Abstract
Description
しかしながら、脾臓摘出は侵襲性が高く門脈血栓、免疫能の低下が報告されていること、PSEでは脾膿瘍や敗血症を引き起こすことが報告されていること、血小板輸血には輸血に伴う副作用 (輸血関連急性肺障害や感染症)のリスクを有することや血小板製剤自体の有効期限が短いこと(採血後4日間) 等の問題点があり、必ずしも安全かつ簡便な処置ではない。また、血小板を輸血する場合、繰り返し輸血を実施することにより抗体の産生を促し、血小板輸血不応症を引き起こす可能性があることも知られている。
このような状況の下、副作用が無く、簡便に投与可能な血小板減少症治療薬が望まれている。
従って、本発明の目的は、十分な血小板数の増加効果を得ることと、血小板数を過剰に増加させないことの両方を満たす、トロンボポエチン受容体アゴニスト作用を有する化合物を含有する医薬組成物を提供することにある。
(1)トロンボポエチン受容体アゴニスト作用を有する化合物を含有し、
投与される患者の血小板数が、
投与開始前と比較して2万/μL以上増加し、かつ、
5万/μL以上となった場合は、以後の投与を中止することを特徴とする、
血小板増加のための医薬組成物。
(2)血小板減少患者に対して投与される、(1)記載の医薬組成物。
(3)当該医薬組成物が投与される患者の投与前の血小板数が、5万/μL未満である、(1)または(2)記載の医薬組成物。
(4)前記の投与される患者が、侵襲術の実施前の患者である、(2)または(3)記載の医薬組成物。
(5)前記の侵襲術が待機的侵襲術である、(4)記載の医薬組成物。
(6)前記の侵襲術がラジオ波焼灼療法の施術である、(4)または(5)記載の医薬組成物。
(7)前記のラジオ波焼灼療法が、肝癌に対するものである、(6)記載の医薬組成物。
(8)前記の肝癌が、原発性肝癌である、(7)記載の医薬組成物。
(9)前記の投与される患者が慢性肝疾患の患者である、(1)~(8)のいずれかに記載の医薬組成物。

「投与される患者の血小板数が、投与開始前と比較して2万/μL以上増加し、かつ、5万/μL以上となった場合」とは、投与開始後のある時点での血小板数が、前記の投与開始前の血小板数と比較して2万/μL以上増加し、かつ、その時点での血小板数が5万/μL以上となった場合を意味する。
血小板数の過度の増加は、血栓の発現リスクを高めると考えられることから、投与患者の安全性を確保するために、血小板数による投与中止基準を設定することにした。健康成人を対象として実施したlusutrombopagの単回投与試験及び反復投与試験の薬物動態及び血小板数推移データを用いて、lusutrombopagを慢性肝疾患患者に7日間反復投与したときの血小板数の推移をモンテカルロシミュレーションで予測した。シミュレーションにあたっては、まず、健康成人のデータから、健康成人におけるPK/PDモデルの各種パラメータを推定し(試験例1)、さらに、健康成人におけるPK/PDモデルを基に、慢性肝疾患患者におけるモデルパラメータを算出した(試験例2)。そのパラメータを基に、lusutrombopagの投与期間中に血小板数が投与開始前より2万/μL以上増加しかつ5万/μL以上となった時点で以後のlusutrombopagの投与を中止する基準についての検証(試験例3)を行った。
健康成人男性を対象とした国内第1相試験において得られた54例の血漿中濃度データ及び血小板データを用いて、薬物動態及び血小板数推移を説明するPK/PDモデルを構築した。薬物動態を説明するPKモデルには3-コンパートメントモデルを用い、吸収の遅れを説明するために吸収過程に3つのコンパートメントを組み込んだモデルを採用した。また血漿中濃度と血小板推移の関係を説明するPDモデルには、血小板の成熟プロセスを説明する5-コンパートメントを用いたモデルを採用した。図1にPK/PDモデルの模式図をその物質収支式とともに示す。表1に健康成人のデータから推定したPK/PDモデルパラメータを示す。なお、各モデルパラメータの分布及びモデルの残差誤差の分布は対数正規分布に従うと仮定した。パラメータの推定にはソフトウェアNONMEM (version VI, level 2.0) のfirst order conditional estimation method with interactionを用いた。

モンテカルロシミュレーションにおけるPK/PDパラメータの設定
(試験例1)で述べた健康成人におけるPK/PDモデルを基本に、以下の仮定に基づき、慢性肝疾患患者におけるモデルパラメータを設定した。設定した慢性肝疾患患者における血小板数予測に用いたPK/PDモデルパラメータを表2に示す。

PK パラメータ (CL,V2/F,Q2/F,V3/F,Q3/F,V4/F,Q4/F,ktr,ka):
Lusutrombopag 0.75 mg投与後のChild-Pugh 分類A及びBの肝障害患者を対象とした第1相単回投与臨床試験の結果から、肝障害患者におけるlusutrombopagの薬物動態は健康成人と同様であったことから、PKパラメータは健康成人の値を用いた。なお、ka及びktr は臨床使用条件に合わせて食後と錠剤における推定値を用いた。
血小板数のベースライン (PLT0):
慢性肝疾患患者で想定される血小板数のベースラインを基に、PLT0の平均を3 万/μL、範囲を2 ~ 4.9 万/μLに設定した。
血小板コンパートメントからの消失速度定数 (kdeg):
慢性肝疾患患者の生体内における血小板の分解は亢進しており(参照文献1)、血小板の生存期間は健康成人の約2/3倍であるという報告から(参照文献2,3)、血小板の消失速度を表すkdegを健康成人の約1.5倍の値、すなわち0.009 hr-1に設定した。また、ベースラインにおいては血小板の産生速度と消失速度が一致するという関係に基づいて、血小板産生速度を表すkprを式PLT0 × kdegから算出した結果、0.027 万/μL/hrと健康成人の値 (0.116 万/μL/hr) よりも低い値であった。この設定は、慢性肝疾患患者において血小板の産生速度は低下しているという報告と一致している(参照文献1)。
PDコンパートメント間の移行速度定数 (km):
血小板の成熟速度を表すkm は、慢性肝疾患患者と健康成人との間で同様と仮定し、健康
成人の推定値を用いた。
シグモイドEmaxモデルのパラメータ (Emax, EC50, r):
血漿中濃度がEC50に対し十分に低いとき (EC50 >> C) のlusutrombopagの薬効はEmax/EC50 で近似することができる。本試験対象患者におけるEmax/EC50は、同効薬であるeltrombopagの臨床成績を基に(参照文献4,5)、健康成人の約3倍であると仮定した。さらに、本モデルにおける血小板の上限 (PLT0×(1+Emax)) は慢性肝疾患患者と健康成人とで同じと仮定し、Emaxを29.3、EC50を180 ng/mLに設定した。一方、rは患者と健康成人とで同様と仮定し、健康成人の値を用いた。
フィードバック係数 (γ):
式(PLT0/PLT)γで表される、血小板の増加に伴う血小板産生速度の低下 (負のフィードバック現象) は、血小板数が正常レベルである健康被験者特有の現象で、血小板のベースラインが低下した慢性肝疾患患者においては負のフィードバック現象は起こらないと仮定した。そのため、本シミュレーションにおいて負のフィードバック現象は考慮しなかった。
体重:
体重の分布は正規分布に従うと仮定し、平均を60 kg,変動係数を20%,範囲を40 から100 kg に設定した。
PD モデルパラメータの個体間変動:
各PD パラメータの個体間変動は、健康成人の値を用いた。
血漿中濃度及び血小板数の個体内変動:
血漿中濃度の個体内変動は、本シミュレーションでは考慮しなかった。血小板数の個体内変動は、健康成人の値を用いた。

BW (体重): 体重の分布は正規分布に従うと仮定し、平均を60 kg、変動係数を20%、範囲を40 ~ 100 kg に設定した。
PLT0: シミュレーションでは範囲を2.0 ~ 4.9 万/μLに設定した。
門脈血栓発現リスクを軽減する投与方法を検討することを目的として、上記(試験例2)で設定したパラメータを基に、lusutrombopagを1日1回2 mgを慢性肝疾患患者に7日間反復投与したときの血小板数の推移および、lusutrombopagを1日1回2 mgを慢性肝疾患患者に7日間反復投与するが、その投与期間中に血小板数が投与開始前より2万/μL以上増加しかつ5万/μL以上となった場合はそれ以降のlusutrombopagの投与を中止する基準を設けた場合の血小板数の推移を、それぞれモンテカルロシミュレーションで予測した。モンテカルロシミュレーションにはソフトウェアTrial Simulator (version 2.2.1) を用いた。その予測で算出された投与開始8日目の血小板数が投与開始前より2万/μL以上増加しかつ5万/μL以上となる患者の割合 (有効性) 及び観察期間中に血小板数が20万/μLを超える患者の割合 (血栓高リスク群の割合)を、表3に示す。
(表3の説明)
投与方法:2 mg 1日1回7日間投与。
a) モンテカルロシミュレーションによる有効性及び安全性の予測は、モンテカルロシミュレーションにより仮想患者1000人の経時的な血小板数を予測し、投与8日目の血小板数が投与開始前より2万/μL以上増加し、かつ5万/μL以上となる患者の割合及び観察期間中(投与開始より投与開始30日目まで)の血小板数が20万μLを超える患者の割合を算出した。
b) 投与中止基準:lusutrombopagの投与期間中に血小板数が投与開始前より2万/μL以上増加し、かつ5万/μL以上となった時点でlusutrombopagの投与を中止。
慢性肝疾患による血小板減少患者を対象に、経皮的肝癌焼灼術の前処置としてlusutrombopagを、上記実施例1で設定した中止基準を適用しつつ、7日間反復経口投与し、血小板数がどのように推移するかを調べた。なお、本実施例で焼灼術とはラジオ波焼灼療法を意味する。
経皮的肝癌焼灼術を予定している慢性肝疾患による血小板減少患者
下記の選択基準を満足する患者を対象とする。
1) 年齢:20歳以上 (同意取得時)
2) 本人から文書にて同意取得が可能な患者
3) B型又はC型肝炎ウイルスによる慢性肝疾患の合併又は既往がある患者
4) 原発性肝癌に対し、経皮的肝癌焼灼術を予定している患者
5) スクリーニング時の血小板数が、5万/μL未満の患者
6) ECOG*のPerformance StatusがGrade 0~1の患者
7) 登録以降、後観察終了時まで避妊が可能な患者
下記の基準に該当する患者は除外する。
1) 脾臓摘出術を受けた患者
2) 血小板減少を呈する合併症を有している患者
3) 肝移植を受けた患者
4) 肝障害の程度が、Child-Pugh分類のC
5) 薬剤コントロールが不可能な肝性脳症を有する
6) 薬剤コントロールが不可能な腹水を有する
7) 原発性肝癌以外の悪性腫瘍を合併している患者
8) 血栓症または血栓性疾患の合併又は既往がある患者
9) 門脈血流方向が求肝性でないことが判明した患者
10) 出血のリスクを有する疾患の合併又は既往のある患者
11) 登録前90日以内に、血小板数に影響を及ぼす治療薬の投与または療法を受けた患者
のいずれかを受けた患者
12) 登録前14日以内に、血液製剤 (赤血球製剤は除く) を投与された患者
13) 過去にTPO受容体アゴニストを投与されたことがある患者
治験実施中に併用を禁止、制限あるいは可能とする薬 (療法) は下記のとおりとする。
登録以降、後観察終了時まで、以下の薬剤の使用を禁止する。
1)血小板及び赤血球製剤以外の血液製剤
全血製剤、ヒト免疫グロブリン製剤、血液凝固因子製剤、フィブリノゲン、アンチトロンビンIII、新鮮凍結血漿等
2)抗癌剤
3)インターフェロン製剤
4)コロニー刺激因子 (G-CSF,M-CSF) 製剤
5)エリスロポエチン
6)TPO受容体アゴニスト
7)抗血栓薬
8)止血剤
カルバゾクロムスルホン酸ナトリウム等
9)ビタミンK
10)他の治験薬
それぞれ割付けられた用量群で、lusutrombopagを1日1回7日間反復経口投与し、投与開始8日目の観察終了後、投与開始13日目までの間に経皮的肝癌焼灼術を実施し、投与開始から約30日間観察する。
lusutrombopagを1日1回7日間反復経口投与する。投与開始1日目は来院時に投与する。なお、投与開始3~7日目(2.0 mg投与群は5~7日目)は、当日測定した血小板数を確認してから投与すること。また、治験薬投与後の臨床検査で以下の基準(投与中止基準)に該当する場合は、以後のlusutrombopagの投与を中止すること。
投与中止基準:血小板数が投与開始前と比較して2万/μL以上増加し、かつ血小板数が5万/μL以上となる場合
なお、投与開始前の血小板数としては、投与開始前7日以内に測定した血小板数を用いた。
一日あたりlusutrombopagとして2.0,2.5,3.0及び4.0 mgのいずれかを投与した。
評価項目1
投与開始8日目の血小板数が、投与開始前より2万/μL以上増加、かつ5万/μL以上となる患者の割合

各用量における有効性に関するデータを表4に示す。一日あたりlusutrombopagとして2.0、2.5、3.0 mg及び4.0 mgの用量での試験の個々の被験者での血小板数の推移を図2~5にそれぞれ示す。いずれの用量においても有効性が確認された。また、血小板数が観察期間中に20 万/μL を超える患者は何れの試験においても皆無であった。特に一日あたり2 mg、2.5 mg及び4.0 mg投与群においては、本発明にかかる投与中止基準に従って投与を中止した患者において、投与中止後においても一定期間血小板数の増加が見られたが、その最大値は7日間投与を受けた患者の最大値に比べ小さいものであった。本発明にかかる投薬中止基準に従って投薬を中止したことにより、過剰な血小板の増加を防止できたと考えられる。本試験の結果から、十分な血小板数が確保しつつ、過剰な血小板数の上昇は起こらず、本発明にかかる投与中止基準が優れていることが実証された。
(表4の説明)

以下に示す製剤例は例示にすぎないものであり、発明の範囲を何ら限定することを意図するものではない。
製剤例1 錠剤
本発明化合物 15mg
乳糖 15mg
ステアリン酸カルシウム 3mg
ステアリン酸カルシウム以外の成分を均一に混合し、破砕造粒して乾燥し、適当な大きさの顆粒剤とする。次にステアリン酸カルシウムを添加して圧縮成形して錠剤とする。
本発明化合物 10mg
ステアリン酸マグネシウム 10mg
乳糖 80mg
を均一に混合して粉末又は細粒状として散剤をつくる。それをカプセル容器に充填してカプセル剤とする。
本発明化合物 30g
乳糖 265g
ステアリン酸マグネシウム 5g
をよく混合し、圧縮成型した後、粉砕、整粒し、篩別して適当な大きさの顆粒剤とする。
Claims (22)
- トロンボポエチン受容体アゴニスト作用を有する化合物を含有し、
投与される患者の血小板数が、
投与開始前と比較して2万/μL以上増加し、かつ、
5万/μL以上となった場合は、以後の投与を中止することを特徴とする、
血小板増加のための医薬組成物。 - 血小板減少患者に対して投与される、請求項1記載の医薬組成物。
- 当該医薬組成物が投与される患者の投与前の血小板数が、5万/μL未満である、請求項1または2記載の医薬組成物。
- 前記の投与される患者が、侵襲術の実施前の患者である、請求項2または3記載の医薬組成物。
- 前記の侵襲術が待機的侵襲術である、請求項4記載の医薬組成物。
- 前記の侵襲術がラジオ波焼灼療法の施術である、請求項4または5記載の医薬組成物。
- 前記のラジオ波焼灼療法が、肝癌に対するものである、請求項6記載の医薬組成物。
- 前記の肝癌が、原発性肝癌である、請求項7記載の医薬組成物。
- 前記の投与される患者が慢性肝疾患の患者である、請求項1~8のいずれかに記載の医薬組成物。
- 前記の慢性肝疾患がB型肝炎ウイルスまたはC型肝炎ウイルスによる慢性肝疾患である、請求項9記載の医薬組成物。
- 前記のトロンボポエチン受容体アゴニスト作用を有する化合物が(E)-3-[2,6-Dichloro-4-[4-[3-[(S)-1-hexyloxyethyl]-2-methoxyphenyl]-thiazol- 2-ylcarbamoyl]-phenyl]-2-methylacrylic acidまたはその塩である、請求項1~10のいずれかに記載の医薬組成物。
- 経口投与することを特徴とする、請求項11記載の医薬組成物。
- (E)-3-[2,6-Dichloro-4-[4-[3-[(S)-1-hexyloxyethyl]-2-methoxyphenyl]-thiazol-2-ylcarbamoyl]-phenyl]-2-methylacrylic acidまたはその塩の量として2.0~4.0mg/日を投与されることを特徴とする、請求項12記載の医薬組成物。
- 1日1回投与で最大投与期間が14日間以下の期間であることを特徴とする請求項13記載の医薬組成物。
- 血小板減少症を伴う慢性肝疾患における侵襲術実施前の血小板産生促進のための、請求項1記載の医薬組成物。
- 血小板減少症を伴う慢性肝疾患における待機的侵襲術実施前の血小板産生促進のための、請求項1記載の医薬組成物。
- 原発性肝癌に対する局所療法の施術前の血小板産生促進のための、請求項1記載の医薬組成物。
- 原発性肝癌に対するラジオ波焼灼療法の施術前の血小板産生促進のための、請求項1記載の医薬組成物。
- 血小板減少症を伴う慢性肝疾患における各種低侵襲性観血的治療前の血小板産生促進のための、請求項1記載の医薬組成物。
- トロンボポエチン受容体アゴニスト作用を有する化合物を含有する医薬組成物を投与し、
投与される患者の血小板数が、
投与開始前と比較して2万/μL以上増加し、かつ、
5万/μL以上となった場合は、以後の投与を中止することを特徴とする、
