4905.One of the most urgent problems of dermatology at the current stage is the problem of the spread of dermatoses, as an average increase of 595 skin diseases is observed annually. Allergodermatoses occupy a special place, accounting for up to 5095 of all dermatological diseases. Eczema, as one of the most common dermatoses, in different regions of Ukraine is from 8 to 3195 dermatological diseases, and psoriasis - from 2 to 4905.
A significant place in the treatment of acute and chronic dermatoses belongs to external therapy, in which corticosteroids occupy a special place. In a number of cases, especially with a severe course of hormone-dependent skin diseases, rationally selected means for local use give such a high therapeutic effect that they allow to significantly reduce the dosage of oral corticosteroids or do without them altogether, thereby preventing complications caused by means for systemic use .
Betamethasone valerate (9a-Fluor-16B-methylprednisolone) belongs to the class of corticosteroids - the main active substance of the claimed agent, which has an anti-inflammatory, anti-allergic, anti-exudative effect, stops itching. Its quantitative content in the claimed agent is necessary and sufficient to ensure the required specific effect. If the amount of betamethasone valerate exceeds the declared values, negative side effects are possible.
Cetylpyridinium chloride (M-Cetylpyridinium chloride) in the proposed composition performs several functions: as an active substance that exhibits antimicrobial and fungistatic effects, and an auxiliary substance that, due to surface-active properties, performs the functions of an emulsifier of the 1st kind, a preservative and a stabilizer of dispersed systems in cream base. The quantitative content of cetylpyridinium chloride in the proposed composition is necessary and sufficient to achieve the maximum effect. When its quantity is greater than its stated values, toxicity and an irritating effect on tissues are manifested.
Synthetic primary alcohols of higher fatty fractions C1i6-Cgo (SVZh alcohols) and cetostear alcohol (hydrophobic surfactants and substances) belong to type 2 emulsifiers, which, when mixed in the stated proportions with type 1 emulsifier, provide the most complete stability, high dispersion and necessary visco-plastic properties of the dosage form - cream. The completeness and speed of release of active substances from the base largely depends on the ratio of these components. If the declared quantitative values of the content of SVZ alcohol or cetostearyl alcohol are not observed, the structural-mechanical and rheological properties of the dosage form are violated, and the level of specific activity decreases.
Propylene glycol performs the function of a hydrophilic solvent, as well as a penetrator, ensuring optimal penetration of active substances in areas of tissue damage and the release of antimicrobial substances from the base. At the same time, propylene glycol acts as a preservative and ensures compliance with microbiological purity standards.
The introduction of a hydrophobic substance - vaseline oil into the composition of the claimed product - is traditional when creating a dosage form in the form of a cream (vaseline oil and white vaseline in the prototype and analogues). But the authors believe that the introduction of a significant amount of hydrophobic substances into the dosage form reduces its osmotic activity, as a result of which the drying effect is lost, water and exudate are not adsorbed.
Purified water, which belongs to hydrophilic solvents, together with other substances forms a mixed solvent, which flexibly combines the ability to stabilize substances dissolved in it and ensures the fulfillment of one of the most important medical and biological requirements for a dosage form in the form of a cream - the ability to wet the surface of the affected fabric and spread over it.
Thus, a violation of the qualitative and quantitative composition of the claimed agent leads to a decrease in the level and a change in the spectrum of specific activity, as well as to a violation of the structural-mechanical and physico-chemical properties of the medicinal form of the agent.
Conducted preclinical and clinical studies have shown that the claimed product fully corresponds to foreign analogues in terms of the level and spectrum of specific activity, and in some respects exceeds them. Tables 1, 2, C show the data of a comparative analysis of the results of the treatment of patients with various dermatological diseases, which indicate certain advantages of the claimed agent over the prototype agent (for example, resolution of the rash 4-5 days earlier).
