Abstract
La presente divulgación se refiere a un método para cuantificar un biomarcador de la replicación de un virus humano respiratorio, que es mRNA subgenómico. El método descrito permite adicionalmente hacer seguimiento a un sistema de administración de un medicamento para reducir la replicación viral de un virus respiratorio en la mucosa del tracto respiratorio superior e inferior en un paciente infectado con dicho virus.
Description
MÉTODO PARA CUANTIFICAR mRNA SUBGENÓMICO Y DETERMINAR ACTIVIDAD DE REPLICACIÓN VIRAL
CAMPO TÉCNICO
La presente invención se encuentra en eí campo de ía biotecnología, ía infectología y el diagnóstico médico, en particular en el campo de métodos de detección y cuaníificación de biomarcadores de replicación viral y su aplicación a ía determinación de la reducción de ía replicación viral.
ANTECEDENTES
El SARS-CoV-2 (coronavims del síndrome respiratorio agudo severo tipo-2), causa ía enfermedad conocida como CQVID-l 9, que ha causado una emergencia de salud pública global. El COV1D-19 continúa siendo un desafío sin precedentes para las autoridades sanitarias de todo eí mundo debido particularmente a la transmisibilidad y patogenicidad de las diferentes variantes del virus.
Dada 1a necesidad de abordar ía problemática causada por 1a pandemia del COVID-19, existen múltiples estudios y aproximaciones para su prevención, diagnóstico y tratamiento. Pin particular, la reacción en cadena de la poíimerasa con transcriptasa reversa o RT-PCR (por sus siglas en inglés) se considera el estándar dorado clínico para la detección del SARS-CoV-2. Esta prueba establecida en el protocolo de Berlín (Gorman V, et al. Detection of2G19 novel coronavirus (2019-nCoV) by real -time RT-PCR. Euro Surveill. 2020;25(3) - 23 enero 2020), se basa en un primer taxnizaje de rutina mediante la prueba para el gen E, seguido de confirmación mediante 1a prueba para el gen RdRp.
Sin embargo, la detección del virus por este medio no proporciona información relacionada con la severidad y el curso de la enfermedad, como se propone en el artículo de Liu et al (Viral dynamics in mild and severe cases of COVID-19. Lancet Infecí Dis - marzo 19, 2020), en donde se observó que los pacientes con infección severa tienden a tener una carga viral más alta (del orden de 60 veces más que la de los pacientes con infección moderada) y un periodo de propagación del virus más prolongado. Adicionalmente, en los casos de infección moderada se encontró un aclaramiento del
virus más temprano (90 % de los pacientes resultaron negativos por RT-PCR en el día 10 desde eí inicio de la enfermedad), en contraste con ios casos de infección severa, en donde la prueba seguía siendo positiva hasta después del día 10. Estos resultados sugieren que la carga viral del SARS-CoV -2 puede ser un biomarcador útil para evaluar la severidad y prognosis de la enfermedad.
En ese sentido, otros autores han sugerido que, dado que el RNA viral puede ser deteetable por RT-PCR hasta varias semanas después de la recuperación clínica, esta técnica no prueba solo la presencia de virus infecciosos, sino también de RNA viral residual, por lo cual se requieren metodologías alternativas para la determinación de la carga viral y su posible asociación con la capacidad de contagio. Tal es el caso del articulo de Perera eí ai (SARS-CoV -2 Virus Culture and Subgenomic RNA for Respiratory Specimens from Patients with Mild Coronavirus Disease. Emerging Infecüous Diseases;26(l l):270!-04 - noviembre 11, 2020), en donde se compararon los resultados de pacientes confirmados por RT-PCR, para la carga viral determinada mediante la cuantificaeión del número de copias del gen de laproteina N, cultivo del virus y detección de RNA subgenómico (sgRNA). Los resultados obtenidos mediante cultivo del víais y sgRNA tuvieron una concordancia moderada. El estudio mostró que, mientras que el RNA viral fue deteetable durante varias semanas por RT-PCR, el aislamiento del virus y la detección de sgRNA fueron positivos en los primeros 8 días desde el inicio de la enfermedad y raramente se detectaron después del día 8, lo que sugiere que los pacientes con enfermedad leve a moderada podrían sérmenos contagiosos 8 días después del inicio de los síntomas, a pesar de que la prueba RT-PCR siga siendo positiva.
Considerando que la duración de la capacidad de contagio después del inicio de la enfermedad o de la aparición de los síntomas aún es poco entendida, así como los efectos a largo plazo de la misma (conocidos como Síndrome post-COVID, COV1D crónico o long-haul COVID), persiste la necesidad de biomareadores que permitan diferenciar, entre otras cosas, los pacientes que son contagiosos de los que ya no lo son de manera oportuna, con eí propósito de prevenir nuevos contagios y al mismo tiempo establecer mejores políticas de alta médica para el aislamiento individual, así como para hacer un mejor seguimiento de las medidas de tratamiento. Dado que los sgRNA evidencian los intermediarios de la replicación del virus, se sugiere que estos pueden ser un biomarcador útil para suplir dichas necesidades.
BREVE DESCRIPCION
La presente divulgación se refiere a un método para cuantificar un biomarcador de la replicación de un virus humano respiratorio, que es mRNA subgenómico. En un aspecto, el virus humano respiratorio es un coronavirus y más particularmente SARS-CoV-2. DESCRIPCIÓN DETALLADA Método para cuantificar un bi omarcador de la replicad ón viral
La presente divulgación se refiere a un método para cuantificar un biomarcador de la replicación de un virus humano respiratorio, que es mRNA subgenómico. Para efectos de la presente divulgación, un biomarcador o marcador biológico es una sustancia o estructura que puede medirse y evaluarse objetivamente y sirve como indicador de un proceso biológico o patogénico o como indicador de 1a respuesta a una intervención terapéutica. En el contexto de ía presente divulgación, un virus humano respiratorio se selecciona entre, sin limitarse a virus de RNA, virus respiratorio sincitial, virus de parainfluenza, metapneumovirus, rinovirus y coronavirus, en donde se entiende por virus RNA a ios viras que usan importina (IMP) a/bΐ y se seleccionan entre DENV 1-4, virus del Niio Occidental, virus de la encefalitis equina venezolana (VEEV) y la influenza. En un aspecto particular de la presente divulgación, el coronavirus se selecciona entre virus del SARS-COV, viras del SARS-COV-2 y virus del MERS-CoV. En otro aspecto particular, el virus es SARS-COV-2.
En una modalidad, el método de la presente divulgación comprende cuantificar mRNA subgenómico (sgRNA) en una muestra de un paciente infectado con un viras humano respiratorio. En una modalidad particular, el método comprende cuantificar sgRNA de un coronavirus. En otra modalidad particular el coronavirus es SARS-CoV-2. En otra modalidad particular, el sgRNA de SARS-CoV-2 se selecciona entre, sin limitarse a S, 3, E, M, WHSA, 6, 7a, 7b y 8. En otra modalidad particular, el sgRNA es WHSA.
Método para ciiantiflcar sgRNA
La cuantificación del sgRNA de acuerdo con la presente divulgación se puede realizar mediante cualquier método conocido en ia técnica.
En una modalidad, el método para cuantificar sgRNA comprende transcribir de forma inversa rnRNA, amplificar el cDNA obtenido de la transcripción inversa y cuantificar el cDNA amplificado mediante inserción en un plásmido que tiene la secuencia de nucleótidos que se muestra en la SEQ ID NO: 1, o una secuencia de nucíeótidos con 90 a 99 % de identidad de la SEQ ID NO: 1.
Para efectos de ia presente divulgación, el RNA se extrae mediante cualquier método conocido en la técnica. En una modalidad, se emplea un kit comercial para la extracción del RNA. En una modalidad particular, se emplea el kit VN143 Viral RNA Mini Kit (Genolution). En otra modalidad particular, la etapa de extracción comprende un periodo de elución de 120 segundos, temperatura de iisis de 65 °C y tiempo de lisis adicional de 30 segundos.
La transcripción reversa se realiza con cualquier kit de reacción de PCR conocido en la técnica. En una modalidad, el kit comprende iniciadores para sgRNA que se seleccionan entre, sin limitarse a S, 3, E, M, WEISA, 6, 7a, 7b y 8.

Método para hacer seguimiento a un sistema de administración de un medicamento para reducir la replicación viral de un virus respiratorio en la mucosa del tracto respiratorio superior e inferior
En un aspecto adicional de la presente divulgación, se describe un método para hacer seguimiento a un sistema de administración de un medicamento para reducir la replicación viral de un virus respiratorio en la mucosa del tracto respiratorio superior e inferior en un paciente infectado con dicho virus, en donde dicho método comprende
cuantifícar la replicación viral a través de la cuantificación de sgRNA en una muestra de dicho paciente, de acuerdo con el método descrito anteriormente y comparar el resultado de dicha cuantificación con el obtenido de un paciente no tratado.
En una modalidad el sistema de administración comprende administrar un medicamento para reducir la replicación viral. En otra modalidad, el sistema comprende administrar dicho medicamento en un dispositivo que libera una dosis terapéuticamente efectiva de dicho medicamento directamente en los pulmones, en forma de una niebla inhalable. En otra modalidad, el sistema de administración comprende administrar una niebla inhalable de una dosis terapéuticamente efectiva de dicho medicamento en la mucosa deí tracto respiratorio superior e inferior.
En una modalidad, el virus respiratorio es un viras cuya replicación ocurre en el tracto respiratorio, que se selecciona entre, sin limitarse a virus de RNA, viras respiratorio sincitial, virus de parainfluenza, metapneum ovirus, rinovirus y coronavirus. En una modalidad particular, el coronavirus se selecciona entre virus del SARS-COV, virus del SARS-CQV-2 y viras del MERS-CoV. En otra modalidad particular, el virus es SARS- COV-2.
En una modalidad, el medicamento que reduce la replicación viral se selecciona entre, sin limitarse a ivermectina, nitazoxinida, cloroquina, hidroxicloroquina, selamectina, doramectina, eprinomectina, abamectina remdesivir, nafamostat, molnupiravir, bamlanivimab, bamlanivirnab/etesevimab terapia combinada, terapia combinada lopinavir-ritonavir, casirivimab, imdevimab, tociiizumab, etesevimab, VIR-7831, EXQ~ CD24, MIR-19 y moléculas de siRNA, o combinaciones de los mismos. En una modalidad particular el medicamento es una avermectina que se selecciona entre, sin limitarse a ivermectina, selamectina, doramectina, eprinomectina y abamectina. En otra modalidad particular, el medicamento es ivermectina.
En una modalidad se administra ivermectina al 1 % por nebulización durante la fase temprana de la infección. La ivermectina se administra en dosis de 3 mL cada 8 horas durante 5 días. En otra modalidad, se prepara una solución mezclando 3 mL de ivermectina al 1 % con 0,3 mL de solución de dexametasona (en concentración de 4 mg
/ mL) y se administran 3 mL de esta solución al sujeto, directamente a los pulmones, en forma de una niebla inhaíabíe, 3 veces al día durante 5 días.
En una modalidad, para la cuantifi catión de sgRNA, la muestra se obtiene de muestras de hisopado naso-faríngeo. En otra modalidad particular, la muestra se toma a los días 0, 3, 5 y 7 del tratamiento con el medicamento para reducir la repiicación viral y en los mismos días se toman muestras de sujetos que han recibido un placebo, y se realiza la cuantificación de sgRNA tal como se describió anteriormente. En una modalidad particular, en la curva de calibración se interpolan los resultados de cantidad de sgRNA obtenidos por qPCR para cada muestra de cada paciente.
En otra modalidad particular, un Log de la cuantificación de sgRNA menor o igual a 4 o un logaritmo natural de la cuantificación de sgRNA menor o igual a 11, indica una reducción de la repiicación viral que se puede correlacionar con los resultados de pacientes no reactivos para antígeno por quimioluminiscencia (CLÍA).
La presente invención será presentada en detalle a través de los siguientes ejemplos, los cuales son suministrados solamente con propósitos ilustrativos y no con el objetivo de limitar su alcance.
EJEMPLOS
Ejemplo l. Método de detección de mKNÁ subgenómko
Extracción de RNA
El RNA viral se extrajo de muestras de hisopado nasofaríngeo de pacientes con diagnóstico positivo para COVID-19 (mediante prueba PCR) y enfermedad leve a moderada que se distribuyeron aleatoriamente en dos grupos, a saber: Grupo 1 : pacientes positivos que recibieron intervención con el medicamento para reducir la repiicación viral, y Grupo 2: pacientes positivos que recibieron un placebo (solución salina normal 0,9 %).
Se empleó el kit VN143 Viral RNA Mini Kit (Genolution) de acuerdo con las instrucciones del fabricante a excepción de las siguientes condiciones que fueron adaptadas:
* periodo de elución: 120 segundos,
• temperatura de lisis: 65 °C y
® tiempo de lisis adicional: 30 segundos.
RT-PCR y aPCR
La RT-PCR se realizó con el iniciador específico para SARS-CoV-2 WHSA-29950R, que tiene la secuencia: 5'-TCTCCTAAGAAGCTATTAAAAT-3'. La amplificación se realizó con el kit Superscript II (Thermo Fisher Scientific).
El cDNA obtenido de la transcripción reversa se amplificó por qPCR con el kit de PCR que comprende AmpliTaq Gold DNA Polimerasa (ThermoFisher Scientific), con el iniciador forward FAM WHSA-00025F, que tiene la secuencia 5'- CCAACCAACTTTCGATCTCTTGTA-3' BHQI, y el iniciador reverse FAM WHSA- 29925R, que tiene la secuencia 5 '-ATGGGGATAGCACTACTAAAATTA-3 ' BHQI. Las condiciones de amplificación fueron de 40 ciclos de: i) 94 ° C por 30 s, ii) 56 ° C por 30 s, y iii) 72 ° C por 1.5 min.
El fragmento amplificado tiene una secuencia de 584 nucleótidos como se muestra en la SEQ ID NO: 22.
Curva de calibración
Se insertó el fragmento amplificado en el piásmido pORF de 2596 bp. El plásrnido pORF tiene regiones de corte para las enzimas Aflií y EcoRV, en donde se inserta el fragmento amplificado, tal como se obsesiva en la Figura 6,
El fragmento amplificado se insertó en cuatro concentraciones conocidas (100, 1000, 10000 y 1000000 de copias / mi. La curva de calibración se generó después de medir ios
valores de Ct de cuatro estándares con diferentes concentraciones del plásmido 10z, 103, 104 y 103. La curva de calibración obtenida se observa en la F1G. 5.
Ejemplo 2. Seguimiento de un tratamiento anti-viral por determinación de la replicaeión viral
Se realizó un estudio clínico doble ciego aleatorizado (ClinicalTrials.gov NCT04595136) con un total de 60 adultos, con menos de tres días de sintomatología y prueba positiva para SARS-CoV-2 determinada por PCR. Treinta pacientes recibieron un medicamento para reducir la replicaeión viral (Grupo 1) y treinta pacientes recibieron un placebo (Grupo 2).
Los sujetos infectados con SARS CoV 2 que calificaron para ser incluidos en el estudio, recibieron ivermeetina al 1 % administrada en forma de nebulización durante la fase temprana de la infección a dosis de 3 mL (0,03 g) cada 8 horas, durante 5 días.
De manera similar, una solución de ivermeetina para nebulización se preparó mezclando 3 mi. de ivermeetina al 1 % (10 mg / mL, proporcionada por Vecol, Bogotá Colombia htps://yccol.com.co) con 0,3 mL (1,2 mg) de solución de dexametasona (a 4 mg /mL), glicerol y propilenglicol.
Se administraron 3 mL de la solución al sujeto directamente en los pulmones en forma de una niebla inhalable. La ivermeetina combinada con dexametasona se administró por un nebulizador 3 veces ai día durante 5 días.
Se tomaron muestras de los dos grupos de pacientes y se procesaron tal como se describe en el Ejemplo 1. Las muestras se tomaron los días 0 (antes de iniciar la intervención), 3, 5 y 7 durante el tratamiento. Las muestras fueron analizadas mediante la prueba RT-PCR para COVID-19 y por detección de sgRNA como se describe en el Ejemplo 1.
Para la solución de ivermeetina, el sistema de administración redujo la replicaeión viral, medida por sgRNA y consecuentemente la carga de viras SARS Co V 2 activos en el tracto respiratorio superior e inferior en más del 90 %, dando como resultado una mejoría clínica significativa, incluyendo la severidad y la duración de la enfermedad.
Adicionalmente, se observó una disminución estadísticamente significativa en ia replicación viral medida por sgRNA después de la administración de la combinación de ivermectina y dexametasona, en comparación con un placebo.
En la F1G. 1 se puede observar la diferencia en la replicación viral entre el Grupo 1 (ivermectina + dexametasona) y el Grupo 2 (placebo) usando la prueba estadística de Wilcoxon-Mann-Whitney. Se observa una distribución heterogénea de los valores de sg RNA en los dos grupos (placebo en negro vs. medicamento en blanco), con un desplazamiento hacia la izquierda en el grupo de tratamiento a partir del día quinto (FIG. 1 B)
La FIG. 2 ilustra el comportamiento de la cuantificación del sgRNA en los días 0, 3, 5 y 7 para el Grupo 2 (placebo). Como se puede observar, no hay una reducción significativa de la replicación viral. En contraste, la Figura 3 ilustra el comportamiento para el Grupo
1 (ivermectina + dexametasona), en donde se puede observar una reducción significativa de 1a replicación viral después del día 3, con una disminución de entre 6 y 7 unidades logarítmicas hacia el día 7. Adicionalmente fue posible observar que la reducción de la replicación viral se asocia directamente con la severidad de los síntomas, 1a cual también tuvo una reducción significativa en eí Grapo 1, en comparación con el Grupo 2.
Si se establece un punto de corte arbitrario en Log de la cuantificación de sgRNA igual a 4 en el día 7, entre el paciente con el mayor valor cuantificado de sgRNA del Grupo 2 en comparación con el Grupo 1, se observará que más del 50 % de los pacientes del Grapo
2 exhibe mayor replicación viral que los pacientes del Grapo 1 , lo que permite ilustrar el efecto del medicamento (reducción de la replicación viral), dado que existe una diferencia estadísticamente significativa en la disminución de sgNA entre los dos grupos (p=0.02434), como se puede observar en la FIG. 4 que ilustra cómo el sobrelapamiento entre los dos grupos disminuye progresivamente día a día hasta el día 7 en donde no se observa ningún sobrelapamiento entre los dos grupos.
Finalmente, se utilizaron contra muestras previamente analizadas por RT-PCR y sgRNA para realizar la prueba de antígenos por quimioluminiscencia. Con los datos obtenidos se encontraron evidencias que correlacionan estos hallazgos con los resultados de
pacientes no reactivos para antígeno por CLIA. Corno se observa en la FIG. 7, las muestras con un valor de Ln de la cuantiílcación de sgRNA menor o igual a 11 se correlacionan con resultados no reactivos en la prueba de antigenos. Los resultados obtenidos están en concordancia con el hecho de que al cesar la replicación viral (menor número de copias de sgRNA), también se detiene la producción de ia proteina N detectada mediante la prueba de antigenos. La ventaja de la cuantificación de sgRNA como biomarcador, tal como se describe en la presente divulgación, es que permite detectar virus activos (resultado “reactivo” en la prueba de antígenos) y adicionalmente permite hacer una cuantificación de los mismos.
De acuerdo con lo anterior, se sugiere que el sgRNA es un biomarcador apropiado para medir (cuantificar) la replicación viral y determinar si hay o no reducción de la replicación viral en un paciente infectado con un virus respiratorio, como consecuencia de la administración de un medicamento para reducir la replicación viral de dicho virus respiratorio en el tracto respiratorio superior e inferior.
Claims
1. Un método para cuantificar 1a repiicación viral en una muestra de un paciente infectado con un virus respiratorio, que comprende cuantificar mRNA subgenómico (sgRNA) en dicha muestra.
2. El método según la Reivindicación 1, en donde el virus respiratorio se selecciona entre virus de RNA, influenza, virus respiratorio smcitial, virus de parainfluenza, metapneumovirus, rinovirus, coronavirus.
3. El método según la Reivindicación 2, en donde el coronavirus se selecciona entre virus del SARS-COV, virus del SARS-COV-2 y virus del MERS-CoV.
4. El método según la Reivindicación 3, en donde el coronavirus es SARS-CoV-2.
5. Un método para hacer seguimiento a un sistema de administración de un medicamento para reducir la repiicación viral de un viras respiratorio en la mucosa del tracto respiratorio superior e inferior en un paciente infectado con dicho virus, que comprende cuantificar la repiicación viral de acuerdo con el método de la Reivindicación 1
6. Ei método según la Reivindicación 5, en donde un Log de la cuantifícación de sgRNA menor o igual a 4 o un logaritmo natural de la cuantifícación de sgRNA menor o igual a 11, indica una reducción de la repiicación viral que se puede correlacionar con ios resultados de pacientes no reactivos para antígeno por quimiolummiscencia.
