IMPLANT TEXTILE DE REPARATION
La présente invention a pour objet un implant obturateur ou prothèse textile de réparation destinée, en particulier mais non exclusivement, à l'obturation de hernies.
La paroi abdominale chez l'être humain est constituée de muscles et de graisse. Il arrive que cette paroi laisse passer le péritoine et les viscères qui forment alors une excroissance à la surface de la peau. On parle alors d'une hernie ombilicale ou inguinale en fonction de sa localisation.
De façon, à résoudre les problèmes concernant les hernies, les praticiens chirurgiens ont recours à différentes techniques chirurgicales pour refermer l'orifice herniaire :
- par fil de suture sous tension; - par utilisation d'un bouchon obturateur désigné couramment par« PLUG «;
- par coeliochirurgie : Mise en place de la prothèse de renfort en TEP ou en TAP.
Afin de réparer la paroi abdominale dans la région herniaire on implante chez l'homme un bouchon textile. Ce bouchon ou « plug » sert à éviter une récidive. Une fois colonisé par un tissu cicatriciel, celui-ci a tendance à être expulsé vers l'extérieur par le phénomène de la pression abdominale.
Un certain nombre de solutions techniques de formation de bouchons pouvant résister à cette pression ont été proposées, notamment dans les documents US-A-5 356 432, US-A-5 116 357, US-A-5 147 374, US-A-5 456 720 qui font appel à un thermoformage pour donner la forme désirée de cône ou de cylindre. De tels implants connus comprennent une base colonisable mono ou multicouches qui, après pose, prend une forme en tronc de cône ou de cylindre, la base étant appliquée contre la paroi de l'ouverture par des moyens de rappel
élastiques centrifuges insérés soit entre deux couches de tricot, soit intégrés à la paroi externe de l'implant.
Le brevet américain US-A-2391343 décrit un obturateur qui dans une première position est plat et peut être mécaniquement conformé en cône lors de sa mise en place. Des bandes élastiques diamétrales sont disposées entre deux couches de 1 ' implant imperméable et provoquent le déploiement du cône lorsqu ' il est en place.
Il a, par ailleurs, été proposé dans FR-A-2 769 825 un bouchon dont la conformation obturatrice n'est obtenue que lors du positionnement de celui-ci dans la cavité à obturer. Les éléments de renforts reσtilignes sont radiaux et ne s'étendent, depuis le bord externe de la base, que sur une longueur inférieure au rayon de la base lorsque celle-ci est circulaire. Ces éléments de renfort rigides axialement se prêtent mal à la conformation du bouchon à 1 ' intérieur du trou à obturer lorsque celui-ci est de forme irrégulière en épaisseur.
Un premier objet de l'invention est de pallier cet inconvénient.
Selon l'invention, l'implant obturateur du défaut d'une paroi comprenant une base plane dans une première position et des moyens élastiques s 'adaptant, après pose, au volume à obturer dans une seconde position est caractérisé en ce qu'il comprend une base de tricot, au centre de laquelle sont disposés au moins deux enroulements cylindriques diamétraux creux.
Ces rouleaux ou enroulements cylindriques creux peuvent être constitués dans des feuilles de même nature que la base. Ils peuvent être fixés en leur milieu, par exemple par suture au centre géométrique de la base de tricot, leur partie extérieure (ou supérieure après pliage) profitant ainsi d'un certain débattement qui leur permet une adaptation en fonction de la forme de l'ouverture. La couture des
cylindres provoque leur aplatissement selon une surface sensiblement carrée qui rigidifie le fond de l'implant qui subit la pression maximale. Ces renforts élastiques se trouvent ainsi à 1 ' intérieur du bouchon alors que dans les implants connus, ils se trouvaient à l'extérieur de celui- ci.
Selon une autre caractéristique de l'invention, un lien est passé à travers le tricot de la base. Il suffit ainsi de tirer sur les deux extrémités du lien pour que 1 ' implant prenne une forme cylindrique facilitant 1 ' implantation en formant un certain nombre de cavités creuses et de cavités remplies par les enroulements plies selon leur axe central. Les implants peuvent ainsi être livrés à plat ce qui facilite leur manutention.
Dans un autre mode de réalisation, les enroulements ne sont pas fixés au centre, mais sont solidarisés de la base par le lien. On obtient ainsi un nombre de cavités multiplié par deux, l'enveloppe externe de l'implant étant ainsi plus régulière.
Par leur forme, les enroulements conservent, dans la seconde position "repliée", une élasticité axiale et radiale ce qui permet à l'implant d'épouser parfaitement la forme de l'ouverture, même si celle-ci est irrégulière en section et/ou en direction axiale. La forme cylindrique des enroulements provoque par traction sur le lien une structure extérieurement plissée et, par écrasement des cylindres, l'élasticité de la paroi du bouchon, bien que le textile utilisé ne soit pas spécialement élastique.
D ' autres caractéristiques et avantages de l'invention apparaîtront au cours de la description qui va suivre d'un mode particulier de réalisation, donné uniquement à titre d'exemple non limitatif, en regard des dessins qui représentent :
- la figure 1, une vue d'un implant à plat dans un premier mode de réalisation;
- la figure 1A, une vue de profil de 1 ' implant de la figure 1; - la figure 2, une vue du même implant conformé en vue de sa pose;
- la figure 3, un second mode de réalisation d'un implant dans lequel le lien passe a travers la base et à travers les enroulements en position de pose; - la figure 3A, une vue de profil de l'implant de la figure 3;
- la figure 4 , une vue de 1 ' implant de la figure 3 conformé en vue de sa pose.
Sur l'ensemble des figures, les mêmes références désignent les mêmes éléments . Sur la figure 1 , on voit que 1 ' implant se compose d'une base 1 tricotée qui, dans l'exemple représenté, est circulaire. Dans le centre de la base sont suturés dans la région centrale 5 de la base 1, deux rouleaux ou enroulements 2 et 3, perpendiculaires l'un à 1 ' autre et qui peuvent être constitués à partir de deux disques analogues au disque de base. Un lien ou tresse 4 est passé par exemple en quatre points dans les mailles de la base 1 à une distance réglable du bord externe ou du centre. Les enroulements 2 et 3 restent libres par rapport à la feuille de base sauf dans leur région de fixation.
La figure 1A montre mieux le passage du lien 4 qui, dans ce mode de réalisation, passe au-dessus des rouleaux et, sur la figure, au-dessus du rouleau 3. Les points de passage du lien sont, par exemple situés à 10 millimètres du bord de la base 1.
Comme cela apparaît sur la figure 2, une traction sur le lien provoque la formation d'une structure sensiblement cylindrique, les enroulements étant repliés autour de leur ligne de couture. Dans l'exemple représenté, on voit que le nombre de cavités ainsi formées est égal à quatre.
La figure 3 représente un second mode de réalisation d'un implant selon l'invention à plat et en vue par-dessus. Comme précédemment, il est formé par une base 1 comme la précédente. Mais dans ce mode, le lien est disposé de manière différente. En effet, le lien 4 qui passe dans ce cas en huit points A à H à travers la base passe également en deux points de chaque enroulement 2,3. Le lien 4 passe en huit points à travers la base passe également en deux point de chaque enroulement. Dans ce cas, le lien 4 passe dans les cylindres 2 et 3 en deux points, l'implant prenant, comme précédemment, par traction sur le lien 4, une forme sensiblement cylindrique facilitant l'implantation en formant un certain nombre de cavités creuses 11 et de cavités 12 remplies par les enroulements 2,3 plies selon leur axe central, par traction sur le lien 4.
L'implant conformé par le chirurgien, comme représenté sur la figure 4, prend une forme sensiblement cylindrique facilitant l'implantation.
Dans le cas de passage du lien tel que décrit en regard des figures 3 et 4, les enroulements peuvent ne pas être fixés au centre 5, mais solidarisés de la base par le seul lien 4. Ils peuvent ainsi présenter un certain débattement par rapport à celui-ci ce qui permet à l'implant de s'insérer dans des orifices de formes très irrégulières.
La prothèse est une prothèse textile tricotée avec un monofilament de polypropylène ou en tout autre matériau biologiquement compatible. Avantageusement, elle peut être constituée de fils de PGA ( acide polyglycolique) , de PLLA (acide poly-L-lactique) ou leurs copolymères, présentant la propriété d'être résorbables à long terme. La pose de cette prothèse permet une chirurgie ambulatoire et peut se faire sous anesthésie locale ou loco-régionale.
Pour poser cet implant, le chirurgien tire sur la tresse ce qui conforme l'implant en forme de cylindre. Les cavités creuses et pleines s ' appliquent automatiquement contre la
