マイクロ波は、消化器、肝臓、膀胱、前立腺、子宮、血管、腸管等の生体組織を低温で凝固(固定化)できることが知られている。そして、マイクロ波を用いた手術支援用の種々のデバイスが開発されている。
マイクロ波を照射する医療用処置具は多数報告されているだけでなく、市販化されている。例えば、以下が報告されている。

本発明の医療用処置具(100)(200)に関し、ホルダーカバー(19)は、マイクロ波照射による発熱放射を防ぐ(参照:図1、2)。これにより、本発明の医療用処置具(100)(200)は、マイクロ波照射対象ではない生体組織の熱損傷を防ぐことができる。
ホルダーカバー(19)の材質は、マイクロ波照射による発熱放射を防ぐことができれば特に限定されないが、例えば、樹脂素材、特に、ポリエーテルエーテルケトン樹脂等を例示することができる。
本発明の医療用処置具(100)(200)に関し、マイクロ波印加用アンテナの内側突起(20)は、図3及び図4に記載のように、ホルダーカバー(19)に格納されているホルダー(22)の内側(矢印a)に向かって伸長している。マイクロ波印加用アンテナの内側突起(20)の材質は、特に限定されないが、セラミック、樹脂素材(特に、ポリエーテルエーテルケトン樹脂)、ステンレス等を例示することができる。
マイクロ波印加用アンテナの内側突起(20)の先端は、荷重方向(矢印b)からの荷重があった場合に、ホルダー(22)の内側に接触し、該突起を支点として荷重がかかる。これにより、絶縁体(14)の強度を向上させることができる。
本発明の医療用処置具(100)(200)に関し、マイクロ波印加用アンテナ(12)の刃線の先端での垂直方向の長さ(矢印c)が、刃線の末端での垂直方向の長さ(矢印d)に対して30%~80%であることが好ましい(参照:図5、図6)。
マイクロ波受手用アンテナ(10)の峰の先端での垂直方向の長さ(矢印e)が、峰の末端の垂直方向の長さ(矢印f)に対して30%~80%であることが好ましい(参照:図5、図6)。
このように両アンテナ全体の形状が先端に向けてシャープになっていくので、マイクロ波加熱及び生体組織・癒着の剥離の際、組織への挿入が容易となる。
本発明の医療用処置具(200)に関し、マイクロ波印加用アンテナ(12)及びマイクロ波受手用アンテナ(10)が閉じた状態では、マイクロ波印加用アンテナ(12)及びマイクロ波受手用アンテナ(10)の末端から先端への伸長(参照:線g)は処置具の水平伸長方向線(参照:線h)に対して10°~20°のカーブ(cu1)を有する(参照:図12)。加えて、マイクロ波受手用アンテナ(10)が開いた状態では、マイクロ波受手用アンテナ(10)の末端から先端への伸長は処置具の水平伸長方向線に対して20°~40°のカーブを有する。
上記カーブにより、両アンテナが術者の視野(マイクロ照射対象組織の視野)を見やすくする効果を有する。
さらに、上記開いた状態と閉めた状態でのカーブの範囲が異なる(マイクロ波受手用アンテナのカーブをマイクロ波印加用アンテナのカーブより強くする)ことにより、マイクロ波受手用アンテナを閉じる際に、徐々にマイクロ波印加用アンテナのカーブがマイクロ波印加用アンテナに沿うように変形することで、絶縁体(14)を含むマイクロ波印加用アンテナ(12)とマイクロ波受手用アンテナがハサミとして機能する。
本発明の医療用処置具(100)(200)に関し、マイクロ波受手用アンテナ(10)の刃線の末端から先端への伸長(線i)は、刃線の水平伸長(線j)に対して2°~15°のR形状(R1)を有する(参照:図7、図8)。このR1形状は、「潰し切るような切断」を改善することに効果がある。
同様に、マイクロ波受手用アンテナ(10)の峰の末端から先端への伸長(線k)は、峰の水平伸長(線l)に対して2°~15°のR形状(R2)を有する。同様に、マイクロ波印加用アンテナ(12)の峰の末端から先端への伸長(線m)は、峰の水平伸長(線n)に対して2°~15°のR形状(R3)を有する。
これらのR2、R3形状は、「マイクロ波照射対象組織を傷つけることなくスムーズに該組織に挿入する」に効果がある。
これらのR1、R2及びR3形状を有するアンテナは、R形状を有しないアンテナと比較して、潰し切るような切断にはなく、さらに、マイクロ波照射対象組織を傷つけることなくスムーズに該組織に挿入することでる。
すなわち、マイクロ波受手用アンテナのR形状はマイクロ波照射対象組織の切断向上効果を有する。
本発明の医療用処置具(100)(200)に関し、マイクロ波受手用アンテナ(10)は図13に記載のようなエッジ形状を有する。エッジ形状を示すt は、 板厚(T)×0.2~1.5の範囲内である。
マイクロ波受手用アンテナ(10)のエッジ形状は、図13に記載のようなエッジ形状を有しないアンテナと比較して、切れ味が向上している。
本発明の医療用処置具(100)(200)に関し、図9及び図10に記載のように、ホルダーカバー(19)は、ホルダー(22)を収納している。
ホルダー(22)の材質は、特に、限定されないが、ステンレス鋼材を例示することができる。ホルダー(22)は、マイクロ波がマイクロ波受手用アンテナ(10)以外に飛ぶことを防ぐことができる。これにより、ホルダー(22)は、マイクロ波を効率的にマイクロ波受手用アンテナ(10)に飛ばすことができる。
アンテナカバー(114)は、マイクロ波印加用アンテナと同軸ケーブルの接続部を収納する。
さらに、本発明の医療用処置具(100)に関し、アンテナカバー(114)は、マイクロ波印加用アンテナと同軸ケーブルの接続部を収納するストレート部(101)を有する。ストレート部(101)は、末端側から該接続部を収納できる中空構造(中空管)であり、一定の軸の長さ(例えば、5 mm)を有することにより、マイクロ波印加用アンテナが実質的に同軸ケーブルとストレートに接続(勘合)することができるので、同軸ケーブルの中心導体の座屈折れを防止することができる。加えて、アンテナカバー(114)の材質は、特に、限定されないが、絶縁体の樹脂(特に、PEEK樹脂)を例示することができる。
本発明の医療用処置具(100)は、図14に記載のように、さらに必要に応じて、ハンドル1(102)、ハンドル2(103)、アーム1(104)、アーム2(105)、指掛け1(106)、指掛け2(107)、指環1(108)及び指環2(109)を有する。
指掛け1(106)及び指環1(108)は、それぞれ、処置具の中心線(線o)に対して、指掛け2(107)及び指環2(109)と実質的に対象である。これにより、術者が右左利きのどちらでも本発明の医療用処置具(100)を容易に操作することができる。
ハンドル、アーム、指掛け及び指環の材質は、特に限定されないが、ポリカーボネート(特に、ガラス繊維入りポリカーボネート)を例示することができる。
本発明の医療用処置具(100)は、図15に記載のように、ホルダーカバー(19)の末端側に同軸ケーブル保護部材(110)を有する。同軸ケーブル保護部材(110)は、ハンドル1(102)及びハンドル2(103)が同軸ケーブル(17)に接触するのを防ぐために、ハンドル1(102)及びハンドル2(103)と同軸ケーブル(17)の間に設置されている。同軸ケーブル保護部材(110)が、該位置に設置されていることにより、同軸ケーブル(17)を保護する。さらに、同軸ケーブル保護部材(110)がハンドル1(102)及びハンドル2(103)と重なり合うことにより、医療用処置具(100)の全体の剛性(特に、ホルダーカバー(19)若しくはアンテナとハンドル1(102)及びハンドル2(103)の接触部の剛性)を向上させている。
保護部材の材質は、マイクロ波照射による発熱放射を防ぐことができれば特に限定されないが、例えば、樹脂素材、特に、ポリエーテルエーテルケトン樹脂等を例示することができる。
本発明の医療用処置具(100)のマイクロ波受手用アンテナ(10)の末端は、好ましくは、図23に記載のように、主支持板(112)及び補助支持板(113)と結合している。また、主支持板(112)は、ヘミング曲げ等により、複数の板で構成されていても良い。より具体的には、マイクロ波受手用アンテナ(10)の末端と補助支持板(113)の先端は、主支持板(112)の先端を挟んで結合している。結合方法の一例として、図24に記載のように、複数の箇所(r)をスポット溶接をする。
マイクロ波受手用アンテナ(10)の末端が、主支持板(112)及び補助支持板(113)の先端と結合することにより、アンテナとハンドルの接触部の剛性及びハンドルの剛性の向上させることができる。
主支持板及び補助支持板の材質は、特に限定されないが、ステンレス鋼(SUS304)を例示することができる。
本発明の医療用処置具(100)に関し、図16に記載のように、ハンドル1(102)及び/又はハンドル2(103)の操作により、ハンドル2(103)の先端部が移動しても、ハンドル2(103)は、該カバーとハンドル2(103)の先端部の隙間に組織が入り込まないような先端形状(111)を有する。
本発明の医療用処置具(200)は、図2に記載のように、さらに必要に応じて、シャフト(201)、ローテーター(202)、ハンドスイッチ(203)、レバー(204)及びグリップ部(205)を含む本体カバー(213)を有する。レバー(204)は、揺動自在に取り付けられており、該揺動の動きは、自体公知のスライダクランク機構により、後に説明するピンを有する牽引ロッド(207)の前後の動きに変換される。
また、ローテーター(202)は、後に説明するように、シャフト(201)と共に360°回転できる機構になっている。
医療用処置具(200)に関し、図17に記載のように、絶縁体(14)の先端は凹又は凸形状でありかつマイクロ波印加用アンテナ(12)の先端は凸又は凹形状であることが好ましい。絶縁体(14)とマイクロ波印加用アンテナ(12)は凹凸結合している。これにより、絶縁体(14)は、マイクロ波印加用アンテナ(12)からは脱落することを抑制している。
本発明の医療用処置具(200)は、図18に記載のように、先端(中心導体)が突起形状を有するセミリジットケーブル(206)及び末端に陥没形成されたスリットを有するセミリジットケーブル結合溝を有するマイクロ波印加用アンテナ(12)であることが好ましい。該突起と該結合溝による結合構造(嵌合構造、接続構造)により、マイクロ波印加用アンテナ(12)と中心導体の結合を安定的に接続させることができるので、セミリジットケーブル(206)とマイクロ波印加用アンテナ(12)の切断(結合解除)を防ぐことができる。
図19に記載のように、本発明の医療用処置具(200)は、ホルダー(22)を貫通しかつ絶縁体(14)の内側方向に向かって伸長する突起(例えば、ピン)である「絶縁体(14)内側面に向かって伸長する突起(208)」を有する。絶縁体(14)は、該突起を収納するための突起収納溝(例えば、凹み形状)である「絶縁体(14)内側面の突起収納溝(209)」を有する。該突起が該収納溝に収納されることは、マイクロ波受手用アンテナ(10)に穴形状を形成しないことにより、マイクロ波受手用アンテナの強度を維持することができる。
さらに、図19に記載のように、本発明の医療用処置具(200)は、ホルダー(22)を貫通しかつ本処置具の上側方向に向かって伸長する突起(例えば、ピン)である「絶縁体に向かって伸長する突起(210)」を有する。絶縁体(14)は、該突起を収納するための突起収納溝(例えば、凹み形状)である「絶縁体の突起収納溝(211)」を有する。該突起が該収納溝に収納されることは、マイクロ波受手用アンテナに穴形状を形成しないことにより、マイクロ波受手用アンテナの強度を維持することができる。
本発明の医療用処置具(200)は、異なる方向の2つの突起と2つの該突起の収納溝の結合によりマイクロ波受手用アンテナ(10)と絶縁体(14)を固定している。これにより、2つのアンテナ(特に、マイクロ波印加用アンテナ(12))の強度を維持しながら2つのアンテナの抜けとめ防止を達成することができる。
突起(ピン)の材質は、特に限定されないが、ステンレス鋼(SUS304)を例示することができる。
本発明の医療用処置具(200)は、図20に記載のように、先端に本処置具の内側方向に伸長するピン(216)を有する牽引ロッド(207)(材質は、例えば金属)を有し、かつ、マイクロ波受手用アンテナ(10)の峰側末端の穴である「マイクロ波受手用アンテナ峰側末端の穴(212)」を有する。該ピンは該穴に挿入されている。
図20に記載のように、牽引ロッド(207)が本処置具の末端方向に移動することにより(参照:矢印p)、牽引ロッド(207)のピン(216)が同様に末端方向(参照:矢印p)に移動し、マイクロ波受手用アンテナ末端の突起(208)が支点となり、マイクロ波受手用アンテナ(10)の回転動作へ変更される(参照:矢印q)。具体的には、牽引ロッド(207)が本処置具の末端方向に移動することによりマイクロ波受手用アンテナ(10)が閉まり、該ロッドが本処置具の伸長方向に移動することによりマイクロ波受手用アンテナ(10)が開く。
本発明の医療用処置具(200)は、上記のマイクロ波受手用アンテナ(10)の開閉操作機構を採用することにより、ホルダーカバー(19)全体のスリム化を達成している。ホルダーカバー(19)全体のスリム化は、医療用処置具(1)の先端を細くすることができるので、該処置具の先端を微細な組織や腫瘍へ挿入することを容易にしている。
本発明の医療用処置具(200)は、図21に記載のように、ローテーター(202)、該ローテーターの内部を貫通しているシャフト(201)、本体カバー(213)、該本体カバーに収納されているセミリジットケーブル回転用コネクタ(218)及び同軸ケーブル回転用コネクタ(217)を有しても良い。
セミリジットケーブル(206)及び同軸ケーブル(17)は、それぞれ、セミリジットケーブル回転用コネクタ(218)及び同軸ケーブル回転用コネクタ(217)を介して結合している。
両コネクタにより、ローテーター(202)を回転することによりシャフト(201)が360°回転して、アンテナも360°回転することができる。
本発明の医療用処置具(200)は、図22に記載のように、ハンドスイッチ(203)、本体カバー(213)、該本体カバーに収納されたハンドスイッチの動力を受け取るマイクロ波照射基板(214)、及び該本体カバーに収納されたスイッチカバー(215)を有しても良い。スイッチカバーを(215)の材質は特に限定されないが、エラストマー等を例示することができる。加えて、スイッチカバー(215)はハンドスイッチ(203)とマイクロ波照射基板(214)の間に位置する。ハンドスイッチ(203)は直接マイクロ波照射基板(214)に接触しない。スイッチカバー(215)は、マイクロ波照射基板(214)の防水・防静電気効果となり、マイクロ波照射基板(214)の経年劣化防止となる。
本発明の医療用処置具(200)のホルダーカバー(19)は、必要に応じて、図17に記載のように、末端側の形状がテーパー形状(219)であっても良い。該テーパー形状は、ホルダーカバー(19)が伸長方向に抜けるのを防ぐことができ、さらに、末端方向から容易に取り外することができる。
さらに、シャフト(201)は、図10に例示されるように、1以上又は複数のツメ形状部(220)を有しても良い。さらに、ホルダーカバー(19)は、図17に記載のように、該ツメ形状部を収納する1以上又は複数の貫通孔(221)を有しても良い。該ツメ形状部が該貫通孔に挿入することにより、ホルダーカバー(19)の脱落を防止することができる。
本発明者らは、図1に記載の医療用処置具(100)を使用して豚肝臓組織を凝固し、凝固面を確認した。図1に記載の医療用処置具(100)を使用した場合には、図25(A)に記載のように、均等に凝固を行えたことを確認した。一方、従来の医療用処置具を使用した場合には、図25(B)に記載のように、凝固の中心に凝固が弱い部分があり凝固ムラを確認した。
さらに、スパーク発生を確認しなかった。
図2に記載の医療用処置具(200)は、図1に記載の医療用処置具(100)と同様に、均等に凝固を行うことができることを確認した。
さらに、本発明者らはスパーク発生を確認しなかった。

- 医療用処置具であって、
マイクロ波受手用アンテナを含む第1電極、
絶縁体を含むマイクロ波印加用アンテナを含む第2電極、ここで、該絶縁体はマイクロ波受手用アンテナに対向する面に設置されており、
中心導体及び外部導体を含む同軸ケーブル又はセミリジットケーブル、ここで、該中心導体の先端及び該外部導体の先端は、それぞれ、該マイクロ波印加用アンテナの末端及び該マイクロ波受手用アンテナの末端に直接又は間接的に接続しており、
マイクロ波伝送用同軸ケーブル、
ホルダーカバー、ここで、該ホルダーカバーは、該マイクロ波印加用アンテナ及び該マイクロ波受手用アンテナの末端側の一部を収納しており、並びに
マイクロ波印加用アンテナの内側突起、ここで、該突起は、該ホルダーカバーの内側に向かって伸長している、
医療用処置具。
- 前記マイクロ波印加用アンテナの刃線の先端での垂直方向の長さが、刃線の末端での垂直方向の長さに対して30%~80%であり、かつ、
前記マイクロ波受手用アンテナの峰の先端での垂直方向の長さが、峰の末端の垂直方向の長さに対して30%~80%である、
請求項1に記載の医療用処置具。
- 前記マイクロ波印加用アンテナ及び前記マイクロ波受手用アンテナが閉じた状態では、該マイクロ波印加用アンテナ及び該マイクロ波受手用アンテナの末端から先端への伸長は処置具の水平伸長方向線に対して10°~20°のカーブを有し、かつ
該マイクロ波印加用アンテナ及び該マイクロ波受手用アンテナが開いた状態では、該マイクロ波受手用アンテナの末端から先端への伸長は処置具の水平伸長方向線に対して10°~40°のカーブを有する、
請求項1又は2に記載の医療用処置具。
- 前記マイクロ波受手用アンテナの刃線の末端から先端への伸長は、刃線の水平伸長に対して2°~15°のR形状を有し、かつ
該マイクロ波受手用アンテナの峰の末端から先端への伸長は、峰の水平伸長に対して2°~15°のR形状を有し、かつ
前記マイクロ波印加用アンテナの峰の末端から先端への伸長は、峰の水平伸長に対して2°~15°のR形状を有する、
請求項1~3のいずれか1に記載の医療用処置具。
- 前記マイクロ波受手用アンテナはエッジ形状を有し、ここで、該エッジ形状は、 板厚×0.2~1.5である、
請求項1~4のいずれか1に記載の医療用処置具。
- さらに、ホルダー及びアンテナカバーを有し、該ホルダーは前記ホルダーカバーに収納されており、かつ該アンテナカバーは前記マイクロ波印加用アンテナと前記同軸ケーブル又は前記セミリジットケーブルの接続部を収納する、

- 前記アンテナカバーは、前記マイクロ波印加用アンテナと前記同軸ケーブルの接続部を収納するストレート部を有する、

- 前記医療用処置具は、鉗子を形成するために、ハンドル1、ハンドル2、アーム1、アーム2、指掛け1、指掛け2、指環1及び指環2を有し、ここで、該指掛け1及び指環1は、それぞれ、処置具の中心線に対して、指掛け2及び指環2と実質的に対象に位置する、

- 前記ホルダーカバーは、該カバーの末端側に同軸ケーブル保護部材を有し、ここで、該同軸ケーブル保護部材は、前記ハンドル1及び前記ハンドル2と前記同軸ケーブルの間に設置されている、

ここで、前記セミリジットケーブル及び前記同軸ケーブルは、それぞれ、該セミリジットケーブル回転用コネクタ及び該同軸ケーブル回転用コネクタを介して結合している、
