Abstract
Description
The invention relates to medicine, namely, to ophthalmology and can be used for anesthesia during the operation of cyclophotocoagulation in patients with chronic pain syndrome.
A known method of anesthesia in patients with the syndrome of "chronic" pain (see M. Malroy. Illustrated practical guide, "Local anesthesia." M .: BINOM, 2003, c.230-232), which consists in performing a peripheral block, in in the form of peribulbar anesthesia, including an injection of a local anesthetic, administered transconjunctival. The amount of anesthetic administered in the known method is up to 10 ml, moreover, 2 injections are performed for its administration. Anesthesia at the same time comes slowly. In addition, the disadvantage of this method is a weak analgesic effect with blockade of the peripheral link due to the impact on the only component of the pathological chain of pain. As a result of this, it is not possible to obtain a good analgesic effect during surgery in patients with chronic pain syndrome.
The present invention solves the problem of developing a new method of analgesia during the operation of cyclophotocoagulation in patients with chronic pain syndrome. The technical result obtained in this case consists in the efficiency of the performed manipulation, which allows to block all the links of the pathological pain chain and to obtain a fast onset of a good anesthetic effect. The hormonal drug dexamethasone allows you to suspend the subsequent hormonal - humoral response, the antipsychotic - droperidol provides a decrease in the effect of the reticular formation, benzodiazepine, such as diazepam. protects the limbic system, and in combination with the peripheral block performed in our proposed method, a new positive effect is achieved in the analgesia of a group of patients with chronic pain syndrome during the operation of cyclophotocoagulation. Due to the proposed combination of medicines and the performed manipulations, the flow of chaotic impulses from the cerebral cortex to the peripheral link is interrupted, which allows breaking the pathological circle of pain. The resulting positive result was achieved by us as a result of the studies.
The specified technical result is achieved by the fact that in the anesthesia method during the operation, cyclophotocoagulation in patients with the syndrome of "chronic" pain, which consists in performing a peripheral block in the form of peribulbar anesthesia, including an injection of local anesthetic, administered transconjunctively, 5 minutes before surgery before peribulbar anesthesia intravenously the hormonal drug dexamethasone is administered sequentially in a dose of not more than 8 mg, then the antipsychotic droperidol in a dose of not more than 2.5 mg, after perif matic block at the start of the operation intravenously administered benzodiazepine, such as diazepam at a dose of not more than 5 mg, and when performing peribulbarnoy anesthesia is administered no more than 6 ml of local anesthetic.
The method of anesthesia during surgery cyclophotocoagulation in patients with the syndrome of "chronic" pain is as follows.
5 minutes before the operation, the hormone drug dexamethasone in a dose of not more than 8 mg is first sequentially administered intravenously, then the antipsychotic droperidol in a dose of not more than 2.5 mg. After the introduction of the above drugs, a peripheral block is performed in the form of peribulbar anesthesia, including an injection of a local anesthetic administered transconjunctively. As a local anesthetic, a 2% solution of ultracain in a dose of not more than 6 ml is used. After the peripheral block is completed, benzodiazepine, for example, diazepam, is administered intravenously at a dose of not more than 5 mg.
