Abstract
Изобретение относится к области фармацевтики, а именно фармакологической композиции для лечения железодефицитной анемии (ЖДА), содержащей сульфат железа(II), отличающейся тем, что она дополнительно содержит гексацианоферрат железа-калия-натрия, сульфат калия и микроцеллюлозу при следующем соотношении компонентов, мас. %: KNaFe[Fe(CN)6] - от 20 до 65; FeSO4 - от 2 до 10; K2SO4 - от 2 до 15; микроцеллюлоза - остальное. Техническим результатом заявляемого изобретения является создание фармакологической композиции для лечения ЖДА, которая сочетает высокую лечебную эффективность, хорошую переносимость и низкую токсичность по сравнению с существующими препаратами.

Заявляемое изобретение относится к медицине и ветеринарии, в частности к фармакологическим препаратам, применяемым для лечения железодефицитной анемии (ЖДА) различной этиологии у людей и животных.
Группами риска развития анемии являются женщины детородного возраста, беременные и кормящие грудью, дети в период быстрого роста, доноры. Даже в развитых странах Европы от 7 до 14% женщин репродуктивного возраста страдают железодефицитным малокровием. В странах с более низким уровнем жизни частота анемии достигает 50-60%.
Таким образом, проблема профилактики и лечения этого состояния имеет большое социальное значение. Железодефицитная анемия является крайней степенью дефицита железа в организме, когда, наряду с изменениями параметров феррокинетики и снижением содержания гемоглобина, формируются морфофункциональные нарушения со стороны различных органов и систем, снижается уровень антиоксидантной защиты, ухудшается физиологический статус организма в целом.
Широко известно использование в качестве средства для лечения ЖДА неорганических солей двухвалентного железа (железа (II)), однако отмечено [Scheider W. Arzneim Forsch. Drug Res., 1987, 37 (1), N 12, p. 92-95], что препараты, содержащие простые соли железа (II), вызывают побочные эффекты и плохо переносятся пациентами. Токсичность этих препаратов объясняется их способностью хорошо абсорбироваться, что приводит к избытку железа в организме, и вызванному окислением ферроионов образованию радикалов ОН, способных воздействовать на мембраны клеток.
Также известно [Лопахин В. К. и др. Клиническая фармакология с международной номенклатурой лекарств. Медицина, 1988, ч. II] использование в качестве средства для лечения ЖДА неорганических солей железа (III), например хлорида железа FeC13. Однако ферроионы подвергаются гидролизу на более ранних этапах следования по ЖКТ и в кишечник попадают преимущественно в виде гидроксидов, плохо усваиваемых рецепторами железа.
Существует большое количество фармацевтических композиций (лекарственных форм) на основе простых солей железа (II), в частности сульфата, в которых различными добавками стараются уменьшить плохую переносимость лекарства. Известна фармацевтическая композиция,
представляющая собой капсулы, содержащие 0,150 г сульфата железа (II) во множестве мелких гранул, покрытых слоями полимерного материала, растворяющегося в ЖКТ. Постепенное высвобождение сульфата железа позволяло избежать высоких локальных концентраций, приводящих к непереносимости [Jarry Н., Rote M.D. Intern. Record Med., 1958, 171, p. 87-91].
Также известна [Largiader A. Schweiz. Rundschau Med. (praxis), 1973, 62, p. 173-175] фармацевтическая композиция тардиферон, включающая на одно драже 0,08 г железа в виде сульфата железа (II) и 0,08 г мукопротеазы, являющейся макромолекулярной фракцией слизистой оболочки кишечника овец; мукопротеаза снижает способность железа (II) к окислению.
Также известна [Israels M.C.G., Simmons A.V. Lancef, 1967, N 1, p. 1297-1299] фармацевтическая композиция, содержащая 0,105 г сульфата железа (II) и 0,50 г аскорбиновой кислоты (ферроградумет - 500), повышающей абсорбцию железа и участвующей во внутриклеточном метаболизме железосвязывающих белков.
Также известна [патент США N 3076747, НКИ 424-147, 1963] фармацевтическая композиция, включающая от 1 до 12 эквивалентов янтарной кислоты на 1 эквивалент железа в виде сульфата железа (II). Янтарная кислота увеличивает всасывание железа. Применение янтарной кислоты так же, как и аскорбиновой, позволяет уменьшить дозу железа, применяющегося для лечения.
Тем не менее, железо (II), включенное в известные лекарственные формы, вступает в окислительно-восстановительные реакции; таким образом, добавки, введенные в фармацевтические композиции, снижают плохую переносимость солей железа (II), но не устраняют ее совсем.
Существует довольно большое количество энтеральных форм железосодержащих препаратов [Лепахин В.К., Белоусов Ю.Б., Моисеев В. С. Клиническая фармакология с международной номенклатурой лекарств. "Медицина", 1988]. Наиболее распространенными являются зарубежные препараты, включающие сульфатные формы железа: ферроплекс, конферон, тардиферон. Находят также применение ферроцерон (натриевая соль ортокарбоксибензоилферроцел) и сироп алоэ с хлоридом железа. За рубежом имеются также препараты железа на основе глюконата и фумарата.
Практически все имеющиеся железосодержащие препараты для приема внутрь плохо переносятся пациентами [Токарев Ю.Н., Сеттарова Д.А. Механизм воздействия и клинические проявления перегрузки организма железом при наследственном гемахроматозе // Гематология и трансфузиология. 1987, N 2, с. 52-57]. Они вызывают расстройство желудочно-кишечного тракта. Освобождающееся в слизистой кишечника
Fe2+ денатурирует протеин этой оболочки и в результате отслаивается слой мукозы со стенки желудка. Контакт с кислотами желудка приводит к его воспалению. При этом возможны тошнота, рвота, диаррея. Их выраженность тем сильнее, чем больше останется в кишечнике невсосавшегося железа. При приеме препаратов железа внутрь часто возникают запоры, так как железо связывает в кишечнике сероводород, являющийся физиологическим стимулятором перистальтики. Чтобы повысить биосовместимость препаратов, соли железа помещают в труднорастворимые в желудочном соке оболочки (ферроградулит, феоспан Югославия). Эти препараты довольно хорошо переносимы пациентами, но в то же время их лечебная эффективность снижается из-за недостаточного всасывания железа из таблеток.
В связи с нежелательными явлениями иногда оказывается невозможным провести курс лечения до конца и достигнуть желаемых результатов. Поэтому поиск новых средств энтерального лечения ЖДА остается актуальным.
Наиболее близким к заявляемой композиции является сульфат двухвалентного железа FeS04 [Лопахин В.К., Белоусов Ю.К., Моисеев В. С. Клиническая фармакология с международной номенклатурой лекарств. Медицина, 1988, ч. II], выбранный нами в качестве прототипа. Сульфат железа (II) хорошо растворим в воде, в желудке находится в форме гидратированного иона Fe(H20)62+ и хорошо усваивается рецепторами железа. Но при этом ионы железа (II) окисляются в желудочно-кишечном тракте (ЖКТ) кислородом воздуха до ионов железа (III), которые при значении pH выше 3 подвергаются гидролизу. Таким образом, сульфат железа (II) является источником как ионов железа (II), так и гидроксидов железа (III) [Erm J. et al. Arzneim Forsch. Drug Res., 1984, 34 (III), N 11, p. 1555-1559], поэтому, с одной стороны, часть железа из-за гидролиза переходит в не усваиваемую форму, вызывающую отрицательные побочные эффекты (тошноту, диарею, и т.д.), а с другой стороны, избыточное количество иона железа(П), может привести к образованию радикалов ОН, повреждающих клеточные мембраны, к избыточному накоплению железа в организме и даже к токсическому отравлению.
Техническим результатом заявляемого изобретения является создание фармакологической композиции для лечения ЖДА, которая сочетает высокую лечебную эффективность, хорошую переносимость и низкую токсичность по сравнению с существующими препаратами.
Для достижения указанного технического результата предлагается фармакологическая композиция для лечения железодефицитной анемии, з
содержащая сульфат железа(П), гексацианоферрат железа-калия-натрия, сульфат калия и микроцеллюлозу, при следующем соотношении компонентов в массовых процентах %:
KNaFe[Fe(CN)6] от 20 до 65
FeS04 от 2 до 10
K2S04 от 2 до 15
Микроцеллюлоза остальное
Технологически фармакологическую композицию для лечения железодефицитной анемии выполняют в форме таблеток, капсул, порошков, жевательных конфет.
Новизна изобретения заключается в том, что в состав композиции вводится гексацианоферрат железа-калия-натрия, осажденный на целлюлозу. Введение ферроцианида железа-калия-натрия в композицию обеспечивает регуляцию всасывания в организме необходимого количества железа. При этом композиция приобретает дополнительные свойства по выведению из организма радионуклидов и токсичных тяжелых металлов (таллия, свинца, кадмия, ртути, мышьяка и др.). Наличие в составе микроцеллюлозы, обладающей восстановительными свойствами, препятствует окислению железа (II).
Кроме этого в состав композиции вводится сульфат калия, как регулятора перистальтики для предотвращения образования запоров.
Способ приготовления композиции заключается в смешении компонентов. При этом первоначально все компоненты, кроме микроцеллюлозы, замешивают в воде в объемном соотношении 1:1, затем добавляется необходимое количество микроцеллюлозы. После тщательного промешивания композиция высушивается до постоянного веса.
Технических решений, совпадающих с совокупностью существенных признаков заявляемого изобретения, не выявлено, что позволяет сделать вывод о соответствии заявляемого изобретения условию патентоспособности «новизна».
Заявляемые существенные признаки, предопределяющие получение указанного технического результата, явным образом не следуют из уровня техники, что позволяет сделать вывод о соответствии заявляемого изобретения условию патентоспособности «изобретательский уровень».
Условие патентоспособности «промышленная применимость» подтверждается конкретными примерами применения композиции.
Пример 1
Состав композиции:
KNaFe[Fe(CN)6] - 20%;
FeS04 - 10%;
K2S04 - 15%;
Микроцеллюлоза - 55%.
Больная M, возраст 56 лет, с диагнозом железодефицитная анемия и уровнем гемоглобина в крови 94 г/л, получала препарат в количестве 3 г/сутки в течение 60 дней. Уровень гемоглобина через 30 дней составил 110 г/л, к концу приема - 120 г/л. Отрицательных побочных эффектов при применении композиции не наблюдалось.
Пример 2
Состав композиции:
KNaFe[Fe(CN)6] - 44%;
FeS04 - 6%;
K2S04 - 15%;
Микроцеллюлоза - 35%.
Больная Н, возраст 49 лет, с диагнозом железодефицитная анемия и уровнем гемоглобина в крови 90 г/л, получала препарат в количестве 3 г/сутки в течение 60 дней. Уровень гемоглобина через 30 дней составил 108 г/л, к концу приема - 124 г/л. Отрицательных побочных эффектов при применении композиции не наблюдалось.
Пример 3
Состав композиции:
KNaFe[Fe(CN)6] - 65%;
FeS04 - 2%;
K2S04 - 2%;
Микроцеллюлоза - 31%.
Больная А, возраст 33 года, с диагнозом железодефицитная анемия и уровнем гемоглобина в крови 82 г/л, получала препарат в количестве 3 г/сутки в течение 60 дней. Уровень гемоглобина через 30 дней составил 102 г/л, к концу приема - 122 г/л. Отрицательных побочных эффектов при применении композиции не наблюдалось.

1. Фармакологическая композиция для лечения железодефицитной анемии, содержащая сульфат железа(П), отличающаяся тем, что она дополнительно содержит гексацианоферрат железа-калия-натрия, сульфат калия и микроцеллюлозу при следующем соотношении компонентов, мас.%: KNaFe[Fe(CN)6] от 20 до 65
FeS04 от 2 до 10
K2S04 от 2 до 15
