要約 肝生検に比べ極めて侵襲性が低く、技術者の技量に依存することなく簡便な操作により、非アルコール性脂肪性肝炎(NASH)の検出を補助することができる方法、及び、NASHに関連する病態の進行程度の判定を補助することが可能な方法を提供する。 本発明は、生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することを含む、非アルコール性脂肪性肝炎の検出を補助する方法を提供する。また、本発明は、生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することを含む、非アルコール性脂肪性肝炎に関連する病態であって、脂肪変性、炎症、風船様変性、及び線維化からなる群から選ばれる少なくとも1種の病態の進行程度の判定を補助する方法を提供する。
Description
本発明は、非アルコール性脂肪性肝炎の検出を補助する方法、及び非アルコール性脂肪性肝炎に関連する病態の進行程度の判定を補助する方法に関する。
非アルコール性脂肪性肝疾患(nonalcoholic fatty liver disease、以下「NAFLD」ともいう)は、組織診断又は画像診断で脂肪肝が認められ、ウイルス性肝炎、自己免疫性肝炎及びアルコール性肝障害などの肝疾患を除外した病態であり、肥満人口の増加に伴い全世界的に急増している。非アルコール性脂肪性肝疾患(NAFLD)は、病態がほとんど進行しないと考えられる非アルコール性脂肪肝(nonalcoholic fatty liver、以下「NAFL」ともいう)と、進行性で肝硬変や肝癌の発症原因にもなる非アルコール性脂肪性肝炎(nonalcoholic steatohepatitis、以下「NASH」ともいう)とに分類される。非アルコール性脂肪性肝炎(NASH)に特徴的な病態としては、脂肪変性、風船様変性、炎症、線維化などがある。
肝臓の脂肪変性から始まる種々の病態を鑑別したり、NAFLとNASHを区別するためには、肝生検による確定診断を行う必要がある。しかしながら、肝生検は侵襲性の検査であり高額であるため様々な面で患者を消耗させる。また、肝生検では一定の確率でサンプリングエラーが起こるなど実施する技術者の力量に左右されることがあり、採取した検体の診断についても同様である。このため、患者は一定レベルの医療を受けるために特定の機関を訪れる必要があり、患者の負担が大きいという問題があった。
一方、肝疾患に関し一般的に実施されている臨床検査項目としては、AST、ALT、AST/ALT比等や、炎症・サイトカインのマーカーであるTNF-α、好感度CRP、フェリチン等、アポトーシスのマーカーであるサイトケラチン18断片、線維化のマーカーであるヒアルロン酸、4型コラーゲン7S等がある(特許文献1及び2)。しかしながら、NASHに関連する複数の病態で検出されるような、NASHを疑うバイオマーカーとして確立されたものは存在していなかった。すなわち、NASHに関わる病態を総合的に反映するバイオマーカーはこれまで見出されていなかった。
本発明の目的は、肝生検に比べ極めて侵襲性が低く、技術者の技量に依存することなく簡便な操作により、非アルコール性脂肪性肝炎(NASH)の検出を補助することができる方法、及び、NASHに関連する病態の進行程度の判定を補助することが可能な方法、を提供することである。
本願発明者らは、鋭意研究の結果、生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することにより、肝生検を実施しなくても簡便な操作によりNASHの検出を補助することができ、また、NASHに関連する進行程度の判定を補助できることを見出し、本発明を完成した。
すなわち、本発明は以下のとおりである。
[1]生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することを含む、非アルコール性脂肪性肝炎の検出を補助する方法。
[2]前記被検血液試料中のLDL-TGの存在量を指標とし、前記LDL-TGの存在量が非アルコール性脂肪肝を患う患者の血液試料中のLDL-TGの存在量よりも多いことが、非アルコール性脂肪性肝炎を発症している可能性が高いことを示す、[1]に記載の方法。
[3]前記被検血液試料中のLDL-TG/LDL-C比を指標とし、前記LDL-TG/LDL-C比が非アルコール性脂肪肝を患う患者の血液試料中のLDL-TG/LDL-C比よりも高いことが、非アルコール性脂肪性肝炎を発症している可能性が高いことを示す、[1]又は[2]に記載の方法。
[4]前記被検血液試料中のApoE rich HDL-C/HDL-C比を指標とし、前記ApoE rich HDL-C/HDL-C比がアルコール性脂肪肝を患う患者の血液試料中のApoE rich HDL-C/HDL-C比よりも低いことが、非アルコール性脂肪性肝炎を発症している可能性が高いことを示す、[1]~[3]のいずれかに記載の方法。
[5]生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することを含む、非アルコール性脂肪性肝炎に関連する病態であって、脂肪変性、炎症、風船様変性、及び線維化からなる群から選ばれる少なくとも1種の病態の進行程度の判定を補助する方法。

[4]前記被検血液試料中のApoE rich HDL-C/HDL-C比を指標とし、前記ApoE rich HDL-C/HDL-C比がアルコール性脂肪肝を患う患者の血液試料中のApoE rich HDL-C/HDL-C比よりも低いことが、非アルコール性脂肪性肝炎を発症している可能性が高いことを示す、[1]~[3]のいずれかに記載の方法。
[5]生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することを含む、非アルコール性脂肪性肝炎に関連する病態であって、脂肪変性、炎症、風船様変性、及び線維化からなる群から選ばれる少なくとも1種の病態の進行程度の判定を補助する方法。
本発明により、患者に対する負荷やリスクが大きい肝生検を極力使用することなく、また、肝生検を実施する技術者の力量に依存することなく、安全かつ簡便な操作によりNASHの検出を補助することができ、又は、NASHに関連する病態の進行程度や有無の判定を補助することができる。その結果、早期にNASHの治療を開始することができ、また、その進行リスクを知ることで治療方針の決定を補助することができ、重症化や死に至る進行を防ぎ又は遅らせることができる。
本発明は、生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することにより、非アルコール性脂肪性肝炎(NASH)の検出を補助する方法、及び、非アルコール性脂肪性肝炎(NASH)に関連する病態の進行程度の判定を補助する方法である。
本発明において、LDL-TGは低密度リポ蛋白(以下、LDLと称する)中のトリグリセリド(TG)を意味し、ApoE rich HDL-Cはアポリポタンパク質Eを多く含む高密度リポ蛋白(以下、ApoE rich HDLと称する)中のコレステロール(C)を意味する。
本発明においては血液試料を被検試料とするため、肝生検と比べると極めて侵襲性が低く、一部分を切り出した特定のサンプルに依存しない肝臓全体の病態を総体的に把握することができる。また、被検血液試料の採取は容易であり、血液試料に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量の測定に自動分析装置対応の測定キットを使用すれば、日常診療や健康診断等と同程度の簡便な操作で測定することが可能である。本発明における血液試料としては、全血、血漿及び血清が挙げられる。
(非アルコール性脂肪性肝炎(NASH)の検出を補助する方法)
以下に、本発明において、非アルコール性脂肪性肝炎(NASH)の検出を補助する具体的な方法を説明する。

(1) LDL-TGの存在量
被検血液試料中のLDL-TGの存在量(LDL-TG量ともいう)を測定しこれを指標とすることにより、被検血液試料中のLDL-TG量が非アルコール性脂肪肝(NAFL)の患者(NAFL患者)の血液試料中のLDL-TG量よりも有意に多い場合に、NASHである可能性が高いと判断することができる(実施例1、実施例4、図4を参照)。非アルコール性脂肪性肝炎患者(NASH患者)の血液試料中のLDL-TG量は、カットオフ値を15.1mg/dLに設定したときの感度が82.9%、特異度が55.6%であり、カットオフ値を17.6mg/dLに設定したときの感度が65.7%、特異度が88.9%である。更にはカットオフ値を12.1mg/dLに設定したときの感度が94.3%、特異度が22.2%であり、カットオフ値を20.2mg/dLに設定したときの感度が48.6%、特異度が100%である。ここで、NASH患者の判定に用いるカットオフ値は、本発明を使用する者が求める感度、特異度に応じた範囲で適宜設定することができ、例えば、LDL-TG量のカットオフ値は12.1~21.2mg/dLの範囲内であり、好ましくは17.6mg/dL±20%の範囲内であり、最適値は17.6mg/dLである。NAFL患者の血液試料中のLDL-TG量は、予めNAFL患者の集団から採取した血液試料を分析しておくことにより特定することができる。
被検血液試料中のLDL-TGの存在量(LDL-TG量ともいう)を測定しこれを指標とすることにより、被検血液試料中のLDL-TG量が非アルコール性脂肪肝(NAFL)の患者(NAFL患者)の血液試料中のLDL-TG量よりも有意に多い場合に、NASHである可能性が高いと判断することができる(実施例1、実施例4、図4を参照)。非アルコール性脂肪性肝炎患者(NASH患者)の血液試料中のLDL-TG量は、カットオフ値を15.1mg/dLに設定したときの感度が82.9%、特異度が55.6%であり、カットオフ値を17.6mg/dLに設定したときの感度が65.7%、特異度が88.9%である。更にはカットオフ値を12.1mg/dLに設定したときの感度が94.3%、特異度が22.2%であり、カットオフ値を20.2mg/dLに設定したときの感度が48.6%、特異度が100%である。ここで、NASH患者の判定に用いるカットオフ値は、本発明を使用する者が求める感度、特異度に応じた範囲で適宜設定することができ、例えば、LDL-TG量のカットオフ値は12.1~21.2mg/dLの範囲内であり、好ましくは17.6mg/dL±20%の範囲内であり、最適値は17.6mg/dLである。NAFL患者の血液試料中のLDL-TG量は、予めNAFL患者の集団から採取した血液試料を分析しておくことにより特定することができる。
(2) LDL-TG/LDL-C比
被検血液試料中のLDL-TG量とLDL-C量を測定し、LDL-TG/LDL-C比を求めて指標とすることにより、被検血液試料中のLDL-TG/LDL-C比が健常者又はNAFL患者の血液試料中のLDL-TG/LDL-C比よりも有意に高い場合に、NASHを発症している可能性が高いと判断することができる(実施例2-1、実施例5-1、図5を参照)。ここで、LDL-C量とは、低密度リポ蛋白(LDL)中のコレステロール(C)の存在量である。NASH患者の血液試料中のLDL-TG/LDL-C比は、カットオフ値を0.103に設定したときの感度が100%、特異度が55.6%であり、カットオフ値を0.133に設定したときの感度が94.3%、特異度が66.7%である。更には、カットオフ値を0.149に設定したときの感度が82.9%、特異度が77.8%であり、カットオフ値を0.203に設定したときの感度が31.4%、特異度が100%である。ここで、NASH患者の判定に用いるカットオフ値は、本発明を使用する者が求める感度、特異度に応じた範囲で適宜設定することができ、例えば、LDL-TG/LDL-C比のカットオフ値は0.103~0.203の範囲内であり、好ましくは0.133±20%の範囲内であり、最適値は0.149もしくは0.133である。

さらに、被検血液試料中のLDL-C/LDL-TG比を求めて指標とすることにより、LDL-C/LDL-TG比が健常者又はNAFL患者の血液試料中のLDL-C/LDL-TG比よりも低い場合に、NASHを発症している可能性が高いと判断することもできる(実施例2-2、実施例5-2、図6を参照)。NASH患者の血液試料中のLDL-C/LDL-TG比は、カットオフ値を9.75に設定したときの感度が100%、特異度が55.6%であり、カットオフ値を7.50に設定したときの感度が94.3%、特異度が66.7%である。更にはカットオフ値を6.70に設定したときの感度が82.9%、特異度が77.8%であり、カットオフ値を4.93に設定したときの感度が31.4%、特異度が100%である。この場合、LDL-C/LDL-TG比はLDL-TG/LDL-C比の逆比となる。ここで、NASH患者の判定に用いるカットオフ値は、本発明を使用する者が求める感度、特異度に応じた範囲で適宜設定することができ、例えば、LDL-C/LDL-TG比のカットオフ値は4.93~9.75の範囲内であり、好ましくは7.50±20%の範囲内であり、最適値は7.50もしくは6.70である。
(3) ApoE rich HDL-C/HDL-C比
被検血液試料中のApoE rich HDL-C量とHDL-C量を測定し、ApoE rich HDL-C/HDL-C比を求めて指標とすることにより、被検血液試料中のApoE rich HDL-C/HDL-C比がNAFL患者の血液試料中のApoE rich HDL-C/HDL-C比よりも有意に低い場合に、NASHを発症している可能性が高いと判断することができる(実施例3-1、実施例13-1、図8を参照)。ここで、HDL-C量とは、高密度リポ蛋白(HDL)中のコレステロール(C)の存在量である。NASH患者の血液試料中のApoE rich HDL-C/HDL-C比は、カットオフ値を0.0912に設定したときの感度が80.0%、特異度が66.7%であり、カットオフ値を0.0899に設定したときの感度が74.3%、特異度が88.9%である。更にはカットオフ値を0.0840に設定したときの感度が42.9%、特異度が100%であり、カットオフ値を0.0971に設定したときの感度が91.4%、特異度が11.1%である。ここで、NASH患者の判定に用いるカットオフ値は、本発明を使用する者が求める感度、特異度に応じた範囲で適宜設定することができ、例えば、ApoE rich HDL-C/HDL-C比のカットオフ値は0.0899±20%の範囲内であり、好ましくは0.0840~0.0971の範囲内であり、最適値は0.0899である。
被検血液試料中のApoE rich HDL-C量とHDL-C量を測定し、ApoE rich HDL-C/HDL-C比を求めて指標とすることにより、被検血液試料中のApoE rich HDL-C/HDL-C比がNAFL患者の血液試料中のApoE rich HDL-C/HDL-C比よりも有意に低い場合に、NASHを発症している可能性が高いと判断することができる(実施例3-1、実施例13-1、図8を参照)。ここで、HDL-C量とは、高密度リポ蛋白(HDL)中のコレステロール(C)の存在量である。NASH患者の血液試料中のApoE rich HDL-C/HDL-C比は、カットオフ値を0.0912に設定したときの感度が80.0%、特異度が66.7%であり、カットオフ値を0.0899に設定したときの感度が74.3%、特異度が88.9%である。更にはカットオフ値を0.0840に設定したときの感度が42.9%、特異度が100%であり、カットオフ値を0.0971に設定したときの感度が91.4%、特異度が11.1%である。ここで、NASH患者の判定に用いるカットオフ値は、本発明を使用する者が求める感度、特異度に応じた範囲で適宜設定することができ、例えば、ApoE rich HDL-C/HDL-C比のカットオフ値は0.0899±20%の範囲内であり、好ましくは0.0840~0.0971の範囲内であり、最適値は0.0899である。
さらに、血液試料中のHDL-C/ApoE rich HDL-C比を求めて指標とすることにより、HDL-C/ApoE rich HDL-C比が健常者又はNAFL患者の血液試料中のHDL-C/ApoE rich HDL-C比よりも有意に高い場合に、NASHを発症している可能性が高いと判断することもできる(実施例3-2、実施例13-2、図9を参照)。NASH患者の血液試料中のHDL-C/ApoE rich HDL-C比は、カットオフ値を10.97に設定したときの感度が80.0%、特異度が66.7%であり、更にはカットオフ値を11.12に設定したときの感度が74.3%、特異度が88.9%である。更にはカットオフ値を10.30に設定したときの感度が91.4%、特異度が11.1%であり、カットオフ値を11.90に設定したときの感度が42.9%、特異度が100%である。この場合、HDL-C/ApoE rich HDL-C比は、前記ApoE rich HDL-C/HDL-C比の逆比である。ここで、NASH患者の判定に用いるカットオフ値は、本発明を使用する者が求める感度、特異度に応じた範囲で適宜設定することができ、例えば、HDL-C/ApoE rich HDL-C比のカットオフ値は11.12±20%の範囲内であり、好ましくは10.30~11.90の範囲内であり、最適値は11.12である。
以上の通り、被検血液試料中のLDL-TGの存在量、LDL-C/LDL-TG比、及びApoE rich HDL-C/HDL-C比を指標としてNASHを発症している可能性が高いと判断する方法を説明したが、これらの指標はいずれかを単独で用いるだけでもNASHの検出を十分に補助することができるが、複数の指標を組み合わせて用いると更にNASHの検出を効果的に補助することができる。
このように被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定し、これらの測定値を用いた指標を用いることにより、従来は肝生検を行う必要があったNASHとNAFLの鑑別を簡便な操作により容易に補助することができる。
(非アルコール性脂肪性肝炎に関連する病態の進行程度の判定を補助する方法)
本発明において、非アルコール性脂肪性肝炎(NASH)に関連する病態であって、脂肪変性、風船様変性、炎症及び線維化からなる群から選ばれる少なくとも1種の病態の進行程度の判定を補助する具体的な方法を以下に説明する。

(1) LDL-TGの存在量
被検血液試料中のLDL-TG量を測定しこれを指標とすることにより、被検血液試料中のLDL-TG量が多いほど、脂肪変性、炎症、又は風船様変性の進行のステージが高いと判定することができる。

具体的には、被検血液試料中のLDL-TG量が12.2mg/dL以上、更には13.7mg/dL以上、特には14.2mg/dL以上である場合には、脂肪変性のステージが1以上に進行している可能性が極めて高いと判定することができる(実施例6-1、図10を参照)。
また、被検血液試料中のLDL-TG量が15.1mg/dL以上、更には16.9mg/dL以上である場合には、炎症のステージが1以上に進行していると判定することができ、被検血液試料中のLDL-TG量が22.0mg/dL以上、更には22.5mg/dL以上である場合には、炎症のステージが3以上に進行している可能性が高いと判定することができる(実施例8、図15参照)。
また、被検血液試料中のLDL-TG量が15.1mg/dL以上、更には17.2mg/dL以上、特には19.5mg/dL以上である場合には、風船様変性のステージが1以上に進行している可能性が非常に高いと判定することができる(実施例10-1、図17参照)。
また、被検血液試料中のLDL-TG量が15.1mg/dL以上、更には16.9mg/dL以上である場合には、炎症のステージが1以上に進行していると判定することができ、被検血液試料中のLDL-TG量が22.0mg/dL以上、更には22.5mg/dL以上である場合には、炎症のステージが3以上に進行している可能性が高いと判定することができる(実施例8、図15参照)。
また、被検血液試料中のLDL-TG量が15.1mg/dL以上、更には17.2mg/dL以上、特には19.5mg/dL以上である場合には、風船様変性のステージが1以上に進行している可能性が非常に高いと判定することができる(実施例10-1、図17参照)。
(2) LDL-TG/sd LDL-C比
被検血液試料中のLDL-TG量及びsd LDL-C量を測定し、LDL-TG/sd LDL-C比を指標とすることにより、被検血液試料中のLDL-TG/sd LDL-C比が低いほど脂肪変性の進行のステージが高いと判定することができる。ここで、sd LDL-C量とは、小粒子低密度リポ蛋白質(sd LDL)中のコレステロール(C)の存在量を意味する。具体的には、被検血液試料中のLDL-TG/sd LDL-C比が1.11以下、更には0.1~1.06である場合には、脂肪変性のステージが1以上に進行している可能性が高いと判定することができる(実施例6-2、図11参照)。
被検血液試料中のLDL-TG量及びsd LDL-C量を測定し、LDL-TG/sd LDL-C比を指標とすることにより、被検血液試料中のLDL-TG/sd LDL-C比が低いほど脂肪変性の進行のステージが高いと判定することができる。ここで、sd LDL-C量とは、小粒子低密度リポ蛋白質(sd LDL)中のコレステロール(C)の存在量を意味する。具体的には、被検血液試料中のLDL-TG/sd LDL-C比が1.11以下、更には0.1~1.06である場合には、脂肪変性のステージが1以上に進行している可能性が高いと判定することができる(実施例6-2、図11参照)。
また、被検血液試料中のLDL-TG/sd LDL-C比が高いほど線維化の進行のステージが高いと判定することができる。具体的には、被検血液試料中のLDL-TG/sd LDL-C比が0.39以上、更には0.60以上である場合には、線維化のステージが2以上に進行している可能性が極めて高いと判定することができ、被検血液試料中のLDL-TG/sd LDL-C比が0.61以上、更には0.72以上、特には0.88以上である場合には、線維化のステージが4以上に進行している可能性が高いと判定することができる(実施例11-3、図22を参照)。
更に、被検血液試料中のsd LDL-C/LDL-TG比を指標とすることにより、脂肪変性又は線維化の進行ステージを判定することもできる。この場合、sd LDL-C/LDL-TG比は前記LDL-TG/sd LDL-C比の逆比である(図12、図23)。
(3) ApoE rich HDL-C/HDL-C比
被検血液試料中のApoE rich HDL-C量及びHDL-C量を測定し、ApoE rich HDL-C/HDL-C比を指標とすることにより、被検血液試料中のApoE rich HDL-C/HDL-C比が高いほど脂肪変性の進行のステージが高いと判定することができる。具体的には、被検血液試料中のApoE rich HDL-C/HDL-C比が0.089以上、更には0.09以上である場合には、脂肪変性のステージが1以上に進行している可能性が高いと判定することができる(実施例7-1、図13参照)。
被検血液試料中のApoE rich HDL-C量及びHDL-C量を測定し、ApoE rich HDL-C/HDL-C比を指標とすることにより、被検血液試料中のApoE rich HDL-C/HDL-C比が高いほど脂肪変性の進行のステージが高いと判定することができる。具体的には、被検血液試料中のApoE rich HDL-C/HDL-C比が0.089以上、更には0.09以上である場合には、脂肪変性のステージが1以上に進行している可能性が高いと判定することができる(実施例7-1、図13参照)。
また、被検血液試料中のApoE rich HDL-C/HDL-C比を指標とすることにより、被検血液試料中のApoE rich HDL-C/HDL-C比が低いほど線維化の進行のステージが高いと判定することができる。具体的には、被検血液試料中のApoE rich HDL-C/HDL-C比が0.095以下、更には0.094以下、特には0.092以下である場合には、線維化のステージが2以上に進行している可能性が高いと判定することができ、被検血液試料中のApoE rich HDL-C/HDL-C比が0.094以下、更には0.086以下、特には0.084以下である場合には、線維化のステージが4以上に進行している可能性が高いと判定することができる(実施例12-1、図24参照)。
更に、被検血液試料中のHDL-C/ApoE rich HDL-C比を指標とすることにより、脂肪変性又は線維化の進行ステージを判定することもできる。この場合、HDL-C/ApoE rich HDL-C比は前記ApoE rich HDL-C/HDL-C比の逆比である(図14、図25)。
(4) ApoE rich HDL-C量
被検血液試料中のApoE rich HDL-C量を測定しこれを指標とすることにより、被検血液試料中のApoE rich HDL-C量が少ないほど炎症の進行のステージが高いと判定することができる。具体的には、被検血液試料中のApoE rich HDL-C量が5.5mg/dL以下、更には4.6mg/dL以下、特には4.4mg/dL以下である場合には、炎症のステージが1又は2に進行している可能性が極めて高いと判定することができる(実施例9、図16を参照)。
被検血液試料中のApoE rich HDL-C量を測定しこれを指標とすることにより、被検血液試料中のApoE rich HDL-C量が少ないほど炎症の進行のステージが高いと判定することができる。具体的には、被検血液試料中のApoE rich HDL-C量が5.5mg/dL以下、更には4.6mg/dL以下、特には4.4mg/dL以下である場合には、炎症のステージが1又は2に進行している可能性が極めて高いと判定することができる(実施例9、図16を参照)。
(5) LDL-TG/LDL-C比
被検血液試料中のLDL-TG量とLDL-C量を測定し、LDL-TG/LDL-C比を求めて指標とすることにより、被検血液試料中のLDL-TG/LDL-C比が高いほど風船様変性の進行のステージが高いと判定することができる。具体的には、被検血液試料中のLDL-TG/LDL-C比が0.15以上、更には0.18以上、特には0.19以上である場合には、風船様変性のステージが1以上に進行している可能性が高いと判定することができる(実施例10-2、図18参照)。
被検血液試料中のLDL-TG量とLDL-C量を測定し、LDL-TG/LDL-C比を求めて指標とすることにより、被検血液試料中のLDL-TG/LDL-C比が高いほど風船様変性の進行のステージが高いと判定することができる。具体的には、被検血液試料中のLDL-TG/LDL-C比が0.15以上、更には0.18以上、特には0.19以上である場合には、風船様変性のステージが1以上に進行している可能性が高いと判定することができる(実施例10-2、図18参照)。
また、被検血液試料中のLDL-TG/LDL-C比を指標とすることにより、被検血液試料中のLDL-TG/LDL-C比が高いほど線維化の進行のステージが高いと判定することができる。具体的には、被検血液試料中のLDL-TG/LDL-C比が0.14以上、更には0.17以上である場合には、線維化のステージが2以上に進行している可能性が極めて高いと判定することができる(実施例11-1、図20参照)。
更に、被検血液試料中のLDL-C/LDL-TG比を指標とすることにより、風船様変性又は線維化の進行ステージを判定することもできる。この場合、LDL-C/LDL-TG比は前記LDL-TG/LDL-C比の逆比である(図19、図21)。
以上の通り、被検血液試料中のLDL-TGの存在量、LDL-TG/sd LDL-C比、ApoE rich HDL-C/HDL-C比、ApoE rich HDL-C量、LDL-TG/LDL-C比を指標として、NASHに関連する病態の進行程度の判定を補助する方法を説明したが、これらの指標はいずれかを単独で用いても十分に判定を補助する効果があるが、複数の指標を組み合わせて用いると前記病態の進行程度の判定を更に効果的に補助することができる。
このように被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定し、これらの測定値を用いた指標を用いることにより、従来は肝生検を行う必要があったNASHに関連する病態の進行ステージの判定を、簡便な操作により容易に補助することができる。
本発明において、血液試料中に含まれるLDL-TG、ApoE rich HDL-C、HDL-C、LDL-C、及びsd LDL-Cの存在量を測定する方法としては、従来から公知の方法を用いることができる。例えば、超遠心法、電気泳動法又は高速液体クロマトグラフィー等の分画分離操作により対象のリポ蛋白を分離した後に、定量操作によりトリグリセリドやコレステロールを定量する方法や、分画操作を行うことなく第1工程で対象のリポ蛋白以外の全てのリポ蛋白中のトリグリセリドやコレステロールを除去した後に、第2工程で対象のリポ蛋白中のトリグリセリドやコレステロールを測定する方法等が挙げられる。
具体的には、LDL-TG量の測定方法としてはWO2013/157642に記載された方法を用いることができ、HDL-C量の測定方法としてはWO98/26090に記載された方法を用いることができ、LDL-C量の測定方法としてはWO98/47005に記載された方法を用いることができ、sd LDL-C量の測定方法としてはWO2009/048143に記載された方法を用いることができる。
以下、本発明を実施例に基づき具体的に説明するが、本発明は下記実施例に限定されるものではない。
<比較例1>
健常者(図においてHealthと表記)36名、NAFL患者9名、NASH患者35名からなる全80名の集団から採取した血液中の総TG量を測定し、それぞれの群を比較した。総TG量は臨床検査の場で用いられている自動分析装置を用いてトリグリセリド測定用試薬であるTG-EX「生研」(酵素法)(デンカ生研社製)により測定した。比較したグラフを図1に示す。
その結果、NASH群の総TG量は、健常者群と比べて有意に高かった(p<0.0001)が、NAFL群と差はなかった。NAFL群の総TG量は健常者群と比べて有意に高かった(p<0.05)。

比較例1と同じ、健常者(Health)36名、NAFL患者9名、NASH患者35名からなる全80名の集団から採取した血液中の総コレステロール(TC)量と総TG量を測定し総TG/TCの比を求め、それぞれの群を比較した。総TG量は比較例1と同様の方法により測定し、TC量は臨床検査の場で用いられている自動分析装置を用いてコレステロール測定用試薬である自動分析用試薬「生研」T-CHO(S)(酵素法)(デンカ生研社製)により測定した。比較したグラフを図3に示す。
その結果、NASH群の総TG/TC比は、健常者群と比べて有意に高かった(p<0.0001)が、NAFL群と比べて差はなかった。また、NAFL群の総TG/TC比は健常者群と比べて差はなかった。

その結果、NASH群の総TG/TC比は、健常者群と比べて有意に高かった(p<0.0001)が、NAFL群と比べて差はなかった。また、NAFL群の総TG/TC比は健常者群と比べて差はなかった。
<実施例1>

その結果、NASH群のLDL-TG量は、NAFL群と比べて有意に高く(p<0.05)、健常者群と比べて有意に高かった(p<0.0001)。また、NAFL群のLDL-TG量は、健常者群と比べて有意に高かった(p<0.05)。
比較例1と同じ、健常者(Health)36名、NAFL患者9名、NASH患者35名からなる全80名の集団から採取した血液中のLDL-TG量を測定し、それぞれの群を比較した。LDL-TG量は臨床検査の場で用いられている自動分析装置を用いてLDL-トリグリセライド測定用試薬であるLDLTG-EX“SEIKEN”(デンカ生研社製)により測定した。比較したグラフを図4に示す。
その結果、NASH群のLDL-TG量は、NAFL群と比べて有意に高く(p<0.05)、健常者群と比べて有意に高かった(p<0.0001)。また、NAFL群のLDL-TG量は、健常者群と比べて有意に高かった(p<0.05)。
<実施例2-1>
比較例1と同じ、健常者(Health)36名、NAFL患者9名、NASH患者35名からなる全80名の集団から採取した血液中のLDL-TG量とLDL-C量を実施例1及び比較例2と同様の方法により測定し、LDL-TG/LDL-Cの比を求め、それぞれの群を比較した。比較したグラフを図5に示す。

<比較例1><比較例2><比較例3>より、総TG量やLDL-C量やTC量を測定することによりNASHとNAFLを明確に差別化することはできなかった。しかしながら、本発明の<実施例1>によれば、LDL-TG量を測定することによりNASHとNAFLと健常者の三者を明確に差別化することができた。また、本発明の<実施例2-1><実施例2-2>によれば、LDL-TGとLDL-Cの比を算出することにより、更に明確にNASHとNAFLを差別化することができた。
<比較例4>

<比較例4>より、HDL-C量を単独で比較しても、NASHとNAFLを差別化することはできなかったが、<実施例3-1><実施例3-2>によれば、ApoE rich HDL-CとHDL-Cの比を算出することにより、明確にNASHとNAFLを差別化することができた。
<実施例4>
NAFL患者9名、NASH患者35名からなる全44名のNAFLD集団から採取した血液中のLDL-TG量を測定し、ROC解析を行ったところ、AUCは0.78(95%CI:0.63~0.92)と良好であった。
この集団のカットオフ値を15.1mg/dLに設定したときの感度、特異度はそれぞれ82.9%、55.6%であり、17.6mg/dLに設定したときの感度、特異性はそれぞれ、65.7%、88.9%であった。更にはカットオフ値を12.1mg/dLに設定したときの感度、特異度はそれぞれ、94.3%、22.2%であり、カットオフ値を20.2mg/dLに設定したときの感度、特異度はそれぞれ48.6%、100%であった。
NAFL患者9名、NASH患者35名からなる全44名のNAFLD集団から採取した血液中のLDL-TG量を測定し、ROC解析を行ったところ、AUCは0.78(95%CI:0.63~0.92)と良好であった。

本発明の<実施例7-1><実施例7-2>によれば、ApoE rich HDL-CとHDL-Cの比を評価することで、脂肪変性(Steatosis)の病態進行を推測することが出来る。
<実施例8>
炎症(Inflammation)のステージが判定されているNAFLD患者群(n=38)から採取した血液中のLDL-TG量を実施例1と同様の方法により測定し、それぞれのステージを比較した。比較したグラフを図15に示す。
その結果、LDL-TG量は、ステージ3群がステージ0,1,2群に対して有意に高く(p<0.001,p<0.05,p<0.05)、ステージ2群がステージ0群に対して有意に高く(p<0.05)、ステージ1群がステージ0群に対して有意に高かった(p<0.0001)。

その結果、LDL-TG量は、ステージ3群がステージ0,1,2群に対して有意に高く(p<0.001,p<0.05,p<0.05)、ステージ2群がステージ0群に対して有意に高く(p<0.05)、ステージ1群がステージ0群に対して有意に高かった(p<0.0001)。
<実施例9>
炎症(Inflammation)のステージが判定されているNAFLD患者群(n=38)から採取した血液中のApoE rich HDL-C量を実施例3-1と同様の方法により測定し、それぞれのステージを比較した。比較したグラフを図16に示す。
その結果、ApoE rich HDL-C量は、ステージ1群がステージ0群に対して有意に低かった(p<0.05)。

本発明の<実施例8><実施例9>によれば、LDL-TG量やApoE rich HDL-C量を評価することで、炎症(Inflammation)の病態進行を推測することが出来る。

風船様変性(Ballooning)のステージが判定されているNAFLD患者群(n=37)から採取した血液中のLDL-TG量を実施例1と同様の方法により測定し、それぞれのステージを比較した。比較したグラフを図17に示す。
その結果、LDL-TG量は、ステージ1と2を合わせた群がステージ0群に対して有意に高かった(p<0.01)。

その結果、LDL-C/LDL-TG比は、ステージ2,3,4群がステージ1群に対して有意に低かった(p<0.005,p<0.005,p<0.0005)。

その結果、LDL-TG/sd LDL-C比は、ステージ4群がステージ1群に対して有意に高く(p<0.05)、ステージ3群に対して高い傾向であった。また、ステージ2群はステージ1群に対して高い傾向であった。

その結果、sd LDL-C/LDL-TG比は、ステージ4群がステージ1、3群に対して有意に低かった(p<0.005,p<0.05)。また、ステージ2群はステージ1群に対して低い傾向であった。
線維化(Fibrosis)のステージが判定されているNASH患者群(n=27)から採取した血液中のLDL-TG量およびsd LDL-C量を実施例1及び実施例6-2と同様の方法により測定してその比を求め、それぞれのステージ群で比較した。比較したグラフを図23に示す。
その結果、sd LDL-C/LDL-TG比は、ステージ4群がステージ1、3群に対して有意に低かった(p<0.005,p<0.05)。また、ステージ2群はステージ1群に対して低い傾向であった。
本発明の<実施例11-1~4>によれば、LDL-TGとLDL-Cとの比やLDL-TGとsd LDL-Cとの比を評価することで、線維化(Fibrosis)の病態進行を推測することが出来る。
<実施例12-1>
線維化(Fibrosis)のステージが判定されているNASH患者群(n=27)から採取した血液中のApoE rich HDL-C量およびHDL-C量を実施例3-1及び比較例4と同様の方法により測定してその比を求め、それぞれのステージ群で比較した。比較したグラフを図24に示す。
ApoE rich HDL-C/HDL-C比は、ステージ4群がステージ1,2群に対してそれぞれ有意に低かった(p<0.005,p<0.05)。

<実施例13-1>
実施例4と同じ集団の血中ApoE rich HDL-C量とHDL-C量を測定し、ApoE rich HDL-C/HDL-C比を算出し、ROC解析を行ったところ、AUCは0.80(95%CI:0.65~0.94)と良好であった。
この集団のApoE rich HDL-C/HDL-C比のカットオフ値を0.0912に設定したときの感度、特異度が80.0%、66.7%であり、カットオフ値を0.0899に設定したときの感度、特異度が74.3%、88.9%であった。更にはカットオフ値を0.0840に設定したときの感度、特異度が42.9%、100%であり、カットオフ値を0.0971に設定したときの感度、特異度が91.4%、11.1%であった。
<実施例13-1>

Claims (5)
- 生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することを含む、非アルコール性脂肪性肝炎の検出を補助する方法。
- 前記被検血液試料中のLDL-TGの存在量を指標とし、前記LDL-TGの存在量が非アルコール性脂肪肝を患う患者の血液試料中のLDL-TGの存在量よりも多いことが、非アルコール性脂肪性肝炎を発症している可能性が高いことを示す、請求項1記載の方法。
- 前記被検血液試料中のLDL-TG/LDL-C比を指標とし、前記LDL-TG/LDL-C比が非アルコール性脂肪肝を患う患者の血液試料中のLDL-TG/LDL-C比よりも高いことが、非アルコール性脂肪性肝炎を発症している可能性が高いことを示す、請求項1又は2記載の方法。
- 前記被検血液試料中のApoE rich HDL-C/HDL-C比を指標とし、前記ApoE rich HDL-C/HDL-C比が非アルコール性脂肪肝を患う患者の血液試料中のApoE rich HDL-C/HDL-C比よりも低いことが、非アルコール性脂肪性肝炎を発症している可能性が高いことを示す、請求項1~3のいずれか1項に記載の方法。
- 生体から分離された被検血液試料中に含まれるLDL-TG及び/又はApoE rich HDL-Cの存在量を測定することを含む、非アルコール性脂肪性肝炎に関連する病態であって、脂肪変性、炎症、風船様変性、及び線維化からなる群から選ばれる少なくとも1種の病態の進行程度の判定を補助する方法。
