To all whom it may concern:
Be it known that'I, ROBERT J. FENNELLI, a citizen of the United States, residing at Bronx, in the county of New York and State of New York, have invented certain new and useful Improvements in Dentists Record-Holders; and I do declare the following to be a full, clear, and exact description of the invention, such as will enable others skilled in the art to which it appertains to make and use the same.
This invention relates to dentists record holders, and it relates more particularly to a combined radiograph holder and verbal record blank.
One object of this invention is to provide a convenient and practical means whereby the radiographic record of a toothor teeth is always adjacent to the verbal record or written record relating to the tooth or teeth.
Another object is to provide a dentists record sheet which is constructed to hold the radiograph and written record in a manneg to protect both of these forms of recor Another object is to provide, a record holder of this character in which the radiograph is visible while in its normal protected position in engagement with the protectihg sheet.
Another object is toprovide a radiograph holder or photograph holder which permits inspection of both sides of the radiograph or photograph while the latter is held by the holder. and which permits free access of light through the radiograph or photograph from either side while thus held.
Other objects and advantages may become apparent to persons who read the following deta ls of description in connect on with the accompanying drawings. in which:

which similar reference characters corre I spond with similar parts throughout the several views, this record holder comprises an opaque sheet 7 a second opaque sheet 8 and a transparent sheet 9, the radiograph being indicated by the numeral 10.
The sheet 7 is provided with an opening or Window 11 which is covered by the transparent sheet 9 and the latter is provided with W-shaped slits 12 which receive the corners, respectively, of the radiograph film 10, so as to holdthis radiograph film securely while protecting the major portion of one side thereof by means of the middle portion of the transparent sheet. In order that the transparent sheet may be securely and permanently held over the opening 11, without interfering with placing the films in engagement with the transparent sheet, I providethe opaque sheet 8 with an opening 13 which registerswiththe opening ll of the opaque sheet 7 and I secure the edge portions of the opaque sheet 8 to the sheet 7 by any approprlate means, such means being preferably employed also for. securing the edge portions of the transparent sheet between sheets 7 and 8.- seen; therefore. that when the film'or radiograph is held by the transparent plate. the same may be observed or inspected through the transparent sheet; but if a more careful and thorough inspection is desired, the

and these inner surfaces are brought into contact with one another, the otherwise exposed-surface of the film is now protected by the inner surface of the printed and written record, while the latter is protected by the inner surfaces of the film and sheet 8. I

Although 1 have described this embodiment of my invention very specifically, it is to be understood that,this invention is not limited to these exact details, but I am entitled to make minor changes without departing from the scope of the'inventive ideadisclosed in the foregoing description and following claims. 4 WVhat I claim as myvinvention is: I
1. A record holder comprising a sheet having a window therethrough and having 'a fold-line which provides a pr ctingflap,
- a transparent sheethaving its edges secured to the first said sheet at the margin of said window and having its middle \part closing said window, and a picture'on saidtransparent sheet and having one side protected thereby and having its other side normally protected by the said flap, the latter being. normally turned on the fold-line so as to lie over and protect the other side of said picture, said picture being visible from either side of said window when the sheet is unfolded. 2.-A record holder I comprising a sheet having a window therethrough and having thereonva verbal record of a person and of an object relating to that person, said sheet having a fold-line between said window and verbal -record to provide a protecting flap,
said protecting flap containing said verbal protecting flap, the latter being normally turned on said fold-line so as to lie over and protect the other side of said picture and also protect the verbal record, said picture being visible from either side of said win dow when the sheet is unfolded. I

A record holder comprising an opaque sheet having an-opening therethrough and having a plurality of blank record spaces and words-indicating the subjects to be written in said record spaces, said sheet also having a fold-line between said opening and record spaces a transpareht sheet closing .said opening and provided with means to secure thereto the corners of a transparent sheet containing a photographyand a third sheet, the latter having an opening registering with'the'first said opening, said third sheet having its edges secured to the first said sheet and cooperating with the latterto hold said transparent sheet;
