コロナウイルス感染症の重症化リスクの評価方法であって、(a)対象由来のT細胞を、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、(b)前記タンパク質またはその断片に対する前記T細胞の応答性に基づいて、前記コロナウイルス感染症の重症化リスクを評価する工程と、を含む、コロナウイルス感染症の重症化リスクの評価方法。また、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片を含む、コロナウイルス感染症の重症化リスク評価キット。

Description

コロナウイルス感染症の重症化リスクの評価方法、およびコロナウイルス感染症の重症化リスク評価キット
 本発明は、コロナウイルス感染症の重症化リスクの評価方法、およびコロナウイルス感染症の重症化リスク評価キットに関する。
 本願は、2021年7月29日に、日本に出願された特願2021-124390号に基づき優先権を主張し、その内容をここに援用する。
 COVID-19は、SARS-CoV-2を原因ウイルスとするウイルス感染症である。COVID-19症例のうち、重症例は、最大約20%(中等症~重症が14%、重篤が5%、死亡が2%)である。高齢者、ならびに高血圧、糖尿病、心血管疾患、慢性呼吸器疾患および癌などの基礎疾患を有する症例等で、重症化リスクが高く、概ね6~14%が重症化するとされている。COVID-19流行地域では、重症患者、高齢者および基礎疾患を有する患者が、それぞれICUおよび一般病床を占拠し、医療崩壊を招いている。
 最近、SARS-CoV-2特異的なCD8陽性T細胞およびCD4陽性T細胞が、それぞれ70%および100%のCOVID-19回復期患者において、検出されたことが報告されている(非特許文献1)。非特許文献1では、非感染者の約40~60%において、SARS-CoV-2と反応するCD4陽性T細胞が検出されたことも報告されている。

 COVID-19が重症化しやすい者と重症化しにくい者とを事前に弁別できれば、積極的で効率的な医療および行政の介入を行うことが可能である。例えば、感染初期に重症化リスクを判定し、積極的な治療的介入を行うことで、重症化を抑制できる可能性がある。また、ワクチン普及後も、免疫能の維持を定期的に確認することで、医療と経済のバランスを図る指標となり得る。現在は、年齢および基礎疾患の有無で重症化リスクが判断されるのみであり、COVID-19の重症化リスクを評価できる検査法は存在しない。そのため、COVID-19の重症化リスクを簡易に判定可能な検査法の開発が求められている。
 ワクチンの普及によりCOVID-19の流行が克服された場合でも、将来、新たなコロナウイルス感染症が蔓延する可能性がある。そのような新規コロナウイルス感染症に対しても、重症化しやすい者と重症化しにくい者とを事前に弁別できれば、効率的な医療および行政の介入が可能になる。
 そこで、本発明は、コロナウイルス感染症の重症化リスクを簡易に判定可能な、コロナウイルス感染症の重症化リスクの評価方法、およびコロナウイルス感染症の重症化リスク評価キットを提供することを目的とする。
 本発明は以下の態様を含む。
[1]コロナウイルス感染症の重症化リスクの評価方法であって、(a)対象由来のT細胞を、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、(b)前記タンパク質またはその断片に対する前記T細胞の応答性に基づいて、前記コロナウイルス感染症の重症化リスクを評価する工程と、を含む、コロナウイルス感染症の重症化リスクの評価方法。
[2]前記タンパク質またはその断片が、前記原因ウイルスが受容体とする分子とは異なる分子を受容体とする第1の他種コロナウイルスに由来し、前記(b)の工程が、前記タンパク質またはその断片に対する前記T細胞の応答性が低い場合に、前記コロナウイルス感染症の重症化リスクが高いと評価する工程である、[1]に記載のコロナウイルス感染症の重症化リスクの評価方法。
[3]前記タンパク質またはその断片が、前記原因ウイルスが受容体とする分子と同じ分子を受容体とする第2の他種コロナウイルスに由来し、前記(b)の工程が、前記タンパク質またはその断片に対する前記T細胞の応答性が高い場合に、前記コロナウイルス感染症の重症化リスクが高いと評価する工程である、[1]に記載のコロナウイルス感染症の重症化リスクの評価方法。
[4]前記工程(a)が、(a1)前記T細胞を、前記原因ウイルスが受容体とする分子とは異なる分子を受容体とする第1の他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、(a2)前記T細胞を、前記原因ウイルスが受容体とする分子と同じ分子を受容体とする第2の他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、を含み、前記(b)の工程が、(b1)前記第1の他種コロナウイルスに由来するタンパク質またはその断片に対する前記T細胞の第1の応答性を測定する工程と、(b2)前記第2の他種コロナウイルスに由来するタンパク質またはその断片に対する前記T細胞の第2の応答性を測定する工程と、(b3)前記第1の応答性に対する前記第2の応答性の比の値(第2の応答性/第1の応答性)に基づいて、前記コロナウイルス感染症の重症化リスクを評価する工程と、を含む、[1]に記載のコロナウイルス感染症の重症化リスクの評価方法。
[5]前記原因ウイルスおよび前記タンパク質またはその断片が由来する前記コロナウイルスが、ベータコロナウイルス属またはアルファコロナウイルス属である、[1]~[4]のいずれか1つに記載のコロナウイルス感染症の重症化リスクの評価方法。
[6]前記原因ウイルスがSARS-CoV-2である、[1]~[5]のいずれか1つに記載のコロナウイルス感染症の重症化リスクの評価方法。
[7]コロナウイルス感染症の重症化リスク評価キットであって、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片を含む、コロナウイルス感染症の重症化リスク評価キット。
[8]T細胞が抗原刺激に応答して分泌するサイトカインに対する特異的結合物質をさらに含む、[7]に記載のコロナウイルス感染症の重症化リスク評価キット。
[9]固相担体に結合された前記特異的結合物質と、標識物質で標識された前記特異的結合物質とを含む、[8]に記載のコロナウイルス感染症の重症化リスク評価キット。
[10]前記サイトカインがインターフェロンγ、インターロイキン2、及び腫瘍壊死因子アルファからなる群より選択される少なくとも1種である、[8]または[9]に記載のコロナウイルス感染症の重症化リスク評価キット。

 本発明によれば、コロナウイルス感染症の重症化リスクを簡易に判定可能な、コロナウイルス感染症の重症化リスクの評価方法、およびコロナウイルス感染症の重症化リスク評価キットが提供される。

[定義]
 「を含む」(comprise)という用語は、対象となる構成要素以外の構成要素を含んでいてもよいことを意味する。「からなる」(consist of)という用語は、対象となる構成要素以外の構成要素を含まないことを意味する。「から本質的になる」(consist essentially of)という用語は、対象となる構成要素以外の構成要素を特別な機能を発揮する態様(発明の効果を完全に喪失させる態様など)では含まないことを意味する。本明細書において、「を含む」(comprise)と記載する場合、「からなる」(consist of)態様、及び「から本質的になる」(consist essentially of)態様を包含する。
 タンパク質、ペプチド、及び細胞は、単離されたものであり得る。「単離された」とは、天然状態又は他の成分から分離された状態を意味する。「単離された」ものは、他の成分を実質的に含まないものであり得る。「他の成分を実質的に含まない」とは、単離された成分に含まれる他の成分の含有量が無視できる程度であることを意味する。単離された成分に含まれる他の成分の含有量は、例えば、10質量%以下、5質量%以下、4質量%以下、3質量%以下、2質量%以下、1質量%以下、0.5質量%以下、又は0.1質量%以下であり得る。本明細書に記載されるタンパク質、ペプチド、及び細胞は、単離されたタンパク質、単離されたペプチド、及び単離された細胞であり得る。
 「コロナウイルス」とは、コロナウイルス科(Coronaviridae)に属するウイルスをいう。コロナウイルス科のウイルスは、一本鎖プラス鎖RNAウイルスであり、エンベロープを有する。ウイルスの分類は、例えば、国際ウイルス分類委員会(International Committee on Taxonomy of Viruses:ICTV)の分類による。コロナウイルスは、ウイルス分類学上、ニドウイルス属コロナウイルス科に分類される。コロナウイルス科は、さらにコロナウイルス亜科(オルトコロナウイルス亜科)およびトロウイルス亜科に分けられる。コロナウイルス亜科は、アルファ(α)コロナウイルス属、ベータ(β)コロナウイルス属、ガンマ(γ)コロナウイルス属、およびデルタ(δ)コロナウイルス属の4つの属にさらに分類される。SARS-CoV-1、MERS-CoV、SARS-CoV-2は、ベータコロナウイルス属に分類される。4種類のヒト風邪コロナウイルスのなかでは、HCoV-HKU1およびHCoV-OC43が、ベータコロナウイルス属に分類される。残りのヒト風邪コロナウイルスであるHCoV-229EおよびHCoV-NL63は、アルファコロナウイルス属に分類される(El-Sayed A, et al. Environ Sci Pollut Res Int. 2021 Apr;28(16):19589-19600)。アミノ酸配列の相同性の比較では、SARS-CoV-2は、同じベータコロナウイルス属であるHCoV-HKU1とは75.7%、HCoV-OC43とは74.8%と高い相同性がある。これに対し、SARS-CoV-2は、アルファコロナウイルス属であるHCoV-229Eとは64.9%、HCoV-NL63とは56.3%と相同性が低い(Cueno ME, et al. Front Med (Lausanne). 2021 Jan 14;7:594439.)。相同性が高いと細胞性免疫学的に交差反応性が高くなることが期待できる。
 一実施形態において、コロナウイルスはヒトコロナウイルスである。「ヒトコロナウイルス」とは、ヒトに感染するコロナウイルスをいう。ヒトコロナウイルスとしては、アルファコロナウイルス属(Alphacoronavirus)のHCoV-229EおよびHCoV-NL63;ならびにベータコロナウイルス属(Betacoronavirus)のHCoV-HKU1、HCoV-OC43、SARS-CoV、MERS-CoV、およびSARS-CoV-2等が挙げられる。SARS-CoV-2は、アルファ株、ベータ株、ガンマ株、デルタ株、ラムダ株、ミュー株、イプシロン株、イータ株、イオタ株、カッパ株、およびオミクロン株等の既存の変異株、ならびに将来発生し得る変異株を包含する。
 「コロナウイルス感染症」とは、コロナウイルスの感染によって引き起こされる疾患をいう。例えば、HCoV-229E、HCoV-NL63、HCoV-HKU1、およびHCoV-OC43は、風邪を引き起こす。SARS-CoVは、重症急性呼吸器症候群(Severe acute respiratory syndrome;SARS)を引き起こす。MERS-CoVは、中東呼吸器症候群(Middle East respiratory syndrome;MERS)を引き起こす。SARS-CoV-2は、新型コロナウイルス感染症(COVID-19)を引き起こす。
 「コロナウイルス感染症の重症化リスク」とは、コロナウイルス感染症の症状が進行するリスクをいう。コロナウイルス感染症の重症化リスクは、例えば、対象がコロナウイルス感染者である場合、当該コロナウイルスの症状が進行し、医療上の積極的な介入が必要となる状況になる可能性の定性的指標となり得る。コロナウイルス感染症の重症化リスクは、例えば、対象がコロナウイルス非感染者である場合、前記対象がコロナウイルスに感染したときに当該コロナウイルスの症状が進行し、医療上の積極的な介入が必要となる状況になる可能性の定性的指標となり得る。コロナウイルス感染症の重症化リスクは、例えば、対象がコロナウイルスに感染した場合に、感染症の進行に伴う各種症状を発症するリスクであってもよい。コロナウイルス感染症の進行に伴って発症する症状としては、例えば、呼吸困難、肺炎、低酸素症、呼吸不全、急性呼吸窮迫症候群、急性肺障害、敗血症、および多臓器不全等が挙げられる。コロナウイルス感染症がCOVID-19である場合、重症化リスクは、中等症以上(例えば、新型コロナウイルス感染症(COVID-19)診療の手引き・第5版(厚生労働省)(https://www.mhlw.go.jp/content/000785119.pdf)による中等症I以上、中等症II以上または重症以上)となるリスクであってもよい。前記新型コロナウイルス感染症(COVID-19)診療の手引き・第5版によれば、COVID-19の重症度は、以下のように分類される。
 軽症:酸素飽和度(SpO2)≧96%で、呼吸器症状なし、または、咳のみで呼吸困難なし。いずれの場合であっても肺炎所見を認めない。
 中等症I(呼吸不全なし):93%<SpO2<96%で、呼吸困難,肺炎所見を認める。
 中等症II(呼吸不全あり):SpO2≦93%で、酸素投与が必要。
 重症:ICUに入室、または人工呼吸器が必要。
 重症化リスクは、対象がCOVID-19罹患後の治癒者である場合、SARS-CoV-2の変異株の出現により、将来、再びSARS-CoV-2に感染してCOVID-19を発症し、重症化するリスクであってもよい。
 重症化リスクは、対象がCOVID-19罹患後の治癒者である場合、対象の免疫記憶の低下により、将来、再びSARS-CoV-2に感染してCOVID-19を発症し、重症化するリスクであってもよい。
 対象がSARS-CoV-2のPCR検査におけるPCR陽性の無症状者である場合、COVID-19の発症約1週後に重症化に至るか自然治癒するかのどちらかの経過をたどることが多い。この場合、重症化リスクは、COVID-19を発症して重症化に至るリスクであってもよい。
 重症化リスクは、対象がSARS-CoV-2のワクチン接種者である場合、ワクチン低感受性のSARS-CoV-2変異株の出現により、将来、再びSARS-CoV-2に感染してCOVID-19を発症し、重症化するリスクであってもよい。
 重症化リスクは、対象がSARS-CoV-2のワクチン接種者である場合、対象におけるワクチン効果の減弱とともに、将来、再びSARS-CoV-2に感染してCOVID-19を発症し、重症化するリスクであってもよい。
 「抗体」とは、抗原結合活性を有する免疫グロブリンを意味する。抗体は、抗原結合活性を有していれば、インタクトな抗体に限定されず、抗原結合断片であってもよい。本明細書において、「抗体」という用語は、抗原結合断片を包含する。「抗原結合断片」とは、抗体の一部を含むポリペプチドであって、元の抗体の抗原結合性を維持しているポリペプチドである。抗原結合断片は、元の抗体の6つの相補性決定領域(complementarity determining region:CDR)を全て含むものが好ましい。すなわち、重鎖可変領域のCDR1、CDR2、CDR3、並びに軽鎖可変領域のCDR1、CDR2、CDR3を全て含むことが好ましい。抗原結合断片としては、例えば、Fab、Fab’、F(ab’)、可変領域断片(Fv)、ジスルフィド結合Fv、一本鎖Fv(scFv)、sc(Fv)等が挙げられる。
 抗体は、いずれの生物に由来するものであってもよい。抗体が由来する生物としては、例えば、哺乳類(ヒト、マウス、ラット、ウサギ、ウマ、ウシ、ブタ、サル、イヌ等)、鳥類(ニワトリ、ダチョウ)等が挙げられるが、これらに限定されない。
 抗体は、免疫グロブリンのいずれのクラス及びサブクラスであってもよい。抗体は、モノクローナル抗体であってもよく、ポリクローナル抗体であってもよいが、モノクローナル抗体が好ましい。
 抗体は、免疫法、ハイブリドーマ法、ファージディスプレイ法等の公知の方法により作製することができる。
 「結合対」とは、互いに結合する2つの分子(1対の分子)をいう。結合対の「第1要素」とは、結合対を構成する1対の分子のうちの一方の分子をいう。結合対の「第2要素」とは、結合対を構成する1対の分子のうちの他方の分子をいう。結合対の例としては、例えば、ビオチン若しくはその誘導体及びアビジン若しくはその誘導体(ストレプトアビジン、ニュートラアビジン等)等が挙げられる。
 「受容体」とは、ウイルスが細胞に侵入する際に結合する分子をいう。受容体は、通常、ウイルス感染の対象となる宿主細胞の細胞表層に存在する。コロナウイルスは、スパイクタンパク質で宿主細胞の細胞表層に存在する受容体に結合し、宿主細胞に感染すると考えられている。アルファコロナウイルス及びベータコロナウイルスの主な種の受容体を表1に示す。表1中、APN1はaminopeptidase Nを表し、ACE2はangiotensin coverting enzyme 2を表し、DPP4はdipeptidyl peptidase-4を表す。

 HCoC-NL63及びSARS-CoVは、SARS-CoV-2と同じ分子(すなわち、ACE2)を受容体としている。
 HCoC-229E、HCoC-OC43、HCoC-HKU1、及びMERS-CoVは、SARS-CoV-2とは異なる分子を受容体としている。
[コロナウイルス感染症の重症化リスクの評価方法]
 本開示の第1の態様は、コロナウイルス感染症の重症化リスクの評価方法である。本態様にかかる方法は、下記(a)及び(b)の工程を含む。
 (a)対象由来のT細胞を、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程。
 (b)前記タンパク質又はその断片に対する前記T細胞の応答性に基づいて、前記コロナウイルス感染症の重症化リスクを評価する工程。
 一実施形態において、本態様にかかる評価方法は、ELISPOTアッセイの手法を用いて実施することができる。ELISPOTアッセイにおけるT細胞刺激抗原としては、重症化リスクの評価対象であるコロナウイルス感染症とは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片を用いることができる。本態様にかかる評価方法は、in vitroで行われる方法である。
<工程(a)>
 工程(a)では、対象由来のT細胞を、コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質又はその断片に接触させる。

 「対象」とは、コロナウイルス感染症の重症化リスクの評価対象となる個体をいう。対象は、コロナウイルス感染症に罹患する動物であれば、特に限定されない。対象としては、ヒト、ヒト以外の哺乳動物が挙げられる。ヒト以外の哺乳動物としては、例えば、サル、チンパンジー、ゴリラ、マーモセット等の霊長類;マウス、モルモット、ハムスター、ラット等のげっ歯類;イヌ、ネコ等の食肉目等が挙げられるが、これらに限定されない。一実施形態において、対象は、ヒトである。
 対象は、重症化リスク評価の対象となるコロナウイルス感染症(以下、「対象コロナウイルス感染症」という)の原因ウイルス(以下、単に「原因ウイルス」ともいう)の感染者であってもよく、非感染者であってもよい。「感染者」とは、原因コロナウイルスが検出される状態にある者をいう。「非感染者」とは、原因コロナウイルスが検出されない者をいう。コロナウイルスの検出は、対象から採取された生体試料(唾液、鼻腔ぬぐい液、鼻咽頭ぬぐい液等)を用いて、公知の方法(PCR法、抗原法等)により行うことができる。
 対象が、原因コロナウイルス感染者である場合、原因コロナウイルスの感染初期であることが好ましい。感染初期に重症化リスクを評価することにより、対象が、対象コロナウイルス感染症を発症し、その症状が重症化するかを予測することができる。その結果、適切な治療的介入を行うことができる。
 非感染者の重症化リスクを評価することにより、対象が原因ウイルスに感染した場合に、対象コロナウイルス感染症の症状が重症化するかを予測することができる。その結果、ワクチン接種等の適切な防御的介入を行うことができる。
 対象は、対象コロナウイルス感染症の発症者であってもよく、非発症者であってもよい。「発症者」とは、原因コロナウイルスに感染し、対象コロナウイルス感染症の1つ以上の症状を発症している者をいう。「非発症者」とは、原因コロナウイルスに感染し、対象コロナウイルス感染症の症状を発症していない者をいう。
 対象が、対象コロナウイルス感染症発症者である場合、コロナウイルス感染症の発症初期であることが好ましい。発症初期に重症化リスクを評価することにより、対象において、対象コロナウイルス感染症の症状が重症化するかを予測することができる。その結果、適切な治療的介入を行うことができる。
 対象が、対象コロナウイルス感染症の非発症者であって、原因ウイルス感染者である場合、重症化リスクを評価することにより、対象において、対象コロナウイルス感染症を発症し、その症状が重症化するかを予測することができる。その結果、適切な治療的介入を行うことができる。
 対象は、対象コロナウイルス感染症の既感染者であってもよく、未感染者であってもよい。「既感染者」とは、原因ウイルスに1度以上感染したことがある者であって、対象コロナウイルス感染症が治癒している者をいう。「未感染者」とは、原因ウイルスに1度も感染したことがない者をいう。
 既感染者の重症化リスクを評価することにより、対象が原因ウイルスに再度感染した場合に、対象コロナウイルス感染症を発症し、その症状が重症化するかを予測することができる。その結果、ワクチン接種等の適切な防御的介入を行うことができる。
 未感染者の重症化リスクを評価することにより、対象が原因ウイルスに感染した場合に、対象コロナウイルス感染症を発症し、その症状が重症化するかを予測することができる。その結果、ワクチン接種等の適切な防御的介入を行うことができる。
 対象は、対象コロナウイルス感染症のワクチン接種を受けた者(ワクチン接種者)であってもよく、ワクチン未接種者であってもよい。
 ワクチン接種者の重症化リスクを評価することにより、ワクチン接種により、対象コロナウイルス感染症の重症化を回避できる程度の免疫が獲得できたかを評価することができる。その結果、再度のワクチン接種等の適切な防御的介入を行うことができる。
 ワクチン未接種者の重症化リスクを評価することにより、対象が、ワクチン接種によらずに対象コロナウイルス感染症の重症化を回避できる程度の免疫を有しているかを評価することができる。その結果、ワクチン接種等の適切な防御的介入を行うことができる。
(対象のT細胞)
 対象のT細胞は、対象の血液から、公知の方法により、単離することができる。例えば、密度勾配遠心法により、血液から、T細胞を含む末梢血単核球(Peripheral Blood Mononuclear Cells;PBMC)画分を分離することができる。PBMCは、T細胞(CD8陽性T細胞、CD4陽性T細胞等)のほか、樹状細胞、単球・マクロファージ、B細胞等の抗原提示細胞を含んでいる。工程(a)では、対象のT細胞を含む細胞集団として、PBMCを用いてもよい。
 あるいは、T細胞マーカー(CD8、CD4等)に特異的に結合する抗体等を用いて、PBMCからT細胞を単離してもよい。一実施形態において、T細胞は、CD8陽性T細胞およびCD4陽性T細胞からなる群より選択される少なくとも1種を含む。一実施形態において、T細胞は、CD8陽性T細胞およびCD4陽性T細胞の両方を含む。
(他種コロナウイルスに由来するタンパク質またはその断片)
 「他種コロナウイルス」は、原因ウイルスとは異なる種類のコロナウイルスである。他種コロナウイルス(以下、「抗原ウイルス」ともいう)は、コロナウイルス科に属するウイルスであって、原因ウイルスとは異なる種類のコロナウイルスであれば、特に限定されない。一実施形態において、抗原ウイルスは、原因ウイルスとは異なる種に属するコロナウイルスである。一実施形態において、抗原ウイルスは、原因ウイルスと同じ属に属し、且つ原因ウイルスと異なる種に属するコロナウイルスである。例えば、原因ウイルスがベータコロナウイルス属である場合、抗原ウイルスはベータコロナウイルス属の異なる種であってもよい。抗原ウイルスはアルファコロナウイルス属であってもよい。抗原ウイルスは、対象に感染可能なコロナウイルスであることが好ましい。
 例えば、原因ウイルスがSARS-CoV-2である場合、抗原ウイルスとして、ベータコロナウイルス属のウイルスを用いることができる。例えば、原因ウイルスがSARS-CoV-2であり、対象がヒトである場合、抗原ウイルスとして、ベータコロナウイルス属のヒトコロナウイルス(HCoV-OC43、HCoV-HKU1等)を用いることができる。
 抗原ウイルスは、原因ウイルスが受容体とする分子とは異なる分子を受容体とするコロナウイルス(以下、「第1の抗原ウイルス」又は「第1の他種コロナウイルス」ともいう)であってもよい。例えば、原因ウイルスがSARS-CoV-2であり、対象がヒトである場合、抗原ウイルスとして、ACE2以外の分子を受容体とするコロナウイルスを用いることができる。そのようなコロナウイルスとしては、HCoV-229E、HCoV-OC43、HCoV-HKU1、MERS-CoV等が挙げられる。
 抗原ウイルスは、原因ウイルスが受容体とする分子と同じ分子を受容体とするコロナウイルス(以下、「第2の抗原ウイルス」又は「第2の他種コロナウイルス」ともいう)であってもよい。例えば、原因ウイルスがSARS-CoV-2であり、対象がヒトである場合、抗原ウイルスとして、ACE2を受容体とするコロナウイルスを用いることができる。そのようなコロナウイルスとしては、HCoV-NL63、SARS-CoV等が挙げられる。
 抗原ウイルスは、1種のみを用いてもよく、2種以上を用いてもよい。抗原ウイルスとしては、第1の抗原ウイルス及び第2の抗原ウイルスの両方を用いることが好ましい。
 第1の抗原ウイルスとしては、HCoV-229E、HCoV-OC43、HCoV-HKU1、及びMERS-CoVからなる群より選択される1種以上を用いることが好ましく、2種以上を用いることがより好ましく、3種以上を用いることがさらに好ましい。第1の抗原ウイルスとしては、HCoV-229E、HCoV-OC43、及びHCoV-HKU1の全てを用いることが好ましい。
 第2の抗原ウイルスとしては、HCoV-NL63、及びSARS-CoVからなる群より選択される1種以上を用いることが好ましい。第2の抗原ウイルスとしては、HCoV-NL63を用いることが好ましい。
 抗原ウイルスに由来するタンパク質またはその断片は、T細胞刺激抗原として用いられる。「抗原ウイルスに由来するタンパク質」(以下、「抗原ウイルスタンパク質」ともいう)とは、抗原ウイルスが有するタンパク質または抗原ウイルスのプロウイルスにより発現されるタンパク質をいう。抗原ウイルスタンパク質としては、例えば、スパイクタンパク質(S蛋白)、ヌクレオカプシドタンパク質(N蛋白)、膜タンパク質(M蛋白)、エンベロープタンパク質(E蛋白)、RNA依存性RNAポリメラーゼ(RdRp)等が挙げられる。
 抗原ウイルスタンパク質は、受容体に結合するタンパク質を用いることが好ましい。原因ウイルスと受容体が同じ抗原ウイルスに対する免疫記憶は、原因ウイルスの感染に際し、過剰な免疫応答を誘導し、原因ウイルスによるコロナウイルス感染症の重症化を招く可能性がある。コロナウイルスは、通常、スパイクタンパク質(S蛋白)により受容体に結合する。そのため、抗原ウイルスタンパク質としては、S蛋白を用いることが好ましい。
 抗原ウイルスタンパク質の断片は、抗原ウイルスタンパク質のアミノ酸配列の一部を有する限り特に限定されない。抗原ウイルスタンパク質の断片は、T細胞の活性化が可能な程度の大きさを有することが好ましい。例えば、抗原ウイルスタンパク質の断片は、8アミノ酸以上、9アミノ酸以上、又は10アミノ酸以上の抗原ウイルスタンパク質の断片であってもよい。一実施形態において、抗原ウイルスタンパク質の断片は、抗原ウイルスタンパク質のT細胞エピトープを含む。
 抗原ウイルスタンパク質は、抗原ウイルスに対する細胞性免疫を誘導できる限り、改変されたものであってもよい。例えば、抗原ウイルスタンパク質は、天然の抗原ウイルスが有するタンパク質のアミノ酸配列において、1個又は数個(例えば、2~20個、2~15個、2~10個、2~5個、2~4個、2個又は3個)のアミノ酸が置換、欠失、及び/又は付加されたアミノ酸配列を有するものであってもよい。
 抗原ウイルスタンパク質またはその断片は、抗原ウイルスタンパク質のアミノ酸配列に基づいて合成してもよい。あるいは、抗原ウイルスタンパク質またはその断片をコードするポリヌクレオチドを用いて、細胞または無細胞合成系により合成してもよい。抗原ウイルスタンパク質のアミノ酸配列または遺伝子配列は、GenBank等の配列データベースから取得することができる。例えば、HCoV-OC43のS蛋白のアミノ酸配列は、NCBI Reference Sequence:YP_009555241.1(配列番号1)又はGenBank No.AMK59677.1(配列番号2)で登録されているものを用いることができる。例えば、HCoV-HKU1のS蛋白のアミノ酸配列は、NCBI Reference Sequence:YP_173238.1(配列番号3)又はGenBank No.AYN64561.1(配列番号4)で登録されているものを用いることができる。例えば、HCoV-229EのS蛋白のアミノ酸配列は、NCBI Reference Sequence:NP_073551.1(配列番号5)で登録されているものを用いることができる。例えば、HCoV-NL63のS蛋白のアミノ酸配列は、NCBI Reference Sequence:YP_003767.1 (配列番号6)で登録されているものを用いることができる。
 抗原ウイルスタンパク質またはその断片は、市販のものを用いてもよい。例えば、HcoV-OC43のS蛋白、及びHcoV-HKU1のS蛋白は、Sino Biological Inc.等から市販されている。例えば、SARS-Cov-2のS蛋白、及びSARS-Cov-2のペプチドプールは、mabtech AB等から市販されている。
 抗原ウイルスタンパク質またはその断片は、抗原ウイルスタンパク質の全長タンパク質を用いることが好ましい。全長タンパク質を用いることにより、T細胞エピトープを含む可能性が高くなる。また、変異株にも対応しやすい。
(接触方法)
 対象由来のT細胞を、抗原ウイルスタンパク質またはその断片に接触させる方法は特に限定されない。例えば、ELISPOTアッセイで通常行われる方法を用いることができる。一実施形態において、T細胞と、抗原ウイルスタンパク質またはその断片との接触は、抗原提示細胞の存在下で行われる。抗原提示細胞としては、樹状細胞、B細胞、マクロファージ等が挙げられる。抗原提示細胞は、対象から採取されたものであることが好ましい。対象の血液から単離したPBMCは、T細胞及び抗原提示細胞を含む。そのため、抗原ウイルスタンパク質またはその断片との接触に、対象の血液から取得したPBMCを用いてもよい。
 抗原ウイルスタンパク質またはその断片は、抗原提示細胞に提示されたものであってもよい。抗原ウイルスタンパク質またはその断片を抗原提示細胞と培養すると、抗原ウイルスタンパク質またはその断片が抗原提示細胞に取り込まれ、MHC/ペプチド複合体として抗原提示細胞に提示される。このMHC/ペプチド複合体を提示する抗原提示細胞を、対象由来のT細胞との接触に用いてもよい。
 反応に用いる対象由来のT細胞の量は、特に限定されない。T細胞の量としては、例えば、10~10個、10~10個、10~10個、または10~10個等が挙げられる。PBMCを用いる場合、反応に用いるPBMCの細胞数としては、例えば、10~10個、10~10個、10~10個、または10~10個等が挙げられる。
 反応に用いる抗原ウイルスタンパク質またはその断片の量は、特に限定されない。抗原ウイルスタンパク質またはその断片の量としては、例えば、0.1μg/mL以上、0.5μg/mL以上、1μg/mL以上、2μg/mL以上、5μg/mL以上、または10μg/mL以上等が挙げられる。抗原ウイルスタンパク質またはその断片の上限値は特に限定されないが、例えば、100μg/mL以下、60μg/mL以下、50μg/mL以下、40μg/mL以下、30μg/mL以下、20μg/mL以下、または15μg/mL以下等が挙げられる。前記上限値および下限値は、任意に組合せ可能である。
 反応時間は、特に限定されず、抗原ウイルスタンパク質またはその断片によるT細胞の免疫応答が誘導される程度の時間であればよい。反応時間としては、例えば、1時間以上、2時間以上、5時間以上、7時間以上、10時間以上、15時間以上、または20時間以上等が挙げられる。反応時間の上限は、特に限定されないが、例えば、50時間以下、40時間以下、30時間以下、または25時間以下等が挙げられる。前記上限値および下限値は、任意に組合せ可能である。
 反応温度としては、例えば、20~40℃、25~40℃、または30~40℃が挙げられる。反応温度は、典型的には、37℃である。
 対象由来のT細胞と抗原ウイルスタンパク質またはその断片との接触は、ELISPOT用のウェルプレート等を用いて行ってもよい。ELISPOT用プレートは、市販のものを用いることができる。ELISPOT用のウェルプレートには、抗原刺激によりT細胞が分泌するサイトカインに特異的に結合する捕捉用抗体が固定されていてもよい。前記サイトカインとしては、例えば、インターフェロンγ(IFN-γ)、インターロイキン2(IL-2)、IL-4、IL-5、IL-10、IL-13、パーフォリン、グランザイムB(GzB)、腫瘍壊死因子アルファ(TNF-α)等が挙げられるが、これらに限定されない。捕捉用抗体は、1種であってもよく、2種以上であってもよい。一実施形態において、捕捉用抗体は、抗IFN-γ抗体、および抗IL-2抗体である。
 抗原ウイルスタンパク質またはその断片に対して応答性を有するT細胞は、抗原ウイルスタンパク質またはその断片との接触により、サイトカインを分泌する。捕捉用抗体は、T細胞から分泌されたサイトカインに結合し、ウェルプレート上に捕捉し得る。
 捕捉用抗体を固定するために、ELISPOT用プレートのウェル底面はポリフッ化ビニリデン(PVDF)膜で形成されていてもよい。
 対象由来のT細胞と抗原ウイルスタンパク質又はその断片の接触は、抗原タンパク質またはその断片が由来する抗原ウイルスの種類毎に行うことが好ましい。すなわち、1種類の抗原ウイルスに由来する抗原ウイルスタンパク質又はその断片を、T細胞と接触させることが好ましい。これにより、抗原ウイルスの種類毎に、T細胞の応答性を測定することができる。例えば、抗原ウイルスとして、HCoV-229E、HCoV-OC43、HCoV-HKU1、及びHCoV-NL63の4種類を用いる場合、HCoV-229Eに由来するタンパク質またはその断片とT細胞との接触;HCoV-OC43に由来するタンパク質またはその断片とT細胞との接触;HCoV-HKU1に由来するタンパク質またはその断片とT細胞との接触;及びHCoV-NL63に由来するタンパク質またはその断片とT細胞との接触を、それぞれ別々に行うことができる。
 抗原ウイルスタンパク質またはその断片は、1種の抗原ウイルスについて複数種類のタンパク質またはその断片を用いてもよく、1種類のタンパク質またはその断片のみを用いてもよい。タンパク質またはその断片は、S蛋白またはその断片を含むことが好ましい。
<工程(b)>
 工程(b)では、抗原ウイルスタンパク質またはその断片に対する対象由来のT細胞の応答性に基づいて、対象コロナウイルス感染症の重症化リスクを評価する。
 抗原ウイルスタンパク質またはその断片に対する対象由来のT細胞の応答性は、公知の方法に基づいて、測定することができる。T細胞は、抗原ウイルスタンパク質またはその断片への接触により活性化すると、増殖してサイトカインを生成する。そのため、抗原ウイルスタンパク質またはその断片に対するT細胞の応答性は、T細胞の増殖又はサイトカインの生成に基づいて、測定してもよい。
 例えば、T細胞の応答性は、抗原ウイルスタンパク質またはその断片による刺激に対するT細胞からのサイトカインの分泌に基づいて測定することができる。例えば、抗原ウイルスタンパク質またはその断片との接触によりT細胞から分泌されるサイトカイン量が多いほど、応答性が高いと評価することができる。サイトカインの分泌量は、例えば、サイトカインに特異的に結合する抗体を用いた免疫学的手法により測定することができる。そのような免疫学的手法としては、例えば、ELISA法、ウェスタンブロッティング法等が挙げられるが、これらに限定されない。あるいは、T細胞の応答性は、T細胞集団が発現するサイトカインのmRNA量に基づいて評価してもよい。サイトカインのmRNA量を測定する方法としては、例えば、RT-qPCR、ノーザンブロッティング等が挙げられるが、これらに限定されない。
 T細胞の応答性は、抗原ウイルスタンパク質またはその断片との接触によりサイトカインを分泌するT細胞の数により評価してもよい。例えば、抗原ウイルスタンパク質またはその断片との接触によりサイトカインを分泌するT細胞の数が多いほど、応答性が高いと評価することができる。サイトカインを分泌するT細胞数は、例えば、サイトカインに特異的に結合する抗体を用いた免疫学的手法により測定することができる。そのような免疫学的手法としては、例えば、ELISPOTアッセイ等が挙げられるが、これに限定されない。
 例えば、抗原ウイルスタンパク質またはその断片に対する対象由来のT細胞の応答性は、ELISPOTアッセイの手法を用いて測定することができる。ELISPOTアッセイは、例えば、以下のように行うことができる。ELISPOTアッセイでは、T細胞およびT細胞刺激抗原(抗原ウイルスタンパク質またはその断片)をウェル内でインキュベートし、T細胞から分泌されるサイトカインを、ウェルに固定した捕捉用抗体で捕捉する。次いで、ウェルを洗浄してT細胞を除去した後、標識化された検出用抗体をウェルに添加する。検出用抗体としては、捕捉用抗体と同じサイトカインに特異的に結合する抗体を用いることができる。検出用抗体は、捕捉用抗体に捕捉されたサイトカインに結合する。次いで、検出用抗体が有する標識に応じて、発色反応を行うことにより、サイトカインを検出することができる。ELISPOTアッセイでは、サイトカインを分泌したT細胞が位置した場所に、発色反応によるスポットが生じる。スポット数は、ELISPOTリーダー等によりカウントすることができる。スポット数が多いほど、抗原ウイルスタンパク質またはその断片に対するT細胞の応答性が高いと評価することができる。
 サイトカインを検出する発色反応は、発色反応を触媒する酵素(以下。「発色酵素」という)と、当該酵素の基質を用いて行うことができる。発色酵素としては、例えば、ペルオキシダーゼ(例、西洋ワサビペルオキシダーゼ;HRP)、アルカリホスファターゼ(AP)等が挙げられる。発色酵素は、検出用抗体に直接標識してもよく、ビオチン-アビジン結合等の結合対を利用して、検出用抗体に結合してもよい。2種以上のサイトカインを検出する場合には、サイトカイン毎に異なる発色酵素を用いてもよい。
 検出用抗体として蛍光色素で標識された抗体を用いてもよい。この場合、T細胞を除去後のウェルに検出用抗体を添加して反応させると、サイトカインを分泌したT細胞が位置した場所に、蛍光スポットが生じる。蛍光スポット検出リーダー等で、この蛍光スポットをカウントしてもよい。
 抗原ウイルスタンパク質またはその断片に対する対象由来のT細胞の応答性は、対象コロナウイルス感染症の重症化リスクと関連している。
 例えば、抗原ウイルスタンパク質またはその断片に対する対象由来のT細胞の応答性が低い場合、前記対象は、対象コロナウイルス感染症に対する重症化リスクが高いと評価することができる。
 例えば、抗原ウイルスタンパク質またはその断片に対する対象由来のT細胞の応答性が高い場合、前記対象は、対象コロナウイルス感染症に対する重症化リスクが低いと評価することができる。
 抗原ウイルスタンパク質またはその断片に対する対象由来のT細胞の応答性が高いことは、当該対象が、抗原ウイルスに感染した経験があり、抗原ウイルスに対する細胞性免疫が維持されていることを意味し得る。抗原ウイルスに対して応答性を有するT細胞は、交差反応により、対象コロナウイルス感染症の原因ウイルスに対しても応答すると考えられる。そのため、抗原ウイルスに対する細胞性免疫を有する対象では、原因ウイルスの増殖が抑制されて、対象コロナウイルス感染症の重症化を回避できると考えられる。
 抗原ウイルスタンパク質またはその断片が、第1の抗原ウイルスに由来する場合、前記抗原タンパク質またはその断片に対するT細胞の応答性が低い場合に、対象コロナウイルス感染症の重症化リスクが高いと評価してもよい。原因ウイルスと受容体が異なる第1の抗原ウイルスに対する応答性は、対象コロナウイルス感染症の重症化リスクと負の相関を示す傾向がある。例えば、原因ウイルスがSARS-CoV-2である場合、第1の抗原ウイルスとしては、HCoV-OC43、HCoV-HKU1、HCoV-229E、及びMERS-CoVが挙げられる。抗原ウイルスタンパク質またはその断片として、これらのコロナウイルスに由来するタンパク質又はその断片を用いた場合、T細胞の応答性が低い場合に、COVID-19の重症化リスクが高いと評価してもよい。
 抗原ウイルスタンパク質またはその断片が、第2の抗原ウイルスに由来する場合、前記抗原タンパク質またはその断片に対するT細胞の応答性が高い場合に、対象コロナウイルス感染症の重症化リスクが高いと評価してもよい。原因ウイルスと受容体が同じ第2の抗原ウイルスに対する応答性は、対象コロナウイルス感染症の重症化リスクと正の相関を示す傾向がある。例えば、原因ウイルスがSARS-CoV-2である場合、第2の抗原ウイルスとしては、HCoV-NL63、及びSARS-CoVが挙げられる。抗原ウイルスタンパク質またはその断片として、これらのコロナウイルスに由来するタンパク質又はその断片を用いた場合、T細胞の応答性が高い場合に、COVID-19の重症化リスクが高いと評価してもよい。
 対象のT細胞の応答性の評価は、予め設定した基準値に基づいて行ってもよい。例えば、抗原ウイルスタンパク質またはその断片に対するT細胞の応答性が、基準値よりも高い場合に、応答性が高いと評価してもよい。例えば、抗原ウイルスタンパク質またはその断片に対するT細胞の応答性が、基準値よりも低い場合に、応答性が低いと評価してもよい。
 前記基準値は、例えば、任意の人数で構成される健常者グループにおいて測定された応答性から、統計処理等を行って算出された数値であってもよい。あるいは、任意の人数で構成される対象コロナウイルス感染症の感染者又は発症者のグループにおいて測定された応答性から、統計処理等を行って算出された数値であってもよい。基準値は、例えば、対象コロナウイルス感染症の軽症者グループにおいて測定された応答性から算出された数値であってもよい。あるいは、基準値は、対象コロナウイルス感染症の中等症以上の患者グループ(例えば、COVID-19の場合には、中等症I、中等症II、及び重症)において測定された応答性から算出された数値であってもよい。対象コロナウイルス感染症の感染者又は発症者の応答性から基準値を設定する場合、対象コロナウイルス感染症の回復期にある患者から採取されたT細胞を用いることが好ましい。
 基準値は、抗原ウイルスの種類毎に、設定されることが好ましい。例えば、対象コロナウイルス感染症がCOVID-19であり、抗原ウイルスとして、HCoV-229E、HCoV-OC43、HCoV-HKU1、及びHCoV-NL63用いる場合、これらの4種類の抗原ウイルス毎に基準値を設定することができる。
 本態様にかかる方法は、工程(a)が、下記工程(a1)及び工程(a2)を含んでもよい。
 (a1)対象由来のT細胞を、原因ウイルスが受容体とする分子とは異なる分子を受容体とする第1の他種コロナウイルス(第1の抗原ウイルス)に由来するタンパク質またはその断片に接触させる工程。
 (a2)対象由来のT細胞を、原因ウイルスが受容体とする分子と同じ分子を受容体とする第2の他種コロナウイルス(第2の抗原ウイルス)に由来するタンパク質またはその断片に接触させる工程。
 本態様にかかる方法は、工程(b)が、下記工程(b1)~工程(b3)を含んでもよい。
 (b1)第1の他種コロナウイルス(第1の抗原ウイルス)に由来するタンパク質またはその断片に対する対象由来のT細胞の第1の応答性を測定する工程。
 (b2)第2の他種コロナウイルスに由来するタンパク質またはその断片に対する対象由来のT細胞の第2の応答性を測定する工程。
 (b3)第1の応答性に対する第2の応答性の比の値(第2の応答性/第1の応答性)に基づいて、コロナウイルス感染症の重症化リスクを評価する工程。
 工程(a1)は、第1の抗原ウイルスに由来するタンパク質またはその断片を用いて、工程(a)を行えばよい。
 工程(a2)では、第2の抗原ウイルスに由来するタンパク質またはその断片を用いて、工程(a)を行えばよい。
 工程(b1)では、工程(a1)により誘導された対象由来のT細胞の応答性を測定すればよい。工程(b1)により得られた応答性を第1の応答性とする。
 工程(b2)では、工程(a2)により誘導された対象由来のT細胞の応答性を測定すればよい。工程(b2)により得られた応答性を第2の応答性とする。
 T細胞の応答性は、上述の方法により測定することができる。T細胞の応答性は、ELISPOTアッセイにより測定することが好ましい。T細胞の応答性は、例えば、ELISPOTアッセイによる有効スポット数として定量化してもよいし、有効スポット数インデックスとして定量化してもよい。有効スポット数は、T細胞刺激抗原の存在下でT細胞をインキュベートした区画(ウェル)で得られたスポット数から、陰性対照区画(ウェル)のスポット数を差し引いた値として算出される。陰性対照区画は、T細胞刺激抗原の非存在下でT細胞をインキュベートした区画である。有効スポット数インデックスは、有効スポット数を陽性対照区画(ウェル)のスポット数で除した値として算出される。陽性対照区画は、T細胞刺激抗原としてすべてのT細胞が強く反応する抗CD3抗体を用いてT細胞をインキュベートした区画である。
 工程(b3)では、第1の応答性に対する第2の応答性の比の値(第2の応答性/第1の応答性)に基づいて、コロナウイルス感染症の重症化リスクを評価する。前記比の値(第2の応答性/第1の応答性)は、適当な係数を乗じたものであってもよい。例えば、重症化リスクは、下記式(1)により求められる重症化リスク値(Y)により評価することができる。

 式(1)中、Yは重症化リスク値;kは係数;Aは第1の応答性;Bは第2の応答性である。係数kは、例えば、1~10の範囲の数値を取り得る。
 第1の抗原ウイルスが2種以上である場合、当該2種以上の第1のウイルスのそれぞれについて、前記式(1)により、重症化リスク値(Y)をそれぞれ算出し、それらを加算して、総合的な重症化リスク値(Y)を得てもよい。2種以上の第1の抗原ウイルスのそれぞれについて、重症化リスク値(Y)を算出する場合、前記式(1)中のBとしては、2種以上の第1の抗原ウイルス間で同一の値を用いることが好ましい。
 例えば、重症化リスク値(Y)は、下記式(2)により求めてもよい。

 式(2)中、Yは重症化リスク値;kおよびkは係数;Aは1種類目の第1の抗原ウイルスによる第1の応答性;Aは2種類目の第1の抗原ウイルスによる第1の応答性;Bは第2の応答性である。係数kおよびkは、例えば、1~10の範囲の数値を取り得る。3種類目以降の第1の抗原ウイルスについても、同様に加算していけばよい。
 原因ウイルスがSARS-CoV-2である場合、第1の抗原ウイルスとしては、HCoV-HKU1、HCoV-OC43、及びHCoV-229Eの3種を用いることが好ましい。第2の抗原ウイルスとしては、HCoV-NL63を用いることが好ましい。この場合、重症化リスク値(Y)は、下記式(I)により求めてもよい。

 式(I)中、Yは重症化リスク値;k、kおよびkは係数;NL63はHCoV-NL63に由来するタンパク質またはその断片に対するT細胞の応答性;HKU1はHCoV-HKU1に由来するタンパク質またはその断片に対するT細胞の応答性;OC43はHCoV-OC43に由来するタンパク質またはその断片に対するT細胞の応答性;229EはHCoV-229Eに由来するタンパク質またはその断片に対するT細胞の応答性である。係数k、kおよびkは、例えば、1~10の範囲の数値を取り得る。係数k、kおよびkは、例えば、1~5の範囲の数値であってもよく、1~3の範囲の数値であってもよい。たとえば、一例として、k=3、k=1およびk=3が挙げられる。
 上記式(1)、(2)又は(I)で求められる重症化リスク値(Y)は、コロナ感染症の重症化リスクと正の相関を示す。そのため、対象コロナ感染症の重症化リスクを、重症化リスク値(Y)により評価することができる。例えば、ある対象について求められた重症化リスク値(Y)が、予め設定した基準値(カットオフ値)よりも高い場合には、当該対象は、重症化リスクが高いと判定することができる。ある対象について求められた重症化リスク値(Y)が、予め設定した基準値(カットオフ値)よりも低い場合には、当該対象は、重症化リスクが低いと判定することができる。基準値は、例えば、任意の人数により構成される感染者又は発症者のグループにおいて算出された重症化リスク値(Y)を統計処理等して得られる数値であってもよい。
 本態様にかかる方法によれば、簡易な方法で、コロナウイルス感染症の重症化リスクを評価することができる。重症化リスクを評価することで、社会的及び医療的に適切な措置を講じることが可能となる。
 本態様にかかる方法では、T細胞刺激抗原として、対象コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルス(抗原ウイルス)に由来するタンパク質またはその断片を用いる。そのため、原因ウイルスの未感染者、および原因ウイルスの既感染者またはワクチン接種者であるが原因ウイルスに対する細胞性免疫が消失した対象であっても、対象コロナウイルス感染症の重症化リスクを評価することができる。
 本態様にかかる方法によれば、未知のコロナウイルス感染症に対しても、重症化リスクを評価することができる。
<任意工程>
 本態様にかかる方法は、上記工程に加えて、任意の工程を含んでもよい。任意工程としては、例えば、(c)対象が原因ウイルス感染者である場合に、対象コロナウイルス感染症の重症化リスクの評価結果に基づいて、対象コロナウイルス感染症の治療方針を決定する工程、(d)対象コロナウイルス感染症の重症化リスクが高いと評価された対象が原因ウイルスの感染者である場合に、対象コロナウイルス感染症の症状の観察、および対象コロナウイルス感染症の治療からなる群より選択される少なくとも1種の措置を行う工程、ならびに(e)対象コロナウイルス感染症の重症化リスクが高いと評価された対象が原因ウイルスの非感染者である場合に、対象コロナウイルス感染症のワクチン接種を行う工程、等が挙げられる。工程(c)~(e)のいずれかを含む場合、本態様にかかる方法は、コロナウイルス感染症の重症化を予防する方法であってもよい。工程(c)及び工程(d)のいずれかを含む場合、本態様にかかる方法は、コロナウイルス感染症を治療する方法であってもよい。
(工程(c))
 対象が原因ウイルス感染者である場合、重症化リスクに基づいて、対象コロナウイルス感染症の治療方針を決定することができる。重症化リスクが低いと評価された感染者は、積極的な治療を行わず、経過観察のみを行うと決定してもよい。重症化リスクが高い感染者は、症状の進行を慎重に観察し、感染初期から積極的な治療を施すことで、重症化を回避できる可能性がある。そこで、重症化リスクが高いと評価された感染者に対しては、感染初期から積極的な治療を行うと決定してもよい。対象コロナウイルス感染症の治療方法は、対象コロナウイルスの種類に応じて選択可能である。治療方法としては、抗ウイルス薬または中和抗体等の投与が挙げられる。対象コロナウイルス感染症がCOVID-19である場合、抗ウイルス薬としては、モルヌピラビルル(商品名:ラゲブリオカプセル)、ニルマトレルビル/リトナビル(商品名:パキロビッドパック)、レムデシビル(商品名:ベルクリー点滴静注液)、デキサメサゾン、トシリズマブ(商品名:アクテムラ)、ファビピラビル(商品名:アビガン)等が挙げられる。中和抗体としては、カシリビマブとイムデビマブの抗体カクテル療法(商品名:ロナプリーブ注射液セット)が挙げられる。
(工程(d))
 対象コロナウイルス感染症の重症化リスクが高いと評価された対象が原因ウイルスの感染者である場合、感染初期から症状の慎重な経過観察を行い、積極的な治療を施すことで、重症化を回避できる可能性がある。そこで、重症化リスクが高い感染者に対しては、症状の観察、および/またはコロナウイルス感染症の治療を行ってもよい。治療方法としては、上記と同様のものが挙げられる。また、経過観察および重症化したときの迅速な対応に備えて、病院への入院措置を行ってもよい。工程(d)は、重症化リスクが高いと判定された原因ウイルス感染者に対し、コロナ感染症に対する治療を行う工程であってもよい。例えば、工程(d)は、重症化リスクが高いと判定された原因ウイルス感染者に対し、原因ウイルスに対する抗ウイルス薬又は中和抗体を投与する工程であってもよい。
(工程(e))
 対象コロナウイルス感染症の重症化リスクが高いと評価された対象が原因ウイルスの非感染者である場合、対象が原因ウイルスに感染すると重症化する恐れがある。そのため、そのような対象に対しては、対象コロナウイルス感染症のワクチン接種を行うことが考えられる。ワクチン接種を行って対象コロナウイルス感染症に対する免疫を獲得させることで、当該対象において、対象コロナウイルスに対する重症化リスクを低下させ得る。工程(e)は、重症化リスクが高いと判定された非感染者に対し、対象コロナウイルス感染症のワクチン接種を行う工程であってもよい。
[コロナウイルス感染症の重症化リスクの評価キット]
 本開示の第2の態様は、コロナウイルス感染症の重症化リスク評価キットである。本態様にかかるキットは、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスに由来するタンパク質(抗原ウイルスタンパク質)又はその断片を含む。
<抗原ウイルスタンパク質またはその断片>
 抗原ウイルスタンパク質またはその断片としては、上記と同様のものが挙げられる。抗原ウイルスは、原因ウイルスと同じ属に属し、且つ原因ウイルスと異なる種に属するコロナウイルスであることが好ましい。例えば、原因ウイルスがベータコロナウイルス属である場合、抗原ウイルスはベータコロナウイルス属の異なる種であってもよい。抗原ウイルスは、アルファコロナウイルス属であってもよい。抗原ウイルスは、対象に感染可能なコロナウイルスであることが好ましい。
 原因ウイルスが、SARS-CoV-2である場合、抗原ウイルスは、ベータコロナウイルス属のウイルスを用いることができる。原因ウイルスがSARS-CoV-2であり、対象がヒトである場合、抗原ウイルスとして、ベータコロナウイルス属のヒトコロナウイルス(HCoV-OC43、HCoV-HKU1等)を用いることができる。
 抗原ウイルスは、第1の抗原ウイルスであってもよい。例えば、原因ウイルスがSARS-CoV-2であり、対象がヒトである場合、抗原ウイルスタンパク質またはその断片としては、HCoV-229E、HCoV-OC43、HCoV-HKU1、またはMERS-CoVに由来するものを用いることができる。
 抗原ウイルスは、第2の抗原ウイルスであってもよい。例えば、原因ウイルスがSARS-CoV-2であり、対象がヒトである場合、抗原ウイルスタンパク質またはその断片としては、HCoV-NL63、またはSARS-CoVに由来するものを用いることができる。
 抗原ウイルスタンパク質またはその断片は、1種であってもよく、2種以上であってもよい。抗原ウイルスタンパク質またはその断片は、第1の抗原ウイルスに由来するものと、第2の抗原ウイルスに由来するものの両方を含むことが好ましい。例えば、原因ウイルスがSARS-CoV-2であり、対象がヒトである場合、本態様にかかるキットは、HCoV-229E、HCoV-OC43、HCoV-HKU1、及びMERS-CoVからなる群より選択される1種以上に由来する抗原ウイルスタンパク質またはその断片と、HCoV-NL63、及びSARS-CoVからなる群より選択される1種以上に由来する抗原ウイルスタンパク質またはその断片と、を含むことが好ましい。
 本態様にかかるキットは、HCoV-HKU1に由来する抗原ウイルスタンパク質またはその断片と、HCoV-OC43に由来する抗原ウイルスタンパク質またはその断片と、HCoV-229Eに由来する抗原ウイルスタンパク質またはその断片と、HCoV-NL63に由来する抗原ウイルスタンパク質またはその断片とを含むことが好ましい。
 抗原ウイルスタンパク質としては、S蛋白、N蛋白、M蛋白、およびE蛋白等が挙げられ、S蛋白が好ましい。
<任意の構成>
 本態様にかかるキットは、抗原ウイルスタンパク質またはその断片に加えて、任意の構成を含んでいてもよい。任意の構成としては、例えば、T細胞が抗原刺激に応答して分泌するサイトカインに対する特異的結合物質、固相担体、発色酵素、発色酵素基質、洗浄バッファー、希釈バッファー、使用説明書等が挙げられる。
(T細胞が抗原刺激に応答して分泌するサイトカインに対する特異的結合物質)
 本態様にかかるキットは、T細胞が抗原刺激に応答して分泌するサイトカインに対する特異的結合物質を含んでもよい。
 「特異的結合物質」とは、特定の生体分子に対して、特異的結合性を有する物質を意味する。「特異的結合性を有する」とは、特定の生体分子に対して高い結合親和性を有するが、他の生体分子に対しては、極めて低い結合親和性しか有さないことを意味する。特異的結合物質は、好ましくは、特定の生体分子に高い結合性を有するが、他の生体分子にはほとんど結合性を有しない。タンパク質に対する特異的結合物質としては、抗体、およびアプタマー等が挙げられる。一実施形態において、特異的結合物質は、抗体である。
 T細胞が抗原刺激に応答して分泌するサイトカインとしては、上記と同様のものが挙げられる。サイトカインに対する特異的結合物質の具体例としては、例えば、抗IFN-γ抗体、抗IL-2抗体、抗IL-4抗体、抗IL-5抗体、抗IL-10抗体、抗IL-13抗体、抗パーフォリン抗体、抗グランザイムB抗体、抗TNF-α抗体等が挙げられるが、これらに限定されない。特異的結合物質は、1種を単独で用いてもよく、2種以上を併用してもよい。2種以上のサイトカインを検出する場合、前記2種以上のサイトカインにそれぞれ結合する特異的結合物質を用いる。
 サイトカインに対する特異的結合物質は、捕捉用抗体として、固相担体に結合されていてもよい。固相担体に対する特異的結合物質の結合は、公知の方法により、行うことができる。固相担体としては、例えば、ウェルプレートが挙げられる。特異的結合物質はウェルプレートのウェル底面に結合されてもよい。固相担体は、例えば、ELISPOT用ウェルプレートであってもよい。固相担体は、ウェル底面がPVDFメンブレンで構成されたものでもよい。
 サイトカインに対する特異的結合物質は、検出用抗体として、標識物質で標識されていてもよい。標識物質は、発色酵素であってもよく、発色酵素を結合するための結合対の第1要素であってもよい。結合対の第1要素で標識化された特異的結合物質の具体例としては、ビオチン標識抗体が挙げられる。発色酵素としては、上記と同様のものが挙げられる。例えば、サイトカインとしてIFN-γを検出する場合、特異的結合物質の具体例としては、FITC標識抗ヒトIFN-γ抗体等が挙げられる。例えば、サイトカインとしてIL-2を検出する場合、特異的結合物質の具体例としては、ビオチン標識抗ヒトIL-2抗体等が挙げられる。
 検出用抗体は、蛍光色素で標識されていてもよい。蛍光色素は、公知のものを特に制限なく用いることができる。蛍光色素としては、例えば、カルボキシフルオレセイン(FAM)、6-カルボキシ-4’,5’-ジクロロ2’,7’-ジメトキシフルオレセイン(JOE)、フルオレセインイソチオシアネート(FITC)、テトラクロロフルオレセイン(TET)、5'-ヘキサクロロ-フルオレセイン-CEホスホロアミダイト(HEX)、フィコエリスリン(PE)、Cy3、Cy5、Alexa568、Alexa647等が挙げられるが、これらに限定されない。
(発色酵素)
 発色酵素は、特に限定されず、公知のものを用いることができる。発色酵素としては、HRP、およびAP等が挙げられる。発色酵素は、検出用抗体に結合されていてもよい。検出用抗体が、結合対の第1要素(例えば、ビオチン)で標識されている場合、発色酵素は結合対の第2要素(例えば、アビジン又はその誘導体)で標識されていてもよい。例えば、検出用抗体がFITC標識抗ヒトIFN-γ抗体である場合、発色酵素はFITC標識HRP(FITC-HRP)であってもよい。例えば、検出用抗体がビオチン標識抗ヒトIL-2抗体である場合、発色酵素はストレプトアビジン標識AP(Strep-Ap)であってもよい。
(発色酵素基質)
 発色酵素基質は、発色酵素の種類に応じて、適宜選択することができる。
 発色酵素がHRPである場合、発色酵素基質としては、3,3’-ジアミノベンジジン(DAB)、3,3’,5,5’-テトラメチルベンジジン(TMB)、2,2’-アジノビス[3-エチルベンゾチアゾリン-6-スルホン酸](ABTS)、o-フェニレンジアミン二塩酸塩(OPD)等が挙げられる。
 発色酵素がAPである場合、発色酵素基質としては、ニトロブルーテトラゾリウムクロライド(NBT)、5-ブロモ-4-クロロ-3-インドリルホスフェート(BCIP)、p-ニトロフェニルリン酸(PNPP)等が挙げられる。
 本態様にかかるキットは、ELISPOTアッセイ用キットとして提供されてもよい。
 本態様にかかるキットは、前記第1の態様にかかる評価方法を実施するために使用することができる。
 他の態様において、本開示は、コロナウイルス感染症の重症化リスクの評価キットを製造するための、抗原ウイルスタンパク質またはその断片の使用を提供する。他の態様において、本開示は、コロナウイルス感染症の重症化リスクの評価するための、抗原ウイルスタンパク質またはその断片の使用を提供する。他の態様において、本開示は、コロナウイルス感染症の重症化リスクの評価するための、抗原ウイルスタンパク質またはその断片を提供する。抗原ウイルスタンパク質としては、上記と同様のものが挙げられ、第1の抗原ウイルスに由来するタンパク質、及び第2の抗原ウイルスタンパク質の組合せが好ましい。
 本開示は、以下の態様を含み得る。
 コロナウイルス感染症の重症化リスクの判定方法であって、(a)対象由来のT細胞を、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、(b)前記タンパク質またはその断片に対する前記T細胞の応答性に基づいて、前記コロナウイルス感染症の重症化リスクを判定する工程と、を含む、コロナウイルス感染症の重症化リスクの判定方法。
 コロナウイルス感染症の重症化リスクの高い対象に由来するT細胞含有試料を選別する方法であって、(a)前記T細胞含有試料中のT細胞を、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、(b)前記タンパク質またはその断片に対する前記T細胞の応答性に基づいて、前記コロナウイルス感染症の重症化リスクが高い対象に由来する前記T細胞含有試料を選別する工程と、を含む、コロナウイルス感染症の重症化リスクの高い対象に由来するT細胞含有試料を選別する方法。
 以下、実施例により本発明を説明するが、本発明は以下の実施例に限定されるものではない。
[実施例1]
<被験者>
 被験者は、東北大学病院ならびに関連施設(栗原中央病院、気仙沼市立病院)でCOVID-19の治療を受けたCOVID-19感染回復期にある患者とした。被験者には、事前に説明して同意を得た。治癒後の回復期に、末梢血10mL程度を専用の容器に採取した。COVID-19に罹患歴のない健常者を対照群として、同様に採血を行った。
 被験者におけるCOVID-19の重症度は、「新型コロナウイルス感染症(COVID-19)診療の手引き・第5版(厚生労働省)」(https://www.mhlw.go.jp/content/000785119.pdf)に準じて、以下のように分類した。
 軽症:酸素飽和度(SpO2)≧96%で、呼吸器症状なし、または、咳のみで呼吸困難なし。いずれの場合であっても肺炎所見を認めない。
 中等症I(呼吸不全なし):93%<SpO2<96%で、呼吸困難,肺炎所見を認める。
 中等症II(呼吸不全あり):SpO2≦93%で、酸素投与が必要。
 重症:ICUに入室、または人工呼吸器が必要。
 被験者は、健常者が3例、COVID-19既感染者が11例(軽症が4例、中等症Iが3例、中等症IIが4例)であった。
<末梢血単核球(PBMC)の調製>
 COVID-19感染後に6週以上経過して治癒期にある患者、および対照群である非感染者から、末梢血を採取した。BDバキュティナ(登録商標)CPT単核球分離用採血管(Becton,Dickinson and Company)を用いて、1500~1800G、室温で、15分間遠心分離して単核球層を分離および採取した。CTL-wash suppliment(Cellular Technologies,Cleveland,OH,USA)添加したRPMI-1640で、細胞を洗浄した。次いで、300G、室温で、15分間遠心分離した。この洗浄操作を2回繰り返した。その後、細胞保存液(CELLBANKER2 (日本全薬工業))を加えてPBMCを-80℃で保存した。
<ELISPOTアッセイ>
 Tリンパ球の反応性は、分泌されるIFN-γとIL-2を対象として評価した。ELISPOTアッセイには、Human IFN-γ/IL-2 Double-Color ELISPOT Kit:hIFNgIL2-2M,CTL(Cellular Technologies,Cleveland,OH,USA)を用いた。
 ELISPOTアッセイは、文献(Czerkinsky C, et al. Immunol. Methods,1988.)に記載の方法を一部変更して行った。具体的には、ウェル底面がPVDFメンブレンとなっている96ウェルのELISPOT用プレートに、ヒトIFN-γ/IL-2キャプチャー抗体をコートして、4℃で一晩インキュベートした。PBSでプレートを洗浄した後、Tリンパ球刺激抗原および共刺激の抗CD28抗体を添加して、37℃で、20分間インキュベートした。Tリンパ球刺激抗原として、ベータコロナウイルス属(HCoV-OC43、HCoV-HKU1)のスパイク(S)蛋白、SARS-CoV-2のS蛋白、SARS-CoV-2のS蛋白+ヌクレオカプシド(N)蛋白+膜(M)蛋白のペプチドプールを用いた。用いたTリンパ球刺激抗原を表2に示す。ネガティブコントロールのウェルには、Tリンパ球刺激抗原を添加しなかった。

 上記インキュベートにおけるTリンパ球刺激抗原濃度は、SARS-CoV-2のS蛋白およびペプチドプールでは、2μg/mL(最終濃度)とした。βコロナウイルス属(HCoV-OC43、HCoV-HKU1)のS蛋白では、12.5μg/mL(最終濃度)とした。
 上記インキュベート後、健常者3例、COVID-19既感染者11例(軽症:4例、中等症I:3例、中等症II:4例)から採取したPBMCを、300,000個/ウェルで添加した。次いで、37℃で、24時間インキュベートした。PBSで洗浄後、0.05% Tween/PBSで洗浄した。次いで、IFN-γ/IL-2検出用抗体(anti-human IFNガンマ(FITC) Detection AbおよびAnti-human IL-2(Biotin) Detection Ab)を添加し、室温で、2時間インキュベートした。0.05% Tween/PBSで洗浄後、Strep-APおよびFITC-HRPの混合溶液をウェルに添加し、室温で、1時間インキュベートした。0.05% Tween/PBSで洗浄後、蒸留水で洗浄した。次いで、発色基質であるBlue developer溶液を添加し、室温で、15分間インキュベートした。水道水で洗浄後、蒸留水で洗浄した。次いで、Red developer溶液を添加し、室温で、10分間インキュベートした。水道水で洗浄後、クリーンベンチ内で24時間自然乾燥させた。
 反応したTリンパ球数の評価は、Spot-Forming Cells(SFC)として、自動免疫スポット分析器Automated Immunospot Analyzer,CTL(Cellular Technologies,Cleveland,OH,USA)で自動解析した。カウントされたスポット数から陰性対照群のスポット数を差し引いた値(以下、「有効スポット数」ともいう)を、以下の基準に従い評価した。

<結果>
 図1に、ELISPOTアッセイの結果を示す。上図はELISPOTアッセイにおけるウェル画像である。下図は上記評価基準による評価結果である。健常者では、HCoV-OC43、HCoV-HKU1、およびSARS-CoV-2のいずれのS蛋白に対しても応答を認めなかった。
 回復期のCOVID患者においては、軽症例では、HCoV-OC43およびHCoV-HKU1に対して中等度~強応答であった。中等症Iでは、弱応答であった。中等症IIでは、ほぼ応答性を認めなかった。
 これらの結果は、ベータコロナウイルス属(HCoV-OC43、HCoV-HKU1)にあらかじめ細胞性免疫能を有すると、交差反応性により、COVID-19に対して保護的に作用して軽症で済むことを示す。一方、ベータコロナウイルス属に細胞性免疫能を有さないと、COVID-19の重症化を招くことを示す。
 上記の結果により、SARS-CoV-2と季節性ベータコロナウイルスとの類似点に起因する細胞性免疫における交差反応性が、COVID-19の重症度を低下させることが示された。
 図2A~図2Cは、図1のELISPOTアッセイの結果を定量化したグラフである。軽症例では、中等症I及び中等症IIと比較して、HCoV-OC43およびHCoV-HKU1に対する有効スポット数が多いことが示された。この結果から、HCoV-OC43および/又はHCoV-HKU1の抗原タンパク質を用いたELISPOTアッセイにより、SARS-CoV-2に対する重症化リスクを評価できることが確認された。
[実施例2]
<被験者>
 被験者は、健常者が37例、COVID-19既感染者が27例(軽症が8例、中等症Iが6例、中等症IIが9例、重症が4)であった。COVID-19既感染者の被験者は、実施例1と同様に、東北大学病院ならびに関連施設(栗原中央病院、気仙沼市立病院)でCOVID-19の治療を受けたCOVID-19感染回復期にある患者とした。
<末梢血単核球(PBMC)の調製>
 実施例1と同様の方法で、PBMCを調製し、-80℃で保存した。
<ELISPOTアッセイ>
 表2に示すTリンパ球刺激抗原に加えて、表3に示すTリンパ球刺激抗原を用いて、ELISPOTアッセイを行った。

 ELISPOTアッセイは、蛍光法により行った。
 Tリンパ球の反応性は、分泌されるIFN-γとIL-2、TNF-α、GranzymeBを対象として評価した。ELISPOTアッセイには、Human IFN-γ/IL-2/TNF-α/GranzymeB Four-Color FluoroSPOT Kit:hT4000F-10,CTL(Cellular Technologies,Cleveland,OH,USA)を用いた。ELISPOTアッセイは、文献(Czerkinsky C, et al. Immunol. Methods,1988.)に記載の方法を一部変更して行った。具体的には、ウェル底面がPVDFメンブレンとなっている96ウェルのELISPOT用プレートに、ヒトIFN-γ/IL-2/TNF-α/GranzymeBキャプチャー抗体をコートして、4℃で一晩インキュベートした。PBSでプレートを洗浄した後、Tリンパ球刺激抗原および共刺激の抗CD28抗体を添加して、37℃で、20分間インキュベートした。Tリンパ球刺激抗原として、表2に示したTリンパ球刺激抗原に加えて、表3に示したアルファコロナウイルス属(HCoV-NL63、HCoV-229E)のスパイク(S)蛋白を用いた。ポジティブコントロールのウェルには、Tリンパ球刺激抗原に代えて抗CD3抗体を添加した。ネガティブコントロールのウェルには、Tリンパ球刺激抗原を添加しなかった。
 上記インキュベートにおけるTリンパ球刺激抗原濃度は、SARS-CoV-2のS蛋白およびペプチドプールでは、2μg/mL(最終濃度)とした。ヒト風邪コロナウイルス(HCoV-OC43、HCoV-HKU1、HCoV-NL63、HCoV-229E)のS蛋白では、12.5μg/mL(最終濃度)とした。
 上記インキュベート後、健常者、COVID-19既感染者から採取したPBMCを、300,000個/ウェルで添加した。次いで、37℃で、24時間インキュベートした。PBSで洗浄後、0.05% Tween/PBSで洗浄した。次いで、IFN-γ/IL-2/TNF-α/GranzymeB検出用抗体(anti-human IFN-γ(FITC) Detection Ab、Anti-human IL-2(Hapten2) Detection Ab、Anti-human TNF-α(Hapten1) Detection AbおよびAnti-human GranzymeB(Biotin) Detection Ab)を添加し、室温で、2時間インキュベートした。0.05% Tween/PBSで洗浄後、Anti-FITC Alexa Fluor488、Anti-Hapten2 CTL-Red、Anti-Hapten1 CTL-YellowおよびStreptavidin eFluor450の混合溶液をウェルに添加し、室温で、1時間インキュベートした。蒸留水で洗浄後、クリーンベンチ内で24時間自然乾燥させた。
 反応したTリンパ球数の評価は、Spot-Forming Cells(SFC)として、自動免疫スポット分析器Automated Immunospot Analyzer,CTL(Cellular Technologies,Cleveland,OH,USA)で自動解析した。
<ELISPOTアッセイの結果>
 Tリンパ球刺激抗原として、HCoV-HKU1のS蛋白(HKU1-S)(図3A)、HCoV-OC43のS蛋白(OC43-S)(図3B)、HCoV-NL63のS蛋白(NL63-S)(図4A)、HCoV-229EのS蛋白(229E-S)(図4B)、SARS-CoV-2のS蛋白(CoV2-S)(図5A)及び、SARS-CoV-2のペプチドプール(CoV2-all)(図5B)をそれぞれ用いて、ELISPOTアッセイを行った結果を図3A~図5Bにそれぞれ示す。
 ベータコロナウイルス属であって、ACE2を受容体としないコロナウイルス(HKU1、OC43)のS蛋白に対しては、中等症以上の症例と比較して、軽症例において反応が強い傾向があった(図3A、図3B)。
 アルファコロナウイルス属であって、ACE2を受容体とするコロナウイルス(NL63)のS蛋白に対しては、軽症例と比較して、中等症以上の症例において反応が強い傾向があった(図4A)。
 アルファコロナウイルス属であって、ACE2を受容体としないコロナウイルス(229E)のS蛋白に対しては、中等症以上の症例と比較して、軽症例において反応が強い傾向があった(図4A)。
 SARS-CoV-2の蛋白に対しては、重症度が高くなるほど、反応が強い傾向があった(図5A、図5B)。
 図6A~図8Bに、中等症I、中等症II及び重症を「中等症以上」としてまとめた場合の結果を示す。いずれの場合も、上記の傾向がより顕著になった。
<重症化リスク値の計算式の検討>
 上記の結果から、以下の傾向が確認された。
(a)ACE2を受容体とするコロナウイルス(NL63)に対しては、軽症例よりも、中等症以上の症例において反応が強い。
(b)ACE2を受容体としないコロナウイルス(HKU1、OC43、229E)に対しては、中等症以上の症例よりも、軽症例において反応が強い。
 上記の結果を考慮し、ELISPOTアッセイの有効スポット数インデックスから、COVID-19重症化リスクの予測値を算出可能な計算式を検討した。その結果、重症化リスク値(Y)の計算式として、下記式(I)を作成した。

 前記式(I)中、NL63、HKU1、OC43、及び229Eは、Tリンパ球刺激抗原として、NL63-S、HKU1-S、OC43-S、及び229E-Sをそれぞれ用いてELISPOTアッセイを行ったときの有効スポット数インデックスである。
 前記式(I)中、k、k、及びkは、係数である。
 図9に、前記式(1)により得られたY値を、健常者、軽症例、及び中等症以上の症例のグループで示したグラフを示す。図9では、前記式(I)中の各係数は、k=3、k=1、及びk=3を用いた。図9に示すように、Y値は、軽症例と比較して、中等症以上の症例で有意に高くなった。
 Y値のカットオフ値(X)を20に設定した場合、以下の結果となった。
 X>20:100%の確率で中等症以上。
 X≦20:8/15(53%)の確率で軽症。
 上記の結果から、COVID-19重症化リスクは、前記式(I)で判定できることが確認された。
[実施例3]
<被験者>
 被験者は、ファイザー製ワクチン(BNT162b2)を接種したワクチン接種者とした。被験者は、28名であった。ワクチン接種前とワクチン接種後(おおむね6週から9週後)に採血を行った。
<末梢血単核球(PBMC)の調製、ELISPOTアッセイ>
 PBMCの調製、及びELISPOTアッセイは、実施例2と同様の方法で行った。
<ELISPOTアッセイ結果>
 Tリンパ球刺激抗原として、HKU1-S(図10A)、OC43-S(図10B)、NL63-S(図11A)、229E-S(図11B)、CoV2-S(図12A)及び、CoV2-all(図12B)をそれぞれ用いて、ELISPOTアッセイを行った結果を図10A~図12Bにそれぞれ示す。図中、「Pre」はワクチン接種前を示し、「Post」はワクチン接種後を示す。
 ACE2を受容体としないコロナウイルス(HKU1、OC43、229E)のS蛋白に対する反応は、ワクチン接種前とワクチン接種後とで、有意な差はなかった(図10A、図10B、図11B)。
 ACE2を受容体とするコロナウイルス(NL63)のS蛋白に対する反応は、ワクチン接種前と比較して、ワクチン接種後で有意に上昇した(図11A)。
 SARS-CoV-2の蛋白に対する反応は、ワクチン接種前と比較して、ワクチン接種後で有意に上昇した(図12A、図12B)。
<ワクチン接種前後の重症化リスク値(Y)>
 ワクチン接種前とワクチン接種後とで、前記式(I)により、重症化リスク値(Y)を算出した。その結果を図13A及び図13Bに示す。
 ワクチンの接種前とワクチン接種後とで、Y値に有意な差はなかった。ただし、何人かの被験者では、ワクチン接種後にY値が明らかに低下した(図13B)。
[実施例4]
 公知のCOVID-19重症化リスク因子と重症化リスク値(Y)との相関を解析した結果を図14A~図15Bに示す。重症化リスク値(Y)は、年齢(図14A)、BMI(図14B)、喫煙指数BI(図15A)、及び喫煙歴(図15B)のいずれとも有意な相関がなかった。
 実施例2の被験者では、重症度は、年齢(図16A)及びBMI(図16B)のいずれとも有意な相関はなかった。実施例2の被験者では、重症度は、喫煙指数BI(図17A)及び喫煙歴(図17B)と有意な相関があった。
 本発明によれば、コロナウイルス感染症の重症化リスクを簡易に判定可能な、コロナウイルス感染症の重症化リスクの評価方法、およびコロナウイルス感染症の重症化リスク評価キットが提供される。本発明にかかる評価方法および評価キットは、COVID-19の重症化リスクの評価に適用可能である。また、将来の未知のコロナウイルス感染症に対しても適用可能である。
 以上、本発明の好ましい実施形態を説明および図示してきたが、これらは本発明を例示するものであり、限定的なものとみなされるべきではないことを理解すべきである。本発明の精神または範囲から逸脱することなく、追加、省略、置換、およびその他の変更を行うことができる。したがって、本発明は、前述の説明によって限定されるものとはみなされず、添付の請求項の範囲によってのみ限定される。

Claims (13)

  1.  コロナウイルス感染症の重症化リスクの評価方法であって、
     (a)対象由来のT細胞を、前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、
     (b)前記タンパク質またはその断片に対する前記T細胞の応答性に基づいて、前記コロナウイルス感染症の重症化リスクを評価する工程と、
     を含む、コロナウイルス感染症の重症化リスクの評価方法。
  2.  前記タンパク質またはその断片が、前記原因ウイルスが受容体とする分子とは異なる分子を受容体とする第1の他種コロナウイルスに由来し、
     前記(b)の工程が、前記タンパク質またはその断片に対する前記T細胞の応答性が低い場合に、前記コロナウイルス感染症の重症化リスクが高いと評価する工程である、請求項1に記載のコロナウイルス感染症の重症化リスクの評価方法。
  3.  前記タンパク質またはその断片が、前記原因ウイルスが受容体とする分子と同じ分子を受容体とする第2の他種コロナウイルスに由来し、
     前記(b)の工程が、前記タンパク質またはその断片に対する前記T細胞の応答性が高い場合に、前記コロナウイルス感染症の重症化リスクが高いと評価する工程である、請求項1に記載のコロナウイルス感染症の重症化リスクの評価方法。
  4.  前記工程(a)が、
     (a1)前記T細胞を、前記原因ウイルスが受容体とする分子とは異なる分子を受容体とする第1の他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、
     (a2)前記T細胞を、前記原因ウイルスが受容体とする分子と同じ分子を受容体とする第2の他種コロナウイルスに由来するタンパク質またはその断片に接触させる工程と、を含み、
     前記(b)の工程が、
     (b1)前記第1の他種コロナウイルスに由来するタンパク質またはその断片に対する前記T細胞の第1の応答性を測定する工程と、
     (b2)前記第2の他種コロナウイルスに由来するタンパク質またはその断片に対する前記T細胞の第2の応答性を測定する工程と、
     (b3)前記第1の応答性に対する前記第2の応答性の比の値(第2の応答性/第1の応答性)に基づいて、前記コロナウイルス感染症の重症化リスクを評価する工程と、
     を含む、請求項1に記載のコロナウイルス感染症の重症化リスクの評価方法。
  5.  前記原因ウイルスおよび前記タンパク質またはその断片が由来する前記コロナウイルスが、ベータコロナウイルス属またはアルファコロナウイルス属である、請求項1~4のいずれか一項に記載のコロナウイルス感染症の重症化リスクの評価方法。
  6.  前記原因ウイルスがSARS-CoV-2である、請求項1~5のいずれか一項に記載のコロナウイルス感染症の重症化リスクの評価方法。
  7.  コロナウイルス感染症の重症化リスク評価キットであって、
     前記コロナウイルス感染症の原因ウイルスとは異なる種類のコロナウイルスである他種コロナウイルスに由来するタンパク質またはその断片を含む、
     コロナウイルス感染症の重症化リスク評価キット。
  8.  T細胞が抗原刺激に応答して分泌するサイトカインに対する特異的結合物質をさらに含む、請求項7に記載のコロナウイルス感染症の重症化リスク評価キット。
  9.  固相担体に結合された前記特異的結合物質と、標識物質で標識された前記特異的結合物質とを含む、請求項8に記載のコロナウイルス感染症の重症化リスク評価キット。
  10.  前記サイトカインがインターフェロンγ、インターロイキン2、及び腫瘍壊死因子アルファからなる群より選択される少なくとも1種である、請求項8または9に記載のコロナウイルス感染症の重症化リスク評価キット。
  11.  前記タンパク質またはその断片が、
     前記原因ウイルスが受容体とする分子とは異なる分子を受容体とする第1の他種コロナウイルスに由来するタンパク質またはその断片と、
     前記原因ウイルスが受容体とする分子と同じ分子を受容体とする第2の他種コロナウイルスに由来するタンパク質またはその断片と、
     を含む、請求項7~10のいずれか一項に記載のコロナウイルス感染症の重症化リスク評価キット。
  12.  前記原因ウイルスおよび前記タンパク質またはその断片が由来する前記コロナウイルスが、ベータコロナウイルス属またはアルファコロナウイルス属である、請求項7~11のいずれか一項に記載のコロナウイルス感染症の重症化リスク評価キット。
  13.  前記原因ウイルスがSARS-CoV-2である、請求項7~12のいずれか一項に記載のコロナウイルス感染症の重症化リスク評価キット。
