  • the laparoscopic suturing needle and needle holder have been utilized separately to suture digestive organs. How ever, one often finds ii difficult to separately use them to suture the thick inner abdominal wall near incisional or inguinal hernias under a laparoscope. Therefore, man ⁇ ' surgeons use a stapler to attach the surgical mesh. To our disappointment, the common shallow stapler cannot entirely penetrate the mesh, peritoneum and abdominal wall muscles simultaneously, and while the spiral-shaped deep stapler can fasten the thick layers, its ligation function is sometimes insufficient. Furthermore, this stapling procedure requires a dissection of the peritoneum, and the high cost of the stapler is also a burden. Consequently, a special suturing apparatus is needed to help surgeons to entirely suture the mesh, peritoneum, and abdominal w all muscles simultaneously with common thread under a laparoscope.

Abstract

Cette invention se rapporte à une aiguille de suture extracorporelle, qui est constituée par une aiguille de suture courbe associée à la pince et au manche (D) du porte-aiguille de façon à former un seul corps allongé. Cette aiguille comporte un chas (A) à sa pointe, une partie courbe (B), une tige (C) et un manche (D). Sa longueur totale atteint environ 35 cm de sorte qu'elle peut être introduite dans la cavité abdominale à l'aide d'un laparoscope. De là, elle peut pénétrer dans toute l'épaisseur du filet chirurgical et de la paroi abdominale jusqu'à ce que le chas (A) à la pointe de l'aiguille émerge hors du corps du patient, pour que l'on puisse y passer un fil et effectuer une ligature. Ainsi, le filet chirurgical, le péritoine et les muscles de la paroi abdominale peuvent être suturés ensemble avec cette aiguille, et on peut y réaliser une ligature avec un fil commun appliqué par voie extracorporelle.
