Definitions
- the invention relates to an intervertebral implant with at least one hollow channel for receiving filler, in particular a pasty mass made of bone substance, for connecting two vertebrae adjacent in the spine according to the preamble of claim 1.
- Such an intervertebral implant has at least one holding element for the filler within the at least one hollow channel.
- an intervertebral implant with several openings is known which can be filled with bone material and enables the bone to grow into the implant.
- the disadvantage is the complex filling of the implant with a removable cover.
- the published patent application DE 195 41 114 A describes a flange-like intervertebral implant with a central opening for a filler (bone or bone substitute material), support plates with anchoring elements and a spacer with openings for a good nutrient supply.
- a filler bone or bone substitute material
- support plates with anchoring elements and a spacer with openings for a good nutrient supply With this implant, the hold of pasty fillers in the central opening is low, so that the filler can fall out during the implantation.
- An implant is known from EP 0 966 930 A that has a fillable hollow body and subsequent end caps that lie flat on adjacent vertebral bodies.
- the implant can easily be filled with bone or bone replacement material.
- the complex three-part construction of the implant is disadvantageous.
- the present invention seeks to provide an intervertebral implant that for fillers, especially pasty fillers, offers sufficient hold.
- an intervertebral implant with the features of claim 1. Accordingly, an intervertebral implant of the type mentioned at the beginning has at least one holding element for the filler, which is arranged within the hollow channel.
- the presence of the at least one holding element allows (pasty) fillers to adhere well to the intervertebral implant mentioned and prevents the filler from falling out during the implantation.
- the hollow channel extends in the intended position of the intervertebral implant in a direction that runs essentially parallel to the longitudinal axis of the spine running through the centers of the vertebral bodies adjacent to the intervertebral implant within the intervertebral implant.
- the at least one holding element is a groove arranged in the intervertebral implant, which, according to a variant of the invention, is at least partially designed as a groove running around the hollow channel. It is also possible for the groove to run around the hollow channel in a thread-like manner.
- the at least one holding element is a projection of the intermediate vertebral implant in the direction of the center of the hollow channel.
- the arrangement of a series of projections in the wall of the intervertebral implant that delimits the hollow channel is also advantageous. As a result, a good hold of (pasty) fillers in the implant is achieved.
- the intervertebral implant according to the invention has both at least one groove and at least one projection within the hollow channel as a holding element.
- the joint use of grooves and projections can have particularly favorable effects on the adhesion of (pasty) fillers in the hollow channel of the implant.
- the holding elements of the intervertebral implant arranged in the wall of the at least one hollow channel are preferably arranged essentially orthogonally to a central axis of the hollow channel.
- the holding elements arranged in the wall of the at least one hollow channel can also lie on a plane which is arranged essentially orthogonally to the central axis of the hollow channel, the central axis running through the center of the hollow channel of the intervertebral implant. This also has favorable effects on the adhesion of (pasty) fillers in the hollow channel.
- the holding element arranged in the hollow channel can be designed as a rough surface of the wall of the intervertebral implant that forms the hollow channel.
- a rough surface can e.g. B. by radiation with corundum be achieved.
- a mean roughness value of R a > 5 ⁇ m is advantageous.
- the intervertebral implant according to the invention has two sagittal legs which run essentially parallel to one another and are each connected to one another by an anterior, that is to say ventral, and a posterior, that is to say dorsal, transverse leg.
- the designations of the legs of the intervertebral implant according to the invention relate to the position of the intervertebral implant in the body of a patient which results when the implant is used as intended.
- the intervertebral implant according to the invention has an essentially annular structure. This provides optimal support for the neighboring vertebrae of the spine through the implant.
- the intervertebral implant according to the invention has two bone contact surfaces each running in a transverse plane. These bone contact surfaces, on which one of two neighboring vertebrae in the spine comes to rest after the implant has been introduced into the body of a patient, are each formed by the sagittal and transverse legs of the implant, each of which is essentially in a transverse plane sides of these sagittal and transverse legs. A good distribution of forces is guaranteed by the bone contact surfaces.
- at least one of the bone contact surfaces has a profile.
- at least one connecting element for example in the form of a mandrel, is arranged on at least one of the bone support surfaces.
- the intervertebral implant has a coupling element in the form of a threaded hole which is used to insert the intervertebral implant.
- the hole runs in a proper position of the intervertebral implant along a sagittal axis, this hole being particularly advantageously arranged centrally in the anterior transverse leg of the intervertebral implant.
- the surgeon can bring the intervertebral implant according to the invention into its intended position between two vertebral bodies.

- Figure 5C is a cross-sectional view of the intervertebral implant shown in Figure 4A, the section being taken along the median plane B-B.
- FIGS. 1A and IB show the same cervical intervertebral implant 1 from opposite perspectives. The following description therefore refers to both FIG. 1A and FIG. IB.
- the spatial orientation of the intervertebral implant 1 shown results from the coordinate system shown.
- the intervertebral implant 1 has two sagittal limbs 4, 5 which run essentially parallel to one another, each of which is interconnected by an anterior transverse limb 6 and a posterior transverse limb 7 are connected.
- the intervertebral implant 1 thus has an essentially ring-shaped structure.
- the first sagittal leg 4 forms, together with the posterior transverse leg 7 and the second sagittal leg 5, a horseshoe-shaped structure which is closed into a ring by the anterior transverse leg 6.
- the side surfaces of the posterior transverse leg 7 flatten to the rear so that these surfaces meet and a posterior edge is formed.
- the intervertebral implant 1 has a hollow channel 2 for receiving filler, in particular for receiving a pasty mass of bone substance.
- the hollow channel 2 runs parallel to the direction of the longitudinal axis of the spine, that is to say essentially along the longitudinal body axis within the intervertebral implant 1.
- a holding element in the form of a groove 3, which is designed here as a groove 3 which partially surrounds the hollow channel 2.
- the groove 3 can be formed in particular by drilling or milling the wall of the intervertebral implant 1 forming the hollow channel 2.
- the groove 3 shown is designed as a groove partially encircling the hollow channel 2, since the groove 3 is only provided by a surface section of the wall of the wall located centrally in the posterior transversal limb 7
- Hollow channel 2 does not run.
- the holding element arranged in the hollow channel can also be designed as a roughened surface of at least a portion of the wall of the intervertebral implant 1 forming the hollow channel 2.
- Such roughening of sections of the wall of the hollow channel 2 can by rough radiation, for. B. by air pressure irradiation with corundum, a material obtained from bauxite.
- An average roughness value of R a > 5 ⁇ m of the wall of the hollow duct 2 is preferred, the roughened section being able to have a geometrically undefined shape.
- the intervertebral implant 1 shown has a first bone support surface 8 and a second bone support surface 9. These bone support surfaces 8, 9, which essentially run within a transverse plane, are each formed by the sagittal legs 4, 5 and transverse legs 6, 7 of the intervertebral implant 1.
- the bone contact surfaces 8, 9 shown here each have a profile that facilitates adhesion to the adjacent vertebral bodies within the spine. Only a rear section of the posterior transverse leg 7 has no profiling in the variant of the invention shown here.
- a connecting means which in the embodiment shown here is designed as a mandrel 10.
- the variant of the invention shown here has three spikes 10 on the first Bone support surface 8, wherein a mandrel 10 is formed on the lateral ends of the anterior transverse leg 6, and a further mandrel 10 is arranged centrally on the posterior transverse leg 7.
- a second arrangement of thorns 10 corresponding to this first arrangement can be found on the opposite, second bone support surface 9.
- the intervertebral implant 1 has a coupling element in the form of a bore with a thread 11, which is arranged centrally in the anterior transverse leg 6 and, starting from a position of the intervertebral implant 1 as intended, runs along a sagittal axis.
- a circular cavity 12 is arranged laterally to the bore 11 in the front of the anterior transverse arm 6.
- Both the bore 11 and the circular cavity 12 are used by an operator to insert the intervertebral implant 1 into the intended position between two adjacent vertebrae of a patient's spine.
- an insertion instrument not shown here, is used, which has a holding means which is inserted into the bore 11 of the intervertebral implant 1.
- the cavity 12 serves for the spatial orientation of the implant in connection with the insertion instrument. This ensures correct implantation.
- the insertion instrument used by the surgeon has a holding means and a projection.
- the holding means can be designed in the form of a pin which is connected via an outer thread. Via this external thread, the holding means of the insertion instrument is screwed into the coupling element in the form of a bore 10 with an internal thread.
- a protrusion of the insertion instrument engages in a circular cavity 12 of the intervertebral implant 1 in order to enable spatial orientation of the implant 1 and thus to facilitate correct implantation by the surgeon.
- the operative insertion of the intervertebral implant 1 into the body of a patient is carried out ventrally after removal of the corresponding intervertebral disc.
- the intervertebral implant 1 stabilizes the vertebrae to one another, the hollow channel 2 filling with bone after healing and bridging the vertebrae.
- FIG. 2A shows a top view of the intervertebral implant 1 shown in FIGS. 1A and IB.
- the intervertebral implant in FIGS. 1 and 2 is therefore the same embodiment.
- the essentially annular structure of the intervertebral implant 1 is formed by the first sagittal leg 4 and the second sagittal leg 5, which are each connected to one another by the anterior transverse leg 6 and the posterior transverse leg 7. It can be seen that the first bone support surface 8 has a profiling which is formed from parallel, transversely running, pointed roof-like structures. There are also three connecting elements in the form of thorns
- FIG. 2B shows the intervertebral implant 1 shown in FIG. 2A, which was cut along the frontal plane AA shown in FIG. 2A, in a top view of the cut surface.
- the posterior transverse limb 7 is laterally delimited by the first sagittal limb 4 and the second sagittal limb 5, both sagittal limbs running essentially perpendicular to the sectional plane.
- a groove 3 is arranged in the posterior transverse leg 7 and is interrupted by the bore 11.
- Both the first bone support surface 8 and the second bone support surface 9 each have spikes 10 for improved retention of the intervertebral implant 1 with the adjacent vertebral bodies.
- FIG. 2C shows the intervertebral implant shown in FIG. 2A in a sectional view, the cut being made along the median plane designated BB.
- the first sagittal leg 4 is delimited by the anterior transverse leg 6 and the posterior transverse leg 7, the two transverse legs 6, 7 being shown in section.
- a groove 3 for holding the (pasty) filling compound runs inside the first sagittal leg 4 of the intervertebral implant 1.
- the profiling of both the first bone support surface 8 and the second bone support surface 9 can also be seen.
- Thorns 10 are arranged on both bone support surfaces 8, 9.
- the section through the intervertebral implant 1 runs through the center of the bore 11, which is arranged in the posterior transverse leg 7 and which serves to introduce the holding means.
- the holding means is part of the insertion instrument available to the surgeon for inserting the implant.
- FIGS. 3, 4 and 5 each show further embodiments of the intervertebral implant 1, which differ in particular in the design of the holding element 3, 3 ', 30, 300. Therefore, only the distinguishing features are discussed below.
- FIGS. 3 to 5 each show a plan view of the intervertebral implant 1 on a first bone support surface 8 in the illustration designated as A.
- a cross-sectional view of the intervertebral implant 1 shown in the corresponding illustration A is shown, the cut being made along the frontal plane AA.
- Figure C shows a cross-sectional view of the corresponding figure A, with the intervertebral implant 1 shown in each case being cut along the median plane BB.
- the holding means is also designed as a groove 3 '. However, in this case the groove 3 'is arranged in a spiral around the hollow channel 2, so that a thread-like structure results.
- the thread-shaped groove 3 ′ runs mirror-symmetrically within the wall adjacent to the hollow channel 2.
- the plane E extends equidistant from the bone support surfaces 8, 9 at the same height through all four legs 4, 5, 6, 7 of the intervertebral implant 1.
- the central axis M runs perpendicular to the plane E within the hollow channel 2 the central axis M (in the case of a circular hollow channel 2) through its (geometric) center point.
- the holding means is shaped in the form of projections 30.
- projections 30 are essentially cuboid and likewise allow good (pasty) fillers (e.g. bone or bone replacement material) to adhere within the intervertebral implant 1.
- a plurality of projections 30 are arranged in a circumferential manner in the wall delimiting the hollow channel 2.
- the projections 30 all lie within a plane E connecting them.
- This plane E is orthogonal to the central axis M of the hollow channel 2.
- the plane E can be a plane of symmetry of the intervertebral implant 1.
- the holding means is designed as a pin 300.
- the projection 30 shown in FIG. 4 extends with its longest side along the wall of the hollow channel 2
- a pin 300 extends with its longest side essentially perpendicular to the wall of the hollow channel 2, that is to say with its longest side in the direction of through the hollow channel 2 central axis M.
- these pins 300 have an X-ray-proof material or consist of an X-ray-proof material, they can simultaneously serve as X-ray markers for determining the position of the intervertebral implant 1.
