Abstract
Description
Description of the invention
The invention relates to medicine, namely to surgery, and can be used in physiotherapy of the distal 2nd part of the colon in the treatment of an inert rectum.
A well-known method of physiotherapeutic treatment of an inert rectum is direct electrical stimulation (endorectal) with the help of electrodes implanted in the wall of the intestine from the side of its opening, and performing electrical stimulation of the extremities of the parasympathetic nerves with rectangular bipolar pulses with a frequency of 5ΩHz. (1, 21. 70) The method has inherent disadvantages: invasiveness, difficulty of execution, lack of long-term effect, impossibility of assessment and correction of barosensitivity and kinetic changes on the part of the rectum before, during, and after stimulation, which reduces its effectiveness.
The closest analogue of the method chosen as a prototype is endorectal direct electrical stimulation of the extremities of the parasympathetic nerves of the rectum (21). According to the method, electrostimulation is carried out using 19 electrodes implanted in the wall of the intestine from the side of its opening, and electrostimulation of the extremities of parasympathetic nerves is performed with rectangular bipolar electric pulses with a frequency of 50 Hz.
The method has inherent disadvantages: invasiveness, complexity of implementation, lack of long-term effect, impossibility of assessment and correction of barosensitivity and kinetic changes on the part of the rectum before, during, and after stimulation, which reduces its effectiveness.
The task of the claimed invention is to create a non-invasive method of treating an inert rectum, which, due to stimulation of the nerve endings of its walls by periodically changing the pressure in a balloon inserted endorectally, would be able to provide the possibility of assessing and controlling barosensitivity and kinetic changes on the part of the rectum to , during and after stimulation, increased its effectiveness and provided a stable, long-lasting treatment effect.
The task is achieved by the fact that in the known method, which includes stimulation of the nerve endings of the rectum, according to the invention, stimulation is carried out at the level of the primary urge to defecate by periodically changing the pressure in a balloon inserted into the rectum (pneumostimulation), which is inflated with a frequency of 3 -6 oscillations per minute for 30-40 minutes followed by a control assessment of its kinetic properties.
--The essence of the method is explained graphically in Fig.1. where 71 is a rubber probe with a final balloon with a capacity of 10 Oml, made of elastic thin latex rubber; 2 - a cylinder with a capacity of 5 ml., located at 2 cm. proximal to the FU of the first; C - a syringe for injecting air into the balloon; 4 - analyzer of motor activity of intracavitary organs "Jaguar-2"; 5 - a personal computer with appropriate software. --
The method is performed as follows. Gee)
After cleaning the ampoule of the rectum from the contents, a rubber probe with balloons is inserted into the rectum of the patient lying on his side in such a way that the final balloon with a capacity of 1OOml. (1) was located in the ampoule of the rectum, and the balloon ee, with a capacity of bml. (2) in the anal canal at the level of the internal sphincter. Both cylinders are connected each to a separate channel of the analyzer of motor activity of intracavitary organs "Jaguar-2" (4), which is connected to a personal computer (5) with the appropriate software. In a cylinder with a capacity of 100 ml. (1), "equipped with an additional channel, with the help of a syringe (3) inject air until the pressure in the cylinder is equal to 10 mm Hg. This pressure is sufficient to balance the tonic pressure of the intestinal wall without irritating it. The pressure in the cylinder is registered on the computer monitor in the form of a graph

After 10 minutes (the period of adaptation of the intestine to the presence of a balloon) in a balloon with a capacity of 100 ml. (1) successively inject 10 ml. air to the level of feeling the primary urge to defecate. At this level, the pressure in the rectum is determined, which is displayed on the pneumokinesogram and characterizes the barosensitivity threshold of the rectum, and the total volume b of the injected air. These indicators reflect the degree of inertness of the rectum. The pressure of the primary claim
Ge | is considered the basic level on which pneumostimulation will be performed (on average 60-8 Ohm.Hg). Next, the initial motor activity of the distal part of the colon, which is usually weakened in patients with an inert rectum, is recorded. The recording is made within 20 minutes. After that, a cylinder with a capacity of 10 Oml. (1) (Te) 20 is connected to the pneumostimulation mode, which is carried out by successively changing the pressure in the cylinder from 40 to 7Omm.rt.st. with a frequency of 3-6 times per minute, which corresponds to the frequency of physiological contractions of the distal part of the large intestine. Pneumostimulation is carried out for 30-40 minutes, during which, with the help of a balloon with a capacity of 5 ml. (2), control the kinetic changes of the rectum. After completion of pneumostimulation, a control recording and evaluation of the pneumokinesogram are also carried out. 29 Stretching of the intestine stimulates through the reflex arc parasympathetic nerves 5", Зз, З, which innervate the left v. half of the colon and rectum. The hypertonicity of the parasympathetic nervous system of this section of the colon and rectum contributes to increasing its contractile activity and restoring normal motility and barosensitivity. Pneumostimulation sessions are performed daily until normal motility and barosensitivity are restored, which is usually observed after 5-6 sessions. 60 An example of a specific implementation.
Patient M., 48 years old, medical history Mo2052, was hospitalized in the proctology department of the clinic on 24.12.2002 with a diagnosis of inert rectum. The patient underwent 7 daily sessions of pneumostimulation. After the 2nd session, the patient noticed an independent stool. At the end of the pneumostimulation course, a control recording and assessment of the pneumokinesogram was performed, which revealed an increase in the contractile activity of the rectum and the restoration of normal motility and barosensitivity. Such courses were held monthly for 4 months. After the last course, the patient did not undergo pneumostimulation for 2 months. After that, the patient underwent a control recording and evaluation of the pneumokinesogram, which revealed a significant improvement in the contractile activity of the rectum, its motility and barosensitivity, which is confirmed by the improvement of the clinical picture, the presence of an independent stool.
The proposed method of treating an inert rectum was used in 45 patients and showed the possibility of providing a stable, long-lasting treatment effect with the help of a simple, non-invasive method.
A fundamental distinguishing feature of the invention in comparison with the prototype is that the stimulation of the nerve endings of the rectal wall is not carried out by electric current, but by periodically changing the pressure in the balloon, /o introduced endorectally, with the possibility of assessing and controlling barosensitivity and kinetic changes on the part of the rectum to , during and after stimulation, which leads to the creation of a conditioned defecation reflex instead of a lost or partially lost unconditioned one.
