Abstract

A method for the surgical treatment of the primary glaucoma with the concomitant cataract comprises facoemulsification of the cataract with the implantation of the intraocular lens and viscocanalostomy. The latter is performed from inside of the anterior chamber. In addition, the local cyclodialysis with the implantation of antiglaucoma collagenous drainage XENOPLAST is performed. Upon facoemulsification of the cataract with the implantation of the intraocular lens, the anterior chamber is filled with viscoelastic through paracentesis opening. Under the control of gonioscope, the cannula is inserted. The internal wall of the venous sinus of sclera is perforated. The viscoelastic is inserted under control of the shift of blood strip. The spatula is used for the local cyclodialysis followed by the implantation of the antiglaucoma collagenous drainage XENOPLAST. The viscoelastic is washed out. The corneal accesses are subjected to hydroadaptation.

Необхідно відзначити, що оперативне втручання, а також післяопераційний період в обох випадках протікали без ускладнень. Пацієнти виписувалися із стаціонару на 6 післяопераційну добу з мінімальними запальними змінами у вигляді легкої кон'юнктивальної ін'єкції.It should be noted that the operative intervention, as well as the postoperative period in both cases, proceeded without complications. Переваги запропонованого способу: 1. Імплантація колагенового антиглаукомного дренажу КСЕНОПЛАСТ забезпечує стійку та тривалу нормалізацію офтальмотонуса, завдяки тому, що дренаж майже не піддається біодеструкції, а фільтрація рідини відбувається по всій структурі дренажа, що підтримує функцію дренажної системи ока на протязі багатьох років. 5. Запропонований спосіб не вимагає спеціального дорогого обладнання і витратних матеріалів, малотравматичний та може бути застосований в умовах будь-якого спеціалізованого очного стаціонару і в амбулаторній хірургії.Advantages of the proposed method: 1. The proposed cataract phacoemulsification operation with IOL implantation and endoviscocanalostomy with local cyclodialysis is an effective and technically simple method of combined surgical treatment of cataracts in patients with primary glaucoma, which allows to reduce the number of surgical manipulations and achieve ophthalmotonus compensation without the formation of an additional antiglaucomatous access . 2. The method allows to avoid the development of astigmatism associated with the formation of a filtering cushion. 3. The method is pathogenetically oriented, as it is aimed at activating the outflow of intraocular fluid through natural collectors - through the scleral sinus and uveascleral pathways. 4. Implantation of XENOPLAST collagen anti-glaucoma drainage provides stable and long-term normalization of ophthalmotonus, due to the fact that the drainage is almost non-biodegradable, and liquid filtration occurs throughout the structure of the drainage, which supports the function of the drainage system of the eye for many years. 5. The proposed method does not require special expensive equipment and consumables, is low-traumatic and can be used in the conditions of any specialized eye hospital and in outpatient surgery.
