Abstract
Description
The invention relates to the field of medicine - endocrinology.
Diabetes mellitus is a chronic polyethylogical disease characterized by profound disorders of carbohydrate, fat, protein and mineral metabolism, which leads to disruption of microcirculation. These disorders are due to a lack of insulin, absolute or relative. The absolute lack of insulin is characterized by a sharp decrease in its synthesis by insulin-secreting β-cells of pancreatic islets and its secretion into the blood, and therefore the level of insulin in the blood serum is very low. With a relative deficiency, its synthesis by β-cells is not disturbed, the level of insulin in the blood serum is normal or often increased, but the sensitivity of peripheral tissues to it is reduced. Diabetes mellitus is a widespread disease. It affects about 3-4% of the population and the incidence of diabetes is steadily increasing. Diabetes mellitus is divided into two different types of diabetes mellitus: insulin-dependent diabetes mellitus type I and non-insulin-dependent diabetes mellitus type II. In turn, non-insulin-dependent diabetes mellitus is divided into two subgroups: with obesity and with normal body weight. In addition, a small group of secondary or symptomatic diabetes is isolated.
Type I diabetes mellitus - insulin-dependent, is characterized by an absolute lack of insulin, usually develops up to 30 years. Although the disease can develop at any age, "young diabetes" or juvenile diabetes. Type II diabetes mellitus in the vast majority of patients develops after the age of 40 and is called "elderly diabetes." "Fat Diabetes" - in 85-90% of patients with type II diabetes mellitus obesity occurs. MODY is a special type of diabetes mellitus II in young people, which develops against the background of obesity, "adult diabetes in youth." LADA is a subgroup of patients with type I diabetes, in whom diabetes develops gradually, like type II diabetes. For 2–3 years, these patients do not require insulin treatment, and then all the classical manifestations of type I diabetes develop and the need for insulin treatment appears. Usually these are young people with normal body weight. Evidence that this type I diabetes mellitus is slowly progressing is the presence of antibodies to the islets of HLAB8, B15, DR3 and DR4 by the onset of diabetes. This subtype of diabetes is called (“latent autoimmune adult diabetes”). In recent years, the number of patients with type I and type II diabetes has been growing. On the one hand, this is due to an increase in the number of elderly and senile patients suffering from diabetes mellitus and in young people, as well as in connection with an improvement in diagnosis, an increase in the number of endocrinological centers and an improvement in medical examination. On the other hand, such a phenomenon as an increase in the incidence rate as a result of a sharp “rejuvenation” of diabetes mellitus and its appearance in high-risk groups is important. This is due to the global increase in psycho-emotional stress, which leads to disruption of lipid and carbohydrate metabolism, a change in the immune status and the development of diabetes.
The modern task in the treatment of type II diabetes mellitus is to restore blood microcirculation and nutrition of the tissue of the damaged organ with the help of drugs that can improve metabolism and microcirculation. Treatment of a patient with diabetes should end with recovery and improvement of the function of the organ damaged by glycemia.
206-276.The following therapeutic actions are used in the treatment of diabetes mellitus: limiting the intake of carbohydrates with food, the appointment of immunosuppressive pharmacological drugs (delagil, azathioprine) to reduce the amount of hypoglycemic agents administered, see the Guide to Clinical Endocrinology, ed. N.T. Starkova, St. Petersburg, Peter, 1996, from 206-276.
The disadvantages of this method are: the process of disruption of the microcirculation of the disease is not eliminated, which leads to impaired organ function, not contributing to the restoration of metabolism in the tissues of organs and microcirculation in damaged organs.
The traditional modern treatment of patients with diabetes is carried out using therapeutic and surgical methods. In therapeutic treatment, a number of medications are used: diet No. 9, multivitamins, exercise therapy (exercise therapy) are prescribed, sugar-lowering pharmacological drugs for type II diabetes, for example, mannin-5, are prescribed to reduce blood sugar levels. Therapeutic methods for treating diabetes mellitus lead to a decrease in blood glucose levels, which should restore metabolism and microcirculation in organs damaged by diabetes mellitus, see the textbook Internal Medicine edited by S. I. Ryabov, V. I. Almazova, E. V. . Shlyakhto, St. Petersburg, Spetslit, 2000, p. 751-768, as the closest solution.
The disadvantages of the traditional generally accepted method for the treatment of diabetes are: insufficient treatment effectiveness, because traditional sugar-lowering drugs for diabetes do not restore the metabolism and microcirculation of organs, does not eliminate the manifestations of the disease and its complications.
The technical result of the proposed solution is to increase the effectiveness of treatment by normalizing the blood sugar of the patient and carbohydrate metabolism by reducing the level of hyperglycemia.
This result is achieved by the fact that in the method of treatment of patients with type II diabetes mellitus, including the appointment of diet No. 9, multivitamins, a sugar-lowering drug, for example maninil-5, as well as the appointment and intravenous administration of an Actovegin solution of 400 mg 2 times a day for 10 days, then intramuscular injection of acotvegin solution 200 mg 2 times a day for 10-15 days, then taking Actovegin 200 mg tablets 2 times a day for 30 days and 3 months after the course of treatment, taking Actovegin 2 tablets 200 mg 2-3 ra and a day for 15 days.
The essence of the solution is expressed in the aggregate of essential features sufficient to achieve the technical result provided by the solution.
Существенными признаками способа, совпадающими с известными признаками прототипа, являются: назначение диеты №9, поливитаминов, сахароснижающего лечебного средства, например, манинил-5.The essential features of the method, which coincide with the known features of the prototype, are: the appointment of diet No. 9, multivitamins, a sugar-lowering therapeutic agent, for example, mannil-5.
Salient features of the device are: intravenous administration of actovegin solution 400 mg 2 times a day for 10 days; intramuscular injection of Actovegin 200 mg 2 times a day for 10-15 days; then taking 2 tablets of Actovegin 3 times a day for 30 days and 3 months after the course of treatment, taking Actovegin 2 tablets of 200 mg 2-3 times a day for 15 days.
С. Примеры конкретного исполнения указаны в приложениях №1 и №2.In the modern treatment of diabetes there is a systematic search for drugs that could increase the effectiveness of the treatment of type II diabetes. One of the areas of modern endocrinology is the use of drugs that improve metabolism and microcirculation. One of these drugs is a drug from the hemoderivative family - Actovegin. It effectively restores the structure of tissues and capillaries. In patients with diabetes mellitus, as a rule, a violation of microcirculation in the blood in various organs develops, therefore it is very important to optimally select the pathogenetic therapy. It is proposed, using generally accepted measures, - the appointment of diet No. 9, multivitamins, physiotherapy exercises, sugar-lowering medicine - maninil-5. Then, the introduction of Actovegin in the form of an intravenous solution of 400 mg 2 times a day for 10 days, then the intramuscular injection of Actovegin solution of 200 mg 2 times a day for a duration of 10-15 days. Then, taking Actovegin in 2 tablets of 200 mg 3 times a day for 30 days and 3 months after the course of treatment, taking Actovegin in 2 tablets of 200 mg 2-3 times a day for 15 days. Actovegin activates cellular metabolism. It increases the transport of glucose and oxygen in organs damaged in diabetes, which leads to an acceleration of metabolism and the accumulation of energy resources of the cell. Actovegin in conditions of impaired carbohydrate metabolism stimulates the energy processes of functional metabolism, which is very important in the treatment of patients with type II diabetes mellitus subject to the selected treatment regimen, which includes: 1 - the appointment of diet No. 9; 2 - intake of hypoglycemic drugs, for example maninil-5; 3 - the appointment of Actovegin at the recommended course of administration. This method of treatment of type II diabetes mellitus was treated 26 patients in the hospital №20, St. Petersburg. Examples of specific performance are indicated in annexes No. 1 and No. 2.
Using the invention, “A method for the treatment of type II diabetes mellitus patients” compared with the generally accepted method of treatment, allows to increase the effectiveness of treatment by normalizing blood sugar and carbohydrate metabolism by reducing the level of hyperglycemia, due to the fact that using the traditional method of treatment, namely, the appointment of diet 9, multivitamins, physiotherapy exercises, a hypoglycemic drug - maninil-5, also prescribe a medicinal product from the hemoderivative family - actovegin according to a certain administration schedule, i.e. intravenous administration of Actovegin solution with a dose of 400 mg 2 times a day for 10 days, then intramuscular administration of Actovegin solution with a dose of 200 mg 2 times a day for 10-15 days, then the appointment of taking Actovegin tablets 2 tablets of 200 mg 3 once a day for 30 days and 3 months after the course of treatment, taking Actovegin in 2 tablets of 200 mg 2-3 times a day for 15 days. The therapeutic effect of the use of Actovegin in the treatment of type II diabetes mellitus is confirmed by controlling blood sugar (reducing it to normalization), ultrasound of the abdominal organs. Thus, the use of Actovegin in patients with type II diabetes mellitus leads to increased metabolism and microcirculation. This leads to a quick recovery of the patient’s performance and does not lead to his disability. Actovegin prevents the formation of scar tissue at the site of necrosis by improving microcirculation. Mobilization of Actovegin metabolism and microcirculation helps prevent the development of ischemic preconditioning of damaged organs. Due to the high efficiency of treatment with Actovegin according to a specific scheme for its administration, i.e., timing and dose, type II diabetes mellitus, the proposed method is recommended to be widely used in clinical practice. In hospital No. 20 of St. Petersburg, 26 patients were cured.
