A useful model belongs to medicine, namely to gastroenterology. It can be used in the treatment of 2 gallstone disease.
Gallstone disease occupies one of the leading places among the most common surgical diseases in the world and has acquired a steady growth trend. Cholecystectomy became the most frequent operation after appendectomy. For many decades, since the first cholecystectomy in 1882, the only radical method of treating cholecystolithiasis was surgery. New opportunities for the treatment of 70 cholecystolithiasis were opened in the early 80s, when devices for remote crushing of stones in the patient's body appeared in clinical practice and a new method of treating gallstone disease - extracorporeal shock wave lithotripsy - appeared. Crushing gallstones has become quite widespread in clinical practice.
- С.20-23)However, clinicians increasingly face the problem of optimizing the process of fragmenting calculi, reducing the number of recurrences of gallstones after lithotripsy, and finding new methods to improve the elimination of sand from the gallbladder after the stone crushing procedure. As you know, the process of eliminating sand and small fragments of stones is most often associated with a pain syndrome. Small stones remain in the bladder, some of them, falling into the common bile duct (choledochus), cause pain and mechanical jaundice. The occurrence of pain and mechanical jaundice can be caused by the following factors: mechanical irritation by fragments of destroyed stones of the gallbladder wall or ducts, overstretching of the gallbladder wall due to increased intracavitary pressure, spasm of the muscles of the gallbladder and bile ducts, spasm of the sphincter of Oddi. (Ilchenko A.A. Dysfunctional disorders of the biliary tract. Sopsiiysht tedisit (Appendix. Gastroenterology) 2001. - Mo1. - P.20-23)
Contraction of the smooth muscles of the gastrointestinal tract occurs when muscarinic receptors on the surface of the muscle cell are stimulated by acetylcholine. This leads to the opening of sodium channels and entry of Mat into the cell. The resulting depolarization of the cell, in turn, promotes the opening of calcium channels and the entry of Sazh2 into the cell.
The intracellular level of Sazh2, which increases, contributes to the phosphorylation of myosin and, accordingly, to the contraction of the muscle and the occurrence of muscle spasm, which ultimately causes the appearance of pain. In turn, symptomatic impulses contribute to the release of DO from the cell and San2 from the calcium depot, closing of calcium C channels and relaxation of the muscle. A number of neurogenic mediators are involved in the regulation of San2 ion transport: Ge»! acetylcholine, catecholamines (norepinephrine), serotonin, cholecystokinin, motilin and others.
Thus, taking into account the fact that in the formation of pain and mechanical jaundice after lithotripsy of stones - gall bladder lies excessive contraction of the smooth muscles of the wall of the gall bladder, bile ducts and sphincter of Oddi, the necessity of using antispasmodics becomes obvious. 3o Preparations for spasm relief and gastrointestinal pain relief remain traditional and modern myotropic antispasmodics, which by their mechanism of action are divided into antispasmodics of direct action and selective antispasmodics. Spasmolytics of direct action of drotaverine hydrochloride (no-shpa and no-shpa forte, papaverine, eufilin) do not have selectivity of action, because "they affect all tissues that have smooth muscles." shsh

From" prototype. No-shpa (drotaverine) is a stronger antispasmodic than, for example, papaverine. However, like the latter, it can cause dizziness, decrease in myocardial excitability, violation of intraventricular conduction, up to the development of anrioventricular blockade, contraindicated in glaucoma and prostatic hyperplasia. The main drawback of this pharmacological agent is that while reducing pain, it simultaneously causes relaxation of the sphincter of Oddi, which reduces the motor function of the biliary tract and, as a result, makes it difficult to evacuate small fragments in general, leading to mechanical jaundice.
A useful model solves the problem of choosing a tool that provides an antispasmodic effect and at the same time normalizes the tone of the sphincter of Oddi, that is, without reducing the motor function of the biliary tract, which
Ta" 20 makes it optimal when used after lithotripsy to prevent mechanical jaundice and relieve pain syndrome.
The task is solved by the fact that during the week before the lithotripsy session and immediately after the session, patients are prescribed duspatolin (mebeverine) in a therapeutic dose, continuing its use for 3 weeks at the rate of 1 capsule 3 times a day 20 minutes before meals.
SS o5 The novelty of the method consists in the use of the selective antispastic drug duspatolin (mebeverin, "Boimau Rpag-ta"), which has a direct blocking effect on the fast sodium channels of the cell membrane of the myocyte, which, as a final result, prevents the sequence of events that lead to m' ulcer spasm, and therefore the development of pain syndrome and mechanical jaundice. The choice of this drug is due to the fact that one of the main advantages of duspatolin, unlike other antispasmodic drugs, including the 60 used method-prototype of the declared useful model, is not only spasm relief, but also the absence of smooth muscle hypotonia.
The method is carried out as follows.
A patient with a preliminary diagnosis of "calculous cholecystitis" is being examined to resolve the issue of indications and contraindications and the possibility of using extracorporeal or shock wave lithotripsy (EUHL) as a treatment. Perform ultrasound of the liver, bile ducts, pancreas and kidneys.
They establish the presence of a stone, the presence and/or absence of deformation of the biliary tract and inflammation (thickening of the walls). The function of the biliary tract is determined, for which the patient is prescribed a choleretic breakfast followed by the determination of the contractile function of the bladder, a biochemical blood test and a coagulogram are performed. If there is a solitary stone no larger than 1.5 cm, a normally functioning gall bladder, the absence of hematomas and cysts in the projection of the shock wave passage, normal biochemical blood parameters - a decision is made to perform EUHL for this patient and start his preparation. One month before lithotripsy, the patient is prescribed Ursofalk in a dose of 7-12 mg per kg of the patient's weight, and 5-7 days before the lithotripsy session, duspatalin (mebeverine) is additionally prescribed in a therapeutic dose (1-2 capsules). After performing the 1st session of lithotripsy 70, duspatolin is given directly to the patient in a dose of 3 capsules per day. The next day after lithotripsy, a control ultrasound is performed. If the stone underwent partial fragmentation, repeated sessions are prescribed until the stone is completely destroyed, while during the entire treatment, including during 15-21 days after the last session, the patient continues to receive duspatalin in a therapeutic dose.

Thus, it can be concluded that against the background of the use of the claimed method in patients with EUHL, there is a positive trend in pain reduction, as well as the normalization of both laboratory and ultrasound parameters. The obtained results allow us to recommend duspatalin to patients with various types of sphincter of Oddi dysfunction, both as monotherapy and in combination with other drugs. If necessary, it can be combined with antibacterial, enzyme preparations, as well as with prokinetics.
