Існуючий відомий спосіб загальної озонотерапії при гострому панкреатиті проводиться в формі внутрішньовенних інфузій озонованого фізіологічного розчину з концентрацією озону - мг/л. З моменту поступлення кожен день проводять загальну озонотерапію по 800мл озонованого фізіологічного розчину з концентрацією озону мг/л на протязі 4-5 діб. Автори спостерігали чіткі антигіпоксичний та дезінтоксикаційний ефекти озону (Старосек В.Н., Хайкин Я.Б., Колбасин П.Н., Фомочкин И.Л., Скоромньй А.Н., Власов А.К., ГоровецThe existing known method of general ozone therapy for acute pancreatitis is carried out in the form of intravenous infusions of ozonated physiological solution with an ozone concentration of mg/l. Недоліком відомого способу є те, що загальна озонотерапія не попереджує розвиток постнекротичних гнійних ускладнень, а низька концентрація озону безпосередньо в некротичних вогнищах підшлункової залози та парапанкреатичній клітковині при внутрішньовенному застосуванні не сприяє зупинці прогресування гнійно- некротичних ускладнень.The disadvantage of the known method is that general ozone therapy does not prevent the development of post-necrotic purulent complications, and the low concentration of ozone directly in the necrotic foci of the pancreas and parapancreatic tissue when administered intravenously does not help stop the progression of purulent-necrotic complications.

Недоліком відомого способу є необхідність приготування великої кількості озонованих розчинів для санації та зниження бактерицидної концентрації озону в промивних розчинах, що призводить до недостатньої ефективності фізичного впливу озонованих розчинів лише для санації інфікованих некротичних вогнищ, тому для стійкого зниження бактеріального забруднення виникає необхідність частих повторних промивань озонованими та іншими дезінфікуючими розчинами в значних об'ємах. Крім того відомо, що при перитоніті, гнійних процесах в заочеревинній клітковині мікроорганізми знаходяться не лише у перитонеальному ексудаті, а також знаходяться в глибині тканин (як наприклад при парапанкреатиті). In addition, it is known that with peritonitis, purulent processes in the retroperitoneal tissue, microorganisms are not only in the peritoneal exudate, but also in the depth of the tissues (as, for example, in parapancreatitis). At the same time, the lavage efficiency is quite low.
Задачею заявленої корисної моделі є покращення результатів хірургічного лікування хворих з важкими формами гострого деструктивного панкреатиту, з високим ризиком інфікування некротичних вогнищ та інфікованим панкреонекрозом.The purpose of the declared useful model is to improve the results of surgical treatment of patients with severe forms of acute destructive pancreatitis, with a high risk of infection of necrotic foci and infected pancreatic necrosis.
Задача досягається тим, що в парапанкреатичній клітковині створюють ретроперитонеум газовою озоно- кисневою сумішшю перед операцією, інтраопераційно та в післяопераційному періоді. Озоно-киснева суміш з концентрацією озону 5-бмг/л застосовують для накладення парапанкреатичного ретроперитонеуму (з розрахунку лікувальної дози озону 8Омкг на 1кг маси тіла хворого).The task is achieved by creating a retroperitoneum in the parapancreatic tissue with a gaseous ozone-oxygen mixture before surgery, intraoperatively, and in the postoperative period. Спосіб виконують таким чином - з першого дня госпіталізації хворого з гострим деструктивним панкреатитом за методикою по Роману виконують ін'єкції озоно-кисневої суміші з концентрацією озону в суміші 4мг/л (з метою профілактики інфікування панкреонекрозу) в заочеревинну парапанкреатичну клітковину по 100смЗ3 в обидві поперекові ділянки. Під час операції з приводу інфікованого панкреонекрозу ретроперитонеум створюють ін'єкціями озоно-кисневої суміші з концентрацією озону 5-бмг/л, через задню парієтальну очеревину в місцях поширення процесу. В залежності від локалізації гнійно-некротичного ураження підшлункової залози, характеру розповсюдження процесу по клітковині заочеревинного простору після операції даний метод виконують через день на протязі 4-5 діб з концентрацією озону в озоно-кисневій суміші 5-Ємг/л.The method is carried out as follows - from the first day of hospitalization of a patient with acute destructive pancreatitis, according to the Roman method, injections of an ozone-oxygen mixture with an ozone concentration of 4 mg/l (for the purpose of preventing infection of pancreatic necrosis) are performed in the retroperitoneal parapancreatic tissue in the amount of 100 cm3 in both lumbar areas During an operation for infected pancreonecrosis, retroperitoneum is created by injections of an ozone-oxygen mixture with an ozone concentration of 5-bmg/l, through the posterior parietal peritoneum in the places of spread of the process. Depending on the location of the purulent-necrotic lesion of the pancreas, the nature of the spread of the process through the tissue of the retroperitoneal space after surgery, this method is performed every other day for 4-5 days with an ozone concentration in the ozone-oxygen mixture of 5-Emg/l.
Приготування озоно-кисневої суміші здійснювали з допомогою установки "Медозонс 95-2". Вказаний метод озонотерапії розроблений з врахуванням методичних рекомендацій, затверджених Міністерством охорони здоров'я України (2001р.).Preparation of the ozone-oxygen mixture was carried out using the "Medozons 95-2" installation. Прикладом застосування даного способу є історія хвороби Ме0740201300/36. Хворий Ж. 1958 року народження, стать чоловіча поступив 12.02.03. з діагнозом інфікований панкреонекроз інфільтративно- некротичний парапанкреатит. На фоні загальноприйнятої консервативної інфузійної терапії інтраопераційно та після операції застосували запропонований метод парапанкреатичного озонованого ретроперитонеуму.An example of the application of this method is case history Me0740201300/36. При застосуванні даної методики відмітили швидку нормалізацію основних лабораторних показників, зменшення контамінованості некротичних вогнищ в парапанкреатичній клітковині. Доброякісне протікання захворювання підтвердили більш раннім зниженням молекул середньої маси та інших показників ендогенної інтоксикації. При проведенні методу окрім позитивної динаміки основних клініко-лабораторних показників, достовірно покращились показники всіх основних ланок імунітету, значно активізувались системи окислювального і антирадикального захисту. Через 14 діб стан хворого нормалізувався. Після лише однієї операції хворий був виписаний із стаціонару в задовільному стані.With the application of this technique, a rapid normalization of the main laboratory indicators and a reduction in the contamination of necrotic foci in the parapancreatic tissue were noted. Таким чином, експериментально-клінічна апробація заявленого способу на кафедрі хірургії КМАПО більш ніж в 17 випадках дозволяє зробити висновки, що запропонований спосіб лікування важких форм гострого деструктивного панкреатиту з високим ризиком інфікування та інфікованого панкреонекрозу є високоефективним і найбільш оптимальним при цій патології. Він дозволяє уникнути недоліків попередніх методик, а саме: недостатньої антибактеріальної дії при внутрішньовенному застосуванні; недостатньої ефективності (фізичного впливу) озонованих розчинів, що використовують для санації гнійно-некротичних вогнищ підшлункової залози та парапанкреатичної клітковини. Серед переваг цього методу є те, що при даному способі застосування озон проявляє як місцеву антибактеріальну дію, так і системний вплив з вираженим дезінтоксикаційним, антигіпоксантним, та неспецифічним імуномодулюючим ефектами. Метод дозволяє потенціювати дію озону, крім того, є ефективним профілактичним засобом попередження розвитку постнекротичних гнійних ускладнень, що значно перевищує по ефективності попередні методики. Наш досвід свідчить про доцільність включення методу озонотерапії в комплекс лікування хворих з важким протіканням гострого деструктивного панкреатиту з високим ризиком інфікування та інфікованим панкреонекрозом в якості методу профілактики і лікування інфікованих форм гострого деструктивного панкреатиту.Thus, the experimental and clinical approbation of the claimed method at the department of surgery of KMAPO in more than 17 cases allows us to conclude that the proposed method of treatment of severe forms of acute destructive pancreatitis with a high risk of infection and infected pancreatic necrosis is highly effective and the most optimal for this pathology. It allows you to avoid the shortcomings of previous methods, namely: insufficient antibacterial effect when administered intravenously; insufficient effectiveness (physical impact) of ozonated solutions used for sanitation of purulent-necrotic foci of the pancreas and parapancreatic tissue. Among the advantages of this method is that with this method of application, ozone exhibits both a local antibacterial effect and a systemic effect with pronounced detoxification, antihypoxant, and nonspecific immunomodulatory effects. The method allows you to potentiate the effect of ozone, in addition, it is an effective prophylactic means of preventing the development of post-necrotic purulent complications, which significantly exceeds the effectiveness of previous methods. Our experience shows the feasibility of including the ozone therapy method in the complex of treatment of patients with severe acute destructive pancreatitis with a high risk of infection and infected pancreonecrosis as a method of prevention and treatment of infected forms of acute destructive pancreatitis.
