These antimicrobial agents are gentle and do not destroy healthy gum tissue. Other chemical-based mouth rinses use alcohol to effectively kill germs, but may dry the mouth or cause sloughing of gum tissue.

Description

Natural Antigingivitis and Antiplaque Mouth Rinses, and Methods of Use Thereof
RELATED APPLICATIONS

BACKGROUND OF THE INVENTION
According to the National Institutes of Health, about 80 percent of U.S. adults currently have some form of gum or periodontal disease. It begins when the bacteria in plaque causes the gums to become inflamed and bleed easily. Periodontal disease often reflects serious health risks. The disease ranges from simple gum inflammation (gingivitis) to serious disease that results in tooth loss and damage to the bone structures that support the teeth (periodontitis). Periodonitis may be associated with heart disease, stroke, and systemic infections. There is also evidence that premature births happen more often to women who have gum disease before or during their pregnancies.
There has been a significant increase in the last decade in the use of alternative medicine, including use of natural or herbal oral health care products. An estimated one- third to one -half of all Americans practice some form of alternative medicine, including use of natural or herbal oral hygiene products. The use of these products has increased significantly in the last decade. American consumers who gravitate toward using natural or herbal products often view these products as being safer than conventional products. Consumers are also interested in natural oral hygiene products because they can be produced from renewable resources. Oral hygiene products labeled as "natural" typically do not include synthetic ingredients, such as synthetic sweeteners, artificial colors, preservatives, additives, and synthetic flavors and fragrances. The term "herbal" on the label implies that most of a product's active ingredients are plant-based. It is highly desirable to this consumer group that oral hygiene compositions consist essentially of herbal agents, and do not have undesirable materials, such as those that are primarily stimulants, depressants, or the like, as active ingredients. Moreover, certain herbal ingredients have been shown to have antimicrobial properties. Certain herbs (e.g., some of those used in traditional Chinese medicine for colds, virus, infection, cancer, mumps, hepatovirus, and chronic fatigue) have been shown clinically to be effective as antimicrobials. Further, herbal ingredients have been used to treat bone and joint inflammation, especially in eastern countries, and they are increasingly used for this purpose in western countries. For example, a number of compositions composed of natural ingredients and said to be useful in reducing inflammation have been disclosed. U.S. Pat. No. 5,494,668 (Patwardhan, incorporated herein by reference) discloses a method of treating degenerative musculoskeletal diseases such as rheumatoid arthritis and osteoarthritis in an animal, typically a human, involving administering (typically enterally) extracts of the ashwagandha, sallai Guggul, turmeric, and ginger plants, in a predetermined proportion relative to each other with or without other biologically active inorganic ingredients. U.S. Pat. No. 5,683,698 (Chavali et al., incorporated herein by reference) discloses an orally administered herbal formulation for reducing or alleviating symptoms associated with rheumatoid arthritis, osteoarthritis, and reactive arthritis and for reducing the production of pro-inflammatory cytokines, wherein the formulation contains herbal extracts taken from Alpinia, Smilax, Tinospora, Tribulus, Withania, and Zingiber plants. U.S. Pat. No. 5,916,565 (Rose et al., incorporated herein by reference) discloses an orally administered composition for prophylaxis and therapy of joint and connective tissue disorders in vertebrates, wherein the composition contains metabolic precursors, herbal phytochemicals, and palatability agents. Specific herbal phytochemicals disclosed include cayenne, ginger, turmeric, yucca, Devil's claw, nettle leaf, Black Cohosh, alfalfa and celery seeds. U.S. Pat. No. 5,854,291 (Laughlin et al., incorporated herein by reference) discloses a topically- applied pain reliever composition for treating such discomforts as arthritis pain, the composition being composed of capsaicin and, optionally, a plant extract selected from the group consisting of nettle extract, yarrow extract, coltsfoot extract, birch extract, rosemary extract, horsetail extract, ginger extract, chamomile extract, comfrey extract, lavender extract, and bergamot extract. U.S. Pat. No. 5,910,307 (Kwak, et al., incorporated herein by reference) discloses a combined medicinal plant composition for alleviating acute/chronic inflammation, composed of Clematis Radix, Trichosanthes root, and Prunella Herba (which contains oleanolic acid ursolic acid) in a certain ratio. SUMMARY OF THE INVENTION
One aspect of the present invention relates to oral hygiene compositions comprising a betaine derived from renewable natural resources as a natural flavor emulsifϊer and surfactant. One aspect of the present invention relates to an oral hygiene composition, comprising water; vegetable glycerin; aloe vera gel; Echinacea; Goldenseal; Calendula; citric acid; potassium citrate, grapefruit seed extract; at least one natural flavor; at least one natural colorant; and at least one natural betaine.
Another aspect of the present invention relates to an oral hygiene composition, consisting essentially of water; vegetable glycerin; aloe vera gel; Echinacea; Goldenseal; Calendula; citric acid; potassium citrate; grapefruit seed extract; at least one natural flavor; at least one natural colorant; and at least one natural betaine.

The present invention also relates to the use of a composition of the invention to treat or prevent dental caries, oral microbial infections, oral inflammation, dentine hypersensitivity, or any of combination thereof.
The present invention also relates to a kit, comprising a composition of the invention; and instructions for use thereof.
DETAILED DESCRIPTION OF THE INVENTION
Anti-gingivitis/ Anti-plaque Agent
Many commercially available oral rinses claim antigingivitis/antiplaque efficacy based on cetylpyridinium chloride or an essential oil blend consisting of menthol, methyl salicylate, thymol and eucalyptol. Cetylpyridinium chloride, while recognized as effective, may cause side effects including teeth-staining and taste impairment. The essential oil blend used in many mouth rinses is also effective, however, it requires upwards of 27% alcohol to solubilize the essential oils. Consumers who want to avoid alcohol and cetylpyridium chloride have limited choices. The Antigingivitis/Antiplaque monograph is currently a Tentative Final Monograph at the FDA. Aloe vera gel at a minimum of 20% is recognized as a Category III active for antigingivitis/antiplaque function. Currently, there are no oral rinses on the market which apply aloe vera gel at 20% for an antigingivitis/antiplaque use. Possible reasons for not using aloe vera include limitations of using aloe vera gel IX which may include off-taste and odor. One aspect of the present invention relates to the use of an aloe vera gel powder which has been freeze-dried and concentrated to about 200X. As a result of the concentration process, the aloe vera gel is cleaner in taste and economically feasible to use in the final formulation. We have been able to quantify the aloe vera gel in both fresh and aged samples of the product, thereby establishing that the aloe vera concentration is 20% in the finished product over the recommended shelf- life of the oral rinse product.
Surfactants
Surfactants make it possible for oil-based ingredients to dissolve in a water-based solution. A surfactant molecule is characterized by its two distinct functional properties: a polar, hydrophilic property and a nonpolar, hydrophobic property. This dual receptivity allows oils to be dissolved into solution with water. Surfactants are added to the oral rinse compositions to serve as natural flavor emulsifϊers to allow the natural oil flavoring agents to mix with the aqueous ingredients of the composition. While some manufacturers of natural oral rinse compositions use surfactants derived from petroleum products (e.g., Poloxomer 335 and 407), the compositions of the present invention use betaines derived from renewable natural resources. Examples of natural betaines for use in the invention include, but are not limited to cocamidopropyl betaine, olivamidopropyl betaine or soyamidopropyl betaine.
Demulcents A demulcent is an agent, such as an oil, that forms a soothing film when administered onto the surface of a mucous membrane. A demulcent is meant to relieve the irritation of an inflamed mucous membrane. Demulcent herbs often have a high content of mucilage, and they have often been used to soothe and protect irritated or inflamed internal tissues of the body. The FDA recognizes glycerin as an oral demulcent that soothes sore gums and mouth irritations. Natural Flavors
The term "natural flavor" or "natural flavoring" means the essential oil, oleoresin, essence or extractive, protein hydro lysate, distillate, or any product of roasting, heating or enzymolysis, which contains the flavoring constituents derived from a spice, fruit or fruit juice, vegetable or vegetable juice, edible yeast, herb, bark, bud, root, leaf or similar plant material, meat, seafood, poultry, eggs, dairy products, or fermentation products thereof, whose significant function in food is flavoring rather than nutritional. Natural flavors include the natural essence or extractives obtained from the plants listed in 21 CFR §§ 182.10, 182.20, 182.40, and 182.50; the direct food substances listed in 21 CFR § 184; and the substances listed in 21 CFR § 172.510, "Natural flavoring substances and natural substances used in conjunction with flavors"; all of the preceding sections of the Code of Federal Regulations are hereby incorporated in their entirety by reference.
According to 21 CFR § 172.510, natural flavoring substances and natural adjuvants may be safely used in food in accordance with two conditions. First, they are used in the minimum quantity required to produce their intended physical or technical effect and in accordance with all the principles of good manufacturing practice. Second, in the appropriate forms (plant parts, fluid and solid extracts, concretes, absolutes, oils, gums, balsams, resins, oleoresins, waxes, and distillates) they consist of material from an approved source, used alone or in combination with flavoring substances and adjuvants generally recognized as safe in food, previously sanctioned for such use, or regulated in any section of 21 CFR. Natural Colorants
In addition to beta-carotene, the FDA has permitted many color additives for foods (21 CFR § § 73.1-73.615) derived from natural sources, such as annatto, carrot oil, oleo resins turmeric and paprika, corn gluten (endosperm) oil, saffron and the like. All of the preceding sections of the Code of Federal Regulations are hereby incorporated in their entirety by reference. In certain embodiments, a natural food colorant is or is derived from a fruit or vegetable extract, carmine, carotene, annatto, turmeric, curcumin, raspberry, cherry, blueberry or other natural food colors known in the art.
Selected Aloe Vera Gel-Containing Compositions of the Invention Naturally occurring antimicrobial agents — Echinacea, Goldenseal, and grapefruit seed extract — can be used to inhibit the growth of germs in the mouth. These antimicrobial agents are gentle and do not destroy healthy gum tissue. Other chemical-based mouth rinses use alcohol to effectively kill germs, but may dry the mouth or cause sloughing of gum tissue. Vegetable glycerin and aloe vera gel provide a soothing and anti-inflammatory effect on the gums. Vegetable glycerin acts as a protective barrier whereas aloe vera gel serves as a moisturizer. Xylitol also provides a soothing effect on the gums, as well as flavor and antiplaque properties. Echinacea can be used to treat inflammation of the mouth and pharynx. Echinacea is analgesic and has anti-inflammatory and anti-microbial properties. Echinacea angustifolia and Echinacea purpurea may be used. Goldenseal, such as Hydrastis canadensis is found in plant extracts and has antiseptic and antimicrobial properties. Calendula {Calendula officinalis flower) is used to treat inflammation of the oral and pharyngeal mucosa. Grapefruit seed extract has anti-fungal and antimicrobial properties. Citrus paradisi seed may be used. Witch hazel can be used in the mouth rinse to provide an astringent feel to the mouth. Sodium bicarbonate (baking soda) can also be added as a flavor agent and breath freshener.

Such kits provide a convenient and effective means for assuring that the methods are practiced in an effective manner. The compliance means of such kits includes any means which facilitates practicing a method of this invention. Such compliance means include instructions, packaging, and dispensing means, and combinations thereof. Kit components may be packaged for either manual or partially or wholly automated practice of the foregoing methods.

Natural Dentist Inc. has discovered an effective natural oral rinse containing herbs having anti-inflammatory and antimicrobial properties and does not contain emulsifiers or surfactants derived from petroleum products. The natural oral rinse contains emulsifiers derived from olives and is equally as effective in dispersing natural flavor oils as petroleum-based products. Many adults who undergo oral surgery, or periodontal procedures would benefit from the rinse. Below is the formulation for orange zest natural oral rinse and peppermint twist natural oral rinse and the composition of unflavored concentrate 8339G.

Example 3 An in vitro antimicrobial study to demonstrate the effectiveness of the new mouth rinse against a series of oral pathogen was performed by Dr. Anne Haffajee. To permit comparisons of efficacy among test agents, the data are usually expressed as minimum inhibitory concentrations (MICs) of the test agent(s) against a range of microbial species of interest. The MIC is usually performed using the broth or agar dilution techniques; i.e., serial dilutions of the agent(s) are incorporated in agar (or broth) media and the lowest concentration of the agent that completely inhibits growth of a given strain is recorded as the MIC.
While the methods are routine, occasionally one encounters "pitfalls" for some agents or combinations of agents. These include: a) insolubility of the agent(s) in the test medium, b) unforeseen interaction of a medium constituent with one or more of the test agents (e.g., precipitation) or c) volatility of an important ingredient of the test mixture (such as alcohol).
Another important consideration is the range of expected minimum inhibitory concentrations. For antibiotics this would typically be in the microgram or sub microgram per mL range, for other antibacterials the range might be much higher leading to difficulty in dissolving the test product. Nonetheless, if possible to perform on a given formulation, the MIC is the most reliable and easily interpreted method of comparison of formulations in use today. Methods
An agar dilution method was employed to assess the inhibitory effect of the test agents. All MICs were performed in duplicate. Strains employed
The strains employed are summarized in the accompanying Table. The numbers next to each species indicate the strain designation and an asterisk symbol indicates that the component was added after autoclaving. The basal medium is composed of the following constituents:

The basal medium was sterilized in individual tubes (1 per agar plate) by autoclaving at 121 C for 15 min and then placed in a 50 0C water bath. The volume in each tube was adjusted so that the final volume of about 20 mL was obtained when the filter- sterilized test agents were added.
Preparation of test media The test agents supplied by Natural Dentist were filter-sterilized by passage through a 0.22 μm Nalgene filter. Dilutions of each agent were prepared to provide final concentrations of 1, 2, 4, 8, 16, 32, 64, 128, 256 and 512 μg/mL.
The media was aseptically dispensed in a total of 20 mL in 100 mm plastic petri plates, allowed to set and used the next day. Preparation of inoculum
The organisms were grown on blood agar plates under an atmosphere of 80% N2, 10% H2 and 10% CO2 at 35 0C for 3 days. Growth was aseptically harvested from the plate and suspended in 1 mL of sterile Mycoplasma broth. Each suspension was adjusted to an optical density approximating a MacFarland #0.5 standard. Inoculation of test media
100 μL of each strain suspension were placed in the wells of a sterile microtiter plate. A MIC 2000 inoculator (Dynatech) was used to transfer 1.5 μL of inoculum to the surface of the prepared plates. Control plates, without the test agents, were inoculated at the beginning and end of the series of inoculations. These plates were used to indicate that the inoculum is present and viable throughout the series.
Reading of plates and interpretation of data
The plates were incubated under an atmosphere of 80% N2, 10% H2 and 10% CO2 at 35 0C and evaluated daily. The MIC was interpreted as the lowest concentration of the agent that completely inhibits the growth of the test species.
Results
The following table compares the lowest concentration of the agent that completely inhibits the growth of a given strain for Listerine and the Peppermint Twist natural mouth rinse. The test was performed using the method presented in Example 2. The results indicate that the new natural mouth rinse requires lower concentrations of the agents to inhibit the growth of a number of given species, such as A. israelii, F. nuc. Nucleatum, S. salivarius, and is much more potent compared to the Listerine. Overall, this study shows that the new natural mouth rinse is highly effective at killing the microorganisms that cause bad breath.

Example 4 Additional Natural Oral Rinse Formulations (Prophetic Example)
Additional natural oral rinse formulations with anti-inflammatory and antimicrobial properties will be investigated. Examples of additional formulations are listed below, along with a range of estimated weight percentages for the individual ingredients. The weight percent of all the ingredients should total 100 percent of the composition. Additional ingredients may be added to the proposed formulations such as zinc chloride for tartar control, fluoride to prevent tooth decay, preservatives, antioxidants, and stabilizers.

The oral hygiene composition of any one of claims 100-111, wherein water is about 60-80% by weight of said composition; vegetable glycerin is about 15-30% by weight of said composition; Echinacea is about 0.5-5% by weight of said composition; aloe vera gel is about 1-5% by weight of said composition; Goldenseal is about 0.1-3% by weight of said composition; Calendula is about 0.1-3% by weight of said composition; citric acid is about 0.1-3% by weight of said composition; said natural flavor is orange in about 0.01-2% by weight of said composition; grapefruit seed extract is about 0.01-2.0% by weight of said composition; said natural colorant is red in about 0.01-3% by weight of said composition; and said natural betaine is about 0.01-1% by weight of said composition.

The oral hygiene composition of any one of claims 100-111, wherein water is about 60-80% by weight of said composition; vegetable glycerin is about 15-30% by weight of said composition; Echinacea is about 0.5-5% by weight of said composition; aloe vera gel is about 1-5% by weight of said composition; Goldenseal is about 0.1-3% by weight of said composition; Calendula is about 0.1-3% by weight of said composition; citric acid is about 0.1-3% by weight of said composition; said at least one natural flavor is orange in about 0.01-2% by weight of said composition; grapefruit seed extract is about 0.01-2.0% by weight of said composition; said natural colorant is red in about 0.01-3% by weight of said composition; said natural betaine is about 0.01-1% by weight of said composition and said at least one natural preservative is 0.01-0.5% by weight of said composition.

The oral hygiene composition of any one of claims 100-111, wherein water is about 69% by weight of said composition; vegetable glycerin is about 24% by weight of said composition; Echinacea is about 1.5% by weight of said composition; aloe vera gel is about 3% by weight of said composition; Goldenseal is about 0.7% by weight of said composition; Calendula is about 0.7% by weight of said composition; citric acid is about 0.8% by weight of said composition; said natural flavor is Orange Zest (487927) in about 0.05% by weight of said composition; grapefruit seed extract is about 0.2% by weight of said composition; said natural colorant is red in about 0.09% by weight of said composition; and olivamidoproyl betaine is about 0.14% by weight of said composition.

The oral hygiene composition of any one of claims 112-117, wherein water is about 60-80% by weight of said composition; vegetable glycerin is about 15-30% by weight of said composition; witch hazel is about 0.1-5% by weight of said composition; aloe vera gel is about 1-5% by weight of said composition; sodium bicarbonate is about 0.1-5% by weight of said composition; xylitol is about 0.1-3% by weight of said composition; natural flavor is about 0.01-2% by weight of said composition; and natural betaine is about 0.01- 1 % by weight of said composition.

The oral hygiene composition of any one of claims 100- 117, wherein water is about 60-80% by weight of said composition; vegetable glycerin is about 15-30% by weight of said composition; witch hazel is about 0.1-5% by weight of said composition; aloe vera gel is about 1-5% by weight of said composition; natural flavor is about 0.01-2% by weight of said composition; natural betaine is about 0.01-1% by weight of said composition; and ascorbic acid is about 0.1-2% by weight of said composition.
219. The oral hygiene composition of any one of claims 100-117, wherein the pH ranges from about 3.0 to about 10.5.
220. The oral hygiene composition of any one of claims 100- 117, wherein the pH ranges from about 3.0 to about 7.0.
221. The oral hygiene composition of any one of claims 100- 117, wherein the pH ranges from about 3.0 to about 5.0.
222. The oral hygiene composition of any one of claims 100-117, wherein the pH ranges from about 3.0 to about 4.0.
223. The oral hygiene composition of any one of claims 100- 117, wherein the pH ranges from about 3.0 to about 3.5.
224. The oral hygiene composition of any one of claims 100-117, wherein the pH is about 3.1.
225. A method of treating or preventing dental caries, comprising applying to the oral cavity of a mammal in need thereof a therapeutically effective amount of a composition of any one of claims 1-224.
226. A method of treating or preventing oral microbial infections, comprising applying to the oral cavity of a mammal in need thereof a therapeutically effective amount of a composition of any one of claims 1-224.
227. A method of treating or preventing oral inflammation, comprising applying to the oral cavity of a mammal in need thereof a therapeutically effective amount of a composition of any one of claims 1-224.
228. A method of treating dentine hypersensitivity, comprising applying to the oral cavity of a mammal in need thereof a therapeutically effective amount of a composition of any one of claims 1-224.
229. A method of treating or preventing dental caries and oral microbial infections, comprising applying to the oral cavity of a mammal in need thereof a therapeutically effective amount of a composition of any one of claims 1-224.
230. A method of treating or preventing dental caries and oral inflammation, comprising applying to the oral cavity of a mammal in need thereof a therapeutically effective amount of a composition of any one of claims 1-224.
231. A method of treating or preventing oral microbial infections and oral inflammation, comprising applying to the oral cavity of a mammal in need thereof a therapeutically effective amount of a composition of any one of claims 1 -224.
232. The method of any one of claims 225-231 , wherein said mammal is a primate, bovine, ovine, caprine, equine, porcine, rodent, feline, or canine.
233. The method of any one of claims 225-231 , wherein said mammal is a primate, equine, feline, or canine.
234. The method of any one of claims 225-231 , wherein said mammal is a human.
235. A kit, comprising : a container comprising a composition of any one of claims 1-224; and instructions for use thereof.
