살아있는 사람의 관상동맥 혈관을 고속으로 스캐닝하는 장치가 제공된다. 상기 장치는: 제1 대상체의 혈관 내에 삽입되어 회전하면서 혈관벽을 이미징하는 이미징 카테터, 및 상기 이미징 카테터의 한 측에 체결되어 광간섭단층촬영(OCT) 장치로부터 조사된 빛을 상기 이미징 카테터로 전달하고 상기 이미징 카테터를 회전시키는 회전접합부를 포함할 수 있다.

Description

관상동맥 혈관 고속 스캐닝 장치 및 방법
살아있는 사람의 관상동맥(Coronary Artery) 혈관을 고속으로 스캐닝하는 기술에 연관되며, 보다 상세하게는 사람의 관상동맥을 빠른 시간 내에 광간섭단층촬영술(Optical Coherence Tomography, OCT)로 이미징하기 위한 고속 스캐닝 장치 및 그 방법에 연관된다.
관상동맥은 심장 근육에 산소와 영양분을 공급하는 역할을 한다. 최근, 현대인의 식습관 변화 및 스트레스로 인해서 관상동맥 질환을 갖고 있는 인구가 증가하고, 현대인의 주요 사망원인으로 자리매김하게 되었다. 관상동맥 질환 중 대표적인 질환으로는 동맥경화와 심근경색을 꼽을 수 있는데, 이 중 동맥경화는 관상동맥이 다양한 요인에 의해 좁아지게 되는 질환이다. 동맥경화의 증상이 지속되어 발전하면 관상동맥을 흐르는 혈류가 감소하고 관상동맥이 막히게 되는데, 이러한 경우 심근이 괴사하게 되어 심각한 경우에 사람이 사망에 이르게 될 수 있다. 따라서 이러한 관상동맥 질환을 예방하는 것이 중요하며, 이를 위해서는 관상동맥 혈관벽을 관찰하는 이미징 기술이 중요하다.
관상동맥 질환 진단을 위한 이미징 기술로는 대표적으로 혈관조영술(angiography), 혈관 내 초음파(intravascular ultrasound, IVUS) 및 광간섭단층촬영(OCT)이 있다. 이 중 광간섭단층촬영 이미징 시스템은 뛰어난 해상도(resolution)와 민감도(sensitivity)로 혈관 이미징 기술로 각광을 받고 있으며, 임상에서 널리 사용되고 있다. 광간섭단층촬영술을 이용하여 관상동맥 혈관벽을 이미징하기 위해서는 혈관 내에 OCT 이미징용 내시경을 혈관 내에 삽입하여야 한다. 또한, 빛은 혈액에 대해 매우 높은 흡수 계수를 갖기 때문에 혈관벽을 이미징하기 위해서는 혈액을 일시적으로 조영제(contrast agent)로 대체하여 이미징을 수행해야 한다. 다만, 너무 오랫동안 대체재를 이용하는 경우 심근에 산소와 영양분이 공급되지 못하여 심근이 괴사할 수 있고, 심하면 사람이 사망에 이를 수 있기 때문에 수 초 내에 이미징을 마쳐야 한다. 이러한 이유로 관상동맥 이미징을 위한 광간섭단층촬영 이미징 시스템용 고속 스캐닝 장치 개발이 요구된다.
광간섭단층촬영술을 이용하여 살아있는 사람의 관상동맥 혈관벽 이미징을 하려는 시도는 2002년 하버드 의대 연구팀에 의해 세계 최초로 수행되었다. 프레임 속도는 초당 약 4 혹은 8 프레임이었기 때문에 제한된 이미징 시간에 관상동맥의 일부분만이 이미징이 가능했다. 이 연구를 필두로 관상동맥 질환진단을 위한 광간섭단층촬영 시스템 개발이 본격화되었다. 2세대 광간섭단층촬영술(the second generation optical coherence tomography, 2G-0CT)이 관상동맥 질환진단을 위한 광간섭단층촬영 시스템에 적용되면서 임상에 적용하는데 걸림돌이었던 이미징 속도가 향상되었다. 기준거울(reference mirror)를 움직여서 이미지를 획득하던 1세대 광간섭단층촬영술과 달리, 2세대 광간섭단층촬영술은 파장 변환 레이저를 이용하여 이미지를 획득하는 기술이다. 2세대 광간섭단층촬영술은 광 주파수 영역 이미징(optical frequency domain imaging, OFDI) 혹은 swept-source optical coherence tomography, SS-OCT)로 불린다. 광원의 파장 변환 방식의 발전으로 2세대 광간섭단층촬영술의 이미징 속도는 더욱 개선되었고, 2008년에는 하버드 연구팀이 고속의 임상용 2세대 광간섭단층촬영 시스템을 구축하여 세계 최초로 환자의 관상동맥 전부분을 초당 100 프레임 정도의 고속으로 이미징할 수 있었다. 그러나, 혈관 종방향으로의 해상도는 약 200

정도로 혈관의 횡단면 해상도에 비해 수 배 이상 차이가 나기 때문에 정확한 관상동맥 혈관벽 이미지를 얻기에 어려움이 있다. 제한된 이미징 시간 동안 고해상도 이미지를 획득하기 위해, 이미징 속도의 개선만으로는 한계가 있으며, 이미징 과정에서 사용되는 스캐닝부(scanning part)의 속도 또한 중요한 결정인자로서 보다 개선될 필요가 있다.
일측에 따르면, 살아있는 사람의 관상동맥 혈관을 고속으로 스캐닝하는 장치가 제공된다. 상기 장치는: 제1 대상체의 혈관 내에 삽입되어 회전하면서 혈관벽을 이미징하는 이미징 카테터, 및 상기 이미징 카테터의 한 측에 체결되어 광간섭단층촬영(OCT) 장치로부터 조사된 빛을 상기 이미징 카테터로 전달하고 상기 이미징 카테터를 회전시키는 회전접합부를 포함할 수 있다.
일실시예에 따르면, 상기 이미징 카테터는 상기 회전접합부로부터 전달받은 빛을 상기 혈관벽에 조사하고, 상기 혈관벽의 조직으로부터 후방산란(backscattering) 된 빛으로부터 간섭신호를 검출할 수 있다.
일실시예에 따르면, 상기 장치는 상기 회전접합부의 적어도 한 측을 지지하도록 장착되어, 상기 이미징 카테터의 평형이동을 지원하는 풀백 스테이지를 더 포함할 수 있다.
일실시예에 따르면, 상기 이미징 카테터의 양쪽 끝 단 중 제1 측에는 상기 회전접합부에 체결되는 광 커넥터가 부착되고, 상기 제1 측과 상이한 제2 측에는 어느 한 면이 연마된 볼 모양의 렌즈가 융착(splicing)된다.
또한, 상기 이미징 카테터는: 상기 회전접합부와 연결된 광섬유를 이용하여 상기 제1 대상체의 혈관벽을 스캐닝 하는 이미징 프로브, 및 상기 회전접합부의 한 측에 고정되어, 상기 이미징 카테터의 회전 시 상기 이미징 프로브 및 상기 제1 대상체의 혈관을 보호하는 외피를 포함하며, 상기 이미징 프로브는 상기 외피 내에 삽입되어 회전하고, 상기 외피와 미리 지정된 유격을 유지한다.
여기서, 상기 이미징 프로브는: 상기 회전접합부의 어댑터와 체결되어 상기 광간섭단층촬영(OCT) 장치로부터 조사되는 빛을 전달받는 광 커넥터와, 스프링 형태로 감겨있는 토크코일과, 상기 광 커넥터를 통해 상기 회전접합부와 연결되고 상기 토크코일 내에 삽입되어 상기 빛을 상기 제1 대상체의 혈관벽 방향으로 가이드하는 광섬유, 및 상기 광섬유를 통해 전달받은 상기 빛을 혈관벽에 조사하는 볼 렌즈를 포함할 수 있다.
또한, 상기 광섬유는, 폴리미드(polyimide) 재질로 코팅된 것일 수 있다.
일실시예에 따르면, 상기 회전접합부는: 광섬유를 이용하여 상기 광간섭단층촬영(OCT) 장치로부터의 빛을 상기 이미징 카테터로 전달하는 복수 개의 시준기와, 상기 복수 개의 시준기 중 적어도 하나를 고정시키는 고정부, 및 상기 복수 개의 시준기 중 적어도 하나를 회전시키는 회전부를 포함할 수 있다.
이 때, 상기 복수 개의 시준기는: 상기 광간섭단층촬영(OCT) 장치와 광섬유를 통해 연결되어 빛을 전달받는 제1 시준기, 및 상기 제1 시준기로부터 상기 빛을 전달받아 상기 이미징 카테터로 전달하는 제2 시준기를 포함하며, 상기 제1 시준기는 상기 고정부에 결합되고, 상기 제2 시준기는 상기 회전부에 결합되며, 상기 제1 시준기 및 상기 제2 시준기는 서로 대칭하도록 배치될 수 있다.
다른 일측에 따르면, 관상동맥 혈관 고속 스캐닝 장치를 이용하여 제1 대상체의 혈관벽을 스캔하는 방법이 제공된다. 상기 방법은: 상기 고속 스캐닝 장치의 회전접합부가 광간섭단층촬영(OCT) 장치로부터 조사된 빛을 상기 고속 스캐닝 장치의 이미징 카테터로 전달하는 단계와, 상기 이미징 카테터가 제1 대상체의 혈관 내에 삽입되어 평형이동 및 회전이동 하면서 상기 전달받은 빛을 상기 혈관벽에 조사하는 단계, 및 상기 이미징 카테터가 상기 혈관벽의 조직으로부터 후방산란(backscattering) 된 빛으로부터 간섭신호를 검출하는 단계를 포함할 수 있다.
일실시예에 따르면, 상기 조사된 빛을 상기 고속 스캐닝 장치의 이미징 카테터로 전달하는 단계는: 상기 회전접합부의 고정부에 결합된 제1 시준기가, 광섬유를 통해 연결된 광간섭단층촬영(OCT) 장치로부터 빛을 전달받는 단계, 및 상기 회전접합부의 회전부에 결합된 제2 시준기가, 상기 제1 시준기로부터 전달받은 빛을 상기 이미징 카테터로 전달하는 단계를 포함하며, 상기 제1 시준기 및 상기 제2 시준기는 서로 대칭하도록 배치될 수 있다.
일실시예에 따르면, 상기 이미징 카테터는 상기 회전접합부의 적어도 한 측과 체결되어 회전이동 하고, 상기 회전접합부의 적어도 한 측을 지지하도록 장착된 풀백 스테이지에 의해 평형이동 할 수 있다.
일실시예에 따르면, 상기 간섭신호를 검출하는 단계는: 이미징 프로브가 외피 내에 삽입되어 회전하면서, 상기 회전접합부와 연결된 광섬유를 이용하여 상기 제1 대상체의 혈관벽을 스캐닝 하는 단계를 포함할 수 있다.
또한, 상기 이미징 카테터의 양쪽 끝 단 중 제1 측에는 상기 회전접합부에 체결되는 광 커넥터가 부착되고, 상기 제1 측과 상이한 제2 측에는 어느 한 면이 연마된 볼 모양의 렌즈가 융착(splicing)된다.

이미징 카테터(110)는 진단 대상인 제1 대상체(100)의 대퇴부 정맥을 통해 혈관 내로 삽입된 후, 관상동맥까지 도달하여 혈관벽을 이미징 한다. 여기서, 이미징이란, 혈관벽에 빛을 조사(irradiance)하여 혈관벽 조직으로부터 후방산란(backscattering) 된 빛을 받아 간섭신호를 검출하는 과정을 의미한다. 제1 대상체(100)의 혈관벽을 이미징 하기 위해, 이미징 카테터(110)는 OCT 시스템(140)으로부터 전달받은 빛을 혈관벽 조직에 조사하고, 상기 혈관벽 조직에 의해 후방산란 된 빛을 수광하여 다시 OCT 시스템(140)으로 전달할 수 있다. 상술한 바와 같이, 상기 이미징 카테터(110)가 혈관벽의 어느 한 지점에 빛을 조사하는 경우, OCT 시스템(140)에서는 (상기 후방산란 된 빛을 통해) 상기 혈관벽의 깊이 방향으로의 정보를 한꺼번에 획득할 수 있어 단층촬영이 가능해진다. 이 때, 상기 이미징 카테터(110)는 혈관벽에 빛을 조사하여 후방산란 된 빛을 받는 과정에서 1회전 하면서 하나의 혈관 단면 영상을 획득하게 되는데, 혈관벽에 대한 3차원 영상을 획득하기 위해서는 상기 이미징 카테터(110)가 회전하는 동시에 평형이동 할 필요가 있다. 상기 이미징 카테터(110)의 회전 및 평형이동을 위해서는 회전접합부(120) 및 풀백 스테이지(130)가 필요하다.
회전접합부(120)는 상기 이미징 카테터(110)에 체결될 수 있는 구조로서, OCT 시스템(140)으로부터 수신한 빛을 상기 이미징 카테터(110)로 전달하는 동시에 상기 이미징 카테터(110)를 회전시킬 수 있다. 또한, 상기 이미징 카테터(110)의 평형이동을 추가적으로 지원하기 위해, 회전접합부(120)는 상기 이미징 카테터(110)가 체결된 상태에서 풀백 스테이지(130)에 장착된다.
풀백 스테이지(130)는 평형이동이 가능한 구조로서, 이미징 카테터(110)와 체결된 회전접합부(120)를 풀백 스테이지(130) 상에 장착함으로써 상기 이미징 카테터(110)가 상기 제1 대상체(100)의 혈관 내에서 평형이동 할 수 있도록 지원한다.
도 2는 일실시예에 따른 관상동맥 혈관 고속 스캐닝 장치(200)를 도시하는 블록도이다.
관상동맥 혈관 고속 스캐닝 장치(200)는 광간섭단층촬영(OCT) 장치로부터 조사되는 빛을 이용하여 진단 대상인 제1 대상체의 관상동맥 혈관벽을 빠른 속도로 이미징 한다. 상기 관상동맥 혈관 고속 스캐닝 장치(200)는 이미징 카테터(210), 회전 접합부(220) 및 풀백 스테이지(230)를 포함할 수 있다. 다만, 상기 풀백 스테이지(230)는 선택적인(optional) 구성으로서, 일부 실시예에서는 상기 풀백 스테이지(230)가 생략될 수도 있다.
먼저, 이미징 카테터(210)는 상기 제1 대상체의 혈관 내에 삽입되어 회전하면서 혈관벽을 이미징 할 수 있다. 이미징 카테터(210)는 광간섭단층촬영 장치로부터 조사되는 빛을 회전접합부(220)를 통해 전달받아 상기 제1 대상체의 혈관벽에 조사하고, 상기 혈관벽의 조직에 의해 후방산란(backscattering) 되는 빛으로부터 간섭신호를 검출한다. 이를 위해, 상기 이미징 카테터(210)의 양쪽 끝 단 중 제1 측에는 상기 회전접합부(220)에 체결되는 광 커넥터가 부착되고, 다른 끝 단인 (상기 제1 측과는 상이한)제2 측에는 어느 한 면이 연마된 형태의 볼 렌즈가 융착(splicing) 될 수 있다.
보다 구체적으로, 상기 이미징 카테터(210)는 상기 회전접합부(220)와 연결된 광섬유를 이용하여 상기 제1 대상체의 혈관벽을 스캐닝 하는 이미징 프로브(imaging probe)와, 상기 회전접합부(220)의 한 측에 고정되어 상기 이미징 카테터의 회전 시 상기 이미징 프로브 및 상기 제1 대상체의 혈관을 보호하는 외피(sheath)로 구분될 수 있다. 이미징 카테터(210)가 상기 제1 대상체의 혈관벽을 이미징하는 과정에서, 상기 이미징 프로브는 상기 외피 내에 삽입되어 회전하며, 회전 시 회전중심축이 흔들리는 것을 방지하기 위해 상기 외피와 미리 지정된 유격을 유지한다. 또한, 상기 이미징 프로브는 광 커넥터, 토크코일, 광섬유 및 볼 렌즈를 포함한다. 상기 광 커넥터는 상기 광간섭단층촬영 장치로부터 조사되는 빛을 전달받기 위해 상기 회전접합부(220)의 어댑터와 체결되는 부분이다. 상기 토크코일은 이미징 카테터(210)의 회전을 위해 스프링 형태로 촘촘히 감겨있는 부분으로, 고속 회전에도 버틸 수 있도록 다중으로 제작될 수 있고, 외부에 테플론 코팅(teflon coating) 처리하여 회전 시 마찰력을 줄일 수 있다. 상기 광섬유는 상기 광 커넥터를 통해 상기 회전접합부(220)와 연결되고, 상기 토크코일 내에 삽입되어 상기 빛을 상기 제1 대상체의 혈관벽 방향으로 가이드한다. 이 때, 상기 광섬유는 이미징 카테터(210)가 혈관 내에서 회전하면서 받는 외력(이를 테면, 굴곡진 혈관 내에서 비틀림이나 구부러짐)을 버티기 위해 폴리미드(polymide) 재질로 코팅된 것일 수 있다. 그리고 상기 볼 렌즈는 상기 광섬유를 통해 전달받은 상기 빛을 상기 혈관벽에 조사하는데, 상기 빛이 전반사를 통해 상기 혈관벽으로 조사될 수 있도록 어느 한 면이 연마된 형태이며, 상기 볼 렌즈를 보호하기 위해 상기 토크코일의 끝 단에 하이포튜브(hypotube)가 부착될 수 있다. 상기 이미징 카테터(210)의 세부 구성에 대해서는 도 3a, 도 3b와 도 4a, 도 4b, 도 4c를 참조하여 상세히 설명한다.
회전접합부(220)는 이미징 카테터(210)의 한 측에 체결되어 상기 광간섭단층촬영 장치로부터 조사되는 빛을 상기 이미징 카테터(210)로 전달하고, 상기 이미징 카테터(210)를 회전시킬 수 있다. 상기 회전접합부(220)는 광섬유를 이용하여 상기 광간섭단층촬영 장치로부터의 빛을 상기 이미징 카테터(210)로 전달 하는 복수 개의 시준기와, 상기 복수 개의 시준기 중 적어도 하나를 고정시키는 고정부, 및 상기 복수 개의 시준기 중 적어도 하나를 회전시키는 회전부를 포함한다. 상기 복수 개의 시준기는 상기 광간섭단층촬영 장치와 광섬유를 통해 연결되어 빛을 전달받는 제1 시준기와, 상기 제1 시준기로부터 상기 빛을 전달받아 상기 이미징 카테터(210)로 전달하는 제2 시준기를 포함하며, 상기 제1 시준기는 상기 고정부에 결합되고 상기 제2 시준기는 상기 회전부에 결합되는 구조일 수 있다. 또한, 상기 제1 시준기 및 상기 제2 시준기는 서로 대칭하도록 배치될 수 있다. 상기 회전접합부(220)의 세부 구성에 대해서는 도 5 및 도 6을 참조하여 상세히 설명한다.
풀백 스테이지(230)는 회전접합부(220)의 적어도 한 측을 지지하도록 장착되며, 상기 이미징 카테터(210)의 평형이동을 지원할 수 있다. 이미징 카테터(210)와 체결된 회전접합부(220)가 풀백 스테이지(230)의 어느 한 측에 장착되는 경우, 상기 풀백 스테이지(230)가 평형이동 함으로써 상기 이미징 카테터(110)의 혈관 내 평형이동이 가능하다.
도 3a 및 도 3b는 일실시예에 따른 이미징 카테터의 이미징 프로브(300)의 세부구성을 설명하는 도면이다. 도 3a는 이미징 프로브(300)의 정면도이고, 도 3b는 이미징 프로브(300)의 한쪽 끝 단 부분을 확대한 도면이다.
이미징 카테터는 크게 도 3a, 도 3b의 이미징 프로브(imaging probe)와 도 4a, 도 4b, 도 4c의 외피(sheath)로 구분된다. 먼저, 이미징 프로브(300)는 회전접합부와 연결된 광섬유를 이용하여 상기 제1 대상체의 혈관벽을 스캐닝 하는 부분으로, 광 커넥터(310), 토크코일(320), 볼 렌즈(350)를 포함할 수 있다. 또한, 이미징 프로브(300)는 광간섭단층촬영(OCT) 장치로부터의 빛을 말단 부분에까지 전달하기 위해 충분히 긴 길이의 광섬유를 포함하며, 회전접합부와 체결되어 상기 광간섭단층촬영 장치로부터의 빛을 전달받아 제1 대상체의 혈관 내에서 회전 및 평형이동하면서 혈관벽 이미징을 수행한다. 상기 광섬유가 상기 회전접합부와 연결되기 위해, 이미징 프로브(300)의 양쪽 끝 단 중 제1 측에는 광 커넥터가 부착된다. 일반적으로 사용되는 광섬유는 아크릴 도료(acrylic)로 외부를 코팅된 형태이지만, 이는 회전 시 받게되는 비틀림이나 굴곡진 혈관 내에서의 구부러짐에 적합하지 않다. 따라서, 이러한 비틀림이나 구부러짐과 같은 외력에 잘 버티기 위해, 상기 이미징 프로브(300)는 폴리미드(polyimide) 재질로 코팅된 광섬유를 채택하여 이용할 수 있다. 폴리미드 재질에 의한 코팅의 경우, 아크릴 도료보다 더 얇게 코팅이 가능하면서도, 외력에 잘 버틸 수 있어 고속 회전을 하기에 보다 적합하기 때문이다.
도 3a에서, 광 커넥터(310)는 상기 광간섭단층촬영 장치로부터 조사되는 빛을 전달받기 위해 상기 회전접합부의 광섬유 시준기 및 어댑터와 체결되는 부분이다. 또한, 이미징 프로브(300)는 혈관 내 회전이 용이하도록 스프링 형태로 감겨있는 토크코일(320)을 포함한다. 상기 토크코일(320)은 이미징 프로브(300)가 상기 제1 대상체의 관상동맥까지 도달하는 과정에서 거쳐야 하는 굴곡진 혈관 내에서 용이하게 회전할 수 있도록 스프링 형태로 촘촘히 감겨있는 부분으로, 고속 회전에도 버틸 수 있도록 2겹 이상의 다중으로 제작될 수 있고, 외부에 테플론 코팅(teflon coating) 처리하여 회전 시 마찰력을 줄일 수 있다. 이미징 프로브(300)에 포함되는 상기 광섬유는 상기 토크코일(320) 내에 삽입되어 고정된 상태에서 상기 혈관벽 이미징에 이용된다.
한편, 이미징 프로브(300)의 양쪽 끝 단 중 광 커넥터(310)가 부착되지 않은 제2측에는 어느 한 면이 연마된 형태의 볼 렌즈(350)가 융착되며, 이는 도 3b와 같이 도시된다. 상기 토크코일(320)에 삽입된 광섬유는 상기 회전접합부로부터 전달받은 빛을 볼 렌즈(350)로 가이드 한다. 혈관벽을 이미징 하기 위해서는 상기 혈관벽 방향으로 빛이 조사되어야 한다. 이를 위해, 어느 한 면이 연마된 형태의 볼 렌즈(350)를 상기 토크코일(320)의 말단에 융착시키며, 상기 광섬유를 통해 가이딩 되어 온 빛이 방사선 불투과성 링(340)을 통과하여 상기 볼 렌즈(350)의 연마된 면에서 전반사된 후 상기 혈관벽 방향으로 조사되도록 한다. 또한, 상기 볼 렌즈(350)를 보호하기 위해, 금속 재질의 윈도우(window)가 있는 하이포튜브(hypotube)가 상기 토크코일의 끝 단에 부착될 수 있다.
도 4a, 도 4b 및 도 4c는 일실시예에 따른 이미징 카테터의 외피(400)의 세부구성을 설명하는 도면이다. 도 4a는 이미징 프로브(300)가 삽입되는 외피(400)의 정면도이고, 도 4b는 외피(400)의 한쪽 끝 단 부분을 확대한 도면이다. 또한, 도 4c는 이미징 카테터와 회전접합부의 체결되는 원리를 나타낸다.
도 3a의 이미징 프로브(300)는 혈관벽 스캐닝을 위해 혈관 내에서 회전해야 하는데, 회전 시 혈관에 영향을 주지 않도록 폴리머 재질의 외피(400)에 삽입된 상태에서 상기 혈관에 삽입된다
외피(400)는 회전접합부의 어느 한 측에 고정되어, 상기 이미징 카테터의 회전 시 상기 이미징 프로브(300) 및 상기 제1 대상체의 혈관을 보호한다. 이로 인해, 상기 이미징 카테터가 상기 제1 대상체의 혈관 내에 삽입되어 회전하더라도, 외피(400)는 고정된 상태에서 상기 이미징 프로브(300)만 상기 외피(400) 내에서 회전하는 구조일 수 있다. 이 때, 이미징 프로브(300)의 토크코일(320)과 외피(400) 사이의 유격이 크면 상기 이미징 프로브(300)의 회전 시 회전 중심축이 흔들리면서 하나의 중심축으로 회전하지 못할 것이고, 반대로 상기 유격이 작으면 회전 시 마찰로 인해 고속 회전이 충분히 이루어지지 못하는 문제점이 발생할 수 있다. 이를 방지하기 위해, 이미징 프로브(300)와 외피(400) 사이의 유격이 적절한 수준에서 유지되어야 하며, 현재 최적설계된 둘 사이 간격은 약 60-70 um정도이다. 또한, 이미징 프로브(300)의 볼 렌즈(350) 부분에서 혈관벽 쪽으로 빛이 조사되어야 하므로, 상기 이미징 프로브의 말단 부분에 대응하는 외피 부분은 투명한 재질로 제작된다.
외피(400)는 캡(cap, 410)이라는 부품에 의해 회전 접합부에 고정되며, 이미징 프로브(300)가 풀백 스테이지 움직임에 의해 평형이동하더라도 외피(400)는 고정되도록 하기 위해 풀백부(pullback part)가 추가될 수 있다. 상기 풀백부를 통해, 상기 이미징 프로브(300)가 풀백하는 경우에도 외피(400)는 고정된 상태에서 상기 이미징 프로브(300)만 풀백된다. 상기 외피(400)의 풀백부 및 캡(410)은 도 4b에서 확인할 수 있다. 상기 풀백부는 외피(400) 끝단에 위치하며, 내관(inner tube)과 외관(outer tube)으로 구성될 수 있다. 상기 내관은 상기 외관 내에 존재하고, 상기 내관에는 캡(410)이 고정되어 있다. 또한, 상기 외관에는 풀백 고정부(pullback holder)가 장착되어, 이미징 카테터가 평형이동 하더라도 외피(400)는 고정된 상태에서 상기 내관만 움직이며, 상기 이미징 프로브(300)와 내관, 캡(410)이 함께 회전접합부에 체결되어 움직이는 구조로 이해될 수 있다.
한편, 이미징 카테터의 광 커넥터(310) 부분은 회전접합부와 연결되기 위해서 외피(400)의 외부로 노출된 상태에서 회전하게 되는데, 이 때 노출 길이가 고속 회전에 큰 영향을 주게 된다. 이를 테면, 상기 노출 길이가 너무 길면 외피(400) 내에서 이미징 카테터가 회전하지 못하고 외부로 빠져나오게 되며, 반대로 상기 노출 길이가 너무 짧으면 외피(400) 내 마찰이 커져 회전이 용이하게 이루어지지 못하고 이미징 카테터 안에서 기계적 진동(beating)을 발생시킬 수 있다. 이를 방지하기 위해서는, 적절한 노출 길이 설정이 필요하지만, 이는 이미징 카테터마다의 특성에 따라 달라질 수 있으므로, 이미지 카테터에 따라 상기 노출 길이의 조절이 가능하도록 도 4c와 같은 수단이 필요하다. 도 4c를 참조하면, 캡(410) 내의 외피 길이를 가변할 수 있도록 캡(410)과 내관의 고정부를 고정 나사(set screw)로 고정시킨다. 또한, 캡(410)이 캡 어댑터(cap adaptor)에 체결될 때 상기 캡(410)과 이미징 프로브(300)가 동시에 체결될 수 있도록 커넥터 홀더(connector holder, 420)가 상기 캡(410) 내에 위치하며, 상기 캡(410)과 상기 캡 커넥터 홀더(420)는 스레드(thread, 421) 형태로 연결된다. 상기 캡 어댑터는 스프링(431)을 포함하고 있는데, 상기 캡(410)이 상기 내관과 체결될 때 스프링(431)을 압축시켜 상기 캡(410)이 장착될 때 이미징 프로브 커넥터가 회전 접합부의 광 어댑터에 체결된다. 또한, 체결된 것을 해제하는 경우에는, 상기 캡(410)이 약간 뒤로 빠지면서 상기 이미징 프로브 커넥터가 커넥터 홀더(420)로부터 빠져나오도록 하여, 회전 시 캡(410)의 영향을 받지 않고 이미징 프로브(300)만 회전될 수 있다. 이미징 카테터를 해제할 때에는 상기 캡 어댑터의 레버(lever)를 누르면 상기 캡(410)이 스프링(431)의 복원력에 의하여 튀어나오도록 구현된다.
도 5는 일실시예에 따른 회전접합부(500)의 구조를 설명하는 도면이다.
회전접합부(500)는 광간섭단층촬영(OCT) 장치로 조사되는 빛을 전달받아 이미징 카테터로 전달하는 동시에, 상기 이미징 카테터를 회전시키는 역할을 수행한다. 회전 접합부(500)는 두 개의 광섬유 시준기(fiber collimator, 510 및 520)를 기반으로 구성될 수 있다. 상기 광섬유 시준기(510, 520)에 대해서는 도 6을 참조하여 상세히 설명한다.
도 6은 일실시예에 따른 회전 접합부(500)에 포함되는 광섬유 시준기(510, 520)의 세부구성을 설명하는 도면이다.
상기 광섬유 시준기는 광간섭단층촬영 장치와 광섬유를 통해 연결되어 빛을 전달받는 제1 시준기(610)와, 상기 제1 시준기(610)로부터 상기 빛을 전달받아 이미징 카테터로 전달하는 제2 시준기(620)로 구분될 수 있다. 상기 제1 시준기(610)가 광간섭단층촬영 장치와 연결된 광섬유를 통해 빛을 전달받으면 상기 빛이 자유 공간(free space)으로 전달되고, 이는 다시 맞은 편의 제2 시준기(620)로 전달되는 방식이다. 이 때, 상기 제1 시준기는 회전 접합부(500)의 고정부(530)에 결합되고, 상기 제2 시준기는 회전 접합부(500)의 회전부(540)에 결합되는 구조이며, 상기 제1 시준기 및 상기 제2 시준기는 상기 자유 공간을 사이에 두고 서로 대칭하도록 배치될 수 있다. 또한, 상기 제2 시준기(620)는 제1 시준기(610)에 비해 짧은 길이의 광섬유로 연결되어, 커넥터 및 광 어댑터(optical adaptor)를 통해 이미징 카테터와 체결될 수 있다.
다시 도 5를 참조하면, 회전 접합부(500)에 포함되는 두 개의 광섬유 시준기 중 제1 시준기(510)는 고정부(stationary part, 530)라는 금속성 구조물에 고정되고, 제2 시준기(520) 및 광 어댑터는 회전부(rotary part, 540)라는 금속성 구조물에 고정된다. 또한, 상기 고정부(530) 및 상기 회전부(540) 사이에는 베어링(bearing)이 위치하고 있어, 상기 회전부(540)가 상기 고정부(530) 내에 위치 고정된 상태에서 회전이 가능한 구조이다. 도 5의 회전접합부(500)에서는, 두 개의 시준기(510, 520)를 이용하여 광간섭단층촬영 장치로부터의 빛을 이미지 카테터로 전달하기 때문에 회전부(540) 부분의 짧은 길이의 광섬유 시준기(520)가 회전하더라도 상기 빛을 온전히 전달받을 수 있다. 또한, 상기 회전부(540) 부분은 고속 회전에 유리하도록 경량 재질인 티타늄을 이용하여 소형으로 제작될 수 있다.
한편, 상기 회전부(540)의 짧은 길이 광섬유 시준기(520)와 상기 고정부(530)의 광섬유 시준기(510)는 각 금속부에 에폭시를 이용하여 고정되어야 하며, 두 개의 시준기가 정밀하게 축이 맞도록 정렬되어 있어야 한쪽 시준기가 회전하면서 광 투과율(optical transmission)이 큰 편차 없이 유지되면서 빛을 전달받을 수 있다. 이를 위해, 시준기들을 잘 정렬하여 각 금속부에 본딩하는 과정이 필요하고, 이 과정을 위한 정렬 장치를 회전 접합부 정렬 장치(rotary junction alignment device)라고 한다. 이러한 회전 접합부의 정렬 과정 및 정렬 장치에 대해서는 아래 도 8a 및 도 8b에서 설명한다.
도 7은 일실시예에 따라 회전 접합부(700)가 하우징(710)에 설치된 구조를 나타내는 도면이다.
회전 접합부(700)의 회전 동력은 브러시리스 직류 모터(Brush-less Direct Current motor, BLDC)로부터 타이밍 밸트를 통해 전달받을 수 있다. 동력 손실을 고려하면 기어를 이용하는 것이 유리하지만, 고속 회전을 위해서는 타이밍 밸트를 이용하는 것이 유리하기 때문이다. 따라서 타이밍밸트를 이용한 동력 전달 방식을 채택하였다. 도 7과 같이, 회전 접합부(700)는 상기 모터(720)와 함께 회전 접합부 하우징(710)에 고정되는 구조이며, 상기 하우징(710)에는 이미징 카테터의 캡(도 4c 참조)을 고정시킬 수 있는 캡 어댑터 부분이 체결된다.
도 8a 및 도 8b는 일실시예에 따라 회전 접합부를 고정하는 정렬장치의 세부 구조를 설명하는 도면이다.
회전 접합부의 회전부의 기계적 파트와 회전 광섬유 시준기를 에폭시로 고정하기 전에, 상기 회전 광섬유 시준기의 회전에 따른 광 전달률의 편차가 크지 않고 일정 수준으로 유지하기 위해 광섬유 시준기를 정렬할 수 있는 장치가 필요하다.
도 8a에서 볼 수 있듯이, 회전접합부의 정렬 장치는 크게 평형 이동 스테이지(translational stage, 810), 경사 스테이지(tilt stage, 820), 회전부 홀더(Rotor part holder, 830) 및 시준기 홀더(Collimator holder, 840)로 구분될 수 있다. 상기 정렬장치는 상기 회전부 홀더(830)가 회전접합부의 회전부를 고정시키는 구조로서, 상기 회전접합부의 회전부가 회전할 때 함께 회전한다. 상기 회전부의 광섬유 시준기에 광원을 연결하면 빛이 시준기로부터 방출되는데, 이 때 상기 회전부를 회전하면서 빔 프로파일러(beam profiler) 등을 이용하여 빔의 움직임을 살핀다. 빔 정렬이 제대로 이루어진 경우에는 회전에 따라 거의 움직임이 없으나, 빔 정렬이 달 수행되지 않은 경우에는 빔 정렬을 위해 정렬 장치의 스테이지들을 이용하여 시준기의 위치 및 각도를 조절할 수 있다. 이를 위해, 상기 정렬 장치를 회전부에 고정시키기에 앞서, 상기 회전부와 광섬유 시준기 사이를 작업 시간이 긴 에폭시로 채우고 정렬 과정을 진행한다. 이 때, 상기 회전부의 광학적 성능을 정량적으로 측정하기 위해, 정렬된 회전부의 광섬유 시준기로부터 방출된 빛을 반대쪽의 다른 시준기를 통과하도록 하여 회전에 따른 광 투과율이 일정 기준 이상이 되는지 확인할 수 있다. 상기 회전부와 광섬유 시준기 사이를 채워넣은 에폭시가 굳어진 상태에서, 상기 정렬 장치를 상기 회전부로부터 분리시키면 회전부 정렬과정이 완료된다.
상기 회전부 정렬과정이 완료된 이후에는, 회전접합부의 고정부 부분의 광섬유 시준기를 정렬해야 한다. 상기 고정부의 기계적 부품은 뚜껑 형태로 되어 있어 나사 체결로 상기 고정부 및 상기 회전부가 서로 결합될 수 있다. 상기 고정부의 기계적 부품에는 고정부 광섬유 시준기가 고정되며, 상기 회전부로부터 전달되는 빛을 받을 수 있는 구멍이 있다. 6축 정밀 스테이지에 고정되어 있는 고정부의 광섬유 시준기를 상기 구멍 안으로 삽입한 후, 회전부의 광섬유 시준기로부터 오는 빛을 전달받아 광 투과율을 측정하며, 광 투과율이 최대이면서 상기 회전부의 회전에 따른 편차가 최소가 되도록 정밀 스테이지를 조절하는 방식으로, 상기 고정부의 광섬유 시준기를 정렬한다. 이 때, (회전부 정렬 시와 마찬가지로) 시준기 정렬 과정에 앞서 시준기와 고정부 기계적 부품 사이에 에폭시로 채워넣은 상태에서 정렬을 진행하며, 에폭시가 완전히 굳으면 상기 고정부의 광섬유 시준기를 정밀 스테이지에서 분리하여 도 8b와 같이 고정부 정렬과정을 마무리한다.
상술한 바와 같이, 정렬 과정에 앞서 에폭시를 이용함으로써, 정렬 과정 중 광 투과율에 이상이 있으면 에폭시를 제거하고 새로운 시준기로 교체하여 다시 정렬 과정을 진행할 수 있다. 또한, 두 개의 시준기 정렬 과정 중, 고정부의 시준기에만 문제가 있다면 상기 고정부의 시준기를 고정하는 뚜껑 형태의 기계적 파트에서 나사를 풀어 다시 제작 가능하고, 회전부에 대해서는 처음부터 다시 제작할 필요가 없으므로, 제작이 용이하다는 장점이 있다.
도 9는 일실시예에 따른 관상동맥 혈관 고속 스캐닝 방법을 도시하는 흐름도이다.
관상동맥 혈관 고속 스캐닝 장치는 광간섭단층촬영(OCT) 장치로부터 조사되는 빛을 이용하여 진단 대상인 제1 대상체의 관상동맥 혈관벽을 빠른 속도로 이미징 하는 방법을 제공한다.
단계 910에서는, 상기 광간섭단층촬영 장치로부터 조사된 빛이 상기 고속 스캐닝 장치의 이미징 카테터로 전달될 수 있다. 단계 910에서, 상기 고속 스캐닝 장치의 회전접합부는 상기 광간섭단층촬영 장치로부터의 빛을 상기 이미징 카테터로 전달하는데, 이를 위해 상기 회전접합부에 포함되는 두 개의 시준기가 이용될 수 있다. 상기 회전접합부의 고정부에 결합되는 제1 시준기는 광섬유를 통해 연결된 상기 광간섭단층촬영 장치로부터 빛을 전달받고, 상기 회전접합부의 회전부에 결합되는 제2 시준기는 상기 제1 시준기로부터 상기 빛을 전달받아 상기 이미징 카테터(210)로 전달한다. 이 때, 상기 제1 시준기 및 상기 제2 시준기는 서로 대칭하도록 배치될 수 있다.
단계 920에서는, 상기 이미징 카테터가 상기 제1 대상체의 혈관 내에 삽입되어 평형이동 및 회전이동 하면서, 단계 910에서 전달받은 빛을 상기 혈관벽에 조사할 수 있다. 여기서, 상기 이미징 카테터는 상기 회전접합부의 적어도 한 측과 체결되어 회전이동 하고, 상기 회전접합부의 적어도 한 측을 지지하도록 장착된 풀백 스테이지에 의해 평형이동 할 수 있다.
단계 930에서는, 상기 이미징 카테터가 상기 혈관벽의 조직에 의해 후방산란(backscattering) 된 빛으로부터 간섭신호를 검출할 수 있다. 상기 이미징 카테터는 이미징 프로브(imaging probe) 및 외피(sheath)로 구분될 수 있다. 단계 930에서, 상기 이미징 프로브는 상기 외피 내에 삽입되어 회전하면서 상기 회전접합부와 연결된 광섬유를 이용하여 상기 제1 대상체의 혈관벽을 스캐닝하고, 상기 외피는 상기 회전접합부의 한 측에 고정되어 상기 이미징 카테터의 회전 시 상기 이미징 프로브 및 상기 제1 대상체의 혈관을 보호한다. 이 때, 상기 이미징 프로브는 상기 외피 내에 삽입되어 회전하며, 회전 시 회전중심축이 흔들리는 것을 방지하기 위해 상기 외피와 미리 지정된 유격을 유지한다. 상기 이미징 카테터는 혈관벽 스캐닝을 위해, 양쪽 끝 단 중 제1 측에는 상기 회전접합부에 체결되는 광 커넥터가 부착되고, 다른 끝 단인 (상기 제1 측과는 상이한)제2 측에는 어느 한 면이 연마된 형태의 볼 렌즈가 융착(splicing) 될 수 있다.
이상과 같이 실시예들이 비록 한정된 실시예와 도면에 의해 설명되었으나, 해당 기술분야에서 통상의 지식을 가진 자라면 상기의 기재로부터 다양한 수정 및 변형이 가능하다. 예를 들어, 설명된 기술들이 설명된 방법과 다른 순서로 수행되거나, 및/또는 설명된 시스템, 구조, 장치, 회로 등의 구성요소들이 설명된 방법과 다른 형태로 결합 또는 조합되거나, 다른 구성요소 또는 균등물에 의하여 대치되거나 치환되더라도 적절한 결과가 달성될 수 있다.
그러므로, 다른 구현들, 다른 실시예들 및 특허청구범위와 균등한 것들도 후술하는 특허청구범위의 범위에 속한다.

Claims (14)

  1. 제1 대상체의 혈관 내에 삽입되어 회전하면서 혈관벽을 이미징하는 이미징 카테터; 및
    상기 이미징 카테터의 한 측에 체결되어, 광간섭단층촬영(OCT) 장치로부터 조사된 빛을 상기 이미징 카테터로 전달하고 상기 이미징 카테터를 회전시키는 회전접합부
    를 포함하는 관상동맥 혈관 고속 스캐닝 장치.
  2. 제1항에 있어서,
    상기 이미징 카테터는,
    상기 회전접합부로부터 전달받은 빛을 상기 혈관벽에 조사하고, 상기 혈관벽의 조직으로부터 후방산란(backscattering) 된 빛으로부터 간섭신호를 검출하는 관상동맥 혈관 고속 스캐닝 장치.
  3. 제1항에 있어서,
    상기 회전접합부의 적어도 한 측을 지지하도록 장착되어, 상기 이미징 카테터의 평형이동을 지원하는 풀백 스테이지
    를 더 포함하는 관상동맥 혈관 고속 스캐닝 장치.
  4. 제1항에 있어서,
    상기 이미징 카테터의 양쪽 끝 단 중 제1 측에는 상기 회전접합부에 체결되는 광 커넥터가 부착되고, 상기 제1 측과 상이한 제2 측에는 어느 한 면이 연마된 볼 모양의 렌즈가 융착(splicing) 되는 관상동맥 혈관 고속 스캐닝 장치.
  5. 제4항에 있어서,
    상기 이미징 카테터는,
    상기 회전접합부와 연결된 광섬유를 이용하여 상기 제1 대상체의 혈관벽을 스캐닝 하는 이미징 프로브; 및
    상기 회전접합부의 한 측에 고정되어, 상기 이미징 카테터의 회전 시 상기 이미징 프로브 및 상기 제1 대상체의 혈관을 보호하는 외피
    를 포함하며,
    상기 이미징 프로브는 상기 외피 내에 삽입되어 회전하고, 상기 외피와 미리 지정된 유격을 유지하는 관상동맥 혈관 고속 스캐닝 장치.
  6. 제5항에 있어서,
    상기 이미징 프로브는,
    상기 회전접합부의 어댑터와 체결되어 상기 광간섭단층촬영(OCT) 장치로부터 조사되는 빛을 전달받는 광 커넥터;
    스프링 형태로 감겨있는 토크코일;
    상기 광 커넥터를 통해 상기 회전접합부와 연결되고, 상기 토크코일 내에 삽입되어 상기 빛을 상기 제1 대상체의 혈관벽 방향으로 가이드하는 광섬유; 및
    상기 광섬유를 통해 전달받은 상기 빛을 혈관벽에 조사하는 볼 렌즈
    를 포함하는 관상동맥 혈관 고속 스캐닝 장치.
  7. 제6항에 있어서,
    상기 광섬유는, 폴리미드(polyimide) 재질로 코팅된 것인 관상동맥 혈관 고속 스캐닝 장치.
  8. 제1항에 있어서,
    상기 회전접합부는,
    광섬유를 이용하여 상기 광간섭단층촬영(OCT) 장치로부터의 빛을 상기 이미징 카테터로 전달하는 복수 개의 시준기;
    상기 복수 개의 시준기 중 적어도 하나를 고정시키는 고정부; 및
    상기 복수 개의 시준기 중 적어도 하나를 회전시키는 회전부
    를 포함하는 관상동맥 혈관 고속 스캐닝 장치.
  9. 제8항에 있어서,
    상기 복수 개의 시준기는,
    상기 광간섭단층촬영(OCT) 장치와 광섬유를 통해 연결되어 빛을 전달받는 제1 시준기; 및
    상기 제1 시준기로부터 상기 빛을 전달받아 상기 이미징 카테터로 전달하는 제2 시준기
    를 포함하며,
    상기 제1 시준기는 상기 고정부에 결합되고, 상기 제2 시준기는 상기 회전부에 결합되며, 상기 제1 시준기 및 상기 제2 시준기는 서로 대칭하도록 배치되는 관상동맥 혈관 고속 스캐닝 장치.
  10. 관상동맥 혈관 고속 스캐닝 장치가 제1 대상체의 혈관벽을 스캔하는 방법에 있어서,
    상기 고속 스캐닝 장치의 회전접합부가 광간섭단층촬영(OCT) 장치로부터 조사된 빛을 상기 고속 스캐닝 장치의 이미징 카테터로 전달하는 단계;
    상기 이미징 카테터가 상기 제1 대상체의 혈관 내에 삽입되어 평형이동 및 회전이동 하면서 상기 전달받은 빛을 상기 혈관벽에 조사하는 단계; 및
    상기 이미징 카테터가 상기 혈관벽의 조직으로부터 후방산란(backscattering) 된 빛으로부터 간섭신호를 검출하는 단계
    를 포함하는 관상동맥 혈관 고속 스캐닝 방법.
  11. 제10항에 있어서,
    상기 조사된 빛을 상기 고속 스캐닝 장치의 이미징 카테터로 전달하는 단계는,
    상기 회전접합부의 고정부에 결합된 제1 시준기가, 광섬유를 통해 연결된 광간섭단층촬영(OCT) 장치로부터 빛을 전달받는 단계; 및
    상기 회전접합부의 회전부에 결합된 제2 시준기가, 상기 제1 시준기로부터 전달받은 빛을 상기 이미징 카테터로 전달하는 단계
    를 포함하며, 상기 제1 시준기 및 상기 제2 시준기는 서로 대칭하도록 배치되는 관상동맥 혈관 고속 스캐닝 방법.
  12. 제10항에 있어서,
    상기 이미징 카테터는,
    상기 회전접합부의 적어도 한 측과 체결되어 회전이동 하고, 상기 회전접합부의 적어도 한 측을 지지하도록 장착된 풀백 스테이지에 의해 평형이동 하는 관상동맥 혈관 고속 스캐닝 방법.
  13. 제10항에 있어서,
    상기 간섭신호를 검출하는 단계는,
    이미징 프로브가 외피 내에 삽입되어 회전하면서, 상기 회전접합부와 연결된 광섬유를 이용하여 상기 제1 대상체의 혈관벽을 스캐닝 하는 단계
    를 더 포함하는 관상동맥 혈관 고속 스캐닝 방법.
  14. 제10항에 있어서,
    상기 이미징 카테터의 양쪽 끝 단 중 제1 측에는 상기 회전접합부에 체결되는 광 커넥터가 부착되고, 상기 제1 측과 상이한 제2 측에는 어느 한 면이 연마된 볼 모양의 렌즈가 융착(splicing) 되는 관상동맥 혈관 고속 스캐닝 방법.
