Проблема лечения больных острым холециститом, осложненного холангитом, является и по сей день весьма актуальной из-за летальности, которая при этом осложнении больше, чем при неосложненном холецистите. Так, по данным Б.А. Королева и Д.Л. Пиковского [1] летальность при осложненном холецистите составляет 5-15%, тогда как при неосложненном - 1-2%. По данным В.Я. Васюткова, Г. Г. Гвиниашвили и С.Е. Козлова [2], общая послеоперационная летальность больных с холедохолитиазом, осложненным гнойным холангитом, составляет 11,5%, тогда как при неосложненных формах желчно-каменной болезни 0,8-2,4%. Частота же гнойного холангита за последние 15 лет по данным Веронского Г.И. и Штофина С.Г. [3] составляет 10,4-10,5%. Как видно из источников информации, летальность остается достаточно высокой. Главную роль в развитии холангита оказывает инфекция. В монографии Б.А. Королева и Д.Л. Пиковского отмечается, что вопросы борьбы с бактериальной инфекцией у больных с острым холециститом, особенно осложненными формами, остаются актуальными. Подчеркивается, что даже отсутствие роста микробов на обычных средах не означает стерильности материала. По данным М.И. Кузина (1986), так называемое стерильное содержимое желчного пузыря при гангренозном холецистите может означать как раз наличие анаэробной неклостридиаль-ной инфекции. Так же небезинтересны данные В.А. Гольбрайха (1986) о том, что в первые 72 часа после первого приступа холецистита высеваемость увеличивается до 92% [1]. Во-первых, введение в желчные пути газообразного озона (в данном случае озон-кислородной смеси) может привести к гипертензии в желчных путях и вызвать осложнения заболевания. За прототип предлагаемого изобретения выбран известный способ лечения холангита, включающий проведение холецистэктомии, дренирование холедоха и подведение лекарственного препарата к зоне воспаления [патент РФ 2157660. Способ лечения холангита / Чернеховская и др. Официальный бюллетень Роспатента "Изобретения. Полезные модели", 29, 2000, с.314]. В качестве препарата используют озонированный изотонический раствор хлорида натрия, который вводят в общий желчный проток в процессе холедохоскопии во время операции и дополнительно такой же раствор вводят в просвет общего желчного протока по Т-образной дренажной трубке после операции до нормализации цвета и прозрачности желчи. Используют раствор с концентрацией озона 5 мг/л в количестве 40 мл. Концентрация озона 5 мг/л отнюдь не является безопасной. Так, еще в 1997 г. Потехина Ю.П. показала, что озон в концентрации свыше 4 мг/л вызывает существенную активацию процессов перекисного окисления липидов (ПОЛ) без компенсации со стороны системы антиоксидантной защиты и значительные изменения в перекисной резистентности мембран эритроцитов по данным биохемилюминисценции крови. Поставленная задача решается тем, что в известном способе лечения больных холангитом, включающем проведение холецистэктомии, дренирование холедоха и подведение лекарственного препарата к зоне воспаления, при проведении холецистэктомии осуществляют интраоперационную холангиографию, дренирование холедоха осуществляют через культю пузырного протока завальцованным катетером по методике Холстеда-Пиковского, через который ежедневно вводят озонированный изотонический раствор хлорида натрия с концентрацией озона в растворе 2 мг/л в объеме 20 мл, до статистически достоверного снижения титра микробных тел в 2-3 раза. В качестве группы сравнения 15 больным вводился обычный изотонический раствор хлорида натрия с гепарином. Так же до и после введения раствора определялась желчь на посев. Существенных различий в посевах до и после введения в этой группе мы не получили. Как показали проведенные нами исследования, введение 20 мл озонированного изотонического раствора с концентрацией озона 2 мг/л снижают титр микробов в посеве желчи, причем он остается пониженным и на следующий день после введения. У всех пациентов, которым применялся предлагаемый способ, не наблюдались какие-либо осложнения. Процедура хорошо переносилась. Все больные выписаны в удовлетворительном состоянии.
