Description of the invention
The invention relates to medicine, namely to otolaryngology and can be used in the treatment of 2 chronic tonsillitis and its complications.
Currently, in clinical otolaryngology, to a greater or lesser degree, almost all known physical factors are used, which, according to the modern classification, includes different groups of artificially observed and natural physical factors used for the treatment of chronic diseases of the ENT organs. Preference is given to such physiotherapeutic factors as direct currents, medicinal electrophoresis, exposure to 710 ultrasound, etc.
Тому між випромінювачем і шкірою 12 приміщають контактне середовище (вазелінова олія, гліцерин).There is a known method of treating chronic tonsillitis by affecting the tonsils with ultrasound. The influence of ultrasound is carried out through the soft tissues of the neck, through close contact of the radiating surface of the applicator with the sounding tissue, because an air layer of 0.001 mm almost completely prevents the energy of ultrasonic oscillations of tissues (in the accepted frequency range) from reaching the tissues. Therefore, a contact medium (vaseline oil, glycerin) is placed between the emitter and the skin 12.
оThe contact medium is applied directly to the working surface of the applicator and tightly pressed to the place that is sounded. In clinical otolaryngology, a stable (immobile) sounding technique is used - the emitter in the form of an applicator is fixed in one position. The intensity of the effect is 0.2-0.4w/cm"7, the duration of the procedure is 5 minutes for each tonsil, the course of treatment is 10-15 procedures performed daily. 720 When affecting the area of the tonsils, the applicator is fixed directly under the angle of the lower jaw. When choosing the position of fixation of the applicator, it is necessary to find such a position of the patient's head in which the sterno-mastoid muscle should be relaxed, especially with its hypertrophy, and this should be done on each side separately. At the same time, it should be strictly ensured that the contact surface of the applicator does not touch the bone the protrusion of the lower jaw 11. The disadvantage of this method is its low efficiency, especially in the case of significant fat deposits in the region of the neck 25, as well as the thickness of the muscle layer and five fascial membranes, which are layer by layer in the tissues of the neck.
(1, 21.The closest to the method of treatment of chronic tonsillitis that is claimed, and taken as the closest analogue, is a method based on the use of medicinal electrophoresis - a special electropharmacological method, which is based on the complex effect of direct current on the diseased organ and the introduction of medicinal substances with its help. During medicinal electrophoresis, the ions of medicinal substances enter the patient's organ in an electrically active state and interact with the tissues, the properties of which have been changed by the galvanic electrophoresis current, while the ions of the medicinal substances penetrating the tissues form an ion depot, from which gradually within 1-20 days are absorbed into the blood and lymph with the possibility of creating the necessary concentration of the drug in the area of the diseased organ. p 35 The method is as follows. Electrodes consisting of a hydrophilic pad and a conductive plate, which is the anode, are placed on the lateral surfaces of the neck from the submaxillary fossa to the lower third of the sterno-mastoid muscle. Hydrophilic pads are moistened with 595 solution of freshly prepared novocaine or other medicinal substance. The second electrode, which is a cathode, made in the form of a lead plate, is placed in the region of the lower cervical vertebrae. Anode and cathode are connected to interchangeable poles of a source of direct current of the Potik-1 electroplating device type. When conducting medical electrophoresis, the power of З7З s current is set equal to 3--bmA, the duration of the procedure is 10--20 minutes. for a course of 10--15 procedures (1, 21.
The disadvantage of the method of the closest analogue, as well as the method of treatment of chronic tonsillitis using ultrasound, is caused by a thick muscular fascial layer, which significantly weakens the penetration of medicinal substances to the diseased organ, that is, the tonsils.
The basis of the proposed method is the task of improving the method of treatment of chronic tonsillitis and obtaining an optimal therapeutic effect on the tonsils through their direct contact with the electrodes and the introduction of medicinal substances into them in order to obtain the maximum clinical effect in the treatment of chronic tonsillitis. se | The set task is achieved by the fact that the method of treatment of chronic tonsillitis, which involves the effect of 5p medicinal substance on the tonsils with a sequential effect on them by direct current with the help of electrodes, g" additionally provides, before the treatment, application anesthesia of the oropharyngeal mucosa with 290 g of demedrol solution, then in the oropharynx electrodes placed on the tonsils are inserted into the patient, after which the tonsils are affected with a medicinal substance and electric current. At the same time, peloidin (an extract of medicinal mud) is used as a medicinal substance. During an electrophoresis session, the current strength is

There are well-known devices for conducting medical electrophoresis, which use electrodes that (F) consist of a hydrophilic pad and a conductive plate. The latter are connected to different poles
Ф source of direct current, type of apparatus for conducting electrophoresis. During electrophoresis, the medicinal substance is applied directly to the hydrophilic pad (1). However, these devices do not allow to fully optimize the mode of influence of medicinal substances on the diseased organ.
As the device of the closest analogue, a device for conducting medical electrophoresis of the tonsils was chosen, consisting of two anodic contact electrodes of a rounded shape, connected by a bifurcated wire to one pole (anode), and a third electrode, made in the form of a plate, connected to the other pole (cathode) of a direct current source (21. 65 The disadvantage of this device is the shape and design of the anode contact electrodes, which does not allow them to be used for direct contact with the tonsils in the oropharynx region.
The basis of the invention is the task of improving the form and design of the anode contact electrodes used in the device for the treatment of chronic tonsillitis.
The task is achieved by the fact that in the device for the treatment of chronic tonsillitis, which contains a connecting element with anode contact electrodes fixed at its ends, a cathode contact plate and current conductors for their connection to a direct current source, according to the invention, the connecting element is made of an O-shape, formed by at least one stretching turn, and the anode contact electrodes are spherical chambers, the concave surfaces of which are perforated, and each chamber is equipped with pipelines for the introduction of medicinal substances into the chambers, while the ends of the connecting 7/0 element are insulated with a dielectric material. Devices for conducting electrophoresis such as "Potik-1" and others are used as a source of direct current.
Пропонований спосіб лікування хронічного тонзіліту відрізняється від найближчого аналога тим, що лікарські речовини вводяться безпосередньо в хворий орган, тобто його уражені хронічним запальним процесом тканини, шляхом безпосереднього контакту сфероподібних камер, заповнених лікарською речовиною, із хворим /5 органом (мигдалинами).The proposed method of treating chronic tonsillitis differs from the closest analogue in that medicinal substances are injected directly into the diseased organ, i.e. tissue affected by a chronic inflammatory process, through direct contact of spherical chambers filled with a medicinal substance with the diseased organ (tonsils).
The novelty and inventive level of the proposed technical solution lies in the fact that in the device for the treatment of chronic tonsillitis, the anode contact electrodes are made in the form of spherical chambers with concave perforated surfaces, equipped with pipelines for the introduction of medicinal substances into the chambers, while the fixation of the anode contact electrodes is performed directly on the patients organs (tonsils) inside the mouth using a connecting element made of an elastic wire covered with a dielectric (insulating material). This makes it possible to create the maximum concentration of medicinal substances directly in the organ affected by the disease and to create a regulated depot of the medicinal drug in the cells of this organ by introducing medicinal substances by electrophoresis.
Зазначені елементи пропонованого способу і пристрою для його здійснення визначають новизну і суттєві сч об ознаки передбачуваного винаходу, що у сукупності з відомими забезпечують одержання нового позитивного ефекту, що полягає в тому, що завдяки створенню оптимальних для усмоктування лікарських речовин умов у о зоні хворого органа підвищується ефективність лікувального процесу, за рахунок створення максимальної кількості лікарських речовин в одиниці обсягу хворого органа (клітини).The specified elements of the proposed method and the device for its implementation determine the novelty and essential features of the proposed invention, which, in combination with the known ones, provide a new positive effect, which consists in the fact that thanks to the creation of optimal conditions for the absorption of medicinal substances in the area of the diseased organ, the the effectiveness of the treatment process, due to the creation of the maximum amount of medicinal substances per unit volume of the diseased organ (cell).
