Abstract

The technique for the reconstructive jejunogastroplasty in acutely bleeding gastric cancer following gastrectomy according to Roux with fashioning intestinal reservoir and duodenal switch provides for cutting the loop of the jejunum. The efferent loop of jejunum 28- in length is cut proximal to Roux enteroanastomosis at the distance of and mobilized on the vascular mesenteric arcade. The tailored segment is transferred proximally retrocolic. The proximal part of the tailored segment is folded in U-shape as the double loop and sutured along 10- with the linear stapler "Proximate" . The sutured double loop is sutured to the distal end of the efferent loop of jejunum distal to esophagojejunoanastomosis along 10- with the linear stapler "Proximate" fashioning the triple loop and the artificial intestinal reservoir (jejunogastroplasty). The distal end of the segment tailored from jejunum is anastomosed with the stump of duodenum with the aid of the circular stapler "Ethicon" generating jejunoduodenoanastomosis. The efferent loop of the jejunum is plugged proximal to Roux enteroanastomosis. The continuity of jejunum is restored with end-to-end Roux enteroanastomosis.
