Description

NOUVEAU DISPOSITIF DESTINE A REMEDIER AUX INCONTINENCES URINAIRES LEGERES NEW DEVICE FOR THE REMEDY OF LIGHT URINARY INCONTINENCES
The present invention relates to a new device intended to remedy mild urinary incontinence.
The present invention relates more precisely to a device intended to eliminate or limit the drawbacks linked to what is called stress incontinence and which more particularly affects young and active women.
It is estimated that the pathology of urinary incontinence affects in FRANCE more than a million subjects of the female sex, and many are the young and active women who have difficulties to refrain, often enough, from urinating so that it bothers them in their daily life.
This incontinence is essentially characterized by a leakage of urine in a jet which occurs during a sudden effort accompanied by an abdominal overpressure; this can be caused by coughing, laughing, sneezing or even a sudden change in position. This is a real handicap which progressively worsens and manifests itself frequently in active or sporting life (running, brisk walking, tennis, golf ...).
Among the various mechanisms incriminated in these phenomena, the most frequently encountered corresponds to a ptosis, that is to say a descent of the bladder at the level of the bladder neck following a relaxation of the muscles of the pelvic diaphragm: the ureterovesical junction thus finds outside the enclosure of the pressures of the abdomen and the tightness of the bladder can no longer always be guaranteed.
In addition to wearing disposable pads, the treatment of these stress incontinence consists in trying to rehabilitate the perineal muscles, that is to say a strengthening of the muscular floor which supports the bladder and closes the urethra; such re-education unfortunately often only gives disappointing results.
Until now, if this rehabilitation failed, the only treatment consisted of a surgical intervention carrying out a suspension of the anterior vaginal wall, often moored to the retropublian ligaments. However, this is a "heavy" treatment which is for this reason reserved for cases of major discomfort. The object of the invention is to remedy this drawback by proposing an alternative to the above-mentioned treatment, in particular to women who do not wish to undergo a surgical intervention or whose discomfort is insufficient to justify such an intervention.
To this end, the invention provides a device for remedying mild urinary incontinence in women, intended to be introduced into the vaginal cavity and maintained under the bladder so as to support the latter as well as the urethra through the vaginal wall. anterior and to oppose the ptosis of the bladder neck.
The international patent application PCT / FR93 / 01206 already describes a device constituted by a removable body devoid of sharp angles made of a non-adherent material, inert from the chemical and biological point of view and not likely to create problems of local tolerance or general allergy.
This is why, in accordance with a preferred characteristic of the invention, this document defines the removable body as consisting of a biocompatible plastic material.
A titre d'exemple, non limitatif, on a cité avantageusementBy way of non-limiting example, advantageously cited
It should be noted that fluorinated resins have already been the subject of numerous biomedical applications (bone, vascular prostheses and catheters) and have always made it possible to meet the requirements specific to this particular field.
It was found that the use of a polytetrafluoroethylene for the production of the device defined in the international patent application PCT / FR93 / 01206 created an industrial problem difficult to solve. The molding of polytetrafluoroethylene is practically impossible to carry out and, therefore, a part in such a material does not can be produced only by extruding and digging the piece with the cookie cutter to form a cavity. As a result, polytetrafluoroethylene parts of this conformation cannot be mass produced and must be machined by hand or with a small tool. This is the reason why it appeared desirable to be able to have a device for treating stress incontinence which has similar characteristics but which can be manufactured in a simple and easily industrializable manner from a biocompatible and completely inert material.
The object of the invention is, in fact, to make available to the public a device intended to be introduced into the vaginal cavity and to be maintained under the bladder, easy to put in place and easy to remove, characterized in that '' it is made up of two symmetrical male and female half-shells which fit into each other by a series of flanges, to form an ovoid body pierced with orifices in the central part and having, at the lower part, a set of ribs facilitating gripping .
The two half-shells (l ') and (1 ") are joined by a common insertion point (6) placed on the inner rim and around which the two half-shells can turn to open or nest by a series of male and female tabs arranged symmetrically with respect to the vertical axis.
In a preferred embodiment of the device according to the invention, shown in Figure 2, the device, ovoid in shape, has at the middle part, a succession of vertical ribs (2) (2 ') (2 ") intended to facilitate holding the device in place. Figure 2 also shows the two half-shells (V) and (1 ") open around the point of articulation (6) and having assembly tabs (4) (4 ' ) (4 ") and (5) (5 ') and (5") arranged symmetrically at the rate of two at the top, one of a more elongated shape in the central part and two at the bottom of the device. By pressing on the two half-shells, the male tabs (5) (5 * ) (5 ") are inserted into the female tabs thus ensuring the closure of the device. The female tabs (4) (4 ') (4 ") have the shape of a gutter in which the male tab is inserted by coming to bear on the inner rim of said gutter.
We figured the assembly tabs seen in section and arranged symmetrically around the ovoid body. The device (1) has, at the top, a slight opening (9) and at the bottom, a wider opening (10).
The device is shown seen from the front, closed, with three vertical ribs in the center and three horizontal ribs in the lower part.
In another embodiment shown in Figure 2, the vertical ribs (2) (2 ') (2 ") can be replaced by a multiplicity of small circular orifices (8) of variable diameter, arranged in horizontal rows over all the periphery of the median part of the device. These rows of orifices can be more or less significant. It has appeared that two to six rows of orifices represent the optimum number to ensure a regular and easy flow of local secretions. orifices of the same diameter are advantageously arranged along straight lines oblique to the horizontal plane of the device according to the invention.
In a particular embodiment, an internal periodic protection article (7), not shown, in defibrated paper pulp (fluff) or in compressed cellulose wadding, is previously placed inside the cavity of the device.
In this particular embodiment, it is possible to use the device closed by vertical ribs (2) (2 ') and (2 ") and horizontal ribs (3) or that providing a multitude of small circular orifices (8), arranged in horizontal rows around the entire periphery of the middle part of the device.
In either of these devices, small circular holes deposited in horizontal rows are placed around the entire periphery of the upper part of the device and allow the flow of blood in the cavity.
The device according to the invention is made of inert material, non-allergenic and capable of being formed by molding.
In this regard, high density polyethylene, polypropylene or polyamides are the materials of choice.
The dimensions of the device are calculated so as to fit as closely as possible to the dimensions of the vaginal cavity. Depending on the conformation, the total length of the device can vary from 40 to 50 mm and the diameter of the device from 25 to 35 mm. The device according to the invention can easily be inserted. Its extraction can be done using a flexible wire disposed at the base of said device in a permanent manner and which allows or which facilitates its traction. When the device includes an internal periodic protection article, its extraction is done by this same wire.
Advantageously, the device according to the invention can be put on the market, packed in a case and this case can conveniently be used to throw the device away after use.
The invention has been described in broad outline with reference to the drawings. It is understandable that it will be possible to modify the execution or the presentation of the device without departing from the scope of the invention.
In an additional embodiment, it may be advantageous to provide, in addition, a welding point which closes the shell.
The device according to the invention can also be formed of two halves which fit into a horizontal plane, one of the halves having a collar with a diameter very slightly smaller than that of the other half so as to ensure a perfect fitting.
