Abstract
Marcador (1 ) quirúrgico, que comprende: - un mango (2) alargado asible con la mano a modo de rotulador, y - unos medios de fijación intercambiable de unas puntas (3, 4, 5) intercambiables, provistos en dicho mango (2), donde las puntas intercambiables comprenden: - una punta de material poroso empapable en tinta (3) para marcado en piel, - una punta de grafito (4) para marcado en hueso, y - una punta inyectora (5) para marcados de precisión de puntos para el diseño de plastias cutáneas mediante inyección en dermis y que no se borre con el lavado quirúrgico.

MARCADOR QUIRURGICO
DESCRIPCIÓN OBJETO DE LA INVENCIÓN
La presente invención se refiere a un marcador quirúrgico. Se encuadra en el sector de la técnica de instrumental médico-quirúrgico. ANTECEDENTES DE LA INVENCION
Los marcadores quirúrgicos actuales básicamente son unos rotuladores estériles de diferente grosor de punta y colores, que utilizan distintos tipos de tintas (azul de metileno, violeta de genciana, etc) que funcionan fundamentalmente en superficies secas y que con el lavado quirúrgico se atenúan o desaparecen. Ninguno de ellos es capaz de pintar o marcar sobre superficies húmedas ni hueso de forma duradera.
Para conseguir marcar por debajo de la epidermis se conocen por ejemplo marcadores como el descrito en el documento US4789582, que comprende un cartucho de aguja para una pluma de operación quirúrgica o instrumento de tatuaje que tiene un depósito lleno de pigmento líquido adaptado para ser descargado en la piel a través de una aguja con tres puntas de actuación simultánea; si bien requiere una pluma específica con impulsión por aire.
También el documento US5496304 recoge un aparato y método que incluye un miembro de penetración hacia el interior de la capa más externa de la epidermis y un depósito suministra un agente de marcado de modo que a medida que el miembro de penetración penetra en la epidermis, agente de marcado se introduce bajo la superficie más externa de la misma, utilizando como marcador por ejemplo azul de metileno. El miembro de penetración tiene forma de rueda con púas para marcar líneas. Por tanto no es adecuado para utilización en marcados puntiformes o que no requieran desplazamientos más o menos largos del marcador, como por ejemplo en partes anatómicas con pliegues o poco espacio, y solo es utilizable sobre la piel. Con una configuración similar y las mismas ventajas e inconvenientes se conoce el marcador propuesto en el documento US5713890 y en el documento US2004/01 16907.
El documento US5909978 describe un marcador que comprende un cartucho con tintas,
hermético al aire a excepción de una abertura en el extremo distal -para recibir la punta de un marcador- y un orificio de ventilación, y donde las paredes del cuerpo de la pluma que porta el marcador son flexibles para producir la salida de las tintas, regulando también con la cobertura parcial del orificio de ventilación. De esta forma busca forzar la salida y producir un flujo constante de tinta evitando el inconveniente de la obstrucción de la punta o la fallo de funcionamiento por mezclarse la punta con grasas o restos tisulares, si bien no es capaz de inyectar subcutáneamente.
También se conoce el documento US6197034, que describe un marcador útil para utilización en el tejido blando y duro de un mamífero, así como una pluma que contiene un dispositivo de marcado médico y métodos de uso de tales plumas para hacer líneas y marcas en el tejido blando y duro de un mamífero. La pluma, que se puede utilizar para otras aplicaciones de marcado quirúrgico se utiliza como un instrumento de escritura ordinaria. Comprende un alojamiento con un depósito interior con tintas y dos extremos, uno de ellos cerrado mientras que el opuesto tiene un miembro extendido con un canal, y un tubo hueco alineado coaxialmente con el canal. En el interior del depósito hay una masa desplazable verticalmente unida a un vástago de longitud algo mayor que el tubo hueco, de forma que la masa obtura la salida de las tintas mientras que el vástago no sea empujado superiormente. Es igual a ciertos dispositivos de dibujo lineal antiguos y presenta los mismos inconvenientes de secado de las tintas en el tubo hueco, además de la salida incontrolada de las tintas en determinadas circunstancias
El documento US2013/0197358 describe otro marcador quirúrgico que comprende: un cuerpo de la pluma con una primera cámara que contiene un colorante en polvo, con una segunda cámara separada de la primera que contiene un disolvente, y en donde la primera cámara y la segunda cámara se colocan selectivamente en comunicación de fluido uno con el otro y, una punta en comunicación fluida con la primera o la segunda cámara, en el que el al menos un colorante en polvo y el disolvente, cuando se mezcla, forma una solución de colorante que es desechable sobre una superficie de la piel o tejido y es capaz de ser visualizado de forma simultánea por un usuario sin ayuda y con un generador de imágenes de fluorescencia. Sirve para marcado de zonas que antes precisaban necesariamente generadores de luz fluorescente para visualización, pero requiere la configuración específica con las diferentes cámaras, y tampoco asegura la permanencia del marcado con los lavados quirúrgicos.
También se conoce el documento US2015/01 19866, que describe un marcador quirúrgico para procedimientos laparoscópicos, el cual se conecta a una barra y puede ser empujado a través de una cánula laparoscópica al tejido diana que ha de ser marcado. El marcador puede regular (disminuir) su longitud moviendo la ubicación del depósito de tinta desde la ubicación en un marcador típico a una ubicación en una tapa, y comprende un conector que tiene un casquillo formado en el mismo para recibir una clavija de una varilla y una punta para contener y distribuir la tinta sobre el tejido,
El documento US 5665092 describe un marcador quirúrgico para huesos que comprende una cánula guía tubular, un tubo insertable dentro de la cánula que tiene un extremo proximal y un extremo distal , una barra flexible y desplazable que se extiende a lo largo y dentro del tubo, y un puntero de marcado que está unido a un extremo proximal de la barra flexible y se extiende más allá del extremo proximal del tubo, y que permite marcar el lugar de operación con precisión y con mínimo dolor para el paciente. El marcado se produce por erosión ósea y el marcador en general tiene una constitución y manejo complejos.
Todos estos marcadores quirúrgicos tienen un uso exclusivo, ya sea de marcación subcutánea, lineal o no, de erosión del hueso o de marcado simple, por lo que su versatilidad es muy limitada. Además en pocos casos se asegura la permanencia del marcado frente a los lavados quirúrgicos.
DESCRIPCION DE LA INVENCION
El marcador quirúrgico de la invención tiene una configuración con puntas multisuperficie de pequeño tamaño intercambiables, pudiendo usar sólo aquellas necesarias según la técnica y tejido a marcar, y permitiendo marcados del campo quirúrgico que los dispositivos actuales no hacen.
Además, el marcado realizado en cada caso es resistente a los lavados quirúrgicos y realiza un marcado duradero.
De acuerdo con la invención, el marcador comprende:
-un mango alargado asible con la mano a modo de rotulador o útil de escritura, y
-unos medios de fijación intercambiable de unas puntas intercambiables, provistos en dicho mango y en las puntas, y donde las puntas intercambiables comprenden:
-una punta de material poroso empapable en tinta, similar a la de un rotulador, para marcado en
piel,
-una punta de grafito para marcado en hueso aunque este esté húmedo o con sangre y que permite usar sierras con irrigación sin desaparecer y tener que remarcar, y
-una punta inyectora, para marcados de precisión de puntos subcutáneos para el diseño de plastias cutáneas mediante inyección en dermis, y que no se borre con el lavado quirúrgico, de forma que en un mismo acto quirúrgico es posible usar varias puntas según la técnica a realizar.
Además, el marcado realizado en cada caso es perdurable durante los trabajos, ya que la punta de material poroso empapable en tinta se usa con tinta resistente a la humedad, la punta de grafito permite marcar sobre le hueso aunque esté húmedo y permite usar sierras con irrigación sin desaparecer y tener que remarcar, y en la inyección subcutánea la propia piel protege a la tinta.

Las figuras 2, 3 y 4 muestran respectivamente una vista lateral del marcador de la invención con la punta de material poroso empapable en tinta, la punta de grafito y la punta inyectora montadas, grafiando las líneas ocultas y con las tapas de las puntas seccionadas.
La figura 5 muestra una vista lateral, alzado y planta inferior del marcador de la invención con la punta inyectora montada, y donde en líneas ocultas en la vista en alzado se aprecia el resalte macho de los medios de fijación intercambiable de la punta en el mango introducido en la muesca hembra.
La figura 6 muestra un detalle ampliado del clipaje de una de las puntas con el mango del marcador de la invención. Este clipaje es igual para todas las puntas.
DESCRIPCION DE UNA REALIZACION PRACTICA DE LA INVENCION
El Marcador (1 ) quirúrgico de la invención comprende (ver fig 1 ):
- un mango (2) en forma de cuerpo de rotulador, tubular, asible con la mano, y
- unos medios de fijación intercambiable de unas puntas (3, 4, 5) intercambiables, provistos dicho mango (2) y en las puntas (3, 4, 5),
donde las puntas intercambiables comprenden:
-una punta de material poroso empapable en tinta (3) para marcado en piel,
-una punta de grafito (4) para marcado en hueso aunque este esté húmedo o con sangre, - una punta inyectora (5) para marcados de precisión de puntos para el diseño de plastias cutáneas mediante inyección en dermis y que no se borre con el lavado quirúrgico. Esta punta es especialmente útil para el marcado de colgajos de geometría precisa como en el labio leporino, otras plastias de geometría precisa y la realización de otoplastias.
Todo ello de forma que en un mismo acto quirúrgico es posible usar varias puntas según la técnica a realizar.
Las puntas de material poroso empapable en tinta (3) comprenden un aplicador (31 ) de dicho material y una reserva (30) del mismo o similar material, que aumentan la cantidad de tinta contenida y por lo tanto su autonomía. El aplicador (31 ) y la reserva (30) están montados en un primer soporte (32) donde están implementados los medios de fijación intercambiable al mango (2). Además, dichas puntas de material poroso empapable en tinta (3) se encuentran empapadas en tinta resistente a la humedad, y comprenden tintas en varios colores y/o grosores, disponiendo, al menos, tres grosores diferentes: fino, medio y grueso. Por su parte, la punta de grafito (4) comprende una porción de grafito (41 ) de dureza, al menos, HD de forma que permita un marcado óseo efectivo, que se encuentra montada en un segundo soporte (42) donde están implementados los medios de fijación intercambiable al mango (2).
En cuanto a la punta inyectora (5), comprende idealmente una aguja hueca (50) para la inyección subcutánea de la tinta, comunicada con un depósito (51 ) elástico, comprimible a través de un orificio (20) provisto en el mango (2) que permite pulsar dicho depósito (51 ) elástico proporcionando la presión suficiente para inyectar subcutáneamente la tinta a través de la aguja hueca (51 ). Dicha aguja hueca (50) y el depósito (51 ) están montados en un tercer soporte (53) donde están implementados los medios de fijación intercambiable al mango (2). Además, dicho tercer soporte (53) comprende preferentemente una palanca elástica (52) flexible recuperablemente en coincidencia posicional con el orificio (20) y con el depósito (51 ) para comprimir éste con mayor facilidad, encontrándose dispuesto el orificio (20) en el extremo del mango (2) donde se encuentran provistos los medios de fijación intercambiable de las puntas (3, 4, 5) y en coincidencia posicional con el deposito (51 ), de forma que se tiene un depósito de capacidad suficiente, y no demasiado grande como para que la tinta se seque o la presión ejercida se disperse. La tinta utilizada en la punta inyectora (5) sería azul de metileno o violeta
de genciana, que no se borrará con el lavado quirúrgico, manteniéndose en el lugar diseñado inicialmente aunque ya esté abierta la herida.
Los medios de fijación intercambiable de las puntas (3, 4, 5) intercambiables provistos en el mango (2) comprenden en esta realización preferente (ver fig 6) un clipaje anular (19) extraíble y una muesca hembra (9) y un resalte macho (10) complementario, lo que hace que la unión no sea rotatoria y que el encaje sea rápido y eficaz. Dicha muesca hembra (9) está provista en el mango (2) y el resalte macho (10) en las puntas (3, 4, 5) como se muestra en las figuras, o viceversa.
Además, se ha previsto que el mango (2) comprenda una regla (21 ) (ver fig 5) para medir los marcados, la cual es posible usar con las puntas (3, 4, 5) montadas o desmontadas en el marcador (1 ). Dicha regla (21 ) está idealmente graduada en milímetros, y puede implementarse en el mango (2) mediante impresión y/o moldeado.
Las puntas (3, 4, 5) comprenden idealmente una tapa (8) para que no se seque el material poroso y/o para proteger el grafito y aguja de inyección. Dicha tapa (8) se encuentra fijada mediante medios de encaje a la punta (3, 4, 5) correspondiente, cuyos medios de encaje presentan una resistencia a la tracción menor que los medios de fijación intercambiable de las puntas (3, 4, 5) (que el clipaje anular (19) en este ejemplo), para poder extraer la tapa (8) sin desmontar la punta montada en el mango (2). Dichos medios de encaje comprenden por ejemplo un resalte anular (1 1 ) y un encaje anular (12) para dicho resalte anular (1 1 ); estando dicho resalte anular (1 1 ) dispuesto en la punta (3, 4, 5) y el encaje anular en la tapa (8), o viceversa.
Además, la punta (3, 4, 5) comprende un resalte de sellado (15) troncocónico contra un rebaje interior (80) de la tapa (8), para evitar el paso de aire y secado de las tintas.
Por último, indicar que el mango (2) y/o las puntas (3, 4, 5) se encuentran materializadas en plástico para abaratar costes, y que las puntas (3, 4, 5) comprenden un tratamiento de esterilización previo y un empaquetado estéril, no representado, de protección para uso inmediato.
Descrita suficientemente la naturaleza de la invención, así como la manera de realizarse en la práctica, debe hacerse constar que las disposiciones anteriormente indicadas y representadas en los dibujos adjuntos son susceptibles de modificaciones de detalle en cuanto no alteren el principio fundamental.
Claims
1 .-Marcador (1 ) quirúrgico caracterizado porque comprende:
-un mango (2) alargado para asido con la mano a modo de útil de escritura, y
-unos medios de fijación intercambiable de unas puntas (3, 4, 5) intercambiables, provistos en dicho mango (2) y en las puntas (3, 4, 5),
donde las puntas intercambiables comprenden:
-una punta de material poroso empapable en tinta (3) para marcado en piel,
-una punta de grafito (4) para marcado en hueso, y
-una punta inyectora (5) para marcados de precisión de puntos para el diseño de plastias cutáneas mediante inyección en dermis.
2. -Marcador (1 ) quirúrgico según reivindicación 1 caracterizado porque las puntas de material poroso empapable en tinta (3) comprenden un aplicador (31 ) de dicho material y una reserva (30); encontrándose el aplicador (31 ) y la reserva (30) montados en un primer soporte (32) donde están implementados los medios de fijación intercambiable al mango (2).
3. -Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones 1 o 2 caracterizado porque las puntas de material poroso empapable en tinta (3) se encuentran empapadas en tinta resistente a la humedad.
4. -Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones 1 a 3 caracterizado porque comprende puntas de material poroso empapable en tinta (3) de varios grosores.
5.-Marcador (1 ) quirúrgico según reivindicación 1 caracterizado porque la punta de grafito (4) comprende una porción de grafito (41 ) de dureza, al menos, HD, que se encuentra montada en un segundo soporte (42) donde están implementados los medios de fijación intercambiable al mango (2).
6.-Marcador (1 ) quirúrgico según cualquiera de las reivindicación 1 caracterizado porque la punta inyectora (5) comprende una aguja hueca (50) para la inyección subcutánea de la tinta
comunicada con un depósito (51 ) elástico comprimible a través de un orificio (20) provisto en el mango (2); encontrándose la aguja hueca (50) y el depósito (51 ) montados en un tercer soporte (53) donde están implementados los medios de fijación intercambiable al mango (2).
7.-Marcador (1 ) quirúrgico según reivindicación 6 caracterizado porque el tercer soporte (53) comprende una palanca elástica (52) flexible recuperablemente en coincidencia posicional con el orificio (20), para comprimir el depósito (51 ).
8. -Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones 6 o 7 caracterizado porque la tinta utilizada en la punta inyectora (5) se encuentra seleccionada entre:
azul de metileno, o
violeta de genciana.
9. -Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones anteriores caracterizado porque los medios de fijación intercambiable de las puntas (3, 4, 5) intercambiables provistos en el mango (2) comprenden un clipaje anular (19) extraíble y una muesca hembra (9) y un resalte macho (10) complementario antigiro; encontrándose la muesca hembra (9) provista en el mango (2) y el resalte macho (10) en las puntas (3, 4, 5) o viceversa.
10.-Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones anteriores caracterizado porque el mango (2) comprende una regla (21 ) para medir los marcados.
1 1 .-Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones anteriores caracterizado porque las puntas (3, 4, 5) comprenden una tapa (8).
12. -Marcador (1 ) quirúrgico según reivindicación 1 1 caracterizado porque la tapa (8) se encuentra fijada a través de medios de encaje a la punta (3, 4, 5) correspondiente; cuyos medios de encaje presentan una resistencia a la tracción menor que los medios de fijación intercambiable de unas puntas (3, 4, 5).
13. -Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones anteriores caracterizado porque la punta (3, 4, 5) comprende un resalte de sellado (15) contra un rebaje interior (80) de la tapa (8).
14.-Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones anteriores caracterizado porque el mango (2) y/o las puntas (3, 4, 5) se encuentran materializadas en plásticos.
15.-Marcador (1 ) quirúrgico según cualquiera de las reivindicaciones anteriores caracterizado porque las puntas (3, 4, 5) comprenden un tratamiento de esterilización previo y un empaquetado estéril de protección.
