Abstract
The knee bandage comprises a sleeve (20) for the thigh and a sleeve (21) for the leg connected by a first diagonal strip (22) crossing a second diagonal strip (23). Between the cross-over point of the diagonal strips (22, 23) and the sleeve (20) for the thigh, a strip-free loacation is provided for the ball-joint. The sleeves (20, 21) may have a closed annular shape or be open, and have fixing means (25) at their ends. A process for determining the position of the intermediate strips of the bandage consists in making a projection of the part to be supported of the anatomical ligament and/or muscular apparatus onto the periphery of the knee joint, and in placing the intermediate strips along the projection. These intermediate strips may also be placed along the geometrical resultant of two or more projections.
Description
KNIEBANDAGE KNEE BANDAGE
Die Erfindung betrifft eine elastische Kniebandage vom Typus zum Hineinschlüp fen oder zum Anlegen, vorzugsweise eine zum Anlegen offene Kniebandage.The invention relates to an elastic knee brace of the type to slip into or put on, preferably an open knee brace for putting on.
Zum prophylaktischen Schutz des Kniegelenks, aber auch zur Unterstützung der Funktion eines leicht oder schwerer geschädigten oder des alternden Knies, dienen aller Art Knie-Bandagen und -Orthesen. Kniebandagen unterstützen durch ihre elastische Uirkung den Band- und Muskelapparat, während die Knieorthesen mit nicht-elastischen, knochenfunk t ionsähnlich starren Elementen Band- und Muskelapparat des Knies noch zusätzlich festigen und unterstützen sollen. Scharnierstellen von Orthesen sind jedoch schwierig zu plazieren und vor allen Dingen in den Bewegungsablauf einzubringen. Ein sich beugendes Knie verhält sich jedoch nicht wie ein einfaches Scharnier mit einem festen Drehpunkt, vielmehr verschiebt sich dieser kontinuierlich beim Abrollen des Condylus, was biomechanisch sehr schwierig nachbildbar ist. So bewähren sich immer noch die elastischen Unterstützungsmechanismen in Form von Bandagen aller Art.
Es ist nun das 2iel der Erfindung eine Knie-Bandage zu schaffen, deren elastische Zugwirkungen den Band- und Muskelapparat des Knies gezielter unterstützen, als dies bei einfachen elastischen Kniebandaoen der Fall ist.All types of knee supports and orthoses are used for prophylactic protection of the knee joint, but also to support the function of a slightly or severely damaged or aging knee. Die Erfindung geht von folgender Idee aus. Der Anteil der knöchernen Führung ist beim Kniegelenk trotz markanter Zug-, Druck- und Torsionsbel astungsfähigkeit recht gering, die Hauptbelastung wird vom passiven Band- und vom aktiven Mus- kelapparat getragen, welche das Kniegelenk "zusammenhalten". Dazu haben sich nebst einfachen, direkten Verbindungen wie bspw. das Ligamentum col laterale, auch mehr oder weniger stark gekreuzte Verbindungen wie bspw. das Ligamentum - cruciatum anterius im Zusammenspiel mit dem Ligamentum - cruciatum posterius (Kreuzbänder) als stärker oder wie die menisci.als schwächer gekreuzte Verbindungen. Während die steiler gekreuzten Kreuzbänder im Bereich zwischen den - Kondylen angeordnet sind, umschlingen die schwächer gekreuzten Menisken die Kondylen im Aussenbereich. Die kollateralen Bänder sowie das Ligamentum patallae mit dem Tendo quadricipitis (Kniescheibensehne) verbinden die Femur mit der Tibia und Fibula in direkter Verbindung. Alle diese schräg bis gerade verlaufenden Verbindungen werden in praktisch jedem Belastungsfall, Zug-, Druck-, Rotationsbelastung Lind Mischformen davon, in ihrem Zusammenspiel optimal belastet. Diesen Mechanismus echt nachbildende Kniebandagen kann es aus Anordnungsgründen nicht geben, aber durch Projektionen
dieses biologischen Halte-Apparates an die Gelenkperipherie mit dem Verlauf derProjektion folgenden elastischen Bänder, lässt sich eine sehr gute Gelenkhilfe realisieren. Projiziert man bspw. die Lage der Kreuzbänder lateral und/oder medial sowie anterior und/oder posterior, so können elastische Bänder einer Kniebandage diesen Projektionen nachgeführt werden oder in einer Vereinfachung folgt die Bandführungeinzelnen Resultierenden von Projektionen.The invention is based on the following idea. Im gezeigten Beispiel greift das Diagonalband 22 an der Stelle M medial (zwischen den Beinen) an und verläuft schräg nach unten unter der Kniescheibe vorbei zur Stelle L, wo das Diagonalband 22 dann lateral (Knieaussenseite) verankert ist. Das andere Diagonalband 23 verläuft von der Knieaussensei te schräg nach unten zur Knieinnenseite. Gut zu sehen sind nun die Bandage- Diagonalbänder 22 und 23, wie sie sich unter der Kniescheibe kreuzen. Bei beiden Figuren 2 und 3 erkennt man den gleichsinnig ähnlichen Verlauf der Bandage- Diagonalbänder 22,23 mit den Kreuzbändern 10,11 des Kniegelenks, die so durch die Zugwirkung der elastischen Bandage- Bänder im Belastungsfall teilentlastet werden. Dies ist auch beim gebeugten Knie der Fall (Figur 2). Der diagonale Verlauf der Di agonalbänder unterstützt die Haltewirkung des Band- und Muskelapparat des Kniegelenks bei Rotationsbewegungen zwischen Femur und Tibia: bei den Kreuzbändern entlastet das Bandage- Di agonal band 22 bspw. das hintere Kreuzband 11 und
das Bandage- Diagonalband 23 das vordere Kreuzband 10. Direkt werden auch die kollateralen Bänder 15 in ihrer torsionshemmenden Wirkung unterstützt. Ferner bewirken die rund um Ober- und Unterschenkel laufenden Schenkelmanschetten, durch Druck auf die Schenkel- Muskeln eine indirekte aktive Unterstützung der muskulären Verbindungsstellen im Kniegelenk. Wie Figur 4 zeigt, besteht diese Kniebandage aus zwei Schenkelmanschetten 20,21, die hier in einer offenen Version mit Verschlussbändern 25 dargestellt sind, und aus zwei an den Schenkelmanschetten befestigten gekreuzten Diagonalbändern 22 und 23. Die Schenkelmanschetten sind in der Regel für Ober- und Unterschenkel verschieden gross, entweder einstellbar, bspw. mittels dem praktischen Klettenverschluss, oder fest, durch ungeteilte
Manschetten zum Hineinschlüpfen. Die e l ast i schen Bänder sind vorzugsweise so gewoben, dass sie sich durch verschiedene Elastizitätsstärken der Ränder auch in der Bandebene krummziehen lassen. Die Längs-Elastizität des Bandes ist bspw. an den Aussense iten 31,32 geringer, damit sich die Manschetten dem konischen Verlauf der Schenkel entsprechend anpassen. Ebenso kann man mit den Diagonalbändern 22,23 verfahren, die übereinandergelegt gekreuzt an den Stellen 28 mit den Sehen-
ke lmansche t ten vernäht sind und bspw. an den Seiten 33 einer geringere Elastizität aufweisen als an der anderen Parallelseite. Auf diese Weise schmiegen sich die Diagonalbänder von unten her sanft an die Kniescheibe ohne sie hoch zu drücken.FIG. 5 shows the knee brace of FIG. 4 in its open form. Mit einem im Bereich der Oeffnung P angeordneten Ring 35, bspw. aus Gummi, Silikon o.ä. bestehend, der mit, hier an der Ober- und Unterschenkelmanschette befestigten, Bändern gehalten ist, lässt sich die verstärkte Patellaführung recht einfach realisieren. Figur 6 zeigt noch eine vorteilhafte Ausgestaltung der Kniebandage mit einer längeren Oberschenkelmanschette und einer kürzeren Unterschenkelmanschette, wie dies bei den üblichen anatomischen Gegebenheiten des Beines angebracht ist. Als Verschlussmittel sind hier Oesen 38 gezeichnet, in welche die Bandenden 39 eingeschlauft werden können.
Die Bemessungen der Länge der Diagonalbänder und der Kreuzungswinkel richten sich nach anatomischen Erfordernissen aus. Auch ist es möglich neben einer gemäss statistischen anatomischen Abmessungen ausgelegten "Standard-Kniebandage", die mehr zur Prophylaxe gedacht ist, mit Hilfe der gezeigten Projektionsmethode auch spezielle posttraumatische Laxitäten individuell auszugleichen. Die Führung von weiteren Bändern zwischen den Schenkelmanschetten, bewirkt zusätzliche Hilfs- funktionen und die spezielle Ausgestaltung der verwendeten Bänder bezüglich Schnitt und bezüglich Elastizitätsgradienten im Band, öffnet weitere Möglichkeiten zur verfeinerten Ausgestaltung der Kniebandage gemäss Erfindung.
