Description of the invention
The invention relates to medicine, namely to urology, and can be used for metaphylaxis of 2 oxalate urolithiasis.
Treatment of recurrent urolithiasis is one of the most difficult problems in urology. The effectiveness of modern approaches to the treatment of oxalate urolithiasis remains low due to the high percentage of recurrent stone formation.
60-85,690.Calcium oxalate is almost insoluble, and postoperative relapses occur in 2095 (11. 70 Oxalate urolithiasis is the most common and is among other types of stone formation, both primary and recurrent, 60-85,690.
Magnesium cation and vitamin Bb have an inhibitory effect on the formation of calcium oxalate in urine. Magnesium also reduces the absorption of oxalates from the intestine.
The closest to the proposed technical solution is the method of metaphylaxis of oxalate urolithiasis with the use of magnesium oxide and vitamin Вб ИЗ). Magnesium oxide was prescribed in doses of 0.2; 0.3 and 0.4 g three times a day, vitamin Bb - 0.002 and 0.02 g Oral orally and in a dose of 590 ml of solution every other day intramuscularly, a total of 15 injections. The average period between examinations was 1.2-0.4 months.
It has been established that magnesium oxide and vitamin Bb are effective for suppressing oxalate hyperexcretion, and magnesium oxide is effective for eliminating oxalate crystalluria.
However, this method has the following disadvantages: vitamin Bb should be administered intramuscularly; in addition, the dosage of these drugs is quite difficult.
The invention is based on the task of improving the method of metaphylaxis of oxalate urolithiasis by using the antioxidant drug o-tocopherol (vitamin E), which reduces hyperoxaluria and, as a result, prevents the formation of recurrent uroliths. 29 The problem is solved by the fact that, according to the invention, o-tocopherol (vitamin E) is used at 200 mg 4 times a day for 7-10 days, reducing the dosage for the next 7-10 days - 20 mg 3 times a day.
The method is carried out as follows.
The indicator for prescribing a stone formation inhibitor was increased excretion of oxalic acid, or oxalate crystalluria. The choice of the latter was due to the fact that the drug (vitamin E) increases the activity of F 3o enzymes: o-glycerate dehydrogenase and alanine-glyoxylate-aninotransferase, which leads to a decrease in (o) hyperoxaluria in patients with recurrent oxalate-oxylate urolithiasis. 30 patients with primary and recurrent oxalate-oxalate nephrolithiasis were examined. The age of the patients ranged from 19 to 68 years (13 men and 7 women). Pyelolithotomy was performed in 10 patients, ureterolithotomy - b, EUHLT session - 3, and spontaneous exit - 1. (ав) from Nephrostomy was performed during surgery in 12 cases. Kidney drainage using stents was performed in 3 cases. The operated patients were divided into two groups according to composition, age, methods of operative treatment and other features of the disease. Patients of the first group (18 people) received traditional modern therapy in the early and late periods. The complex therapy of the second group in the early and late postoperative periods included o-tocopherol in a dose of 200 mg x 4 times a day for 7-10 days and 20,200 mg x 3 times a day in the following 7-10 days. During 1 year, the course of treatment was repeated three times. Subjective improvement in well-being was noted in patients with c. Long-term intake of o-tocopherol did not lead to undesirable effects. A comparative assessment of some indicators of the metabolic statute before and after taking o-tocopherol made it possible to establish that the use of this drug destroys hyperoxaluria and the formation of oxalate crystals, reduces the activity of lipid peroxidation, and reduces oxalemia.
Через рік у хворих першої групи рецидиви каменеутворення виникли у 2395 (7 чоловік), а у пацієнтів другої б групи лише у 1195 хворих (З спостереження).A year later, stone formation relapses occurred in 2395 (7 people) patients of the first group, and in only 1195 patients of the second group (from observation).
Thus, the analysis of the obtained data allows us to conclude that the drug A-tocopherol is a more effective way of correcting the crystalloid state of urine, which in complex therapy allows to significantly reduce the number of "sl of recurrent nephrolithiasis, and thus, to increase the effectiveness of treatment of patients with oxalate-oxalate 5or Urolithiasis. An example of a specific implementation of the method. Patient K., 46 years old. He went to the clinic with complaints of pain in (Che) lower back on the left. He has been suffering from urolithiasis for 20 years, and underwent three surgeries for kidney stones in the last 5 years. The examination according to the X-ray examination revealed the presence of a recurrent 1.5 cm long left kidney stone. After stenting the patient's left urinary tract, the stone was crushed with the help of extracorporeal shock wave lithotripsy with the help of EUHLT. In the postoperative period, the patient was prescribed the drug o-tocopherol according to the proposed scheme against the background of traditional metaphylactic therapy. Over the course of a year, the patient was examined three times for the recurrent formation of calculi. In the past, the period of recurrent stone formation in this patient was, as a rule, 3-6 months. During the treatment by the proposed method, no recurrence of stone formation was observed over the course of a year, thanks to the inclusion of the drug urolithiasis inhibitor - d-tocopherol in the therapy.
Thus, preventing the formation of recurrent stones, there was an opportunity to reduce the disability and maladjustment of the socially active population while reducing the number of surgical interventions.
Compared to the prototype, the proposed method allows to treat hyperoxaluria and oxalate crystalluria with a more stable effect, to prevent the subsequent formation of recurrent kidney stones, which eliminates 65 unwanted numerous interventions, including organ harvesting, improves mental status, including sleep quality and the disappearance of embezzlement.
