/2タンパク質のリン酸化活性を抑制させる物質であれば特に限定されない。ここで、MEK1/2阻害剤は、MEK1及びMEK2の阻害剤であってよく、MEK1の阻害剤であってもよく、MEK2の阻害剤であってもよい。MEK1/2阻害剤は、例えばMEK1及び/又は2に結合する物質であってよく、MEK1及び/又は2の基質に結合してMEK1及び/又は2によるリン酸化を抑制する物質であってもよく、p-MEK1及び/又は2を脱リン酸化して不活性化させる物質であってもよい。MEK1/2阻害剤は、MEK1/2及びMEK1/2の基質のいずれかを認識する物質(例えば低分子化合物又は抗体)であってよい。

抑制物質は、細胞透過性を有することが好ましい。したがって、抑制物質は、上記の例示の中でも、低分子化合物のものが好ましい。あるいは、上記で例示した抗体等に細胞透過性を付与したものも好ましい。抑制物質は、分子量が700以下の低分子化合物であってよく、分子量が700以下であり、芳香環を2つ以上有し、フッ素原子を少なくとも1つ有する化合物であってよい。
抑制物質が、MAP3K8が関与するMAPKシグナル伝達経路中、上流のプロテインキナーゼの活性を抑制する物質であることも好ましい。抑制物質の好ましい態様としては、例えばMAP3K8阻害剤、MAP3K8遺伝子発現抑制剤、及びMEK1/2阻害剤が挙げられる。抑制物質のより好ましい態様としては、MEK1/2阻害剤が挙げられ、更に好ましくは低分子化合物(例えば分子量が700以下)であるMEK1/2阻害剤が挙げられる。
抑制物質は、トラメチニブ、ビニメチニブ、セルメチニブ、及びコビメチニブ、並びにMEK1/2阻害活性を有するそれらの誘導体からなる群より選択される少なくとも1種であってもよい。

抑制物質が抗体である場合、抗体は、MAP3K8が関与するMAPKシグナル伝達経路中のプロテインキナーゼ等に結合してその活性を阻害する抗体であってよく、プロテインキナーゼに結合することによりプロテインキナーゼがその基質に結合できないようにする抗体であってよい。
上記の抗体は、モノクローナル抗体であってよく、ポリクローナル抗体であってもよい。また、抗体は、IgG、IgM、IgA、IgD及びIgEのいずれのアイソタイプであってもよい。

上記の抗体は、例えば、マウス抗体、ヒト型CDR移植抗体、ヒト型キメラ抗体、ヒト化抗体、又は完全ヒト抗体であってよく、低分子抗体であってもよい。
ヒト型CDR移植抗体は、ヒト以外の動物の抗体のCDRを、ヒト抗体のCDRで置換した抗体である。ヒト型キメラ抗体は、ヒト以外の動物の抗体に由来する可変領域と、ヒト抗体に由来する定常領域からなる抗体である。また、ヒト化抗体とは、ヒト以外の動物の抗体において、安全性の高い一部の領域を残して、ヒトの抗体に由来する部分を組み込んだものをいい、ヒト型キメラ抗体、及びヒト型CDR移植抗体を包含する概念である。
低分子抗体とは、抗体の断片又は抗体の断片に任意の分子を結合させたものであって、もとの抗体と同一のエピトープを認識する抗体である。具体的には、VL、VH、CL及びCH1領域からなるFab;2つのFabがヒンジ領域でジスルフィド結合によって連結されているF(ab’)2;VL及びVHからなるFv;VL及びVHを人工のポリペプチドリンカーで連結した一本鎖抗体であるscFvのほか、sdFv、Diabody、sc(Fv)2が挙げられるが、これらに限定されない。
ヒト型CDR移植抗体は、ヒト以外の動物の抗体のCDRを、ヒト抗体のCDRで置換した抗体である。ヒト型キメラ抗体は、ヒト以外の動物の抗体に由来する可変領域と、ヒト抗体に由来する定常領域からなる抗体である。また、ヒト化抗体とは、ヒト以外の動物の抗体において、安全性の高い一部の領域を残して、ヒトの抗体に由来する部分を組み込んだものをいい、ヒト型キメラ抗体、及びヒト型CDR移植抗体を包含する概念である。
低分子抗体とは、抗体の断片又は抗体の断片に任意の分子を結合させたものであって、もとの抗体と同一のエピトープを認識する抗体である。具体的には、VL、VH、CL及びCH1領域からなるFab;2つのFabがヒンジ領域でジスルフィド結合によって連結されているF(ab’)2;VL及びVHからなるFv;VL及びVHを人工のポリペプチドリンカーで連結した一本鎖抗体であるscFvのほか、sdFv、Diabody、sc(Fv)2が挙げられるが、これらに限定されない。
抑制物質が低分子化合物である場合、低分子化合物は公知の方法で製造してもよく、市販のものを購入することにより入手してもよい。
抑制物質が抗体である場合、対象とするプロテインキナーゼ又はそのフラグメントを免疫原として用い、公知の方法で製造することができる。

抑制物質は、上記した物質の薬理学的に許容可能な塩であってもよい。薬理学的に許容可能な塩は、例えば、医薬として許容される塩基や酸との塩を含む。
薬理学的に許容可能な塩の非限定的な具体例としては、無機酸(塩酸、臭化水素酸、ヨウ化水素酸、硫酸、リン酸等)の付加塩、有機酸(p-トルエンスルホン酸、メタンスルホン酸、シュウ酸、p-ブロモフェニルスルホン酸、カルボン酸、コハク酸、クエン酸、安息香酸、酢酸等)の付加塩、無機塩基(水酸化アンモニウム又はアルカリ若しくはアルカリ土類金属水酸化物、炭酸塩、重炭酸塩等)の付加塩、及びアミノ酸の付加塩等が挙げられる。
薬理学的に許容可能な塩の非限定的な具体例としては、無機酸(塩酸、臭化水素酸、ヨウ化水素酸、硫酸、リン酸等)の付加塩、有機酸(p-トルエンスルホン酸、メタンスルホン酸、シュウ酸、p-ブロモフェニルスルホン酸、カルボン酸、コハク酸、クエン酸、安息香酸、酢酸等)の付加塩、無機塩基(水酸化アンモニウム又はアルカリ若しくはアルカリ土類金属水酸化物、炭酸塩、重炭酸塩等)の付加塩、及びアミノ酸の付加塩等が挙げられる。
HAMの治療又は予防剤は、上記の抑制物質を含んでいる限り特に限定されないが、さらに薬学的に許容される担体及び/又は添加剤を含んでいてもよく、製剤化されていてもよい。
製剤化の形態としては、例えば、錠剤、被覆錠剤、丸剤、散剤、顆粒剤、カプセル剤、液剤、懸濁剤、及び乳剤等の経口剤;並びに、注射剤、輸液、坐剤、軟膏、及びパッチ剤等の非経口剤が挙げられる。
担体又は添加剤の含有量については、医薬品分野において通常採用されている範囲に基づいて適宜設定すればよい。

上記の抑制物質が低分子化合物であるとき、当該低分子化合物は、薬学的に許容される担体と共に製剤化された経口剤(例えば、錠剤、被覆錠剤、丸剤、散剤、顆粒剤、カプセル剤、液剤、懸濁剤、又は乳剤)として、経口投与経路で投与されることが好ましい。
上記の抑制物質がMAP3K8、MEK1/2、及び/又はERK1/2のようなプロテインキナーゼを認識する抗体であるとき、当該抗体は、薬学的に許容される担体と共に製剤化された注射剤又は輸液として、非経口投与経路(例えば、静脈内、筋肉内、皮膚内、腹腔内、皮下又は局所)で投与されることが好ましい。
抗体を含む注射剤又は輸液は、溶液、懸濁液又は乳濁液として用いてもよい。用いる溶剤としては、例えば、注射用蒸留水、生理食塩水、ブドウ糖溶液、及び等張液(例えば、塩化ナトリウム、塩化カリウム、グリセリン、マンニトール、ソルビトール、ホウ酸、ホウ砂、プロピレングリコール等の溶液)等が挙げられる。これらの溶剤は1種単独で用いてもよく、2種以上を組み合わせて用いてもよい。
抗体を含む注射剤又は輸液は、溶液、懸濁液又は乳濁液として用いてもよい。用いる溶剤としては、例えば、注射用蒸留水、生理食塩水、ブドウ糖溶液、及び等張液(例えば、塩化ナトリウム、塩化カリウム、グリセリン、マンニトール、ソルビトール、ホウ酸、ホウ砂、プロピレングリコール等の溶液)等が挙げられる。これらの溶剤は1種単独で用いてもよく、2種以上を組み合わせて用いてもよい。

上記の抑制物質がMAP3K8遺伝子発現抑制剤であるとき、抑制物質は非ウイルスベクター又はウイルスベクターの形態で投与することもできる。
MAP3K8遺伝子発現抑制剤が非ウイルスベクター形態のとき、投与の方法としては、リポソームを用いて核酸分子を導入する方法(リポソーム法、HVJ-リポソーム法、カチオニックリポソーム法、リポフェクション法、リポフェクトアミン法等)、マイクロインジェクション法、及び遺伝子銃(Gene Gun)でキャリア(金属粒子)とともに核酸分子を細胞に移入する方法等が挙げられる。
MAP3K8遺伝子発現抑制剤がウイルスベクター形態のとき、組換えアデノウイルス、レトロウイルス等のウイルスベクターを利用することができる。無毒化したレトロウイルス、アデノウイルス、アデノ随伴ウイルス、ヘルペスウイルス、ワクシニアウイルス、ポックスウイルス、ポリオウイルス、シンドビスウイルス、センダイウイルス、及びSV40等のDNAウイルス又はRNAウイルスに、siRNA又はshRNAを発現するDNAを導入し、細胞又は組織にこの組換えウイルスを感染させることにより、細胞又は組織内に遺伝子を導入することができる。
MAP3K8遺伝子発現抑制剤が非ウイルスベクター形態のとき、投与の方法としては、リポソームを用いて核酸分子を導入する方法(リポソーム法、HVJ-リポソーム法、カチオニックリポソーム法、リポフェクション法、リポフェクトアミン法等)、マイクロインジェクション法、及び遺伝子銃(Gene Gun)でキャリア(金属粒子)とともに核酸分子を細胞に移入する方法等が挙げられる。
MAP3K8遺伝子発現抑制剤がウイルスベクター形態のとき、組換えアデノウイルス、レトロウイルス等のウイルスベクターを利用することができる。無毒化したレトロウイルス、アデノウイルス、アデノ随伴ウイルス、ヘルペスウイルス、ワクシニアウイルス、ポックスウイルス、ポリオウイルス、シンドビスウイルス、センダイウイルス、及びSV40等のDNAウイルス又はRNAウイルスに、siRNA又はshRNAを発現するDNAを導入し、細胞又は組織にこの組換えウイルスを感染させることにより、細胞又は組織内に遺伝子を導入することができる。

本実施形態の医薬製剤は、本実施形態のHAMの治療又は予防剤を含む。言い換えると、医薬製剤は、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させる上記いずれかの物質(抑制物質)を含む。
本実施形態の医薬製剤を、例えばヒト、並びにラット、マウス、ウサギ、ヒツジ、ブタ、ウシ、ネコ、イヌ、及びサル等の他の哺乳動物に対して、その有効量を投与することにより、HAMを予防又は治療することができる。
HAMの治療又は予防剤、あるいはそれらを含む医薬製剤の投与量は、目的、疾患の重篤度、患者の年齢、体重、性別、既往歴、有効成分の種類等を考慮して、適宜設定してもよい。
本実施形態のHAMの治療又は予防剤は、HAM患者に特異的に異常発現しているMAP3K8が引き起こす炎症性サイトカインの過剰産生を抑制し得る。したがって、HAMの治療又は予防剤は、HTLV-1感染細胞による慢性的な炎症ループの予防、改善、及び/又は進行の防止を提供し得る。本実施形態のHAMの治療又は予防剤は、HAMの病態若しくは症状の改善、HAMの進行の防止若しくは低減、及び/又はHTLV-1感染細胞キャリアのHAM発症の予防に有効であり得る。
[HAMの治療方法]
上述のとおり、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させることによりHAMを治療又は予防することができる。
本実施形態の一態様は、HAM患者に、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させる物質(抑制物質)を薬理学的有効量で投与することを含む、HAMの治療又は予防方法である。
上述のとおり、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させることによりHAMを治療又は予防することができる。
本実施形態の一態様は、HAM患者に、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させる物質(抑制物質)を薬理学的有効量で投与することを含む、HAMの治療又は予防方法である。
HAMの治療又は予防方法を適用する対象としては、ヒトであってよく、非ヒト哺乳類(例えば、ラット、マウス、ウサギ、ヒツジ、ブタ、ウシ、ネコ、イヌ、及びサル等)であってよい。

上記いずれの方法においても、MAP3K8が関与するMAPKシグナル伝達経路の活性化レベルの測定方法は、特に限定されないが、例えば対象の末梢血中のMAP3K8濃度を測定すること;対象の末梢血中のMEK1及び/又は2あるいはERK1及び/又は2の濃度を測定すること;対象の末梢血中のMEK1及び/又は2のリン酸化レベルを測定すること;対象の末梢血中のERK1及び/又は2のリン酸化レベルを測定すること;対象の末梢血中のMEK1及び/又は2あるいはERK1及び/又は2をコードするmRNAの発現レベルを測定すること等が挙げられる。これらのマーカーは末梢血からだけでなく、その他の方法(例えば髄液からの取得)により取得してもよい。
MAP3K8濃度は抗MAP3K8抗体を用いて測定してもよい。p-MEK1及び/又は2は、抗p-MEK1抗体及び/又は抗p-MEK2抗体を用いて測定してもよい。p-ERK1及び/又は2は、抗p-ERK1抗体及び/又は抗p-ERK2抗体を用いて測定してもよい。MEK1及び/又は2並びにERK1及び/又は2の濃度は、それぞれ抗MEK1抗体及び/又は抗MEK2抗体並びに抗ERK1抗体及び/又は抗ERK2抗体を用いて測定してもよい。mRNAの発現レベルは、定量的RT-PCR等の公知の方法を用いて測定してもよい。
HAMの活動性評価方法やHAMに罹患しているかどうかを診断する方法において、従来は髄液中のCXCL10やネオプテリンをマーカーとして用いている。本実施形態の一態様に係る上記の方法のいずれにおいても、MAP3K8が関与するMAPKシグナル伝達経路の活性化レベルは末梢血のマーカーを用いて測定することができるため、より簡便にHAMの活動性評価をすることができ、またより簡便にHAMに罹患しているかどうかを診断することができる。
本実施形態の更なる別の一態様は、上記いずれかの方法に用いるキットである。当該キットは、MAP3K8が関与するMAPKシグナル伝達経路の活性化レベルの測定のための抗体を含んでいてよい。当該キットは、抗MAP3K8抗体、抗p-MEK1抗体、抗p-MEK2抗体、抗p-ERK1抗体、及び/又は抗p-ERK2抗体を含んでいてもよい。
本明細書で述べた上記の実施形態のいずれにおいても、MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させる物質(抑制物質)は、本明細書において記載する態様、並びに本明細書において好ましい、より好ましい、及び更に好ましいとして記載する態様(これらの全ての態様を総称して、以下、「好ましい態様」という)のうちの任意の態様であり得る。また、好ましい態様の任意の組み合わせであってもよい。
以下、本発明を実施例及び比較例を用いてより具体的に説明する。本発明は、以下の実施例によって何ら限定されるものではない。
[HAMの原因因子の同定]
健常者、HTLV-1感染未発症者、HAM患者、及びATL患者の4群について、RNA-seq解析及びATAC-seq解析の統合解析を行った。
概要としては、図2に示すように、ATAC-seq解析によるHAM特異的なクロマチン構造異常の特定、RNA-seq解析によるHAM患者において過剰に発現している遺伝子の特定、及びRNA-seq解析によるHAM特異的に過剰発現している遺伝子の特定を介して、HAMの原因因子を推定した。
健常者、HTLV-1感染未発症者、HAM患者、及びATL患者の4群について、RNA-seq解析及びATAC-seq解析の統合解析を行った。
概要としては、図2に示すように、ATAC-seq解析によるHAM特異的なクロマチン構造異常の特定、RNA-seq解析によるHAM患者において過剰に発現している遺伝子の特定、及びRNA-seq解析によるHAM特異的に過剰発現している遺伝子の特定を介して、HAMの原因因子を推定した。
RNA-seq解析は、以下のようにして行った。まず、健常者、HTLV-1感染未発症者、HAM患者、及びATL患者のPBMC(末梢血単核球)から、フローサイトメトリーを用いてHTLV-1非感染T細胞(CD4陽性/CADM1陰性/CD7陽性細胞)及びHTLV-1感染T細胞(CD4陽性/CADM1陽性/CD7陽性細胞、もしくはCD4陽性/CADM1陽性/CD7陰性細胞)を分取した。これらのT細胞について、RNA-seq解析によって、全遺伝子発現データを取得した。RNA-seqデータから得られるリードカウントデータについて、総リード数と各転写産物長で補正したTPM値とを求め、比較解析を行った。
ATAC-seq解析は、以下のようにして行った。RNA-seq解析と同様に取得した細胞集団を対象に、ATAC-seq解析を行い、ゲノムワイドのオープンクロマチン構造解析を行なった。ATAC-seq解析はActive Motif社に依頼し、クロマチン調製、ATAC反応、及びシークエンスまで実施した。得られたデータから各サンプルのオープンクロマチン領域データを解析した。
上記の解析に用いた検体は以下のとおりである。
・正常T細胞 3例(Resting.CD4.)
・HTLV-1感染未発症者由来CD4陽性/CADM1陰性/CD7陽性T細胞 10例(AC_P)
・HTLV-1感染未発症者由来CD4陽性/CADM1陽性/CD7陽性HTLV-1感染T細胞 8例(AC_D)

以上の解析により、HAMの原因因子として、MAP3K8が推定された。
RNA-seq解析の一部を図3に示す。図3から、HAM患者由来細胞群(HAM_D)でMAP3K8が過剰に発現していることが明らかになった。図3中、TPM値がプロットされる。HAM_D群は、その他の全ての群と有意差をもってMAP3K8の発現量が上昇していた(P<0.05)。
RNA-seq解析の一部を図3に示す。図3から、HAM患者由来細胞群(HAM_D)でMAP3K8が過剰に発現していることが明らかになった。図3中、TPM値がプロットされる。HAM_D群は、その他の全ての群と有意差をもってMAP3K8の発現量が上昇していた(P<0.05)。
また、MAP3K8、IFNG、TBX21遺伝子座における各検体のオープンクロマチンデータとmRNA発現レベルを検討した結果を図4に示す。図4からは、MAP3K8遺伝子座において、HAM特異的にクロマチン構造が開くエンハンサー領域が存在し、HAMで特異的に過剰発現していることがわかる。
次に、複数のHAM患者において、HAM由来感染細胞における各遺伝子の発現レベルと、髄液中のCXCL10及びネオプテリンの濃度との相関を調べた。なお、CXCL10及びネオプテリンは、HAMの疾患活動性を評価するためのバイオマーカーとして用いられている。
図5に示すように、MAP3K8の発現レベルがHAMの疾患活動性と非常に強い相関を有することが示唆された。細胞の炎症状態と相関するIFNG及びTBX21の発現レベルもCXCL10及びネオプテリンの濃度と高い相関を示すことを考慮すると、MAP3K8の発現レベルと細胞の炎症状態とが相関することも示唆された。
図5に示すように、MAP3K8の発現レベルがHAMの疾患活動性と非常に強い相関を有することが示唆された。細胞の炎症状態と相関するIFNG及びTBX21の発現レベルもCXCL10及びネオプテリンの濃度と高い相関を示すことを考慮すると、MAP3K8の発現レベルと細胞の炎症状態とが相関することも示唆された。
[MEK1/2阻害剤の作用検討]
MAP3K8が関与するMAPKシグナル伝達経路の活性を抑制させる物質のHAM治療に対する有効性を確認するため、以下の実験を行った。

ウエスタンブロッティングの結果を図6に示す。トラメチニブによる細胞内のERK1/2のリン酸化レベルの抑制が認められた。抑制効果は10nMから検出された。

また、HAM由来感染細胞株HCT-4を1×105細胞ずつ12 well plateに播種し、トラメチニブを終濃度1,000nMで加え、37℃、5%CO2条件下で0、24、48、72、及び96時間培養した。各培養時間の細胞から全RNAを抽出し、IFNG、及びCXCL10遺伝子のmRNAレベルを定量的RT-PCRで測定した。β-actin遺伝子mRNAも同様に測定し、内部標準として使用した。
独立した3回の定量的RT-PCR実験の結果(平均値)及びその標準偏差を図7に示す。トラメチニブ処理により、HAM由来感染細胞株におけるIFNG及びCXCL10の発現が有意に抑制された。抑制効果は24時間から検出された。

また、終濃度1,000nMのMEK1/2阻害剤(トラメチニブ、又はビニメチニブ)の存在下、37℃、5%CO2条件下で24時間前処理培養したHAM由来感染細胞株HCT-4をPBSで洗浄した。このHCT-4を、10% FBS、100U/ml Recombinant Human IL-2を含む新たなRPMI1640培地に懸濁し、3×105細胞ずつ12 well plateに播種したのち、MEK1/2阻害剤(トラメチニブ、又はビニメチニブ)を終濃度1,000nMの条件で加え、37℃、5%CO2条件下で24時間培養した。その後、培養上清中に産生されたIFN-γ量をBD(商標)Cytometric Bead Arrayを用いて測定した。

HAM由来感染細胞株HCT-4を1×105細胞ずつ12 well plateに播種し、トラメチニブを終濃度1,000nMの条件で加え、37℃、5%CO2条件下で24時間培養した。各培養時間の細胞から全RNAを抽出し、RNA-seq解析によって、全遺伝子発現データを取得した。RNA-seq解析データから得られるリードカウントデータについて、総リード数と各転写産物長で補正したTPM値とを求め、比較解析を行った。
独立した3回のRNA-seq解析の結果から、MEK1/2阻害剤処理による減少レベルが統計的に有意(P<0.05)と認められた452遺伝子について、Gene ontology解析を行った結果を図10上に示す。MEK1/2阻害剤処理により、免疫応答などに関連する遺伝子の発現抑制が認められた。
またMEK1/2阻害剤処理による減少レベルを図10下に示す。HAMで過剰に発現することが知られているIFNG及びCCR4等の遺伝子発現の減少が検出された。

ヒトCD4陽性T細胞白血病細胞株Jurkatに対し、MAP3K8をコードするcDNAを挿入するレンチウイルスベクターを導入し、MAP3K8を安定的に発現するT細胞株を樹立した。コントロール細胞(LV-Empty)及びMAP3K8発現細胞(LV-MAP3K8)に対し、トラメチニブを終濃度1,000nMの条件で加え、37℃、5%CO2条件下で24時間培養した。その後、全タンパク質を抽出し、細胞内のリン酸化MEK1/2レベル(p-MEK1/2)、及びリン酸化ERK1/2レベル(p-ERK1/2)をウエスタンブロッティングで評価した。発現させたMAP3K8は抗HA抗体、及びMAP3K8特異的抗体を用いて検出した。細胞内コントロールとして、全MEK1/2及び全ERK1/2の発現レベルも評価した。
ウエスタンブロッティングの結果を図11左に示す。MAP3K8の発現誘導により、MEK1/2及びERK1/2のリン酸化レベルの増強が認められた。またMEK1/2阻害剤処理によって、誘導されたERK1/2のリン酸化レベルが抑制されることもわかった。
同細胞のIFNG発現レベルを定量的RT-PCRで評価した結果を図11右に示す。
なお、MAP3K8遺伝子のベクターは、市販のMAP3K8発現ベクター(pCLXSN-Tpl2/Cot(addgene))のN末端にHAタグをつけた配列をCSII-EF-MCS-IRES2-Venusベクター(RIKEN BRC)にサブクローニングすることにより作製した。

MEK1/2阻害剤としては、ビニメチニブ(小野薬品工業、Bini)、コビメチニブ(ロシュ、Cobi)、セルメチニブ(アストラゼネカ、Selum)、及びトラメチニブ(ノバルティスファーマ、Tram)を用いた。ビニメチニブは終濃度が1、10、100又は1000nMとなるように添加し、コビメチニブ、セルメチニブ、及びトラメチニブは終濃度が100nMとなるように添加した。
MEK1/2阻害剤に代えて、同量のDMSOを添加した群(DMSO)、及び終濃度100nMのB-Raf阻害剤として知られるベムラフェニブ(Cayman Chemical、Vemur)を添加した群をネガティブコントロールとした。また、MEK1/2阻害剤に代えて、終濃度1μg/mLのステロイド治療薬として知られるプレドニゾロン(LKT Laboratories, Inc., St. Paul, MN、PSL)を添加した群をポジティブコントロールとした。
(自発的増殖活性へのMEK1/2阻害剤の作用)
8例のHAM患者PBMCを培地(10% FBSを含むRPMI1640培地)に懸濁し、1×105細胞ずつ96 well round bottom plateに播種し、MEK1/2阻害剤を加え合計0.2mLの培養液において37℃、5%CO2条件下で培養した。
MEK1/2阻害剤としては、ビニメチニブ(小野薬品工業、Bini)、コビメチニブ(ロシュ、Cobi)、セルメチニブ(アストラゼネカ、Selum)、及びトラメチニブ(ノバルティスファーマ、Tram)を用いた。ビニメチニブは終濃度が1、10、100又は1000nMとなるように添加し、コビメチニブ、セルメチニブ、及びトラメチニブは終濃度が100nMとなるように添加した。

培養開始から6日後、各ウェルに1μCi 3H-Thymidineを添加し、37℃、5%CO2条件下で16時間培養を行った。その後、培養細胞をセルハーベスター(Tomtec MH3 PerkinElmer)を用いてガラスフィルター(Printed Filtermat A PerkinElmer)に吸着させ、乾燥させた。次いで、固体シンチレータMeltilex-A(PerkinElmer)を染み込ませ、MicroBeta (WALLAC MicroBeta TriLux 1450-021)を用いて細胞に取り込まれた3H-Thymidine量の測定を行った。各HAM患者PBMCのDMSO群における3H-Thymidineのカウントの平均を100%として、各群の相対値を算出し、HAM患者8例の3H-Thymidine取り込み率の平均値を求めた。その結果、PSLと同様に全てのMEK1/2阻害剤がHAM-PBMCの自発的増殖活性を抑制する効果を示した。結果を図12に示す。
(CXCL10産生へのMEK1/2阻害剤の作用)
HAM患者PBMCによるCXCL10産生に対するMEK1/2阻害剤の作用について解析するため、8例のHAM患者PBMCを培地(10% FBSを含むRPMI1640培地)に懸濁し1×105細胞ずつ96 well round bottom plateに播種し、MEK1/2阻害剤を加え合計0.2mLの培養液において37℃、5%CO2条件下で培養した。
用いたMEK1/2阻害剤及びその濃度、並びにネガティブコントロール及びポジティブコントロールの条件は(自発的増殖活性へのMEK1/2阻害剤の作用)に記載の条件と同様とした。

用いたMEK1/2阻害剤及びその濃度、並びにネガティブコントロール及びポジティブコントロールの条件は(自発的増殖活性へのMEK1/2阻害剤の作用)に記載の条件と同様とした。
培養開始から7日後、培養液を遠心分離して培養上清のみを回収した。培養上清中のCXCL10濃度はCytokine Beads Array kit(BD Biosciences)を用いて、フローサイトメーターFACSCantoII(BD Biosciences)により測定した。
DMSO群の培養液中におけるCXCL10濃度を100%として、各群の相対値を算出し、HAM患者8例のCXCL10濃度の平均値を求めた。その結果、MEK1/2阻害剤はHAM-PBMCによるCXCL10産生に対して作用を示さなかった。結果を図13に示す。

[HAM-PBMCにおける各種細胞群に対するMEK1/2阻害剤の作用検討]
(実験手順)
HAM患者PBMCにおける各種細胞群に対するMEK1/2阻害剤の作用について解析するため、HTLV-1 Tax特異的細胞傷害性T細胞(CTL)を検出可能な7例のHAM患者PBMCを用いて実験をおこなった。各HAM患者のPBMCを培地(10% FBSを含むRPMI1640培地)に懸濁し、1×105細胞ずつ96 well round bottom plateに播種した。各ウェルにMEK1/2阻害剤を加え、合計0.2mLの培養液において37℃、5%CO2条件下で培養した。
MEK1/2阻害剤としては、1μMビニメチニブ、及び100nMトラメチニブを用いた(薬剤の濃度は通常の投与量における血中濃度を参照して決定した。)。MEK1/2阻害剤に代えて、同量のDMSOを添加した群(DMSO)をネガティブコントロールとした。また、MEK1/2阻害剤に代えて、終濃度1μg/mL PSLを添加した群をポジティブコントロールとした。
(実験手順)
HAM患者PBMCにおける各種細胞群に対するMEK1/2阻害剤の作用について解析するため、HTLV-1 Tax特異的細胞傷害性T細胞(CTL)を検出可能な7例のHAM患者PBMCを用いて実験をおこなった。各HAM患者のPBMCを培地(10% FBSを含むRPMI1640培地)に懸濁し、1×105細胞ずつ96 well round bottom plateに播種した。各ウェルにMEK1/2阻害剤を加え、合計0.2mLの培養液において37℃、5%CO2条件下で培養した。
MEK1/2阻害剤としては、1μMビニメチニブ、及び100nMトラメチニブを用いた(薬剤の濃度は通常の投与量における血中濃度を参照して決定した。)。MEK1/2阻害剤に代えて、同量のDMSOを添加した群(DMSO)をネガティブコントロールとした。また、MEK1/2阻害剤に代えて、終濃度1μg/mL PSLを添加した群をポジティブコントロールとした。
培養開始から7日後、PBMCをCD3抗体-BV510(Biolegend)、CD4抗体-APC(eBioscience)、CD8抗体-PECy7(eBioscience)、CCR4抗体-BV421(Biolegend)、又はHLA-A*24:02 HTLV-1 Tax 301-309 Tetramer抗体-PE(MBL)で4℃、30分間染色した。染色後、FACSバッファー(0.5% BSA、0.1%アジ化ナトリウム添加PBS)で洗浄し、70%エタノールに懸濁して-20℃で90分間透過化処理を行った。透過化処理後のPBMCを2回洗浄し、増殖細胞をKi67抗体-FITC(BD)を用いて染色した(室温、30分間)。その後、PBMCを洗浄後にFACSバッファーに懸濁し、フローサイトメーターCelesta(BD)を用いて解析を行った。
(HAM-PBMCに対するMEK1/2阻害剤の作用検討)
HAM患者7例のHAM-PBMC中のKi67+細胞の割合の平均値を求めた。その結果、ビニメチニブ、トラメチニブ又はPSLを添加した場合において、DMSOを添加した場合と比較して増殖細胞の減少が認められた(図14)。
また、解析に用いたHAM-PBMCには、PSL添加により増殖細胞の減少が認められるPBMC(PSL応答(+))と増殖細胞の減少が認められないPBMC(PSL応答(-))が存在した。他方、ビニメチニブ及びトラメチニブはPSL応答(+)、及びPSL応答(-)のどちらのPBMCに対しても増殖細胞を減少する作用を示した(図15)。解析を行った7例のHAM-PBMCにおいてPSL応答(+)は3例、PSL応答(-)は4例であったが、ビニメチニブ及びトラメチニブは7例全てのPBMCに対して増殖細胞を減少する作用を示した(図16)。
HAM患者7例のHAM-PBMC中のKi67+細胞の割合の平均値を求めた。その結果、ビニメチニブ、トラメチニブ又はPSLを添加した場合において、DMSOを添加した場合と比較して増殖細胞の減少が認められた(図14)。
また、解析に用いたHAM-PBMCには、PSL添加により増殖細胞の減少が認められるPBMC(PSL応答(+))と増殖細胞の減少が認められないPBMC(PSL応答(-))が存在した。他方、ビニメチニブ及びトラメチニブはPSL応答(+)、及びPSL応答(-)のどちらのPBMCに対しても増殖細胞を減少する作用を示した(図15)。解析を行った7例のHAM-PBMCにおいてPSL応答(+)は3例、PSL応答(-)は4例であったが、ビニメチニブ及びトラメチニブは7例全てのPBMCに対して増殖細胞を減少する作用を示した(図16)。
(HAMのCD4+T細胞に対するMEK1/2阻害剤の作用検討)
HAM患者7例においてHTLV-1感染細胞を主に含む細胞群であるCD4+T細胞及びCD4+CCR4+T細胞における増殖細胞に対するMEK1/2阻害剤の作用を解析した。HAM患者7例のCD3+CD4+細胞におけるKi67+細胞の割合の平均値を求めた結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合と比較して増殖細胞の減少が認められた(図17)。また、HAM患者7例のCD3+CD4+CCR4+細胞におけるKi67+細胞の割合の平均値を求めた結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合と比較して増殖細胞の減少が認められた(図18)。
HAM患者7例においてHTLV-1感染細胞を主に含む細胞群であるCD4+T細胞及びCD4+CCR4+T細胞における増殖細胞に対するMEK1/2阻害剤の作用を解析した。HAM患者7例のCD3+CD4+細胞におけるKi67+細胞の割合の平均値を求めた結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合と比較して増殖細胞の減少が認められた(図17)。また、HAM患者7例のCD3+CD4+CCR4+細胞におけるKi67+細胞の割合の平均値を求めた結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合と比較して増殖細胞の減少が認められた(図18)。
(HAMのCD8+T細胞に対するMEK1/2阻害剤の作用検討)
CD8+T細胞は、HTLV-1感染細胞中のウイルス発現に応答し、増殖すると共に炎症性サイトカインを産生しHAMの過剰免疫応答を引き起こす細胞群である。このCD8+T細胞の増殖に対するMEK1/2阻害剤の作用を解析した。HAM患者7例のCD3+CD8+細胞におけるKi67+細胞の割合の平均値を求めた結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合に対して増殖細胞の減少が認められた(図19)。
また、HTLV-1より発現するTaxに特異的な細胞傷害性T細胞(CTL)に対するMEK1/2阻害剤の作用を検討するため、HAM-PBMC中のCD3+CD8+HLA-A*24:02 HTLV-1 Tax 301-309 Tetramer+細胞の割合変化を解析した。その結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合と比較してTax特異的なCTLの減少が認められた(図20)。
CD8+T細胞は、HTLV-1感染細胞中のウイルス発現に応答し、増殖すると共に炎症性サイトカインを産生しHAMの過剰免疫応答を引き起こす細胞群である。このCD8+T細胞の増殖に対するMEK1/2阻害剤の作用を解析した。HAM患者7例のCD3+CD8+細胞におけるKi67+細胞の割合の平均値を求めた結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合に対して増殖細胞の減少が認められた(図19)。
また、HTLV-1より発現するTaxに特異的な細胞傷害性T細胞(CTL)に対するMEK1/2阻害剤の作用を検討するため、HAM-PBMC中のCD3+CD8+HLA-A*24:02 HTLV-1 Tax 301-309 Tetramer+細胞の割合変化を解析した。その結果、ビニメチニブ又はトラメチニブを添加した場合において、DMSOを添加した場合と比較してTax特異的なCTLの減少が認められた(図20)。
Claims (15)
- マイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性を抑制させる物質を含む、HTLV-1関連脊髄症(HAM)の治療又は予防剤。
- 前記物質が、MEK1/2阻害剤である、請求項1に記載のHAMの治療又は予防剤。
- 前記物質が、MAP3K8阻害剤又はMAP3K8遺伝子発現抑制剤である、請求項1に記載のHAMの治療又は予防剤。
- 前記物質が、ERK1/2阻害剤である、請求項1に記載のHAMの治療又は予防剤。
- 対象におけるマイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性化レベルを測定することを含む、前記対象におけるHTLV-1関連脊髄症(HAM)の治療又は予防の有効性を判定する方法。
- 対象におけるマイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性化レベルを測定することを含む、前記対象におけるHTLV-1関連脊髄症(HAM)の活動性を評価する方法。
- 対象におけるマイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性化レベルを測定することを含む、前記対象がHTLV-1関連脊髄症(HAM)に罹患しているかどうかを診断する方法。
- 前記対象の末梢血中のMAP3K8濃度を測定することを含む、請求項5~7のいずれか1項に記載の方法。
- 前記対象の末梢血中のMEK1/2又はERK1/2のリン酸化レベルを測定することを含む、請求項5~7のいずれか1項に記載の方法。
- 抗MAP3K8抗体を含む、請求項8に記載の方法のためのキット。
- 抗リン酸化MEK1/2抗体又は抗リン酸化ERK1/2抗体を含む、請求項9に記載の方法のためのキット。
- HTLV-1関連脊髄症(HAM)患者に、マイトジェン活性化プロテインキナーゼキナーゼキナーゼ8(MAP3K8)が関与するMAPKシグナル伝達経路の活性を抑制させる物質を薬理学的有効量投与することを含む、HAMの治療方法。
- 前記物質が、MEK1/2阻害剤である、請求項12に記載のHAMの治療方法。
- 前記物質が、MAP3K8阻害剤又はMAP3K8遺伝子発現抑制剤である、請求項12に記載のHAMの治療方法。
- 前記物質が、ERK1/2阻害剤である、請求項12に記載のHAMの治療方法。
