Abstract
Procedimiento para fotografía micro estereoscópica de la retina caracterizado por la combinación en dos ejes, X e Y, mediante la manipulación de las partes móviles de un retinógrafo, situando su óptica a la distancia axial de enfoque sobre la retina en posición primaria, captando seguidamente imágenes en dos pases estereoscópicos sucesivos, izquierdo superior y derecho inferior, visualizándose el resultado mediante un visor paralelo con ajuste interpupiiar. Dado que las fibras nerviosas de la retina son longitudinales, casi no revelarían esteriopsis en el eje X, pero si en el Y, por lo tanto la combinación de ambos, objeto de la presente invención, es idónea para poder apreciar la morfología tridimensional de un objeto que no es apreciable en el eje X simétrico horizontal.

PROCEDIMIENTO PARA FOTOGRAFÍA MICRO ESTEREOSCÓPICA DE LA RETINA
La presente memoria descriptiva se refiere a un procedimiento para fotografía micro estereoscópica de Ia retina caracterizado por Ia combinación en dos ejes, X e Y, mediante Ia manipulación de las partes móviles de un retinógrafo, situando su óptica a Ia distancia axial de enfoque sobre Ia retina en posición primaria, captando seguidamente imágenes en dos pases estereoscópicos sucesivos, izquierdo superior y derecho inferior, visualizándose el resultado mediante un visor paralelo con ajuste interpupilar. Dado que las fibras nerviosas de Ia retina son longitudinales, casi no revelarían esteriopsis en el eje X, pero si en el Y, por Io tanto Ia combinación de ambos, objeto de Ia presente invención, es idónea para poder apreciar Ia morfología tridimensional de un objeto que no es apreciable en el eje X simétrico horizontal.
La correcta apreciación de Ia morfología tridimensional de Ia retina, objetivo de Ia presente invención, puede tener aplicaciones diversas, siendo de utilidad sus resultados para el control diagnostico de patologías morfológicas y funcionales de las estructuras del cuerpo humano registradas en oftalmología.
La imagen final obtenida mediante Ia fotografía micro estereoscópica de Ia retina, con el correspondiente soporte fotográfico en película, diapositiva o sistema digital, puede ser utilizada posteriormente para el control diagnóstico de patologías morfológicas y funcionales de las estructuras registradas en oftalmología. En consecuencia se trata de proveer una tecnología que facilite, proveyendo un resultado intermedio, Ia tarea diagnóstica posterior por profesionales oftalmológicos.

En Ia actualidad la fotografía micro estereoscópica que realizan los distintos retinógrafos comerciales carecen de un protocolo estandarizado, y no se acogen a las fórmulas ópticas que garanticen un grado de esteriopsis equivalente como puede ser necesario en un estudio multicéntrico internacional, en retinopatía diabética o excavación papilar. Es más, poniendo como ejemplo las imágenes estereoscópicas que son tomadas a un mismo paciente en seguimiento del glaucoma con el mismo retinógrafo, pueden tener distintos grados de esteriopsis, (capacidad de visión binocular del ojo humano) al carecer de un protocolo estandarizado, Io cual puede inducir a error en Ia evolución de Ia patología, siendo su técnica distinta en función de los resultados que se desee obtener; así las técnicas pueden dividirse en dos clases:
. Emulación estereoscópica humana. . Fotografía hiper estereoscópica.
Las imágenes obtenidas mediante Ia emulación estereoscópica humana, permite por ejemplo, al oftalmólogo, observar las imágenes tridimensionales de forma análoga a como se verían en una observación directa mediante el oftalmoscopio binocular. Siendo útil para observar cambios morfológicos, como el edema macular diabético o Ia retinopatía diabética, así como, las alteraciones de Ia cabeza del nervio óptico.
Las imágenes obtenidas mediante Ia fotografía hiper estereoscópica permite el aumento proporcional por necesidades de resaltar Ia esteriopsis, obteniéndose un relieve que mantiene las proporciones para evitar errores en Ia evolución del diagnostico diferencial.
La fotografía estereoscópica se basa en el principio de separación interpupilar de los ojos, Ia cual es distinta en función de Ia especie animal. Como es evidente, las técnicas están basadas en Ia

separación interpupilar humana, Ia cual está situada en 65mm de media.
El área media de disco óptico normal varia desde 2,1 mm2 a 2,9 mm2 en raza caucasiana siendo mayor en otras razas como en negros americanos (2,63 a 2,94 mm2). Ejemplos:
OD diámetro vertical/horizontal 1,52 xl,58 mm. OI diámetro vertical/horizontal 1,48 xl.50 mm. OD diámetro vertical/horizontal 1,95 xl,97 mm.
OI diámetro vertical/horizontal 1,88 x 2,04 mm. OD diámetro vertical/horizontal 2,00 x 2,15 mm. OI diámetro vertical/horizontal 1,96 x 2,19 mm.
Estos datos son fundamentales, ya que son necesarias unas medidas de referencia para poder aplicar las fórmulas que nos permitan realizar un perfil y protocolo personalizado para cada retinógrafo, software y hardware. La visión estereoscópica humana esta limitada en función de Ia distancia, para que el efecto tridimensional sea apreciable, los objetos no deberán de estar a más de 60 metros, esta distancia está vinculada a Ia distancia interpupilar media de 65 mm.
Así nos encontramos con dos ejemplos tales como Ia fotografía macro estereoscopia y micro estereoscopia que siguen perfectamente los patrones fundamentales de referencia pero en aplicaciones diferentes, así Ia fotografía macro estereoscopia se utiliza para Ia toma de fotografías estereoscópicas aéreas destinadas a los relieves topográficos y su fórmula se basa en dividir por 50 Ia distancia del avión al suelo, siendo el resultado Ia proporción a Ia distancia interpupilar media de 65mm.

La micro estereoscopia es el caso que compete a Ia microfotografía, cuya relación es inversa a Ia hiper estereoscopia, y consiste en reducir proporcionalmente Ia distancia interpupilar media de 65mm en función de Ia magnificación de Ia imagen, y es aquí donde nos ayudaremos del área media de disco óptico normal para calcular Ia hipo-distancia interpupilar que se halla dividiendo Ia distancia interpupilar media de 65mm por el rango de magnificación.
Todos los parámetros indicados se deben determinar bajo Ia tutela de unos protocolos establecidos y asumidos por toda Ia comunidad científica, así como, Ia creación de un organismo que tutele y homologue los equipos de imagen para los distintos protocolos. Adicionalmente las soluciones técnicas existentes en Ia actualidad generan imágenes no exactamente conformes con Ia realidad morfológica de Ia retina, precisando corregir tal indeterminación con enormes dosis de valoración, con el correspondiente menoscabo en Ia seguridad técnica del resultado intermedio así obtenido.
Para solventar Ia problemática existente en Ia actualidad se ha ideado un procedimiento para fotografía micro estereoscópica combinada de Ia retina en dos ejes, X e Y, caracterizado por Ia incorporación de estos factores diferenciadores frente a Ia técnica fotográfica bidimensional, solamente en el eje X, introduciendo por un lado, Ia imagen tridimensional bajo protocolo estandarizado, , introduciendo por otro lado Ia esteriopsis combinada con el eje "Y".
Dado que las fibras nerviosas de Ia retina son longitudinales, casi no revelarían esteriopsis en el eje X, pero si en el Y, por Io tanto Ia combinación de ambos, objeto de Ia presente invención, es idónea para poder apreciar Ia morfología tridimensional de un objeto que no es apreciable en el eje X simétrico horizontal.

La justificación de este nuevo eje "Y", se basa en el ángulo de Ia disposición de los axones y su recorrido desde las ramas temporales hasta Ia entrada en el nervio óptico en Ia zona nasal, el cual dispone a los haces de axones en giro horizontal.
Es por ello que se pretende crear varios protocolos 0 para lograr que imágenes tomadas, (a modo de resultado intermedio) en cualquier parte del planeta y con independencia del modelo y marca comercial de retinógrafo o dispositivo similar utilizado, puedan ser valoradas con Ia misma objetividad.
En el caso de las imágenes microscópicas de Ia retina, Ia apreciación de Ia tridimensionalidad puede ser complicada si queremos diferenciar relieves topográficos referentes a unas pocas mieras, ver figura
Para conseguir una imagen tridimensional, según el procedimiento fotográfico objeto de Ia presente invención, se realizan dos fotografías con una separación determinada en el eje horizontal X, que luego serán visionadas con alguno de los diferentes sistemas de visión en 3d, que básicamente fusionan las dos imágenes bidimensionales en una sola tridimensional.
Para realizar el procedimiento de fotografía micro estereoscópica combinada de Ia retina en dos ejes, X e Y, se debe colocar al sujeto sobre Ia mentonera destinada en el retinógrafo para proporcionar el apoyo, fijación e inmovilización de Ia cabeza, seguidamente se sitúa Ia óptica a Ia distancia axial de enfoque sobre Ia retina, regulando el retinógrafo en posición primaria.

Se procede seguidamente a Ia bascularización de las partes móviles del retinógrafo fotografiando en los pases estereoscópicos izquierdo superior y derecho inferior.
Para Ia visualización tridimensional de las fotografías usaremos un visor paralelo con ajuste interpupilar.
Este procedimiento para fotografía micro estereoscópica combinada de Ia retina en dos ejes, X e Y que se presenta aporta múltiples ventajas sobre los sistemas disponibles en Ia actualidad siendo Ia más importante que permite obtener Ia visión para poder observar Ia morfología tridimensional de un objeto que no es apreciable en el eje X simétrico horizontal, obteniendo un óptimo resultado intermedio para ser manejado, en su caso, por un profesional oftalmológico en el desempeño de sus funciones.
Como ventaja importante podemos decir que este procedimiento permite crear varios protocolos, para lograr que imágenes, a modo de resultados intermedios, tomadas en cualquier parte del planeta y con independencia del modelo y marca comercial de retinógrafo o dispositivo similar utilizado, puedan ser valoradas con Ia misma objetividad.
Para comprender mejor el objeto de Ia presente invención, en el plano anexo se ha representado una realización práctica preferencial de Ia misma:

El procedimiento para fotografía micro estereoscópica combinada de Ia retina en dos ejes, X e Y, objeto de Ia presente invención, se realiza colocando al paciente sobre Ia mentonera destinada en el retinógrafo para proporcionar el apoyo, fijación e inmovilización de Ia cabeza, habiendo preparado con anterioridad al paciente con Ia medicación apropiada para Ia dilatación pupilar.
Seguidamente se sitúa Ia óptica a Ia distancia axial de enfoque sobre Ia retina, según el protocolo estandarizado, regulando el retinógrafo en posición primaria.
Se procede seguidamente a Ia bascularización de las partes móviles del retinógrafo fotografiando en los pases estereoscópicos izquierdo superior y derecho inferior según el protocolo estandarizado a realizar.
Para Ia visualización tridimensional de las fotografías usaremos un visor paralelo con ajuste interpupilar.

1 - Procedimiento para fotografía micro estereoscópica de Ia retina caracterizado por Ia combinación en dos ejes, X e Y, mediante Ia manipulación de las partes móviles de un retinógrafo, situando su óptica a Ia distancia axial de enfoque sobre Ia retina en posición primaria, captando seguidamente imágenes en dos pases estereoscópicos sucesivos, izquierdo superior y derecho inferior, visualizándose el resultado mediante un visor paralelo con ajuste interpupilar.
