My invention relates to a combined statement-lolhnk and envelop, and its object is to provide a statement-sheet or similar device whichvcan be folded into the forni of an envelop to utilize the superscription olz the statement as the address of the envelop and which can readily be opened, leaving` the statement in the usual form.
My invention contemplates a sheet having wings upon each side at its lower portion and provided with perforations in alinernent with edges of the upper half ol the sheet. The eXtreme lower p art of the sheet comprises a the sheet is folded in the form of an envelop the name, and address of the person to whom the statement is rendered is visible.
In my invention when the sheet is doubly folded at the center the lower half thereof inoloses the upper half and each side wing is folded upon itself. The envelop may then be sealed by merely securing together said folded parts of the wing by adhesive material ailixed thereto or in any other suitable manner.
