Abstract

La patente que se presenta se refiere a una plataforma ortésica ambulatoria para Ia monitorización, diagnóstico de temblor y validación de estrategias de control, tanto activas como pasivas, para Ia supresión del temblor patológico al tiempo que preserva el movimiento natural. El dispositivo se desarrolla en torno a una estructura articulada que permite soportar un conjunto de sensores y actuadores necesarios para Ia realización de las tareas de monitorización, diagnóstico y aplicación de estrategias de control.

DISPOSITIVO ORTÉSICO DINÁMICO PARA LA MONITORIZACIÓN,
DIAGNÓSTICO Y SUPRESIÓN DE TEMBLOR PATOLÓGICO
SECTOR DE LA TÉCNICA
La presente invención se refiere a dispositivos biomecánicos o biomédicos para Ia monitorización, diagnóstico y validación de estrategias de control, tanto activas como pasivas, para suprimir el temblor patológico, específicamente en el miembro superior. Como tales, son de aplicación en Ia industria ortoprotésica y en Ia rehabilitación y tratamiento médico.
ESTADO DE LA TÉCNICA
El temblor patológico es un síntoma fuertemente discapacitante caracterizado por contracciones rítmicas e involuntarias de los músculos del cuerpo humano. El temblor puede afectar a distintas partes del organismo. Estas oscilaciones rítmicas están superpuestas al movimiento voluntario del paciente. Las características de amplitud y frecuencia de las oscilaciones temblorosas dependen del tipo de temblor, sea este de reposo, postural o cinético. Además, para un mismo paciente estas características pueden verse alteradas en función de Ia actividad o condiciones que desarrolle o a que esté sometido.
La frecuencia de estas oscilaciones varía en función del tipo de temblor y de Ia patología. El rango comúnmente aceptado oscila entre 2 Hz y 12 Hz.
El tratamiento clásico del temblor es Ia medicación y Ia estimulación eléctrica talámica o subtalámica. Algunos trabajos muestran que el tratamiento médico por medicación fracasa en un 85 por ciento de los casos y los tratamientos por estimulación son efectivos en un mayor número de casos pero implican una intervención quirúrgica delicada que no está indicada para un buen número de pacientes potenciales (E. Rocon, J. M. Belda-Lois, JJ. Sanchez-Lacuesta, J.L.Pons, Pathological Tremor management: modelling, compensatory technology and evaluation, Technology and Disability, 16, pp. 3- 18, 2004). Estos hechos han promovido el desarrollo de dispositivos para el tratamiento alternativa del temblor patológico. Existen en Ia actualidad varios sistemas que han logrado resultados significativos en Ia reducción del temblor o de sus efectos. Existen dos aproximaciones básicas para este tratamiento:
• Aislamiento de Ia tarea del movimiento tembloroso • Reducción de Ia amplitud del temblor
El concepto de aislamiento de Ia tarea del movimiento tembloroso está basado en Ia inserción de un dispositivo en serie entre el cuerpo humano y Ia tarea. Esta aproximación se puede realizar electrónicamente o de forma mecánica. Un procedimiento establecido clínicamente para reducir Ia amplitud del temblor es Ia aplicación de cargas biomecánicas al miembro afectado y está basado en dispositivos ortésicos u ortesis (E. Rocon, J. M. Belda-Lois, JJ. Sanchez-Lacuesta, J.L.Pons, Pathological Tremor management: modelling, compensatory technology and evaluation, Technology and Disability, 16, pp. 3- 18, 2004). La aplicación de cargas biomecánicas puede ser implementado tanto en sistemas activos como en sistema pasivos.
En sistemas activos, los actuadores generan un movimiento en el miembro de igual amplitud pero en el sentido contrario al movimiento tembloroso, sustrayendo así el temblor del movimiento global del miembro. En los sistemas pasivos, los actuadores aplican cargas mecánicas resistivas, sean estas disipativas o inerciales.
Estas ortesis pueden ser portátiles o no portátiles. Las ortesis no portátiles están sujetas al miembro afecto y a una plataforma sujeta el suelo (que puede ser el propio suelo, Ia silla de ruedas, una mesa, etc). Las ortesis portátiles están sujetas únicamente al brazo del paciente. El modo en que ambas aproximaciones ejercen las cargas en el brazo para Ia supresión del temblor es radicalmente opuesto. Las ortesis portátiles eliminan el temblor entre elementos consecutivos de Ia cadena cinemática del brazo. Las ortesis no portátiles eliminan el temblor aplicando las cargas desde el suelo al brazo. Entre los sistemas no portátiles más representativos podemos destacar el dispositivo comercial Neater Eater, el joystick amortiguado del MIT, Ia ortesis controlada de disipación de energía CEDO y el brazo modulado de disipación de energía MED. Existe un menor número de soluciones portátiles para Ia supresión del temblor. Entre ellas destaca el "Viscous Beam", una ortesis pasiva para Ia supresión del temblor de flexo-extensión de muñeca que funciona aplicando un sistema con fricción viscosa paralelamente a Ia muñeca. Sin embargo, su diseño mecánico restringe los movimientos de prono-supinación y desviación radial-cubital de Ia muñeca.
La solución ideal consiste en aplicar cargas biomecánicas únicamente al movimiento tembloroso. Sin embargo, en todas las realizaciones descritas en el estado de Ia técnica estos dispositivos aplican una carga biomecánica disipativa de forma independiente al tipo de temblor que afecta al paciente. Esta carga es además independiente del tipo de movimiento sobre el que se aplica, ya sea voluntario, tembloroso o una combinación de ambos. Como consecuencia el paciente nota también una carga resistiva ante su movimiento intencional. El miembro superior tiene como función principal permitir que Ia mano realice sus funciones y pueda alcanzar cualquier punto del espacio, especialmente cualquier punto de Ia superficie corporal del individuo, de forma que sea posible aproximar, manipular y mover objetos, desde el cuerpo y/o hacia el mismo. Por eso, Ia cadena cinemática formada por el hombro, el codo, el antebrazo, Ia muñeca y Ia mano, tiene un alto grado de movilidad, así como capacidad distal de prensión con un infinito número de posiciones y funciones. El miembro superior es una de las partes del cuerpo más complejas anatómica y fisiológicamente.
Además, el miembro superior es muy importante pues es capaz de desarrollar actividades cognoscitivas, de expresión y de manipulación, e interviene en las funciones exploratorias del entorno y en todos los actos motores reflejos.
Así pues, cualquier alteración o patología que afecte el rango de movimiento de los miembros superiores, su fuerza muscular, sensibilidad y/o Ia integridad de su piel, alterará el funcionamiento de los mismos. Por ello, los objetivos de las ortesis del miembro superior deberán ser:
• Mantener Ia alineación articular,
• Asistir al movimiento deficitario, • Incrementar la coordinación y Ia función en varias tareas,
• Mantenimiento de Ia funcionalidad de Ia mano, y
• Prevención de las restricciones.
Una ortesis o plataforma ortésica es definida como un dispositivo médico que actúa exteriormente a un segmento del cuerpo humano con el fin de establecer algún tipo de compensación funcional. Las ortesis posturales o pasivas representan Ia forma común de actuación de las ortesis del miembro superior, que tiene como principal objetivo preservar Ia posición funcional de Ia mano (E. Viosca y otros, Guía de uso y prescripción de productos ortoprótesicos a medida, Publicaciones IBV, 1999).
Cuando una ortesis aplica algún tipo de fuerza sobre el cuerpo humano con Ia finalidad de mejorar Ia funcionalidad de algún miembro, esta ortesis es clasificada como ortesis dinámica. Una ortesis dinámica se puede definir como un grupo de férulas articuladas que, mediante Ia utilización de diversos dispositivos dinámicos, consiguen mejorar Ia funcionalidad del algún miembro. Las ortesis dinámicas cuentan con secciones independientes en los segmentos del antebrazo, muñeca, mano y dedos, a las que se les añade diversos componentes articulares para obtener el movimiento y Ia funcionalidad del miembro. Para que las ortesis funcionales dinámicas puedan desarrollar las funciones antes reseñadas se necesita conseguir un buen efecto de fijación como requisito previo. Respecto a este efecto de fijación se puede considerar dos aspectos diferentes:
1. La fijación de Ia ortesis sobre el segmento corporal al que se dirige, y 2. La fijación necesaria para que actúen los elementos dinámicos.
La fijación de Ia ortesis sobre el miembro superior se consigue utilizando una serie de cinchas de sujeción, que permiten aplicar fuerzas distribuidas a Io largo de los diferentes segmentos corporales implicados. Por otro lado, para que los elementos dinámicos desarrollen su acción, es necesario disponer de una base, o estructura rígida (Ia propia ortesis), que sirva de punto de aplicación de aquellas fuerzas externas que provocan el efecto dinámico.
En el estado de Ia técnica existen algunas patentes cuyo objetivo es el desarrollo de ortesis dinámicas para el miembro superior. Tal es caso de Ia patente US4180870 (Universal-orthese, Radulovic R., Karisik M., Wolfer K.R.) que se centra en el desarrollo de una ortesis universal para el miembro superior cuya función es asistir los movimientos del hombro y del codo en pacientes con algún tipo de debilidad en el miembro superior. Éste también es el objetivo de Ia patente US6179799 (Orthosis for supination and pronation of the wrist, Doran R.E.), con Ia diferencia de que el movimiento asistido es el movimiento de prono-supinación del antebrazo. La principal desventaja de estos dispositivos es Ia incapacidad de aplicar fuerzas controlables externamente en cada una de las articulaciones en que actúan. Además, no están dotadas de sensores para evaluar o medir el comportamiento del miembro superior. La medida de los movimientos del miembro superior es el objetivo de otras patentes. Un ejemplo es Ia patente US6334852B1 (Joint movement monitoring system, Seyl V.C.), que se centra en el desarrollo de una ortesis de miembro superior para Ia medición de varios movimientos de Ia articulación en Ia muñeca de su usuario. En el estado de Ia técnica también existen algunos dispositivos cuyo objetivo es el amortiguamiento del movimiento tembloroso a través de pesas, patentes US6458089 (Methods and devices for reducing trembling, Ziv-Av A.), US5058571 (Hand-held gyroscopic device, Hall; W.D.) y US6695794B2 (Active tremor control system, Kaiser; K.W., Hall; G. E.). Este tipo de dispositivos se caracteriza por poseer una masa que oscila en contrafase al movimiento tembloroso, cancelando de esta manera el temblor. Esta carga mecánica es independiente del tipo de movimiento realizado por el paciente (voluntario, tembloroso o una combinación de ambos). Como consecuencia el paciente siente una carga resistiva ante su movimiento intencional y por tanto una falta de confort. Otros inconvenientes son que estos tipos de dispositivos deben ser ajustados a las características de cada paciente y además, son capaces de controlar el temblor solamente en una dirección previamente definida.
En resumen, no existe en el estado de Ia técnica dispositivos ortésicos que: - permitan Ia aplicación específica de cargas en función de las características concretas del tipo de temblor (frecuencias y amplitudes variables) ni de las características particulares en un instante determinado del temblor en un paciente, que por Io general serán distintas a las de cualquier otro instante.
- sean capaces de aplicar cargas biomecánicas, activas o pasivas, al movimiento tembloroso en el mismo nivel en que es producido, o sea, en Ia articulación del miembro.
DESCRIPCIÓN DE LA INVENCIÓN Breve descripción de Ia invención
La patente que se presenta se refiere a una plataforma ortésica ambulatoria para Ia monitorización, diagnóstico de temblor y validación de estrategias de control, tanto activas como pasivas, para Ia supresión del temblor patológico al tiempo que preserva el movimiento natural. El dispositivo se desarrolla en torno a una estructura articulada que permite soportar un conjunto de sensores y actuadores necesarios para Ia realización de las tareas de monitorización, diagnóstico y aplicación de estrategias de control.
La arquitectura de Ia plataforma está formada por un programa informático para Ia gestión, configuración y presentación de Ia información del sistema de ayuda al diagnóstico y cancelación del temblor, un dispositivo programable para Ia ejecución de los algoritmos de adquisición de Ia señal de los sensores y control de Ia ortesis, los algoritmos para Ia captación, interpretación de Ia señal de los sensores y activación de los actuadores, circuitos electrónicos para Ia interfaz entre los sensores y actuadores con el dispositivo programable, Ia estructura de Ia ortesis y fuentes de alimentación para los actuadotes, además de los propios actuadores.
DESCRIPCIÓN DETALLADA DE LA INVENCIÓN
La presente invención se enfrenta al problema de proporcionar nuevos dispositivos ortésicos u ortesis dinámicas para Ia evaluación, el diagnóstico y tratamiento de pacientes humanos con temblor patológico en extremidades o miembros superiores.
La presente patente se refiere a un dispositivo ortésico dinámico de estructura rígida o elástica para Ia fijación de actuadores y sensores que posibilitan así Ia aplicación de diferentes estrategias de control para Ia supresión del temblor sobre tres articulaciones del miembro superior humano (el hombro, el codo y Ia muñeca). Opcionalmente, Ia estructura del dispositivo ortésico puede ser flexible (textiles o materiales elásticos) cuando se emplea como herramienta de objetivación, monitorización y/o diagnóstico del temblor, en cuyo caso no incluye los actuadores aunque si el resto de elementos. La aplicación de una fuerza externa (a través de un actuador externo) proporciona a Ia ortesis un efecto corrector. Secundariamente colabora en el mantenimiento de una correcta alineación de los diferentes segmentos implicados.
Esta ortesis está basada en Ia aplicación de esfuerzos internos, es decir, Ia fuerza se aplica entre segmentos consecutivos del miembro para suprimir el temblor de tres articulaciones. Los esfuerzos pueden ser resistivos, disipativos, inerciales o los resultantes de estrategias de control activas.
Este dispositivo también permite implementar diversas estrategias de control, tanto activas como pasivas, para Ia supresión del temblor en las articulaciones responsables de Ia flexo/extensión de Ia muñeca, flexo extensión del codo, además de Ia prono-supinación del antebrazo, de manera que se pueda aplicar estrategias de control independientes en cada una de estas articulaciones mediante, por ejemplo, Ia utilización del método protegido en Ia patente española P200302927 (método y dispositivo biomecánico de cancelación de temblor patológico) que protege una estrategia de control para Ia cancelación activa de temblor patológico. Este método podría ser programado en el controlador de Ia ortesis objeto de esta patente. Los sensores se emplearían para capturar el movimiento del usuario. Un algoritmo permitiría Ia determinación de Ia frecuencia principal del temblor. Esta información se emplearía para generar el movimiento en contrafase que cancela el temblor.
La utilización de este dispositivo ortésico dinámico se relaciona, por un lado, con una herramienta de objetivación, evaluación y diagnóstico de pacientes con temblores patológicos en condiciones normales o a los que se les aplique cualquier estrategia de supresión del temblor y, por otro lado, con una herramienta para el tratamiento de dichos pacientes con temblores. El temblor patológico al que se puede aplicar el objeto de Ia presente invención (tanto en su vertiente de objetivación, evaluación y diagnóstico, como en su vertiente de cancelación) es el temblor de reposo (característico de Ia enfermedad de Parkinson), el temblor postural (característico del temblor esencial) y el temblor cinético o intencional (de múltiple etiología).
Las principales ventajas de este dispositivo ortésico dinámico objeto de esta patente pueden ser resumidas como:
• Es un dispositivo portátil, no invasivo y ambulatorio (puede funcionar sin Ia necesidad de referencias externas),
• Se obtienen datos biomecánicos a partir de una multiplicidad de sensores cinemáticos y/o cinéticos relativos o absolutos, • Ofrece información directa del par y/o potencia del temblor de cada articulación del miembro superior, Io que nos permite estimar Ia contribución de cada articulación sobre el movimiento total del temblor en toda Ia cadena cinemática del miembro superior,
• Ofrece información directa de Ia contribución de cada articulación sobre el movimiento tembloroso total, y
• No limita el rango y Ia libertad del movimiento del miembro superior.
El esquema de este grupo de ortesis pasivas sirve como módulo básico sobre el que se puede añadir nuevos componentes. La ortesis facilita Ia interfaz entre el sistema de supresión del temblor sin referencia externa y el miembro superior. Esta aproximación presenta las siguientes ventajas:
- La ortesis facilita Ia interfaz con el miembro,
- Proporciona alineación entre Ia articulación y Ia ortesis,
- - Proporciona una estructura rígida que permite Ia aplicación de par para cancelación del temblor, y - - Los límites del rango de movimiento resultan en una seguridad adicional.
Así, un objeto de Ia presente invención Io constituye un dispositivo ortésico dinámico útil para Ia monitorización, diagnóstico y supresión de temblor patológico en cualquiera de sus variantes del miembro superior humano, en adelante dispositivo ortésico u ortesis de Ia presente invención, constituida básicamente por los siguientes componentes (Figura 1 ): a) Un estructura ortésica con articulaciones (1) para Ia flexo-extensión del codo, prono-supinación del antebrazo y flexo-extensión de Ia muñeca que permite el acoplamiento de un conjunto de actuadores para Ia aplicación de esfuerzos (4) y sensores (2, 3), responsable de Ia interfaz entre los actuadores (4) de Ia ortesis y el miembro superior del paciente, compuesta por: i. Una multiplicidad de sensores que permiten obtener Ia información cinemática del brazo humano, ii. Una multiplicidad de sensores que permitan obtener Ia información cinética sobre Ia fuerza de interacción entre el brazo y Ia estructura descrita en a), y iii. Una multiplicidad de actuadotes rotativos que permiten actuar sobre el órgano afectado; b) Un dispositivo electrónico programable responsable de Ia ejecución de diferentes estrategias de control para Ia supresión del temblor basado en las lecturas de los sensores descritos en los puntos a. i) y a.ii), de Ia generación de señales de control para el gobierno de los actuadores descritos en a. iii) y de Ia comunicación con un dispositivo externo (8). c) Modulo eléctrico de potencia (5) conectado al dispositivo electrónico descrito en b) y capaz de convertir Ia señal de comando proveniente de b) en una señal de potencia para el gobierno de los actuadores, y d) Un dispositivo externo conectado al dispositivo electrónico descrito en b) capaz de proveer una interfaz para acceder a todas las funcionalidades de Ia plataforma ortésica que permita: i. El ajuste de Io parámetros de las estrategias de control, ii. La definición de modo de operación de Ia ortesis, iii. La presentación de las variables medidas por Ia ortesis, iv. El análisis del movimiento tembloroso, v. La presentación de diagnósticos, vi. Almacenamiento de los datos adquiridos por Ia ortesis, vii. Generación de informes, y viii. Ia comunicación con Ia ortesis. Un objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que los sensores utilizados para medir las variables cinemáticas (posición, velocidad y aceleración angulares) de a.i) son sensores inerciales capaces de funcionar en un rango mínimo de frecuencias entre 0 y 30 Hz, y entre otros, a título ilustrativo y sin que limite el alcance de Ia invención, pertenecientes al siguiente grupo: goniómetros, giroscopios y acelerómetros y de fuerza. Se han descrito temblores hasta una frecuencia máxima de 17-20 Hz, por Io que el rango hasta 30 Hz es conservador y comprende todos los posibles temblores. En el ejemplo, se emplea un sensor que va más allá.
Otro objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que los sensores utilizados para medir las variables cinéticas descritos en a.ii) son sensores de fuerza capaces de funcionar en un rango mínimo de frecuencias entre 0 y 30 Hz, y entre otros, a título ilustrativo y sin que limite el alcance de Ia invención, pertenecientes al siguiente grupo: conjunto de galgas extensométricas montadas en Ia configuración de Puente de Wheatstone.
Otro objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que los actuadores rotativos de a.iii) son motores para producir Ia acción canceladora del temblor, y entre otros, a título ilustrativo y sin que limite el alcance de Ia invención, pertenecientes al siguiente grupo: motores ultrasónicos y un conjunto de motor de corriente continua y reductor. Como una realización concreta de Ia invención se ha utilizado un motor DC plano como ejemplo de motor de corriente continua y reductor. Otro objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que el dispositivo electrónico programable de b) es capaz de adquirir señales de los sensores, procesarlas, analizarlas y generar una señal de control de los actuadores y de implementar estrategias de seguimiento e identificación del temblor basadas en un algoritmo en el dominio de Ia frecuencia o del tiempo, entre otros, a título ilustrativo y sin que limite el alcance de Ia invención, pertenecientes al siguiente grupo: estándar IEEE-1057, Filtro de Kalman y el Filtro de Benedict-Bordner (J. M. Belda-Lois, J. Sanchez- Lacuesta, M. J. Vivas-Broseta, E. Rocon, L. Bueno, J. L. Pons, Tremor movement analysis techniques: an approach towards ambulatory systems, Asistive Technology - Shaping the future, pp. 827-831 , 2003).
Una realización particular del dispositivo ortésico de Ia invención es aquel en el que el dispositivo electrónico programable de b) pertenece al siguiente grupo: un microcontrolador, un microprocesador, un DSP o un ordenador ejecutando algún sistema operativo de tiempo real.
El dispositivo ortésico de Ia presente invención puede acoplarse o adaptarse por un experto en Ia materia a otras realizaciones existentes del mercado, como por ejemplo puede utilizarse el método protegido por Ia patente española P200301767 como el algoritmo de cancelación del temblor ejecutado por el dispositivo electrónico programable descrito en el punto 1.b).
Otro objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que el dispositivo externo de d) es capaz de comunicarse con el dispositivo electrónico programable de b) funcionando como una herramienta de configuración capaz de: a) ajustar los parámetros de las estrategias de control ejecutadas por el dispositivo electrónico de b), y b) definir el modo de operación de Ia ortesis. Otro objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que el dispositivo externo de d) funciona como una herramienta de interfaz entre el dispositivo ortésico y el observador capaz de: a) presentar las variables medidas por Ia ortesis, b) analizar el movimiento tembloroso, c) presentar diagnósticos basados en los valores medidos, d) almacenar los datos adquiridos por Ia ortesis, y e) generar informes sobre los experimentos realizados.
Otro objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que el dispositivo externo d) puede operar en ambos modos, es decir, como una herramienta de configuración y de interfaz. Otra realización particular del dispositivo ortésico de Ia invención es aquel en el que el dispositivo externo d) pertenece al siguiente grupo: un ordenador, una PDA y un Pocket PC.
Otro objeto particular de Ia invención Io constituye el dispositivo ortésico de Ia invención en el que Ia comunicación entre el dispositivo electrónico programable de b) y el dispositivo externo de d) se realiza, entre otros medios, a título ilustrativo y sin que limite el alcance de Ia invención, pertenecientes al siguiente grupo: a través cables utilizando protocolos de comunicación TCP/IP o seriales, de manera inalámbrica utilizando Ia tecnología Bluetooth, Ia tecnología Hornero, Ia tecnología Wifi y Ia tecnología de infrarrojos IrDA.
Otro objeto particular de Ia presente invención es el dispositivo ortésico dinámico de Ia invención en que Ia estructura del mismo no es rígida sino que es flexible (textiles o materiales elásticos). Esta alternativa del dispositivo se emplearía como herramienta para Ia objetivación, monitorización y/o diagnóstico del temblor, en cuyo caso además no incluye los actuadores aunque si el resto de elementos.
Por otro lado, Ia plataforma objeto de esta patente posee tres modos operativos prácticos distintos:
1.- Monitorización - Ia plataforma actúa como un sistema de medida y caracterización del temblor tanto cualitativa como cuantitativamente. De esta manera no ofrece ningún tipo de oposición al movimiento del paciente, sea este movimiento voluntario o tembloroso. En este modo de operación Ia plataforma trabaja en tiempo real, permitiendo leer, procesar los datos y registrar el temblor para diferentes tareas específicas a realizar por el paciente. De esa manera se comporta como una herramienta de adquisición y análisis de señales de temblor y puede ser usado como una ayuda para el diagnóstico médico (el tipo y grado de temblor que padece un paciente) y para observar el progreso de un tratamiento.
2.- Intervención Pasiva - Ia plataforma actúa como un dispositivo capaz de amortiguar mecánicamente los efectos causados por el temblor, aplicando fuerzas sobre el miembro superior para compensar las vibraciones originadas por el temblor y estabilizar el movimiento voluntario del usuario. Es importante resaltar que en este modo Ia plataforma permite Ia evaluación del funcionamiento de Ia estrategia de control a través de Ia lectura de las señales de los sensores. De esa manera se puede apreciar Ia reducción, o no, de los efectos del temblor sobre el miembro superior.
3.- Intervención Activa - Ia plataforma suprime el temblor como un sistema dinámico, generando señales disipativas proporcionales a Ia intensidad del temblor. Como en el modo Intervención Pasiva, en este modo Ia plataforma también permite Ia evaluación del funcionamiento de Ia estrategia de control a través de Ia lectura de las señales de los sensores, permitiendo así que se aprecie la reducción, o no, de los efectos del temblor sobre el miembro superior. Este tipo de intervención está basada en el método descrito en Ia patente P200301767.
Así, otro objeto de Ia presente invención Io constituye Ia utilización del dispositivo ortésico de Ia presente invención, en adelante utilización del dispositivo ortésico, en Ia monitorización y diagnóstico de enfermos humanos con temblores patológicos en condiciones normales o a los que se les aplique cualquier estrategia de supresión del temblor y, además, en el tratamiento de enfermos humanos con temblores. El temblor patológico al que se puede aplicar el objeto de Ia presente invención (tanto en su vertiente de objetivación, evaluación y diagnóstico, como en su vertiente de cancelación-tratamiento) pertenece, entre otros, a titulo ilustrativo y sin que limite el alcance de Ia presente invención, al siguiente grupo: el temblor de reposo (característico de Ia enfermedad de Parkinson), el temblor postural (característico del temblor esencial) y el temblor cinético o intencional (de múltiple etiología).
EXPLICACIÓN DETALLADA DE LOS DIBUJOS
Figura 1.- La figura representa esquemáticamente Ia arquitectura del dispositivo ortésico propuesto. El bloque 1 representa Ia estructura rígida con articulaciones para el codo, prono-supinación del antebrazo y flexo- extensión. Los bloques 2 y 3, respectivamente, representan los sensores cinemáticos y cinéticos acoplados a ésta estructura rígida. El bloque 4 representa los actuadores rotativos acoplados a Ia estructura rígida que permiten actuar sobre el órgano afectado. El bloque 5 representa un módulo eléctrico de potencia conectado al dispositivo electrónico programable caracterizado por convertir Ia señal de comando en una señal de potencia para el gobierno de los actuadores. El bloque 6 representa un dispositivo electrónico programable caracterizado por ejecutar diferentes estrategias de control para Ia supresión del temblor y capaz de generar señales de control para el gobierno de actuadores y de comunicarse con un dispositivo externo. El bloque 7 representa un dispositivo externo conectado al dispositivo electrónico capaz de proveer una interfaz para acceder a todas las funcionalidades de Ia plataforma ortésica y presentar los datos de una forma apropiada a los clínicos. El bloque 8 representa la comunicación entre el bloque 6 y el bloque 7 que puede ser implementada tanto de manera inalámbrica (Bluetooth, Hornero, Wifi or IrDA) como a través de cables (TCP/IP, serial cable).
Figura 2.- Ortesis para cancelación del temblor a nivel del codo. La figura ilustra esquemáticamente Ia estructura rígida para Ia articulación del codo. Acoplados a ésta estructura rígida están un conjunto de sensores cinemáticos y cinéticos además de un actuador rotativo que permiten actuar sobre Ia articulación del codo.
EJEMPLO DE REALIZACIÓN DE LA INVENCIÓN
Ejemplo 1.- Desarrollo de una solución ortésica para cancelación del temblor a nivel del codo.
Como una realización particular de Ia presente invención se describe una ortesis para Ia cancelación del temblor a nivel del codo. La ortésis está formada por una estructura rígida articulada que permite soportar un conjunto de sensores y actuadores para implementar las estrategias de control para Ia cancelación del temblor (Figura 2).
En este ejemplo se emplean como sensores cinemáticos dos giroscopios situados sobre Ia estructura rígida a ambos lados de cada articulación y que ofrecen información sobre Ia velocidad angular absoluta de los segmentos (brazo y antebrazo) en el plano de movimiento de Ia articulación. El rango frecuencial de medida está comprendido entre 0 y 50 Hz, cubriendo así todo el rango frecuencial de los posibles temblores.
Los sensores cinéticos empleados en este ejemplo para Ia medición de Ia fuerza de interacción entre Ia ortesis y el miembro son galgas extensométricas. Las galgas son responsables de Ia medida del par de cancelación aplicado por los motores sobre Ia estructura de Ia plataforma ortésica, por Io tanto, están montadas sobre Ia estructura de manera que miden solamente Ia fuerza perpendicular al eje del motor, de esta forma sus medidas no son influenciadas por fuerzas originadas en direcciones indeseadas. Las galgas extensiométricas están conectadas en un circuito de Puente de Wheatstone en una combinación de cuatro galgas activas (puente completo). Como actuador se utiliza un conjunto motor DC plano y reductor para generar el par de cancelación del temblor en Ia articulación de interés. El par total que puede ser generado por el conjunto motoreductor es de 8 N.m pero está limitado a través de Ia energía entregada por el modulo de potencia al motor a 3 N.m, suficiente para contrarrestar el par medio del temblor a este nivel sin Ia posibilidad de causar daños al usuario.
Todos los algoritmos utilizados para Ia cancelación del temblor fueron implementados utilizando los entornos de programación Matlab y RealTime Workshop. Los algoritmos son ejecutados por un ordenador personal sobre el Kernell de tiempo real xPCTarget. Este ordenador incorpora tarjetas de adquisición de Ia suficiente velocidad de captura y resolución, en este caso 200 Khz. y 12 bits respectivamente, además de una tarjeta de red con puerto de comunicación TCP/IP.
Se implementa una estrategia de identificación y seguimiento del temblor de acuerdo con el método descrito en Ia patente española P200301767. La frecuencia de muestreo seleccionada para el lazo de identificación es de 1 KHz. Toda vez que Ia estrategia seleccionada otorga Ia estimación con un retardo de una única muestra, el retardo de Ia estimación se reduce a 1 ms, despreciable en comparación con Ia frecuencia intrínseca del temblor que está comprendida entre 2 y 12 Hz.
Además, se implementa un modelo dinámico del miembro superior capaz de estimar en tiempo real los valores de par y potencia del temblor en cada articulación. Con este modelo es posible evaluar Ia contribución de cada articulación del brazo sobre el temblor en Ia extremidad del brazo. El método para Ia obtención de Ia cinemática del brazo es el método de las matrices de transformación homogénea (Fu, K. S.; González, R. C; Lee, C. S. G; Robotics: control, sensing, visión, and intelligence. McGraw-Hill, Inc. New York, NY, USA, 1997). El modelo desarrollado está basado en Ia parametrización Denavit- Hartenberg. Para modelar Ia dinámica del brazo humano fue utilizada una combinación entre el modelo cinemático, desarrollado mediante Ia técnica de matrices de transformación y parámetros antoprométricos humanos extraídos de las tablas de Leva (P. de Leva, Adjustments to Zatsioorsky-Seluyanov's segment inertia parameters, Journal of Biomechanics, 29, 9, pp. 1223-1230, 1996). Los parámetros relacionados a Ia dinámica del miembro (centro de masa, masa del miembro, matriz de inercia) han sido obtenidos a través de las correcciones de Leva a las tablas biomecánicas de Zatsiorsky and Selyyanoy (V.M. Zatsiorsky and V.N. Seluyanov and L.G. Chugunova, Methods of determining mass-inertial characteristics of human body segments, en Contemporary problems of biomechanics, CRC Press, pp. 272-291 , 1990). Para Ia implementación del modelo desarrollado se ha utilizado el algoritmo recursivo de Newton de manera que se permite Ia implementación de los algoritmos de estimación de una forma modular. Como dispositivo externo responsable por Ia implementación de Ia interfaz con el usuario se utiliza un ordenador personal. La interfaz con el usuario está programada en el lenguaje de programación orientado a objetos Visual C++. La comunicación entre el ordenador de control y el ordenador de interfaz es realizada a través de cables utilizando el protocolo de comunicación TCP/IP.

Claims

REIVINDICACIONES
1.- Dispositivo ortésico dinámico útil para Ia monitorización, diagnóstico y supresión de temblor patológico en cualquiera de sus variantes caracterizado porque comprende: a) Una estructura con articulaciones rígidas para el codo, prono- supinación del antebrazo y flexo-extensión de Ia muñeca que permite el acoplamiento de un conjunto de actuadores y sensores compuesta por: i.- Una multiplicidad de sensores de las señales eléctricas correspondientes a movimientos temblorosos que permiten obtener Ia información cinemática del brazo humano, ii.- Una multiplicidad de sensores de las señales eléctricas correspondientes a movimientos temblorosos que permiten obtener Ia información cinética sobre Ia fuerza de interacción entre el brazo y Ia estructura descrita en punto a), y iii. Una multiplicidad de actuadores rotativos que permiten actuar sobre el órgano afectado; b) Un dispositivo electrónico programable responsable de Ia ejecución de diferentes estrategias de control para Ia supresión del temblor basado en las lecturas de los sensores descritos en los puntos a. i) y a.ii), de Ia generación de señales de control para el gobierno de los actuadotes descritos en a.iii) y de Ia comunicación con un dispositivo externo de d); c) Modulo eléctrico de potencia conectado al dispositivo electrónico descrito en b) capaz de convertir Ia señal de comando proveniente de b) en una señal de potencia para el gobierno de los actuadores; y d) Un dispositivo externo conectado al dispositivo electrónico descrito en b) capaz de proveer una interfaz para acceder a todas las funcionalidades de Ia plataforma ortésica y de presentar los datos de una forma apropiada.
2.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque los sensores utilizados para medir las variables cinemáticas de a. i) son sensores inerciales capaces de funcionar en un rango mínimo de frecuencias entre 0 y 30 Hz.
3.- Disposi tivo ortésico según Ia reivindicación 2 caracterizado porque los sensores inerciales pertenecen al siguiente grupo: giroscopios, goniómetros, acelerómetros y de fuerza.
4.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque los sensores utilizados para medir las variables cinéticas descritos en a.ii) son sensores de fuerza capaces de funcionar en un rango mínimo de frecuencias entre 0 y 30 Hz.
5.- Dispositivo ortésico según Ia reivindicación 4 caracterizado porque los sensores de fuerza son un conjunto de galgas extensométricas montadas en Ia configuración de Puente de Wheatstone.
6.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque los actuadores rotativos de a.iii) son motores ultrasónicos.
7.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque los actuadores rotativos de a.iii) son un conjunto de motor de corriente continua y reductor.
8.- Dispositivo ortésico según Ia reivindicación 7 caracterizado porque el motor de corriente continua es un motor DC plano.
9.- Dispositivo ortésico según Ia reivindicación 7 caracterizado porque el reductor acoplado al motor de corriente continua es un reductor plano.
10.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque el dispositivo electrónico programable de b) es capaz de adquirir señales de los sensores, procesarlas, analizarlas y generar una señal de control de los actuadotes y de implementar estrategias de seguimiento e identificación del temblor.
11.- Dispositivo ortésico según Ia reivindicación 9 caracterizado porque las estrategias están basadas en un algoritmo en el dominio de Ia frecuencia o del tiempo.
12. Dispositivo ortésico según Ia reivindicación 10 caracterizado el algoritmo pertenece al grupo siguiente: estándar IEEE-1057, Filtro de Kalman y el Filtro de Benedict-Bordner.
13.- Dispositivo ortésico según las reivindicaciones 9 a Ia 11 caracterizado porque el dispositivo electrónico programable pertenece al siguiente grupo: un microcontrolador, un microprocesador, un DSP y un ordenador ejecutando algún sistema operativo de tiempo real.
14.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque el dispositivo externo de d) es capaz de comunicarse con el dispositivo electrónico programable de b) funcionando como una herramienta de configuración capaz de: a) ajustar los parámetros de las estrategias de control ejecutadas por el dispositivo electrónico de b), y b) definir el modo de operación de Ia ortesis.
15.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque el dispositivo externo de d) funciona como una herramienta de interfaz entre el dispositivo ortésico y el observador capaz de: f) presentar las variables medidas por Ia ortesis, g) analizar el movimiento tembloroso, h) presentar diagnósticos basados en los valores medidos, i) almacenar los datos adquiridos por Ia ortesis, y j) generar informes sobre los experimentos realizados.
16.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque el dispositivo externo d) puede operar en ambos modos según las reivindicaciones 13 y 14.
17.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque el dispositivo externo d) pertenece al siguiente grupo: un ordenador, una PDA o un Pocket PC.
18.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque Ia comunicación entre el dispositivo electrónico programable de b) y el dispositivo externo de d) se realiza a través cables utilizando protocolos de comunicación
TCP/IP o seriales.
19.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque Ia comunicación entre el dispositivo electrónico programable de b) y el dispositivo externo de d) se realiza de manera inalámbrica utilizando Ia tecnología
Bluetooth.
20.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque Ia comunicación entre el dispositivo electrónico programable de b) y el dispositivo externo de d) se realiza de manera inalámbrica utilizando Ia tecnología Wifi.
21.- Dispositivo ortésico según Ia reivindicación 1 caracterizado porque Ia comunicación entre el dispositivo electrónico programable de b) y el dispositivo externo de d) se realiza de manera inalámbrica utilizando Ia tecnología de infrarrojos IrDA.
22.- Dispositivo ortésico según las reivindicaciones 1 a Ia 21 caracterizado porque Ia estructura del mismo es flexible, preferentemente de materiales textiles o materiales elásticos y no incluye los actuadores.
23.- Utilización del dispositivo ortésico según las reivindicaciones 1 a Ia 22 en Ia monitorización y diagnóstico de temblores de enfermos humanos o para Ia evaluación del efecto de medicamentos sobre el temblor.
24.- Utilización del dispositivo ortésico según las reivindicaciones 1 a Ia 22 en el tratamiento de temblores de enfermos humanos.
25.- Utilización según Ia reivindicación 23 y 24 caracterizada porque el temblor patológico pertenece al siguiente grupo: el temblor de reposo (característico de Ia enfermedad de Parkinson), el temblor postural (característico del temblor esencial) y el temblor cinético o intencional (de múltiple etiología).
