Abstract

The technique for pylorus-sparing pancreatoduodenal resection provides for crossing the duodenum below the pyloric sphincter, followed by the creation of intestinal reservoir by suturing the triple loop of jejunum on the mesenteric peduncle with linear stapler "Proximat-55" and fashioning subpyloric duodenojejunoanastomosis with the proximal end of the jejunum segment, choledochojejunoanastomosis with the jejunum segment distal to subpyloric duodenojejunoanastomosis and fashioning invagination sleeve-shaped pancreatojejunoanastomosis by anastomosing the pancreatic stump with the tailored intestinal reservoir by two-row suture. Then transjejunal guided externally drainage of choledoch and the major pancreatic duct follows. The distal jejunoanastomosis is provided between the distal end of the jejunum segment and inferior horizontal part of duodenum. The continuity of jejunum is stored with end-to-end entero-enteroanastomosis and jejunostoma is applied onto the efferent jejuna loop distal to entero-enteroanastomosis.
