Abstract

The invention relates to a gravity infusion system which has a main bottle (11) and a reserve bottle (10) whose outlets meet in the application tube. An intercalary tube (13) connects one to the other directly by means of two hollow punches (131, 133) which pierce the rubber plugs. When the level in the main bottle goes down and approaches the empty level, the punch (131) of the intercalary connection is left exposed and the air in the bottle (11) passes through said connection to the reserve bottle (10) which, upon receiving such air, begins to supply liquid, thereby replacing the empty bottle. Such repeated process finds particular application in prolonged treatments. The object of the invention is to avoid troubles resulting from a delay in the replacement of the bottle and, consequently, to avoid having medical staff and other persons accompanying the patient dependent on the moment of the bottle replacement.

5 infusión parenteral con recambio automático de frasco conforme se describe a continuación y se representa gráficamente en dibujos a efectos de su mejor comprensión.5 parenteral infusion with automatic bottle replacement as described below and is plotted in drawings for the purpose of better understanding.
A conventional parenteral gravity infusion system consists of a bottle containing the liquid to be perfused suspended at a certain height and in an inverted position. Its rubber stopper is pierced by a needle that gives way to the outside air, a necessary condition for the exit of the liquid, and by a hollow punch that connects it to a tube terminated in a needle usually introduced into the patient's vein. The liquid descends by gravity and, due to its hydrostatic pressure, penetrates the vein. A drip chamber interspersed in the tube makes visible the perfusion rate that is regulated by means of a clamp that narrows the walls of this tube more or less.
In intravenous treatments that require prolonged infusion and therefore a continuous replacement of bottles containing drugs, the delay in the replacement of any of them results in disorders that may have some importance. They occur because when the bottle is exhausted, the outlet tube is also emptied, the liquid height of which decreases until it reaches a level determined by the venous pressure and at that moment the flow of the liquid stops.
The consequences of this situation are two: the first is that the air present in the tube, prevents the replacement of the new bottle can be done directly since its liquid, when descending, introduce air into the vein. This requires disconnecting the system and purging it of air before resetting. The second and most important consequence is that when the exit of liquid through the needle, blood penetrates it. As blood outside its natural ducts tends to clot, and more so if it is without movement and in contact with air, the needle is likely to be blocked. In that case it is forced to remove that needle and - perform a new venous puncture. Finding another point to do so can be easy, but for patients who have been undergoing treatment for a long time and have suffered many punctures, there are fewer and fewer useful areas to achieve an effective intravenous route, so it can be difficult to get it and in In some cases there is no recourse but to practice venous catheterization.
Living these situations as a professional, patient or companion, is enough to persuade yourself of the need to have a device that prevents them from happening and that is also as affordable as the perfusion system itself so that its usefulness can reach all patients They need it. This way I know. You can also discharge the monitoring voltage that is added to the patient's environment and healthcare staff for this reason.
A system fulfilling the desired and automatically connecting a previously installed reserve bottle, is achieved with an optoelectronic type liquid level detector applied to the bottle in use, which upon detecting a decrease below an established, active value, by means of the suitable circuit, an electrovalve or an actuator that operates a three-way valve which connects the reserve bottle to the common tube. Although effective, the disadvantages of this system are the cost, its complexity and the need for maintenance and energy source.
The object of this invention aims to perform the function of automatic replacement, but with a different approach, so as not to use mechanisms or circuits and avoid the aforementioned inconveniences. In order to dispense with the valve, I don't know directly controls the exit of the liquid from the reserve bottle, but its air intake is controlled, which is what determines the exit of the liquid. For this, the free end of its air intake tube is introduced into the bottle in use and there acts as a liquid level detector. When the liquid descends to a level lower than the tube, the air penetrates through it to the reserve bottle and determines the exit of its liquid to the common tube taking the relief of the spent bottle. The structure, operation and features of the present invention are described in detail with reference to the attached drawings.
Figure 1 shows the overall arrangement of a parenteral perfusion system, 10 with automatic bottle replacement.
Los frascos 11 y 12 suspendidos en posición de servicio y con sus bocas sensiblemente al mismo nivel, tienen sus tapones perforados por los punzones 14 y 15 que los comunican con sus tubos de salida 16 y 17 los cuales se unen por medio de un conector en "Y" 18, al tubo de salida com n 19, que es continuado por todos los elementos y accesorios que puedan ser empleados en este tipo de perfusiones tales como la cámara de goteo 20, la pinza de control de flujo 21, el conector en "Y" para recibir otra conexión 22, el bulbo de goma para inyección de medicación suplementaria 23 y la aguja para la inyección parenteral 24.The bottles 11 and 12 suspended in service position and with their mouths substantially at the same level, have their caps perforated by the punches 14 and 15 that communicate them with their outlet tubes 16 and 17 which are joined by means of a connector in "Y" 18, to the common outlet pipe 19, which is continued by all the elements and accessories that can be used in such infusions such as the drip chamber 20, the flow control clamp 21, the connector in "Y" to receive another connection 22, the rubber bulb for supplementary medication injection 23 and the needle for parenteral injection 24.
Directly communicating the jars is the intercalar duct 13 consisting of two hollow punches 131 and 133 joined by the tube 132. The hollow punches, have an outlet at right angles and their light is progressively increasing from the small upper hole 134 and 135 to the smaller whose diameter corresponds approximately to that of the union tube 132. It has been designed thus because the direct union of a thin vertical tube with another thick horizontal tube introduces a discontinuity which, being a capillary diameter, It causes a loss of charge when the air-liquid interface meniscus passes. The length of the vertical part of the punches allows their holes 134,135 to be located at a level higher than that of the respective hollow punches 14 and 15 of liquid outlet, a necessary condition for its correct operation since they are intended to pass the bottle air
It is convenient that the jars are arranged with their mouths substantially at the same level. If they are different in size as shown in Figure 6, a compensation element such as that in Figure 7 can be used.
Once this assembly is installed as described, the infusion cannot be started since there is no outside air intake in the system. To start it, the rubber cap of one of the jars is drilled, e.g. 11 with a needle 40 provided with a filter, and this bottle will begin to supply the liquid through its outlet tube 16. The other bottle 12 remains inactive because it has no air intake. When the bottle 11 is running out, the hole 134 is uncovered and as the liquid level continues to fall, the air from the bottle that enters through the air intake 40 passes through this hole 134 to the hollow punch 131, tube 132 and punch 133, leaving in bubbles through hole 135 in the other bottle. The interleaving duct 131,132,133, filled with liquid before, due to the high level in the bottle 11, has become permeable to the air and has become the air intake of the bottle 12 which, therefore, can initiate its supply of liquid to the system , taking over from bottle 11 without any personnel intervention.
However, the function of this device is not limited to this particular case of launching a reserve bottle, but rather that this process can be repeated, it fulfills its mission of serving for prolonged treatments. If it is necessary to continue the treatment, the spent bottle 11 can be removed and replaced by a new one whose cap is pierced with the punches 14 and 131, but not with the air intake needle 40. In the absence of air intake is inoperative, becoming a reserve bottle. Its liquid now floods the interleaving duct 131,132,133 which, therefore, can no longer transport air to the flask 12 for operation. Therefore, it is necessary to change the needle 40, which was in the depleted bottle, to this bottle 12 so that it acted as an air intake and continued its operation.
The new situation after the replacement of the bottle is similar to the initial one in Figure 1, with the difference that now the bottle 12 is the main bottle and when it is exhausted the new bottle installed as a reserve that has replaced 11 will come into operation. Obviously, this process can be repeated every time a new bottle is needed.
The advantages of this automatic replacement device are several: The first is the avoidance of the disorders produced by the delay in the replacement of the depleted bottle as previously described. Another advantage is that the operation of installing a new bottle while the bottle is running, can be done at any time. -The new bottle will be operational only when the bottle in service runs out. This means that to replace a bottle in service, there is a margin of time equal to that used in emptying, which can be a few hours. The health personnel or the companions of the patient are freed from exercising a close watch on the infusion, because the moment of the supply and installation of a new bottle becomes independent of the moment in which the automatic replacement of the bottle is exhausted; That is why this supply of the new bottle can be left for when it is more convenient for the service. Another advantage of this automatic replacement device can also be seen considering that in a conventional infusion, if it were established as criterion that a bottle can be removed and replaced from the moment it is not left, p. ex. , more than 10% of its content, the time frame to do it is 10% of the time it takes to consume the entire bottle (consumption time). It turns out that: "The time frame to make a substitution is proportional to the waste you want to tolerate." A reduction of the% of waste leads to a reduction in the room for maneuver, which requires increased vigilance.
With this automatic replacement device, the operating range is always 100% and the waste * zero, since the bottle is completely depleted.
Another advantage is its ease of manufacture, since the materials are those that are usually used in this type of devices and therefore its low cost is in the range of these parenteral perfusion systems, and as they are sterilizable and disposable.
Figure 2 shows another embodiment in which the two punches, the liquid outlet and the intercalar duct, are fused, forming a single punch 25 with two tubular connections 26 and 27 and two holes 28 and 29. The connection of Liquid outlet 27 is connected to an air evacuator sleeve 30. This modality has the advantage of making the replacement of the bottle easier and ensuring that the hole in the intercalating duct 28 is higher than that of the liquid outlet 29. Figure 3 Show the punch section.
La figura 5 muestra una sección de este conjunto .Figure 4 shows another embodiment in which the air intake, which in the preceding modes is made by the needle 40, is incorporated into the single punch 31 with its inlet 32 provided with a plug 33 and its hole 34. The advantage In this way, the system has the two possible air intakes incorporated and the selection of one of them is it does by removing the corresponding cap. The air evacuator 35 has also been incorporated into the punch. Figure 5 shows a section of this assembly.
Figure 6 shows a solution to compensate for large differences in the sizes of the bottles, by means of an adjustable suspension element 36, in order to ensure that their mouths are substantially at the same level.
