Definitions
- the present invention relates to a surgical instrument used for replacing or taking out the indwelling catheter from the body.
- the present invention relates to a surgical instrument which, having a hollow cylindrical form containing cylindrical blade structure inside, allows to access and disconnect the cuff connection that ensures the indwelling catheter to be fixed.
- catheters are inserted to the patients who need frequently or continuously drug administration or enzyme replacement into the vein. By entering through a small incision on the body, the catheter is inserted into a thick vein.
- Indwelling catheter which is a type of catheter that can be used for a longer time, reaches the vein through a tunnel created under the skin.
- cuff is used for the indwelling catheter having an average usage time of 6-12 months to remain for such a long time and adapt to the body.
- the cuff located on the outer surface of the catheter s tube part entering the body swells out when it comes into contact with blood and adheres to the area where it is located. Also, the cuff prevents the spread of infection on the catheter. Permanent vascular access becomes a necessity especially in the patients who have kidney failure and so have to enter the dialysis machine every day.
- the cuff During the removal or replacement of the indwelling catheter, the cuff must be separated from the body. In the conventional method, separating the connection formed by the cuff structure from the body is quite laborious and time consuming. Besides, too many incision should be done during this conventional procedure.
- the surgical instrument mentioned in the patent document no. US20170056067A1 has a form similar to the scissors structure and at its tip a design that can grasp the connection diameter of the clean and dirty blood passage of the hemodialysis catheter.
- a method of removing the connection junction of the hemodialysis catheter by means of said scissor-shaped surgical instrument is mentioned.
- Aim of the invention is to present a surgical instrument which allows the cuff connection ensuring the fixation of the indwelling catheter to be accessed and disconnected in order to take out or replace the indwelling catheter from the body and for this, has a hollow cylindrical form containing cylindrical blade structure inside.
- the present invention is a surgical instrument which is used for replacing or taking out the indwelling catheter from the body and comprises
- Mentioned blade structure comprises more than one blade column which is positioned with equal distances from each other.
- Each of mentioned blade columns has cutting edge facing the same rotational direction.
- Said control channel comprises
- Figure 1 a perspective view that belongs to the security mode of the present innovative surgical instrument is given.
- FIG 3a a close-sectional view of l-shaped control channel that allows forth-and-back movement control of the blade structure of said innovative surgical instrument is given.
- Figure 3b a close-sectional view of T-shaped control channel that allows both forth-and-back movement control and rotational movement control of the blade structure of the innovative surgical instrument is given.
- Present invention relates a surgical instrument (1) which allows the cuff connection ensuring the fixation of the indwelling catheter to be accessed and disconnected in order to take out or replace the indwelling catheter from the body and for this, has a hollow cylindrical form containing cylindrical blade (20) structure inside. Thanks to present innovative surgical instrument (1), removal operation of the indwelling catheter becomes a very practical and fast procedure.
- FIG. 1 A perspective view of the present innovative surgical instrument (1) is given in Figure 1.
- the present surgical instrument (1) comprises an outer cylinder (10) having a control channel (11), a blade (20) guided throughout inside mentioned outer cylinder (10) by means of its own control pin (21) remaining in mentioned control channel (11).
- Said surgical instrument (1) is kept closed in security mode while it is not used.
- mentioned control pin (21) is fixed with a security nut (30) while the blade (20) is in the closed position.
- required security is provided by preventing the situation that the blade (20) spontaneously gets out from said outer cylinder (10).
- a close view of the control pin (21) and the security nut (30) fixing this control pin (21) is given in the detail A.
- the blade (20) structure inside the innovative surgical instrument (1) becomes controllable when the control pin (21) is released.
- the control pin (21) connected to the blade (20) structure is moved forward in the control channel (11)
- the blade (20) structure starts to go outside the outer cylinder (10) structure.
- the control channel (11) can be in T, T and ‘L’ shapes.
- the l-shaped control channel (11) allows the movement of the control pin (21) only along the X channel (11.1) lying on X axis while the T and L-shaped control channels (11) allow its movement along Y channel (11.2) lying on Y axis as well as its movement along X channel (11.1).
- the l-shaped control channel (11) allows only the forth and back movement of the blade (20) structure while the T and L-shaped control channel (11) allows rotational movement of the blade (20) as well as its forth and back movement. Since the l-shaped control channel (11) does not allow the rotational movement of the blade (20) to be controlled by means of the control pin (21), in this embodiment of the invention, the surgeon have to rotate the surgical instrument (1) around the x-axis with her/his own hand.
- Figure 3a, 3b and 3c views of the control channel (11) structure in the shapes of T, T and ‘L’ are given respectively.
- FIG. 2 A perspective view of the surgical instrument’s (1) state in which the blade (20) part is out is given in Figure 2.
- the blade (20) structure consists of many blade columns (20.1) and, these blade columns (20.1) are joined with a fixed interval/space/gap between them as forming a cylindrical form together.
- the cutting edge (20.2) of the each blade column (20.1) is towards same rotational direction (clockwise or counter clockwise) with other.
- the cutting edge (20.2) of a blade column (20.1) is moved up to the starting point of the next adjacent blade column (20.1).
- the surgeon cuts the catheter from the tube part that is close to the body. Then, in order to remove the part of the catheter remaining in the body, the surgeon carries/pushes the present innovative surgical instrument (1) forward through around the catheter tube up to the part where the cuff is.
- the surgeon feels it due to the tissue difference and allows the blade columns (20.1) forming the blade (20) to enter within the cuff by moving the control pin (21) forward.
- the length of the blade columns (20.1) is preferably same with the size of the cuff.
- the control pin (21) also comes to the end of the X channel (11.1).
- the length of the X channel (11.1) should be same length with the length of the cuff.
- the surgeon moves the control pin (21) on the surgical instrument (1) along the Y channel (11.2) and thus, the rotational movement of the blade (20) structure around the x-axis is realized. With this movement, the cutting edge (20.2) of each blade column (20.1) advances rotationally inside the cuff and provides the connection formed by the cuff in the body to be broken. Thus, the remaining part of the indwelling catheter in the body can be easily removed.
- the present innovative surgical instrument (1) should be in a separate sterile bag within the indwelling catheter set.
- the surgical instrument (1) coming out together with the catheter set is given to the patient after the catheter insertion operation and so, it would be used for catheter removal operation that will require in the future.
- a security gap/problem emerges out. Therefore, it is necessary to use a security nut (30) or a similar structure in order to ensure the security of the surgical instrument (1).
- a security nut (30) or a similar structure in order to ensure the security of the surgical instrument (1).
