Abstract
본 발명은 갑상선 안구 질환 진단용 바이오마커 조성물 및 이의 용도에 관한 것으로, 더욱 상세하게는 VDB 및 CHI3L1을 포함하는 갑상선 안구 질환 진단용 바이오마커 조성물 및 이의 용도에 관한 것이다. 본 발명자들은 정상인 및 갑상선 안구 질환 환자의 누액에서 분리한 엑소좀 내 단백질을 분석하여, 정상인에 비해 갑상선 안구 질환 환자에서 유의적으로 발현이 증가한 유전자를 갑상선 안구 질환 진단용 마커로 발굴하였는바, 본 발명에 따른 마커 조성물은 갑상선 안구 질환을 조기에 진단할 수 있어 효율적으로 이용될 수 있을 것으로 기대된다.

본 출원은 2020년 12월 02일 출원된 대한민국 특허출원 제 10-2020-0166914호를 우선권으로 주장하고, 상기 명세서 전체는 본 출원의 참고문헌이다.
본 발명은 갑상선 안구 질환 진단용 바이오마커 조성물 및 이의 용도에 관한 것으로, 더욱 상세하게는 VDB(GC vitamin D binding protein)(NCBI 접근 번호: NM_000583.4; NM_001204306.1; NM_001204307.1; XM_006714177.3) 및 CHI3L1(chitinase 3 like 1)(NCBI 접근 번호: NM_001276.4)을 포함하는 갑상선 안구 질환 진단용 바이오마커 조성물 및 이의 용도에 관한 것이다.
갑상선 안구 질환(Thyroid eye disease, TED)은 그레이브스 눈 질환(Graves' eye disease)으로도 불리며 면역 세포가 과도한 양의 갑상선 호르몬을 분비하는 경우 갑상선을 공격하면서 갑상선 기능 이상과 동반하여 나타나는 눈의 이상 병증이다. 갑상선 안구질환은 자가면역 갑상선 질환에서 갑상선 외 징후 중 가장 흔한 것으로 알려져 있으며, 초기에는 눈꺼풀이 붓고 결막이 충혈되는 경미한 증상으로 시작하여, 눈이 앞으로 돌출되거나 눈이 크게 떠지는 등 외안근이 비대해지는 외관상의 변화를 보일 수 있고, 지방 및 결합조직이 증가하여 안구 외근이 증가하고 외안근 운동이 제한되며, 중증의 경우 각막 파괴로 인한 시야 상실 또는 압박성 시신경 병증을 야기한다.
갑상선 안구 질환을 치료하기 위해서는 갑상선 기능 이상을 조절하기 위한 내과적 치료 및 안구에 스테로이드제 등의 약물 치료를 병행하는 안과적 치료가 수행될 수 있고, 수술적 치료로는 안와감압술, 사시교정술, 안검성형술 등의 외과적인 치료가 이루어질 수 있다. 단, 갑상선 안구 질환의 치료가 늦어 시신경 손상이 진행되는 경우 회복하기가 어렵기 때문에 갑상선 안구 질환을 조기에 발견하고 치료를 진행하는 것이 치료에 효과적이다.
한편, 엑소좀은 30 ~ 150 nm 크기의 세포 외 소포로 혈장, 소변, 림프, 조직액, 활액, 모유, 타액 및 누액(눈물)과 같은 체액에 존재한다. 엑소좀은 통제된 방식으로 세포에 의해 방출되며 생물학적 정보를 전달한다. 대부분의 단백질, mRNA, miRNA, lncRNA 및 지질을 포함한다. 엑소좀은 세포 내 및 세포 외 신호 전달을 매개하고 면역반응을 조절하며 세포 폐기물을 관리한다.
엑소좀에 대한 연구는 주로 암 분야에서 연구가 활발히 진행되고 있으며 갑상선 암종 등의 연구도 진행된 바 있으나(Int J Mol Sci. 2019 Mar; 20(5): 1114.), 현재 안구 질환에서의 역할에 대해 알려진 바는 거의 없어 갑상선 안구 질환의 빠른 진단을 위한 엑소좀 기반 신규 바이오마커의 개발이 필요한 실정이다.
본 발명자들은 갑상선 안구 질환을 진단하기 위한 바이오마커를 발굴하기 위해 정상인 및 갑상선 안구 질환 환자의 누액을 채취하고 이로부터 엑소좀(exosome)을 분리하여 엑소좀 단백질을 분석한 결과, 정상인에 비해 갑상선 안구 질환 환자의 누액 유래 엑소좀에서만 유의하게 발현이 증가하는 단백질을 발견하고, 이에 기초하여 본 발명을 완성하였다.
이에, 본 발명은 VDB(GC vitamin D binding protein)(NCBI 접근 번호: NM_000583.4; NM_001204306.1; NM_001204307.1; XM_006714177.3) 및 CHI3L1(chitinase 3 like 1)(NCBI 접근 번호: NM_001276.4)을 포함하는 유전자 또는 상기 유전자가 암호화하는 단백질을 포함하는, 갑상선 안구 질환 진단용 마커 조성물을 제공하는 것을 목적으로 한다.
또한, 본 발명은 VDB(GC vitamin D binding protein)(NCBI 접근 번호: NM_000583.4; NM_001204306.1; NM_001204307.1; XM_006714177.3) 및 CHI3L1(chitinase 3 like 1)(NCBI 접근 번호: NM_001276.4)을 포함하는 유전자의 mRNA 또는 상기 유전자가 암호화하는 단백질 수준을 측정하는 제제를 포함하는, 갑상선 안구 질환 진단용 조성물 및 상기 진단용 조성물을 포함하는 키트를 제공하는 것을 다른 목적으로 한다.

본 발명자들은 정상인 및 갑상선 안구 질환 환자의 누액에서 분리한 엑소좀 내 단백질을 분석하여, 정상인에 비해 갑상선 안구 질환 환자에서 유의적으로 발현이 증가된 단백질을 갑상선 안구 질환 진단용 마커로 발굴하였는바, 본 발명에 따른 마커 조성물은 갑상선 안구 질환을 조기에 진단할 수 있어 효율적으로 이용될 수 있을 것으로 기대된다.
도 1은 누액에서 분리된 엑소좀을 나타낸 것으로, 도 1a는 안구 스폰지를 통해 채취한 누액 및 이를 원심분리하여 수득한 펠릿(검은색 화살표)을 나타낸 것이고, 도 1b는 TEM을 통해 갑상선 안구 질환 환자로부터 분리된 누액 엑소좀의 형태에 대한 현미경 분석 결과를 나타낸 것이다.
도 2는 대조군의 누액 엑소좀에 대비하여 갑상선 안구 질환 환자에서 누액 엑소좀 특성의 변화를 확인한 것으로, 도 2a는 엑소좀의 크기 분포를 보여주는 NTA 분석을 나타낸 것이고, 도 2b는 엑소좀의 농도 변화를 나타낸 것이고, 도 2c는 Alix, Flotillin 2, TSG101, CD63 및 CD81에 대한 항체를 사용한 엑소좀의 웨스턴 블롯 분석을 나타낸 것이고, 도 2d는 ELISA 분석을 통해 CD9의 발현을 확인한 결과를 나타낸 것이다.
도 3은 Proteome Profiler Human XL Cytokine array에 의한 엑소좀의 단백질 분석을 나타낸 것으로, 도 3a는 3개의 갑상선 안구 질환 누액 엑소좀 및 3개의 대조군 누액 엑소좀을 Proteome Profiler Human XL Cytokine array kit로 분석한 결과를 나타낸 것이고, 도 3b는 도 3a의 결과를 히트맵 분석으로 정렬한 것으로 105개의 단백질 중 23개의 유의미한 단백질을 검출하여 나타내고 있으며, 도 3c는 대조군과 비교하여 갑상선 안구질환 환자 누액 엑소좀 내 특정 단백질 수준의 증가를 나타낸 것이다.
도 4는 정상 안와 섬유아세포에 갑상선 안구 질환 환자의 누액 유래 엑소좀을 처리하는 경우에 염증성 사이토카인의 발현의 증가를 나타낸 것이다.
본 발명자들은 갑상선 안구 질환을 진단하기 위한 바이오마커를 발굴하기 위해 정상인 및 갑상선 안구 질환 환자의 누액을 채취하고 이로부터 엑소좀(exosome)을 분리하여 엑소좀 단백질을 분석한 결과, 정상인에 비해 갑상선 안구 질환 환자의 누액 유래 엑소좀에서만 유의하게 발현이 증가하는 단백질을 확인하고, 이에 기초하여 본 발명을 완성하였다.

본 발명에서 사용되는 용어, “갑상선 안구 질환(Thyroid eye disease, TED)”은 과도한 양의 갑상선 호르몬을 분비에 의해 면역 체계가 눈 주변 조직을 공격하여 눈 근육 또는 지방이 확장되는 증상을 나타내는 질환으로, 갑상선 기능 이상과 동반하여 나타나는 안 질환을 총칭하는 의미이다.
본 발명에서 사용되는 용어 “진단(diagnosis)”이란 넓은 의미로는 환자의 병의 실태를 모든 면에 걸쳐서 판단하는 것을 의미한다. 판단의 내용은 병명, 병인, 병형, 경중, 병상의 상세한 양태 및 합병증의 유무 등이다. 본 발명에서 진단은 갑상선 안구 질환의 발병 여부 및 진행단계 수준 등을 판단하는 것이다.
본 발명자들은 구체적인 실시예를 통해 갑상선 안구 질환의 진단을 위한 신규 마커로써 본 발명에 따른 특정 단백질의 용도를 규명하였다.
본 발명의 일실시예에서는 웨스턴 블랏을 통해 엑소좀의 마커에 대한 항체를 분석한 결과, Alix, Flotillin 2, TSG101, CD63 및 CD81에 대한 항체 단백질 발현 수준은 갑상선 안구 질환 환자군이 대조군에 비해 현저히 높게 나타나는 것을 확인하였다(실시예 3 참조).
본 발명의 다른 실시예에서는 갑상선 안구 질환 환자의 누액 유래 엑소좀에서 유의적으로 발현이 증진된 단백질을 분석한 결과, VDB, CRP, CHI3L1, MMP-9 및 VCAM-1의 발현이 증가되는 것을 확인하였으며, 구체적으로 대조군에 비해 VDB이 2.9배, CRP이 2.1배, CHI3L1이 1.7배, MMP-9 1.5배 및 VCAM-1이 1.4배 증가된 것을 확인하였다(실시예 4 참조).
본 발명의 또 다른 실시예에서는 안와 섬유아세포에 대조군 유래 엑소좀 및 갑상선 안구 질환 환자 유래 엑소좀을 처리하고, ELISA를 사용하여 IL-6, IL-8 및 MCP-1의 수준을 분석한 결과, 갑상선 안구 질환 환자의 엑소좀을 투여한 안와 섬유아세포에서 IL-6, IL-8 및 MCP-1의 농도가 현저하게 증가하는 것을 확인하였다(실시예 5 참조).
상기 실시예로부터 Alix, Flotillin 2, TSG101, CD63, CD81, VDB, CRP, CHI3L1, MMP-9, VCAM-1, IL-6, IL-8 또는 MCP-1이 갑상선 안구 질환 진단을 위한 마커로 이용될 수 있음을 유추할 수 있다.

이하 본 발명을 실시예를 통하여 보다 상세하게 설명한다. 그러나 이들 실시예는 본 발명을 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 한정되는 것은 아니다.
[실시예]
실시예 1. 재료 및 방법
1-1. 누액 유래 엑소좀 분리 및 정량화
누액은 갑상선 안구 질환 환자 8명(평균 연령 49세(범위 29-73세)) 및 8명의 대조군(평균 연령 66 세 (범위 58-79 세))에서 채취하였다. 수술실에서 전신 마취 하에 하 결막 낭에 폴리 비닐 아세탈 스피어스 안구(eye) 스펀지 (Merocel, BVI, Waltham, MA)를 20분 내지 30분 침지한 후, 안구 스폰지를 0.2ml PCR 튜브 (Eppendorf, Hamburg, Germany)에 넣고 13,000rpm, 4℃에서 30분 동안 원심분리하여 누액을 수득하였다. 샘플은 -80℃ 초저온 냉동고에서 새 튜브에 보관하였다. 이후, 엑소좀 키트(ExosomePlus inc., Seoul, Korea)를 사용하여 엑소좀을 분리하였다. 펠릿을 여과된 인산염 완충 식염수(PBS)에 현탁시켰다. 엑소좀의 정량화는 BCA 분석 (Thermo Fisher Scientific, Waltham, MA)을 사용하여 결정되었다. 플레이트를 30 초 동안 볼 텍싱하고 대조군 단백질 (Biotek, Winooski, VT, USA)이있는 마이크로 플레이트를 사용하여 540nm에서 판독하였다.
1-2. 안와(orbital) 섬유아세포의 배양
인간의 정상 안와 섬유아세포는 상부 안검 성형술을 받은 환자로부터 수득하였다. 안와 지방 외식 편은 T-25 플라스크에 10 % 소태아혈청(FBS; GIBCO BRL)이 보충된 Dulbecco의 수정된 Eagle's 배지(GIBCO BRL, NY, Grand Island)가 포함된 배지와 함께 첨가되었고, 5 % CO2에서 37℃ 조건에서 배양되었다. 안와 지방은 1주일 동안 T-25 플라스크의 상단 내부 표면에 떠서 부착되었다. 다음으로, 배지를 제거하고 추가된 배지 플라스크를 뒤집어 세포가 바닥으로 이동시켰다. 대략 80 %의 융합성(confluent) 안와 섬유아세포에 트립신/EDTA(GIBCO BRL, Invitrogen, USA)를 처리하고 계대하였다. 안와 섬유아세포(10,000cell/well)를 24시간 동안 24 well 플레이트에서 갑상선 안구 질환 환자 및 대조군의 두 가지 유형의 엑소좀(5μg/well)과 함께 공동 배양하였다. ELISA(R&D Systems, Inc., Minneapolis, MN)에 의해 IL-8, IL-6 및 Monocytic chemotactic protein-1(MCP-1)의 수준이 분석될 때까지 상층 액을 수집하고 -20℃에서 보관하였다.
1-3. 투과전자 현미경 및 면역-금(immune-gold) 라벨링
항-CD81을 이용한 면역 라벨링을 위해, 엑소좀 샘플을 실온에서 1시간 동안 4% 파라포름알데히드에 배열하고 10μl를 폼바(formvar)가 코팅된 니켈 그리드 (300 mesh)에 스팟팅(spotting)하였다. 인산염 완충액(PB)으로 5분 세척하는 동안 그리드를 3회 헹궈주었다. 그리드는 1% BSA (w/v)를 포함하는 PBS 7.4에서 1시간 동안 차단되었다. 실온에서 1시간 동안 배양한 후, 그리드를 세척 완충액(PBS) 5X로 10분 동안 세척하였다. 그리드는 PBS에서 1.4 nm anti-mouse nanogold(Nanoprobes, Inc. NY, USA)에 1시간 동안 부유시켰다. 실버 강화는 HQ 실버 강화 키트(Nanoprobes, Inc., NY, USA)를 사용하여 2분 동안 수행되었다. 다음으로, 1% 수성 Uranyl Acetate에서 음성 염색을 신속하게 수행하고 모든 샘플을 건조하였으며, 자연 건조를 허용하였다. 펠릿은 100kV에서 투과 전자 현미경 (JEM-1010, Tokyo, Japan)으로 검사하였다.
1-4. 나노 입자 추적 분석
나노 입자 추적 분석(ExoCope monoTM, Exosome plus, Inc. South Korea)을 사용하여 엑소좀을 분석하였다. 측정은 25℃에서 서로 다른 하위 부피 위치에서 15 회 반복하였다. 분석은 ExoCope 추적기 소프트웨어 버전 1.005로 수행되었다. 강도 임계 값은 20 레벨로 설정되었다(픽셀 당 디지털 숫자의 직교 노이즈 레벨). 최소 추적 입자 크기는 50nm로 설정되었다. 분리 가능한 최소 입자 거리는 5.7 픽셀로 설정되었다. 이 시스템은 5 가지 농도의 100nm 폴리스티렌 비드로 보정되었다. 각 샘플은 200 nm 여과된 PBS로 1/500 ~ 1/2000 농도로 희석하여 각 이미지 프레임에 표시되는 입자 수가 30 ~ 200 개가 되도록 설정하였다.
1-5. 효소 면역 분석법
Exo Enzyme-linked immunosorbent assay(ELISA) 정량 분석 키트(EXOEL-CD9A-1, System Biosciences, CA, USA)를 사용하여 엑소좀의 발현 수준을 측정하였다. 엑소좀의 CD9 반응은 마이크로 플레이트 리더를 사용하여 450nm에서 측정하였다(Biotek, Winooski, VT, 미국).
1-6. 웨스턴 블롯 분석
엑소좀(10μg)을 4-15% sodium dodecyl sulfate polyacrylamide 겔에서 전기영동하고 polybinylidene difluoride(PVDF) 막으로 이동시켰다. 5 % 소혈청알부민 (BSA)으로 블록킹(bolcking)한 다음 anti-Alix, anti-CD63 (Santa Cruz biotechnology, CA, USA), anti-Flotillin 2, anti-TSG101 및 항-CD81 (Novus Biologicals, Littleton, CO, USA) 항체와 함께 4℃에서 밤새 배양하였다. 항-마우스 및 항-토끼 HRP-접합 2차 항체는 세포 신호 전달 기술 (Danvers, MA, USA)에서 수득하였다. 신호는 ECL Plus 시스템(Amersham, Pittsburgh, PA)을 사용하여 시각화하였다. 모든 이미지는 Syngene PXi 4 digital imager(Syngene, Fredrick, MD, USA)를 사용하여 감지되었다. 단백질 밴드의 정량화는 Fujifilm Multi Gage V3.0을 사용하여 수행되었다. 재-프로빙(re-probing)을 위해, 각 막을 웨스턴 블롯 스트리핑 버퍼(Thermo Fisher Scientific, Waltham, MA, USA)에서 배양하였다.
1-7. Proteome Profiler Human XL Cytokine array
갑상선 안구 질환 환자 또는 대조군의 누액에서 정제된 엑소좀(100μg)은 니트로 셀룰로스에서 막에서 발견된 105개의 서로 다른 포획 항체를 포함하는 Proteome Profiler Human XL Cytokine array kit(ARY022, R&D Systems, Minneapolis, MN, USA)로 제조업체의 프로토콜에 따라 분석되었다. 면역-점(Immune-spots)은 Syngene PXi 4 digital imager로 캡처하고 데이터는 Fujifilm Multi Gage V3.0으로 분석하였다. 제시된 데이터는 대조군과 비교하여 갑상선 안구 질환 환자에서 상대적인 단백질 수준(선 상자로 표시된 어레이 참조로 정규화)의 배수 변화를 나타낸 것이다. 모든 절차는 서울 성모 병원 기관 심의위원회 (KC19TES10673)와 헬싱키 선언의 교리에 따라 기증자로부터 사전 동의를 얻은 후 수행하였다.
1-8. 통계 분석
GraphPad Prism (버전 7.0, GraphPad Software Inc., San Diego, CA)을 사용하여 통계 분석을 수행하였다. 결과는 평균 ± 표준 편차 (SD)로 표현하였다. 계산된 모든 통계 평가는 student's two-tailed t-검정을 사용하여 수행되었다. ** p 값 <0.01 또는 * p- 값 <0.05는 통계적으로 유의 한 것으로 간주하였다.
실시예 2. 갑상선 안구 질환 환자의 특성 확인
실시예 1-1에서 수득한 갑상선 안구 질환 환자 및 대조군의 누액을 사용하였으며, 갑상선 안구 질환 환자의 누액은 전신 마취하에 안와 감압 수술에서 채취되었다. 모든 갑상선 안구 질환 환자는 그레이브스 병 진단을 받았으며 메티마졸(methimazole) 치료를 수행하였다. 그 중 5 명의 환자는 이전에 질병의 급성기에 프레드니솔론(prednisolone) 치료를 받았다. 갑상선 안구 질환 환자의 질환 발생 기간은 6 개월에서 72 개월인 것을 확인하였다. 갑상선 자극 면역 글로불린 (TSI) 생물 검정 (정상; <140 %)의 평균 값은 372 % (범위 157 - 684)였으며, 임상 활동 점수 (CAS)는 1 ~ 4이었고 NOSPECS 점수는 1 ~ 6으로 확인되었다.
대조군은 비루관 폐쇄 환자로 구성되었으며 누낭 비강 절개술시 누액을 채취하였다. 대조군은 안구 표면 변화를 유발할 수있는 갑상선 질환이나 안구 건조증, 결막염, 마이 봄샘 기능 장애, 녹내장 등과 같은 다른 안 질환이 없는 것을 확인하였다.
실시예 3. 갑상선 안구 질환 환자 및 대조군의 누액에서 얻은 엑소좀의 특성 확인
원심 분리 후 얻은 분리된 엑소좀 "펠릿"은 도 1a에 도시한 바와 같다. 또한, 엑소좀의 특정 마커인 CD81로 면역 금 라벨링을 통해 투과 전자 현미경으로 엑소좀의 현미경 형태를 확인한 결과, 도 1b에 나타낸 바와 같이 두 그룹 모두에서 펠릿은 CD81 수용체에 양성인 마이크로 입자와 원형 또는 난형 엑소좀을 포함하는 것을 확인하였다. 또한, 도 2a에 나타낸 바와 같이 나노 입자 추적 분석 결과 갑상선 안구 질환 그룹에서 120.75 ± 6.0 nm이고, 대조군에서 120.75 ± 7.9 nm으로 엑소좀의 크기에 차이가 없는 것을 확인하였다. 그러나, 도 2b에 나타낸 바와 같이 엑소좀의 농도의 경우 평균 농도가 갑상선 안구 질환 환자군에서 4.2 배 증가하였다(TED 환자에서 3.53 x 1011 나노 입자/μg 단백질 대 대조군에서 1.48 x 1010 ± 나노 입자/μg 단백질; p <0.05).
이에 더하여, 엑소좀의 마커에 대한 항체를 분석한 결과, 도 2c에 나타낸 바와 같이 Alix, Flotillin 2, TSG101, CD63 및 CD81에 대한 항체는 두 그룹 모두에서 웨스턴 블랏 분석을 통해 쉽게 검출되었으며 단백질 발현 수준은 갑상선 안구 질환 환자군이 대조군에 비해 현저히 높게 나타나는 것을 확인하였으며, 해당 단백질의 NCBI 정보를 하기 표 1에 도시하였다.

또한, 항-CD 9 항체가 있는 ELISA에 의해 엑소좀을 추가로 특성화한 결과, 도 2d에 나타낸 바와 같이 CD 9의 광학 밀도는 대조군과 비교하여 갑상선 안구 질환 환자의 엑소좀에서 유의하게 증가된 것을 확인하였다(p <0.05).
실시예 4. 갑상선 안구 질환 환자의 누액 엑소좀 처리에 따른 단백질 발현 변화 확인
Proteome Profiler Human XL Cytokine array kit를 사용하여 누액의 엑소좀에서 특정 염증 관련 단백질 및 발현 수준을 감지하였다. 도 3a 및 3b에 나타낸 바와 같이, 키트에 포함된 105개의 인간 단백질 중 23개의 단백질이 검출되었다. 그 중에서도, 도 3c에 나타낸 바와 같이 대조군에 비해 갑상선 안구 질환 환자의 엑소좀에서 유의적으로 VDB, CRP, CHI3L1, MMP-9 및 VCAM-1의 발현이 증진된 것을 확인할 수 있었으며, 각각 대조군에 비해 Vitamin D binding protein (VDB)이 2.9배, C-reactive protein (CRP)이 2.1배, Chitinase 3-like 1 (CHI3L1)이 1.7배, matrix metalloproteinase-9 (MMP-9) 1.5배 및 vascular adhesion molecule-1 (VCAM-1)이 1.4배 발현이 증가된 것을 확인하였으며(p <0.05), 해당 단백질의 NCBI 정보를 하기 표 2에 도시하였다.

갑상선 안구 질환 환자의 안와 조직에서는 IL-6, IL-8, IL-10, IL-16, TNF-α, RANTES 및 MCP-1과 같은 수많은 강력한 자극제 역할을 하는 사이토카인이 검출되고 그 발현이 증가하는 것으로 알려져있다. 이에, 안와 섬유아세포에 대한 엑소좀의 염증 효과를 확인하기 위해 건강한 안와 섬유아세포를 갑상선 안구 질환 환자의 엑소좀 및 대조군의 엑소좀, 두 유형의 엑소좀으로 자극하고, 염증성 사이토카인의 발현을 확인하기 위해, 시험관 내 ELISA를 사용하여 IL-6, IL-8 및 MCP-1의 수준을 분석하였으며, 해당 단백질의 NCBI 정보를 하기 표 3에 도시하였다. 이때, 정상 안와 섬유아세포는 24시간 동안 5μg 엑소좀과 함께(CON exosomes treated) 또는 엑소좀 없이(Untreated) 배양하였다.

그 결과, 도 4에 나타낸 바와 같이, IL-6, IL-8 및 MCP-1의 농도는 갑상선 안구 질환 환자의 엑소좀을 투여한 안와 섬유아세포에서 현저하게 증가하는 것을 확인하였다. 이에 더하여, 엑소좀을 처리하지 않은 무처리군과 갑상선 안구 질환 환자 또는 대조군의 엑소좀을 처리한 결과와 비교했을때 IL-6의 경우 갑상선 안구 질환 환자의 엑소좀을 처리한 경우 4.2 배 증가하였고 대조군 엑소좀을 처리한 경우 2.3 배 증가하였으며, IL-8의 경우 갑상선 안구 질환 환자의 엑소좀을 처리한 경우 7.2 배 증가하였고, 대조군 엑소좀을 처리한 경우 3.5 배 증가 하였으며, MCP-1의 경우 갑상선 안구질환 환자의 엑소좀을 처리한 경우 8.6 배 증가하였고, 대조군의 엑소좀을 처리한 경우 4.6 배 증가하는 것을 확인하였다(p < 0.05). 상기 결과로부터 갑상선 안구 질환 환자의 엑소좀을 처리하는 경우 정상 대조군의 엑소좀을 처리할 때와 비교하여 염증성 사이토카인의 발현이 약 2배 이상 증가되는 것을 확인할 수 있었으며 이로부터 갑상선 안구 질환 환자가 함유하는 엑소좀이 안와 섬유아세포에 염증을 활성화시킬 수 있다는 것을 확인할 수 있었다.
전술한 본 발명의 설명은 예시를 위한 것이며, 본 발명이 속하는 기술분야의 통상의 지식을 가진 자는 본 발명의 기술적 사상이나 필수적인 특징을 변경하지 않고서 다른 구체적인 형태로 쉽게 변형이 가능하다는 것을 이해할 수 있을 것이다. 그러므로 이상에서 기술한 실시예들은 모든 면에서 예시적인 것이며 한정적이 아닌 것으로 이해해야만 한다.
