Abstract
Cette invention permet non seulement aux fumeurs d'arrêter de consommer du tabac, sans limitation de durée et sans encourir d'effets indésirables importants, mais aussi aux consommateurs de drogues toxicomanogènes en général (opiacés, psychostimulants, alcool) d'arrêter la consommation des drogues concernées. CONTEXTE DE L'INVENTION. Sur le marché, les produits utilisés dans la lutte contre le tabagisme sont de trois types : a. les apports de nicotine (patch, gomme à mâcher...) : La nicotine n' a pas d' effet toxique mais les apports de nicotine seule n'ont un effet significatif sur la consommation de tabac que pendant les premières semaines de traitement. Les rechutes sont très fréquentes (supérieures à 75 %). b. le Zyban (Bupropion) : Le Zyban, consommé avec ou sans nicotine, n'agit pas comme un substitut addictif. Sur le plan clinique, on constate 80% de rechute après un mois de traitement. Il a de plus des effets cardiovasculaires indésirables. c. le Champix (la Varénicline): Le Champix est de commercialisation récente. Il a pour fonction d'inhiber en partie l'effet de la nicotine. Il n'agit pas comme substitut addictif. Sur le plan clinique, les taux d'arrêt communiqués jusqu'à présent sont de l'ordre de 44% après un mois, avec des niveaux de rechute dans les mois qui suivent très importants. Des effets secondaires non négligeables tels que les troubles du sommeil sont décrits depuis sa mise sur le marché. Les principales recherches actuelles ne permettent pas de combler les lacunes de ces produits : a. Le Rimonabant, un produit antagoniste des récepteurs aux cannabinoïdes, n'a pas obtenu sa mise sur le marché car les essais cliniques ont montré un effet dépressogène important. b. D'autres recherches portent sur le rôle de composés associés à la nicotine pour expliquer le fort potentiel addictif du tabac; néanmoins, les composés sélectionnés n'apparaissent pas comme des solutions efficaces. En effet, plusieurs travaux suggèrent que les inhibiteurs de monoamines oxydases (IMAOs), contenus dans le tabac et sa fumée, sont les composés qui, associés à la nicotine, confèrent au tabac son pouvoir addictif (4, 10, 11). Les recherches se concentrent sur l'ajout d'inhibiteurs de monoamines oxydases (IMAOs) avec la nicotine. Cette solution manque d'efficacité dans la mesure où le blocage des monoamines oxydases peut avoir des conséquences physiologiques dangereuses (syndrome sérotoninergique). Par conséquent, il n'existe pas actuellement sur le marché, ou en développement à notre connaissance, de produits de substitution au tabac d'efficacité suffisante dans la lutte contre le tabagisme. - Aucun produit de substitution n' existe pour les psychostimulants (cocaïne, amphétamine) et deux produits existent sur le marché pour la substitution aux opiacés, le subutex (buprénorphine) et la méthadone. Ces derniers, bien qu'efficaces en tant que produit de substitution, présentent l'inconvénient de maintenir le consommateur dans un état de dépendance physique importante (10 jours et plus) qui rend le sevrage très difficile, voire impossible. Enfin, il n'existe pas de produit de substitution pour l'alcool; les produits utilisés actuellement ont pour but de dégoûter le buveur et ne suffisent pas à obtenir un sevrage.

1. La reproduction artificielle des conditions pharmacologiques qui ont conduit, non seulement à la pharmaco-dépendance au tabac, mais aussi aux autres drogues toxicomanogènes (opiacés, psychostimulants, alcool) par consommation de substance psychotrope, soit :
- Composition A : nicotine associée à un antagoniste des récepteurs 5-HTlA ; - Composition B : nicotine associée à un agoniste des récepteurs 5-HT1A.
2. Une substitution effective au tabac à court, moyen ou long terme, permettant de lever les risques cliniques aux drogues toxicomanogènes en général et plus particulièrement liés au tabagisme, et ce, sans effet secondaire notable et sans effet de « lassitude » à la substitution.

3. Une démonstration pré-clinique des modalités de fonctionnement du tabac et des autres drogues toxicomanogènes suite à une démonstration expérimentale apportée par l'inventeur (5), (8), (9)
4. Un conditionnement et des dosages adaptés permettant une consommation efficace des compositions A et B :
Le conditionnement consiste en des dragées composées d'une coque rigide à sucer contenant un agoniste (de 1 à 10 mg) ou un antagoniste des récepteurs 5 -HTlA (par exemple pindolol de 0,2 à 5 mg) , entourant un chewing- gum ou un composé contenant de la nicotine (de 1 à 10 mg) .
L'existence d'une coque rigide à sucer entourant le composé nicotinique permet une ingestion brève de
l'agoniste ou de l'antagoniste des récepteurs 5 -HTlA avant une consommation plus longue de la nicotine. Ce type de préparation permet que le composé, agoniste ou antagoniste, soit présent dans le sang quelques minutes avant la nicotine afin de permettre une efficacité optimum de la nicotine. Dans le cas de la composition B (agoniste + nicotine) si le temps séparant l'entrée de l'agoniste dans le sang et celle de la nicotine devait atteindre une trentaine de minutes, une prise chronique extemporanée de l'agoniste pourrait être envisagée.
5. Une généralisation de la reproduction artificielle des conditions pharmacologiques de la dépendance aux drogues addictives (psychostimulants, alcool, opiacés) par la combinaison de nicotine et d'un agoniste ou d'un antagoniste des récepteurs 5-HT1A.
De manière générale, l'inventeur a prouvé que le pouvoir addictif des drogues telles que les psychostimulants, le tabac, les opiacés et l'alcool est dû à l'activation simultanée, répétée et intense des systèmes noradrénergiques et sérotoninergiques (5), (6), (8), (9).
En particulier, l'inventeur a prouvé que le pouvoir addictif du tabac est dû à l'activation simultanée des systèmes noradrénergiques et sérotoninergiques et que par conséquent, l'activation d'un seul des deux systèmes (noradrénergique ou sérotoninergique) ne permet pas de reproduire les conditions pharmacologiques de dépendance au tabac (6) .
Différents travaux, y compris ceux de l'inventeur, ont montré que lors de la consommation de tabac :
(i) la nicotine déclenche l'activation des systèmes noradrénergiques (1, 2) .
(ii) la nicotine, en présence des autres composés du tabac (comprenant en particulier des IMAOs) , active les systèmes sérotoninergiques (6, 10) .
(iii) A contrario, sans les IMAOs contenus dans le tabac, la nicotine bloque les systèmes sérotoninergiques en stimulant les récepteurs 5-
HTlA (3, 6, 7). Par conséquent, la prise de nicotine seule n'a pas d'effet addictif.
L' inventeur a identifié deux grands types de compositions permettant de stimuler simultanément, sans usage du tabac, les systèmes noradrénergiques et sérotoninergiques et donc de reproduire l'effet addictif du tabac et des autres drogues toxicomanogènes (opiacés, psychostimulants et alcool) :
- Composition A : nicotine associée à un antagoniste des récepteurs 5 -HTlA ;
- Composition B : nicotine associée à un agoniste des récepteurs 5-HT1A.
L'inventeur a décrit les modalités d'action des compositions A et B:
Composition A, l'association de la nicotine et de l'antagoniste des récepteurs 5-HT1A :
(i) déclenche l'activation des systèmes noradrénergiques grâce à la nicotine ;
(ii) bloque les récepteurs 5-HT1A et donc permet une activation sérotoninergique par la nicotine (6) .
Dans le cas de la composition B, l'association de la nicotine et des agonistes des récepteurs 5-HT1A de façon chronique permet :
(i) de déclencher l' activation des systèmes noradrénergiques (nicotine) ;
(ii) de désensibiliser les récepteurs 5-HT1A et par conséquent de permettre l' activation des neurones sérotoninergiques par la nicotine (6) .
L' inventeur a montré que les compositions A et B constituent des substituts efficaces et pérennes non seulement au tabac mais aussi aux drogues toxicomanogènes (opiacés, psychostimulants, alcool) .
CONSIDERATIONS TECHNIQUES :
Nous proposons donc d'associer un composé qui inactive le récepteur 5-HT1A (agoniste ou antagoniste) avec de la nicotine afin d'obtenir un produit de substitution qui permet de supporter la pharmaco-dépendance . Des produits agissant sur le récepteur 5 -HTlA intervenant dans le traitement de la dépendance ont déjà été proposés (par exemple, Colpaert et al., 2002, EP20020770024 , PYRIDIN-2-
YL-METHYLAMINE DERIVATIVES) . Néanmoins, ces produits (i) n'ont jamais été proposés en association avec la nicotine et (ii) ne traitent que le syndrome de sevrage à la morphine, c'est-à-dire la dépendance physique qui ne dure que quelques jours. Notre proposition prétend traiter la dépendance psychique, c'est-à-dire celle qui se maintient
plusieurs mois, voire plusieurs années, après le début du sevrage .
Pour contrecarrer le manque dû à la pharmaco-dépendance, un produit doit pouvoir reproduire ce que réalise la drogue elle-même. Il doit cependant pour posséder des vertus thérapeutiques (i) avoir une action suffisamment courte et (ii) ne pas modifier l'état de conscience. Dans le domaine pré-clinique de l'addiction, il est généralement admis qu'un produit est susceptible de déclencher de la pharmaco-dépendance chez l'Homme lorsqu'il entraîne chez le rongeur, rat ou souris, des réactions telles que l ' hyperactivité locomotrice et un comportement d'auto-administration. Des molécules telles que l'amphétamine, la cocaïne, la morphine ou l'héroïne répondent à ces critères. En revanche, la nicotine ne produit qu'une faible réponse locomotrice chez le rat et aucune chez la souris. Qui plus est, l'auto- administration de nicotine ne peut être réalisée chez le rat ou la souris que dans des conditions expérimentales difficiles et complexes (privation de nourriture ou prétraitement par de la cocaïne, activation lumineuse...) que beaucoup d'expérimentateurs n'arrivent pas à reproduire. Ces résultats suggéraient donc que la nicotine n'avait pas d'effet addictif par elle-même. En 2005 et 2006, deux équipes (4, 12), dont la nôtre (12), ont montré que l'ajout d'inhibiteurs de monoamines oxydases, qui existent dans le tabac, permettait
SUBSBffèJrffirSHliEl(RUbB-administrâtion de nicotine avec une intensité proche de celle obtenue pour la cocaïne (12). Qui plus est, l'ajout d'inhibiteurs de monoamines oxydases permettait d'obtenir une forte réponse locomotrice à la nicotine chez la souris (12 ; figure 2 de l'article correspondant).
Le travail qui justifie nos revendications a consisté à trouver le site d'action des inhibiteurs de monoamines oxydases permettant à la nicotine de devenir addictive, à savoir le récepteur 5-HT1A (6, article joint).
Résultats expérimentaux :
1) Désensibilisation de l'auto-récepteur 5-HT1A par un inhibiteur des monoamines oxydases : Les animaux sont traités par 3 mg/kg de tranylcypromine ou du sérum physiologique 100 minutes avant d'être sacrifiés. Leur cerveau est extrait et le raphé dorsal est prélevé. Les membranes sont recueillies et un western blot est réalisé en présence d'anticorps dirigés contre le récepteur 5-HT1A. Les contrôles sont réalisés à partir de membranes d'hippocampe d'animaux traités et à partir de membranes de cerveaux de souris dépourvues de récepteur 5 -HTlA. Le transporteur de la sérotonine (5- HTT) sert de référence pour montrer que la quantité de membranes déposées est identique. Les résultats montrent une désensibilisation des récepteurs 5-HT1A localisés dans le raphé dorsal sans modification de la quantité de ces récepteurs dans l'hippocampe. Aucun marquage n'est obtenu pour les souris knockout pour le récepteur 5 -HTlA (Figure 1) .
2) Effet de la désensibilisation du récepteur 5-HT1A sur la réponse locomotrice à la nicotine : Les animaux sont traités par un agoniste des récepteurs 5-HT1A, le 8-OH-D-PAT ou du sérum physiologique, 60, 100 et 120 minutes avant de recevoir de la nicotine (1 mg/kg) . L'activité locomotrice est alors mesurée dans un corridor circulaire connecté à un enregistreur
(Imetronic, Bordeaux, France) . Les résultats montrent que les animaux traités par du sérum physiologiques ainsi que ceux qui reçoivent la nicotine 60 minutes après l'injection de 8 -OH-D-PAT ne réagissent pas à a nicotine. En revanche, une réponse locomotrice à la nicotine apparaît très nettement chez les animaux qui ont reçu du 8-OH-D-PAT 100 et 120 minutes avant la nicotine (figure 2). La désensibilisation du récepteur 5-HT1A par un agoniste permet donc l'action de la nicotine.
3) Effets d'un antagoniste des récepteurs 5-HT1A sur la réponse locomotrice à la nicotine :
Les animaux sont traités soit par du sérum physiologique soit par un antagoniste des récepteurs 5 -HTlA, le WAY
100635 (lmg/kg) 30 minutes avant l'injection de nicotine (1 mg/kg) . Dans ces conditions les animaux qui ont été prétraités par le WAY 100635 présentent, contrairement aux animaux contrôles, une réponse locomotrice à la nicotine. Le blocage des récepteurs 5 -HTlA permet donc l'action de la nicotine (figure 3) .
4) Effet de l'injection répétée d'un antagoniste 5-HT1A et de nicotine sur la réponse locomotrice à la nicotine chez des animaux prétraités par un antagoniste 5-HT1A. Les animaux reçoivent quatre fois une injection de WAY 100635 (1 mg/kg) et 30 minutes plus tard une injection de nicotine (1 mg/kg) . Quatre jours plus tard, ils reçoivent une injection de WAY 100635 et 30 minutes plus tard une injection de nicotine (1 mg/kg) et leur activité locomotrice est mesurée. La figure 6 montre la présence
d'une sensibilisation comportementale. Enfin, dans les mêmes conditions expérimentales, la figure 4 montre la présence d'une sensibilisation neurochimique. Ces derniers résultats apportent une preuve supplémentaire que l ' inactivation ou le blocage des récepteurs 5 -HTlA confère à la nicotine des propriétés addictives qu'elle n'a pas lorsque les récepteurs 5-HT1A sont libres.
