Abstract
Die Erfindung beschreibt ein polyaxial ausrichtbares Stabilisierungselement für Endoprothesen. Aufgabe ist es, ein Stabilisierungselement zu entwickeln, welches vorzugsweise nach dem optimalen Positionieren des Implantats montierbar ist und das sich durch eine angepasste Form und funktionelle Bereiche auszeichnet. Es ist dadurch gekennzeichnet, dass eine halbkugelförmige Führungsspitze zur Führung des Stabilisierungszapfens während des Einpressens in vorbereitete untermaßige Bohrungen im Knochen dient, ein Führungsbereich, welcher sich der Führungsspitze anschließt, aus Treibflächen besteht, die kegelförmig polyedrisch ausgeführt sind und zur Verdrehsicherung und Kompression des spongiosen Knochens während des Einpressens dient, ein darauf folgender osteointegrativer Bereich, welcher eine funktional modifizierte Oberfläche besitzt und die mechanische Stabilität des Stabilisierungszapfens nach Verwachsung im Knochen sichert, ein abschließender Spreizbereich, welcher dübelähnlich mit sphärischen Teilflächen aufgebaut ist, zur Fixierung des Stabilisierungszapfens im Implantat dient und die polyaxiale Ausrichtung des Stabilisierungszapfens durch einen Bereich mit Kugelfläche im Spreizbereich des Zapfens ermöglicht wird.

Polyaxial ausrichtbares Stabilisierungselement für Endoprothesen
Technisches Gebiet
[0001] Die Erfindung betrifft ein poly axial ausrichtbares Stabilisierungselement für Endoprothesen.
Stand der Technik
[0002] Das Setzen und die räumliche Stabilisierung von Endoprothesen erfolgt a) durch Einpressen in ein vorbereitetes Implantatlager oder b) durch Einzementierung in ein leicht vergrößertes Implantatlager. Bei gut erhaltener Knochensubstanz lässt sich damit eine hinreichende Implantat-Stabilität (Primärstabilität) erreichen, die sich während des Einheilprozesses durch Ossifikation im Implantat-Implantatlager-Grenzbereich weiter verbessert (Sekundärstabilität).
[0003] Bei unzureichender oder geschädigter Knochensubstanz reicht diese Art der primären Fixierung zur Garantie der Primärstabilität oft nicht aus. Durch zusätzliche Stabilisierungs- bzw. Befestigungselemente wird versucht, im Zusammenwirken von primärer Implantatfixierung und den Stabilisierungswirkungen aller Zusatzelemente die Primärstabilität herzustellen.
[0004] Der Grad der erreichbaren Stabilität ist unterschiedlich, die benutzen
Zusatzelemente sind vielfältig. Ihre Art und Gestaltung ist erfahrungsbasiert und die Entwicklung von Zusatzelementen ist nach wie vor eine aktuelle Fragestellung.
[0005] Die einfachsten verwendeten Stabilisierungselemente sind selbstschneidende
Knochenschrauben für Kortikalis oder Spongiosa, die durch entsprechend gestaltete Implantatöffnungen in den Knochen geschraubt werden und eine feste Verbindung zwischen Knochen und Implantat zumindest für einen Zeitraum sichern sollen.
[0006] Diese Knochenschrauben setzen aber einen einigermaßen intakten Knochen im Bereich des Implantatlagers voraus, ein Befund, der nicht immer gegeben ist.
[0007] Eine zweite Gruppe von Stabilisierungselementen sind Laschen. Laschen können sowohl Bestandteil des Implantats selbst sein oder können am Implantat befestigt werden. Sie werden durch die beschriebenen Knochenschrauben am tragfähigen Knochen fixiert. Die Form dieser Laschen muss sich den anatomischen Anforderungen anpassen. Man findet technische Lösungen sowohl mit vorgeformten Laschen als auch Lösungen mit intraoperativ zu formenden Laschen.
[0008] Eine dritte Gruppe von Zusatzelementen sind Zapfen, die in geeignete
Knochensubstanz getrieben werden und durch Formschluss dem Implantat weitere Festigkeit verleihen. Diese Gruppe von Zusatzelementen wurde bisher nur spärlich eingesetzt, weil die benutzten Lösungen sowohl in ihrer Handhabung schwierig als auch in ihrer Wirkung begrenzt waren.
[0009] Alle bisher eingesetzten Zapfen sind entweder a) untrennbarer Bestandteil des
Implantats oder b) starr mit dem Implantat verbunden oder c) lösbar mit dem Implantat verbunden, aber mit einer nachteiligen Richtungsinvarianz nach ihrer Befestigung behaftet.
[0010] Diese Nachteile sind auch beim deutschen Patent DE 19714050 C2 zu sehen, welches eine Hüftpfanne beschreibt, bei der ein Pfannenkörper wenigstens einen in das Acetabulum eintreibbaren Stabilisierungszapfen trägt. Zur Verbesserung des Sitzes des Stabilisierungszapfens im Acetabulum weist dieser eine sich zum Pfannenkörper konisch erweiternde Außenkontur auf. Am Stabilisierungszapfen ist weiterhin ein sägezahnförmiges Profil ausgebildet, dessen steile Flanken dem Pfannenkörper zugewandt sind.
[0011] Im deutschen Patent DE 10113614 B4 wird ein langgestrecktes Fixationselement beschrieben, welches an die Gelenkpfanne im Bereich von sphärisch berandeten Bohrungen koppelbar ist. Durch die Bildung eines Kugelgelenkes zwischen den sphärisch geformten Bohrungsrändern in der Wandung der Gelenkpfanne und dem sphärisch konkav ausgeformten Ende des Fixationselements ist es möglich, dass das Fixationselement relativ zur Gelenkpfanne verschiedene Winkelstellungen einnehmen kann. Fixiert wird das Element durch eine Gewindeschraube mit poly axialem Kopf, die von innen durch die betreffende Bohrung in der Gelenkpfanne gesetzt wird. Der Operateur setzt ein oder mehrere Fixationselemente in entsprechend vorbereitete Ausfräsungen. Der Pfannenkörper wird anschließend in das Acetabulum gesetzt und mit Gewindebolzen so verschraubt, dass eine dauerhafte Verbindung zwischen Gelenkpfanne und Fixationselement hergestellt ist.
[0012] Der wesentliche Nachteil besteht darin, dass die Fixationselemente vor dem Setzen der Implantatpfanne in den Beckenknochen eingebracht werden müssen und die Pfanne dann passgenau zu den Verschraubungsöffnungen gesetzt werden muss. Damit ergeben sich sehr große Schwierigkeiten beim Setzen des Implantates und auch eine Korrektur des Pfannensitzes nach dem Setzen ist nicht wirklich möglich ohne das Fixationselement in seiner Wirkung einzuschränken. Die beschriebene Winkelvariabilität des Elementes ist begrenzt, da die dafür zur Verfügung stehenden sphärischen Pressflächen die Kontur der Implantatpfanne nicht beeinträchtigen dürfen.
[0013] Bei allen anderen bisher eingesetzten Zapfen ist oder wird dieser vor dem Einsetzen des Implantates mit dem Implantat verbunden. Dieser Fakt hat negativen Einfluss auf die Operationstechnik, da beim Setzen des Implantates auf die Position des Zapfens
Rücksicht genommen werden muss. [0014] Insbesondere für ein nötiges nachträgliches Ausrichten eines Implantats ergeben sich aufgrund der Richtungsinvarianz dieses Zapfens erhebliche Schwierigkeiten, die zu einem nicht optimalen Sitz des Implantats und weiteren damit verbundenen
Komplikationen führen können. [0015] Aus EP 0666068 B 1 ist ein Fixationselement bekannt, das eine Anbringung des
Fixierungselements im Knochengewebe in einer Winkelstellung ermöglicht und damit eine erhöhte Variabilität bei der Anpassung an die jeweils gegebenen anatomischen
Verhältnisse zulässt. [0016] Nachteilig ist jedoch, dass das Fixationselement in den Knochen geschraubt werden muss. Außerdem ist der Winkel durch die Vorgaben von Fixationselement und Pfanne in seiner Öffnungsgröße beschränkt, was die Variabilität wiederum einschränkt. [0017] Die Gestaltung des Zapfen selbst als Bestandteil des Implantats mit bioaktiven
Oberflächeneigenschaften ist derzeit noch nicht berücksichtigt. [0018] Als wesentliche Mängel der bisher verwendeten Lösungen für zapfenartige
Fixationselemente sind damit herausgestellt:
1. Die Montage der Zapfen vor dem Einsetzen des Implantates führt zu Komplikationen beim Setzen des Implantates.
2. Das getrennte Setzen von Zapfen und Implantat, in dieser Reihenfolge, führt zu geometrischen Zwangsbedingungen für die Implantatpositionierung.
3. Die räumlich feste Richtung der Zapfen kann zu Fehlstellungen des Implantates führen.
4. Die Gestaltung von Zapfen als bioaktives Element ist bisher kaum berücksichtigt.
[0019] Die aufgezeigten Mängel der bisherigen Lösung für Implantatzapfen werden mit der erfindungsgemäßen Lösung überwunden.

Stabilisierungselement für Endoprothesen zu entwickeln, welches vorzugsweise nach dem optimalen Positionieren des Implantats montierbar ist und das sich durch eine angepasste Form und funktionelle Funktionsbereiche auszeichnet.
[0021] Die Lösung der Aufgabe erfolgt entsprechend der Patentansprüche. [0022] Das erfindungsgemäße polyaxial ausrichtbare zapfenförmige
Stabilisierungselement für Endoprothesen besitzt mehrere Funktionsbereiche. Eine halbkugelförmige Führungsspitze dient zur Führung des Stabilisierungszapfens während des Eindrückens in das Acetabulum. Ein Führungsbereich, welcher sich der Führungsspitze anschließt, besteht aus Treibflächen, die kegelförmig polyedrisch ausgeführt sind und zur Verdrehsicherung und Kompression des spongiosen Knochens während des Einfahrens dienen. Die kegelförmig polyedrisch ausgeführten Treibflächen sind beispielsweise Vierer- oder Sternflächen.
[0023] Darauf folgt ein osteointegrativer Bereich, welcher eine funktional modifizierte Oberfläche besitzt und die mechanische Stabilität des Zapfens im Knochen als bevorzugter knöcherner Verwachsungsbereich sichert.
[0024] Ein abschließender Spreizbereich ist dübelähnlich mit sphärischen Teilflächen aufgebaut. Er dient zur Befestigung des Stabilisierungszapfens im Implantat.
[0025] Die Oberfläche des osteointegrativen Bereichs ist durch Beschichtung mit Titanpulver stark vergrößert und mit einer biomimetischen, elektrolytisch abgeschiedenen Calciumphosphatschicht (beispielsweise BONIT®) überzogen.
[0026] Die polyaxiale Ausrichtung des Stabilisierungszapfens wird durch einen kugelförmigen Bereich mit Teilkugelfläche im Spreizbereich des Zapfens ermöglicht.
[0027] In einer Ausführungsform wird die Verdrehsicherheit des Zapfens im polyaxialen Zapfenlager, welches entsprechende Aussparungen besitzt, durch zwei sich gegenüberliegende Führungsnoppen im Spreizbereich des Zapfens erreicht.
[0028] Die sphärischen Teilflächen des Stabilisierungszapfens werden im Spreizbereich durch eine spezielle Schraube, welche eine Konusfläche angepasster Steigung besitzt, mit der sphärischen Fläche des Zapfenlagers formschlüssig verpresst.
[0029] Es können ein oder mehrere Stabilisierungszapfen in verschiedenen Größen während einer Operation gesetzt werden.
[0030] Das Einsetzen der Zapfen erfolgt durch Verpressen in untermaßige Bohrungen im Knochen, die durch den Implantatkörper, der sich bereits in seiner endgültigen Stellung befindet, vorbereitend erfolgt sind. Diese Bohrungen werden mittels Führungshülsen in die operativ gewünschten Richtungen ausgeführt.

Weg(e) zur Ausführung der Erfindung
[0032] Für Defektsituationen, bei denen das Implantatlager in Bezug auf die Primärstabilität nicht hinreichend belastbar ist, werden zur Erhöhung der Primärstabilität Stabilisierungszapfen 1 eingesetzt.
[0033] In Figur 1 wird ein erfindungsgemäßes, polyaxial ausrichtbares
Stabilisierungselement dargestellt. Es besteht aus einem Stabilisierungszapfen 1, welcher in verschiedene Funktionsbereiche eingeteilt ist. Weiterhin wird das Zapfenlager 3, welches sich in einem Pfannengrundkörper befindet, und der Konusbolzen 2, der dem Festsetzen des Stabilisierungszapfens 1 dient, gezeigt.
[0034] Der erste Funktionsbereich ist ein halbkugelförmiger Bereich und dient als
Führungsspitze Ia zur Führung des Stabilisierungszapfens 1 während des Eindrückens in die untermäßige Bohrung. Daran schließt sich ein Führungsbereich an, der aus Treibflächen Ib besteht, die kegelförmig polyedrisch (z.B. Vierer- oder Sternflächen) ausgeführt sind und zur Verdrehsicherung und Kompression des spongiosen Knochens während des Einfahrens des Zapfens dienen.
[0035] Ein osteointegrativer Bereich Ic (Haltebereich) sichert durch die bevorzugte
Verwachsung mit dem Knochen die mechanische Stabilität des Stabilisierungszapfens 1 im Knochen. Zur Verbesserung des Einwachsens und zur Vergrößerung der osteointegrativen Oberfläche wird der Bereich Ic des Stabilisierungszapfens 1 in einem ersten Schritt mit einer strukturierten Oberfläche versehen, die in einem weiteren Schritt osteoinduktiv beschichtet wird. Die Strukturierung erfolgt indirekt durch Aufrauung mittels Abrasionsstrahl und nachfolgender Oberflächenvergrößerung durch Aufbringen von Titan-Partikeln in einem plasmagestützten Beschichtungsprozess. Die osteoinduktive Beschichtung erfolgt durch elektrochemische Abscheidung einer biomimetischen Calciumphosphatschicht (z.B. mit TPS, mit BONIT®).
[0036] Das Zapfenlager 3 wird durch eine Kugelfläche 3a mit einem freien
Öffnungswinkel 3c bestimmt. In einer Ausführungsform sind Aussparungen 3b für Führungsnoppen vorgesehen.
[0037] Durch einen Spreizbereich Ie, welcher dübelähnlich mit sphärischen Teilflächen aufgebaut ist, wird der Stabilisierungszapfen 1 in der Revisionspfanne befestigt. Die polyaxiale Ausrichtung des Stabilisierungszapfens 1 wird durch einen Bereich mit Kugelfläche If ermöglicht. Ein Innengewinde Id nimmt eine Konusschraube 2, welcher ein Gewinde 2a und eine Schraubkontur 2c aufweist, auf. Durch Aufspreizung (Spreizpressfit) des Stabilisierungszapfens 1 (Fig. 2 und 3) mit der Konusschraube 2, welcher eine Konusfläche 2b entsprechender Steigung besitzt, werden die sphärischen Teilflächen des Stabilisierungszapfens 1 mit der sphärischen Fläche des Zapfenlagers 3 formschlüssig verpresst.
[0038] Entsprechend den operativen Gegebenheiten können Stabilisierungszapfen 1 in verschiedenen Größen verwendet werden.
[0039] Die einfache Vorbereitung des Sitzes der Stabilisierungszapfen 1 erfolgt durch eine Richtungsbohrung mit reduziertem Durchmesser mittels Führhülsen durch den Implantatgrundkörper. Die Stabilisierungszapfen 1 werden durch das Zapfenlager 3 im Implantatgrundkörper in den Knochen gedrückt und in den Zapfenlagern 3 des Implantats fest verpresst. Ihre Richtung ist in einem vorgegebenen Bereich polyaxial einstellbar. Das Blind-Setzen der Stabilisierungszapfen 1 in Bohrungsrichtung ist möglich.
[0040] Die Figuren 4 und 5 zeigen die durch die Teilkugelfläche If möglichen poly axialen Ausrichtungen mit einem Polarwinkel WP bzw. WP2.
[0041] In einer Ausführungsform (Fig. 5) wird die Verdrehsicherheit im poly axialen Zapfenlager 3 durch zwei gegenüberliegende Führungsnoppen Ig erreicht. Im Zapfenlager 3 sind entsprechende Aussparungen 3b für diese Noppen vorgesehen.
[0042] Der Stabilisierungszapfen 1 wird aus Titanmaterial in notwendiger Güte hergestellt.

Ansprüche
[0001] PoIy axial ausrichtbares Stabilisierungselemert für Endoprothesen mit mehreren
Funktionsbereichen, gekennzeichnet durch a) eine halbkugelförmige Führungsspitze (Ia), welche zur Führung des Stabilisierungszapfens (1) während des Eindrückens diert, b) ein Führungsbereich, welcher sich der Führungsspitze (Ia) anschließt, und der aus kegelförmig polyedrisch ausgefühlten Treibflächen (Ib) besteht, die zur Verdrehsicherung und Kompression des spongiosen Knochens während des Einführens dienen, c) einen darauf folgenden osteoirtegrativen Bereich (Ic), welcher eine funktional modifizierte Oberfläche besitzt, zur Sicherung der mechanischen Stabilität des Stabilisierungszapfens (1) im Knochen, d) einen abschließenden Spreizbereich (Ie), der dübelähnlich mit sphärischen Teilflächen aufgebaut ist, der zur Befestigung des Stabilisierungszapfens (1) in einer Revisionspfanne diert, und e) einen Bereich mit Kugelfläche (If) im Spreizbereich (Ie), durch den die polyaxiale Ausrichtung des Stabilisierungszapfens (1) ermöglicht wird.
[0002] Polyaxial ausrichtbares Stabilisierungselemert nach Anspruch 1, gekennzeichnet durch zwei gegenüberliegende Führungsnoppen (Ig), die in einem polyaxialen
Zapfenlager (3) der Endoprothese, welches ertsprechende Aussparungen (3b) aufweist, zur Verdrehsicherung diert. [0003] Polyaxial ausrichtbares Stabilisierungselemert nach Anspruch 1, dadurch gekennzeichnet, dass die kegelförmig polyedrisch ausgeführten Treibflächen (Ib) Vierer- oder
Sternflächen sind. [0004] Polyaxial ausrichtbares Stabilisierungselemert nach Anspruch 1 dadurch gekennzeichnet, dass die funktional modifizierte Oberfläche des osteoirtegrativen Bereichs (Ic) durch
Beschichtung mit Titanpulver oberflächenvergrößert; ist. [0005] Polyaxial ausrichtbares Stabilisierungselemert nach Anspruch 1 dadurch gekennzeichnet, dass die funktional modifizierte Oberfläche des osteoirtegrativen Bereichs (Ic) mit einer biomimetischen, elektrolytisch abgeschiedenen Calciumphosphatschicht
versehen ist. Polyaxial ausrichtbares Stabilisierungselemert nach Anspruch 1, gekennzeichnet durch eine Konusschraube (2), welche eine Konusfläche (2b) ertsprechender Steigung aufweist, um die sphärischen Teilflächen des Stabilisierungszapfens (1) im Spreizbereich (Ie) mit der sphärischen Fläche des Zapfenlagers (3) formschlüssig zu verpressen.
