Abstract
Description
A useful model refers to medicine, namely narcology, and can be used in the treatment of patients with opiate addiction.
There is a known method of treatment of withdrawal syndrome in heroin addiction (Russian Patent 2191019, publ. 20.10.02, Ab1K31/662, Ab1R25/30| with the help of medicinal preparations, which include infusion agents, angioprotectors, tranquilizers, analgesics and additionally use demiphosphon.
The disadvantages of this method are the need for long-term treatment and the low efficiency of the method.
The method of treatment of patients with opium addiction is closest to the stated one (Russian Patent 2147879, publ. 04/27/2000, Ab1K31/485, Ab1KU/22, Ab1R25/36 | by means of a stepwise effect on the patient, while at the first stage under general anesthesia in the background of artificial lung ventilation, detoxification is carried out using naloxone and naltrexone for 6 hours, then naltrexone-corticosteroid is implanted subcutaneously in the form of tablets for a period of 2-3 months.
The disadvantages of the method are its low efficiency, which is associated with a short duration of action, the need for long-term treatment, and a high degree of physical suffering of patients. A subcutaneously implanted capsule is often covered with connective tissue, which prevents the absorption of drugs into the blood. As a result, patients begin to feel withdrawal symptoms after 2-3 days and often return to drug use.
It is known that the use of opiate drugs causes a number of biochemical and physiological changes in the brain.
Накопичуються у великих кількостях у пресинапсах головного мозку такі нейротрансміттери, як норадреналін і допамін, унаслідок чого виявляються клінічні ознаки абстиненції (при відсутності опіатів).The number of opiate receptors increases and their sensitivity increases. Neurotransmitters such as norepinephrine and dopamine accumulate in large quantities in the presynapses of the brain, resulting in clinical signs of withdrawal (in the absence of opiates).
The basis of a useful model is the task of improving the method of treating opiate addiction by drugging the patient and blocking opiate receptors by staged premedication, general anesthesia, and detoxification, which will significantly shorten the duration of treatment, reduce the patient's physical suffering from withdrawal syndrome, and eliminate the pathological craving for drugs.
The task is solved by the fact that in the method of treatment of opium addiction, which includes a step-by-step drug effect on the patient, according to a useful model, the patient is premedicated by introducing intramuscularly for 4 hours a solution of catapressan at a dose of 50-150 μg/kg of body weight, Dormicum at a dose 2.5-10mg/kg of body weight, vitamin C 1-2g and 1 tablet of immodium, then general anesthesia is carried out by intravenous administration of propofol in a dose of 100-200Omg and dormicum in a dose of 10-15mg, then detoxification of the body is carried out intravenously every hour for 4 hours famotedin at a dose of 0.3-1mg/kg in combination with zofran at a dose of 0.05-5mg/kg, and intragastric naltrexone at a dose of 0.05-1.5mg/kg in combination with catapressan at a dose of 0.1-5mg/ kg, at the same time the patient received a peptide solution subcutaneously three times, after the last dosage of the drug, a naloxone test was performed.
The use of this complex of medicines provides an accelerated release of brain presynapses from neurotransmitters and thus reduces the abstinence period to 3-4 hours instead of 2-3 weeks.
The dosage of the complex of medicines is chosen depending on the age and gender of the patient, as well as on the duration of drug use.
Opiate drugs are strong blockers of AMA2 antagonist receptors. During detoxification, when the patient receives antagonists, a large amount of acetylcholine and serotonin is released in the intestine, in addition, there is an MIR-peptide in the intestine, which regulates the release of a large amount of liquid in the intestine. The use of sandostatin as an artificial peptide allows you to block the release of the M.I.R.-peptide. In this way, the use of sandostatin and catapressan reduces gastrointestinal side effects in patients undergoing treatment for opiate addiction.
Practically, the method is carried out as follows
At the first stage, after a medical examination of the patient and clarification of the possibility of carrying out this method of treatment, the patient is given premedication. Within 4 hours, a solution of catapressan in a dose of 50-150mcg/kg of body weight, Dormicum in a dose of 2.5-10mg/kg of body weight, vitamin C in the amount of 1-2g and one tablet of immodium are administered intramuscularly. Next, the patient is transferred to the intensive care unit, connected to pulse, pressure and oxygen saturation monitors. General anesthesia is performed with intravenous administration of propofol in a dose of 100-200 mg and Dormicum in a dose of 10-15 mg. After losing consciousness, the patient is intubated, a stomach probe and a catheter are inserted into the bladder. Anesthesia is maintained with the help of a dropper with a solution of propofol and dormicum intravenously. Next, the body is detoxified by injecting famotedin intravenously every hour for 4 hours at a dose of 0.3-1Omg/kg in combination with zofran at a dose of 0.05-5mg/kg. Naltrexone is administered intragastrically at a dose of 0.05-1.5 mg/kg in combination with catapressan at a dose of 0.1-5 mg/kg. At the same time, the patient received three times subcutaneously a solution of peptide, which is used as sandostatin. After the last dose of medication, a naloxone test is performed. For this, 0.4-0.8 mg of naloxone is administered intravenously and possible changes in breathing, pulse, pressure, and skin condition are recorded. In the absence of the above-mentioned withdrawal symptoms, general anesthesia is stopped and the patient is extubated.

Patient K., 23 years old, diagnosis: opium addiction, history of use - 1 g of heroin per day intravenously for the past 5 years. She received premedication, during which she was given 5 mg of Dormicum, 75 μg of catapressan, 1 g of vitamin C, and 1 tablet of immodium. After three sessions of premedication with an interval of 1 hour, she was transferred to the intensive care unit, where she was given a dropper with 0.995 physiological solution, connected to a heart monitor, pressure and oxygen saturation. Next, the patient underwent general anesthesia with intravenous administration of 10 mg of Dormicum and 100 mg of propofol. After local anesthesia of the vocal cords with 290 ezracoine solution, intubation was performed and after a few seconds she began to breathe spontaneously. At the same time, a probe was inserted into the stomach and a catheter into the bladder. To maintain anesthesia, the patient constantly received propofol intravenously at the rate of bmg/kg of body weight per hour. After stabilization of the condition, 25 mg of naltrexone and 75 mg of catapressan were administered through a gastric tube. At the same time, 0.05 mg of sandostatin was administered subcutaneously, and 40 mg of famotidine and 4 mg of zofran were administered intravenously. The whole procedure was repeated three times in the same dosage.
An hour after the last dose, a naloxone test was performed by intravenous administration of 0.4 mg of naloxone. Changes in breathing, pulse, pressure, and the condition of the skin were monitored. In the absence of the above-mentioned withdrawal symptoms, propofol administration is stopped. After 25 minutes, the patient woke up and was extubated.

During premedication, the patient received 10 mg of dormicum, 150 μg of catapressan, 2 g of vitamin C and a tablet of immodium.
Premedication lasted 4 sessions, after which general anesthesia was performed, 15 mg of Dormicum and 200 mg of propofol were administered intravenously. After local anesthesia of the vocal cords with ezracoin, intubation was performed.
At the same time, a probe was inserted into the stomach and a catheter into the bladder. After stabilization of the condition, 40 mg of naltrexone and 150 mg of catapressan were administered through a gastric tube. At the same time, 0.1 mg of sandostatin is injected subcutaneously, and 0.0 mg of famotidine and 1 mg of zofran are administered intravenously. The whole procedure continued for 5 hours at the same dosage.
After 5 hours, a naloxone test was performed by intravenous administration of 0.8 mg of naloxone. Changes in breathing, pulse, pressure, and skin condition were observed. In the absence of the above-mentioned withdrawal symptoms, propofol administration is stopped. After 40 minutes, the patient woke up and was extubated.
As is known, methadone is absorbed in fatty tissues and is slowly released during detoxification. Therefore, when treating patients using methadone, the whole procedure lasts longer and a much larger amount of medicine is given.
At the end of treatment according to the Hiller-Serdyuk method, all patients are under the supervision of a narcologist and a psychiatrist for 6 months.
When treating patients with opiate addiction according to the proposed method, a positive effect with drug withdrawal was observed in 7090 patients after 1 year.
The proposed method using this complex of medicines has high clinical efficiency, allows to save patients from the suffering associated with withdrawal syndrome, and to achieve stable remission.
