본 발명은 혈중암세포 진단을 위한 파라핀 블록을 제작함에 있어서 사용되는 혈중 세포 응집용 물질 및 이를 이용한 파라핀 블록의 제작방법에 관한 것으로, 보다 상세하게는 혈액 내에 미량으로 존재하는 혈중암세포(CTC)를 파라핀 블록으로 제작함에 있어서 혈액 시료의 채취를 통해 혈액 내 존재하는 혈중암세포를 검출하는 방법이며, 비침습적이고 간단하기 때문에 암의 재발 및 전이암의 진단 및 예후 예측에 매우 유용하며, 암 발병, 전이 및 재발 기전의 분석을 통해 암 치료의 획기적인 전기를 마련할 수 있다.
Description
본 발명은 파라핀블록 제조용 혈중 세포 응집제 및 이를 이용한 파라핀블록 제조방법에 관한 것으로, 보다 상세하게는 소량의 혈액으로부터 적혈구가 제거된 혈 내 존재하는 혈중 세포를 포함하는 파라핀블록을 제조함에 있어서 혈중 세포의 응집성을 향상시키고 동시에 혈중 세포의 손상 및 소실을 현저하게 방지할 수 있어 재현성이 높고, 민감도 및 특이도가 우수한 혈중암세포 진단을 위한 파라핀 블록을 구현할 수 있는 파라핀블록 제조용 혈중 세포 응집제 및 이를 이용한 파라핀블록 제조방법을 제공하는 것이다.
혈중암세포(Circulating tumor cell; 이하 CTC)는 혈액 중에 존재하여 체내를 순환하는 희소의 종양 세포들로, 암환자의 혈액 내 100만 개의 혈중 세포당 한 개가 존재한다고 알려져 있다. 극히 미량만 혈액 내 존재함에도 불구하고 전이성 유방암, 대장암 및 전립선암 환자에서 혈액 내 혈중암세포의 유무가 생존율과 연관된 중요한 예후 인자임이 밝혀지고 있으며, 최근에는 혈액 내 혈중암세포의 종양 특이적 표지자에 대한 모니터링에 의해 치료반응을 쉽게 예측할 수 있다는 보고들이 있다.
혈액을 통해 암을 진단하는 것을 종래에는 특정한 효소의 발현 증가 등을 통해 암발생 여부를 예측하였고, 대한민국 공개특허 특2003-0036010호는 혈액내 프로테인키나제의 효소활성 측정을 통한 암진단법을 개시하고 있다.
그러나, 아직까지 혈중에서 특정한 효소가 아닌 혈중암세포의 탐지는 혈중 종양 세포의 양, 즉 종양크기(tumor burden) 정도를 가늠하는 정량적 분석에 주로 국한되어 있는 실정이다. 만일 탐지된 혈중암세포를 수거하여 병리학적으로 분석할 수 있다면 이는 조직 검사가 어려운 말기 암환자들에게 조직 검사를 대체할 수 있는 진단법이 될 수 있으며, 불필요한 검사비를 줄이고 암의 조직학적 유형에 맞는 적절한 치료를 시행함으로써 치료 비용을 간접적으로 절감할 수 있어 이에 대한 연구가 계속되고 있다.
이러한 연구를 통해 현재까지 알려진 혈중암세포를 검출하기 위한 대표적인 방법으로는 단클론항체를 이용한 면역학적 분석법과 세포 크기에 의한 분리법이 있으며, 현재 대부분 개발 초기 단계로 일부에서 상용화가 된 상태이다. 그러나, 현재 사용되고 있는 방법은 일회성 검사법으로 혈중암세포의 단순 계수 혹은 혈중암세포에 대한 면역세포화학적 염색만 1 ~ 2회 정도 가능할 뿐이다. 또한 면역학적 분석법의 경우 사용하는 표면표지자가 EpCAM으로, 상피-간엽 이행을 보이는 혈중암세포 혹은 간세포암종이나 악성흑색종 등에서는 표지자가 암세포에 반응을 하지 않아 위음성을 보일 수 있는 단점이 있다. 나아가 세포 크기에 의한 분리법의 경우, 미세공(pore)이 있는 미세여과막을 이용하여 혈중암세포를 수거하는 것으로 아직까지 미세공의 크기가 다양하지 못하기 때문에 미세여과막내 미세공의 크기보다 작은 암세포는 걸러내지 못하며, 반대로 미세공의 크기가 백혈구 응집에 의하여 막힐 수 있다는 단점이 있어, 두 방법 모두 결정적인 문제점이 있다.
또한, 종래의 혈중암세포의 진단 방법은 화학적 처리를 통해 적혈구를 제거한 후, 또다시 미세여과막 또는 표면표지자에 의한 제거법으로 백혈구를 제거하여 혈중암세포만을 추출하는 방법을 사용하는데, 최종 혈중암세포를 추출하기까지 많은 단계를 거쳐야 하기 때문에 재현성이 떨어지는 문제점이 있다.
상기와 같은 문제점을 해결하기 위한 방법으로 혈중암세포를 포함하는 파라핀블록의 제조를 통해 혈중 암세포의 검출을 고려해 볼 수 있다.
통상적으로 파라핀블록은 조직이나 세포의 형태를 파악하고 이를 기초로 특수염색, 면역조직화학 염색 등 질병의 진단, 치료 및 연구목적에 이용되고 있다. 파라핀 블록을 이용한 진단의 적용은 일반적으로 생검조직을 통한 것이며, 생검조직을 고정하고, 고정이 끝난 조직은 파라핀 절편의 제작을 위하여 수세, 탈수, 투명, 파라핀 침투 과정을 거쳐 파라핀 블록을 만들게 되고 일정한 두께로 박절하여 건조 시키는 과정을 거친다.
상기와 같은 파라핀블록을 혈중 세포에 적용하기 위해서는 혈중 세포를 고정액에 고정하여 세포 군집 표본(cell block)을 만든 다음, 파라핀 절편으로 제작하는 과정을 거쳐야 한다. 이때, 통상적으로 생검조직에 대하여 세포 군집 표본을 제조하는 방법으로써 종래에는, a) 원심분리 및 포르말린을 이용한 침전 방법을 사용하는 침전물 고정법; b) 에탄올 속에서 에그 알부민이 응고되는 원리를 이용하는 에그 알부민 사용법; c) 아가(agar) 사용법; d) 혈장-트롬빈 응고법; e) 콜로디온 백 (collodion bag) 사용법; 및 f) 밀포어 필터 (Millpore filter) 방법 등이 사용되어 왔다.
그러나, 상기 세포 군집 표본을 제조하기 위한 종래의 방법 중 아가젤을 이용할 경우 검체를 지지시키는 아가젤이 수축하는 문제가 있고, 이를 통해 파라핀블록 제작 소요시간 및 소요비용이 현저히 증가하여 만족할 만한 성과를 얻지 못하는 단점이 있다.
또한, 상기 방법 중 원심분리를 통해 검체를 침전시키는 침전물 고정법을 혈중 세포에 대해 사용할 경우 혈중 세포의 파괴 및 소실로 인해 세포 군집 표본의 양이 적어지거나 조직이나 세포가 산재되고, 파라핀 침투 후에 세포 군집 표본과 파라핀과의 구분이 어려우며, 포매 시 핀셋으로 포매(embeding) 하기가 불가능하다는 문제점이 있다.
또한, 상기 방법들을 통해 혈중 세포를 세포 군집표본으로 만들어 제조된 파라핀블록의 절편에 염색을 시행할 경우, 배경 염색으로 인한 슬라이드 판독의 어려움이 있을 뿐 아니라, 세포의 파괴 및 소실로 인해 타겟팅한 특정 혈중 세포의 검출을 위한 민감도 및 특이도가 현저히 떨어지는 문제점이 있다.
나아가, 파라핀블록을 이용한 진단방법의 대상은 통상의 생검 조직 또는 체세포들로써, 이러한 생검 조직이나 체세포들은 세포간 접합성이 좋고, 세포들을 응집시키는 세포외 기질성 성분을 포함하고 있음에 따라 세포 군집 표본을 제조하기에 매우 용이하였다. 그러나 혈중 세포는 세포간 접합성이 없고, 세포들을 응집시킬 수 있는 세포외 기질의 기능을 수행하는 물질을 포함하고 있지 않음에 따라 각각의 세포가 응집되지 않고 분리되어 있는 상태이다. 이러한 상태의 혈중세포 시료는 상술한 여러 가지 세포군집 표본을 제조하는 방법을 수행해도 혈중 세포들이 응집되지 않기 때문에 혈중 세포를 대상으로 파라핀블록을 제조시, 제조 과정 중 혈중 세포의 유실이나 분실이 빈번하게 일어나, 목적하는 파라핀블록을 제조하기 매우 어려운 문제점이 있다.
더 나아가, 혈중 세포가 제대로 응집이 되지 않은 채, 포르말린 고정이나 파라핀블록 제조 과정에 처리될 경우, 혈중 세포 그 자체가 외부로 노출되어 보호되지 않음에 따라 파라핀블록을 제조하는 여러 중간 처리 과정 중 혈중 세포가 파괴 및 소실 또는 분실이 더 빈번하게 일어날 수 있다. 특히, 혈중 암세포는 혈액내 포함된다고 하더라도 극히 적은 개수로 포함되기 때문에 파괴 및 소실 또는 분실되는 혈중 세포가 혈중 암세포일 경우 제조된 파라핀블록을 통해 혈중 암세포를 진단할 수 없는 치명적인 문제점이 있다.
본 발명은 상술한 문제를 해결하기 위해 안출된 것으로, 본 발명의 첫 번째 해결하려는 과제는 파라핀 블록의 제조를 보다 용이하게 하고, 제조과정에서 목적한 특정의 혈중 세포의 파괴, 손상, 소실, 분실 등을 방지하여 목적한 혈중 세포의 진단 신뢰성을 현저히 향상시킬 수 있는 혈중 세포 응집제를 제공하는 것이다.
본 발명의 두 번째 해결하려는 과제는 혈중암세포 진단에 있어 재현성, 민감도 및 특이도가 현저히 우수한 혈중암세포 진단용 키트를 제공하는 것이다.
본 발명의 세 번째 해결하려는 과제는 재현성이 높고, 민감도 및 특이도가 현저히 우수한 혈중 세포 응집제를 이용한 파라핀 블록의 제조방법을 제공하는 것이다.
본 발명은 상술한 첫 번째 과제를 달성하기 위하여, 혈중 세포에 대하여 혈중 세포들을 응집시키고, 혈중 세포의 손상 및 소실을 방지하는 세포외기질의 기능을 수행하는 혈중 세포 응집 물질;을 포함하는 파라핀블록 제조용 혈중 세포 응집제를 제공한다.
본 발명의 바람직한 일실시예에 따르면, 상기 혈중 세포 응집물질은 젤라틴을 포함할 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 상기 젤라틴의 농도는 5 내지 35㎎/㎖일 수 있다.
본 발명의 바람직한 또 다른 일실시예에 따르면, 상기 혈중 세포 응집물질은 펙틴(pectin), 콘드로이틴설페이트(chondroitin sulfate), 파이브로넥틴(fibronectin), 전분(starch) 및 라미닌(laminin)으로 이루어진 군에서 선택된 어느 하나 이상의 물질을 더 포함할 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 혈중 세포 응집물질로 젤라틴 이외에 더 포함되는 물질은 젤라틴 중량의 5%이내의 중량일 수 있다.
본 발명의 바람직한 또 다른 일실시예에 따르면, 상기 혈중 세포 응집제는 혈중 세포 응집물질을 발색시키기 위한 발색제를 더 포함하며, 상기 발색제는 혈중 세포 응집물질 100 부피부에 대하여 5 내지 10 부피부로 포함될 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 상기 젤라틴의 농도는 10 내지 30㎎/㎖일 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 상기 파라핀블록 제조용 혈중 암세포 응집제는 혈중 암세포 진단을 위한 용도일 수 있다.
또한, 본 발명은 상술한 두 번째 과제를 달성하기 위하여, 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제를 포함하는 혈중암세포 진단용 키트를 제공한다.
또한, 본 발명은 상술한 세 번째 과제를 달성하기 위하여, (1) 피검체의 혈액에서 적혈구를 제거하고 혈중 세포를 추출하는 단계; (2) 상기 혈중 세포를 본 발명에 따른 혈중 세포 응집제와 혼합하여 혈중 세포 응집체를 제조하는 단계; (3) 상기 혈중 세포 응집체를 경화시켜 단계; 및 (4) 상기 (3) 단계에 따른 경화체에 파라핀블록 형성물질을 처리하는 단계;를 포함하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법을 제공한다.
본 발명의 바람직한 일실시예에 따르면, 상기 (1) 단계에 사용되는 혈액세포가 추출되는 용기(tube)는 젤라틴용액이 코팅될 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 상기 (1) 단계의 적혈구 제거는 피콜(ficoll)을 이용할 수 있다.
본 발명의 바람직한 또 다른 일실시예에 따르면, 상기 (2) 단계는 혈중 세포와 혈중 세포 응집제를 1: 1 ~ 1.5 부피비로 혼합시킬 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 상기 (2) 단계의 혈중 세포 응집제는 젤라틴을 포함하고, 상기 (2) 단계가 수행되는 온도에 따라 혈중 세포 응집제에서의 젤라틴 농도는 하기 조건 (a) 내지 (c) 중 어느 하나를 만족할 수 있다.
(a) (2) 단계 수행온도가 5℃ 이상 15℃ 미만일 때 혈중 세포 응집제에서 젤라틴의 농도는 5 ~ 15 ㎎/㎖임, (b) (2) 단계 수행온도가 15℃ 이상 25℃ 미만일 때 혈중 세포 응집제에서 젤라틴의 농도는 15 ~ 25 ㎎/㎖임, (c) (2) 단계 수행온도가 25℃ 초과 35℃ 미만일 때 혈중 세포 응집제에서 젤라틴의 농도는 25 ~ 35㎎/㎖임.
본 발명의 바람직한 또 다른 일실시예에 따르면, 상기 (4) 단계의 파라핀블록 형성물질은 알코올, 자일렌 및 파라핀을 포함하고, 상기 (4) 단계는 알코올, 자일렌 및 파라핀의 순으로 파라핀블록 형성물질을 처리하여 자동침투과정을 통해 수행될 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 상기 파라핀블록은 혈중암세포(circulating tumor cell)의 조직학적, 면역조직화학적 또는 분자생물학적 검출방법에 이용될 수 있다.
본 발명의 바람직한 또 다른 일실시예에 따르면, 상기 (3) 단계 이후에 (3) 단계에 따른 경화체를 고정(fixation)하는 단계를 더 포함할 수 있다.
본 발명의 바람직한 다른 일실시예에 따르면, 상기 경화체의 고정(fixation)은 포르말린, 메탄올 또는 에탄올로 구성된 군으로부터 선택되는 1종 이상의 것으로 수행될 수 있다.
이하, 본 발명에서 사용된 용어에 대해 정의한다.
본 발명에서 사용한 용어인 “혈중 세포”는 혈액, 바람직하게는 말초혈액에 포함된 세포를 의미하며, 상기 혈중 세포는 유핵 또는 무핵의 세포를 모두 포함하고, 상기 혈중 세포에는 통상적인 혈액세포(blood cell)외에 정상세포 및/또는 암세포를 모두 포함하는 의미이며, 상기 혈액세포는 매크로파지, 림프구(lymphocyte), 단구(monocyte) 등의 말초혈단핵구(peripheral blood mononuclear cell, PBMC), 적혈구, 백혈구 및 혈소판 등의 통상적으로 혈액세포로 분류되는 세포를 모두 포함하는 의미이다.
본 발명은 혈중 세포가 포함된 파라핀 블록을 제작함에 있어서, 종래의 통상적인 세포군집 표본을 제조하는 방법으로는 혈중 세포를 고정화시키기 어려운 난점 및 종래의 방법으로 고정화된 혈중 세포 군집 표본이 파라핀블록의 제조과정에서 손상, 분실 및/또는 소실되는 치명적인 문제점을 해결할 수 있다. 이를 통해 혈중 세포 중에 목적하는 특정 세포의 손상, 분실 및/또는 소실 없이 파라핀블록에 포함시킬 수 있어 이러한 파라핀블록을 통해 목적하는 특정 세포, 예를 들어 혈중 암세포를 조직학적, 면역조직화학적, 분자생물학적 등 다양한 방법을 통해 검출할 수 있다.
또한, 혈중 세포에 대한 여러 번의 원심분리 과정을 거치지 않음으로써 파라핀블록의 제조공정을 단순화할 수 있고, 제조시간 단축 및 제조비용을 절감할 수 있다.
또한, 종래의 혈중 암세포의 검출방법과 같이 혈액에서 적혈구 제거 후, 또다시 미세여과막 또는 표면표지자에 의한 백혈구 제거법과 같은 물리적 또는 화학적 처리 과정을 생략할 수 있어 목적한 혈중 암세포의 손실을 줄이고 세포 보존율을 높일 수 있다.
나아가, 본 발명을 통해 제조된 혈중 세포를 포함하는 파라핀블록은 배경염색을 감소시키고, 얇은 절편을 얻을 수 있어서 해상도가 뛰어나다. 또한, 혈중 암세포의 형태학적 확인이 가능해 오염된 상피세포와의 구별을 가능하게 하게 하고 이를 통해 위양성 판정의 오류를 줄일 수 있어 민감도 및 특이도가 매우 우수하다. 나아가, 단위면적당 포함된 백혈구와 혈중암세포의 계수로 혈액 단위부피당 혈중암세포의 계수가 가능하다. 더 나아가 검체시료가 파라핀블록으로 제조됨으로써 영구 보관이 가능하며, 30회 이상의 다양한 면역조직화학적 검사가 가능하다. 더 나아가 소량의 혈액 채취로도 파라핀블록의 제조가 가능하기 때문에 시간차를 두고 검체로부터 채취된 혈액을 파라핀 블록으로 제조 및 검사함으로써 암의 재발 및 전이암 진단 및 예후 측정이 가능하다.

도 6은 본 발명의 바람직한 일실시예에 따라 유방암세포주(SKBR3) 유래 혈중암세포로부터 제작된 파라핀 절편에 대해서 H&E 염색, c-erbB2 면역조직화학염색 및 Her2/neu 유전자에 대한 형광동소교잡반응(FISH)을 수행한 다음, 현미경을 사용하여 1000배 배율로 관찰한 사진이다.
도 7은 본 발명의 바람직한 일실시예에 따라 간암세포주(HepG2) 유래 혈중암세포로부터 제작된 파라핀 절편에 대해서 H&E 염색 및 CK 면역조직화학염색을 수행한 다음, 암 특이표지자인 EpCAM 및 간암 특이표지자인 글리피칸-3(Glypican-3) 및 알파-펙토단백질(alpha-fetoprotein, AFP)에 대한 유전자 발현을 현미경을 사용하여 400배 배율로 관찰한 사진이다.
도 8은 본 발명의 바람직한 일실시예에 따른 대장암세포주(SW620)유래 혈중암세포로부터 제작된 파라핀 절편에 대해서 CK 면역조직화학염색을 수행한 다음, 암 특이표지자인 EpCAM 및 대장암 특이표지자인 CDX2 및 p53에 대한 유전자 발현을 현미경을 사용하여 400배 배율로 관찰한 사진이다.

이하, 본 발명을 보다 상세히 설명한다.
상술한 바와 같이 파라핀블록은 조직이나 세포의 형태를 파악하고 이를 기초로 특수염색, 면역조직화학 염색 등 질병의 진단, 치료 및 연구목적에 이용되고 있지만, 종래의 방법으로는 혈중 세포를 대상으로 하여 파라핀블록을 제조하기 매우 어려운 문제점이 있었다. 또한, 종래의 방법들을 통해 혈중 세포를 세포 군집표본으로 만들어 파라핀블록을 제조했다고 하더라도 파라핀블록 절편을 염색시킬 경우 배경 염색이 과도하여 타겟팅한 특정 혈중 세포의 검출을 위한 민감도 및 특이도가 현저히 떨어지는 문제점이 있었다. 나아가 혈중 세포는 통상의 파라핀블록 대상이 되는 생검조직과 다르게 세포막 표면에 세포간 응집 등의 세포외 기질의 기능을 수행하는 물질을 포함하고 있지 않음에 따라 각각의 세포가 응집되지 않고 분리됨에 따라 혈중 세포를 대상으로 파라핀블록을 제조시 혈중 세포의 유실이나 분실이 빈번하여 목적하는 파라핀블록을 제조하기가 매우 어려운 문제점이 있었다. 더 나아가, 혈중 세포가 포르말린 고정이나 파라핀블록 제조 과정을 거칠 경우 혈중 세포 그 자체가 외부로 노출되어 보호되지 않음에 따라 파라핀블록을 제조하는 여러 중간 처리 과정에서 혈중 세포가 파괴, 소실 및/또는 분실이 빈번하게 발생하였다. 특히, 혈중 암세포는 혈액내 포함된다고 하더라도 극히 적은 개수로 포함되기 때문에 파괴, 소실 및/또는 분실되는 혈중 세포가 혈중 암세포일 경우 제조된 파라핀블록을 통해 혈중 암세포를 진단할 수 없는 치명적인 문제점이 있었다. 더 나아가 종래의 혈중암세포의 진단 방법은 화학적 처리를 통해 적혈구를 제거한 후, 다시 미세여과막 또는 표면표지자에 의한 백혈구 제거법을 거쳐 혈중암세포만을 추출함에 따라 혈중암세포 추출까지 많은 단계를 거쳐야 하기 때문에 재현성이 떨어지는 문제점이 있었다.
이에 본 발명에서는 혈중 세포에 대하여 혈중 세포들을 응집시키고, 혈중 세포의 손상 및 소실을 방지하는 세포외기질의 기능을 수행하는 혈중 세포 응집 물질;을 포함하는 파라핀블록 제조용 혈중 세포 응집제를 통해 상술한 문제의 해결을 도모하였다. 이를 통해 혈중 세포가 포함된 파라핀 블록을 제작함에 있어서, 종래의 통상적인 생체조직을 대상으로한 세포군집 표본 제조방법으로는 혈중 세포를 세포군집 표본으로 제조하기 어려운 난점 및 종래의 생체조직의 표본화 방법으로 제조된 혈중 세포 군집 표본이 파라핀블록의 제조과정에서 손상, 분실 및/또는 소실되는 치명적인 문제점을 해결할 수 있다. 이를 통해 혈중 세포 중에 목적하는 특정 세포의 손상, 분실 및/또는 소실 없이 파라핀블록에 포함시킬 수 있어 이러한 파라핀블록을 통해 목적하는 특정 세포, 예를 들어 혈중 암세포를 조직학적, 면역조직화학적, 분자생물학적 등 다양한 방법을 통해 검출할 수 있다. 또한, 혈중 세포에 대한 여러 번의 원심분리 과정을 거치지 않음으로써 파라핀블록의 제조공정을 단순화할 수 있고, 제조시간 단축 및 제조비용을 절감할 수 있다. 나아가, 종래의 혈중 암세포의 검출방법과 같이 다단계의 혈구세포 제거를 위한 물리적 또는 화학적 처리 과정을 생략할 수 있어 목적한 혈중 암세포의 손실을 줄이고 세포 보존율을 높일 수 있다.
먼저, 혈중 세포에 대하여 혈중 세포들을 응집시키고, 혈중 세포의 손상 및 소실을 방지하는 세포외기질의 기능을 수행하는 혈중 세포 응집 물질에 대해 설명한다.
구체적으로 상기 혈중 세포는 혈액, 바람직하게는 말초혈액에 포함된 세포를 의미하며, 상기 혈중 세포는 유핵 또는 무핵의 세포를 모두 포함할 수 있고, 상기 혈중 세포에는 통상적인 혈액세포(blood cell)와 정상세포 및/또는 암세포를 모두 포함할 수 있으며, 상기 혈액세포는 매크로파지, 림프구(lymphocyte), 단구(monocyte) 등의 말초혈단핵구(peripheral blood mononuclear cell, PBMC), 적혈구, 백혈구 및 혈소판 등 통상적으로 혈액세포로 분류되는 세포를 모두 포함할 수 있다.
바람직하게는 상기 혈중 세포 응집물질은 적혈구를 제거한 혈중 세포들에 대해서 세포외 기질의 기능을 수행할 수 있다. 본 발명에 따른 혈중 세포 응집물질은 혈중 세포의 구체적 종류에 따라 효과발현의 정도가 달라지지 않을 수 있다. 다만, 혈중 세포 중에서 적혈구가 아닌 다른 특정 혈중 세포, 예를 들어 혈중 암세포를 진단하기 위한 파라핀블록 제조를 목적으로 할 경우 본 발명에 따른 혈중 세포 응집물질은 혈중 세포 중에서 적혈구를 제거한 세포들에 대해서 세포외 기질의 기능을 수행할 수 있다. 통상적으로 혈액내에 포함된 혈중세포 중에서 적혈구는 세포수에 있어 높은 비율을 차지하기 때문에 적혈구를 포함하여 파라핀블록으로 제조시 파라핀블록 절편의 단위면적당 포함되는 혈중세포 중 목적한 특정 종류의 혈중 세포가 포함될 확률은 매우 낮아질 수 있다. 또한, 파라핀블록의 제조과정에서 적혈구가 손상, 파괴 등이 될 경우 목적한 특정 종류 혈중 세포의 세포막, DNA, RNA 및 세포질 등에 영향을 줄 수 있는 각종 효소(enzyme)들이 방출될 수 있어 목적한 특정 혈중 세포의 진단이 매우 불리하거나 불가능할 수 있다. 이에 따라 혈중 암세포 진단을 위한 파라핀블록을 제조하기 위해서 상기 혈중 세포 응집물질은 적혈구를 제외한 혈중 세포들에 대해 세포외기질의 기능을 수행함이 바람직하다.
한편, 본 발명에 따른 혈중 세포 응집물질은 혈중 세포들을 응집시키고, 혈중 세포의 손상 및 소실을 방지하는 세포외기질의 기능을 수행하는데, 이하 혈중 세포 응집물질의 기능에 대해 구체적으로 살펴본다.
통상적으로 세포외기질(extracellular matrix)은 조직내 또는 세포외의 공간을 채우고 있는 생체고분자의 집합체를 의미하며, 모든 조직에 존재하나 결합조직에 특히 다량으로 존재한다. 상기 세포외기질은 그 기능에 있어 조직의 지지와 결합, 물리적 경계, 힘의 흡수, 탄성 등의 물리적 기능에 나아가 최근에는 세포증식, 세포이동, 세포내 대사, 세포분화, 세포의 형태 등을 세포의 외부에서 조절하는 생리화학적 역할까지 담당하는 것으로 연구를 통해 알려져 있다.
본 발명에 따른 혈중세포 응집제에 포함되는 혈중 세포 응집물질 역시 혈중세포에 대하여 상기와 같은 세포외기질의 기능을 수행하는데 다만, 통상적으로 세포외 기질의 기능으로 알려진 세포증식, 세포이동, 세포내 대사 등에 관여하는 생리화학적 역할이 아닌 혈중 세포의 응집, 혈중세포 외부에서 가해지는 물리적 요소의 방어, 흡수, 탄성요소 등의 물리적 기능을 담당하고, 이를 통해 혈중 세포 응집물질은 혈중세포의 손상 및 소실을 방지하는 기능을 수행할 수 있다.
보다 구체적으로 상기 혈중 세포의 응집이란 혈중 세포 응집물질(또는 이를 포함하는 혈중 세포 응집제)를 매개로 하여 개개로 분리된 상태의 혈중 세포가 물리적으로 응집된 형태를 구현하는 것을 의미하며, 혈중세포 응집물질이 리간드가 되어 혈중세포의 세포막 표면의 수용체와 결합에 따른 응집이나 혈중 세포 응집물질의 화학적 변화에 따라 혈중세포가 응집되는 것을 의미하지 않을 수 있다. 이에 따라 본 발명의 바람직한 일실시예에 따르면, 혈중 세포 응집물질은 혈장성분, 보다 바람직하게는 혈장단백질, 예를 들어 피브리노겐, 프로트롬빈 또는 이들의 활성화된 상태의 트롬빈이나 피브린 등을 포함하지 않을 수 있고, 이에 따라 혈중 세포 응집물질이 혈중세포를 응집시키는 것은 통상적인 혈액응고 기작에 의한 응집이 아닐 수 있다. 혈액응고 기작에 의해 혈중 세포가 응집될 경우 응집체내 적혈구가 과다하게 포함되어 파라핀블록으로 제작해도 단위 면적당 포함된 적혈구 외의 혈중 세포의 양이 너무 적어 진단적 효용성이 매우 낮으며, 적혈구 용혈 현상에 따른 세포내 효소 방출로 인하여 혈중세포의 DNA및 RNA에 손상을 주어 제조된 혈중세포 응집체의 진단적 가치가 매우 떨어지고 혈장단백질로는 발명의 목적을 달성할 수 없는 문제점들 있다.
본 발명에 따른 혈중 세포 응집제에 포함되는 혈중 세포 응집물질은 상술한 기능을 발현할 수 있음에 따라 혈중세포를 파라핀블록으로 제조함에 있어 매우 유리할 수 있다. 이는 파라핀블록의 대상은 세포군집 표본으로 제조된 후 상기 표본에 파라핀이 처리되는데, 통상적인 파라핀블록의 제조 대상은 생체조직으로서, 통상적인 생체조직은 세포간 접합성이 있는 세포들이 모여 군집을 이루고, 상기 군집을 이룬 각각의 세포들은 세포 외부에 세포외기질을 포함하여 세포들이 보다 더 응집되고 세포들 자체가 물리적으로 보호될 수 있다. 그러나, 혈중 세포는 생체조직과 다르게 각각의 세포가 독립적으로 혈액내를 부유하며, 세포간 접합성이 없을 뿐 아니라, 세포막 바깥에 세포외기질을 수행하는 물질들을 포함하고 있지 않다. 이러한 특성을 갖는 혈중세포들을 대상으로 세포군집 표본을 제조할 경우 혈중 세포가 응집되지 않아 세포군집 표본 자체를 제조하기 어렵고, 세포들이 응집되지 않은 상태로 파라핀블록을 제조하기 위한 여러 단계를 거칠 경우 목적한 특정 종류의 혈중 세포가 분실될 수 있다. 또한, 혈중 세포가 보호되지 않아 파라핀블록을 제조하기 위한 여러 단계를 거치면서 목적한 특정종류의 혈중 세포가 손상될 수 있고, 60℃ 이상으로 가온 시켜 녹인 파라핀을 처리하는 과정에서 목적한 특정 종류의 혈중 세포가 소실될 수 있는 문제점이 있다. 본 발명에 따른 혈중 세포 응집제에 포함되는 혈중 세포 응집물질은 상기 문제점을 해결할 수 있도록 혈중세포에 대해 세포외기질의 기능을 수행할 수 있음에 따라 혈중세포를 응집시키고, 파라핀블록 제조과정에서 혈중세포의 손상이나 소실을 방지할 수 있다.
본 발명의 바람직한 일실시예 따르면, 상술한 혈중 세포 응집물질은 젤라틴을 포함할 수 있다.
구체적으로 젤라틴은 식품이나 공업용으로 다양한 용도로 사용되며, 냉수에는 팽윤할 뿐이지만 온수에는 녹아 졸(sol)로 되고, 2 ~ 3% 이상의 농도에서는 실온 이하로 식히면 겔(gel)이 되는 성질을 가지고 있어 열에 안정적인 분자이므로, 본 발명의 혈중 세포 응집물질로서 적합할 수 있다. 또한, 다른 종류의 세포외 기질을 수행할 수 있는 물질보다 단독으로 혈중 세포를 응집시킬 때, 혈중 세포의 손상 및 소실을 방지하는 효과가 더 우수하여 본 발명이 목적하는 물성이 우수한 파라핀블럭을 제조하는데 있어 보다 유리하다. 본 발명에 따른 혈중 세포 응집물질로 젤라틴은 콜라겐을 포함하는 의미일 수 있다. 상기 젤라틴은 바람직하게는 용매와 혼합된 젤라틴 용액의 상태로 포함될 수 있다. 상기 용매는 인산완충 용액, 증류수, 생리식염수 등을 포함할 수 있으며, 본 발명의 바람직한 일실시예에 따르면 증류수일 수 있다.
또한, 혈중 세포 응집물질로 포함되는 젤라틴은 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제에 바람직하게는 5 내지 35㎎/㎖ 의 농도로 포함될 수 있고, 보다 바람직하게는 5 ~ 30㎎/㎖ 의 농도로, 보다 더 바람직하게는 5 내지 20㎎/㎖ 의 농도로 포함될 수 있다. 만일 젤라틴의 농도가 5㎎/㎖ 미만일 경우 혈중 세포를 대상으로 파라핀블록을 제조했을 때 젤라틴 응고에 시간이 걸려 세포에 손상을 주거나, 젤라틴이 녹아 혈중세포 진단의 민감도 및 특이도가 현저히 저하될 수 있다. 또한, 만일 젤라틴의 농도가 35㎎/㎖를 초과할 경우 파라핀블록의 경도가 증가하여 파라핀블록의 박절 과정에서 파라핀 절편이 튕겨나가거나 부스러져 혈중세포의 손실을 유발하여 혈중 세포 진단의 민감도 및 특이도가 현저히 저하될 수 있다. 구체적으로 젤라틴 농도 변화에 따른 혈중암세포 수거율을 비교한 하기 표 5를 통해 확인할 수 있듯이, 젤라틴의 농도가 10㎎/㎖일 때 혈중 암세포 수거율이 93%인데 반하여 젤라틴의 농도가 3㎎/㎖인 경우 혈중암세포 수거율이 68%로 현저히 낮은 것을 확인할 수 있으며, 젤라틴의 농도가 50㎎/㎖인 경우 혈중암세포 수거율이 6%에 불과함을 확인할 수 있다.
한편, 상기 혈중 세포 응집물질은 펙틴(pectin), 콘드로이틴설페이트(chondroitin sulfate), 파이브로넥틴(fibronectin), 전분(starch) 및 라미닌(laminin)으로 이루어진 군에서 선택된 어느 하나 이상의 물질을 더 포함할 수 있다. 상기와 같은 물질을 더 포함함을 통해 파라핀블록으로 제조했을 때 혈중세포에 대한 민감도 및 특이도를 향상시킬 수 있다. 특히 젤라틴이 혈중 세포 응집물질로 포함되는 경우 젤라틴은 그 농도에 따라 온도별 성상이 달라질 수 있는데, 특정 온도범위에서 고상 또는 반고상을 갖게 하기 위해 젤라틴의 농도를 높일 경우 목적한 성상은 구현되나 반대로 파라핀블록의 혈중세포에 대한 민감도 및 특이도는 현저히 저하될 수 있는 문제점이 있다. 예를 들어 본 발명에 따른 바람직한 일실시예에 따르면, 젤라틴의 농도가 20㎎/㎖인 경우 실내온도가 28℃인 실내에 10분 이상 거치하면 성상이 액상으로 변하는데(하기 표 6 참조) 실내온도가 28℃에서 성상을 고상 또는 반고상으로 유지시키기 위해 젤라틴의 농도를 20㎎/㎖ 보다 높게 증가시킬 경우 목적하는 대로 고상 또는 반고상의 성상을 유지할 수 있지만, 반대로 목적하는 파라핀블록의 혈중 암세포에 대한 민감도 및 특이도를 구현할 수 없을 수 있다. 구체적으로 하기 표 5에서 젤라틴 농도가 30㎎/㎖일 경우 파라핀블록의 혈중 암세포 수거율은 80%이나 젤라틴 농도가 50㎎/㎖일 경우 파라핀블록의 혈중 암세포 수거율은 6%까지 현저히 저하되어 일정 농도 이상에서 젤라틴의 농도가 증가할수록 혈중 암세포 수거율이 감소하여 혈중 암세포에 대한 파라핀블록의 민감도 및 특이도가 현저히 저하되는 것을 확인할 수 있다.
따라서, 특정 실내온도에서 목적하는 성상을 유지하기 위해 증가시킨 젤라틴 농도에 따라 감소된 혈중세포에 대한 민감도 및 특이도를 보전하기 위해 펙틴(pectin), 콘드로이틴설페이트(chondroitin sulfate), 파이브로넥틴(fibronectin), 전분(starch) 및 라미닌(laminin)으로 이루어진 군에서 선택된 어느 하나 이상의 물질을 혈중 세포 응집물질에 더 포함할 수 있고, 이 경우 특정온도에서 목적한 성상을 유지하는 동시에 목적하는 혈중세포에 대한 민감도 및 특이도를 발현시킴에 있어 보다 유리할 수 있다.
바람직하게는 혈중 세포 응집물질로 젤라틴 이외에 더 포함되는 상기 물질은 젤라틴 중량의 5%이내로 포함될 수 있다. 만일 젤라틴 이외에 혈중 세포 응집물질로 더 포함되는 물질이 젤라틴 중량의 5%를 초과하여 포함될 경우 파라핀블록의 경도가 증가하여 파라핀블록의 박절 과정에서 파라핀블록이 깨지거나 부스러져 혈중세포의 손실을 유발하는 등 혈중 세포 진단의 민감도 및 특이도가 떨어지는 문제점이 있을 수 있다.
한편, 혈중 세포 응집물질에 젤라틴을 포함하지 않고, 상기 젤라틴 이외에 혈중세포 응집물질로 추가되는 물질만을 포함할 경우 목적하는 물성을 구현하기 어려울 수 있다. 구체적으로 상기 전분(starch)은 사용이 간편하지만 분리되기 쉽고 농도조절을 한 후 식으면 다시 열을 가해도 처음과 같은 점성 또는 품질이 나오지 않는 단점이 있으며, 또한 찬물에는 녹지 않으나, 더운물에는 부풀어 호화(糊化)하는 물적 특성이 있어 파라핀 블록의 제조에 사용하는데 있어서 문제점이 있고, 혈중 세포의 응집을 향상시키기 위해 전분의 농도를 고농도로 높일 경우 응집력은 향상되지만 경도가 지나치게 높게 되어 파라핀블록의 박절 과정에서 파라핀블록이 깨지거나 으스러질 수 있고, 이때 혈중 세포의 손상, 소실이 있을 수 있어 혈중세포 응집물질로 단독 사용이 제한적일 수 있다.
또한, 상기 펙틴(pectin)은 탄수화물의 중합체로 식품에 응고제, 증점제, 안정제, 고화방지제, 유화제 등으로 사용되는데, 펙틴의 증점력과 겔화력은 산과 에스터의 균형에 따라 다르며, 산이 50%를 넘지 않거나 또는 비에스터화된 펙틴의 경우 칼슘이온으로 겔을 형성하려는 경향이 강하여, 혈중세포 응집물질로 단독 사용이 제한적일 수 있다.
또한, 상기 파이브로넥틴(fibronectin) 및 라미닌(laminin) 또한 열에 안정적이지 않아 혈중세포 응집물질로 단독 사용이 제한적일 수 있다.
본 발명의 혈중 세포 응집물질로 젤라틴이 포함될 경우 젤라틴은 무색 투명하므로, 용기 내에서 응집화(jelly) 여부를 유관으로 확인하기 어렵기 때문에, 본 발명의 혈중 세포 응집용 물질에는 세포외기질을 발색하기 위한 발색제가 더 포함될 수 있다.
상기 발색제는 브로모페놀 블루(Bromophenol blue), 카본 블랙(carbon black), N,N-디메틸 라미노에탄올(N,N-dimethy laminoethanol), 구리 프탈로시아닌(copper phthaloc yanine), 펜스퍼스 오렌지(pennsperse orange) 등을 포함할 수 있으며, 본 발명의 일실시예에 따른 혈중 세포 응집물질을 발색하기 위한 발색제는 바람직하게는 브로모페놀 블루(Bromophenol blue)가 포함된 로딩 용액(loading buffer)(Takara Bio Inc)일 수 있으며, 상기 발색제는 혈중 세포 응집물질 100 부피부에 대하여 5 내지 10 부피부로 파라핀블록 제조용 혈중세포 응집제에 포함될 수 있다. 만일 혈중 세포 응집물질 100 부피부에 대하여 발색제가 10 부피부를 초과하여 첨가될 경우 검체 응집력이 오히려 약화되거나 검체 응집이 지연된다는 문제점이 있고, 발색제가 5부피부 미만으로 첨가될 경우 포르말린 고정에 따라 발색의 효과가 사라져 검체의 포매가 어려워지는 문제점이 있다.
한편, 본 발명의 다른 구현예에 따르면, 상술한 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제를 포함하는 혈중 암세포 진단용 키트를 구현할 수 있다.
본 발명의 혈중 암세포 진단용 키트를 이용하여 진단할 수 있는 암의 종류에 있어서 제한은 없으나, 암환자의 혈액 내 100만개의 혈중 세포당 한 개가 존재한다고 알려져 있고, 혈액 중에 존재하여 체내를 순환하는 희소의 종양 세포인 혈중암세포(Circulating tumor cell: CTC)를 확인함으로써 진단 가능한 암을 포함할 수 있고, 혈액 중에 극히 미량만 존재함에도 불구하고 혈액 내 혈중암세포의 유무가 생존율과 연관된 중요한 예후 인자임이 밝혀진 전이성 유방암, 대장암, 간암 또는 전립선암을 진단하기 위한 키트일 수 있다.
본 발명의 혈중 암세포 진단용 키트는 파라핀블록 제조용 혈중세포 응집제에 포함된 혈중세포 응집물질 100 부피부에 대하여 혈중세포 응집물질을 발색하기 위한 발색제를 5 내지 10의 부피부로 더 포함할 수 있다. 예를 들어, 혈중 세포가 40㎕일 경우, 혈중 세포에 젤라틴 용액 40㎕, 발색제 2㎕을 넣어 혈중 세포와 젤라틴 용액이 젤리형태로의 응집된 것을 확인할 수 있다. 상기 발색제 함량에 대한 임계적의의는 상술한 바와 동일하여 설명을 생략한다.
한편, 상술한 파라핀블록 제조용 혈중 세포 응집제를 통해 본 발명에 따른 파라핀블록을 제조하는 방법은 (1) 피검체의 혈액에서 적혈구를 제거하고 혈중 세포를 추출하는 단계; (2) 상기 혈중 세포를 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제와 혼합하여 혈중 세포 응집체를 제조하는 단계; (3) 상기 혈중 세포 응집체를 경화시키는 단계; 및 (4) 상기 (3) 단계에 따른 경화체에 파라핀블록 형성물질을 처리하는 단계;를 포함한다. 이를 통해 소량의 혈액으로부터 얻은 혈중 세포들(cell pellet)을 세포의 손상, 소실 등 없이 파라핀블록으로 제조할 수 있고, 이렇게 제조된 파라핀블록은 피검체로부터 오직 5 ~ 6㎖ 정도의 한 번의 채혈만으로 50 ~ 60회까지 목적하는 특정 종류의 혈중 세포에 대한 분석 실험을 진행할 수 있고, 이러한 점은 병리학적으로는 물론 임상적으로도 적용 범위가 매우 넓고, 유용한 검사법에 사용 될 수 있다. 구체적으로, 혈중암세포의 선별, 회수 및 생물학적 분석의 새로운 신기술을 확보함으로써, 종합적인 암 진단 도구와 방법의 개발을 통한 암 진단의 간소화, 저렴화 및 고신뢰도를 획득할 수 있다.
먼저, (1)단계로써 피검체의 혈액에서 적혈구를 제거하고 혈중 세포를 추출하는 단계를 포함한다.
상기 적혈구는 혈액세포 중에서 수적으로 가장 큰 비중을 차지하는 세포로, 적혈구를 포함하여 파라핀블록으로 제조시 파라핀블록 절편의 단위면적당 포함되는 혈중세포 중, 목적한 특정 종류의 혈중 세포가 포함될 확률은 매우 낮아질 수 있다. 또한, 파라핀블록의 제조과정에서 적혈구가 손상, 파괴 등이 될 경우 목적한 특정 종류 혈중 세포의 세포막, DNA, RNA 및 세포질 등에 영향을 줄 수 있는 각종 효소(enzyme)들이 방출될 수 있어 목적한 특정 혈중 세포의 진단이 매우 불리하거나 불가능할 수 있다. 이에 따라 혈중 암세포 진단을 위한 파라핀블록을 제조하기 위해서 피검체에서 채혈된 혈액에서 적혈구를 제거한 혈중세포만을 추출해야 한다.
한편, 종래의 혈중 암세포 진단방법은 피검체의 혈액에서 적혈구뿐만 아니라 여러 화학적, 물리적 처리과정을 거쳐 백혈구까지 제거하고 최종 혈중 암세포를 추출하였는데, 이러한 종래의 방법은 최종 혈중 암세포의 추출까지 여러 단계를 거치며, 여러 번의 원심분리는 혈중 암세포의 손실을 유발할 수 있고, 혈중 암세포의 수거율을 저하시킬 수 있는 문제가 있었다. 그러나 본 발명은 피검체에서 채취한 혈액에서 오직 적혈구만을 제거한 혈중 세포를 표본화하기 때문에 제조과정이 매우 간소화되고, 제조시간이 단축되며, 혈중암세포에 대한 물리적 또는 화학적 처리를 하지 않기 때문에 세포 손실이 적고, 세포 보존율이 높은 매우 효율적인 방법이다.
본 발명에 따른 (1) 단계에서 사용될 수 있는 적혈구 제거방법은 통상적인 원심법, 도립(倒立)원심법, 필터법, 데스트란법 및 세포 용해법 등을 포함할 수 있으나 이에 제한되지 않으나, 보다 바람직하게는 피콜 처리법으로 수행될 수 있다. 구체적으로, 하기의 실험예 3을 통해 확인할 수 있듯이, 정상혈액에 대장암 세포주 SW620 4000개를 스파이킹(spiking)한 후 피콜(ficoll)과 세포 용해액(lysis buffer)을 각각 처리하여 적혈구를 제거한 혈중 세포를 대상으로 제조한 파라핀 블록에서 혈중 암세포 수거율(파라핀 블록에 포함된 혈중암세포의 전체 수를 사이토케라틴(Cytokeratin) 염색 후 계수하여 수거율 산정)을 비교 분석한 결과, 피콜 처리법으로 적혈구를 제거하여 파라핀블록을 제조한 경우가 세포용해법을 이용한 경우에 비하여 수거율이 평균 1.7배 높음을 확인할 수 있다.
또한, 본 발명의 바람직한 일실시예에 따르면, 상기 (1) 단계에 사용되는 혈액세포가 추출되는 용기(tube)는 혈중 세포 응집제가 코팅된 것을 사용할 수 있다. 상기 코팅은 하기 (2) 단계에서 혈중 세포에 혼합될 혈중 세포 응집제와 동일한 조성의 혈중 세포 응집제를 혈중 세포를 추출할 용기 바닥면에 도포하는 것을 의미한다. 이때 코팅시키는 두께는 혈중 세포의 수거가 용이할 수 있도록 너무 얇거나 두껍지 않도록 일정하게 도포하는 것이 중요하며, 바람직한 코팅 두께는 1 ~ 2mm 일 수 있다.
혈중 세포를 추출할 용기(tube)를 미리 혈중 세포 응집제로 코팅하는 이유는 하기 (2) 단계를 통해 수득되는 혈중 세포 응집체(Cell pellet jelly)를 용기에서 분리할 때, 혈중 세포들(cell pellet)의 손상, 손실 및/또는 분실을 방지하기 위함이며, (1) 단계에 사용될 용기에 미리 혈중 세포 응집제를 코팅해 놓음으로써 혈중 세포 응집체의 수거를 용이하게 할 수 있다.
다음으로 본 발명에 따른 (2) 단계로써, (1) 단계에서 추출된 혈중 세포를 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제와 혼합하여 혈중 세포 응집체를 제조하는 단계를 포함한다. 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제는 혈액에서 적혈구가 제거된 혈중 세포에 대하여 세포외기질(extracellular matrix)기능을 수행함에 따라 혈중 암세포의 소실 및 세포질 손상이 되지 않도록 유지시켜 주며, 이를 통해 혈중 암세포의 항원성을 유지시킬 수 있다.
상기 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제에 대한 설명은 상술한 바와 동일하여 생략한다.
본 발명의 바람직한 일실시예에 따르면, 상기 파라핀블록 제조용 혈중 세포 응집제는 혈중 세포(cell pellet) 100 부피부에 대하여 80 내지 150의 부피부로 혼합하는 것이 바람직하며, 이를 통해 목적하는 파라핀블록의 물성 구현에 보다 유리할 수 있다.
한편, 본 발명에 따른 파라핀블록 제조용 혈중 세포 응집제에 포함되는 혈중 세포 응집물질로 젤라틴이 사용되는 경우 상기 혈중 세포 응집제에 포함된 젤라틴의 농도에 따라 특정 온도에서의 혈중 세포 응집체의 성상이 달라질 수 있는바, (2) 단계의 수행 온도를 고려하여 젤라틴의 농도를 달리할 수 있고 바람직하게는 하기 조건 (a) 내지 (c) 중 어느 하나를 만족할 수 있다.
(a) (2) 단계 수행온도가 5℃ 이상 15℃ 미만일 때 혈중내 유핵세포 응집제에서 젤라틴의 농도는 5 ~ 15 ㎎/㎖임
(b) (2) 단계 수행온도가 15℃ 이상 25℃ 미만일 때 혈중내 유핵세포 응집제에서 젤라틴의 농도는 15 ~ 25 ㎎/㎖임
(c) (2) 단계 수행온도가 25℃ 초과 35℃ 미만일 때 혈중내 유핵세포 응집제에서 젤라틴의 농도는 25 ~ 35㎎/㎖임
만일, 혈중 세포 응집물질로 젤라틴이 포함되는 경우 상기의 조건(a) 내지 (c)를 만족하지 못하면 (2) 단계에서 제조된 혈중세포 응집체에서 젤라틴이 녹아 용기에서 혈중세포 응집체를 분리할 수 없고, 목적하는 혈중세포 응집체를 제조하기 어려우며, 하기의 (3) 단계를 이하의 공정을 진행하기 어려운 문제점이 있을 수 있다.
따라서, (2) 단계를 수행하는 온도에 따라 응고된 젤라틴이 녹기 시작하는 농도가 다르기 때문에 (2) 단계를 수행하는 온도를 고려하여 용액의 농도를 달리하는 것이 바람직하다.
또한, 상기 (2) 단계에서는 혈중 세포 응집제를 발색시키기 위한 발색제가 더 포함될 수 있고, 혈중 세포 응집제에 포함된 혈중 세포 응집물질 100 부피부에 대하여 발색제를 5 내지 10의 부피부로 더 포함할 수 있다. 상기 발색제에 대한 구체적인 설명은 상술한 바와 같은바 생략한다.
한편, 본 발명에 따른 (1) 단계에서 피검체의 혈액으로부터 적혈구를 분리하여 혈액세포를 추출하는 과정부터 (2) 단계에서 젤라틴(gelatin) 등의 파라핀블록 제조용 혈중 세포 응집제를 투여하여 혈중 세포 응집체를 제조하는 과정까지는 피검체의 혈액을 채혈한 후 2시간 이내에 종료하는 것이 바람직하다. 채혈된 말초 혈액 내 포함된 혈중암세포는 백혈구에 비하여 세포 고사가 잘 일어나기 때문이다.
구체적으로, 도 5에 나타난 바와 같이 유방암세포주를 이용하여 정상 혈액에 뿌린 세포돌출실험(cell spiking test) 결과는 채혈 직후, 채혈 후 1시간, 채혈 후 2시간까지 시간 의존적으로 방치된 혈액을 대상으로 파라핀 블록을 만들어 세포학적 양상을 비교해 본 결과 채혈 후 2시간이 경과하면 암세포의 세포질이 파괴(degradation) 되고, 항원성이 저하되는 것을 관찰할 수 있다.
다음으로 본 발명에 따른 (3) 단계로써, (2) 단계를 통해 제조된 혈중 세포 응집체를 경화시켜 단계를 수행한다.
(2) 단계를 통해 제조된 혈중세포 응집체는 경화 단계를 거침으로써, 세포를 의 용기에서 혈중 세포 응집체를 분리시킬 수 있으며, 통상적인 파라핀블록 제조 전의 파라핀블록 대상이 되는 일반 생체조직의 처리시간(통상적으로 12시간 이상)을 6시간 이내로 단축시킬 수 있고, 후술하는 (4) 단계를 통한 파라핀블록 형성물질의 처리가 더 용이해지며, 목적하는 물성을 발현하는 파라핀블록으로 구현시키기에 보다 유리할 수 있다. 바람직하게는 상기 (3) 단계의 경화는 (2) 단계에서 제조된 혈중세포 응집체를 2℃ 내지 5℃에서 15 ~ 30분 동안 방치하여 혈중세포 응집체를 젤리 형태로 경화시킬 수 있다. 경화된 혈중세포 응집체는 세포의 보존을 위하여 포르말린, 메탄올 등에 넣어 고정시킬 수 있다.
다음으로, 본 발명에 따른 (4) 단계로써, 상기 (3) 단계에서 제조된 경화체에 파라핀블록 형성물질을 처리하는 단계를 수행한다.
상기 파라핀블록 형성물질은 알코올, 자일렌 및 파라핀을 포함할 수 있으며, (3) 단계에서 제조된 경화체에 대해 알코올, 자일렌 및 파라핀의 순으로 파라핀블록 형성물질을 처리하여 자동침투과정을 통해 파라핀블록을 제조할 수 있다.
구체적으로 상기 자동침투과정은 경화체를 종이에 싸서 알코올, 자일렌 및 파라핀의 순으로 처리함으로써 수행할 수 있다. 상기 자동침투과정을 거치는 이유는 다음과 같다. 먼저, 현미경 관찰 대상이 되는 검체시료를 얇게 박절하기 위해서는 파라핀 침투가 가능하도록 고정액을 통해 고정시킨 검체시료로부터 수분을 제거할 수 있다(알코올 처리단계). 다음으로, 처리된 알코올에 의해서 검체시료 내의 수분은 제거된다 하더라도 알코올과 파라핀이 서로 섞이지 않기 때문에 자일렌을 이용하여 알코올을 제거할 수 있다(자일렌 처리단계). 마지막으로, 검체시료를 현미경 관찰을 위해서는 검체시료를 얇게 박절하는 과정이 요구됨에 따라, 박절 과정의 수행을 위해서 검체시료에 대해 파라핀 침투 과정을 수행할 수 있다(파라핀 처리단계).
이때, 검체의 양이나 크기에 따라서, 알코올 처리단계는 여러 단계로 나누어 수행될 수 있는데, 예를 들어 먼저 저농도 알코올을 처리하고 (복수 회 가능) 이어서 고농도 알코올을 처리하는 (복수 회 가능)방식으로 수행될 수도 있다. 이러한 복수 회에 걸친 처리는 자일렌 처리단계 및 파라핀 처리단계의 경우에도 동일하게 적용될 수 있다.
또한, 본 발명은 상술한 방법에 의해서 제작된 파라핀 블록을 사용하여 검체 시료를 현미경을 통해 관찰하는 방법을 제공하는데, 이는 상기 서술된 방법으로 제작된 파라핀 블록을 적당한 크기로 박절하여 절편화하고, 상기 박절된 파라핀 블록을 염색하고 관찰하는 단계를 포함한다.
상기 박절 단계는 마이크로톰(microtome) 등과 같은 통상적인 박절 기기를 사용하여 수행될 수 있으며, 당업계에서 통상적으로 사용되는 염색시약을 사용하여 염색과정이 수행될 수 있다.
상기 자동침투과정을 수행하여 제조된 본 발명에 따른 파라핀 블록은 3 내지 10㎛ 두께로 박절하여 절편화하고, 상기 박절된 블록을 면역조직화학적 염색을 통하여 관찰할 수 있다.
일반적으로 파라핀 블록은 4 내지 5u㎛의 두께로 박절하는 것이 일반적이나, 본 발명의 파라핀 블록은 혈중암세포의 크기를 고려하여 3 내지 10㎛의 두께로, 바람직하게는 5 내지 8㎛, 더욱 바람직하게는 7 내지 8㎛의 두께로 박절하여 혈중암 세포 수를 계수할 수 있다.
본 발명에 따른 파라핀 블록을 7 ~ 8㎛의 두께로 박절할 경우, 6㎖ 혈액의 채혈시 30회의 검사가 가능하며, 3 ~ 4㎛의 두께로 박절할 경우 60회의 검사가 가능하고, 이에 따라 단 1회 소량 채취된 혈액을 통해 수십 번의 검사가 가능하게 되어 매 회 다른 표지자를 적용할 수 있고, 시간차를 두고 여러 번 검사함으로써 병리학적으로는 물론 임상적으로도 적용 범위가 매우 넓은, 유용한 검사법이 될 수 있다.
상기 면역조직화학적 염색은 당업계에서 통상적으로 사용되는 염색시약을 사용하여 염색과정이 수행될 수 있으며 이에 제한은 없으나, 본 발명의 바람직한 일실시예에 따르면 H&E 염색 또는 사이토케라틴(cytokeratin,CK) 면역조직화학염색일 수 있다.
본 발명의 전술한 방법에 의해서 제작된 파라핀 블록을 사용하여 검체 시료에 대하여 현미경 관찰을 수행함으로써, 혈중암세포의 조직학적, 면역조직화학적 또는 분자생물학적 검사 방법을 수행할 수 있다.
구체적으로, 하기의 실험예 8에서 알 수 있듯이, 혈중암세포의 형태학적 확인 및 계수를 통한 조직학적 검사를 실시하여 오염된 상피세포와 혈중암세포의 구별이 가능해짐에 따라 위양성 판정의 가능성이 없어 정확한 진단을 가능하게 한다.
또한, 하기의 실험예 3 및 실험예 4에서 알 수 있듯이, 사용하고자 하는 표면 표지자를 제한 없이 선택하여 면역조직화학적 검사가 가능하며, 더 나아가 이미 원발 부위가 불명확한 전이성 암종의 병리학적 진단에 사용되고 있는 하기 표 1에 나타낸 조직특이적 표지자를 조합하면 보다 민감하게 혈중암세포를 검출할 수 있다.

또한, 형광동소교잡반응(FISH: Fluorescence in situ hybridization) 및 유전자 변이 검사(mutation analysis)와 같은 분자생물학적 검사가 가능하다.
결국, 암 진단을 위한 진단검사의학 및 영상의학 기술의 발달에도 불구하고, 이들 검사만으로 암을 진단하기에는 한계가 있으며, 가장 확실한 암 진단은 조직 검사를 통한 병리학적 진단이지만 수술적 치료가 불가능하거나 조직 검사를 위한 접근이 어려운 말기암 환자군에서는 조직 검사를 대체할 수 있는 효과적인 진단방법이 될 수 있다.
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하기로 한다. 이들 실시예는 단지 본 발명을 예시하기 위한 것이므로, 본 발명의 범위가 이들 실시예에 의해 제한되는 것으로 해석되지는 않는다.
<실시예 1>
1-1. 채혈
정상인의 말초혈액 6㎖를 EDTA가 첨가된 용기에 채혈 후 상기 혈액내 포함된 백혈구 수를 4×107으로 계수하고, 대장암세포주 SW620을 백혈구 10,000: 암세포 1의 비율에 맞도록 암세포 4000개를 스파이킹(spking)한 후 상온 보관에서 보관하였다.
1-2. 적혈구의 분리
채혈된 말초혈액 6㎖당 15㎖ 용기(tube)를 각 3개씩 준비하여, 각 15㎖ 용기에 피콜(Ficoll)을 5㎖씩 분주하였다. 다음, EDTA 용기(tube)에 채취된 혈액을 50㎖ 용기에 모두 넣은 후, 인산완충용액(PBS)을 혈액의 2배 부피로 넣고 혼합하였다.
15㎖ 용기에 담겨있는 피콜 위에 인산완충용액이 포함된 말초혈액 7㎖씩을 분주 후, 2000rpm에서 20분 동안 원심 분리하였고, 원심분리를 마친 용기 내에서 세 층으로 분리된 혈청(serum), 피콜 및 혈청과 피콜 사이의 하얀색 버피코트 층(buffy coat layer)를 확인하고, 버피코트층에 밀집되어 있는 하얀 세포층만 조심스럽게 수거하였다. 그 후, 새 50㎖ 용기에 상기 수거한 세포를 옮겨담고, 인산완충용액을 넣어 최종 부피를 50㎖로 맞춘 후, 1500rpm에서 10분 동안 원심 분리하였다. 상기 원심분리를 마친 용기 내에서 상층액을 수거한 후, 남은 세포 (cell pellet)에 PBS 1ml 투여 후 13000rpm에서 3분 동안 원심 분리하여, 상층액 수거 후, 적혈구가 제거된 혈중 세포를 얻었다.
1-3. 파라핀블록 제조용 혈중 세포 응집제의 제조
먼저, 혈중 세포 응집물질로 젤라틴 용액을 90 ~ 100℃ 온수에 10㎎/㎖ 농도로 녹여 젤라틴 용액을 만든 뒤 상온에 거치하여 용액의 온도를 실내온도(22℃)로 식혀 사용하였다.
적혈구가 제거된 혈중 세포를 혈중 세포 응집제와 혼합하기 전에, 빈 용기에 동일 농도의 젤라틴 용액을 코팅 후 두께가 1mm 되도록 투입하여 미리 코팅해 놓은 다음, 미리 코팅해 놓은 용기에 혈중 세포를 넣고, 혈중 세포가 담겨진 용기에 젤라틴(Gelatin) 용액을 혈중세포와 젤라틴 용액이 1:1의 부피비가 되도록 혼합하였다. 추가로, 투명한 젤라틴 용액의 응집 확인을 위해, 발색제인 0.05% 브로모페놀 블루(Bromophenol blue)가 포함된 로딩 용액(loading buffer)(Takara Bio Inc)을 젤라틴 용액에 대하여 1:0.05의 부피비로 더 첨가하였다.
혈중세포/젤라틴 용액/발색제 혼합물을 냉장(4℃)에서 20분간 보관 후, 혈중세포/젤라틴 용액/발색제 혼합물이 젤리(jelly)형태로 굳어 경화된 것을 확인한 후, 경화체를 용기에서 분리하여, 15㎖ 용기에 상기 경화체를 넣고, 중성 포르말린(formalin)을 10㎖ 투여 후, 상온에서 18시간 동안 보관하였다.
1-4. 파라핀 블록의 제조
상기 실시예 1-3을 통해 제조된 혈중 세포 응집체의 경화물을 이용하여 파라핀 블록(Paraffin block)을 제조하였다.
일반적인 파라핀 블록 제조방법을 따르나, 일반조직 처리 과정은 12시간 이상 소요되지만, 본 발명의 파라핀 블록의 제조방법은 조직 처리 과정을 6시간으로 단축함으로써 제조시간을 절감할 수 있으며, 간소화할 수 있다.
파라핀 블록의 제조는 하기 표 2의 조직 처리 과정에 따라 수행하였다.

상기 표 2의 과정을 마친 후, 파라핀에 포매(embeding)하여 블록으로 만들어 보관하였다.
상기 표 2의 과정에서 알코올은 에탄올(Ethanol, Fisher Scientic), 자일렌은 Xylene (DUKSAN), 파리핀은 Histosec Pastillen(MERCK)을 사용하였다.
<실시예 2>
실시예 1과 동일하게 실시하여 제조하되, 실시예 1의 1-2에서 적혈구를 분리하기 위하여 피콜 대신에 세포용해액을 사용하여 파라핀 블록을 제조하였다.
<실시예 3 ~ 7>
실시예 1과 동일하게 실시하여 제조하되, 실시예 1의 1-3에서 젤라틴용액의 농도를 10㎎/㎖ 대신에 각각 3㎎/㎖, 20㎎/㎖, 30㎎/㎖, 40㎎/㎖, 50㎎/㎖로 하여 파라핀블록을 제조하였다.
<실시예 8 ~ 9>
실시예 1과 동일하게 실시하여 제조하되, 실시예 1의 1-1에서 스파이킹 되는 암세포 수 및 혈액내 백혈구 수의 비율을 1:104 대신에 각각 1:105 및 1:106이 되도록 암세포를 스파이킹하여 파라핀블록을 제조하였다.
<실시예 10>
실시예 1과 동일하게 실시하여 제조하되, 실시예 1의 1-1에서 스파이킹 되는 암세포의 종류를 대장암세포주 SW620 대신에 유방암세포주 SKBR3를 사용하였고, 실시예 1-1 수행 후 실시예 1-2를 즉시 수행하는 대신에 실시예 1-1 수행 후 1시간 혈액을 방치시킨 뒤 실시예 1-2를 수행하여 파라핀블록을 제조하였다.
<실시예 11>
실시예 10과 동일하게 실시하여 제조하되, 실시예 1-1 수행 후 1시간 혈액을 방치시키는 대신에 2시간 방치한 후 실시예 1과 동일하게 실시하여 파라핀블록을 제조하였다.
<실시예 12 ~ 21>
상기 실시예 1과 동일하게 실시하여 제조하되, 실시예 1의 1-1에서 스파이킹 되는 암세포 수 및 혈액내 백혈구 수의 비율을 1:104 대신에 1:105로 하였고, 실시예 1의 1-3에서 혈중세포 응집물질을 하기 표 7과 같이 변경하여 파라핀블록을 제조하였다.
<비교예 1>
정상 혈액 6㎖ 혈액에 암세포 4000개를 spiking 한 후, 피콜을 처리하여 적혈구를 제거한 후 추출된 혈중 세포에 포르말린을 투여하여 18시간 고정 후, 고정된 검체를 다시 원심분리시킨 후 상층액을 제거하고 검체 침전물을 수득하여 폴리비닐알코올 및 증류수를 첨가하였다. 이 혼합물에 아세톤 및 시아노아크릴레이트를 가하여 다시 한번 더 원심분리시킨 후, 상층액을 제거하고 응집 검체 침전물을 수득하였다. 이 침전물에 대해서 자동침투과정을 수행함으로써 파라핀 블록을 제작한 후,
<실험예 1> - 면역조직화학 염색을 통한 혈중 암세포 검출
실시예 1을 통해 제조된 파라핀블록을 8㎛의 두께로 박절한 절편에 대해 사이토케라틴(CK) 염색을 실시한 후 이를 현미경을 통해 관찰한 사진을 촬영하여 도 2에 나타내었다.
구체적으로 도 2에 나타난 바와 같이, 파라핀 블록을 박절한 절편을 CK 면역조직화학 염색을 시행한 결과 갈색으로 관찰되는 세포가 혈중 암세포임을 확인할 수 있다.
<실험예 2> - 암세포주 유래 혈중암세포의 수거율 분석
실시예 1 및 비교예 1을 통해 제조된 혈중암세포 검출용 파라핀 블록에 대한 혈중암세포 분석에 있어서 민감도를 측정하기 위하여 파라핀 블록에 포함된 암세포 수거율을 비교 분석하였다.
혈중암세포의 크기를 고려하여 파라핀블록을 8㎛의 두께로 전체 블록을 연속 절편으로 만든 뒤, 각 절편에 사이토케라틴 염색을 하여 블록 전체에 포함된 혈중암세포 수를 계수하였고, 실시예 1 및 비교예 1을 각각 3회씩 시행하여 제조된 파라핀블록에 대한 결과의 평균값을 하기 표 3에 나타내었다.

구체적으로 상기 표 3에 나타난 바와 같이, 실시예 1 에 의해 제조된 파라핀블록을 통한 혈중암세포의 수거율이 평균 93%로 비교예 1 에 의해 제조된 파라핀블록을 통한 혈중암세포의 수거율인 25%보다 3.7배 높음을 알 수 있다.
또한, 실시예 1에 따라 제조된 파라핀 블록으로부터 박절된 절편의 수는 30으로 비교예 1에 의한 방법에 비해 적었으나, 각 절편 당 포함된 혈중암세포의 수는 비교예 보다 현저하게 높음을 알 수 있다.
상기 표 3과 같은 결과는 비교예 1에서는 적혈구 분리 후 추가로 두 번의 원심분리 과정이 더 추가됨에 따른 혈중 암세포 분실 및/또는 손실에 의한 것이며, 이에 따라 실시예 1의 경우가 원심 분리과정 중 혈중암세포의 손상 또는 소실을 현저히 감소시킬 수 있음을 알 수 있다.
또한, 파라핀 블록으로 절편을 만드는 과정에서 파라핀을 녹이기 위하여 60℃로 가온시켜야 하는데 이 과정에서 비교예의 경우 세포의 소실이 있는 것으로 볼 수 있고, 이에 반해 실시예 1에 의해 제조된 파라핀 블록은 혈중 세포 응집제를 이용하기 때문에 파라핀의 가온과정에 의한 혈중 세포의 소실이 감소되고 있음을 확인할 수 있다.
또한, 실시예 1에서 제조된 파라핀 블록은 8㎛ 두께로 박절할 경우에는 30회의 검사가 가능하고, 4㎛ 두께로 박절할 경우 60회의 검사가 시행 가능하며, 매 회 다른 표지자를 적용할 수 있음을 확인할 수 있다.
<실험예 3> - 적혈구 제거 방법에 따른 암세포 수거율 분석
실시예 1 및 실시예 2를 통해 제조된 파리핀 블록에서 암세포 수거율 (파라핀 블록에 포함된 암세포의 전체 수를 사이토케라틴(Cytokeratin) 면역조직화학염색 후 계수하여 수거율 산정)을 계산하여 하기 표 4에 나타내었다.

구체적으로 상기 표 3에 나타난 바와 같이, 암세포 수거율 비교 결과 피콜 처리법이 세포용해법에 비하여 암세포 수거율이 평균 1.7배 높음을 확인할 수 있고, 이를 통해 보다 우수한 물성의 파라핀블록을 제조하기 위해 혈액에서 적혈구를 제거하는데 있어 피콜을 처리함이 더 우수함을 확인할 수 있다.
<실험예 4> - 젤라틴 용액 농도 변화에 따른 혈중암세포 수거율 분석
실시예 1 및 실시예 3 ~ 7을 통해 제조된 파라핀블록에 대해 암세포 수거율 (파라핀 블록에 포함된 암세포의 전체 수를 상온 20℃에서 사이토케라틴(Cytokeratin) 면역조직화학염색 후 계수하여 수거율 산정)을 계산하여 하기 표 5에 나타내었다.

젤라틴 용액 농도 변화에 따른 암세포 수거율을 비교한 결과, 상기 표 5에 나타난 바와 같이, 상온에서 실시예 1, 4, 5의 경우 암세포의 수거율이 높은데 반하여, 실시예 3, 6, 7은 암세포 수거율이 현저히 저하된 것을 확인할 수 있다. 특히, 실시예 5에서 젤라틴용액의 농도가 30㎎/㎖인 경우 혈중 암세포 수거율이 80%인 반면에 실시예 6에서 젤라틴 용액의 농도가 40㎎/㎖인 경우 혈중 암세포 수거율이 23%로 수거율이 57%나 저하(수거율 감소율은 71.2%에 달함)됨을 확인할 수 있다.
<실험예 5> - 젤라틴 용액 농도 변화에 따른 실내온도별 용해여부 분석
실시예 1, 4, 5에서 사용된 젤라틴 용액 50㎕ 에 대해 실내 온도를 각각 5℃, 17℃, 28℃로 달리했을 때 거치시간별로 용해여부를 평가하여 녹지 않은 경우를 ×, 녹아 용해된 경우를 ○로 표시하여 하기 표 6에 나타내었다.

구체적으로 상기 표 6에 나타난 바와 같이, 젤라틴 용액의 농도에 따라 실내온도별로 용해여부가 달라지고, 거치시간이 짧을 경우 녹지 않아가 거치시간이 길어지면 용해되는 경우가 있음을 확인할 수 있다.
따라서, 실내온도 혹은 계절에 따라 젤라틴 용액의 농도를 달리해야 함을 알 수 있고, 실내온도가 15℃이하인 동절기에는 젤라틴의 농도가 10㎎/㎖이어도 젤리형태의 혈중세포 응집체를 제조할 수 있으나 실내온도가 25℃ 이상인 하절기에는 젤라틴의 농도가 10㎎/㎖일 경우 젤라틴이 녹음에 따라 젤리형태의 혈중세포 응집체를 제조하기 위해서는 젤라틴의 농도를 20㎎/㎖를 초과하는 농도로 달리해야됨을 확인할 수 있다.
한편, 상술한 실험예 4에서 확인할 수 있듯이, 젤라틴의 농도에 따라 파라핀블록의 암세포 수거율이 현저히 달라지고, 젤라틴 용액의 농도가 30㎎/㎖일 때에 비해 10㎎/㎖일 때 암세포 수거율이 더 우수함에 따라 실내온도가 5 ~ 15℃에서는 파라핀블록을 제조하기 위한 젤라틴 용액의 농도를 5 ~ 15㎎/㎖로 함이 가장 좋은 혈중 세포 응집체를 제조할 수 있는 동시에 현저히 우수한 물성을 갖는 파라핀블록을 제조할 수 있음을 알 수 있다.
또한, 실내온도가 15 ~ 25℃ 일 경우, 젤라틴 용액의 농도를 20 ~ 30㎎/㎖로 함이 가장 좋은 혈중 세포 응집체를 제조할 수 있는 동시에 현저히 우수한 물성을 갖는 파라핀블록을 제조할 수 있음을 알 수 있다.
또한, 실내온도가 상온보다 높은 25 ~ 35℃일 경우, 젤라틴 용액의 농도가 30㎎/㎖일 때 가장 좋은 응집물질을 만들 수 있는 조건임을 확인할 수 있었으며, 실내온도가 높아지면 응고된 젤라틴 용액이 시간이 지남에 따라 쉽게 녹아버리는 문제점이 생기게 되고, 이를 해결하기 위하여 상기 표 5에서 확인할 수 있듯이, 너무 높은 농도의 젤라틴 용액을 사용하게 되면 혈중암세포의 수거율이 떨어지는 또 다른 문제점이 발생하므로 상기 표 5 및 표 6의 결과를 종합해 볼 때, 실내온도 25 ~ 35℃에서는 젤라틴 용액의 농도가 25 ~ 35㎎/㎖일 때 가장 좋은 혈중 세포 응집체를 제조할 수 있는 동시에 현저히 우수한 물성을 갖는 파라핀블록을 제조할 수 있음을 알 수 있다.
<실험예 6> - 혈중 세포 응집물질에 따른 혈중암세포 수거율 분석
실시예 12 ~ 을 통해 제조된 파라핀블록에 대해 암세포 수거율 (파라핀 블록에 포함된 암세포의 전체 수를 상온 20℃에서 사이토케라틴(Cytokeratin) 면역조직화학염색 후 계수하여 수거율 산정)을 계산하여 하기 표 7에 나타내었다.

구체적으로 상기 표 7에서 확인할 수 있듯이, 젤라틴용액을 단독으로 혈중응집물질로 포함한 실시예 12 및 실시예 17이 다른 종류의 혈중 응집물질을 더 포함한 경우에 비해 대체로 혈중 암세포 수거율이 우수한 것을 확인할 수 있다. 다만, 젤라틴용액이 아닌 다른 종류의 혈중 응집물질의 함량에 따라 혈중암세포 수거율이 더 향상될 수 있으며, 이는 실시예 17, 19 및 21을 통해 확인할 수 있다.
<실험예 7> - 파라핀블록을 통한 혈중 암세포 진단의 민감도 분석
실시예 1, 실시예 8 및 실시예 9를 통해 제조된 파라핀 블록에 대해 CK 면역조직화학적 염색을 수행하여 현미경 관찰을 통해 촬영한 사진을 도 3에 나타내었다.
구체적으로 도 3과 같이, 대장암세포주 SW620와 백혈구가 1:106의 비율(실시예 9)일 때에도 혈중암세포의 검출이 가능함을 확인할 수 있다. 이를 통해 본 발명에 의한 혈중암세포 검출용 파라핀블록의 제조방법은 혈중암세포의 검출에 있어 민감도 및 특이도가 우수함을 확인할 수 있다.
<실험예 8> - 파라핀블록을 통한 혈중암세포에 대한 형태학적 분석
본 발명에 따른 혈중암세포 검출용 파라핀블록에 대한 혈중암세포 분석에 있어서 형태학적 확인을 통해 오염에 의한 위양성 검출을 확인하였다.
암세포와 정상 세포는 염색성과 상관없이 세포병리학적으로 구분이 가능하다. 즉, 기존에 표면표지자에 의한 혈중암세포의 진단법에서는 표면표지자 양성 혹은 음성에 따라 혈중암세포의 존재 여부를 결정하기 때문에 오염된 정상 상피세포가 위양성으로 판독될 수 있었다.
실시예 1을 통해 제조된 파라핀블록에 대한 CK 면역조직화학염색 결과, 도 4에 나타난 바와 같이, 채혈 시 피부 각질 세포 혹은 적혈구 처리 및 파라핀 블록의 제작 과정 등에서 피부에서 떨어진 CK 양성 편평 상피세포의 오염(contamination)이 있을 수 있는 것을 확인하였다.
따라서, 본 방법에 의해 제조된 혈중암세포 검출용 파라핀블록에서는 병리학적 검사가 가능하므로 혈중암세포의 형태학적 확인을 통해 오염된 상피세포와 구별하여 위양성 여부를 판별해낼 수 있어, 보다 정확한 진단이 가능하다.
<실험예 9> - 채혈 후 혈액의 방치시간에 따른 혈중 암세포의 항원성 손실여부 분석
실시예 1, 실시예 10 및 실시예 11을 통해 제조된 파라핀블록에 대해서 CK 면역조직화학적 염색을 수행한 후 현미경 관찰을 통해 촬영한 사진을 도 5에 나타내었다.
구체적으로 도 5를 통해 확인할 수 있듯이, 적혈구 제거 처리 과정 수행 전까지 방치시간에 따른 세포학적 양상을 비교해본 결과, 채혈 후 2시간이 지날 경우 암세포의 세포질이 파괴(degradation) 되고 항원성이 떨어지는 것을 관찰할 수 있었다.
<실험예 10> - 혈중암세포 검출용 파라핀블록에 대한 다양한 적용
본 방법의 실시예 1에 의해 제조된 혈중암세포 검출용 파라핀블록을 이용한 다양한 검사의 적용 가능성을 확인하였다.
다양한 세포주유래 혈중암세포를 정상 혈액에 대하여 백혈구 10,000: 암세포 1 의 비율로 스파이킹(spiking)한 후, 상기 실시예 1의 제조방법을 통해 파라핀 블록을 제조하여 각종 암 진단 특이표지자에 대한 면역조직화학염색 및 유전자 검사를 수행하였고, 그 결과를 도 6 내지 도 8에 나타내었다.
구체적으로 도 6에 나타난 바와 같이, 유방암세포주(SKBR3) 유래 혈중암세포에 대한 일반 H&E 염색, c-erbB2 면역조직화학염색 및 Her2/neu 유전자에 대한 형광동소교잡 반응 검사(FISH)를 수행한 결과, 유전자의 발현이 관찰되었다.
또한, 도 7에 나타난 바와 같이 간암세포주(HepG2) 유래 혈중암세포에 대한 면역조직화학염색을 수행한 결과, EpCAM은 음성이나, 간암 특이표지자인 글리피칸-3(Glypican-3) 및 알파-펙토단백질(alpha-fetoprotein, AFP)에 양성 반응을 보임을 확인하였다.
또한, 도 8에 나타난 바와 같이 대장암세포주(SW620) 유래 혈중암세포에 대한 면역조직화학염색을 수행한 결과, 세포질막에서 EpCAM의 발현을 확인하였고, 대장암 진단 표지자인 CDX2의 핵 발현 및 p53 단백질의 발현 또한 확인할 수 있었다.
<실험예 10> - 암 전이 환자에서의 혈중암세포의 세포학적 특성분석
실제 환자에서의 적용 예로써, 병기가 4기인 에스 결장 대장암으로 2008년 결장절제술을 받고 2010년 간에 재발하였던 환자 혈액을 채혈하여 본 발명의 제조과정에 따른 파라핀블록을 제조하여 혈중암세포를 확인하고, 원발 종양의 암세포 및 혈중암세포의 세포학적 특성을 비교하여 그 결과를 도 9 및 도 10에 나타내었다.
구체적으로 도 9에 나타난 바와 같이, 환자의 원발 대장암 및 간암 재발에 대한 육안 사진 및 원발 대장암의 조직 소견을 확인할 수 있으며, 2012년 7월 채혈된 환자 혈액으로 본 발명의 제조과정에 따른 파라핀블록의 절편에서 혈중암세포를 확인할 수 있었다.
또한, 도 10에 나타난 바와 같이 상기 환자의 혈액에서 관찰된 혈중암세포의 H&E 염색 및 CK 면역조직화학염색을 수행한 결과, 일부 세포는 사이토케라틴(CK)에 음성 소견을 보임을 확인할 수 있었다.
이를 통해 이미 원발 부위가 불명확한 전이성 암종의 병리학적 진단에 사용되고 있는 조직특이적 마커를 조합하면 보다 민감하게 혈중암세포를 검출할 수 있는 것이다.
따라서, 본 발명의 파라핀 블록의 제조방법은 혈액 시료의 채취를 통해 혈액내 존재하는 혈중암세포를 검출하는 방법으로 비침습적이고 간단하기 때문에 암의 재발 및 전이암의 진단 및 예후 예측에 매우 유용하며, 암 발병, 전이 및 재발 기전의 분석을 통해 암 치료의 획기적인 전기를 마련할 수 있다.
Claims (15)
- 혈중 세포에 대하여 혈중 세포들을 응집시키고, 혈중 세포의 손상 및 소실을 방지하는 세포외기질의 기능을 수행하는 혈중 세포 응집 물질;을 포함하는 파라핀블록 제조용 혈중 세포 응집제.
- 제1항에 있어서,상기 혈중 세포 응집물질은 젤라틴을 포함하는 것을 특징으로 하는 파라핀블록 제조용 혈중 세포 응집제.
- 제2항에 있어서,상기 응집제에서 젤라틴의 농도는 5 내지 35㎎/㎖ 인 것을 특징으로 하는 파라핀블록 제조용 혈중 세포 응집제.
- 제2항에 있어서,상기 혈중 세포 응집물질은 펙틴(pectin), 콘드로이틴설페이트(chondroitin sulfate), 파이브로넥틴(fibronectin), 전분(starch) 및 라미닌(laminin)으로 이루어진 군에서 선택된 어느 하나 이상의 물질을 더 포함하는 것을 특징으로 하는 파라핀블록 제조용 혈중 세포 응집제.
- 제4항에 있어서,혈중 세포 응집물질로 젤라틴 이외에 더 포함되는 물질은 젤라틴 중량의 5% 이내로 포함되는 것을 특징으로 하는 파라핀블록 제조용 혈중 세포 응집제.
- 제1항에 있어서,상기 혈중 세포 응집제는 혈중 세포 응집물질을 발색시키기 위한 발색제를 더 포함하며, 상기 발색제는 혈중 세포 응집물질 100 부피부에 대하여 5 내지 10 부피부로 포함되는 것을 특징으로 하는 파라핀블록 제조용 혈중 세포 응집제.
- 제3항에 있어서,상기 젤라틴의 농도는 10 내지 30㎎/㎖ 인 것을 특징으로 하는 파라핀블록 제조용 혈중 세포 응집제.
- 제1항에 따른 파라핀블록 제조용 혈중 세포 응집제를 포함하는 혈중암세포 진단용 키트.
- (1) 피검체의 혈액에서 적혈구를 제거하고 혈중 세포를 추출하는 단계;(2) 상기 혈중 세포를 제1항에 따른 혈중 세포 응집제와 혼합하여 혈중 세포 응집체를 제조하는 단계;(3) 상기 혈중 세포 응집체를 경화시켜 단계; 및(4) 상기 (3) 단계에 따른 경화체에 파라핀블록 형성물질을 처리하는 단계;를 포함하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법.
- 제9항에 있어서,상기 (1) 단계에 사용되는 혈중 세포가 추출되는 용기(tube)는 젤라틴 용액이 코팅된 것을 특징으로 하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법.
- 제9항에 있어서,상기 (1) 단계의 적혈구 제거는 피콜(ficoll)을 이용하는 것을 특징으로 하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법.
- 제9항에 있어서,상기 (2) 단계는 혈중 세포와 혈중 세포 응집제를 1: 1 ~ 1.5 부피비로 혼합시키는 것을 특징으로 하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법.
- 제9항에 있어서,상기 (2) 단계의 혈중 세포 응집제는 젤라틴을 포함하고, 상기 (2) 단계가 수행되는 온도에 따라 혈중 세포 응집제에서의 젤라틴 농도는 하기 조건 (a) 내지 (c) 중 어느 하나를 만족하는 것을 특징으로 하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법.(a) (2) 단계 수행온도가 5℃ 이상 15℃ 미만일 때 혈중 세포 응집제에서 젤라틴의 농도는 5 ~ 15 ㎎/㎖임(b) (2) 단계 수행온도가 15℃ 이상 25℃ 미만일 때 혈중 세포 응집제에서 젤라틴의 농도는 15 ~ 25 ㎎/㎖임(c) (2) 단계 수행온도가 25℃ 초과 35℃ 미만일 때 혈중 세포 응집제에서 젤라틴의 농도는 25 ~ 35㎎/㎖임
- 제9항에 있어서,상기 (4) 단계의 파라핀블록 형성물질은 알코올, 자일렌 및 파라핀을 포함하고,상기 (4) 단계는 알코올, 자일렌 및 파라핀의 순으로 파라핀블록 형성물질을 처리하여 자동침투과정을 통해 수행되는 것을 특징으로 하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법.
- 제9항에 있어서,상기 파라핀블록은 혈중암세포(circulating tumor cell)의 조직학적, 면역조직화학적 또는 분자생물학적 검출방법에 이용되는 것을 특징으로 하는 혈중 세포 응집제를 이용한 파라핀블록 제조방법.
