このような難治性の関節リウマチの治療においては、早期発見及び早期治療により、病勢を少なくとも低疾患活動性以下に抑え込むことで、関節破壊の進展を抑制することが重要であるとされる(非特許文献5)。
しかしながら関節リウマチの臨床所見は個人差が大きく、いまだ特異的な検査もなく、他の関節炎との鑑別も難しいため、早期診断には専門的な知識が必要とされ、その治療効果の判定もまた同様で、継続的な専門医の診察が必要と考えられている(非特許文献6)。
関節リウマチ疾患活動性を定量化して評価するために、従来からいろいろな評価方法が編み出されてきたが、正確に定量化することは難しい。その中で、関節リウマチの疾患活動性を正確に定量的に評価する手法として開発されたDAS(Disease Activity Score)は、圧痛関節数と腫脹関節数を調べ、かつ患者の体調の自己評価も加味し、特別な計算式に当てはめて判定する方法であり、臨床研究における疾患活動性の評価法として主流となっている。
しかし、28関節についての熟練した医師の診察が欠かせない上に、平方根、対数を用いる複雑な計算が必要なため、日常の診療では時間がかかり、また足関節や足指関節の症状が反映されないなどの問題点が指摘されていた(非特許文献7)。「DAS28」で「寛解」と評価されても関節破壊が進行するケースもあるため、臨床的な寛解基準としては別途Booleanの基準やSDAIなどの基準に当てはめる必要があるとされている(非特許文献8)。

例えば、第1マーカー(SLC16A4)遺伝子発現量情報、及び第2マーカー(SLCO2B1、PTPRM、SHB又はATP9A)遺伝子発現量情報に基づく関節リウマチの存在を評価する方法(特許文献19)が提案されている。
最近、米国Crescendo社が、血清中のIL-6、EGF、VEGFA、LEP、SAA1、VCAM1、CRP、MMP-1、MMP-3、TNFRSF1A、RETN、及びCHI3L1からなる12個のバイオマーカーを測定し、特別な計算式でスコア化した疾患活動性指標(DAI)により、関節リウマチの診断、モニタリングをする手法(特許文献20、非特許文献11)を開発した。当該手法は、「DAS28」との相関性も高く、熟練した医師による詳細な診断を必要とせずに、より正確な関節リウマチの診断を可能とするものであるといえる。しかしながら、12個ものバイオマーカーの測定には多額の資金が必要であり、複雑な計算式による計算も必要となることから、臨床の現場においてはあまり実用的な手法とはいえない。
このような背景から、複雑な計算式もいらず、できるだけ少ないバイオマーカーによる簡便な測定により、正確に関節リウマチの疾患活動性の評価が可能な新規関節リウマチマーカーの提供が強く望まれていた。
一方、リウマチ性疾患のうち、再発性多発軟骨炎(RP)は極めてまれな疾患であり、その症状が特徴的ではあるが、症状が出たり消えたりする活動期と非活動期があることが知られている。また、特異的な血液検査法として抗コラーゲン抗体を用いる方法があるが、陽性率もそれほど高くなく、関節リウマチでも陽性になってしまうという問題もある(非特許文献13)。そのため、活動期、非活動期ににかかわらず正確に測定できる優れたマーカー及び検査方法の提供が切望されていた。
本発明者らは、以前から種々の疾病に罹患すると、病態に応じて血清中の特定の糖タンパク質の糖鎖組成に変化をもたらす場合があることを経験していたため、関節リウマチの疾患活動性に応じて血清中のMMP-3の糖鎖組成に変化が現れる可能性に思い至った。しかし、血清中のMMP-3タンパク質は、関節リウマチの罹患により増加するといってもたかだか100ng/mlから1000μg/ml程度と極めて微量であり、血中糖タンパク質総量に対する存在比は100~1,000,000分の1以下と極めて少ない。したがって、MMP-3の上に存在する糖鎖の糖鎖変化を、血清に含まれるすべての糖タンパク質の糖鎖から特異的に検出することは、S/N比が低くなり、感度が低いため正確な測定が難しい。そこで、本発明者らは、抗MMP-3抗体を用いた抗体オーバーレイ・レクチンアレイ法を利用して、血清中の微量のMMP-3上の糖鎖構造情報を得る方法を構築した。当該手法を市販のリコンビナントMMP-3タンパク質に対して適用すると、100pgから1ng程度のMMP-3タンパク質量であっても最低限の糖鎖構造情報を得ることが可能であった。これは健常者の血中に含まれる微量のMMP-3(40ng/ml程度)においても鋭敏な感度での分析が可能であり、病状に応じた糖鎖変化を検討できることを示すものである。
その結果、関節リウマチ患者血清中のMMP-3においては、レクチンのACG、Jacalin、ABA、ACAで有意に高いシグナルを観察した。ここで、ACG以外のJacalin、ABA、ACAレクチンは明らかにO-結合型糖鎖(O-glycan)認識レクチンであることから、この結果は、関節リウマチの罹患により血清中のMMP-3上の糖鎖として、O-結合型糖鎖、少なくともJacalin、ABA及びACAレクチンのいずれかが認識するO-結合型糖鎖が存在していることを意味する。従来、血清中の糖タンパク質の糖鎖組成変化としては、主にN-結合型糖鎖が注目されており(特許文献18など)、MMP-3上の糖鎖に至っては、O-結合型糖鎖の存在すら報告がなかったことから、関節リウマチ特異的な糖鎖エピトープの糖鎖構造としては新規な糖鎖構造であると考えられる。
これらの結果は、JacalinとABA、ACAそれぞれの間の糖鎖認識の違いが反映されている可能性がある。本発明者らは、これまでの知見からO-結合型糖鎖のペプチドに結合しているN-アセチルガラクトサミン(GalNAc)の6位への糖鎖修飾の違いが結果に反映されていると考えた。特にJacalinは、O-結合型糖鎖のGalNAcの6位への修飾を許容できず、それにより反応性を消失することが判っている(非特許文献14)。この位置への代表的な修飾としてはシアル酸修飾がある。そこで、前述の患者血清のうちいくつかの血清試料を用いて、シアル酸分解酵素であるシアリアーゼ処理を施した。シアリアーゼ処理前後のサンプルを対象にレクチンアレイ解析を行った結果、処理後のサンプルではJacalinのシグナルもABA、ACAと同様にMMP-3の量に相関を示した。
以上の結果から、MMP-3はリウマチを罹患すると血中濃度が上昇するが、その上に存在するO-結合型糖鎖のペプチドに直接結合しているGalNAcへのシアル酸修飾は、患者によって異なることが明らかになった。しかも、関節リウマチの疾患活動性が悪化している患者群において、Jacalinシグナルが弱くなるという顕著な傾向が見られることから、疾患活動性の悪化と当該シアル酸修飾に逆相関がある可能性が高まった。
このことは、レクチンアレイやレクチン―抗体サンドイッチELISA法に代表されるようなレクチンを用いての特定タンパク質上の糖鎖解析手法により、血清中のMMP-3のJacalinへの反応性と共にABA、ACAもしくはACGへの反応性を測定して両者の比を求めるだけで、熟練した医師の診断や複雑な計算式による煩雑な計算をすることなく関節リウマチの疾患活動性を客観的に把握することができ、DAS28と同程度の関節リウマチの診断精度が提供できることを意味する。
また、Jacalinが認識する糖鎖はO-結合型糖鎖であり、ペプチド鎖のSer/Thrに結合しているO-結合型糖鎖のコア構造-1(Core1:Gal(β1,3)GalNAc)ないしTn抗原(GalNAc)である。そして、それらに存在するGalNAcの6位にシアル酸が導入された場合、Jacalinはその修飾により反応性が阻害される。一方、ABA及びACAもJacalinと同じO-結合型糖鎖認識レクチンであるが、当該位置のシアル酸修飾は、それを認識するか、又は許容できる(Kuno A et al Molecular& Cellular Proteomics 8 (1) 99-108 (2009))。すなわち、上記結果は、関節リウマチに罹患することで、血清中のMMP-3が有するO-結合型糖鎖のコア構造-1でのGalNAcの6位に対するシアル酸修飾が増加していくことを強く示唆する結果である。
この結果は、関節リウマチに罹患すると、その病態の悪化に伴って、滑膜組織内での滑膜細胞で産生されるMMP-3上のJacalin結合性糖鎖エピトープが減少すること、すなわち、同MMP-3上のO-結合型糖鎖のコア構造-1でのGalNAcの6位に対するシアル酸修飾が増加していくことを実証するものである。そして同時に、関節リウマチ罹患患者血清試料中で観察された、MMP-3上のJacalin結合性O-結合型糖鎖エピトープの減少という現象が、滑膜組織由来MMP-3上での糖鎖構造変化を正確に反映していることに他ならない。このことは、被験者にとってより侵襲性の低い血清試料で、正確な関節リウマチ診断が可能であることが実証されたことでもある。
以上の知見を得たことで、本発明を完成するに至った。
本発明の関節リウマチの疾患活動性評価法は、関節リウマチを、適用性、感度、精度の点で、臨床上許容される性能で検出及び判別することを可能とする。しかも、簡便な手法により、医療現場で適用可能な方法で、関節リウマチの疾患活動性を評価することを可能とする。
当該キットにおいてはJacalin及びABA、ACA、ACGから選択されたいずれかのレクチンを含む複数のレクチンを支持体上に固相化した、レクチンアレイが好ましく用いられるが、各レクチンをビオチン化レクチンとし、ストレプトアビジンコートした支持体上に固相化した形態で調製することもできる。
その結果、いずれのレクチンに対しても再発性多発軟骨炎(RP)患者由来試料の反応性が極めて高く、次いでリウマチ性多発筋痛症(PMR)患者由来試料が高く、関節リウマチ(RA)患者及び健常者(HC)由来試料は、ほとんど反応性が無いという有意な結果が得られた。
このことは、MMP-3上のLEL又はSTLレクチン反応性糖鎖が、リウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)の罹患判定用の優れた糖鎖マーカーとなることを示すものである。
また、LELレクチンシグナルとJacalin又はACGシグナルとの比をとることで、リウマチ性多発筋痛症(PMR)、再発性多発軟骨炎(RP)患者及び健常人(HC)由来血清と比較して、関節リウマチ(RA)患者由来血清のみが高い数値を示すことを確認した。
また、被検血清試料中のMMP-3上のLEL又はSTLレクチン反応性糖鎖との反応性シグナルと、Jacalin、ACGなどMMP-3上の前記O-結合型糖鎖との反応性シグナルとの比を測定することで、類似したリウマチ性疾患と鑑別して関節リウマチ(RA)に罹患していることを、正確に診断することができる。
以上の知見を得たことで、本発明の他の課題も解決することができた。
〔1〕被検体液試料におけるMMP-3上のO-結合型糖鎖の糖鎖構造の変化を測定する工程を含む、関節リウマチの疾患活動性の判定方法。
本発明は、以下の様に表現することもできる。

〔2〕前記糖鎖構造の変化の測定が、被検体液試料中のMMP-3全量を示す測定シグナルに、MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナルの逆数を乗じた、もしくは前記糖鎖構造の変化の測定が、被検体液試料中のMMP-3全量を示す測定シグナルの逆数に、MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナルを乗じた、下記の糖鎖修飾判別指数X又はYを算出することで行われる、前記〔1〕に記載の判定方法;
X=被検体液試料中のMMP-3全量を示す測定シグナル/同MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナル、
Y=被検体液試料中のMMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナル/同MMP-3全量を示す測定シグナル。
〔3〕糖鎖修飾判別指数Xの値が高いほど、関節リウマチの疾患活動性が高いと判定する、又は糖鎖修飾判別指数Yの値が高いほど、関節リウマチの疾患活動性が低いと判定する、前記〔2〕に記載の方法。
〔4〕前記MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナルが、Jacalinレクチンを用いて測定される値である、前記〔2〕又は〔3〕に記載の方法。

(4)工程(3)で算出された値(X)または(Y)を基準値(X0)または(Y0)と比較する工程、
(5)工程(4)においてXが基準値X0よりも高い場合(X>X0)に関節リウマチの疾患活動性が高い、又は関節リウマチに罹患していると判定する工程、又は
工程(4)においてYが基準値Y0よりも低い場合(Y<Y0)を関節リウマチの疾患活動性が高い、又は関節リウマチに罹患していると判定する工程。
〔8〕前記基準値X0またはY0が、健常者からの体液試料に対して工程(1)~(3)を経て算出された値である、前記〔7〕に記載の方法。
(1)被検体液試料におけるMMP-3とJacalinとの結合量を示すシグナルを測定する工程、
(2)同被検体液試料におけるMMP-3存在量、又は同MMP-3とABA、ACA及びACGから選ばれるいずれかのレクチンもしくは抗MMP-3抗体との結合量を示すシグナルを測定する工程、

(5)工程(4)の比較工程において、X’については被検日以前より高い数値が出た場合(X’>X’0)に、Y’については被検日以前より低い数値が出た場合(Y’<Y’0)被験者の関節リウマチの疾患活動性が悪化したと判定し、低い数値が出た場合(X’<X’0)または高い数値が出た場合(Y’>Y’0)に、疾患活動性が改善したと判定する工程。
〔10〕前記被検体液試料が、関節リウマチ治療後の被験者由来の体液試料であり、前記被検日以前の体液試料が、治療前の同一被験者由来の体液試料であって、
工程(4)の比較工程において、X’については治療前より低い場合(X’<X’0)に、またはY’については治療前より高い場合(Y’>Y’0)に被験者に施された関節リウマチ治療による治療効果があったと判定し、治療前より高い又は変化がない場合(X’≧X’0)、もしくは治療前より低い又は変化がない場合(Y’≦Y’0)に、治療効果がないと判定する、前記〔9〕に記載の方法。

なお、ここで典型的なポリラクトサミン構造を認識するレクチンは、LELレクチン又はSTLレクチンである。

さらに、被検体液試料中のMMP-3上のLEL又はSTLレクチン反応性糖鎖との反応性シグナルと、Jacalin、ACG、ABA、ACA及びACGから選択されたMMP-3上のO-結合型糖鎖認識レクチンとの反応性シグナルとの比を測定する工程を含む、関節リウマチ(RA)の正確な診断法、及び類似したリウマチ性疾患との鑑別法を提供した。
また、本発明により、抗MMP-3抗体及びLELもしくはSTLレクチンなどのポリラクトサミン構造を認識するレクチンとの組み合わせ、又はさらにJacalin、ACG、ABA、ACA及びACGから選択されたMMP-3上のO-結合型糖鎖認識レクチンを組み合わせることで、類似したリウマチ性疾患と有意に識別可能な、リウマチ性多発筋痛症(PMR)、再発性多発軟骨炎(RP)、及び関節リウマチ(RA)用の診断用キットを提供できた。
本発明において、関節リウマチ診断に逆相関の(負の)糖鎖バイオマーカーとして利用する糖鎖は、血清中で観察可能な滑膜組織由来のMMP-3(matrix metalloproteinases-3)上の糖鎖であり、かつ関節リウマチ疾患活動性の増大に伴って徐々に失われていくとみられるJacalin結合性糖鎖である。
Jacalin結合性糖鎖量を、血清中のMMP-3存在量又はMMP-3上で一定に存在する糖鎖量ないし、MMP-3存在量に相関がある反応性を示すレクチンを代表とする糖鎖認識プローブの結合量で補正した値が、本発明の糖鎖バイオマーカー量ということになる。
MMP-3は関節リウマチの滑膜細胞で大量に産生され、軟骨破壊の進行に強く関わっていることが知られている(非特許文献1~3)。
従来から関節リウマチ診断用の炎症マーカーとして用いられてきた血清CRPやESRが、全身の炎症を反映しているのに対し、血清中のMMP-3値は、関節局所の病態を反映しているといわれており、現在、最も信頼性の高い関節破壊の予後予測因子として注目されている。また、初期の関節リウマチは、変形性関節症などの他の関節性疾患との鑑別が難しいが、軟骨破壊の程度を反映する血清中のMMP-3値によれば、関節リウマチを初期段階で診断することができる。
しかし、関節リウマチ疾患活動性の指標となる「DAS28」との相関性はさほど高くないことから、単独での関節リウマチの病態の指標となる診断用マーカーとして用いることには無理があり、臨床的には「DAS28」などと組み合わせて早期関節リウマチの診断や、治療方針の決定などに用いられている。
MMP-3は、シグナル配列及びプロ配列が除かれ、MMP-3成熟体となって活性化される。MMP-3は、他のMMPファミリーメンバーと同様に、Znと結合する酵素活性ドメイン(結合ドメイン)と、基質の認識に関与するヘモペキシン様ドメインとが75アミノ酸残基以下のヒンジ領域(リンカードメイン)で繋がれたドメイン構造をとっていると考えられる。MMP-3上の糖鎖構造および付加位置情報は乏しく、N-結合型糖鎖に関しては酵素活性ドメインに存在することが知られている。しかしながらO-結合型糖鎖に関する知見は全くない。他のMMPsの知見をもとに予測すると、関節リウマチに罹患後に、滑膜組織及び血清中で糖鎖構造の変化が観察されたMMP-3上のJacalin、ABA、ACAレクチンと結合するO-結合型糖鎖は、MMP-3のヒンジ領域上に複数本存在すると考えられるが、他の領域にも存在する可能性はある。
本発明において、負の糖鎖バイオマーカーとして、関節リウマチの疾患活動性を特異的に判定できる関節リウマチマーカーは、「滑膜組織由来のMMP-3上のレクチンJacalin結合性O-結合型糖鎖」である。なお、「レクチン」とは、「糖鎖を特異的に認識して結合、架橋形成するタンパク質」と定義されるものであり、本発明で用いられるJacalin、ABA、ACA及びACGは、いずれもレクチンの一種である。
本発明の糖鎖マーカーを認識するレクチンである「Jacalin」とは、ジャックフルーツ(学名:Artocarpas integliforia)の種から単離されるレクチンであり、Jacalin-related Lectinファミリーに属する。Galに単糖特異性があることが知られている(レクチンフロンティアデータベース(LfDB)より)。
Jacalinは、ジャックフルーツの種子を破砕後、抽出し、精製することもでき、既知の塩基配列を用いて遺伝子組換え技術により大腸菌宿主などから大量生産可能である。本発明の実施例では、Vector社製Uncongugated Jacalin (商品番号L-1150)およびBiotinylated Jacalin(商品番号B-1155)、又は、Jacalinを含むレクチンアレイ(商品名「LecChipTM(GlycoTechnica社製)」)を用いた。
ABA、ACA及びACGは、いずれもその由来植物またはきのこから抽出することもでき、また各塩基配列情報(各データベースの番号)から遺伝子組換え技術を利用して合成することができるが、ABAはWAKO社(組換え体)及びMBL社(天然由来)などから、ACAはVector社(天然由来)などから、また組換えACGはWAKO社などからそれぞれ市販されており、これらABA、ACA及びACGが配置されたレクチンアレイも商品名「LecChipTM」としてGlycoTechnica社から市販されている。本発明の実施例では、「LecChipTM」を用いている。
本発明の関節リウマチ診断用の負の糖鎖バイオマーカーとなる糖鎖エピトープは、MMP-3上のJacalin結合性O-結合型糖鎖である。そして、当該O-結合型糖鎖は、MMP-3の酵素活性ドメイン(結合ドメイン)と、ヘモペキシン様ドメインとを繋ぐヒンジ領域(リンカードメイン)上に複数本存在すると考えられ、このO-結合型糖鎖が複数本存在することが、レクチン間での大きな反応性の違いを生じさせると考えられる。
また、Jacalinは、ペプチド鎖のSer/Thrに結合しているO-結合型糖鎖のコア構造-1(Core1)である「Gal(β1,3)GalNAc」ないしTn抗原(GalNAc)をリガンドとするが、それらのGalNAcの6位へシアル酸(NeuNAc)の修飾が導入されることで認識がキャンセルされることが知られている。すなわち、Jacalinシグナル量の減少は、上述の糖鎖骨格の「GalNAc」の6位へのシアル酸の修飾率が増加した状態に対応すると考えられる。なお、JacalinはGalNAcへのα2,6シアル酸修飾により結合性が低下する代表例であり、同様の特徴を有するレクチンおよび糖鎖認識抗体、ないし糖鎖認識ペプチド、糖鎖認識アプタマーであれば代用することが可能である。
MMP-3に対するシアリダーゼによるシアル酸切断実験の結果(図9)によれば、MMP-3が有する主要なO-結合型糖鎖の構造はコア1構造(Gal(β1,3)GalNAc)であり、Galへのα2,3シアル酸の修飾がほぼなされていると解される。そして、GalNAcへのα2,6シアル酸修飾の程度は、健常人では低い頻度であるが、関節リウマチの疾患活動性が高まるにつれその修飾程度が増大していくと結論付けられた。
このことから、コア1構造中のGalNAcへのα2,6シアル酸修飾程度が関節リウマチ疾患活動性のマーカーとなること、及びJacalin由来シグナルは負の活動性マーカーとなることと共に、ABA及びACA由来シグナルと共に、ACG由来シグナルもまた、血清中のMMP-3量と同様の挙動を示すことが実証された。
「MMP-3上のO-結合型糖鎖であって、Jacalin非結合性であることを特徴とする、糖鎖エピトープ。」又は
「MMP-3上のO-結合型糖鎖であって、MMP-3のペプチド鎖中のSer/Thrに結合しているGalNAcがα2,6シアル酸で修飾されていることを特徴とする、糖鎖エピトープ。」
そして、このことは、Jacalinに限らず、MMP-3上のSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない物質であれば、関節リウマチ疾患活動性判定用試薬として用いることができることを示している。
具体的には、MMP-3のペプチド鎖中のSer/Thrに結合しているGalNAcに対するα2,6シアル酸の修飾を認識、結合するレクチン、糖鎖認識抗体、糖鎖認識ペプチド、糖鎖認識アプタマーなどを用いることができる。反対に、MMP-3上のSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない物質、すなわちSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない糖鎖認識抗体、結合性レクチン、糖鎖認識ペプチド及び糖鎖認識アプタマーから選択された物質もまた、関節リウマチ疾患活動性判定用試薬として用いることができる。後者の典型例が、本実施例で用いたJacalinである。
(2-1)本発明の関節リウマチの疾患活動性を判定するためのマーカー
本発明は、関節リウマチの疾患活動性を判定するために、被検血液試料におけるMMP-3上のO-結合型糖鎖の糖鎖構造の変化を測定することを特徴としている。
そのための関節リウマチ活動性の判定用マーカーとなる正の糖鎖エピトープは、以下の様に表現できる。
「MMP-3上のO-結合型糖鎖であって、Jacalin非結合性であることを特徴とする、糖鎖エピトープ。」又は
「MMP-3上のO-結合型糖鎖であって、MMP-3のペプチド鎖中のSer/Thrに結合しているGalNAcがα2,6シアル酸で修飾されていることを特徴とする、糖鎖エピトープ。」
当該糖鎖エピトープを特異的に認識する物質、すなわちSer/Thrに結合しているGalNAcがα2,6シアル酸で修飾されているO-結合型糖鎖を特異的に認識する糖鎖認識抗体、結合性レクチン、糖鎖認識ペプチド及び糖鎖認識アプタマーから選択された物質は、当該糖鎖エピトープを検出及び定量するための関節リウマチ疾患活動性判定用試薬となる。
反対に、MMP-3上のSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない物質、すなわちSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない糖鎖認識抗体、結合性レクチン、糖鎖認識ペプチド及び糖鎖認識アプタマーから選択された物質もまた、関節リウマチ疾患活動性判定用試薬として用いることができる。後者の典型例が、本実施例で用いたJacalinである。以下の解析方法においては、主としてJacalinを用いて説明するが、Jacalinに限られるわけではない。
そのようなMMP-3を検出又は定量するためには、上記糖鎖エピトープと同様の解析方法が適用される。
なお、糖鎖修飾判別指数(X又はY)は、「類似した特異性を有する複数のレクチン群があり、特にある母骨格糖鎖構造(例えば本発明ではコア1構造)に対する修飾の組成(例えばシアル酸とかフコースなど)、様式(どの糖鎖にどの位置に)、割合が異なる場合に、各レクチンの結合の強さが異なるとする。この結合の強さを定量可能なシグナルとして変換できる場合、それらを組み合わせ数式化することで、母骨格に対する修飾の状態を比較評価できるようにするための指数。」と定義できる。
X=被検体液試料中のMMP-3全量を示す測定シグナル/同MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナル、
Y=被検体液試料中のMMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナル/同MMP-3全量を示す測定シグナル。
その際、糖鎖修飾判別指数Xの値が高いほど、関節リウマチの疾患活動性が高いと判定することができ、反対に、糖鎖修飾判別指数Yの値が高いほど、関節リウマチの疾患活動性が低いと判定することができる。
Xが基準値X0よりも高い場合(X>X0)に関節リウマチの疾患活動性が高い、又は関節リウマチに罹患していると判定する工程、又はYが基準値Y0よりも低い場合(Y<Y0)を関節リウマチの疾患活動性が高い、又は関節リウマチに罹患していると判定する。
その場合は、被験者由来の体液試料で測定、算出されたX’及びY’を同一被験者の被検日以前の体液試料に対して算出された値(X’0)又は(Y’0)と比較する工程を設けることが好ましい。X’については被検日以前より高い数値が出た場合(X’>X’0)に、Y’については被検日以前より低い数値が出た場合(Y’<Y’0)被験者の関節リウマチの疾患活動性が悪化したと判定し、低い数値が出た場合(X’<X’0)または高い数値が出た場合(Y’>Y’0)に、疾患活動性が改善したと判定する。
関節リウマチ治療後の被験者由来の体液試料を治療前の体液試料と比較することで、治療効果を判定できる。
体液試料中のMMP-3存在量の測定値とは、被検体液試料中に含まれるMMP-3存在量の絶対値の測定値であり、当業者にとって、試料中のMMP-3存在量の測定方法は既知である。具体的には、MMP-3に対するモノクローナル抗体をラテックス粒子にコートして用いた、ラテックス凝集法などを用いることができる。
また、被検体液試料中の「MMP-3存在量に応じて増減する測定シグナル」としては、抗MMP-3抗体(MMP-3のポリペプチド鎖を特異的に認識する抗体)を用いて測定されるシグナル値の他、MMP-3値と同じ挙動を示すABA、ACAもしくはACG由来シグナル量、すなわち、体液試料中のMMP-3と、ABA、ACA又はACGとの結合量を示す測定シグナル値が好ましく用いられる。なお、ABA、ACA、ACGに限らず、MMP-3量と相関の認められるレクチンであれば代用することが可能である。
本発明の最も好ましいX及びYは、以下の通りである。
X=被検体液試料中のABA由来測定シグナル/同Jacalin由来測定シグナル、
Y=被検体液試料中のJacalin由来測定シグナル/同ABA由来測定シグナル。
本発明においては、被検試料をレクチンアレイまたはサンドイッチELISA法を用いてJacalinシグナル値を測定し、その逆数を同時に測定したABA、ACAもしくはACGシグナル値で補正することにより関節リウマチの罹患の有無又は関節リウマチ疾患活動性を判定することができる。ABA、ACAもしくはACGシグナル値は、MMP-3値で代替することができる。
レクチンアレイはMMP-3を蛍光標識しレクチンアレイへアプライし、結合反応後、シグナルを検出する直接法、または未標識のMMP-3をレクチンアレイへアプライ後、抗体オーバーレイ法により結合シグナルを検出する間接法のどちらでも解析することができる。
ここで、被検試料としては、被験者より採取された血液から得られる血清や血漿のみならず、関節液(滑液)、胸水、腹水、滑膜液、リンパ液など他の体液試料を対象とすることができ、被験者は特に限定されず、関節リウマチであるか否か、又は関節リウマチ疾患活動性の判定が必要な者に対して広く適用できる。
なお、被験者の血液(血清、血漿など)を被検試料とすることが被験者の負担も少なく検査時間の短縮化も図れるため最も好ましい。
本発明の検出方法により被検試料における関節リウマチ糖鎖マーカーの減少を検出し、関節リウマチの疾患活動性を的確に評価することができる。また、関節リウマチ治療の効果を的確に判定し、寛解程度を決定することができる。
<MMP-3含有画分の濃縮>
本発明の逆相関の(負の)関節リウマチ糖鎖マーカーとなるJacalin結合性糖鎖は、MMP-3上の糖鎖であるため、被験者の血液試料など被検試料が微量の場合などは、抗MMP-3抗体を用いたアフィニティカラムなどによるMMP-3濃縮画分を用いることで、より正確な関節リウマチ疾患活動性の判定が可能となる。
また、WO2011/007797に記載のストレプトアビジンコート磁気ビーズを用いる精製方法に倣い、抗MMP-3抗体をビオチン化して、ストレプトアビジンコート磁気ビーズにコンジュゲーションした磁気ビーズを用いることでも効率よく濃縮することができる。本発明の実施例では、当該磁気ビーズを用いてMMP-3濃縮画分を得ている。
標識物質の例としては、蛍光物質(例、FITC、ローダミン、Cy-3、Cy-5)、放射性物質(例、14C、3H)、酵素(例、アルカリホスファターゼ、ペルオキシダーゼ(西洋ワサビペルオキシダーゼなど)、グルコースオキシダーゼ、β-ガラクトシダーゼ)、などが挙げられる。また、ビオチンと(ストレプト)アビジンとの結合を利用することもできる。検出剤をビオチン標識し、(ストレプト)アビジンを上記標識物質で標識して、ビオチンと(ストレプト)アビジンとの結合を利用して検出することもできる。
標識物質として酵素を用いる場合は、使用する酵素に応じた適切な基質を用いて検出を行なう。例えば、酵素としてペルオキシダーゼを使用する場合、基質としてはo-フェニレンジアミン(OPD)、テトラメチルベンジジン(TMB)などが使用され、アルカリホスファターゼを使用する場合には、p-ニトロフェニルホスフェート(PNPP)などが使用される。酵素反応停止液、基質溶解液についても、選択した酵素に応じて、従来公知のものを適宜選択して使用することができる。
レクチンアレイとしては、Jacalinレクチンと共に、ABA、ACA及びACGのいずれかのレクチンが含まれてさえいればどのようなレクチンアレイを用いても良い。例えば、本発明者らの開発した特異性の異なる45種の植物レクチンが同一基板上に固定化されているレクチンアレイ(非特許文献16)やLecChipTM Ver.1.0(グライコテクニカ社製)を用いることができるが、適宜公知の方法に従って調製することができる。
レクチンアレイには、Jacalinレクチンと共に、ABA、ACA及びACGのいずれかのレクチンのみでも良いが、ABA、ACA及びACGレクチンすべてを含むことが好ましく、さらに複数の他のレクチンを支持体上に固相化することが好ましい。その際の他のレクチンとしてはMAHレクチン、MALレクチン、WGAレクチンなどが挙げられる。
Jacalin及び他のレクチンを直接支持体上に固相化してもよい(直接法)が、Jacalinなどをビオチン化Jacalinなどとし、ストレプトアビジンコートした支持体上に固相化した形態で調製することにより(間接法)、検出感度の向上と、バックグラウンドの減少を大幅に増進することができる。
レクチンアレイの支持体としては、エバネッセント波が透過可能な透明な物質であればよく、スライドガラス、ポリカーボネートなどの合成樹脂などが一般に用いられる。
Cy-3などで標識した被検試料を緩衝液で希釈しまたは希釈せずにレクチンアレイ反応槽に添加して相互作用させた後、非特異的結合をしている夾雑物をレクチンアレイ用緩衝液(市販されている)で洗浄する。なお、エバネッセント波で検出する場合は洗浄工程を省くこともできる。
一般に、レクチンの糖鎖への結合は抗原抗体反応の結合と比較して一般的に弱く、抗原抗体反応の結合定数が106~109M-1程度であるのに対して、レクチンと糖鎖間の結合定数は104~107M-1とされているため、レクチンの糖鎖結合シグナルの検出には、通常エバネッセント波励起型蛍光検出法を用いて行なうことが好ましく、本発明においてもエバネッセント波励起型蛍光検出法が好ましく用いられる。エバネッセント波励起型蛍光検出法とは、スライドガラスの端面(側面)に全反射が起こるような条件で光を入射させると、ガラス(固相)と水(液相)などの屈折率の異なる二相間の場合、界面から数百nm程度の近接場にだけエバネッセント波と呼ばれるきわめて射程距離の短い光(近接場光と呼ばれる)が滲み出ることを利用する方法である。この方法により、蛍光物質の励起光を端面から入射して近接場に存在する蛍光物質のみを励起し、蛍光観察を行なう。エバネッセント波励起型蛍光検出法は、非特許文献16などに記載されている。この検出には、GlycoStationTMReader 1200(モリテックス:グライコテクニカ社)等を使用することができる。
レクチンアレイによる評価は、体液、例えば血清由来MMP-3中にほぼ一定割合で存在する糖鎖を認識する、ないしMMP-3量と結合シグナル値に相関があるレクチンであるABAレクチンなどを内部標準レクチンとして選択して同一レクチンアレイ基板上に固定し、Jacalinシグナルなど目的レクチンシグナルを相対値化してのちに、あるカットオフ値を超えるないし超えないという判断で行う。このようにあるレクチンの値を基準にして目的レクチンのシグナルを相対値化し判別に用いる方法は本発明者らが以前発表した論文(非特許文献17)に従って行うことができる。カットオフ値の設定は、標本化された複数の関節リウマチ患者の体液、例えば血清を用いて事前に行うことができる。すなわち、あらかじめ複数の関節リウマチ患者由来の血清及び健常人血清を対象としたレクチンアレイ解析から得られた上述の相対値をもとに、後述する糖鎖修飾判別指数(X又はY)を算出する。より好ましくはあらかじめ関節リウマチの疾患活動性の程度に対応させた糖鎖修飾判別指数を複数作成し、被検血液試料由来の糖鎖修飾判別指数と比較して、被験者の関節リウマチの疾患活動性の程度を判定し、被験者が関節リウマチに罹患しているか否か、また、関節リウマチ疾患活動性がどの程度であるかを判定する。
サンドイッチ法では、抗MMP-3抗体及びJacalinなどのレクチンのいずれかを固相に結合させる。以下、固相化した結合物質を「捕捉剤」と呼び、他方を「検出剤」と呼ぶ。捕捉剤を固相化する支持体(固相)としては、プレート(例、マイクロウェルプレート)、マイクロアレイ基板(例、マイクロアレイ用スライドガラス)、チューブ、ビーズ(例、プラスチックビーズ、磁気ビーズ)、クロマトグラフィー用担体(例、Sepharose(商標))、メンブレン(例、ニトロセルロースメンブレン、PVDF膜)、ゲル(例、ポリアクリルアミドゲル)などが例示される。その中でもプレート、ビーズおよびメンブレンが好ましく用いられ、取り扱いの簡便性からプレートが最も好ましく用いられる。捕捉剤は、十分な結合強度が得られる限りいずれの方法で固相化してもよく、例えば、共有結合、イオン結合、物理的吸着などによって固相化する。或いは、予め捕捉剤を固相化した支持体を用いてもよい。
他の方法として、MMP-3結合物質を支持体上に固相化し、被検試料を添加し反応することで、MMP-3を結合物質上にMMP-3をオーバーレイした形態で準備したのちに、標識化したJacalinレクチン及び他の標識レクチンにより当該レクチンへ反応性を示すMMP-3を検出することができる。
被検血液試料など被検体液試料中のJacalin結合性糖鎖を有するMMP-3は、捕捉剤として用いた支持体上のJacalinなどのレクチン又は抗MMP-3抗体と複合体を形成する。この複合体に検出剤の標識化レクチン又は標識化抗MMP-3抗体を適用して生じたシグナルを測定することにより、被検試料中のJacalin結合性糖鎖を検出・定量する。シグナルの測定は、使用した標識物質に応じて適切な測定装置を用いて行なえばよい。
標識化を含め、被検試料の調製方法は、(2-2)のレクチンアレイ解析法と同様である。
次いで、例えばストレプトアビジンでコートしたELISAプレートの各ウェルにビオチン化したJacalinを結合させ、被検血液試料またはそのMMP-3画分を添加し、試料中のMMP-3と相互作用させる。次いで、緩衝液で洗浄するか、または洗浄せずに未反応のJacalinをブロッキング剤でブロッキングして、標識MMP-3抗体を反応させる。
また、サンドイッチELISAにおいて、Jacalin以外のABA、ACA、又はACGレクチンなど他のレクチンを併用することが好ましいのは、レクチンアレイ解析の場合と同様である。
さらに、レクチンコートELISAプレートに代えて、抗MMP-3抗体を支持体上に固相化した抗体コートELISAプレートを用いることができる。その場合、被検血液試料を添加し抗原抗体反応した後に、標識化したJacalin及び他の標識化レクチン(ABA、ACA、又はACG)を反応させ、それぞれのシグナル値を測定することができる。
ELISA法は周知の手法であり、通常の手順に従って実施すればよく、標識ごとに最適な測定装置が適用できる。
本法による関節リウマチマーカーの定量的な検出は、血清などから調製し、タンパク質量が公知であるJacalin結合性MMP-3を標準物質として用い、検量線を作成し、標準物質の相当量として換算することができる。標準物質にはJacalinへの結合性が認められるのであればリコンビナントMMP-3でもよい。たとえば実施例1にある通り、マウスミエローマ細胞(N0細胞)で大量発現し精製した、市販リコンビナントMMP-3(R&D社製、製品番号513-MP-010)を標準物質として用いることができる。また標準物質を用いずに評価する方法としては、上述のレクチンアレイ法に倣い、病変に応じてシグナルが変動しないレクチンを内部標準レクチンとして用い、Jacalinシグナルを相対値化してのちに、あるカットオフ値を超えるないし超えないという判断で行うことができる。なお、内部標準レクチンの選択、およびカットオフ値の設定は、多数の疾患活動性が判明している関節リウマチ患者の血液試料を用いて事前に行うことができる。すなわち、あらかじめ多数の疾患活動性の異なる患者から採取された血液試料を対象としたレクチンアレイ解析から統計学的に事前に内部標準を設定することができる。またあらかじめ多数の疾患活動性の異なる患者から採取された血液試料を対象としたELISA測定により得られた上述の相対値をもとに判別式を作成する。より好ましくは関節リウマチの進行もしくは疾患活動性悪化の程度に対応させた判別式を複数作成し、被検血液試料の関節リウマチの進行もしくは疾患活動性悪化の程度を判定し、被験者が関節リウマチに罹患しているか否か、また、関節リウマチ疾患活動性のステージがどの程度であるかを判定する。
なお、ELISA測定をマイクロアレイやSPRなどの異なる相互作用解析原理に基づく測定法に置き換えることもできる。その場合、これらは周知の手法であるため、通常の手順に従って実施すればよく、手法ごとに最適な測定装置が適用できる。
MMP-3上のSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない物質、好ましくはJacalinを関節リウマチの疾患活動性判定用試薬の有効成分として用いることができる。
さらに血液試料中のMMP-3に対して、MMP-3存在量依存的にMMP-3と特異的に結合する物質、具体的にはABA、ACA及びACGから選択されるレクチン、又は抗MMP-3抗体を組み合わせて、関節リウマチ疾患活動性判定用キットとして用いることができる。
本発明の活動性判定用キットは、例えば下記の組み合わせが可能である。
(1)(a)Jacalinを必須のレクチンとして含み、さらに好ましくはABA、ACA、又はACGの少なくとも1つを含む複数のレクチンが、直接又はビオチン-アビジンを介して間接的に結合された支持体;及び
(b)被検血液試料中のMMP-3を標識可能な標識物質。
(2)(a)Jacalinを必須のレクチンとして含み、さらに好ましくは内部標準レクチンとなるABA、ACA、又はACGの少なくとも1つを含む複数のレクチンが、直接又はビオチン-アビジンを介して間接的に結合された支持体;及び
(b)標識化された抗MMP-3抗体。
(3)(a)抗MMP-3抗体が、直接又はビオチン-アビジンを介して間接的に結合された支持体;及び
(b)標識化されたJacalinを含み、さらに好ましくはABA、ACA、又はACGの少なくとも1つを含む複数の標識化されたレクチン。
その際、支持体は1つで複数のレクチンを固定してもよいが、各レクチンごとに異なる支持体に固定して用いても良い。
また、関節リウマチ患者における、経時的な疾患活動性の病態変化を正確に把握するために用いられる。
そして、関節リウマチ治療を受けている患者における当該治療の効果を客観的に判定するために用いられる。特に、寛解したかどうか、すなわち治療を続ける必要がなくなったかどうかを判定するために用いる。
本発明により、被検体液試料(血清試料など)中のMMP-3上のLEL又はSTLレクチンなどのポリラクトサミン構造を認識するレクチン反応性糖鎖が、リウマチ性疾患のうちでも特に再発性多発軟骨炎(RP)患者体液試料との反応性が有意差をもって極めて高く、次いでリウマチ性多発筋痛症(PMR)患者体液試料との反応性が、関節リウマチ(RA)患者及び健常人(HC)体液試料と際だった有意性をもって高いことが示された。
すなわち、抗MMP-3抗体と共に、LEL及び/又はSTLレクチンなどのポリラクトサミン構造を認識するレクチンとを組み合わせたキットは、再発性多発軟骨炎(RP)診断用キット及び/又はリウマチ性多発筋痛症(PMR)診断用キットとなる。
ここで、LELレクチンは、Lycipersicon esculentum由来のGlcNAc反応性レクチンとして、STLレクチンは、Solanum tuberosum由来のGlcNAc反応性レクチンとして従来から知られており、両レクチンは、ポリラクトサミン構造のGlcNAcを認識すると考えられる。なお、LELレクチンについての他の情報は、本発明者らの文献(Methods in Enzymology, vol.479, 2010, p.185-204)に、またSTLレクチンについての他の情報は、同(Methods in Enzymology, vol.478, 2010, p.181-195)に詳細に記載されている。
また、他のポリラクトサミン構造を認識するレクチンとしては、Galectin-3などが知られている。
また、前記(2-4)に記載のサンドイッチ法を適用し、被検血清試料など被検体液試料を、抗MMP-3抗体と、LEL及び/又はSTLレクチンなどのポリラクトサミン構造を認識するレクチンとを用いたサンドイッチ法、例えば標識抗MMP-3抗体と、LEL及び/又はSTLレクチンを含むレクチンアレイとの組み合わせによるサンドイッチELISA法などである。
当該測定法を用いることで、再発性多発軟骨炎(RP)及び/又はリウマチ性多発筋痛症(PMR)を、特異的に診断することができる。
被検体液試料(血清試料など)中のMMP-3上のLEL又はSTLレクチン反応性糖鎖との反応性シグナルと、Jacalin、ACGなどMMP-3上のO-結合型糖鎖認識レクチンとの反応性シグナルとの比を算出する工程を含む、関節リウマチ(RA)の正確な診断法、及び類似したリウマチ性疾患との鑑別法となる。
具体的には、前記(2-3)に記載のレクチンアレイ法を適用し、被検血清試料など被検体液試料を、MMP-3濃縮画分をLEL及び/又はSTLレクチンなどのポリラクトサミン構造を認識するレクチンと共に、Jacalin、ACGなどのMMP-3上のO-結合型糖鎖認識レクチンを含むレクチンアレイで測定する方法が適用できる。
また、前記(2-4)に記載のサンドイッチ法を適用し、被検血清試料など被検体液試料を、抗MMP-3抗体と、LEL及び/又はSTLレクチンなどのポリラクトサミン構造を認識するレクチンとさらにJacalin、ACGなどMMP-3上のO-結合型糖鎖認識レクチンを用いたサンドイッチ法、例えば標識抗MMP-3抗体と、LEL及び/又はSTLレクチンと共にJacalin、ACGなどのMMP-3上のO-結合型糖鎖認識レクチンを含むレクチンアレイとの組み合わせによるサンドイッチELISA法などである。
本発明におけるその他の用語や概念は、当該分野において慣用的に使用される用語の意味に基づくものであり、本発明を実施するために使用する様々な技術は、特にその出典を明示した技術を除いては、公知の文献等に基づいて当業者であれば容易かつ確実に実施可能である。また、各種の分析などは、使用した分析機器又は試薬、キットの取り扱い説明書、カタログなどに記載の方法を準用して行った。
なお、本明細書中に引用した技術文献、特許公報及び特許出願明細書中の記載内容は、本発明の記載内容として参照されるものとする。
また、各実施例中で使用しているすべての体液(血清及び滑液)は、採取先の臨床機関でインフォームド・コンセント受けた患者に由来するものであり、かつ発明者らの所属する機関それぞれで倫理審査を受け、承認されたものである。
本実施例では、関節リウマチ患者32症例およびその他の関節炎4症例の血清を収集し、そのうち30症例を対象に行った。
血清中MMP-3の簡易分画・濃縮(エンリッチ)は、本発明者らが以前開発した手法(非特許文献14)をベースに、これまで発明者が分析してきたタンパク質に比べ、血中濃度が非常に低いMMP-3タンパク質のために条件を最適化して以下の通り行われた。
(1-1)プレクリア工程:
血清中に含まれるタンパク質のうち、抗原抗体反応場に利用する磁気ビーズへ非特異的に結合するものを事前に除去する作業(プレクリア)を行った。すなわち、1.5 mL容チューブ取り分けた血清 40μLに対し、予め反応バッファー(1%Triton X-100入りTris-buffered saline (TBSTx))で3回洗浄したストレプトアビジン固定化磁気ビーズ(SAビーズ)溶液(多摩川精機社製、10mg/mL)10μLと反応バッファー36μLを加えて溶液調整し、4℃で30分間振盪反応した。反応後、磁石により磁気ビーズを1カ所にトラップし、得られた上清を別の1.5mL容チューブに回収した。
回収した上清に、ビオチン化MMP-3抗体(R&D社製)(50μg/mL)を4μL加え、4℃で一晩振盪しながら抗原抗体反応を行った。反応後、反応バッファーで洗浄済のSAビーズ溶液(10mg/mL)10μLを加えて、4℃で30分間振盪反応した。抗原-抗体複合体と結合した磁気ビーズを磁石で回収し、得られた上清を除去後、ビーズを反応バッファー200μLで3回洗浄した。ビーズを磁石で回収する際、ビーズとビーズの間に挟まって非特異的に回収されている夾雑タンパク質を完全に除去するための洗浄は、磁気ビーズへ反応バッファーを加えてから、超音波分散装置(多摩川精機社製)や多目的スピンダウンミキサーであるバグクラッシャー(タイテック社製)等を用いて、ビーズを完全に分散することで達成された。
ビーズに抗体を介し間接的にトラップされたMMP-3を溶出するために、洗浄したビーズに0.2%SDSを含むTBS 10μLを加え、95℃で5分加熱し、抗体を不活性化することでMMP-3を遊離した。磁気ビーズを磁石で1カ所にトラップし、得られた上清を回収した。
得られた上清には、MMP-3だけでなく不活性化した抗MMP-3抗体も共存している。この抗体はビオチン化されているため、SAビーズにより容易にトラップできる。そこで、1.5mL容チューブに溶出液と、予め反応バッファーで3回洗浄済のSAビーズ(20mg/mL)10μLを加え、4℃で30分間ビオチン-アビジン反応を行い、反応後速やかに磁石にて磁気ビーズを1カ所にトラップした。得られた上清を回収してMMP-3-IP産物(20μL)とした。
各サンプルの回収量を見積もるため、上述のビオチン化抗体を用いてウエスタンブロット(WB)を行った。事前に市販組換え型ヒトMMP-3(rMMP-3, R&D Systems社製)を用い、WBによる当該タンパク質の検出下限を算出したところ、50pgであった。そこで、rMMP-3の50,100, 200, 400pgを標準物質としてサンプルと一緒にゲルにロードし、WBすることで、各サンプル中MMP-3量の半定量的解析を行った。なお、サンプルはそれぞれ0.1μLゲルへロードした。
その結果、いくつかのサンプルでrMMP-3を400pgロードした時のバンド強度を超えたため、それぞれ適宜希釈して、再度WBした。その結果を図1に記す。
エンリッチの妥当性を検証するため、臨床現場で測定されたMMP-3値との比較をしたところ、強い相関を示した(図2右)。これはエンリッチにより得られたMMP-3量は、血中に存在しているときの濃度に依存していることを示唆している。
なお、今回算出した数値は臨床現場で通常測定している値と比較すると、大きなずれがあるが、これは本実施例で使用した標準物質と、臨床現場で用いられている測定系に使用される標準物質は異なるものであり、それにより抗体との反応性が異なるためと考えられた。
さらに、今回は市販健常者血清(コージンバイオ社製)に由来するMMP-3についても同様の実験を行っているが、文献的に報じられている健常者のMMP-3濃度範囲で回収ができていた。
本実施例では、実施例1でMMP-3の簡易分画・濃縮(エンリッチ)がなされた被検血清試料MMP-3-IP中のMMP-3に対して、レクチンアレイによる比較糖鎖プロファイリングを適用し、リウマチに伴うMMP-3上の糖鎖変化を調べた。
実施例(1-4)で得られた各被検MMP-3-IP 試料を2μLとり、レクチンアレイ反応バッファーである1%TritonX-100含有Phosphate-buffered saline (PBSTx)により、60μLに調整した。この溶液をレクチンマイクロアレイ(LecChipTM(グライコテクニカ社製))の各反応槽(ガラス1枚当たり7つの反応槽が形成されている)へ添加し、20℃で10時間以上相互作用反応した。これにより試料中のMMP-3上の糖鎖とアレイ基板上に固定されている45種のレクチンとの結合反応が平衡状態に達する。その後、未反応の基盤上レクチンへ検出用抗体上の糖鎖が結合し、ノイズシグナルが生じることを避けるため、ヒト血清由来IgG溶液(シグマ社製)を2μL(20μg相当)加え、30分反応させた。このブロッキング反応を行った後に、60μLのPBSTxで各反応槽を3回洗浄した後、PBSTx 溶液 58μLに 再度ヒト血清由来IgG溶液を2μL加え、若干攪拌した後に、検出用の抗MMP-3抗体(ビオチン化物)を100ng相当加え、20℃で1時間反応させた。抗原抗体反応後、60μLのPBSTxで各反応槽を3回洗浄し、次いでCy-3標識ストレプトアビジン(GEヘルスケア社製)200ng相当が含有しているPBSTx溶液を加え、さらに30分、20℃で反応した。反応後、60μLのPBSTxで各反応槽を3回洗浄した後、アレイスキャナーGlycoStationTM(グライコテクニカ社製)によりアレイスキャンを行った。
以上より、MMP-3上の糖鎖構造は、図4に示す通り、コア1構造ないしTn構造を骨格とするO-結合型糖鎖であり、シアル酸の修飾の程度が異なる構造群であることが想定される。
この現象はMMP-3量とは相関しないことは興味深いことであり、以後臨床パラメータとの相関性について検証することにした。
(実施例2)で得られたMMP-3にユニークなレクチンシグナルと臨床パラメータとの相関性を調べた。なお、検証にあたり、タンパク質上のO-結合型糖鎖の質的変化を反映させた糖鎖修飾判別指数Xのうち、X=ABA/Jacalinの場合の値を用いた。検証したパラメータのうち関節リウマチの活動性を示すパラメータとの比較を図7に示す。患者VAS以外のパラメータ(DAS28-CRP, DAS28-ESR, CDAI, SDAI, 医師VAS)とある程度の相関を示した。これまでMMP-3量は疾患の活動性とは相関が弱いと報じられていたため、そのタンパク質上の糖鎖の質的変化を反映したABA/Jacalin値が、それに反し活動性と相関があることは非常に興味深い。
その結果、糖鎖修飾判別指数であるABA/Jacalin値は疾患活動性指標とよく相関していた(スピアマンの順位相関係数より求めたP値はP<0.001)。一方、MMP-3量についてもグラフ化したが、いずれの評価法に対しても相関を示さなかった。
このような糖鎖の質的変化がシアル酸の修飾の様式の相違や割合の変化によるものであるかを検証するために、シアリダーゼ消化前後のMMP-3についてレクチンアレイで比較解析することにした。
(実施例3)の結果から、血清中MMP-3の各レクチンに対する反応性の変化は、MMP-3上のO-結合型糖鎖へのシアル酸修飾の様式の相違、割合の変化によって生じている可能性が高いと考えられる。そこで、本実施例では、(実施例1)及び(実施例2)の解析で利用した被検血清試料のうちの糖鎖修飾判別指数(ABA/Jacalin値)低値群及び高値群それぞれ6例ずつ合計12症例を選び、各血清試料についてMMP-3上のシアル酸残基を切断除去した場合に、糖鎖プロファイルへ与える影響を調べた。
(4-1)血清中MMP-3のエンリッチ
(実施例1)の方法にしたがい、前記選定した12の症例の血清試料について再度血清中のMMP-3をエンリッチし、12種類の被検MMP-3-IP試料を得た。糖鎖修飾判別指数(ABA/Jacalin値)低値群のサンプル名はRA07,20,31,33,34,35、及び高値群のサンプル名はRA10,13,14,17,21,24である。
得られたMMP-3IP溶液 20μLのうち2μLをシアリダーゼ消化に使用した。溶液2μLに専用 5×reaction buffer B を2μLとSialidase A(いずれも Prozyme 社製)を0.5μL(final: 0.125 U/mL)加え、超純水により10μLに調製した。37℃にて1時間反応し、MMP-3からシアル酸(α2,3シアル酸とα2,6シアル酸の両方)を切断除去した。コントロールとして、上述組成のうちシアリダーゼを含まない反応溶液も調製し、併せて反応した。反応溶液はすべて反応終了後にしばらく氷上静置した。これにより得られたものをシアリダーゼ消化産物とした。
抗体オーバーレイ・レクチンアレイは実施例1と同様の方法により行った。なお、シアリダーゼ反応溶液は、予めPBSTxで総容量60μLになるように調製したのちにレクチンアレイ解析に使用した。
その結果を図9に示す。シアル酸を切断したのちにO-結合型糖鎖上のシアル酸を認識するレクチン(ACG、MAH)でシグナルがほぼ0になった。それに伴い、シアル酸修飾を一切許容しないコア1構造認識レクチンであるPNAのシグナルがすべてのサンプルで出現した。一方で、Tn構造を認識するレクチンであるHPA, VVA, やGALIはシアル酸消化後もシグナルが得られなかった。
以上により、MMP-3が有する主要なO-結合型糖鎖の構造はコア1構造(Gal(β1,3)GalNAc)であり、Galへのα2,3シアル酸の修飾がほぼなされていると解される。そして、GalNAcへのα2,6シアル酸修飾の程度は、低い頻度でなされていて、その割合が病態に伴い変動すると結論付けられた。このことから、コア1構造中のGalNAcへのα2,6シアル酸修飾程度が関節リウマチ疾患活動性のマーカーとなり、及びJacalin由来シグナルは負の活動性マーカーとなることと共に、ABA及びACA由来シグナルと共に、ACG由来シグナルもまた、血清中のMMP-3量と同様の挙動を示すことが実証された。つまり、糖鎖修飾判別指数X又はYの算出に用いる「被検血清試料中のMMP-3全量を示す測定シグナル」としては、MMP-3存在量の測定値と共に、抗MMP-3抗体を用いて測定されるシグナル値、血清試料中のMMP-3とABA、ACA及びACGから選ばれるいずれかのレクチンとの結合量を示す測定シグナル値を用いることができる。
また、シアル酸消化後のABA規格化糖鎖プロファイルはほぼ一致したことから、サンプルごとに生じていた反応性の違いはシアル酸の修飾の様式、度合い(図9中の右に示した想定される反応前後の構造変化を参照)に起因していることが示唆された。
病変部位となる滑膜組織から生産されたMMP-3は滑液中に放出され、骨破壊に関与することになる。その一部が血中に移動し、血清中のMMP-3量が滑膜組織での生産量に相関があることを前提として、従来から血清中のMMP-3量がマーカーとして利用されてきたと考えられる。しかしながら、(実施例2)においてレクチンアレイ解析により関節リウマチ活動性との相関が見出された血清中のMMP-3上のO-結合型糖鎖でのシアル酸の修飾の様式、度合いの変化が、滑膜組織で生産されたMMP-3上での糖鎖構造の変化を正確に反映したものであるかどうかは推測の域を超えない。
そこで、本実施例では、滑液中に放出されたMMP-3がどのような糖鎖プロファイルをしているかを調べることで、血中MMP-3の糖鎖変化の関節リウマチ活動性との関連の妥当性を検証することにした。
実施例1の方法にしたがい、関節リウマチ(RA)3症例、変形性関節症(OA)3症例について滑液からMMP-3をエンリッチした。なお、RAとOAで滑液中のMMP-3量が大きく異なるため、予め滑液中のMMP-3量を測定し、同一MMP-3量を利用してエンリッチを行った。その結果、図10に示す通り、取得MMP-3量はほぼ同じになった。
抗体オーバーレイ・レクチンアレイは実施例2と同様の方法により行った。その結果を図11に示す。
大変興味深いことにRAとOAのMMP-3は糖鎖プロファイルが大きく異なっていた。そしてその相違は血清中MMP-3と同様に4つのレクチンで説明ができた。RA患者由来のMMP-3は相対的にJacalinシグナルが弱く、ACGは強い傾向にあった(図12)。このJacalinシグナルの傾向は関節リウマチ患者血清で得られた疾患活動性に伴うJacalinシグナルの減弱に一致するものである。
以上により、滑膜細胞は活性化に伴いMMP-3の放出量を増加させるが、活性化による細胞の糖鎖合成マシナリに変化が生じ、その結果としてタンパク質上のO-結合型糖鎖のシアル酸修飾、特にα2,6シアル酸の修飾に変化が生じ、高度になることが示唆された。また、実施例2及び3の被検血清試料において得られた知見は、滑膜組織から産生されたMMP-3での糖鎖構造の変化を正確に反映したものであることが実証された。
(6-1)他のリウマチ性疾患患者由来血清を用いたMMP-3上のJacalinなどのO-結合型糖鎖との反応性比較
本実施例では、関節リウマチ患者(RA)14例、リウマチ性多発筋痛症患者(PMR)8例、再発性多発軟骨炎患者(RP)7例、健常人(HC)5例の血清を対象に、実施例1及び2の検証を行った。RA、PMR、RPはどれも血清MMP-3が高値になりやすい疾患である。臨床診断をする上でRAとPMRは鑑別に苦慮することも多く、RPは臨床症状が比較的特徴的であるものの、感度の高い検査方法が存在しない。具体的には、上記の各対象疾患の患者血清から、実施例1と同様の方法でMMP-3を濃縮し、実施例2と同様の方法で各疾患由来のMMP-3の糖鎖プロファイリングを行った。
その結果、各疾患における糖鎖構造情報を取得することができた。得られた全患者のデータから、まずACG、ABA、ACA、及びJacalin値を比較検討したところ、図13に示す通り、実施例1(1-1)で対象としたリウマチ患者血清での結果(図5)と同様に、ACG、ABA、ACAではMMP-3値と相関し、Jacalinは相関しないという結果が得られた。
そこで全てのリウマチ疾患でMMP-3値と高い相関を示す典型的なレクチンであるACGとJacalinとの比をとり、ACG/Jacalin値を算出してグラフ化した(図15)ところ、この値が高値を示すのはRAの活動性の高い群だけであった。
ところで、実施例4で実証したように、Jacalin値の低下はMMP-3含有O-結合型糖鎖のGalNAcにおけるα2,6シアル酸修飾を反映していると解される。そして、このようなGalNAcのα2,6シアル酸修飾を反映したJacalin値の低下現象は、リウマチ疾患のうちでもRAに特有であり、かつRAの活動性の高い症例に特徴的なものであることが本実施例においてもあらためて実証できた。
(6-1)のように、JacalinのシグナルはRAの活動性を示すものとしてあらためて証明できたが、RAと非RAの鑑別にも用いられることが示唆されるので、他のレクチンのシグナルの差も再度比較検討をした(図16)。
他のレクチンとしては、(6-1)のレクチンアレイプロファイリングの結果、統計学的に有意差があるLEL (Lycipersicon esculentum由来)及びSTL (Solanum tuberosum由来)というポリラクトサミン構造を認識するレクチンが選択された。両レクチンは、ポリラクトサミン構造のGlcNAcを認識すると考えられる。
LEL及びSTLレクチン反応性糖鎖のシグナルを測定し、Steel-Dwass検定により判定したところ、HC、RAと比較してPMR、RPでは有意に高値であるという結果を得、疾患ごとに大きな違いがあることが判明した(図16上段)。
同様のSteel-Dwass検定を血清MMP-3濃度で各疾患について行ったが、このような差は血清MMP-3濃度のみでは見ることができない(図16下段左)。
これらのシグナルを使用し、HC・RAとPMR、HC・RA・PMRとRPを鑑別できるかを、縦軸に真の陽性率、横軸に擬陽性率をとりプロットしたROC曲線(受信者動作特性曲線)により解析した。3群の中で最もRAとの鑑別の難しいPMRについてのROC解析を行ったところ、ROC=1.0でかつAUC(ROC曲線下面積)=1.0という非常に高い鑑別能力を持つことが判明した(図16下段右)。STL、LELの両シグナルとも、PMRとRA/HCもしくは、RPとPMR/RA/HCをAUC=1.0という非常に高い鑑別能力を持つことが判明した(図16下段右と同じグラフ)。
すなわち、被検血清試料中でのMMP-3上のLEL及び/又はSTLレクチン反応性糖鎖が、リウマチ性多発筋痛症(PMR)及び再発性多発軟骨炎(RP)それぞれの優れた糖鎖マーカーとなることを示す結果となった。
このことは、被検血清試料のMMP-3濃縮画分におけるLELもしくはSTLレクチンとの反応性を測定するか、又は抗MMP-3抗体とLELもしくはSTLレクチンとのサンドイッチアッセイ法での測定値により、リウマチ性多発筋痛症(PMR)の罹患の有無、及び/又は再発性多発軟骨炎(RP)の罹患の有無を判定できることを意味する。さらに、従来鑑別が難しかった関節リウマチとリウマチ性多発筋痛症とを正確に鑑別できることを示す結果でもある。
鑑別が難しい症例があるリウマチ性多発筋痛症(PMR)と関節リウマチ(RA)とを正確に識別できたことと共に、正確な診断方法が確立していなかった再発性多発軟骨炎(RP)の正確な診断方法が提供できたことは画期的である。
RAではLEL又はSTLシグナルといったポリラクトサミンを認識するレクチンのシグナルがPMR、RPと比較して低いため、PMR及びRPとは鑑別できるが、HCでもLEL又はSTLシグナルは低いため、HCとの鑑別はできない。そこで、RAについては、HCとの違いを際立たせるために、MMP-3上のACG、JacalinなどのO-結合型糖鎖のシグナルとの比をとることで、RAのみで高値になる指標(例えばJacalin/LELやACG/LEL)を作成することができた(図17上段)。これらのシグナル値を元に疾患の中でRAのみを検出する能力をROC解析で検討したところ、AOC=0.95~0.98という高い鑑別能力がある事が示された(図17下段)。
すなわち、類似したリウマチ性疾患のうちで、関節リウマチであることを正確に鑑別するためには、被検血清試料のMMP-3反応性画分におけるLELもしくはSTLレクチンと、Jacalin、 ACGなどO-結合型糖鎖との反応性の比を測定すれば良いことが示されたことになる。

- 被検体液試料におけるMMP-3上のO-結合型糖鎖の糖鎖構造の変化を測定する工程を含む、関節リウマチの疾患活動性の判定のためのデータを提供する方法。
- 前記糖鎖構造の変化の測定が、被検体液試料中のMMP-3全量を示す測定シグナルに、MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナルの逆数を乗じた、もしくは前記糖鎖構造の変化の測定が、被検体液試料中のMMP-3全量を示す測定シグナルの逆数に、MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナルを乗じた、下記の糖鎖修飾判別指数X又はYを算出することで行われる、請求項1に記載の方法;

Y=被検体液試料中のMMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナル/同MMP-3全量を示す測定シグナル。 - 糖鎖修飾判別指数Xの値が高いほど、関節リウマチの疾患活動性が高いと判定する、又は糖鎖修飾判別指数Yの値が高いほど、関節リウマチの疾患活動性が低いと判定する、請求項2に記載の方法。
- 前記MMP-3上のJacalin結合性O-結合型糖鎖全量を示す測定シグナルが、Jacalinレクチンを用いて測定される値である、請求項2又は3に記載の方法。
- 前記MMP-3全量を示す測定シグナルが、体液試料中のMMP-3存在量の測定値、又は体液試料中のMMP-3存在量に応じて増減する測定シグナルである、請求項2~4のいずれかに記載の方法。
- 前記MMP-3存在量に応じて増減する測定シグナルが、MMP-3のポリペプチド鎖を特異的に認識する抗MMP-3抗体を用いて測定されるシグナル値、又は体液試料中のMMP-3とABA、ACA及びACGから選ばれるいずれかのレクチンとの結合量を示す測定シグナル値である、請求項5に記載の方法。
- 関節リウマチの疾患活動性を判定する方法であって下記の(1)~(5)を含む方法;
(1)被検体液試料におけるMMP-3とJacalinとの結合量を示すシグナルを測定する工程、
(2)同被検体液試料におけるMMP-3存在量、又は同MMP-3とABA、ACA及びACGから選ばれるいずれかのレクチンとの結合量もしくは抗MMP-3抗体との結合量を示すシグナルを測定する工程、
(3)工程(1)で得られた測定値の逆数に、工程(2)で得られた測定値を乗じた糖鎖修飾判別指数Xまたは、または工程(1)で得られた測定値に、工程(2)で得られた測定値の逆数を乗じた糖鎖修飾判別指数Yを算出する工程、
(4)工程(3)で算出された値(X)または(Y)を基準値(X0)または(Y0)と比較する工程、
(5)工程(4)においてXが基準値X0よりも高い場合(X>X0)に関節リウマチの疾患活動性が高い、又は関節リウマチに罹患していると判定する工程、又は
工程(4)においてYが基準値Y0よりも低い場合(Y<Y0)を関節リウマチの疾患活動性が高い、又は関節リウマチに罹患していると判定する工程。 - 前記基準値X0またはY0が、健常者からの体液試料に対して工程(1)~(3)を経て算出された値である、請求項7に記載の方法。
- 被験者の関節リウマチの疾患活動性の経時的な病状変化を判定する方法であって、下記の(1)~(5)を含む方法;
(1)被検体液試料におけるMMP-3とJacalinとの結合量を示すシグナルを測定する工程、
(2)同被検体液試料におけるMMP-3存在量、又は同MMP-3とABA、ACA及びACGから選ばれるいずれかのレクチンもしくは抗MMP-3抗体との結合量を示すシグナルを測定する工程、
(3)工程(1)で得られた測定値の逆数に、工程(2)で得られた測定値を乗じた糖鎖修飾判別指数X’、又は工程(1)で得られた測定値に、工程(2)で得られた測定値の逆数を乗じた糖鎖修飾判別指数Y’を算出する工程、
(4)工程(3)で算出された値(X’)又は(Y’)を、同一被験者の被検日以前の体液試料に対して工程(1)~(3)を経て算出された値(X’0)又は(Y’0)と比較する工程、
(5)工程(4)の比較工程において、X’については被検日以前より高い数値が出た場合(X’>X’0)に、Y’については被検日以前より低い数値が出た場合(Y’<Y’0)被験者の関節リウマチの疾患活動性が悪化したと判定し、低い数値が出た場合(X’<X’0)または高い数値が出た場合(Y’>Y’0)に、疾患活動性が改善したと判定する工程。 - 前記被検体液試料が、関節リウマチ治療後の被験者由来の体液試料であり、前記被検日以前の体液試料が、治療前の同一被験者由来の体液試料であって、
工程(4)の比較工程において、X’については治療前より低い場合(X’<X’0)に、またはY’については治療前より高い場合(Y’>Y’0)に被験者に施された関節リウマチ治療による治療効果があったと判定し、治療前より高い又は変化がない場合(X’≧X’0)、もしくは治療前より低い又は変化がない場合(Y’≦Y’0)に、治療効果がないと判定する、請求項9に記載の方法。 - MMP-3上のO-結合型糖鎖であって、Jacalin非結合性であることを特徴とする、関節リウマチ活動性判定用マーカーとなる糖鎖エピトープ。
- MMP-3上のO-結合型糖鎖であって、MMP-3のペプチド鎖中のSer/Thrに結合しているGalNAcがα2,6シアル酸で修飾されていることを特徴とする、関節リウマチ活動性判定用マーカーとなる糖鎖エピトープ。
- 請求項11又は12に記載の糖鎖エピトープからなる関節リウマチ活動性判定用マーカー。
- MMP-3上のO-結合型糖鎖であって、MMP-3のペプチド鎖中のSer/Thrに結合しているGalNAcがα2,6シアル酸で修飾されているO-結合型糖鎖の関節リウマチの疾患活動性の判定方法における使用。
- 関節リウマチの疾患活動性の判定方法における使用のためのMMP-3のペプチド鎖中のSer/Thrに結合しているGalNAcがα2,6シアル酸で修飾されているO-結合型糖鎖。
- 請求項11もしくは12に記載の糖鎖エピトープ、又は当該糖鎖エピトープを複数含有するMMP-3を特異的に認識する物質を含む、被検体液試料に適用可能な関節リウマチ疾患活動性判定用試薬。
- 前記糖鎖エピトープを特異的に認識する物質が、糖鎖認識抗体、レクチン、糖鎖認識ペプチド及び糖鎖認識アプタマーから選択された物質である、請求項16に記載の関節リウマチ疾患活動性判定用試薬。
- MMP-3上のSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない物質を含む、被検体液試料に適用可能な関節リウマチ疾患活動性判定用試薬。
- 前記MMP-3上のSer/Thrに結合しているαGalNAcを含む糖鎖を認識し、かつ当該αGalNAcにα2,6シアル酸が結合した糖鎖を認識しない物質がJacalinである、請求項18に記載の関節リウマチの疾患活動性判定用試薬。
- 請求項16~19のいずれか一項に記載の関節リウマチ疾患活動性判定用試薬と共に、さらに血液試料中のMMP-3に対して、MMP-3存在量依存的にMMP-3と特異的に結合する物質を含む、関節リウマチ疾患活動性判定用キット。
- 前記体液試料中のMMP-3に対して、MMP-3存在量依存的にMMP-3と特異的に結合する物質が、ABA、ACA及びACGから選択されるレクチン、又は抗MMP-3抗体である、請求項20に記載の関節リウマチ疾患活動性判定用キット。
- 被検体液試料中のMMP-3上の糖鎖であって、ポリラクトサミン構造を認識するレクチンとの結合活性を有する糖鎖を含むことを特徴とする、リウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)診断用マーカーとなる糖鎖エピトープ。
- ポリラクトサミン構造を認識するレクチンがLELレクチン又はSTLレクチンである請求項22に記載の糖鎖エピトープ。
- 請求項22又は23に記載の糖鎖エピトープからなるリウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)診断用マーカー。
- 請求項22もしくは23に記載の糖鎖エピトープ、又は当該糖鎖エピトープを含有するMMP-3を特異的に認識する物質を含む、被検血液試料に適用可能なリウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)診断用試薬。
- 被検体液試料に対し、抗MMP-3抗体への結合活性と共にポリラクトサミン構造を認識するレクチンとの結合活性を示す糖鎖の存在量を測定する工程を含むことを特徴とする、リウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)の診断のためのデータを提供する方法。
- ポリラクトサミン構造を認識するレクチンがLELレクチン又はSTLレクチンである請求項26に記載の方法。
- 前記測定工程が、抗MMP-3抗体と、ポリラクトサミン構造を認識するレクチンとのサンドイッチアッセイ法による測定工程である,請求項26又は27に記載の方法。
- リウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)への罹患の有無を疑う被験者に由来する体液試料に対し、下記の(1)~(3)の工程により判定することを特徴とする、リウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)への罹患の判定方法;
(1)被験者由来の体液試料に対し、抗MMP-3抗体への結合活性と共にポリラクトサミン構造を認識するレクチンとの結合活性を示す糖鎖の存在量を測定する工程、
(2)健常人及び/又は関節リウマチ患者由来の体液試料に対し、抗MMP-3抗体への結合活性と共にポリラクトサミン構造を認識するレクチンとの結合活性を示す糖鎖の存在量を測定する工程、
(3)(1)で得られた糖鎖の存在量を示す数値が(2)で得られた糖鎖の存在量を示す数値を有意に上回っていた場合、リウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)に罹患していると判定する工程。 - 前記(1)及び(2)の測定工程が、抗MMP-3抗体と、LELレクチン又はSTLレクチンとのサンドイッチアッセイ法である請求項29に記載の方法。
- 被検体液試料に対して少なくとも下記の(1)及び(2)の測定工程が適用されることを特徴とする、関節リウマチ(RA)診断のためのデータを提供する方法;
(1)抗MMP-3抗体への結合活性と共にポリラクトサミン構造を認識するレクチンとの結合活性を示す糖鎖の存在量を測定する工程、
(2)抗MMP-3抗体への結合活性と共に、Jacalin、ABA、ACA及びACGから選択された少なくとも一種のO-結合型糖鎖認識レクチンとの結合活性を示す糖鎖の存在量を測定する工程。 - 前記(1)及び(2)測定工程が、前記抗MMP-3抗体と、ポリラクトサミン構造を認識するレクチン及び前記O-結合型糖鎖認識レクチンを含むレクチンアレイを用いたサンドイッチアッセイにより同時に行われることを特徴とする請求項31に記載の方法。
- 関節リウマチへの罹患の有無を疑う被験者に由来する体液試料に対し、下記の(1)~(4)の工程により判定することを特徴とする、関節リウマチへの罹患の判定方法;
(1)抗MMP-3抗体への結合活性と共にポリラクトサミン構造を認識するレクチンとの結合活性を示す糖鎖の存在量を測定する工程、
(2)抗MMP-3抗体への結合活性と共に、Jacalin、ABA、ACA及びACGから選択された少なくとも一種のO-結合型糖鎖認識レクチンとの結合活性を示す糖鎖の存在量を測定する工程。
(3)(1)で得られた糖鎖の存在量に対する(2)で得られた糖鎖の存在量の比を算出する工程、
(4)あらかじめ又は同時に、健常人由来体液試料に対して(1)~(3)の工程を適用し、算出された糖鎖の存在量の比の数値と比較して、(3)で得られた数値が有意に高い場合、被験者が関節リウマチに罹患していると判定する方法。 - 前記(1)及び(2)の測定工程が、前記抗MMP-3抗体と、前記ポリラクトサミン構造を認識するレクチン及び前記O-結合型糖鎖認識レクチンを含むレクチンアレイを用いたサンドイッチアッセイにより同時に行われることを特徴とする請求項33に記載の方法。
- 下記の(1)及び(2)を含むことを特徴とする、リウマチ性多発筋痛症(PMR)及び/又は再発性多発軟骨炎(RP)診断用キット;
(1)抗MMP-3抗体
(2)ポリラクトサミン構造を認識するレクチン。 - ポリラクトサミン構造を認識するレクチンがLELレクチン又はSTLレクチンである請求項35に記載のキット。
- 下記の(1)~(3)を含むことを特徴とする関節リウマチ診断用キット;
(1)抗MMP-3抗体、
(2)ポリラクトサミン構造を認識するレクチン、
(3)Jacalin、ABA、ACA及びACGから選択された少なくとも一種のO-結合型糖鎖を認識するレクチン。 - (2)及び(3)のレクチンが同一のレクチンアレイ上に存在する請求項37に記載のキット。
