PANTIES WITH AN INFLATABLE SEGMENT
Technical field that this invention is concerned about This invention belongs to the medical and the veterinary field of science in general, i.e. to the orthopedic methods or appliances for the conservative treatment of bones and joints relating to the panties with an inflatable segment for treatment of the congenital hips' displasie as well as for preventing of hips' anomalies in the course of development, and hips' sprains with newborn infants. According the International patent classification the sign is as follows: A 6 IF 5/37 and A 61 F 5/34.
Technical problem
This invention has solved the construction problem of the appliances, which, if placed the first day after delivery, will surely prevent painlessly the hips' sprain development with newborn infants, i.e. the congenital displasies or hips anomalies, avoiding injuries of the exceptionally delicate child's skin.
Technique state As for technique state, there are more construction solutions of the appliances being used for treatment of the congenital anomalies of newborn infants. These congenital anomalies appeal in the form of evidently very broadened displasy (flat patella of hips joint) as well as hips sprain appearing in the first weeks after delivery of human newborn infants (Theories concerned about these anomalies have been worked out by the applicant and the inventor). In this purpose4, in the period of completing hips development, it is necessary to provide, according to the world standards, the position of low extremities with flextion in the hips and abduction of 60 (sixty) degrees from the central line. This includes the position of low extremities presenting the stretched child's legs under the angle of 60 (sixty) degrees, to make pressure on the bottom of hip joints' patella. This legs' position (60 (sixty) degrees abduction) can be reached, according to the technique state, with a series of appliances including linen panties making impossible the movement in the hips' joint, as the main defect . This absence of movements in the joints causes directly some damages of the joints' parts, requiring surgical treatment, later on. Besides that, these defect in the construction of nowadays appliances can be clearly seen in the requirements that child's legs must be immediately placed in the position of 60 (sixty) degrees abduction being impossible with respect to the degree child's skeleton development as
well as child's skin what can easily cause some undesirable changes on them with the provoked paints. As for the appliances in technique state being used according to the above mentioned, we have to describe here so called BARLOW'S and FONROSEN'S splint. They have been made by modified aluminium strips of definite shape, width and length being wrapped around predicted parts of the child's body, keeping his legs in the desirable position. These appliances are missing the absolute fixing without possibility of making any movement. The problem is also in difficult child's hygiene keeping with decubit wounds occurrence because they are not removed during two months. The further serious problems with these appliances are evident in the possibility of hips' head decay because of the loss of blood and shortage of circulation as there are no movements in the mentioned period. In the technique state in this field there are the constructive appliances' solutions in the form of soft or hard napkins made by different firms (Waikert, Niva). The above mentioned problems with the infants have been provoked by the hard napkins. There is also so called Puty's appliance in the form of a hard cushion placed in a linen bag that may be placed between child's legs without taking it off. But the problem is in the possibility that the child pulls out one leg while making some movements annulling, in this way, the effects of treatment. In the technique state, the appliances in the form of so called Pavlik's small belts have also been well known, but they are made with the belts system for binding children's legs in the purpose of preventing their movements. These solutions are suitable to be applied only when the child is strong enough to make pressure on the bottom of the joints' patella. As a rule, much time is necessary for the beginning of treatment or prevention has been lost up to them. There is another appliance from the technique state applied in this field, Hilgen- Reiner's device. It is in the form of a steel ring covered with skin in the child's dimensions with the lateral extensions and fastened belts on them. Children's extremities are bound with the belts for the mentioned extensions with total fixation of hip's joints. Defects are practically the same as with the similar device solutions with appearance of decubit wounds. In the technique state there is also the solution called Orthopedic reposition in the form of plaster panties, so called Lorence plaster with the legs fixed in the desirable position. These panties are warn for two months, at least. In this medical field, there is also "Zenith traction" in the form of the appliance fixing the child to the bed with the help of belts while his legs are fixed with strips for extension with pulleys placed on the frame above the child. In the technique state there is a patented solution whose author is the inventor and the applicant of this form. The invention refers to "Pneumatical panties for treatment of the congenital displasy and the hips' strain with the newborn infants. One of this solution's defects refers to the fact that the panties were inflated in the whole volume so that a newborn infant's body was banded while lying in any position. Beside that, the inflation has decreased the angle between the child's legs which must have 60(sixty) degrees; the possibility of moving has also been decreased because the inflated elements envelop the hip on the whole. In the technique state some partial solutions upon the subject of the invention in this branch should be mentioned:
Abductional walking device for hips with the regulator from the patent application P- 920/79,
Device for renewing flexibility of pelvie- femoral joints from the patent application having been submitted by the Soviet organization "Centralno naucno-isledovalski institut traumatologiji I ortopediji imeni NN Priorova".
Abductional walking device for treatment of the congenital -residual hip's displasy from the patent application P-l 1374/85 and
Prothesis for treatment of disturbed and strained joints in the hip from the patent application P-760/82.
Presentation of the essential invention's elements Defined technical problem has been solved according to the invention by constructing the panties with segment for inflation putting the infant's extremities in an ideal position of 60 (sixty) degrees abduction. In this purpose, the panties envelop the central part of twofold "Baby foil", separated by a porous membrane for the air making a triangular chamber in the front part, as well as, the rectangular back chamber also for inflating. A standard value is built into the front chamber, in the most desirable way, through month. The middle part of the panties is lengthened by the part of the reversed "T" shape of the simple "Baby foil" with joints arch lateral sides are prolonged by the vertical flat sides completed with the joint back side. At the same time, the central part of the panties are prolonged, on the opposite side, by a rectangular extension of the simple "Baby foil" in the middle of which there are two buckles for holding and fixing two supporting belts with the ends fixed toward the back part of the reversed "T" shape. There have been predicted two rows of standard clasps placed in a certain distance to button the panties up. Their pairs are placed on the rectangular extension on the opposite side of the panties. The corresponding "woman" parts of the clasps are placed in their interior where can be put all kinds of the changeable napkin as well as the simple ones for the baby's swaddling. According to the above mentioned facts, the essential importance of this invention is as follows. Using the easily accessible position when the infant is lying on his back, the air inhaling is being done (mostly through month) through the mentioned value. Now the back chamber is being filled with the air from the front chamber through the porous partition. In this moment we should take care of the following: If it's impossible to put immediately the infant's legs into the desirable position under the angle of 60 (sixty) degrees what might be the case with the infants with the congenital pathology, it should not be made by force. In this case the panties are placed with the inflatable segment toward the invention and filled up to that tolerant limit that will not provoke painful sensation and skin damages. During this treatment the hips in the panties are free according to the invention, the infant moves normally his legs. This is of a special importance while the infants is sleeping when it is left to the effect of the inflatable segment and when it is making "Human position" with his legs and hips what was the essential purpose of the invention.

Detailed description of the invention
The pictures show the panties with a flat able segment for treatment of a congenital hips' anomalies with newborn infants according to the invention. According to the picture 1 in the developed shape they envelop the middle part from the so called "Baby foil", separated by the porous partition (12) for the air. This middle part of the panties has been properly shaped to insert it between the infant's legs as a part of the panties keeping them in desirable position and under the definite angle. In this purpose, the mentioned middle part has been divided by the partition (12) to the front chamber (9) in a triangle shape and the back chamber (11), both of them serving for inflation. During this treatment, on the front chamber (9), on the accessible place there has been built the standard value (10) for the air that can be taken through month while inflating the both chambers (9) and (11). In this way, the shaped middle part (1) of the panties according to the invention has been prolonged with the extension (2) in the rectangular shape and width made of the same material, i.e. of the simple "Baby foil". This extension (2) has been planned for putting it over the infant's stomach, and on the flat part there have been fixed two standard buckles (3) for holding and fixing of the supporting buckles (7). At the same time, the standard buckles 8,8a have been built on the lateral sides of this extension: buckles 8 as the parts of the same buckles on the part 5 of the panties and the buckles 8a as their "woman" parts in the inside of the panties, serving for free keeping of all kinds of napkins as well as the simple linen ones. On the opposite side of the middle part 1 of the panties, according to the invention, in the continuation of the back chamber 11 for inflating, whose lateral sides have been carried out in the arch shape in relation to the back part of the upper leg thigh, the part 5 of the panties has been made, being, on the whole, in the shape of the letter "T" and made of the same simple "Baby foil" with partially arch shaped lateral sides with arches in the continuation of lateral sides arched of the middle part 1. In the continuation, these lateral sides pass into the vertical flat sides of the suitable width and end the back transversal side 6 of this part 5 of the panties. On this part 5 there have been put two rows Al, A2 and Bl, B2 lateral buckles 8 in a definite distance being predicated for adjusting the volume of the part 5 of the panties, while buttoning them up, depending on stomach size of the infant. At the end, in the middle of the back side 6 of the part 5 in "T" shape of the panties, there have been fastened two parallel supporting belts 7 in the proper length for holding and fixing in buckles 3 placed toward the ends of the extension 2 of the same panties. Panties with an inflatable segment according to the invention aie applied in this way: Their extension 2 is placed over the stomach while the parts in the shape of an arch are put between the, upper legs and the widened part 5 is taken from the back part with the supporting belts 7 which, after throwing them over the infant's shoulder, are pulled into the buckles 3 where they can be buttoned up in a desirable length. At the same time, the interior side 6 of the part 5 of panties together with the back side of the back chamber 11
as well as with the back side of the front chamber 9, lean properly on the infant's body over previously inserted napkins buttoned with "woman" parts of the buckles 8a. In this moment, synchronized buttoning of the suitable buckles 8 is carried out, arranged in the rows Al, A2 and Bl, B2 according to the pictures 1 and 3. In this way, the whole low part of the infant is wrapped with properly placed panties with an inflatable segment in accordance with the invention. Normally in this position of the panties, the value 10 for the air also takes a suitable position, and it is easily accessible when the infant is being on his back. In this position of the infant, the placed panties are being filled with air when the back chamber 11 in the rectangular shape is being filled with the air from the front chamber 9 through the porous partition for the air 12, the attention should be paid to the following: if it's not possible to put the legs immediately in the desirable position under the angle of 60 (sixty) degrees As it's the case with the infant with the congenital pathology, it should not be done by force. In the case the panties with an inflatable segment are placed and filled up to that tolerant limit which will not provoke painful sensation or skin damage. Afterwards, each following day the amount of the air in the segment is increasing and the desirable goal has been reached slowly and painlessly. Material used for making the panties with an inflatable segment for treatment of the congenital anomalies of hips with the newborn infants, according to the invention is partially elastic and waterproof which does not provoke the allergic reactions on the infant's skin and which is known under the commercial name "Baby foil". This material makes possible an easy washing and wiping not requiring more time for drying. Because of the considerable child's growth in the period of using the panties with a segment, according to the invention they have been made in three sizes marked with Roman numerals I, II, III. The panties no. I are applied in the period of the first six weeks of life, the panties no. II for the second six weeks, while the panties no. Ill are applied with the children older than three months where the treatment began later on.
