Abstract
Device (1) for the fixation of bones (11) or bone fragments comprising: A) a longitudinal bone fixation means (2) with a rear end (4) protruding from the surface of bone and a front end (5) being insertable into a bone; and B) at least one securing end cap (3) which is fastenable as a safety device or a head at the rear end (4) of the bone fixation means (2), wherein C) the securing end cap (3) comprises a hollow body having a longitudinal axis (6), a leading end (13) and a trailing end (14), said hollow body having a cavity (17) being open at the leading end (13) and being apt for insertion of the bone fixation means (2); and D) the securing end cap (3) has an outer surface (8) being coaxial to the longitudinal axis (6) and being provided with a three-dimensional structure (9), which is apt to fasten the securing end cap (3) in a bone (11).
Description
Device for the fixation of bones
The invention relates to a device for the fixation of bones or bone fragments according to the concept of claim 1.
In orthopaedic surgery Kirschner wires, pins or elastic nails are often used in order to solve various tasks. These implants are inserted with their front end in the bone until the desired end position is reached. The implants are subsequently cut at their rear end in order to shorten them to the desired length. This procedure shows the drawback that the rear end of the implant has a wire edge after being cut which may cause irritations of the surrounding tissue. Furthermore, the rear end of the implant is not sufficiently secured with regard to displacement of the implant in the bone.
From PCT/CH2004/000330 FRICKER a sleeve is known which may be fixed a the protruding end of a longitudinal bone fixation means. This known device shows the disadvantage that the sleeve in not anchored in the bone so that the sleeve may moved out of the bone together with the rear end of the bone fixation means.
On this point, the invention intends to provide remedial measures. The invention is based on the objective of providing a device with a securing end cap which is apt to be anchored in the bone and which additionally guides and secures the rear end of a bone fixation means against axial movement. Furthermore, the securing end cap protects the surrounding tissue from irritations.
The invention solves the posed problem with a device that displays the features of claim 1.
Additional advantageous embodiments of the invention are characterized in the subclaims.
The advantages achieved by the invention are essentially to be seen in the fact that, thanks to the device according to the invention:
- an undesired displacement of an implant in the bone may be prevented;
- the end of the implant which protrudes from the bone may be supported and simultaneously be covered in order prevent possible irritations of the surrounding tissue;
- an easy and secure manipulation of the securing end cap is possible in case of positioning it on the end of the implant and inserting it in the bone.
In a preferred embodiment the device is provided with the three-dimensional structure having a form of an exterior thread, which allows the fastening of the securing end cap in the bone with common instruments.
In a different embodiment the securing end cap is provided with an unthreaded rear segment between the trailing end and the exterior thread, whereby the transition between the unthreaded rear segment and the segment being provided with the exterior thread is oblique with regard to the longitudinal axis. The advantage of this embodiment is to be seen in the fact that by means of the oblique transition the surgeon may optically decide the insertion depth of the end cap such that an optimal positioning of the end cap is permitted.
In a different embodiment the transition between the unthreaded rear segment and the exterior thread is configured at an angle of between 40° and 60° relative to the longitudinal axis of the securing end cap. Due to this embodiment the securing end cap may be positioned at the rear end of an elastic marrow nail which protrudes from the bone surface at respective angles.
In another embodiment the securing end cap is provided with an unthreaded front segment between the leading end and the exterior thread. Since the end cap is implanted with its longitudinal axis oblique to the bone (40° - 60°) the unthreaded segment firstly permits a coaxial straightforward sliding of the securing end cap on the rear end of the bone fixation means. Furthermore, the unthreaded segment permits a guidance when the thread is cut obliquely into the bone.
In a different embodiment the securing end cap is self-drilling and no advanced drilling procedure is necessary.
In an other embodiment the exterior thread is self-cutting and no advanced cutting of the thread is necessary.
In a different embodiment the exterior thread is self-centering and prevents cutting the longitudinal bone fixation by the end cap.
In a further embodiment the bone fixation means is an elastic marrow nail made of titan.
In yet another embodiment the bone fixation means is an elastic marrow nail made of stainless steel.
The invention and additional configurations of the invention are explained in even more detail with reference to the partially schematic illustration of several embodiments.

As shown in figs. 1 to 4 the device 1 comprises a longitudinal bone fixation means 2 and a securing end cap 3. The longitudinal bone fixation means 2 has a central axis 15, comprises a rear end 4, a front end 5 and is being insertable into a bone 11. The securing end cap 3 comprises a hollow body having a longitudinal axis 6, a leading end 13 and a trailing end 14. The securing end cap 3 has a cavity 17 being parallel to the
longitudinal axis 6 and having an opening at the leading end 13 of the securing end cap 3. The cavity 17 is apt for retaining the rear end 4 of the bone fixation means 2 in a manner where the central axis 15 of the bone fixation means 2 is coaxial to the longitudinal axis 6 of the securing end cap 3. Since the cavity 17 is closed at the trailing end 14 that the securing end cap 3 restricts the axial movement of the bone fixation means 2.
According to figs. 2 to 4 the securing end cap 3 is configured as a hollow body having a cavity 17 with an opening at the leading end 13 for retaining the bone fixation means 2. The securing end cap 3 has a length L measured parallel to the longitudinal axis 6. Furthermore, the securing end cap 3 comprises an outer surface 8 having an essentially cylindrical shape being coaxial to the longitudinal axis 6. The outer surface 8 of the securing end cap 3 is provided with a three-dimensional structure 9 which is configured as an exterior thread 10 being apt to fasten the securing end cap 3 in a bone 11 and which is particularly configured only on a partial arc of the periphery of the securing end cap 3. The outer surface 8 of the securing end cap 3 is provided with an unthreaded rear segment 18 between the trailing end 14 and the exterior thread 10. The transition 20 between the unthreaded rear segment 18 and the segment being provided with the exterior thread 10 is oblique with regard to the longitudinal axis 6 and is configured at an angle of between 40° and 60° to the longitudinal axis 6. Furthermore, the securing end cap 3 comprises an unthreaded front segment 19 between the leading end 13 and the exterior thread 10 permitting an axial straightforward sliding of the securing end cap 3 on the rear end 4 of the bone fixation means 2. The cross-section of the unthreaded rear segment 18 of the outer surface 8 shows a non-circular shape and is provided with flat portions or projections 21.
The surgical procedure during implantation of the device according to the invention preferably comprises the following steps:
a) insertion of an implant in the bone to be treated; b) fixation of the fracture; c) mounting of the securing end cap on the implant; and d) screwing the securing end cap in the bone.
The advantages achieved by means of this surgical procedure are essentially to be seen in the fact that thanks to the securing end cap according to the invention
- a pre-drilling of the bore hole and cutting of the thread are not necessary since the tip is configured self-centering and self-cutting; and
- manipulation of the securing end cap during positioning on the end of the implant and subsequent screwing in of the end cap is possible, since the drive is positioned a the protruding end of the end cap.
Claims
1. Device (1 ) for the fixation of bones (11 ) or bone fragments comprising
A) a longitudinal bone fixation means (2) with a rear end (4) protruding from the surface of bone and a front end (5) being insertable into a bone; and
B) at least one securing end cap (3) which is fastenable as a safety device or a head at the rear end (4) of the bone fixation means (2), characterized in that
C) the securing end cap (3) comprises a hollow body having a longitudinal axis (6), a leading end (13) and a trailing end (14), said hollow body having a cavity (17) being open at the leading end (13) and being apt for insertion of the bone fixation means (2); and
D) the securing end cap 3 has an outer surface (8) being coaxial to the longitudinal axis (6) and being provided with a three-dimensional structure (9), which is apt to fasten the securing end cap (3) in a bone (11 ).
2. Device (1) according to claim 1 , wherein the three-dimensional structure (9) is an exterior thread (10).

Device (1) according to claim 1 or 2, wherein the securing end cap (3) is provided with an unthreaded rear segment (18) between the trailing end (14) and the exterior thread (10) and whereby the transition (20) between the unthreaded rear segment (18) and the segment being provided with the exterior thread (10) is oblique with regard to the longitudinal axis (6).
4. Device (1) according to claim 3, wherein the transition (20) between the unthreaded rear segment (18) and the exterior thread (10) is configured at an angle of between 40° and 60° relative to the longitudinal axis (6) of the securing end cap (3).
5. Device (1) according to one of the claims 2 to 4, wherein the securing end cap (3) is provided with an unthreaded front segment (19) between the leading end (13) and the exterior thread (10).
6. Device (1) according to one of the claims 1 to 5, wherein the securing end cap (3) is self-drilling.
7. Device (1) according to one of the claims 2 to 6, wherein the exterior thread (10) is self-cutting.
8. Device (1) according to one of the claims 2 to 7, wherein the exterior thread (10) is self-centering.
9. Device (1) according to one of the claims 1 to 8, wherein the bone fixation means (2) is an elastic marrow nail made of titan.
10. Device (1) according to one of the claims 1 to 8, wherein the bone fixation means (2) is an elastic marrow nail made of stainless steel.
