I SPECIFICATION forming part of Letters Patent No. 769,933, dated September 13, 1904.

Beit known that I, VILLIAM H. BAssINGuR, a citizen of the United States, residing at Passaic, in the county of Passaic and State of New Jersey, have invented certain new and useful Improvements in Bill-Heads or Statement- Sheets and Envelops, of which the following is a specification, such as will enable those skilled in the art to which it appertains to make and use the same.
The object of this invention is to provide an improved blank comprising a combination bill-head and statement-sheet and envelop for use by bankers, merchants, and others of this class and on which a billor statement may be written, after which the blank may be folded into an envelop and securely sealed, a further object being to provide a blank of this class comprising a body portion containing the business-card of a business man, firm, or company using the same; also a place for the address of the customer or client, said body portion being provided at one side with an extension which constitutes the bill-head or statement-sheet proper and on which is printed the name and address of the client or customer in the manner of an ordinary bill-head or statement-sheet, the opposite ends of the body portion of the blank being provided with folding' members which constitute a part of the envelop, and said body portion being also provided opposite the bill-head or statementsheet extension with a folding flap, on one side of which is placed the directions for opening the envelop.
The invention is fully .disclosed in the following speciication, of which the accompanying drawings form a part, in which the separate parts of my improvement are designated by suitable reference characters in each of the Views, and in which- Figure 1 is a plan view of the sheet or blank which I employ; Fig. 2, a front view showing the sheet or blank folded into an envelop; Fig. 3, a back View showing the sheet or blank partially folded or ready for sealing; Fig. 4, a view similar to Fig. 3, showing the blank completely folded and sealed; and Fig. 5, a section on the line 5 5 of Fig. 4.
In the practice of my invention I provide a Serial No. 205,495. (No model.)
sheet or blank composed of any suitable paper and comprising' a body portion a, provided at one side with an extension o and at each end with a folding end member c and opposite the extension b with a folding Hap member al'. The body a or the back thereof contains in the upper left-hand corner the business-card of the party using the blank, `and in practice the name of the customer or client is also written on the back of the body portion c, said back of the body portion a forming' the front side or face of the envelop when the blank is folded and sealed. The parts c, and d are separated from the body portion a by dotted or folding lines e, e2, and.
e3, respectively, and the extension bis divided by transverse dotted or folding lines f and f2 into three parts in the form of construction shown, said parts being designated by the reference characters g, g2, and g3, and in practice the address of the customer or client is written or printed on one side of the part g, and the partsg2 and g3 are ruled vertically, as Shown at 7L, in the manner of an ordinary bill-head or statement-sheet. Thefpart g of the side extension I) of the body portion a of the blank constitutes the main part of said extension, while the parts g2 and g3 are supplemental parts.
One of the folding end portions c of the blank is provided near the dotted line e2 with an opening 21, which in the construction shown is circular in form, but which may be of any desired shape, and in practice when a banker or other business man, firm, or company desires to use my improved combination bill-head or statement-sheet and envelop the name of the client or customer is written on the back of the body portion c of the blank, as indicated in Fig. 1, and the part g of the eXtension I; contains the business-card of the party orparties using the blank, and the bill or statement is written on the parts g2 and g3 in the usual manner. The blank is then ready to be folded into the form of an envelop, and in this operation the part g3 is folded forwardly on the line f2 in the position of the blank shown in Fig. 1 over the part g2. The part g2, with the part folded thereover, is then folded over the part g, and the part g,

with Vthe parts g2 and g3 folded thereon, is folded backwardly ov-er the body portion a. The
`blank partially folded in this manner is then turned over, the end portions c are folded over, as shown in Fig. 3, after which the flap d is folded and sealed.
The flap Z is provided near its free edge or opposite the body portion ce with a perforated line d2, forming a strip d3, one side of which is gummed, as shown inv Fig. 3, and when the parts are folded as shown in said figure the gummed portion (Z3 of the flap d is sealed to the bottom portion g5 of the part g of the eX- tension It will be observed that the parts g2 and gi of the extension are narrower than the part g of said extension, and when the parts g2 and g3 are folded over the part g, as hereinbefore described, and said part g is folded over the body portion d the fold at f2 of the parts g2 and g3 does not come to the bottom edge of the envelop or to the fold at e of the parts g and (o. and this forms a narrow space g5,(shown in Figs. 3 and 4,) to which the strip Z3 of the flap Z is sealed. When the blank or sheet has -been folded into the form of an envelop in thev manner described and sealed, it is in a form of an ordinary envelop and may be sent through the mail in the usual manner. When it is desired to open this envelop, a cutter is inserted into the opening t' and is passed around the envelop, as indicated by the arrows fr, and along the dotted line d2, and the entire back portion of the envelop will be removed and the parts a, g, g2, and g3 will be freed therefrom and will constitute a bill-head or statement-sheet, as will be readily understood. 1t will also be seen that the bottom edges of the folding end portions c when the latter are folded as shown in Fig. 3 come slightly below the fold f2 of the parts g2 and g3 of the bill-head or statement-sheet, and the hole or opening z', through which the cutter is inserted in order to open the envelop, is above the fold f2, and when the cutter is inserted in the operation of opening the envelop, as hereinbefore described, it strikes on the back of the part g2 and does not enter between the folds of the bill-head or statement-sheet, and said bill-head or statement-sheet is not cut in the operation of passing the cutter around the envelop.
It will be apparent, of course, that the folding end portion c may be sealed together, if desired, when they are folded as shown in Fig. 3;but this is not absolutely necessary and increases the work of folding the sheet into the form of an envelop and sealing the same and also increases the cost of the combination bill-head or statement-sheet and envelop-blank.
By means of my improvement I provide an economical and convenient blank for the purpose specified, and in the use of this blank in the manner and for the purpose described the necessity of writing two addresses is avoided. In ordinary practice it is customary to employ any ordinary envelop and a separate bill-head or statement-sheet, and on each of these the address must be written; but with my improvement the necessity of writing two addresses is avoided, and in practice all that is necessary is to write the name of the customer or client on the part g above the words In account with and write his name and address on the part a, it being understood that the balance of the matter is printed on the parts L and g, as is also the directions lfor opening the envelop on the flap d.
Having fully described my invention, what I claim as new, and desire to secure by Letters Patent, is-

A combination bill-head or statementsheet and envelop-blank, comprising a body portion having folding end members narrower than the body portion, said body portion being also provided at one side with an extension having a supplemental folding member being of less width when folded than said eX- tension, and said body portion being also provided at the side opposite the extension with a folding flap the width of which is equal to the Width of the body portion, said iiap being provided near its free edge with a transverse perforated line forming a strip one side of which is gummed, substantially as shown and described. A combination bill-head or statementsheet and envelop-blank, comprising' a body portion having folding end members narrower than the body portion, said body portion being also provided at one side withan eXtension having a supplemental folding member of less width when folded than said extension, and said body portion being also provided at the side opposite the extension with a folding flap the width of which is equal to the width of the body portion, said'flap being provided near its free edge with a transverse perforated line forming a strip one side of which is gummed, and the body and that part of the extension adjacent thereto being provided one with the business-card of the user of the blank and the other with the address of the customer or client, substantially as shown and described.

A combination bill-head or statementsheet and envelop-blank, comprising a body portion having' folding end members narrower than the body portion, said body portion being also provided at one side with an eXtension having a supplemental folding memberv being of less width when folded than said eX- tension, and said body portion being also provided at the side opposite the extension with a folding flap the width of which is equal to the width of the body portion, said flap being provided near its free edge with a transverse perforated line forming a strip one side of which is gummed, one of the folding end members being also provided adjacent to the body portion and near the extension with a hole, substantially as shown and described.
5. A combination bill-head or statementsheet and envelop-blank, comprising a body portion having folding end members narrower than the body portion, said body portion being also provided at one side with an eXtension having a supplemental folding member of less width when folded than said extension, said body portion being also provided at the side opposite the extension with a foldingl flap the width of which is equal to the width of the body'portion, said iiap being provided near its free edge with a transverse perforated line forming a strip one side of which is gummed, the folding end members and the flap being separated from the body portion by folding lines, and one of the folding end members being also provided adjacent to the body portion and near the extension with a hole, substantially as shown and described.
6. A combination bill-head or statementsheet and envelop-blank, comprising a body portion having folding end members narrower than the body portion, said body portion being' also provided at one side with an eXtensionhaving a supplemental folding member of less width when folded than said extension, said body portion being also provided at the side opposite the extension with a folding iiap the width of which is equal to the width of the body portion, said iiap being provided near its free edge with a transverse perforated line forming a strip one side of which is gummed, the folding end members and the iiap being separated from the body portion by folding lines, and one of the folding end members being also provided adjacent to the body portion and near the extension with a hole, and the body portion and that part of the extension adjacent thereto being provided one with a space for the business-card of the user of the blank and the other with Y a space for the address of the customer or client, and the flap being provided on one side with directions for opening the envelop, substantially as shown and described.
